1. Utilidad diagnóstica de test cognitivos breves en el cribado de deterioro cognitivo

Abstract

Resumen Introduccion y objetivos Evaluar y comparar la utilidad diagnostica (UD) para el cribado de deterioro cognitivo (DC) de los test cognitivos breves (TCB) recomendados por la Guia de practica clinica sobre la atencion integral a las personas con enfermedad de Alzheimer y otras demencias. Material y metodos Estudio de fase iii de evaluacion de pruebas diagnosticas en el que se ha incluido en Atencion Primaria a sujetos con sospecha de DC. A todos se les ha aplicado Mini-Mental State Examination (Mini-Mental), Mini Examen Cognoscitivo (MEC), Short Portable Mental Status Questionnaire (SPMSQ), Memory Impairment Screen (MIS), test del reloj (TdR), Eurotest, Fototest y test de alteracion de memoria (T@M). El diagnostico de DC se ha realizado de forma independiente y cegada con respecto a los resultados de los TCB. La UD se ha evaluado mediante el area bajo la curva ROC (aROC). Resultados Se ha incluido a 141 sujetos (86 con DC). El Eurotest y el T@M (0,91 ± 0,02 [aROC ± EE] y 0,90 ± 0,02, respectivamente), los instrumentos que requieren mas tiempo (7,1 ± 1,8 [media ± DE] y 6,8 ± 2,2 min, respectivamente) tienen una UD significativamente superior a la del Mini-Mental, MEC, SPMSQ y TdR, pero no a la del MIS y Fototest (0,87 ± 0,03 ambas), requiriendo este ultimo menos de la mitad del tiempo (2,8 ± 0,8 min). T@M y MIS solo evaluan memoria y el ultimo no es aplicable a analfabetos. Conclusiones Los instrumentos mas recomendables para el cribado de DC en Atencion Primaria son Eurotest, T@M y Fototest, siendo el ultimo mas eficiente por requerir la mitad de tiempo.

Abstract

Resumen Introduccion y objetivos El desfibrilador automatico implantable (DAI) es una alternativa coste-efectiva para la prevencion secundaria de la muerte subita cardiaca, pero sigue habiendo dudas sobre su eficiencia en prevencion primaria, sobre todo en pacientes con cardiopatia no isquemica. Metodos Analisis de coste-utilidad del DAI mas tratamiento medico convencional frente a tratamiento medico convencional para la prevencion primaria de arritmias cardiacas desde la perspectiva del Sistema Nacional de Salud. Se simulo el curso de la enfermedad mediante modelos de Markov en pacientes con y sin cardiopatia isquemica. Los parametros del modelo se basaron en los resultados obtenidos mediante metanalisis de los ensayos clinicos publicados entre 1996 y 2018 en los que se comparaba el DAI con el tratamiento medico convencional, los resultados de seguridad del ensayo DANISH y el analisis de la practica clinica habitual en un hospital terciario. Resultados Se estimo un beneficio del DAI sobre la muerte por cualquier causa con HR = 0,70 (IC95%, 0,58-0,85) en cardiopatia isquemica y HR = 0,79 (IC95%, 0,66-0,96) en no isquemica. La razon de coste-efectividad incremental estimada mediante analisis probabilistico fue de 19.171 euros/ano de vida ajustado por calidad (AVAC) en pacientes con cardiopatia isquemica, 31.084 euros/AVAC en pacientes con miocardiopatia dilatada no isquemica y 23.230 euros/AVAC en los menores de 68 anos. Conclusiones La eficiencia del DAI monocameral ha mejorado en la ultima decada y este resulta coste-efectivo para los pacientes con disfuncion ventricular izquierda de origen isquemico o no isquemico menores de 68 anos considerando una disposicion a pagar 25.000 euros/AVAC. En pacientes no isquemicos mayores, la razon de coste-efectividad incremental estimada se situa alrededor de los 30.000 euros/AVAC.

Abstract

Introduction and objectives Implantable cardioverter-defibrillators (ICD) are a cost-effective alternative for secondary prevention of sudden cardiac death, but their efficiency in primary prevention, especially among patients with nonischemic heart disease, is still uncertain. Methods We performed a cost-effectiveness analysis of ICD plus conventional medical treatment (CMT) vs CMT for primary prevention of cardiac arrhythmias from the perspective of the national health service. We simulated the course of the disease by using Markov models in patients with ischemic and nonischemic heart disease. The parameters of the model were based on the results obtained from a meta-analysis of clinical trials published between 1996 and 2018 comparing ICD plus CMT vs CMT, the safety results of the DANISH trial, and analysis of real-world clinical practice in a tertiary hospital. Results We estimated that ICD reduced the likelihood of all-cause death in patients with ischemic heart disease (HR, 0.70; 95%CI, 0.58-0.85) and in those with nonischemic heart disease (HR, 0.79; 95%CI, 0.66–0.96). The incremental cost-effectiveness ratio (ICER) estimated with probabilistic analysis was €19 171/quality adjusted life year (QALY) in patients with ischemic heart disease and €31 084/QALY in those with nonischemic dilated myocardiopathy overall and €23 230/QALY in patients younger than 68 years. Conclusions The efficiency of single-lead ICD systems has improved in the last decade, and these devices are cost-effective in patients with ischemic and nonischemic left ventricular dysfunction younger than 68 years, assuming willingness to pay as €25 000/QALY. For older nonischemic patients, the ICER was around €30 000/QALY.

