Vancomycin is a widely used antibiotic and rarely can cause drug-induced thrombocytopenia. A patient with hospital-acquired meningitis after neurosurgery was treated with systemic and intrathecal vancomycin. On 9th day of antibiotic treatment, the patient’s platelets dropped to 0.68×109/L. Multiple platelet transfusions had minimal influence on platelet count. After cessation of vancomycin therapy, platelets returned to normal values without any additional interventions. Diagnosis of vancomycin-induced thrombocytopenia was confirmed by detection of drug-dependent antiplatelet IgG antibodies.

4. Scale-Invariant Modification of COSH Distance for Measuring Speech Signal Distortions in Real-Time Mode

This study considers a new measure of distortions of speaker speech sounds that is invariant with respect to the gain of speech signal in a communication channel. Properties of the measure are investigated in comparison with its closest analogues. A series of theoretical features has been proved. The new measure is shown to combine advantages of the symmetric Itakura distance in relation to the noise immunity of automatic speech processing, on the one hand, and of the COSH distance in relation to the sensitivity to speech signal distortions, on the other hand. Using the proprietary software, an experiment was set up and conducted. Estimates of the new measure dependence on the signal-to-noise ratio were presented. It has been shown that the logarithmic presentation of this relationship has the pattern close to linear. The obtained results are intended to be used in development of new systems and upgrading of existing systems and technologies for digital signal processing and speech quality analysis under the noise exposure.

Проведено комплексное клинико-лабораторное и инструментальное обследование 193 пациентов с ВИЧ-инфекцией в возрасте от 23 до 47 лет с использованием магнитно-резонансной томографии (МРТ), в том числе с внутривенным введением контрастного вещества. МРТ является наиболее эффективным методом лучевой диагностики поражений головного мозга при ВИЧ-инфекции, позволяет выявить зоны поражения и распространенность процесса, провести дифференциальную диагностику между образованием (лимфома при ВИЧ), воспалительным (оппортунистические инфекции) и сосудистым характером процесса, а также на фоне этой патологии выявить поражения вещества головного мозга, типичные для синдрома Вернике — Корсакова.

The aim of the study was to investigate the phenotype of effector T lymphocytes in patients with chronic viral hepatitis C (CVHC) before and after of treatment with direct antiviral drugs depending on the genotype of the virus. 50 patients with CVHC without signs of liver cirrhosis were examined. The diagnosis was made on the basis of epidemiological and clinical laboratory data as recommended by the European Association for the Study of the Liver when specific serological markers of CHCV and RNA of hepatitis C virus (HCV) were detected (EASL, 2016). The determination of HCV RNA was carried out by the method of quantitative polymerase chain reaction in real time. The degree of liver fibrosis in patients with CVHC was assessed using ultrasound elastography. Patients were treated for 3 months with direct antiviral drugs according to the recommendations of the European Association for the Study of the Liver (2016). The control group included 46 practically healthy individuals with severe chronic diseases of various organs and systems excluded during a routine examination, no health complaints, having normal clinical and biochemical blood tests in the absence of markers for viral hepatitis B and C, antibodies to opisthorchis and denying history of alcohol abuse. The study of the subpopulation composition of helper and cytotoxic T lymphocytes was carried out by direct immunofluorescence of whole peripheral blood. We obtained a 100% sustained virological response in patients with 1, 2 and genotypes of CHCV without signs of liver cirrhosis when using therapy with Sofosbuvir (400 mg) and Daclatasvir (60 mg) for 12 weeks. It was found that in CVHC patients were found characteristic features in the phenotypic composition of effector T lymphocytes before and after treatment with direct antiviral drugs in depending on the genotype of HCV. Patients with HCV genotypes 1 and 3 had an increase in the content of terminal differentiated effector (TEMRA) T helpers and effector memory (EM). Only patients with HCV genotype 2 had a decrease in the level of EM T-helper cells in the blood. A decrease in the relative number of T helpers of central memory (СM) was independent of the HCV genotype. The level of effector subpopulations of cytotoxic T lymphocytes in patients with CVHC was consistent with or exceeded control levels in depending on the genotype of HCV. The level of all investigated subpopulations of effector cytotoxic T lymphocytes in patients with HCV genotype 1 was equal to the control values. The number of naïve cytotoxic T cells and CM in peripheral blood in patients with HCV genotype 2 was increased. The content of naïve cytotoxic T lymphocytes, CM and TEMRA in patients with genotype 3 HCV in the blood was increased. The highest viral load was detected in patients with CVHC with genotype 1 HCV. Liver fibrosis was most pronounced in patients with CVHC infection with HCV genotypes 2 and 3. After 3 months of treatment with direct antiviral drugs the patients with CVHC had a reduced content of CM T helpers regardless of the HCV genotype. In addition, patients with HCV genotypes 1 and 3 had a decrease in the number of naïve T helpers and patients with HCV genotypes 2 and 3 had a normalization of the content of naïve cytotoxic T lymphocytes.

The influence of combined treatment with the additional including of systemic enzyme therapy on the clinical manifestations of postinfarction syndrome, cardiohemodynamic parameters and free radical lipid peroxidation indexes has been studied in 42 patients with myocardial infarction and type 2 diabetes mellitus or without it. It was found that additional systemic enzyme therapy enables to achieve positive clinical, hemodynamic and persistent antioxidant effect in patients with postinfarction syndrome and type 2 diabetes mellitus.

