1. STEM-10. BIDIRECTIONAL INTERACTION BETWEEN TUMOR-ASSOCIATED PLATELETS AND GLIOMA STEM CELLS IN GLIOBLASTOMA MULTIFORME

Abstract: Glioblastoma Multiforme (GBM) is the most common and lethal malignant primary adult brain tumor. Therapy resistance and tumor recurrence in GBM have been attributed to glioma stem cells (GSCs), which are found in hypoxic but perivascular niches. We hypothesized that the clinically documented and prognostically relevant increase in platelets in GBM promotes formation of functional hypoxic but perivascular niches that support GSCs due to formation of intravascular thromboses, a hallmark of GBM. Our preliminary in silico analysis from TCGA GBM samples indicates significant correlation of platelet and stemness signature expression (P < 0.001). High expression of platelet gene signatures also inversely correlates with overall survival in GBM patients (P < 0.02). Furthermore, we found significant co-localization of known platelet and stemness markers in primary GBM specimens, supporting our hypothesis and motivating further interrogation of GSC-platelet crosstalk. Our preliminary data suggest that GSCs exposed to either tumor or healthy platelets demonstrate increased self-renewal and stemness, as determined by a significant increase in OCT4, NANOG, and OLIG2 expression compared to unexposed GSCs. This increase in stemness and self-renewal markers was not seen in platelet-exposed differentiated glioma cell (DGC) counterparts. Conversely, platelets are stimulated by GSCs and GSC-conditioned media while DGCs elicit no stimulatory effects as measured by ATP release and aggregation assays. Our results implicate a functional role for platelet-GSC interactions in the maintenance of tumor cellular hierarchy that ultimately contributes to poor clinical outcomes in GBM. RNA-Seq of platelet-privileged GSCs is currently underway to elucidate the mechanism by which platelets impact GSC self-renewal. Successful characterization of potential crosstalk between platelets and GSCs may offer new clinical perspectives into GBM and inform development of a novel treatment paradigm to target these specific cell-to-cell interactions.

Abstract: Methanol9s (CH 3 OH) impact as a source of anthropogenic carbon dioxide (CO 2 ) in denitrification at wastewater treatment plants (WWTPs) has never been quantified. CH 3 OH is the most commonly purchased carbon source for sewage denitrification. Until recently, greenhouse gas (GHG) reporting protocols consistently ignored the liberation of anthropogenic carbon dioxide (CO 2 ) attributable to CH 3 OH. This oversight can likely be attributed to a simplifying notion that CO 2 produced through activated-sludge-process respiration is biogenic because most raw-sewage carbon is un-sequestered prior to entering a WWTP. Instead, a biogenic categorization cannot apply to fossil-fuel-derived carbon sources like CH 3 OH. This paper provides a summary of how CH 3 OH use at DC Water9s Blue Plains Advanced Wastewater Treatment Plant (AWTP; Washington, DC) amounts to 60 to 85% of the AWTP9s Scope-1 emissions. United States Environmental Protection Agency and Water Environment Federation databases suggest that CH 3 OH CO 2 likely represents one quarter of all Scope-1 GHG emissions attributable to sewage treatment in the U.S. Finally, many alternatives to CH 3 OH use exist and are discussed.

Abstract: Contains fulltext : 215333.pdf (Publisher’s version ) (Closed access) BACKGROUND: Patients with stable coronary artery disease and diabetes with previous percutaneous coronary intervention (PCI), particularly those with previous stenting, are at high risk of ischaemic events. These patients are generally treated with aspirin. In this trial, we aimed to investigate if these patients would benefit from treatment with aspirin plus ticagrelor. METHODS: The Effect of Ticagrelor on Health Outcomes in diabEtes Mellitus patients Intervention Study (THEMIS) was a phase 3 randomised, double-blinded, placebo-controlled trial, done in 1315 sites in 42 countries. Patients were eligible if 50 years or older, with type 2 diabetes, receiving anti-hyperglycaemic drugs for at least 6 months, with stable coronary artery disease, and one of three other mutually non-exclusive criteria: a history of previous PCI or of coronary artery bypass grafting, or documentation of angiographic stenosis of 50% or more in at least one coronary artery. Eligible patients were randomly assigned (1:1) to either ticagrelor or placebo, by use of an interactive voice-response or web-response system. The THEMIS-PCI trial comprised a prespecified subgroup of patients with previous PCI. The primary efficacy outcome was a composite of cardiovascular death, myocardial infarction, or stroke (measured in the intention-to-treat population). FINDINGS: Between Feb 17, 2014, and May 24, 2016, 11 154 patients (58% of the overall THEMIS trial) with a history of previous PCI were enrolled in the THEMIS-PCI trial. Median follow-up was 3.3 years (IQR 2.8-3.8). In the previous PCI group, fewer patients receiving ticagrelor had a primary efficacy outcome event than in the placebo group (404 [7.3%] of 5558 vs 480 [8.6%] of 5596; HR 0.85 [95% CI 0.74-0.97], p=0.013). The same effect was not observed in patients without PCI (p=0.76, pinteraction=0.16). The proportion of patients with cardiovascular death was similar in both treatment groups (174 [3.1%] with ticagrelor vs 183 (3.3%) with placebo; HR 0.96 [95% CI 0.78-1.18], p=0.68), as well as all-cause death (282 [5.1%] vs 323 [5.8%]; 0.88 [0.75-1.03], p=0.11). Abstract: Greenhouse gas (GHG) accounting protocols have addressed emissions from wastewater conveyance and treatment using a variety of simplifying methodologies. While these methodologies vary to some degree by protocol, within each protocol they provide consistent tools for organizational entities of varying size and scope to report and verify GHG emissions. Much of the science supporting these methodologies is either limited or the protocols have failed to keep abreast of developing GHG research. This state-of-the-art review summarizes the sources of direct GHG emissions (both those covered and not covered in current protocols) from wastewater handling; provides a review of the wastewater-related methodologies in a select group of popular protocols; and discusses where research has out-paced protocol methodologies and other areas where the supporting science is relatively weak and warrants further exploration.