Abstract

Background and study aims Current clinical guidelines recommend needle-knife precut (NKP) as the primary and best method for performing a biliary cannulation (BC) when simple techniques fail and the criteria are met for difficult BC (DBC). However, many endoscopists avoid or defer early NKP in favor of alternative, simpler techniques that have not been shown to be either safer or more effective. Our goal is to test a device that provides the needle-knife papillotome (NKPT) with traction capability and which can facilitate the learning and execution of NKP. Patients and methods This was a descriptive bicentric observational study of a personal cohort of patients undergoing early NKP to analyse the efficacy and safety of the technique with a “hybrid-tome” (HT) built using the isolated core of a NKPT and a conventional canulotome. Results Over a 4-year period, we performed 43 NKPs with the HT, achieving BC in one or two steps in all cases. The 100 % technical success was matched by a 95 % clinical success rate. We recorded 11 adverse events (23 %): five hemorrhages, four pancreatitis, and two cholangitis. In addition to the objective data, we confirmed that HT facilitates alignment with the duodenal papilla and the execution of pre-cutting, especially if the papilla is intradiverticular or hidden by folds. Conclusions The HT tested seems to help trained endoscopists to perform NKP, especially in some anatomic situations, which can improve compliance with the guidelines recommended for early NKP in patients with DBC.

Abstract

The study of deployable structures has been carried out traditionally by simplifying their constituent elements—joints and rods—to ideal entities. However, in this paper the dimensional thickness of these elements is taken into account, in order to evaluate their incidence on the foldability of four deployable structure systems. We have examined the eccentricity that occurs specifically at the joints themselves. Our study ultimately characterizes the incidence of this factor by defining noteworthy parameters common to both tube bundle and scissor systems, enabling us to establish a comparison and draw relevant conclusions.

Abstract

Background: Although the association between HIV infection and airways obstruction is well known, its etiopathogenesis is not clear. Objectives: Our aim was to analyze the association between biomarkers of systemic inflammation and bacterial translocation and pulmonary function tests in HIV infected patients and compare it between smokers and non-smokers. Methods: Cross-sectional, observational study. Inclusion criteria: people living with HIV with undetectable plasma viral load. Exclusion criteria: other comorbidities associated with systemic inflammation. Outcome variables: spirometry and diffusing capacity for carbon monoxide; explanatory variables: inflammatory biomarkers (interleukin-6, tumor necrosis factor-alpha), bacterial translocation (soluble CD14 [sCD14] and bacterial 16S rDNA), and variables related to HIV infection. Associations were tested using the Pearson/Spearman correlation tests, the student t test, and multivariable linear regression. Results: We included 71 patients (54.9% smokers). We did not observe significant differences in pulmonary function tests according to biomarkers of inflammation or bacterial translocation. In non-smokers (n=32), sCD14 was negatively correlated with forced expiratory volume in 1 second (R = -0.35, P = 0.048) and forced vital capacity (R= -0.40, P=0.023). Age, time since HIV diagnosis and CD4+ nadir were associated with alterations in PFTs. In smokers, the only association observed was between the pack-years and pulmonary obstruction. Conclusion: In non-smokers HIV patients, lung dysfunction can be, at least partially, related to bacterial translocation (sCD14), CD4+ nadir and time since HIV diagnosis.

Abstract

Resumen Introduccion Los resultados de la reparacion de la hernia paraestomal (HP) basados en datos provenientes de registros son escasos. El objetivo del presente trabajo es analizar los datos recogidos sobre la HP en el Registro Nacional de Hernia Incisional (EVEREG) y asi evaluar las practicas actuales y resultados en la reparacion de una HP. Metodos Se analizan los datos de la cohorte de HP registradas en el periodo desde julio de 2012 hasta junio de 2018. Se analizan las complicaciones, recidivas y factores asociados a ellas de la cohorte completa de HP, independientemente del tipo de estoma al que se asocian. Posteriormente, se realiza el mismo analisis del grupo de HP con relacion a una colostomia (grupo mas numeroso). Resultados Se estudiaron 353 HP. De estas, 259 (73%) fueron HP en el contexto de una colostomia terminal, 74 (21%) en el de una ileostomia terminal y 20 (6%) en el de una ureteroileostomia (Bricker). La edad media global fue de 68,7 ± 11,1 anos y 135 (38%) pacientes fueron del sexo femenino. El abordaje abierto y la cirugia electiva fueron predominantes (78% y 92%, respectivamente). El 99% se reparo con una malla sintetica no absorbible. Las complicaciones postoperatorias globales fueron altas (30,6%), asi como la recurrencia global (27,5%) tras un seguimiento medio de 9,4 meses. Conclusiones La reparacion de la HP es poco frecuente comparada con el conjunto de reparaciones de la hernia incisional. La cirugia de la HP parece relacionarse con un porcentaje elevado de complicaciones postoperatorias y recidiva.

