This article explores the labor of contemporary digital privacy advocates and their myriad efforts to protect and preserve public interests during the era of Big Tech companies. It is based on qualitative interviews with professional staff, lawyers, and policy analysts at multiple major advocacy organizations in Washington, DC. We have employed a grounded theory approach to address four labor-related themes that consistently emerged across our interviews: coalition building, agenda formulation, the art of navigating public- and private-sector relationships, and balancing a domestic and global policy landscape. In the current policy landscape, there is an intensifying degree of advocacy–industry coordination taking place, in part because of US regulatory roll-backs under the Trump administration and a gridlocked Congress. As a result, advocacy organization staff members often rely on companies for information to do their assessment and agenda-setting work. They also apply pressure to these companies and force them to think about how their technologies and operations impact users and publics around the world; they mount legal challenges to various media and tech initiatives to ensure public interests are protected; and some end up working with or for these companies in ways that may impart and integrate the values of advocacy organizations within profit-driven organizations. This article explores the multiple dimensions of advocacy labor which itself is often excluded from media policy and industry analysis.

An estimated 4 billion people worldwide still lack reliable access to the Internet, with the majority concentrated in developing countries. It is within this context that a significant expansion of the satellite Internet industry is taking place, bringing grand visions of social and economic growth through connectivity. Previously dismissed as a limited technology due to high operating costs and latency, as well as the failure of early ventures like Teledesic, satellite Internet has re-emerged in recent years as a viable alternative to the time and energy-intensive practice of laying thousands of miles of fiber optic cable. Utilizing innovative non-synchronous orbit constellations of small, mass-produced satellites that lower production costs and improve latency to better compete with broadband, new Silicon Valley–related companies such as OneWeb, SpaceX, and O3b have promised cheap, high-speed Internet access throughout the world. Drawing upon industry research, site visits, and interviews with technical specialists, development experts, and other actors, this article briefly explores the history of satellite Internet services, analyzes the promises of emerging satellite Internet companies, and argues that without serious commitments from governments and the private sector to follow through on this rhetoric, satellite Internet technology could fail to reach the communities that need it most.

Nipple-sparing mastectomy (NSM) is a surgical option to improve patient satisfaction with aesthetic outcome. It can be utilized to decrease body image disturbance by preserving the woman’s skin and areolar complex. Data for cancer treatment and risk reduction in NSM shows the procedure is oncologically safe. Nipple-sparing mastectomy preserves the nipple to improve cosmetic appearance. Patient satisfaction is higher for NSM compared with traditional mastectomy. PubMed and Cochrane databases were searched from January 1, 2013, to December 22, 2019. The following search terms and keywords were used: nipple-sparing mastectomy, microvascular breast reconstruction, breast cancer, and nipple necrosis. Studies published in scientific journals that reported objective results (specific outcomes of skin flap and nipple-areola complex necrosis) were included. Nipple-sparing mastectomy should be discussed as a surgical option for women who meet the selection criteria to assist with psychological benefits related to body image. Research comparing levels of distress and body image disturbances in women undergoing NSM with traditional mastectomy would be helpful in addressing the psychological care of these women. Ongoing data collection needs to continue to validate that NSM is safe from a surgical and an oncologic standpoint.

Google publicists have suggested the Crisis in Darfur is an example of the Google Earth software’s “success at tangibly impacting what is happening on the ground.” Yet whether or not Google Earth’s interface, along with a medley of other media representations of the conflict, have impacted events on the ground or led to coherent policies of humanitarian intervention remains open to debate. This article draws upon critical approaches from media studies—namely discourse analysis—to analyze several aspects of the Google Earth/USHMM Crisis in Darfur project. While this project was no doubt developed with the noble intention of generating international awareness about widespread violence that has recently occurred in the Darfur region, it is important to evaluate how representations of global conflicts are changing with uses of new information technologies and whether such representations can actually achieve their desired impacts or effects. The article begins with a discussion of the Crisis in Darfur project’s history, proceeds to analyze some of the press coverage of the project and then moves to a critique of the layer using four categories of analysis: (1) the shifting role of satellite image; (2) the temporality of the interface; (3) the practice of conflict branding; and (4) the practice of “information intervention.” Throughout the article, I explore how the presentation of Darfur-related materials through Google Earth reproduces problematic Western tropes of African tragedy and misses an opportunity to generate public literacy around satellite images. I also consider how humanitarianism is intertwined with digital and disaster capitalism, and suggest that this instance of “information intervention” makes patently clear that high visual capital alone cannot resolve global conflicts.