Abstract: Background Providing culturally safe health care can contribute to improved health among Aboriginal people. However, little is known about how to make hospitals culturally safe for Aboriginal people. This study assessed the impact of an emergency department (ED)-based continuous quality improvement program on: the accuracy of recording of Aboriginal status in ED information systems; incomplete ED visits among Aboriginal patients; and the cultural appropriateness of ED systems and environments. Methods Between 2012 and 2014, the Aboriginal Identification in Hospitals Quality Improvement Program (AIHQIP) was implemented in eight EDs in NSW, Australia. A multiple baseline design and analysis of linked administrative data were used to assess program impact on the proportion of Aboriginal patients correctly identified as Aboriginal in ED information systems and incomplete ED visits in Aboriginal patients. Key informant interviews and document review were used to explore organisational changes. Results In all EDs combined, the AIHQIP was not associated with a reduction in incomplete ED visits in Aboriginal people, nor did it influence the proportion of ED visits made by Aboriginal people that had an accurate recording of Aboriginal status. 6. GHG Methodologies for Sewer CH4, Methanol-Use CO2, and Biogas-Combustion CH4 and their Significance for Centralized Wastewater Treatment

Abstract: Greenhouse gas (GHG) arising from the treatment of domestic sewage in the developed world is known to contribute to GHG emissions in the atmosphere – principally carbon dioxide (CO2), methane (CH4) and nitrous oxide (N2O). Accounting protocols exist that allow development of emissions inventories on a geographic, utility, or facility-specific basis using uniform methodologies and assumptions. This thesis develops new, improved methodologies for estimating direct GHG emissions sources from: 1) Sewer CH4, 2) Anthropogenic CO2 from methanol use for nutrient removal, and 3) CH4 from incomplete digester gas combustion. The significance of these GHG sources is estimated herein on a national scale for centralized wastewater treatment in the United States (US). Current GHG-emissions-inventorying protocols consistently ignore sewer CH4.However, by the methods in this thesis, sewer CH4 is estimated to represent between 26% and 35% of DC Water’s and more than 54% of centralized-wastewater-related direct GHG emissions in the US. A new sewer CH4 methodology developed in this thesis uses sewer hydraulic models at average flow rates to classify pipe segments as either gravity or surcharged sewers. CH4 generation in surcharged segments is derived using a prior forcemain algorithm while gravity sewers are modelled using a new algorithm. After method verification using exhaust-CH4 mass fluxes from an odour-control fan evacuating air from a regional sewer, the methodology is applied to the entire DC Water (Washington, DC - US) collection system to estimate utility-wide sewer CH4 emissions, which are found to be substantial.Similarly, until recently, GHG-accounting protocols also ignored CO2 emissions attributable to methanol addition for nutrient removal. As most methanol is derived from natural gas, its carbon represents an anthropogenic CO2 emission when released. In this thesis, a site-specific method is developed for DC Water and used to determine that methanol CO2 represents 40% and 60% of DC Water’s direct GHG emissions, before and after anaerobic-digestion upgrades, respectively. When applied to the US treated wastewater flows, considering likely effluent-nitrogen requirements and presence of and/or lack of digestion that would influence methanol use, methanol CO2 is estimated to account for nearly 12% of the country’s centralized-wastewater-treatment GHG emissions.A new method is also developed to account for fugitive CH4 from digester-gas combustion based on a specific end uses of the biogas. This method contrasts with current protocols which uniformly suggest that 1 to 2% of all produced digester gas is vented to the atmosphere (whether through incomplete combustion or simply leakage is unclear). Use of the new methodology could, depending on the biogas-use technologies in operation, more accurately replace a 1% protocol-based estimation of 4,200 MT CO2e/year for DC Water with emission rates ranging from 60 to over 21,000 MT CO2e/year. Accordingly, current protocol-based simplifications are shown to result in an actual 70-time overestimation for DC Water or a possible 5-times underestimation for the poorest-efficiency biogas-combustion devices: conventional, candle-stick flares.As the final subject of this thesis, electricity use, which comprises the largest portion of centralized-treatment GHG emissions inventories, is reviewed. GHG emissions from electricity consumption occur at the power plant, and their make-up and carbon intensity is a function of how that plant produces electricity. While local electricity carbon intensity is normally beyond the control of the wastewater utility, how much is purchased is largely under its control. Aeration-blower and total-plant power consumption over one or two years for 13 large to very-large facilities in New York City (NY - US) are correlated to average flow and total-oxygen-demand loading and removal to evaluate electricity-use metrics. Additionally, the local carbon intensity of power is shown to dramatically impact the magnitude of GHG emissions and to have surprising effects on an example “sustainability-enhancing” upgrade.This research discusses these improved GHG-accounting methodologies and evaluates their significance using large-scale utility process and GHG data. When combined, application of new sewer CH4 and methanol CO2 methods herein would roughly triple direct GHG emissions attributed to an industry that already represents a significant GHG source and energy consumer. An improved understanding of emissions intensity and the significance of various emissions sources would allow an already proactive wastewater industry to better target or incorporate intervention strategies that mitigate GHG emissions. This thesis offers these improved methodologies while providing evidence that: 1) GHG accounting science is far from fully understood; 2) additional research is warranted; so that 3) the industry’s understanding and GHG emissions baselines are accurate and thereby support almost-certain eventual regulation.