9. Regional hematology service registration system for the Russian Federation

Introduction. The hematology service registration system describes actual indicators of hematology service infrastructure and target objects of both assigned and affiliated organizations of Russian Federation regions along with its correlations. This system data base makes it possible to collect, save, and analyze information, creating a data storage library about all current existing resources and taking into account concrete, specific conditions of specialized and high-tech hematological medical assistance organizations at the local levels and formed quality and availability optimization proposals.Aim — description and justification of the need to create a unified, complex, available to all users, informational reference system with the possibility of up-to-date maintenance and accounting that allows for the keep and control of the regional characteristics and hematology service indicators of the Russian Federation.General findings. The creation of and development of the hematology service registration system began in 2018, and work on the project continues. Four main phases were selected as the key stages of the hematology service pasportisation: analyses of the current data, verification of information, creation of a user-friendly tool, system updating. Objects to monitor: medical key and related institutions, universities, affiliated organizations, laboratory service. The data working stages: data collection, manual updating, package uploading. The information sources: official websites, data high-tech medical treatment web portal, web portals of the public health authorities, Compulsory Medical Insurance Fund website, The Unified Data Health Care System, license register of the Federal Service for Healthcare Surveillance, data of the Federal State Statistics Service, official documents of Ministry of Public Health. Based on the developed checklists and forms for entering additional information for specialists of the subjects of the Russian Federation, a reference book of the Ministry of Defense (Excel file format) was created. However, in the course of further remote interaction and conducting field events in the regions of the Russian Federation during 2019–2020 new data needs for evaluating hematology services were determined. The increase of additional data inputs and its structure complication became the main reasons for further developing a new platform design concept and data conversion in database framework of the hematology service registration system (Access file format). The current objectives are as follows: authentication of the internal objects identifiers, implementation of the relational database codifier, modification of cartographic display and objects routing. The hematology service registration system contains data both for the key infrastructure nodes of Russian Federation hematology service and correlations within routing of all key system objects. Based on the positive results, the hematology service registration system is planned to be used by experts of specialized medical organizations, regional executive authorities on public health and professional communities, who will be able to accumulate all changes and indicators in dynamics within one system. The expected result is to provide the Ministry of Defense of the Russian Federation regions with methodological and informational support in specialized and high-tech medical care administration.

10. Prognostic value of minimal residual disease before allogeneic hematopoietic stem cell transplantation in patients with acute leukemia

Introduction. One of the main causes of treatment failure after allogeneic hematopoietic stem cells transplantation (alloHSCT) for acute leukemia (AL) is disease relapse. In recent years, multiparameter fl ow cytometry (MPC) has been widely used to detect minimal residual disease (MRD) because of its capacity to identify patients with a high risk of relapse due to availability and the ability to obtain results in a timely manner.Aim — to evaluate the prognostic value of MRD status before allo-HSCT and the effect of donor type and conditioning intensity on long-term results of allo-HSCT of MOB-positive patients.Patients and methods. The analysis included 107 patients with acute myeloid leukemia (AML) and 63 patients with acute lymphoblastic leukemia (ALL) who underwent allo-HSCT between September 2015 and June 2020. All patients were in complete morphological remission before allo-HSCT. At the time of allo-HSCT 91 patients with AML and 37 patients with ALL were in the first complete remission (CR), in their second and more than two CRs were 16 and 26 patients, respectively. The median follow-up was 18 (1.5–48) months for AML and 14 (1.8–60.1) months for ALL. Immunophenotypic study was performed before allo-HSCT. MRD was detected using a combination of the “different from normal” method and the search for cells with a leukemia-associated immunophenotype.Results. The disease status at the time of transplantation and the presence of MRD before allo-HSCT were independent factors infl uencing the probability of relapse (disease status: HR = 2.911, 95% CI: 1.328–6.379; MRD before allo-HSCT: HR = 7.667, 95% CI: 3.606–16.304) and post-transplant mortality (disease status: HR = 2.911, 95% CI: 1.328–6.379; MRD before allo-HSCT: HR = 7.667, 95% CI: 3.606–16.304). In univariate analysis, the relapse-free survival of MRD+ patients with AL in the first CR was significantly worse than in MRD– (AML: 23 % versus 57 %, p < 0.0001, ALL: 34 % versus 61.7 %, p = 0.0484), and the probability of relapse in MRD+ patients was significantly higher (AML: 75 % versus 12 %, p < 0.0001, ALL: 57 % versus 7 %, p = 0.0072). Pre-transplant MRD status was not prognostically significant for AL-patients in the second and third remission. The development of chronic GVHD reduces post-transplant mortality if it does not require systemic therapy with glucocorticosteroids (HR = 0.006, 95% CI: 0.008–0.446).Conclusion. Testing for MRD of patients with AL in the first CR before allo-HSCT is necessary for risk stratification and identification of patients who will need preventive post-transplant therapy in order to prevent disease relapse.