9. Etiology and prognosis of patients with unexplained syncope and mid-range left ventricular dysfunction

Abstract

Background Syncope in patients with mid-range left ventricular ejection fraction (LVEF) can be due to potentially serious arrhythmic causes. However, there is no clear consensus on the best way to manage these patients. Objectives The objectives of this study were to determine the causes of syncope and assess the diagnostic yield and safety of a stepwise workup protocol in this population. Methods This was a prospective observational study. A stepwise workup protocol was applied to patients with LVEF 35%–50% and unexplained syncope after the initial assessment (step 1). Results One hundred four patients were included {median age 75.6 years; (interquartile range [IQR] 67.6–81.2 years); median LVEF 45% (IQR 40%–48%); median follow-up 2.0 years (IQR 0.7–3.3 years). In 71 patients (68.3%), a diagnosis was reached: 55 (77.5%) in step 2 (hospital admission and electrophysiology study) and 16 (36.5%) in step 3 (implantable cardiac monitor). Arrhythmic causes were the most common etiology (45.2% auriculoventricular block and 9.6% ventricular tachycardia). Sixty patients (57.7%) required the implantation of a cardiac device and 11 had a defibrillation function. Patients diagnosed in step 3 had a higher global risk of recurrence of syncope (hazard ratio 6.5; 95% confidence interval 2.3–18.0). The mortality rate was 8.1 per 100 person-years, and the sudden or unknown death rate was 0.9 per 100 person-years. Conclusion In patients with mid-range left ventricular dysfunction and syncope of unknown cause, a systematic diagnostic strategy based on electrophysiology study and/or implantable cardiac monitor implantation allows a diagnosis to be reached in a high proportion of cases and guides the treatment. Arrhythmia is the most common cause of syncope in this population, particularly auriculoventricular block.

10. A limited and intermittent access to a high-fat diet modulates the effects of cocaine-induced reinstatement in the conditioned place preference in male and female mice

Abstract

Palatable food and drugs of abuse activate common neurobiological pathways and numerous studies suggest that fat consumption increases vulnerability to drug abuse. In addition, preclinical reports show that palatable food may relieve craving for drugs, showing that an ad libitum access to a high-fat diet (HFD) can reduce cocaine-induced reinstatement. The main aim of the present study was to evaluate the effect of a limited and intermittent exposure to HFD administered during the extinction and reinstatement processes of a cocaine-induced conditioned place preference (CPP). Male and female mice underwent the 10 mg/kg cocaine CPP. From post-conditioning onwards, animals were divided into four groups: SD (standard diet); HFD-MWF with 2-h access to the HFD on Mondays, Wednesdays, and Fridays; HFD-24h, with 1-h access every day; and HFD-Ext with 1-h access to the HFD before each extinction session. Our results showed that all HFD administrations blocked reinstatement in males, while only the HFD-MWF was able to inhibit reinstatement in females. In addition, HFD-Ext males needed fewer sessions to extinguish the preference, which suggests that administration of fat before being exposed to the environmental cues is effective to extinguish drug-related memories. HFD did not affect Oprμ gene expression but increased CB1r gene expression in the striatum in HFD-Ext males. These results support that palatable food could act as an alternative reward to cocaine, accelerating extinction and blocking reinstatement, these effects being sex specific.

Abstract

Background Disseminated mycoses other than aspergillosis are infrequently reported in dogs. Case report A 4-year-old female Labrador retriever was evaluated because of hyperthermia, cough and intermittent lameness. Computed tomography showed a soft tissue mass in the cranioventral mediastinum, severe left and central tracheobronchial lymphadenopathy, and moderate bilateral pleural effusion. Magnetic resonance imaging identified an irregular intra-axial well-defined contrast enhancing mass extending from the right frontal lobe to the right thalamus. Fungal culture yielded growth of Chaetomium globosum. Conclusion In this case, the authors report a systemic mycosis in a Labrador retriever caused by C. globosum. To the best of authors' knowledge, this is the first report of systemic disease by this species in veterinary medicine.

Abstract

Background Clustering of cardiovascular risk factors (CVRFs) is extraordinarily common and is associated with an increased risk of cardiovascular disease (CVD). However, the particular impact of the sum of CVRFs on cardiovascular morbidity and mortality has not been sufficiently explored in Europe. Objective The aim of this study was to analyze the differences in survival-free probability of CVD in relation to the number of CVRFs in a Spanish population. Methods A prospective cohort study was conducted from 1992 to 2016 in a Spanish population that included 1144 subjects with no history of CVD (mean age, 46.7 years) drawn from the general population. We calculated the number of CVRFs for each subject (male sex, smoking, diabetes, hypertension, dyslipidemia, obesity, and left ventricular hypertrophy). Cardiovascular morbidity and mortality records were collected, and survival analysis was applied (competing risk models). Results There were 196 cardiovascular events (17.1%). The differences in total survival-free probability of cardiovascular morbidity and mortality of the different values of the sum of CVRFs were significant, increasing the risk of CVD (hazard ratio, 1.30; 95% confidence interval, 1.13-1.50) per each additional risk factor. Conclusion Differences in survival-free probability of CVD in relation to the number of CVRFs present were statistically significant. Further studies are needed to corroborate our results.