Abstract: Background: \ud Glucagon-like peptide 1 receptor agonists differ in chemical structure, duration of action, and in their effects on clinical outcomes. The cardiovascular effects of once-weekly albiglutide in type 2 diabetes are unknown. We aimed to determine the safety and efficacy of albiglutide in preventing cardiovascular death, myocardial infarction, or stroke.\ud \ud Methods: \ud We did a double-blind, randomised, placebo-controlled trial in 610 sites across 28 countries. We randomly assigned patients aged 40 years and older with type 2 diabetes and cardiovascular disease (at a 1:1 ratio) to groups that either received a subcutaneous injection of albiglutide (30–50 mg, based on glycaemic response and tolerability) or of a matched volume of placebo once a week, in addition to their standard care. Investigators used an interactive voice or web response system to obtain treatment assignment, and patients and all study investigators were masked to their treatment allocation. We hypothesised that albiglutide would be non-inferior to placebo for the primary outcome of the first occurrence of cardiovascular death, myocardial infarction, or stroke, which was assessed in the intention-to-treat population. If non-inferiority was confirmed by an upper limit of the 95% CI for a hazard ratio of less than 1·30, closed testing for superiority was prespecified. This study is registered with ClinicalTrials.gov, number NCT02465515.\ud \ud Findings: \ud Patients were screened between July 1, 2015, and Nov 24, 2016. 10 793 patients were screened and 9463 participants were enrolled and randomly assigned to groups: 4731 patients were assigned to receive albiglutide and 4732 patients to receive placebo. On Nov 8, 2017, it was determined that 611 primary endpoints and a median follow-up of at least 1·5 years had accrued, and participants returned for a final visit and discontinuation from study treatment; the last patient visit was on March 12, 2018. These 9463 patients, the intention-to-treat population, were evaluated for a median duration of 1·6 years and were assessed for the primary outcome. The primary composite outcome occurred in 338 (7%) of 4731 patients at an incidence rate of 4·6 events per 100 person-years in the albiglutide group and in 428 (9%) of 4732 patients at an incidence rate of 5·9 events per 100 person-years in the placebo group (hazard ratio 0·78, 95% CI 0·68–0·90), which indicated that albiglutide was superior to placebo (p

Abstract: To assess the prevalence and severity of dysphonia in patients with cystic fibrosis sinusitis. We hypothesized that patients with CF sinusitis, compared with 2 control groups, would have higher self-reported prevalence of dysphonia and greater severity of dysphonia, according to patient-reported outcome measures as well as auditory-perceptual evaluation by expert listeners.Cross-sectional comparative pilot study.Academic tertiary care clinic.Analysis included 37 study participants: 17 patients with CF sinusitis, 10 healthy individuals, and 10 patients with non-CF sinusitis. All participants completed the 10-item Voice Handicap Index (VHI-10) questionnaire and provided voice samples. On all samples, 6 blinded speech-language pathologists independently performed auditory-perceptual evaluation, using Consensus Auditory-Perceptual Evaluation of Voice. To assess severity of sinonasal symptoms, we used the 20-item Sinonasal Outcome Test (SNOT-20). Standard parametric and nonparametric statistical analysis was performed.The differences between the 3 groups in prevalence of abnormal VHI-10 scores were not statistically significant. SNOT-20 scores were similar in the 2 sinusitis patient groups. VHI-10 scores were highest in patients with CF sinusitis, intermediate in patients with non-CF sinusitis, and lowest in healthy individuals (P = .005). Auditory-perceptual evaluation demonstrated greater overall severity of dysphonia in patients with CF sinusitis compared with the 2 control groups (P = .0005).Cystic fibrosis sinusitis appeared to be associated with worse vocal function as measured by patient self-report as well as auditory-perceptual evaluation of voice compared with patients with non-CF sinusitis and healthy controls. Further investigation in this area is warranted.

Abstract: Various approaches have been developed for quantifying and displaying network traffic information for determining network status and in detecting anomalies. Although many of these methods are effective, they rely on the collection of long-term network statistics. Here, we present an approach that uses short-term observations of network features and their respective time averaged entropies. Acute changes are localized in network feature space using adaptive Wiener filtering and auto-regressive moving average modeling. The color-enhanced datagram is designed to allow a network engineer to quickly capture and visually comprehend at a glance the statistical characteristics of a network anomaly. First, average entropy for each feature is calculated for every second of observation. Then, the resultant short-term measurement is subjected to first- and second-order time averaging statistics. These measurements are the basis of a novel approach to anomaly estimation based on the well-known Fisher linear discriminant (FLD). Average port, high port, server ports, and peered ports are some of the network features used for stochastic clustering and filtering. We empirically determine that these network features obey Gaussian-like distributions. The proposed algorithm is tested on real-time network traffic data from Ohio University's main Internet connection. Experimentation has shown that the presented FLD-based scheme is accurate in identifying anomalies in network feature space, in localizing anomalies in network traffic flow, and in helping network engineers to prevent potential hazards. Furthermore, its performance is highly effective in providing a colorized visualization chart to network analysts in the presence of bursty network traffic.