Low energy density in agricultural or biotechnological raw materials dictates the need for their local processing. Small-scale agricultural products or waste processing plants can`t profitably use technologies common in large-scale petrochemistry, including the complex and expensive process of steam reforming of biogas into syngas for its subsequent conversion to liquid fuel. The work suggests using small-scale sources of inexpensive and easily obtained biogas and other renewable hydrocarbon gases for the production of liquid biofuel by their air conversion in recently developed matrix reformers into low-cost nitrogen-rich syngas, followed by Fischer-Tropsch synthesis in a cascade of sequential reactors. Autothermal matrix reformers are very compact, simple in design, have several times more specific volume capacity than traditional technologies, and allow processing biogas with a high concentration of CO2. The advantages of low-cost conversion of biogas to liquid fuel based on nitrogen- enriched syngas with subsequent Fischer-Tropsch synthesis are discussed.

The selection of an adequate combination of drugs for anesthesia of female mice during surgical embryo transfer is an important part of the process, since insufficient depth of anesthesia or the influence of undesirable effects on the pregnant female and the embryo can lead to unsatisfactory results of the experiment. In our work, we use a combination of 2 drugs for anesthesia of an animal: Zoletil 100 and Meditin, as well as Antisedan immediately after transplantation for a faster recovery from anesthesia.

14. Comparison of methods of surgical transplantation of mice embryos into pseudopregnant female recipients

There are a lot of factors that are able to influence the result of embryo transfer to pseudopregnant females, including the transplantation procedure itself. To determine the possible role of the surgical transplantation method on the embryo transfer result, we analyzed the data of transplantation of 6715 microinjected embryos into the oviduct of 471 pseudopregnant female by three ways — into the infundibulum, into the ampullae by the oviduct muscular wall puncture or incision. A significant increase in the pregnancy rate of recipients, as well as the birth rate in total recipients in oviduct puncture/incision groups, has been shown, however, the birth rate in pregnant recipients does not differ between all experimental groups. Survival of pups is also significantly higher in oviduct puncture/incision groups.

In the paper, the issues of modeling information technology for remote monitoring of technological processes and objects with the ability to control the state of monitoring objects are considered. The main objectives and constraints in the form of ISO/IEC/IIC standards and recommendations have been defined. According to the structural analysis and design methodology, a set of input and output parameters, mechanisms, and constraints affecting operations and data flows have been defined. A conceptual model of information technology for monitoring systems has been built and decomposed in the IDEF0 notation, the possibility of its implementation is shown using the example of a data acquisition unit in the form of an intelligent router that provides collection, processing, temporary storage of data and communication with a data processing server. in presented paper the transient characteristics of an asynchronous electric drive of a pump unit with PID-similar fuzzy controller are investigated. A comparative analysis of control quality in a system with traditional and fuzzy controllers is performed. The use of fuzzy controller in the asynchronous electric drive of the pump unit for water supply system is substantiated and it is proved that the use of fuzzy logic leads to the improvement of the quality of the transient process of the system. The simulation was performed using the Scilab mathematical application package.

Musically-acoustic thesaurus is a complex structure which composition of multiple parts is effectuated by multiple channels of receiving information from the outside and ways of manipulation with it. The manipulation is turning of separate facts into systematic knowledge stored in memory. Musically-acoustic thesaurus of collective and individual consists of two groups with duplex connection which are the knowledge of the world as a sonic phenomenon and of human auditory activity and the experience of absorbing the information received, principles of manipulation with it. They fulfill inherent inclination of individual towards usage of auditory images as a requirement for appearance of musical ones. All the sonorities are bracketed in two groups: extra-musical and musical itself. The latter group consists of musical units of acoustically-sonic environment, having utility, practical significance for a person, allowing orientation in given spacetime. At the same time, they are potentially opened to emotional experiences and aesthetic approach towards them. Acoustically-sonic environment possesses basic traits of organization, being reflected in mind by such characteristics as cyclicity, variability, combinationalism, montage structure, simultaneity, interdependency of shape and background.

Актуальность. Эмпиема плевры (ЭП), как и любое гнойно-воспалительное заболевание, требует назначения антибактериальной терапии и определения чувствительности микроорганизмов к антибиотикам. Однако микробный пейзаж и антибиотикорезистентность бактерий постоянно меняются. Это делает актуальным изучение спектра и свойств возбудителей ЭП. Цель исследования: изучить микробиологическую характеристику больных острой ЭП плевры и разработать рекомендации по оптимизации эмпирической антибактериальной терапии. Материалы и методы. Проведено комплексное ретроспективное исследование 124 случаев заболевания острой неспецифической парапневмонической ЭП. Мужчин было 92 (74,2 %). Медиана возраста больных составила 50,3 (38; 61) года. Результаты. При микробиологическом исследовании плеврального экссудата положительный результат получен в 34 (27,4 %) случаях. Необходимо отметить, что большинство больных (101 — 81,5 %) получали предшествующую антибактериальную терапию. 22. Anterior prompt for debridement and myoplasty femoral neck and head osteomyelitis