Abstract

Introduction Factor XI (FXI) deficiency is a mild bleeding disorder, common among Ashkenazis, that may be underestimated in Caucasians. Management of FXI deficiency in women is a challenge, due to its unpredictable bleeding tendency and the little evidence available on this issue. Objective To describe gynaecological/obstetrical bleeding complications and to analyze the effectiveness and safety of the antihaemorrhagic treatment among women with FXI deficiency. Material and methods A retrospective, observational study of 214 Caucasian subjects with FXI deficiency collected during 20 years (1994–2014) without clinical selection. Results We identified 95 women with FXI deficiency. Any haemorrhagic event was communicated by 26/95 (27.4%), being abnormal uterine bleeding the most frequently found (12/95, 12.6%). Nine postpartum haemorrhages were recorded from 136 deliveries (6.6%) in 57 women. Four postsurgical bleeding complications were registered among 25 gynaecological surgeries (16%) in 20 women. Abnormal uterine bleeding, postpartum and postsurgical haemorrhages were related to both a positive bleeding history and FXI:C values ≤43.5%. Prophylaxis with fresh frozen plasma, used in 12/25 (48%) gynaecological surgeries, did not prevent from postoperative bleeding in three cases, but two developed severe adverse reactions. Conclusion Women with FXI deficiency, especially those with a positive history of bleeding or FXI:C ≤43.5%, are at risk of developing gynaecological/obstetrical haemorrhages, most of them mild/moderate. Systematic prophylaxis has questionable effectiveness, but might cause severe side effects.

Abstract

Patients with chronic diseases are frequent users of healthcare services. The systematic use of stratification tools and predictive models for this group of patients can be useful for health professionals in decision-making processes. The aim of this study was to design two new classifier systems for detecting the risk of hospital admission for elderly patients with chronic conditions. In this retrospective cohort study, a set of variables related to hospital admission for patients with chronic conditions was obtained through focus groups, a health database analysis and statistical processing. To predict the probability of admission from the set of predictor variables, a logistic regression within the framework of Generalized Linear Models was used. The target population consisted of patients aged 65 years or older treated in February 2016 at the Primary Health Care Centre of Burjassot (Spain). This sample was selected through the consecutive sampling of the patient quotas of the physicians who participated in the study (1000 patients). The result was two classification systems, with reasonable values of 0.722 and 0.744 for the area under the ROC curve. The proposed classifier systems could facilitate a change in the current patient management models and make them more proactive.

Abstract

OBJECTIVE To determine the prevalence of presumed postictal changes (PC) on brain MRI in epileptic dogs, describe their distribution, and recognize possible correlations with different epilepsy features. ANIMALS 540 client-owned dogs with epilepsy and a complete medical record that underwent brain MRI at 4 veterinary referral hospitals between 2016 and 2019. PROCEDURES Data were collected regarding signalment, seizure type, seizure severity, time between last seizure and MRI, and etiological classification of epilepsy. Postictal changes were considered when solitary or multiple intraparenchymal hyperintense lesions were observed on T2-weighted and fluid-attenuated inversion recovery images and were hypointense or isointense on T1-weighted sequences, which were not confined to a vascular territory and showed no to mild mass effect and no to mild contrast enhancement. RESULTS Sixty-seven dogs (12.4%) showed MRI features consistent with PC. The most common brain sites affected were the piriform lobe, hippocampus, temporal neocortex, and cingulate gyrus. Dogs having suffered cluster seizures or status epilepticus were associated with a higher probability of occurrence of PC, compared to dogs with self-limiting seizures (OR 2.39; 95% confidence interval, 1.33 to 4.30). Suspected PC were detected both in dogs with idiopathic epilepsy and in those with structural epilepsy. Dogs with unknown-origin epilepsy were more likely to have presumed PC than were dogs with structural (OR 0.15; 95% confidence interval, 0.06 to 0.33) or idiopathic epilepsy (OR 0.42; 95% confidence interval, 0.20 to 0.87). Time between last seizure and MRI was significantly shorter in dogs with PC. CLINICAL RELEVANCE MRI lesions consistent with PC were common in epileptic dogs, and the brain distribution of these lesions varied. Occurrence of cluster seizures or status epilepticus, diagnosis of unknown origin epilepsy, and lower time from last seizure to MRI are predictors of suspected PC.