Abstract: Squamous cell carcinomas of the head and neck are appearing with increased frequency in both marrow transplanted and non-transplanted Fanconi anemia (FA) patients. FA patients commonly display radiosensitivity of epithelial tissues, complicating effective radiotherapy. Fancd2–/– mice (C57BL/6J and 129/Sv background) demonstrate epithelial tissue sensitivity to single-fraction or fractionated irradiation to the head and neck and distant marrow suppression (abscopal effect), both ameliorated by intraoral administration of the mitochondrial-targeted antioxidant, GS-nitroxide, JP4-039. We now report that mice of two other FA genotypes, Fancg–/– (B6) and the most prevalent human genotype Fanca–/– (129/Sv), also demonstrate: 1. reduced longevity of hematopoiesis in long-term bone marrow cultures; 2. radiosensitivity of bone marrow stromal cell lines; and 3. head and neck radiation-induced severe mucositis and abscopal suppression of distant marrow hematopoiesis. Abstract: The acute lethality of total-body irradiation (TBI) involves damage to multiple organs, including bone marrow and intestine. Ionizing radiation mitigators that are effective when delivered 24 h or later after TBI include the anti-apoptotic drug, JP4-039 and the anti-necroptotic drug, necrostatin-1. In contrast to effective delivery of JP4-039 at 24 h after TBI, necrostatin-1 is most effective when delivery is delayed until 48 h, a time that correlates with the elevation of necroptosis-inducing inflammatory cytokines and necroptosis-induced serine phosphorylation of receptor-interacting serine/threonine-protein kinase-3 (RIP3) in tissues. The goal of this work was to determine whether administration of JP4-039 influenced the optimal delivery time for necrostatin-1. We measured daily levels of 33 proteins in plasma compared to intestine and bone marrow of C57BL/6NTac female mice over a 7-day time period after 9.25 Gy TBI (LD50/30). Protein responses to TBI in plasma were different from those measured in intestine or bone marrow. In mice that were given JP4-039 at 24 h after TBI, we delayed necrostatin-1 delivery for 72 h after TBI based on measured delay in RIP-3 kinase elevation in marrow and intestine. Sequential delivery of these two radiation mitigator drugs significantly increased survival compared to single drug administration.

Abstract: SUMMARY A post-mature female newborn with a history of birth trauma developed frequent apneic spells coincident with left posterior-quadrant alpha discharges on her electroencephalogram. Except during apneic spells, clinical seizure activity was rare. The alpha discharges occurred at regular intervals and were associated with disrupted sleep cycles and a diminution in the proportion of quiet sleep. Phenobarbital apparently abolished the apnea and alpha discharges and resulted in normal sleep cycles and state proportions. At 20 months of age the patient is neurologically and developmentally normal. She has been free of apnea and seizures since the newborn period, despite withdrawal of the anticonvulsant at eight months of age. The authors relate her favorable course to the etiology, periodicity and therapeutic response of her clinical and electrical seizures. Abgesehen von den Apnoeanfallen traten selten klinische Anfalle auf. Die Alphaentladungen traten in regelmasigen Abstanden auf und gingen mit unterbrochenen Schlafcyclen und mit einer Verkurzung der ruhigen Schlafphase einher. Durch Phenobarbitol konnten die Apnoen und die Alphaentladungen beherrscht werden und es resultierten normale Schlafcyclen und Stadieneinteilungen. Im Alter von 20 Monaten war das Madchen neurologisch und in seiner Entwicklung normal. Sie hat seit der Neugeborenenperiode keine Apnoen und keine Anfalle mehr gehabt, obwohl das Antikonvulsivum abgestzt worden war. El fenobarbitral aparentemente abolio la apnea y las descargas alfa dando lugar a un ciclo de sueno normal y proporcionado. A los 20 meses de edad el paciente es normal neurologicamente y evolutivamente. Abstract: Imaging sediments below salt bodies is challenging because of the inherent difficulty of estimating accurate velocity models. These models can be estimated in a variety of ways with varying degrees of expense and effectiveness. Two methods are commercially viable trade-offs. In the first method, residual-moveout analysis is performed in a layer-stripping mode. The models produced with this method can be used as a first approximation of the subsalt velocity field. A wave-equation migration scanning technique is more suitable for fine-tuning the velocity model below the salt. Both methods can be run as part of a sophisticated interactive velocity interpretation software package that makes velocity interpretation efficient. Performance of these methods has been tested on synthetic and field data examples.

Abstract: Objectives The aim of the present study was to explore Aboriginal patients’ lived experiences of cardiac care at a major metropolitan hospital in Melbourne. Methods The study was a qualitative study involving in-depth interviews with a purposive sample of 10 Aboriginal patients who had been treated in the cardiology unit at the study hospital during 2012–13. A phenomenological approach was used to analyse the data. Results Eight themes emerged from the data, each concerning various aspects of participants’ experiences: ‘dislike of hospitals’, ‘system failures’, ‘engagement with hospital staff’, ‘experiences of racism’, ‘health literacy and information needs’, ‘self-identifying as Aboriginal’, ‘family involvement in care’ and ‘going home and difficulties adapting’. Most participants had positive experiences of the cardiac care, but hospitalisation was often challenging because of a sense of dislocation and disorientation. The stress of hospitalisation was greatly mediated by positive engagements with staff, but at times exacerbated by system failures or negative experiences. Conclusion Cardiac crises are stressful and hospital stays were particularly disorienting for Aboriginal people dislocated from their home land and community. What is known about the topic? Aboriginal people have higher mortality rates due to cardiovascular diseases compared with other Australians. Along with different factors contributing to the life expectancy gap, Aboriginal people also face significant barriers in the use of the healthcare system. What does this paper add? Aboriginal patients’ lived experience of cardiac care at a major metropolitan hospital in Melbourne is explored in this paper. Different issues were revealed during their interaction with the hospital staff and the hospital system in conjunction with their cultural aspect of patient care. What are the implications for practitioners? Positive interactions with staff, ongoing support from family and community, culturally appropriate cardiac rehabilitation programs can improve the cardiac care experiences of Aboriginal patients.