The known anterior prompts to the femoral neck are non-traumatic, but they exclude the possibility of myoplasty and are not large enough for intra-articular sequestrectomy.Objective. The aim of the study was to develop an anterior prompt to the hip joint for sequestrectomy with myoplasty.Materials and research methods. For surgery neck and head of the hip osteomyelitis we perform a semi-oval incision in the projection of the femoral head, from the anterior spines of the ilium we cut off the tendons of the sartorius and rectus head of the quadriceps femoris muscles. An operating field is formed, which allows sequestrectomy to be performed. Plasty of the osteomyelitis cavity is performed with the proximal parts of the two anterior ileofemoral muscles. The operating field, if necessary, can be increased by temporary transection of the lumbar-iliac muscle tendon.Research results. The key to the anterior surgical prompt to the hip joint is the sartorius and rectus head of the quadriceps. The length of the surgical wound reaches 17–22 cm, and the width is 16–18 cm. With the mobilized proximal part of the sartorius muscle, we fill the entire bone cavity in the neck and head of the femur. Of the 12 operated patients, 11 (91.7 ± 7.6 %) had a stable long-term remission.Conclusion. The proposed approach is less traumatic and allows you to operate at an angle of about 90°. It complements the advantages of the known anterior prompts and is devoid of their disadvantages.

23. Gender peculiarities of clinical and instrumental indexes in patients with acute Q-wave myocardial infarction after primary coronary intervention

The aim: to determine gender differences in the course of acute Q-wave myocardial infarction in patients after primary coronary intervention. Materials and methods. Results of clinical and instrumental examination of 58 patients with acute Q-wave myocardial infarction were analyzed. Patients were divided into 2 groups according to gender: group 1 (n = 33) – men, mean age 61.0 (55.0; 69.0) years, group 2 (n = 25) – women, mean age 69.0 (63.0; 78.0) years. All the patients underwent coronary angiography followed by angioplasty and stenting of the infarct-dependent coronary artery. Results. In men, the number of leukocytes was significantly higher (by 24.7 %, P = 0.02) and the plasma level of low-density lipoproteins was significantly lower (by 30.9 %, P = 0.007), there was a tendency towards a higher concentration of creatine kinase-MB (by 41.0 %, P = 0.11) at the time of hospital admission as compared to women. Women showed significantly increased thickness of the posterior wall of the left ventricle (LV) (by 7.31 %, P = 0.02) and a tendency towards the prevalence of the interventricular septum wall thickness (by 7.46 %, P = 0.1). The LV end-diastolic and end-systolic dimensions tended to prevail in men (by 2.41 %, P = 0.47 and by 7.81 %, P = 0.19, respectively). Men were found to have a lower peak A rate (by 23.37 %, P = 0.007), a higher E/A ratio (by 52.27 %, P = 0.009), LV diastolic dysfunction (DD) occurred less frequently (χ2 = 2.89, P = 0.08), and there was a tendency to develop eccentric LV hypertrophy (χ2 = 2.39, P = 0.12) and type 2 LV DD (χ2 = 3.82, P = 0.07). Type 1 DD was more common in women (χ2 = 5.71, P = 0.02). In men, there was a tendency to the formation of zones of LV akinesia (χ2 = 2.5, P = 0.09) and decreased LV ejection fraction (χ2 = 1.57, P = 0.31). Conclusions. In the acute period of Q-wave myocardial infarction after revascularization, in men, unlike in women, systolic dysfunction is developed more often. It is accompanied by acute left ventricular failure, eccentric left ventricular hypertrophy and type 2 diastolic dysfunction formation as well as signs of systemic inflammation. In women, in the acute period of Q-wave myocardial infarction after revascularization, concentric left ventricular remodeling occurs more frequently and is accompanied by type 1 left ventricular diastolic dysfunction.

The safety of the new anti-tuberculosis drug Tiozonide was studied by bioinformatics and preclinical methods using laboratory animals and with the participation of healthy volunteers. The absence of toxic effects on the main life support systems of mammals predicted by QSAR models was confi rmed by the results of acute, subchronic and chronic toxicity tests in various laboratory animals. Monitoring of the vital indicators of volunteers and a comparative analysis of the generalized results of these indicators before (screening), during and after the study showed the absence of reliable and clinically signifi cant changes that threaten the life and health of people.

This article investigates changes in the cellular composition of bronchoalveolar lavage over time in the modeling of acute respiratory distress syndrome (ARDS) in mice, followed by a single administration of Leutragine. In intact animals, macrophages predominate in bronchoalveolar lavage, which is the physiological norm. When modeling ARDS, neutrophils increase. A single administration of Leutragine leads to a significant reduction in the number of neutrophils and a simultaneous increase in macrophages in 72 hours, thus bringing the cellular composition of lavage to normal.

The actual problem of obtaining long products from bearing steel grades with specified microstructure characteristics is considered. The analysis of the capabilities of the existing equipment – a heating furnace and a rolling mill 370/150 of OJSC “BSW – Management Company of the Holding “BMC” was carried out in order to introduce technical measures aimed at reducing carbide heterogeneity in products made of bearing steel grades on the example of steel grade 100Cr6.The influence of different modes of preliminary thermal preparation of continuously cast billets from bearing steel grades on the carbide inhomogeneity in the finished rolling is studied. According to the results of the research, the optimal mode of heat treatment of a continuously cast billet was determined, which allows to produce rolled products that meet the highest requirements of consumers.