Abstract

BACKGROUND Photodynamic therapy (PDT) has become one of the most effective therapies for the treatment of actinic keratosis, allowing the removal of more than one lesion in a single session. However, the pain sustained by the patient during treatment and local skin reactions can limit its use. OBJECTIVES To determine the efficacy and safety of combined PDT (daylight PDT followed by conventional PDT) vs conventional PDT 12 weeks after treatment. METHODS The study was performed as a randomized, single-center, non-inferiority clinical trial with two parallel groups. A total of 51 patients with grade I and II AKs on the scalp or face were randomized. Twenty-five patients received one session of combined PDT (combPDT), and 26 patients received one session of conventional PDT (cPDT). The primary endpoint was the reduction of AKs, 12 weeks after treatment. The secondary endpoint was the reduction in pain and local skin reaction. RESULTS The reduction rate of grade I and II AKs was similar in combPDT and cPDT, showing no statistically significant differences between both groups, 76.67% vs 86.63% [P = .094] and 80.48% vs 83.08% [P = .679], respectively. Abstract

A 62-year-old male presented with severe post-endoscopic retrograde cholangiopancreatography (post-ERCP) acute pancreatitis. He required admission to the Intensive Care Unit (ICU) twice due to respiratory and renal failure and neurological deterioration. On the hospitalization ward, he presented a fluctuating alteration of the level of consciousness, with bradypsychia, disorientation and somnolence, which persisted after hemodynamic, metabolic and renal stabilization. A brain magnetic resonance imaging (MRI) was performed showing focal lesions due to small vessel disease, some with cavitations due to necrosis, and mild to moderate subcortical atrophy. A diagnosis of pancreatic encephalopathy was made given the clinical and radiological findings. The patient recovered full cognitive capacity after one week with adequate nutrition supported by protein supplementation.

Abstract

The urban spatial distribution of public housing is not a widely addressed issue in Spain, from a geographical perspective. This paper analyses the spatial distribution of public housing in the city of Valencia (Spain), as well as to identify its relationship with other socio-residential characteristics of the urban environment. Different techniques of spatial point pattern analysis, exploratory spatial data analysis (ESDA) and clustering methods are implemented. We analyse both the univariate spatial patterns of public housing and its relationship with two variables: a low-income population and median monthly rent. Analysis has revealed that public housing follows a pattern of partial agglomeration and mostly peripheral dispersion in its spatial distribution. However, there does not seem to be a univocal and immanent relationship between such distribution patterns and the characteristics of the socio-residential environment. Conversely, it is possible to point to the existence of multiple local forms of association. The lack of a clear pattern may be due to many reasons: the heterogeneity of profiles eligible for public housing, the size of the projects and the spatial dispersion in their location.

Abstract

The use of local agricultural by-products for animal feed is an alternative that reduces livestock production costs and allows food production of greater environmental sustainability. The aim of this experiment was to study the effect of the inclusion in the dairy goat ration of artichoke by-product silage (ABS) at three levels (25%, 40% and 60%, on a dry matter basis) on the milk yield, composition and quality, and on the metabolic profile of dairy goats. Thirty-six Murciano-Granadina dairy goats in mid-lactation were divided into four groups with homogeneous characteristics. Each group was assigned a diet: a control treatment (C) that consisted of a conventional diet of alfalfa hay and concentrate, and three other treatments that included 25, 40 and 60% ABS: ABS25, ABS40 and ABS60. Small differences were observed in the milk yield and quality and the health status of the animals. Only ABS60 presented a slightly lower milk yield (−20% compared to control group), without relevant differences in the milk composition and mineral profile. Regarding the lipid profile, ABS40 was the treatment with the best milk quality, due to a higher content of polyunsaturated fatty acids (4.37%) and lower atherogenicity (1.90) and thrombogenicity indices (3.05), without differences from C. It was concluded that the maximum inclusion level of ABS in dairy goats’ diet should be equal to 40%.

Abstract

Background: Results from retrospective studies suggest that use of neuromuscular blocking agents during general anaesthesia might be linked to postoperative pulmonary complications. We therefore aimed to assess whether the use of neuromuscular blocking agents is associated with postoperative pulmonary complications. Methods: We did a multicentre, prospective observational cohort study. Patients were recruited from 211 hospitals in 28 European countries. We included patients (aged ≥18 years) who received general anaesthesia for any in-hospital procedure except cardiac surgery. Patient characteristics, surgical and anaesthetic details, and chart review at discharge were prospectively collected over 2 weeks. Additionally, each patient underwent postoperative physical examination within 3 days of surgery to check for adverse pulmonary events. The study outcome was the incidence of postoperative pulmonary complications from the end of surgery up to postoperative day 28. Logistic regression analyses were adjusted for surgical factors and patients' preoperative physical status, providing adjusted odds ratios (ORadj) and adjusted absolute risk reduction (ARRadj). This study is registered with ClinicalTrials.gov, number NCT01865513. Findings: Between June 16, 2014, and April 29, 2015, data from 22 803 patients were collected. The use of neuromuscular blocking agents was associated with an increased incidence of postoperative pulmonary complications in patients who had undergone general anaesthesia (1658 [7·6%] of 21 694); ORadj 1·86, 95% CI 1·53–2·26; ARRadj −4·4%, 95% CI −5·5 to −3·2). Only 2·3% of high-risk surgical patients and those with adverse respiratory profiles were anaesthetised without neuromuscular blocking agents. The use of neuromuscular monitoring (ORadj 1·31, 95% CI 1·15–1·49; ARRadj −2·6%, 95% CI −3·9 to −1·4) and the administration of reversal agents (1·23, 1·07–1·41; −1·9%, −3·2 to −0·7) were not associated with a decreased risk of postoperative pulmonary complications. Neither the choice of sugammadex instead of neostigmine for reversal (ORadj 1·03, 95% CI 0·85–1·25; ARRadj −0·3%, 95% CI −2·4 to 1·5) nor extubation at a train-of-four ratio of 0·9 or more (1·03, 0·82–1·31; −0·4%, −3·5 to 2·2) was associated with better pulmonary outcomes. Interpretation: We showed that the use of neuromuscular blocking drugs in general anaesthesia is associated with an increased risk of postoperative pulmonary complications. Anaesthetists must balance the potential benefits of neuromuscular blockade against the increased risk of postoperative pulmonary complications. Funding: European Society of Anaesthesiology.