Abstract: Successful deployment of energy efficiency initiatives may be increased by recognizing barriers and advancing strategies that overcome them. A national survey recently collected input on barriers from more than 110 wastewater service utilities. A concurrent utility focus group process captured detailed experiences. Motivation is a crucial precondition for energy efficiency. The utilities reported that the strongest motivators for utility staff are reductions in energy expenditures and prioritization by utility management. The project identified the following key barriers: • Capital projects. Economic barriers dominate: inadequate financial feasibility, lack of funding, and resource competition with other organizational priorities. • Operations optimization. Operational conservatism (desire for a margin of safety for permit compliance) is the strongest barrier to implementing operating changes that conserve energy. • Maintenance for energy efficiency. Many utilities have not optimized maintenance intervals for energy efficiency because they prioritize more pressing maintenance activities. The barriers considered by this research include both internal and external challenges. Internal challenges are mitigated by well-developed energy programs, as gauged by the level of implementation of the approaches outlined in the Water Environment Federation (WEF) Energy Roadmap. Utilities with well-developed programs perceive barriers as being lower. External challenges are reduced for water resource recovery facilities (WRRFs) in states with well-funded and accessible energy incentive programs and less-inhibitive energy policies. Abstract: BACKGROUND & AIMS: It is not clear how intake of gluten during infancy affects subsequent risk of celiac disease. We investigated whether gluten intake before 2 years of age increases risk for celiac disease in genetically susceptible children. METHODS: We performed a case-control study of 436 pairs of children, generated from a database of 2525 children with genetic susceptibility to celiac disease in Sweden, matched for sex, birth year, and HLA genotype from September 2004 and February 2010. Children were screened annually for celiac disease using an assay for tissue transglutaminase autoantibodies (tTGA). Intestinal biopsies were collected from children who tested positive for tTGA to confirm the presence of celiac disease. Gluten intake was calculated from 3-day food records collected when the children were 9, 12, 18 and 24 months old. RESULTS: Breastfeeding duration (median 32 weeks) and age at first introduction to gluten (median 22 weeks) did not differ between cases and tTGA-negative children (controls). At the visit prior to tTGA seroconversion, cases reported a larger intake of gluten (median 4.9 g/day) than controls (median 3.9 g/day) (odds ratio [OR], 1.28; 95% confidence interval [CI], 1.13-1.46; P=.0002). Abstract: Acute coronary syndrome (ACS) contributes to the disparity in life expectancy between Aboriginal and non-Aboriginal Australians. Improving hospital care for Aboriginal patients has been identified as a means of addressing this disparity. This project developed and implemented a working together model of care, comprising an Aboriginal Hospital Liaison Officer and a specialist cardiac nurse, providing care coordination specifically directed at improving attendance at cardiac rehabilitation services for Aboriginal Australians in a large metropolitan hospital in Melbourne. A quality improvement framework using a retrospective case notes audit evaluated Aboriginal patients’ admissions to hospital and identified low attendance rates at cardiac rehabilitation services. A working together model of care coordination by an Aboriginal Hospital Liaison Officer and a specialist cardiac nurse was implemented to improve cardiac rehabilitation attendance in Aboriginal patients admitted with ACS to the cardiac wards of the hospital. A retrospective medical records audit showed that there were 68 Aboriginal patients admitted to the cardiac wards with ACS from 1 July 2008 to 30 June 2011. A referral to cardiac rehabilitation was recorded for 42% of these. During the implementation of the model of care, 13 of 15 patients (86%) received a referral to cardiac rehabilitation and eight of the 13 (62%) attended. Implementation of the working together model demonstrated improved referral to and attendance at cardiac rehabilitation services, thereby, has potential to prevent complications and mortality. What is known about the topic? Aboriginal Australians experience disparities in access to recommended care for acute coronary syndrome. This may contribute to the life expectancy gap between Aboriginal and non-Aboriginal Australians. What does this paper add? Abstract: The development of a robust algorithm allowing good recognition of low-quality fingerprints with inexpensive hardware is investigated. A threshold FFT approach is developed to simultaneously smooth and enhance poor quality images derived from a database of imperfect prints. Features are extracted from the enhanced images using a number of approaches including a novel wedge ring overlay minutia detector that is particularly robust to imperfections. Finally, a number of neural net and statistically based classifiers are evaluated for the recognition task. Results for various combinations of the process are presented and discussed with regard to their utility in such a system.