Background. Prompt detection of changes in the cardiovascular system in children with chronic pyelonephritis is important for slowing down the development of arterial hypertension and the progression of chronic kidney disease. Nowadays, daily monitoring of blood pressure and ultrasound Doppler of the renal vessels are the up-to-date non-invasive methods for early diagnosis of cardiovascular disorders. The purpose of the study was to determine the prognostic value of ultrasound symptoms of renal hemodynamic disorders and daily blood pressure monitoring in children with chronic pyelonephritis. Materials and methods . The correlation analysis of indicators of daily blood pressure monitoring and ultrasound Doppler examination have been performed in 107 children aged 3–17 years with chronic pyelonephritis. Results . There was statistically significant correlation between such indicators as pulsation index, resistance index, systolic-diastolic ratio at the level of stem, segmental renal arteries and daily indexes (blood pressure, systolic pressure and diastolic pressure), variation coefficient at night, mean diastolic pressure and diastolic pressure at night. However, correlation between ultrasound indicators of renal hemodynamics and daily blood pressure monitoring at the level of interlobular renal arteries was statistically unreliable. Furthermore, children with chronic pyelonephritis on the background of urinary tract abnormalities had the strongest associations between the indicators of the peripheral resistance at the level of the stem renal arteries and the indexes of daily blood pressure monitoring. Conclusions . Indicators of renal hemodynamics — pulsation index, index of resistance, systolic-diastolic ratio at the level of the stem and segmental arteries of the kidney can distinguish the features of renal hemodynamics in order to predict the development of renal hypertension in children with chronic pyelonephritis. The intensity of deviations of these parameters from the normal rates of renal hemodynamics determines the severity of arterial hypertension in 57 % of cases.

The article considers the problem of automatic segmentation of a speech signal into phonetic units in conditions of their a priori uncertain spectral composition and correlation properties. A guaranteed significance level criterion is developed based on the information–theoretic approach. An example of practical application of this criterion is considered; a full-scale experiment is set up and conducted. It is shown that the proposed criterion can guarantee a stable significance level when processing speech frames of short duration.

In this paper, we analyze effective methods of multi-label classification of image sets in development of visual recommender systems. We propose a two-step algorithm, which at the first step performs fine-tuning of a convolutional neural network for extraction of visual features. At the second stage, the algorithm concatenates the obtained feature vectors of each image from the input set into one descriptor using modifications of a neural aggregation module based on linear squeezing of the feature space and an attention mechanism. We perform an experimental study for the Amazon Product dataset solving a problem of classification of customer interests based on photos of the products they have purchased. We show that one of the highest F1-measure indicators can be achieved for a one-level attention block with squeezing of the feature vectors.

The article continues our previous work [V. V. Savchenko and A. V. Savchenko, Izmeritel’naya Tekhnika, No. 12, 46–52 (2019)]. We study the problem of automatic control of the quality of voice samples recorded and stored in the Unified Biometric System. We propose a solution of the problem of timely updates of the collected samples required because their consumer qualities are lost in the course of time. A new indicator of the acoustic quality of voice samples in the Kullback–Leibler information metric is investigated and a new procedure is proposed for measuring this indicator at the times of contact of the users with the system with service requests. An example of practical implementation of the proposed method in the real-time mode is demonstrated. By using software specially developed by the authors, we performed a full-scale experiment and obtained quantitative estimates of the period of updating of voice samples. Some recommendations concerning their practical application are given. The accumulated results can be used for the purposes of development of new systems and technologies (and upgrading of the existing systems) of automatic quality control and adaptive renewal of the biometric personal data samples.

We consider the problem of determination of the intelligibility of speech of a speaker according to a finite fragment of the speech signal. It is shown that the main difficulties in the solution of this problem are connected with the necessity of analysis of small samples. To overcome the problem of small samples, we proposed a new high-speed method for measuring the intelligibility of speech signals on the sonic level of its perception. The proposed method is based on the information indicator of speech intelligibility in the Kullback-Leibler metric. We consider an example of practical realization of the new method with the use of a self-regression model of minimum sound units from the speech flow of a speaker. The characteristics of efficiency of the new method are analyzed. It is shown that, under certain conditions, the application of the information indicator enables us to realize the general systems principle of guaranteed result. On the basis of the software developed by the authors, we designed and performed full-scale experiments and established quantitative estimates for the speed of this method. It is shown that, with the help of this method, quite accurate and reliable estimates of the information indicator are obtained for short (2–3 min) segments of speech signals. The accumulated results and the conclusions made on their basis are intended for applications in the development of new systems and improvement of the existing systems of automatic speech processing and recognition intended for the operation in the real-time mode.

The possibility of obtaining multilayer cylindrical waveguides by explosion welding is investigated. The fact that the technological welding scheme has a significant impact on the shaping of workpieces and the value of edge effects was established. The studies demonstrated that the nature of wave formation during the manufacture of multilayer cylindrical waveguides from a homogeneous material by explosion welding using a central rod is identical to the wave formation when welding flat multilayer compositions on a rigid base.