Abstract

The development and implementation of artificial intelligence (AI) applications in health care contexts is a concurrent research and management question. Especially for hospitals, the expectations regarding improved efficiency and effectiveness by the introduction of novel AI applications are huge. However, experiences with real-life AI use cases are still scarce. As a first step towards structuring and comparing such experiences, this paper is presenting a comparative approach from nine European hospitals and eleven different use cases with possible application areas and benefits of hospital AI technologies. This is structured as a current review and opinion article from a diverse range of researchers and health care professionals. This contributes to important improvement options also for pandemic crises challenges, e.g., the current COVID-19 situation. The expected advantages as well as challenges regarding data protection, privacy, or human acceptance are reported. Altogether, the diversity of application cases is a core characteristic of AI applications in hospitals, and this requires a specific approach for successful implementation in the health care sector. This can include specialized solutions for hospitals regarding human–computer interaction, data management, and communication in AI implementation projects.

La reconstrucción del párpado inferior sigue siendo un reto, especialmente la reparación de defectos que abarcan más de la mitad de su superficie horizontal. El colgajo de Hughes del párpado superior ipsilateral es la técnica quirúrgica de elección para los tumores de gran tamaño del párpado inferior. Presentamos el caso de una paciente con carcinoma basocelular superficial de párpado inferior izquierdo que fue intervenida con extirpación del tumor con margen de seguridad y reparación del defecto primario mediante este colgajo. Describimos la técnica de Hughes paso a paso, sus potenciales complicaciones, así como los puntos clave en su ejecución para conseguir unos resultados satisfactorios., Lower eyelid reconstrution remains a challenge, especially the repair of defects that cover more than a half of its horizontal surface. The Hughes flap of the ipsilateral upper eyelid is the surgical technique of choice for large tumors of the lower eyelid. We present the case of a patient with superficial basal cell carcinoma of the lower left eyelid who underwent surgery with excision of the tumor with margin of safety and repair of the primary defect using this flap. Abstract

The Scientific Societies of Primary Care, being the area in which there is a considerable prevalence of Arterial Hypertension (AHT), need to periodically evaluate the international guidelines for its management. This is particularly relevant when disparate guidelines make it difficult to make decisions in daily clinical practice. The present document has as its aim to analyse the changes and new developments proposed in the guidelines of the American College of Cardiology and the American Heart Association (ACC/AHA 2017), as well as in the guidelines of the European Society of Cardiology and European Society of Hypertension (ESC/ESH 2018). An analysis will be made of any differences, limitations, and their applicability to Primary Care in Spain. Finally, the most relevant available and appropriate information is extracted and integrated in order to homogenise the care of the hypertensive patient, from a critical, but also a reasoned, perspective. The discrepancies between the recommendations in such essential aspects as the management of the disease, require the compiling and critical analysis of the information that enables us as scientific society, interested in providing all PC physicians with the most relevant, and at the same time, sensible, recommendations of all the guidelines.

Abstract

Resumen Existe una gran variedad de marcadores utiles tanto en el diagnostico como en el seguimiento de la enfermedad inflamatoria intestinal. Clasicamente se han utilizado los serologicos, ampliamente distribuidos y accesibles, pero en los ultimos anos han cobrado importancia los fecales, en especial la calprotectina fecal, por haber demostrado mayor precision a la hora tanto de establecer la sospecha de la enfermedad como de predecir la curacion mucosa o la persistencia de actividad inflamatoria. La calprotectina fecal muestra buena capacidad para predecir estudios endoscopicos patologicos, pero tiene una especificidad limitada ya que puede alterarse en otros cuadros digestivos con sintomas similares. La calprotectina fecal presenta mayor precision cuando se asocia a otros parametros, en especial a la proteina C reactiva, y a escalas clinicas de actividad inflamatoria. Finalmente, hay multiples marcadores de nueva generacion, serologicos y fecales, de los que hay escasa evidencia, aunque algunos han mostrado resultados prometedores en diferentes estudios.

Abstract

This paper compares four systems of deployable scissor-like structures (also known as double layer grid-articulated joint systems). Its aim is to understand which of these require fewer elements to cover/span a given area. The results are assessed for five cases of the same area but with different geometry. As part of the research, two different methods have been used, contrasting the precision and convenience of each one simultaneously. On the one hand, we have used mathematical equations that allow us to quantify the amount of rods and joints needed in each system. On the other hand, in carrying out these evaluations, we have also used several digital models made with Grasshopper, a software that offers us tools to create algorithms. To conclude, we then evaluate the results obtained via both procedures.