This essay introduces a special stream of *Media+Environment* focused on “disaster media.” In the process, the authors conceptualize this term in relation to “natural” and other disasters, including the COVID-19 pandemic, and explore how understandings of “disaster media” are embedded within several areas of humanities-based film and media scholarship. Writing from the social ecological premise that consequences of disasters stem in large part from systemic actions, the introduction develops three general arguments about disaster media as an analytic. First, disasters cause people to rethink what “media” are and to contend with the fact that, especially during disasters, media are constantly changing and being updated; they also escape the screen and sculpt the environment (media are not only representational but also affective and infrastructural). Second, because they come to the fore in relation to crisis situations, disaster media help expose structural inequalities; practices of relief and reform need to happen and can be facilitated (or inhibited) by mediatic means. Finally, disaster media need to be considered in relation to the multiple temporalities of climate disruption (from the *longue duree* of glacial flow to uncertain and sudden extreme weather). Discussing these issues, the authors also introduce pieces in the stream that are focused on humanitarian drone interventions and glacier-melt artworks.

Longtime FQ Editorial Board member and 2018 MacArthur Fellow Lisa Parks charts the shift in critical focus from the potential of social media platforms to unite people around progressive causes to the need for “content moderation,” the practice of cleaning up digital pollution. Parks centers her analysis on The Cleaners (2018), Moritz Riesewieck and Hans Block's provocative documentary that delves into the lives and worlds of commercial content moderators at an unnamed company in the Philippines. The film's account of these digital-labor conditions prompts Parks' critical reflection on a series of issues: the delegation of U.S. media regulation to globally outsourced workers, the problematic trope of “cleaning,” the business of historical sanitization, and the black-boxing of infrastructural information.

Online social networks are major hubs of communications that are used to share information by societies worldwide. However, the ability to freely communicate on these platforms is increasingly restricted in countries across the globe and existing technological solutions do not fully address the needs of affected communities. In this paper we explore the design process of SecurePost, a novel tool that allows verified group anonymity to those communicating publicly on online social networks. We present survey-based research and ethnographic interviews of communities vulnerable to censorship conducted in Zambia, Turkey, and Mongolia between 2013 to 2016. We use analysis of this data to ground our work. We explore needs and requirements of users such as modes of censorship, resistance to network disruption, and appropriate platform consideration. We outline our technological solution and expand on how on-the-ground research of user communities guides technological requirements.

This article explores how social media users in Turkey conceptualize and navigate free speech challenges in the wake of recent government crackdowns on media institutions. The study is based on qualitative interviews with 40 social media users in Istanbul, including people from LGBTQ, feminist, Kurdish, journalist, activist, and academic communities, who have been on the front lines of free speech struggles in Turkey. Informants' comments converged around a theme of transmit-trap dynamics, which emphasizes user experiences in the context of “networked authoritarianism,” and speaks to the ways Turkish social media users find themselves simultaneously empowered by and targeted within social media platforms.

In 1960, the US government and British protectorate of Zanzibar signed an agreement that allowed US contractors working for the National Aeronautics and Space Administration (NASA) to build an earth station that would support Project Mercury, the first manned US satellite mission. This article focuses on the development of the Project Mercury earth station in Zanzibar during 1959-1964. To historicize the earth station’s establishment, the focus lies on the geopolitical and sociotechnical relations that resulted in the Zanzibar station.