Informatization of postmodern society is a promising path to economic, social and educational development. Multipotent mesenchymal stromal cells (MSC) are the key regulators of hematopoiesis. We studied changes in MSC characteristics in patients with myeloid leukemia and patients with lymphoproliferative diseases. MSC were obtained from the bone marrow of patients at the time of diagnostic puncture using a standard technique. Their proliferative potential and expression of genes associated with differentiation and regulation of hematopoiesis were studied. The total cell production of MSC in patients with leukemia at the onset of the disease did not differ from that in the group of healthy donors. The relative expression of the IL6, TGFb1 and TGFb2, PPARG genes was similar in all patients. The relative expression of the JAG1, LIF, IGF1, CSF1, IL1b, and IL1bR1 genes in MSC of patients with leukemia was enhanced and the relative expression of SDF1 was unchanged in comparison with MSC from donors. MSC from patients with leukemia were characterized by enhanced relative expression of PDGFRA and PDGFRB, and reduced expression of SOX9. Changes functions of the stromal microenvironment in patients with hemoblastoses attested to the role of stromal cells in the maintenance and spread of tumor cells.

To analyze the effectiveness of allogeneic hematopoietic stem cell transplantation (allo-HSCT) from a related HLA-identical donor in patients with multiple myeloma (MM).From 2013 to 2018, the study included 8 patients (6 men, 2 women) aged from 27 to 55 years (median 39 years) with MM who underwent allo-HSCT from a related HLA-identical donor (7 patients after auto-HSCT, in 1 case without previous auto-transplantation). All patients required 2 or more lines of induction therapy, while the achieved antitumor effect was unstable. Before allo-HSCT, complete and very good partial remission was determined in isolated cases, in 4 patients the response was regarded as partial remission, stabilization in 1 observation, progression in 1 patient. All patients underwent reduced intensity conditioning (fludarabine 30 mg/m2 6 days + busulfan 4 mg/kg 2 days). Immunosuppressive therapy included the administration of antithymocyte globulin and post-transplant cyclophosphamide.Severe acute GVHD (grade 34) was observed in 3 (37.5%) cases, which resulted in death in 1 case. A stable antitumor response was achieved in 5 (62.5%) patients, complete remission lasts for 2986 months after allo-HSCT. Specific therapy for these patients is not carried out. The 7-year progression-free survival rate was 75%, the 7-year overall survival rate was 84%, with a median follow-up of 65 months. The transplant-related mortality was 12.5%.Allo-HSCT is considered as an alternative method of therapy for young patients with aggressive MM. Allo-HSCT in MM in some cases leads to long-term immunological control of the tumor.Цель. Проанализировать эффективность трансплантации аллогенных гемопоэтических стволовых клеток (алло-ТГСК) от родственного идентичного по человеческим лейкоцитарным антигенам (Human Leukocyte Antigens HLA) донора больным c множественной миеломой (ММ). Материалы и методы. С 2013 по 2018 г. в исследование включены 8 пациентов (6 мужчин, 2 женщины) в возрасте от 27 до 55 лет (медиана 39 лет) с ММ, которым выполнена алло-ТГСК от родственного HLA-идентичного донора (7 больным после трансплантации аутологичных гемопоэтических стволовых клеток, в 1 случае без предшествующей ауто-ТГСК). Всем больным потребовалось 2 и более линий индукционной терапии, при этом достигнутый противоопухолевый эффект оказался нестойким. Перед алло-ТГСК полная и очень хорошая частичная ремиссия определялись в единичных случаях, у 4 больных ответ расценен как частичная ремиссия, стабилизация в 1 наблюдении, прогрессия у 1 пациента. Предтрансплантационное кондиционирование всем больным проведено в режиме пониженной интенсивности (флударабин 30 мг/м2 6 сут + бусульфан 4 мг/кг 2 сут). Иммуносупрессивная терапия включала введение антитимоцитарного глобулина и посттрансплантационного циклофосфамида. Результаты. Тяжелая форма острой реакции трансплантат против хозяина (34-й степени) фиксировалась в 3 (37,5%) наблюдениях, приведя к летальному исходу в 1 случае. Стойкий противоопухолевый ответ достигнут у 5 (62,5%) больных, полная ремиссия у них сохраняется в течение 2986 мес после алло-ТГСК. Специфическая терапия этим больным не проводится. Семилетняя выживаемость без прогрессии составила 75%, 7-летняя общая выживаемость 84% при медиане наблюдения 65 мес. Летальность, связанная с алло-ТГСК, составила 12,5%. Заключение. Алло-ТГСК рассматривается в качестве альтернативного метода терапии молодых больных с агрессивным течением ММ. Выполнение алло-ТГСК при ММ в ряде случаев приводит к реализации длительного иммунологического контроля над опухолью.

Сytokine release syndrome is the common complication of CAR-T therapy. We report a case of patient with B-cell acute lymphoblastic leukemia developing сytokine release syndrome with shock and multiple organ failure and requiring cytokine removal and hemodiafiltration. Remission of the disease was achieved after CAR-T therapy.Распространенным осложнением терапии T-клетками с химерным антигенным рецептором (CAR-T-терапии) является синдром высвобождения цитокинов (СВЦ). Представлено клиническое наблюдение за больным В-острым лимфобластным лейкозом, у которого после CAR-T-терапии развился СВЦ, протекавший с шоком и полиорганной недостаточностью, что потребовало применения сорбции цитокинов в сочетании с гемодиафильтрацией и позволило вывести больного из шока и привести к разрешению СВЦ. В результате CAR-T-терапии достигнута ремиссия лейкоза.