Abstract: Taweelah A2 is the first independent water and power project in Abu Dhabi under the government privatization policy. In response to the Abu Dhabi Water and Electricity Authority (ADWEA) request for proposals, CMS Energy prepared and submitted their offer in April 1998. Following clarifications, the Power and Water Purchase Agreement was signed on 3 October 1998. CMS appointed Mott MacDonald as their technical advisers for the tender and project phases. The Taweelah A2 plant will comprise a 720 MW combined cycle power plant and 50 migd MSF desalination plant. Three Siemens type V94.3a gas turbine generators with exhaust heat recovery boilers will supply steam to two back pressure steam turbine generators. Exhaust steam will be taken to four Hanjung 12.5 migd single deck cross tube MSF distillers, equal in capacity to the largest units ever built. Carbon dioxide gas recovered from the distiller air ejectors will be used for remineralizing the distillate. Seawater will be drawn from an existing intake channel serving the complete complex. The boilers are designed for a high level of supplementary firing to meet the wide variations in electrical load on the Abu Dhabi grid, with full water output. Construction of the US $750 million project, located between the existing Taweelah A1 and B stations, began in April 1999. First power production is scheduled in May 2000, first water production in May 2001 and completion in August 2001. The plant will be owned and operated by Emirates CMS Power, jointly owned by CMS and ADWEA. This paper describes the desalination and power plant and outlines the power and water purchase aspects of the project. The high thermal efficiency of the combined cycle and MSF plant minimizes fuel consumption, heat rejection and atmospheric emissions, making this one of the best plants in the region from an environmental standpoint.

Abstract: Methane (CH4) production from sewers is a suspected, yet relatively undocumented source of greenhouse gases (GHGs). The Intergovernmental Panel on Climate Change (IPCC) published the 2006 IPCC Guidelines for National Greenhouse Gas Inventories which states that "In most developed countries and in high-income urban areas in other countries, sewers are usually closed and underground. Wastewater in closed underground sewers is not believed to be a significant source of CH4." CH4 is a greenhouse gas that has a global warming impact that is 21 times that of carbon dioxide (CO2) and as such is of heightened interest in GHG modeling and inventories. Phase 1 of this project entailed instantaneous methane emissions measurements at 64 raw sewage lift stations throughout the collection system in the DeKalb County, Georgia. During Phase 2 of this project, continuous gaseous- and liquid-phase monitoring were conducted at the discharge of a 16-inch, 3.3-mile-long force main from the Honey Creek Pumping Station. Liquid phase sampling during this phase showed that 8.87 kg of CH4 were emitted per day at the force main receiving manhole during the summer months. The continuous monitoring data was used to calibrate a process model developed by the University of Queensland to predict CH4 and hydrogen sulfide evolution by simulating a force main as a plug-flow, fixed-film reactor. The model was then used to simulate force main CH4 emissions over a calendar year using historical flow and monthly average temperatures. This effort calculated emissions of 52 MT of CO2e/year. Abstract: As electronic systems grow in functional complexity, hence size, the design is often forced into a multi-chip solution. For such systems, the power dissipation due to the off-chip drivers (OCDs) and the off-chip interconnect capacitance can contribute to a significant portion of the overall system power. Often, however, this excessive power dissipation is unwarranted, since a smaller OCD can be used to drive the transmission line load, hence reducing the net capacitance being switched. The objective of this paper is to enable power dissipation trade-off decisions during the high-level phases of design and to minimize the power dissipation of OCDs and their associated interconnect. First, a termination metric is described that uses width optimization of RLC interconnects. Then, in terms of a proposed linear driver model, the low power design objective is posed as an integer programming problem and a branch and bound enumeration algorithm is presented. The driver and interconnect sizes are determined which will preserve signal quality, dispense with additional termination components, meet delay requirements, and minimize the overall power dissipation.

Abstract: The objective of the study was to identify the impact of co-digesting clarifier skimmings on the overall methane generation from the treatment plant and additional energy value of the increased methane production. Biogas production from co-digesting clarifier skimmings and sewage sludge in pilot-scale fed-batch mesophilic anaerobic digesters has been evaluated. The digester was fed with increasing quantities of clarifier skimmings loads: 1.5, 2.6, 3.5 and 7.0 g COD equivalent/(L·d) (COD: chemical oxygen demand). Average volatile solids reduction of 65% was achieved in the scum-fed digester, compared with 51% in the control digester. Average 69% COD removal was achieved at highest scum loading (7 g COD eq/(L·d)) with approximate methane yield of 250 L CH4/kg COD fed (4 ft3/lb COD fed). The results show that scum as co-substrate in anaerobic digestion systems improves biogas yields while a 29% increase in specific CH4 yield could be achieved when scum load is 7 g COD eq/(L·d). Based on the pilot-scale study results and full-scale data from South East Water Pollution Control Plant and Northeast Water Pollution Control Plant the expected annual energy recovery would be approximately 1.7 billion BTUs or nearly 0.5 million kWh.