Abstract

Background Although studies exist comparing low-density lipoprotein cholesterol (LDL-C) and non-high-density lipoprotein cholesterol (HDL-C) in the development of cardiovascular disease (CVD), most have limitations in the mathematical models used to evaluate their prognostic power adjusted for the other risk factors (cardiovascular risk). Objective The aim of this study was to compare LDL-C and non-HDL-C in patients with CVD to determine whether both parameters predict CVD similarly. Methods A cohort of 1322 subjects drawn from the general population of a Spanish region was followed between 1992 and 2006. The outcome was time to CVD. Secondary variables were gender, age, hypertension, diabetes, personal history of CVD, current smoker, body mass index, LDL-C, and non-HDL-C. Two CVD prediction models were constructed with the secondary variables, with only the lipid parameter varying (non-HDL-C or LDL-C). In the construction of the models, the following were considered: multiple imputation, events per variable of 10 or more, and continuous predictors as powers. The validation was conducted by bootstrapping obtaining the distribution of the C statistic (discrimination) and the probabilities observed by smooth curves. These results were compared in both models using graphical and analytical testing. Results There were a total of 137 CVD events. The models showed no differences in the distributions of the C statistic (discrimination, P = .536) or in the calibration plot. Conclusions In our population, LDL-C and non-HDL-C were equivalent at predicting CVD. More studies using this methodology are needed to confirm these results.

Abstract

Kidney disease (KD) in patients with acute myocardial infarction (AMI) is associated with major cardiovascular events (MACE). We sought to compare the long-term variation in KD in patients with AMI versus controls and its value as a risk factor for MACE in patients with AMI. A cohort of 300 outpatients with AMI, recruited between 2014 and 2016 in Barcelona, Spain, were compared with a control cohort matched 1:1 based on age and several risk factors for developing KD. Annual estimated glomerular filtration rate (eGFR) using MDRD-4 formula and albuminuria were collected and patients were followed up for the occurrence of MACE (death, heart failure hospitalization, AMI, or stroke). After a median follow-up of 5.3 years, the decline in eGFR was more pronounced in patients with AMI (−1.15 ml/min/1.73 m2/ per year in patients with AMI vs −0.81 ml/min/1.73 m2 per year in controls, p = 0.018 between the ß coefficients of both regression slopes). Abstract

Desert truffles are hypogeous edible fungi that have been exclusively harvested in wild areas for hundreds of years. Land-use changes coupled with shifts in precipitation pattern and volume, as a result of climate change, have led to a decline in wild production of these fungi. Due to their high nutritional value, as well as rising market prices, efforts were stepped toward domestication more than 20 years ago. The present chapter describes the achievements made to understand the biology and diversity of these desert truffles which have helped to make this resource more sustainable. Most efforts to domesticate this natural resource have begun primarily with Terfezia claveryi Chatin. Biotechnological processes for mycorrhizal plant production as well as plantation management practices are analyzed with the experience accumulated to date. Terfezia cultivation is a totally organic crop, with minimum water irrigation, without the consumption of fertilizers or phytosanitary products and using native fungal and plant species. Thus, the longstanding tradition of desert truffle harvesting looks to the future, by adapting its domestication to modern agriculture.

Abstract

The success of machine-learning methods is spreading their use to many different fields. This paper analyses one of these methods, the Kernel Logistic Regression (KLR), from the point of view of Random Utility Model (RUM) and proposes the use of the KLR to specify the utilities in RUM, freeing the modeller from the need to postulate a functional relation between the features. A Monte Carlo simulation study is conducted to empirically compare KLR with the Multinomial Logit (MNL) method, the Support Vector Machine (SVM) and the Random Forests (RF). We have shown that, using simulated data, KLR is the only method that achieves maximum accuracy and leads to an unbiased willingness-to-pay estimator for non-linear phenomena. In a real travel mode choice problem, RF achieved the highest predictive accuracy, followed by KLR. However, KLR allows for the calculation of indicators such as the value of time, which is of great importance in the context of transportation.