To analyze the results of treatment in patients with acute myeloid leukemia (AML) within protocols AML-17 and modified AML-17 (mOML-17) as part of two consecutive pilot studies in order to develop the best treatment strategy for AML patients aged below 60 years.The study included 89 AML patients who were aged below 60 years and received treatment within the AML-17 and mOML-17 protocols. Cytogenetic and molecular genetic studies were performed in all patients. The presence of mutations in the FLT3, NPM1, CEBPa genes was assessed by fragment analysis. 35 patients underwent a study for mutTP53, mutRUNX1 using next generation sequencing (NGS). The minimum residual population of tumor cells was evaluated by multicolor flow cytometry. Statistical analysis was performed using the procedures of the SAS 9.3 package.Complete remission (CR) was achieved in 89.7% of patients treated with intensive chemotherapy (CT) courses and in 52.4% of patients treated with low-dose CT courses. 8.8% of intensively treated patients were refractory to therapy, and 38% did not respond to low-dose exposure. The early mortality rate was 3%. The overall survival and disease-free 3-year survival for patients included in 2 consecutive studies was were 60% and 67%, respectively. The level of minimal residual disease (MRD) after the first course of induction CT was an important prognostic indicator. The three-year relapse-free survival for patients in whom CR was achieved after the first course of induction CT and in whom MRD was not detected (MRD-negative status was obtained) was 90% compared to 43% for patients who were MRD positive after the first course of induction CT (p=0.00001).The key factor that significantly affects the long-term results of therapy is the rate of MRD after the first course of induction CT.Цель. Проанализировать результаты терапии больных острыми миелоидными лейкозами (ОМЛ) по протоколам ОМЛ-17 и модифицированному ОМЛ-17 (мОМЛ-17) в рамках двух последовательных пилотных исследований с целью разработки оптимальной химиотерапевтической стратегии в лечении больных ОМЛ в возрасте до 60 лет. Материалы и методы. В исследование включены 89 больных ОМЛ в возрасте моложе 60 лет, получавших терапию согласно протоколам ОМЛ-17 и мОМЛ-17. Цитогенетическое и молекулярно-генетическое исследования осуществляли всем больным. Оценивали наличие мутаций в генах FLT3, NPM1, CEBPa методом фрагментного анализа. У 35 больных выполнили исследование на mutTP53, mutRUNX1 методом секвенирования нового поколения (next generation sequencing NGS). Минимальную остаточную популяцию опухолевых клеток оценивали методом многоцветной проточной цитометрии. Статистический анализ проводили с помощью процедур пакета SAS 9.3. Результаты. Полная ремиссия (ПР) достигнута у 89,7% больных, пролеченных по интенсивным программам, и у 52,4% больных, получивших низкодозное воздействие. Рефрактерными к терапии оказались 8,8% больных, пролеченных интенсивно, и 38% не ответили на низкодозное воздействие. Показатель ранней летальности составил 3%. Общая и безрецидивная 3-летняя выживаемость всех больных, включенных в 2 последовательных исследования, составили 60 и 67% соответственно. Значимым стал показатель минимальной остаточной болезни (МОБ) после 1-го курса индукционной терапии. Трехлетняя безрецидивная выживаемость тех больных, у кого ПР достигнута после 1-го индукционного курса и МОБ не выявлена (получен МОБ-негативный статус), составила 90% в сравнении с 43% у тех, у кого при достижении ПР после 1-го курса определялся МОБ-позитивный статус (р=0,00001). Заключение. Ключевой фактор, существенно влияющий на долгосрочные результаты терапии, это показатель МОБ после 1-го курса индукции.

The paper deals with experimental data regarding the effect of internal and external factors on the corrosion decay of Zr1Nb alloy fuel elements. Based on the analysis results, losses of zirconium that transfers to oxide or coolant as per the fuel element wall weight and thickness as well as economic losses from their corrosion decay have been theoretically calculated. To avoid a state-level emergency occurrence, an increase in the fuel element wall thickness up to 660 μm is proposed, which can increase the operating life under the conditions of trouble-free coolant mass transfer hydrodynamic mode.

To date, it is known that haloperidol is used to treat productive psychopathological symptoms in acute alcoholic hallucinosis, but its use is associated with a high risk of developing undesirable drug reactions (NLR). A number of isoenzymes of the cytochrome P-450 family take part in the metabolism of haloperidol. The biotransformation of haloperidol occurs with the participation of the CYP2D6 isoenzyme encoded by the gene of the same name. The CYP2D6 gene is highly polymorphic, and this polymorphism can lead to a change in the activity of the encoded isoenzyme. Changes in the rate of biotransformation of haloperidol may affect the profile of its effectiveness and safety. This review is aimed at analyzing the information accumulated in the literature on the role of genetic factors in the formation of an individual response to haloperidol therapy in patients with acute alcoholic hallucinosis.