Abstract: The U.S. Environmental Protection Agency (U.S. EPA) reports that few wastewater treatment plants with anaerobic digestion beneficially use their biogas beyond process heating. Thus, there must be actual or perceived barriers to broader use of biogas to produce combined heat and power (CHP). In 2011, the Water Environment Research Foundation (WERF) and New York State Energy Research and Development Authority (NYSERDA) conducted a study to determine what barriers wastewater utilities face in implementing combined heat and power projects. The project team developed an online survey to determine the most significant barriers facing utilities. This survey was distributed nationally and completed by more than 200 respondents. The survey findings were presented and discussed with dozens of utility representatives at four focus groups timed with industry conferences. Many of the findings of the project were not surprising. Of the 10 barrier categories introduced as potential barriers at the beginning of the project, nine were deemed significant, according the broad input and testing conducted. However, it became clear that economic barriers – inadequate payback/economics and lack of available capital – were dominant. Abstract: In 2008, the New York State Energy Research and Development Authority (NYSERDA) published the Statewide Assessment of Energy Use by the Municipal Water and Wastewater Sector documenting 2003/2004 energy use at New York State wastewater facilities. Since the publication of these data, interest in energy efficiency and energy generation has grown considerably. Survey data indicated that a third of the respondents had completed capital projects to reduce energy use, and 65% indicated that they have future energy projects planned. At the same time, regulatory and technology changes such as tighter nutrient limits have added to many plants' electrical demands. This research assesses the magnitude of energy performance shifts over the past 10 years based on the net effect of efficiency gains, increased electrical production, and increased process equipment loads. In facilities larger than 75 mgd, an overall trend toward increased average electrical energy consumption was observed, with recent implementation of nutrient removal being the most significant factor in this trend. Small- and medium-sized facilities (less than 75 mgd) appear to be slightly improving their energy performance based on analysis of biochemical oxygen demand (BOD)-normalized energy metrics (kilowatt-hours per pound of BOD). Water resource recovery facilities (WRRFs) with more developed energy approaches were more likely to have completed capital projects, operational optimization, and building-related improvements to reduce energy use. Based on input from researchers active in emerging technologies, mainstream shortcut nitrogen and pyrolysis/gasification appear to be the technology areas most likely to be adopted in the near term. These technologies are the most mature and their deployment timeline is estimated to be relatively short. Mainstream shortcut N removal and mainstream anaerobic treatment are expected to have the greatest impact on energy use in the wastewater sector in the near term. Since it is almost impossible to detect and quantify the presence of all possible pathogens in waste matrices, there is a compelling need to identify a suite of indicators that can be used to predict the presence of pathogenic microorganisms in biosolids. The overall objective of this project was to identify those pathogens and surrogate indicator organisms that are at the highest density in raw (untreated) sewage sludge across the United States, and determine the time-temperature relationship under controlled laboratory conditions. The results provide information about the concentrations of an extensive selection of raw sewage-associated organisms across warm and cool seasons and from locations across the United States. Additionally, the 16S pyrosequencing (non-culture based) analysis allowed for the compilation of a list of bacterial DNA sequences that were present in the raw wastewater samples. The second part of this study compared the time-temperature relationships of a subset of commonly used wastewater indicators and pathogens. The untreated sewage sludge samples were obtained from seven different U.S. locations (Illinois, Ohio, Wisconsin, California, Texas, Georgia, and the District of Columbia). Four samples were obtained from each location. Two samples were collected during the warm season (August–September, 2009), and two samples were collected during the cool season (January–February, 2010). Overall, all locations had relatively similar levels of organisms within ± 1-2 log units. There were no seasonal differences in the levels of total coliforms, fecal coliforms, E.coli, C. perfringens spores, Salmonella spp., Aeromonas spp., or Adenovirus. However, Shigella spp., enterococci, somatic coliphages, male-specific coliphages and culturable enteric viruses were higher in the warmer season than in the cooler season. Legionella spp. were higher in the cooler season as compared to the warmer season. In terms of relative abundance, there were greater numbers of indicator organisms such as fecal coliforms (10 8 MPN/g), E. coli (106 MPN/g), and enterococci (106 MPN/g) as compared to the traditional U.S. EPA 503 target pathogens, Salmonella spp. (< 8 MPN/g), culturable enteric viruses (< 1 PFU/g), and helminth ova (< 1 ova/g) in the raw sewage samples as compared to fecal coliforms (10 8 MPN/g), E.coli (106 MPN/g), and enterococci (106 MPN/g). Known pathogens, such as Shigella spp. (25 MPN/g), Legionella spp. (108 CFU/g), Aeromonas spp. (108 CFU/g), MacConkey sorbitol-negative E. coli populations (104 MPN/g) were, however, present in larger numbers. When real-time PCR-based methods were used, the presence of genetic sequences indicative of pathogens such as Adenovirus (10 7 gene copies/g), Giardia spp. (105 gene copies/g) was also evident. Aerobic spores (1066 CFU/g), Cl. perfringens spores (106 CFU/g), somatic coliphages (105 PFU/g) and male-specific coliphages (105 PFU/g) were also present. This suggests that traditional indicators such as fecal coliforms and E.coli, along with other organisms such as coliphages and enterococci, should be included in sewage monitoring studies. Abstract: Fast and efficient detection of anomalies is essential for maintaining a robust and secure network. This research presents a method of anomaly detection based on adaptive Wiener filtering of noise followed by ARMA modeling of network flow data. We dynamically calculate noise and traffic signal statistics using network-monitoring metrics for traffic features such as average port, high port, server ports, and peered ports. The underlying approach is tested on near-real-time Internet traffic in the wide-area network (WAN) of Ohio University. The average port feature is determined to be the most informative measure in the estimation process. High port, server ports, and peered ports are used for confirmation of the anomaly detection result. We empirically determine that most of the network features obey Gaussian-like distributions. Experiments reveal that the method is highly effective in predicting anomalies in network traffic flow and preventing any hazard that they may cause.

Abstract: Objectives The aim of the present study was to explore Aboriginal patients' lived experiences of cardiac care at a major metropolitan hospital in Melbourne. Methods The study was a qualitative study involving in-depth interviews with a purposive sample of 10 Aboriginal patients who had been treated in the cardiology unit at the study hospital during 2012-13. A phenomenological approach was used to analyse the data. Results Eight themes emerged from the data, each concerning various aspects of participants' experiences: 'dislike of hospitals', 'system failures', 'engagement with hospital staff', 'experiences of racism', 'health literacy and information needs', 'self-identifying as Aboriginal', 'family involvement in care' and 'going home and difficulties adapting'. Most participants had positive experiences of the cardiac care, but hospitalisation was often challenging because of a sense of dislocation and disorientation. The stress of hospitalisation was greatly mediated by positive engagements with staff, but at times exacerbated by system failures or negative experiences. Conclusion Cardiac crises are stressful and hospital stays were particularly disorienting for Aboriginal people dislocated from their home land and community. What is known about the topic? Aboriginal people have higher mortality rates due to cardiovascular diseases compared with other Australians. Along with different factors contributing to the life expectancy gap, Aboriginal people also face significant barriers in the use of the healthcare system. What does this paper add? Aboriginal patients' lived experience of cardiac care at a major metropolitan hospital in Melbourne is explored in this paper. Different issues were revealed during their interaction with the hospital staff and the hospital system in conjunction with their cultural aspect of patient care. What are the implications for practitioners? Positive interactions with staff, ongoing support from family and community, culturally appropriate cardiac rehabilitation programs can improve the cardiac care experiences of Aboriginal patients.