This article describes the complex media environment of urban Zambia based on qualitative interviews with 42 active ICT and social media users in Lusaka. After a contextual discussion of media censorship and Internet freedom, the article draws upon interview data to delineate four circumvention practices: (1) platform jumping; (2) anonymity; (3) self-censorship; and (4) negotiation of legal challenges. Rather than approach circumvention as a set of techniques disseminated from the information capitals of the world to those in the “global south,” this study approaches it as a set of cultural practices that emerges within particular sociohistorical conditions and platforms of communication.

Background Hepatic artery infusion pump (HAIP) use in chemotherapy started in the 1960s as a way to treat liver metastases that were not amendable to locoregional therapy or surgical resection. Because of complications and limited survival benefit, the use of HAIPs fell out of favor. A resurgence has occurred in the use of these pumps, but limited information is available in the literature guiding nursing care of these patients. Objectives The purpose of this study was to review the literature regarding the use, procedures, and nursing care of patients with HAIPs. Methods A systematic literature review was conducted to obtain a comprehensive range of publications. Findings Database searching resulted in 511 titles and abstracts. After eliminating duplicates and nonrelevant titles, 65 remained and were read in full. Of these, 20 were excluded because they did not fit the inclusion criteria.

The Internet is a critical tool for communication and knowledge acquisition in societies across the globe. Unfortunately, its use has become a battlefield for governments, corporations, and individuals to censor speech and access to information. In this paper, we present research into the use of social media for free speech in Turkey, Mongolia, and Zambia as a basis for discussing the limits of Internet freedoms. We discuss the actors, adversaries, social and technological limits, as well as limitations of existing tools for the free exchange of ideas on-line. We conclude with a discussion of how design and development choices for technology can affect marginalized communities, as well as the ethical and technical considerations for developing tools and applications that support Internet freedoms.

This article engages with three different modes of Earth observation—historical network maps, Google Earth interfaces, and fieldwork—to develop the concept of “signal territories” and elucidate a critical approach for studying U.S. broadcast infrastructure. This approach: 1) highlights physical infrastructures—technological hardware and processes in dispersed geographic locations—as important sites for historical and critical analysis in media and communication studies; 2) explores multiple modes of infrastructure representation—ranging from cartography to phenomenology, from hand-drawn maps to digital interfaces, from circuit diagrams to site visits; and 3) foregrounds the biotechnical aspects and resource requirements of broadcast infrastructures, probing their dynamic operations and complex materialisms. Engaging with what Richard Maxwell and Toby Miller call a “materialist ecology” of media, the article explores what is at stake in understanding media infrastructures from up close and afar. Cet article examine trois manieres differentes d’observer la Terre (les cartes de reseau historiques, les interfaces Google Earth et les enquetes sur le terrain) dans le but de developper le concept de « territoires delimites par un signal » et d’elucider une approche critique pour etudier l’infrastructure de la radiodiffusion americaine. Cette approche : 1) souligne les infrastructures physiques (les materiels et processus technologiques dans des lieux geographiques disperses) comme etant des sites importants pour l’analyse historique et critique en etudes des medias et de la communication; 2) explore plusieurs sortes de representation des infrastructures—de la cartographie a la phenomenologie, des cartes geographiques dessinees a la main aux interfaces numeriques, de schemas de circuits aux visites sur place; et 3) met en avant les aspects biotechniques et les besoins en ressources des infrastructures de radiodiffusion, examinant leurs operations dynamiques et leurs materialismes complexes. This article examines the strategies of two satellite operators working across post-communist territories of Central Asia: Eutelsat and Kazsat. To do so it develops a critical approach called footprint analysis, which involves investigating the variety of practices that occur within range of a given satellite's service. Satellites have been used in post-communist territories to circulate broadcast and telecommunication signals, facilitate flows of capital and reshape geographic imaginaries. In addition, satellites have become orbital platforms for the Caspian's booming oil industry. Satellites are used to support everything from surveying oil fields to monitoring drilling operations, from construction of oil rigs to the maintenance of pipelines. The article sets out to develop a model of analysis which can account for the more 'cultural' uses of satellites (i.e. for broadcasting) in relation to their more 'extractive' uses (i.e. for natural resource development).