Anti-CD19 chimeric antigen receptor (CAR) T-cell therapy is a newer and effective therapeutic option approved for patients with relapsed/refractory acute lymphoblastic leukemia and diffuse large B-cell lymphoma. Acute kidney injury is a complication of CAR T-cell therapy that can result in kidney failure. In most cases, it is thought to be related to hemodynamic changes due to cytokine release syndrome. Kidney biopsy in this clinical scenario is usually not performed. We report on a kidney transplant recipient in his 40s who developed a posttransplant lymphoproliferative disorder of B-cell origin refractory to conventional treatments and received anti-CD19 CAR T-cell therapy as compassionate treatment. Beginning on day 12 after CAR T-cell infusion, in the absence of clinical symptoms, a progressive decline in estimated glomerular filtration rate of the kidney graft occurred. A subsequent allograft biopsy showed mild tubulointerstitial lymphocyte infiltrates, falling into a Banff borderline-changes category and resembling an acute immunoallergic tubulointerstitial nephritis. Neither CAR T cells nor lymphomatous B cells were detected within the graft cellular infiltrates, suggesting an indirect mechanism of kidney injury. Although kidney graft function partially recovered after steroid therapy, the posttransplant lymphoproliferative disorder progressed and the patient died 7 months later.

Increasing number of lung diseases, unfavorable environmental factors, dissemination of infectious agents in a patient's body result in increased frequency of purulent-septic complications, thereby making the study on methods of their diagnosis and treatment relevant. Aim of the work: to examine the diagnostic value of lung ultrasound (US) and spiral computed tomography (SCT) in the diagnosis of acute abscesses and in determining the tactics of surgical treatment. Materials and methods. A prospective study of 40 cases of acute lung abscesses with diagnostic value of US and SCT examination to choose a surgical method in accordance with the type of acute lung abscess. Results. Male was dominated in the study (82.5 %). The median age was 52 (43.5; 60.0) years. SCT revealed an abscess with sequestration in 9 (22.5 %) of 40 patients, which was confirmed during surgery. A false negative result was obtained in 5 (35.7 %) patients out of 14. Lung US revealed sequestration in 14 (35.0 %) patients: 2 (14.3 %) of them were false positive. Transthoracic drainage was performed in 24 (92.3 %) of 26 patients without sequestration. In 2 (7.7 %) cases – video abscessoscopy (VAS). The median length of hospital stay was 34.5 (29.0; 43.0) days for patients without sequestration and 32.0 (26.0; 35.0) – with sequestration, Р = 0.16. Conclusions. Modern radiation imaging is an effective tool for the diagnosis of lung abscess. The sensitivity and specificity of computed tomography was 64.3 % and 100.0 %, sonography – 85.7 % and 92.3 %, respectively, in the study. The concurrent use of these diagnostic methods increases their information content: sensitivity up to 100.0 %, specificity up to 92.3 %. Video abscessoscopy with sequestrectomy is an effective method of treatment and etiological diagnosis of the process, which allows achieving the length of hospital stay comparable to patients without sequestration: 32.0 (26.0; 35.0) vs 34.5 (29.0; 43.0), Р = 0.16.

Indications of allogeneic hematopoietic stem cell transplantation (allo-HSCT) in patients with primary myelofibrosis are intermediate-2 and high-risk group of DIPSS (Dynamic International Prognostic Scoring System), beginning of the disease in childhood. The other adverse factors affect engraftment and survival after allo-HSCT, example partialy matched donor. But the result of allo-HSCT from matched related donors and result of allo-HSCT from haploidentical donors are comparable. The method for haploidentical hematopoietic stem cell transplantation is T-cell-depletion. This is clinical case of T-cell-depleted haploidentical hematopoietic stem cell transplantation in patient with primary myelofibrosis, the diagnosis was established in childhood.Показанием к трансплантации аллогенных гемопоэтических стволовых клеток (алло-ТГСК) у пациентов с первичным миелофиброзом является их принадлежность к группе промежуточного-2 и высокого риска. Пациентам с низким или промежуточным-1 риском с дебютом заболевания в детском возрасте также целесообразно рассмотреть включение алло-ТГСК в программу терапии. Перед алло-ТГСК необходимо учитывать наличие других неблагоприятных факторов, которые могут повлиять на приживление трансплантата и выживаемость после алло-ТГСК, например выполнение алло-ТГСК от частично-совместимого донора. При этом результаты нескольких исследований свидетельствуют о том, что алло-ТГСК от родственных полностью совместимых и гаплоидентичных доноров сопоставимы. Одним из подходов к выполнению трансплантации гаплоидентичных гемопоэтических стволовых клеток является T-клеточная/CD19-деплеция. Представлен клинический случай алло-ТГСК с T-клеточной/CD19-деплецией больному первичным миелофиброзом, у которого диагноз установлен в детском возрасте.

We studied ultrastructure and vesicular structures in endothelial cells of myocardial micro-vessels in burn patients. Electron microscopy revealed a significant decrease in volume density of vesicular structures in the endotheliocytes of myocardial capillaries in patients with burn septicotoxemia. The observed structural signs of endothelial dysfunction revealed in this category of patients can be a promising area for further research and for the development of methods of pathogenetic correction of myocardial disorders in the case of burn injury.