46. Caracterización conductual y neuroinmune de la resiliencia al estrés social: Efectos reforzantes de la cocaína

Numerosos estudios preclínicos han demostrado que el estrés social incrementa la vulnerabilidad a los efectos reforzantes de la cocaína. Sin embargo, los resultados obtenidos no son homogéneos, observándose siempre una subpoblación que no muestra dicho incremento. Utilizando el modelo de derrota social (DS) repetida en ratones, en este trabajo hemos querido caracterizar conductualmente a los ratones resilientes al incremento de los efectos reforzantes de la cocaína inducido por el estrés social. Utilizamos ratones adultos macho de la cepa C57/BL6 a los que sometimos al protocolo de DS repetida y tres semanas más tarde, realizamos el Condicionamiento de Preferencia de Lugar (CPL) inducido por una dosis no efectiva de cocaína (1mg/kg). Una vez finalizado este procedimiento se midieron los niveles estriatales de interleucina 6, ya que el estrés social produce una respuesta de neuroinflamación. No se observó CPL en los ratones controles, pero los animales derrotados tomados en conjunto desarrollaron preferencia. Sin embargo, esta muestra se pudo dividir en ratones resilientes (no desarrollaron preferencia) y susceptibles (presentaron CPL). Durante las derrotas sociales, los animales resilientes pasaron menos tiempo en las conductas de huida y sumisión que los catalogados como susceptible y presentaron conductas de ataque hacia el ratón residente, manifestando por tanto resistencia a ser derrotados. No se observaron diferencias en la respuesta de neuroinflamación, probablemente debido al largo periodo de tiempo trascurrido desde la última derrota social. Nuestros resultados sugieren que un estilo de afrontamiento activo al estrés social va a ser determinante en la protección del sujeto a desarrollar un trastorno por uso de drogas. Preclinical studies have shown that social stress increases vulnerability to the reinforcing effects of cocaine. However, the results are not always homogeneous, revealing a subpopulation that does not show a preference for cocaine. Thus, the main aim of the present study was to characterize the behavioral profile of resilient mice to the stress-induced rewarding effects of cocaine using an animal model of repeated social defeat stress (SD). To this end, male adult mice of the C57/BL6 strain were exposed to SD and, three weeks later, assessed using the Conditioned Place Preference paradigm induced by an ineffective dose of cocaine (1mg/kg). Afterwards, the striatal levels of interleukin 6 were measured, as social stress usually induces a neuroinflammatory response. Control mice did not develop CPP, while defeated mice did overall develop a preference for the drug-paired compartment. Based on the conditioning score that they exhibited, the SD sample was subdivided into resilient (did not develop preference) and susceptible mice (developed preference). During the SD sessions, resilient animals showed less flight and submission behaviors than susceptible mice and they presented attack behaviors towards the residents, thereby showing their resistance to being defeated. There were no differences in the neuroinflammatory response, probably due to the long time elapsed after the last SD session. Abstract

[EN] While the founding members of CHERNE gradually retire, the memory of the early steps of the network should not be lost. CHERNE (`Cooperation for Higher Education on Radiological and Nuclear Engineering¿) is the product of a specific Erasmus activity possible in the early 2000s: the intensive programmes (IP). The first step was a collaboration of three partners (Czech Technical University CTU, Institut Supérieur Industriel de Bruxelles ISIB, Universitat Politècnica de València UPV) organising in 2002 the 3-year IP `PAN: Practical Approach to Nuclear techniques¿, soon integrating two other partners (Aachen University of Applied Science AcUAS, XIOS Hogeschool Limburg). A second IP `SPERANSA: Stimulating Practical Expertise in RAdiation and Nuclear SAfety¿ was first organised without Erasmus support in 2005. A workshop was held in 2005 at UPV, including colleagues from other universities. Its main goal was to put in contact professors and researchers from European Institutions in order to share experiences in education and research in Radiation Protection and Nuclear Engineering. The creation of an informal group of universities to develop activities for the benefit of students was discussed. With the addition of Università degli Studi di Bologna (UniBo) to the initial group, the CHERNE network was created. It attracted rapidly more members (6 adhesions in 2006). CHERNE was conceived as a non-formal wide-scope open network, easily integrating new members, offering affordable activities to the students and mostly relying on Erasmus subsidies. The main goal was still the organisation of Erasmus IP¿s based on practical activities, benefitting of the access to big experimental facilities offered by several partners, like reactors, accelerators, or a radiochemical laboratory. SPERANSA was organised from 2006 to 2008, `JUNCSS: JÜlich Nuclear Chemistry Summer School¿ from 2007 to 2011 and `RAPIX-NOCOS: Radiation protection in non-conventional sectors¿ in 2007 and 2008 without Erasmus support. The annual workshops triggered exchanges between partners and attracted more institutions. The first workshops saw intense discussions about the network organisation and the types of activities that could be organised. A kind of maturity was reached from 2008.

49. Novel Time-Frequency Features of the Fibrillatory Waves Improve Catheter Ablation Outcome Prediction of Persistent Atrial Fibrillation

Abstract

[EN] Despite its limited long-term success rate, catheter ablation (CA) is still the first-line treatment for patients suffering from persistent atrial fibrillation (AF). Preoperative prediction on the procedure¿s outcome is gaining clinical interest to provide optimized patient selection, limit repeated procedures, hospitalization rates, and treatment costs. To this respect, dominant frequency (DF) and amplitude of fibrillatory waves (f-waves) reflected on the electrocardiogram (ECG) recording have provided promising results. Hence, this work explores the ability of a novel set of frequency and amplitude features of the f-waves to improve CA outcome prediction. More precisely, in addition to DF and normalized f-wave amplitude (NFWA), three indices such as spectral entropy (SE), spectral flatness measure (SFM), and amplitude spectrum area (AMSA) were also analyzed. The results obtained for 204 6 s-length ECG intervals extracted from 51 persistent AF patients showed that all indices found statistically significant differences between the patients who maintained sinus rhythm and those who relapsed to AF after a follow-up of 9 months. However, they only obtained limited a discriminant ability ranging between 55 and 62%, which was overcome by 15¿23% when NFWA, SE and AMSA were combined. Consequently, this combination of frequency and amplitude features of the f-waves seems to provide new insights about the atrial substrate remodeling, which could be helpful in improving preoperative CA outcome prediction.