Treating the airport checkpoint as part of the new security regime, this article concentrates on three issues. First, it considers the working conditions of Transportation Security Agency (TSA) employees whom the US government pays to screen passengers, and emphasizes the enduring physicality of their labor despite the technologization of the checkpoint. Second, it explores new techniques of inspection implemented at TSA checkpoints and delineates practices of `close sensing' that establish continuities between looking and touching. Finally, the article describes the object-oriented visual economy that takes shape at the checkpoint, and suggests that the x-ray sequence ultimately exposes the state's inability to regulate the flow of objects and matter in the age of globalization. Thus the searches, exposures, and probes that define this threshold should be understood not just in terms of individual privacy invasions, but rather as an opportunity to focus on structural changes in federal labor, state surveillance and globalization that have emerged since 9/11.

Settling into a seat in the darkness to watch Citizenfour, you feel as if you should look over your shoulder, scan the room to see who else is there, and ascertain whether you are being followed. When the film’s closing credits roll nearly two hours later, you realize that any modicum of pri- vacy you once thought you had does not really exist, that you should learn digital encryption techniques immediately and cover your webcam with a Post-it. When I met with the film’s director, Laura Poitras, to discuss this third film in her post-9/11 trilogy, one of the first things she said to me was, “Glad to see you’ve got your webcam covered.” 1 Citizenfour creeps through the shadowy labyrinths of state- and corporate-controlled digital networks and details the encrypted exchanges and closed-door meetings that cul- minated in one of the most significant acts of whistleblowing in US history: Edward Snowden’s revelation of details about the National Security Agency’s mass surveillance program. Though some were not surprised by the news, when the Snowden story broke in June 2013, eighteen months before the film’s release, it directed a collective focus onto the dark side of the digital economy and made citizens starkly aware that their beloved smartphones, iPads, and PCs were being used not only to find romance and tweet revolutions, but also to spy on them. Given the news media frenzy around the Snowden affair, the series of articles Glenn Greenwald has published in the Guardian, Snowden’s online video appearances, and the re- lease of two books (Luke Hardin’s The Snowden Files and Greenwald’s No Place to Hide), it would be easy to approach Citizenfour with a bit of ennui. 2 What else is there possibly to So it begins. Also, for the first time, filmmaker Laura Poitras’s unique role in these events becomes palpable and poignant. After being detained and interrogated dozens of times at US air- ports and having her equipment confiscated, Poitras learned digital encryption techniques to thwart future interceptions of her work. By the time Snowden first contacted her in January 2013, she was encryption-savvy and thus able to communicate and build a rapport with him online for five months. During this period, Poitras not only maintained contact with Snowden but also managed to convince Green- wald to meet with Snowden in Hong Kong. And she had the foresight to record and encrypt everything. Citizenfour is a subtle and textured film about whistle- blowing and surveillance, one that moves between the in- frastructural and the personal, the juridical and the anarchic, the sensational and the indecipherable. As Citi- zenfour straddles these polarities, its narrative encompasses information gathered across international locations, through multiple modes of communication, and from a range of perspectives. The filmmaker sought to minimize her own presence in the film, explaining, “I did not want to make a personal essay film. It can be limiting.” Instead, Poitras clings to principles of cinema verite, preferring to “document the encounter,” as she puts it, to lurk and listen on the sidelines in order to open up spaces, characters, and relationships and allow them to unfold. As Poitras ex- plained, to work in this way, “You have to be okay with not knowing where things are going.” Over its nearly two-hour running time, the film shifts from unencrypted messages typed on-screen to ominous NSA facil- ities twinkling in the night, from legal arguments in a federal courtroom in San Francisco to heated conversations in a Hong Kong hotel room, from leaked NSA PowerPoint slides to F ILM QU A RTE RL Y

This paper provides insights on the nature of electricity supply in the rural village Macha, Zambia. It reports on case study research. Use of Information and Communication Technologies and access to e-services are constrained by the availability of electricity. In Zambia‘s rural areas, 3.5 % of households have access to electricity supply. This paper shows such electricity supply can be erratic. When supply is available, it follows a diurnal pattern. The electricity supply varies considerably, including voltage dips and brown outs. It can cause equipment to enter into unstable states, to fail or to damage. Qualitative engineering aspects interact also with social factors, especially in rural Africa. Interventions must be sensitive to the complex array of challenges for people to be able to appropriate the benefits of ICT and e-services.

Cellular networks have revolutionized the way people communicate in rural areas. At the same time, deployment of commercial-grade cellular networks in areas with low population density, such as in rural sub-Saharan Africa, is prohibitively expensive relative to the return of investment. As a result, 48\% of the rural population in Africa remains disconnected. To address this problem, we design a local cellular network architecture, Kwiizya, that provides basic voice and text messaging services in rural areas. Our system features an interface for development of text message based applications that can be leveraged for improved health care, education and support of local businesses. We deployed an instance of Kwiizya in the rural village of Macha in Zambia. Our deployment utilizes the existing long distance Wi-Fi network in the village for inter-base station communication to provide high quality services with minimal infrastructure requirements. In this paper we evaluate Kwiizya in-situ in Macha and show that the network maintains low delay and jitter (20ms and 3ms, respectively) for voice call traffic, while providing high call Mean Opinion Score of 3.46, which is the theoretical maximum supported by our system.

Media and communication studies scholars have been tracking the US digital terrestrial television (DTV) transition for over a decade, conducting research crucial for understanding the future of media systems. The existing research tends to focus on two areas. First, some scholars have used diffusion theory to consider issues of technological adoption, exploring the relative willingness of consumers to adapt to DTV. Second, others have examined how the transition has impacted the consumer electronics industry and the United States' precarious position in that global industry. Few, if any, have engaged critically with the ways that the federal government, trade organizations, and community groups have communicated with the public in an effort to prepare them for the digital TV transition. In this chapter, Lisa Parks adds a vital dimension to existing work in this area by exploring how technical information about DTV was communicated to the public in the months leading up to the transition. Examining a variety of materials, from press releases to news reports from public service announcements to video kinescopes that document the end of analog service, Parks considers how consumer-citizens were addressed by public outreach initiatives and how they negotiated the digital TV transition. She highlights three issues crucial for understanding this major infrastructural transition: (1) the way fixed-income and minority communities have been singled out in relation to television; (2) the uneven geographies of the transition; and (3) the way viewers and digital corporations have responded to the transition in its aftermath. Most people are socialised to know very little about the infrastructures that surround them in everyday life, whether electrical systems, sewer pipes or broadcast networks. Not only are people socialised to be unaware of such systems; infrastructures are often designed purposefully to be invisible or transparent, integrated with the built environment, whether submerged underground, covered by ceilings and walls, or camoufl aged as ‘nature’. Further, since infrastructures often extend across vast territories they are impossible to grasp in their entirety and are diffi cult to describe. Much has been written about the social history of transportation and communication infrastructures. Studies by Wolfgang Schivelbush, Harold Innis, James Carey and others have helped us to understand the railway, telegraphy and broadcasting in relation to structures of industrialisation and modernity. Insisting upon the relevance of culture to such systems, Carey writes, ‘Technology, the hardest of material artifacts, is thoroughly cultural from the outset: an expression and creation of the very outlooks and aspirations we pretend it merely demonstrates’ (Carey 1992: 9). This chapter explores the topic of infrastructure from a cultural perspective using a partial, objectcentred, and knowledge-oriented approach. Here we might access the work of cultural studies scholar John Fiske and consider what it might mean to study infrastructure from a perspective of ‘popular knowledges’ and ‘technostruggles’.1 As Fiske reminds us, ‘The multiplication of communication and information technologies extends the terrains of struggle, modifi es the forms struggle can take, and makes it even more imperative that people grasp the opportunities for struggle that the multiplying of technologies off ers’ (Fiske 1996: 240).

Objective To compare outcomes and feasibility of double-barreled wet colostomy and ileal conduit (IC) in patients undergoing total pelvic exenteration (TPE). Methods Between 2004 and 2010, 54 patients underwent TPE for pelvic malignancies. Of those patients, 53 had complete records available for analysis. Two groups were identified based on the technique used for urinary diversion, either by way of an IC or a double-barreled wet colostomy (DBWC). Demographics, comorbidities, complications, length of stay, operative times, morbidity, and mortality were compared between the 2 groups. Results Forty-three patients (81%) underwent a DBWC and ten patients (19%) underwent an IC. The 2 groups were similar in terms of age, gender, and comorbidities. Eighteen patients underwent an R0 resection (39%) and twenty-eight (61%) patients had a non-R0 resection. Seven patients (13%) had a complete response to therapy with no evidence of malignancy. A majority of the patients (68%) undergoing TPE had colorectal histology. Thirty-day morbidity directly related to complications of urinary or fecal diversion was 78% in the DBWC group and 58% in the IC group. There was no perioperative mortality in either group. Conclusion DBWC is a safe and feasible alternative to the traditional IC for urinary diversion. This technique is easy to learn and is associated with similar operative times, length of stay, morbidity, and mortality compared with IC.

For the past several months I have been conducting an experiment at airport security gates, shooting photographs of the Transportation Security Administration (TSA) facilities and screeners to determine how long I can go on before I will be asked to stop. After shooting photos in 12 airports I have received only one warning at the US-Canada border while taking a picture of a twenty-something woman of colour being interrogated by TSA workers after she was physically searched in a nearby makeshift room. I only became visible to the TSA at the moment I witnessed her visibility, but in general as a white woman I go relatively unnoticed in a US security regime largely based on racial profiling. If I were a person of colour it is possible that many of these images would not exist, that my camera would have been taken, the images destroyed, or I might not have even taken the risk in the first place. In any case, it has become clear to me that the airport is no longer just a ‘non-place’ as Marc Auge (Auge, 1995) famously described it over a decade ago, but in the context of the US-led war on global terror it has possibly become ‘the place’, a charged and volatile domain punctuated by shifting regimes of biopower.

542 Background: Patients with advanced primary or recurrent colorectal cancers that undergo total pelvic exenteration for cure or palliation require proximal urinary and fecal diversion. The most commonly used diversion technique is use of an ileal conduit (IC) and end colostomy. At our institute, the double-barreled wet colostomy (DBWC) has been shown to be have similar outcomes and technically feasible. Methods: Between 2004 and 2010, 37 patients underwent total pelvic exenteration for advanced primary or recurrent colorectal cancer. Two groups were identified based on the technique used for their urinary diversion, either by way of an IC (n = 4) or DBWC (n = 33). Demographics, periprocedural events, and outcomes were compared between the two groups. Results: The two groups were similar in the terms of age, gender, and comorbidities. Thirty-three patients (89%) underwent a DBWC and four patients (11%) underwent an IC. All of these patients underwent a total pelvic exenteration for advanced primary (27%) or recurrent colorectal cancer (73%) either for cure or for palliation. Twenty patients underwent R0 resection (54%), and 17 patients had non-R0 resection (46%). Complications, length of stay, and operative times between both groups were similar. Median survival for both groups showed no statistical difference. Conclusions: DBWC is a safe and feasible alternative to the traditional ileal conduit for urinary diversion. It provides a single stoma to care for, and an intact contralateral abdominal muscle to use as a vertical rectus abdominus musculocutaneous flap for reconstruction.