Abstract: The combination of high-resolution multi-spectral satellite imagery and advanced COTS object-oriented image processing software provides for an automated terrain feature extraction and classification capability. This information, along with elevation data, infrared imagery, a vehicle mobility model and various meta-data (local weather reports, Zobler Soil map, etc...), is fed into automated path planning software to provide a stand-alone ability to generate rapidly updateable dynamic mobility maps for Manned or Unmanned Ground Vehicles (MGVs or UGVs). These polygon based mobility maps can reside on an individual platform or a tactical network. When new information is available, change files are generated and ingested into existing mobility maps based on user selected criteria. Bandwidth concerns are mitigated by the use of shape files for the representation of the data (e.g. each object in the scene is represented by a shape file and thus can be transmitted individually). User input (desired level of stealth, required time of arrival, etc…) determines the priority in which objects are tagged for updates. This technology was tested at Fort Knox, Kentucky October 11th – 15th 2004. Satellite imagery was acquired in a near-real-time fashion for the selected test site. Portions of the resulting geo-rectified image were compared with surveyed range locations to assess the accuracy of the approach. The derived UGV Path Plans were ingested into a Stryker UGV and the routes were autonomously traversed. This paper will detail the feasibility of this approach based of the results of this testing.

44. Migration weights for pre‐stack converted wave data to reduce the acquisition footprint

Abstract: We have developed a method to reduce the P-S converted wave (c-wave) acquisition footprint caused by irregular geometry effects during acquisition. This method incorporates a weighting scheme based on the relative area in a time slice that is associated with a common conversion point. The first step takes pre-stack input data and produces an offset based common conversion point (CCP) fold distribution map using the RMS p-wave or c-wave velocity field and the Tpp/Tps travel time ratios. The next step computes the c-wave move out time for each input trace. The final step calculates a migration weight for each live common conversion point for each time slice, based on the relative area surrounding the CCP. This weight is used prior to performing a pre-stack migration to reduce the acquisition footprint. The weights are applied to each sample of every input trace based on its CCP and move out time. This method can be applied to either conventional or OBC data. Two examples, one synthetic and one OBC field dataset, are included to demonstrate this method.

Abstract: Classifying selectors are used to control the population of foam-causing organisms in activated-sludge plants to prevent the development of nuisance foams. The term, classifying selector, refers to the physical mechanism by which these organisms are selected against; foam-causing organisms are enriched into the solids in the foam and their rapid removal controls their population at low levels in the mixed liquor. Foam-causing organisms are wasted "first" rather than accumulating on the surface of tanks and thereby being wasted "last", which is typical of the process. This concept originated in South Africa, where pilot studies showed that placement of a flotation tank for foam removal prior to secondary clarifiers would eliminate foam-causing organisms. It was later simplified in the United States by using the aeration in aeration tanks or aerated channels coupled with simple baffling and adjustable weirs to make continuous separation of nuisance organisms from the mixed liquor.

Abstract: A popular approach to the qualitative analysis of thermal patterns has been to identify anomalies through comparison of thermal images against a single baseline or reference image. However, this approach represents an oversimplification as significant variations of thermal patterns due to changing measurement position, changing equipment loading, environmental conditions and varying mechanisms of equipment deterioration are not catered for. To overcome these limitations, the use of neural net and statistically based classifiers has been investigated, in the latter case for both parametric and non parametric designs. An experimental thermal image database characterising normal and abnormal load tap-changer operation of a 63 MVA, 22kV transformer provided the training data. The images were captured at different times, different locations and under varying loads. Various image pre-processing and feature extraction techniques were evaluated, with the objective of determining the best feature extraction-classification combination. Images were first normalised prior to filtering, and thresholding techniques were used to reduce the background clutter and redundant data. Amongst others, Principal component analysis, minimum noise fraction and singular value decomposition were used to intrinsically represent invariant image features. The resulting dimensionality reduced feature vectors were clustered in the N-dimensional feature space to represent the two distinct pattern classes corresponding to normal and defective transformers. Depending on the type of features selected (eg. moments, pixel intensity, texture), a large class overlap resulted in high classification error rates in some cases. However by careful design of the classifier, > 90% classification accuracy was achieved.

Abstract: The clinical criteria for the diagnosis of patent ductus arteriosus are well known and are, in the vast majority of cases, sufficient to establish the diagnosis without recourse to cardiac catheterization or to angiocardiography. Of these criteria, a continuous machinery murmur best heard at the left of the upper sternum has been considered typical of patent ductus and is rarely of other origin (i.e., arteriovenous aneurysm in the same area). However, the earlier insistence that this typical murmur must be present in order to justify surgical exploration1,2 has proved untenable. Furthermore, it has been pointed out3,4 that in infancy or with congestive heart failure a patent ductus may be present with-out murmurs. Several proved cases3,5 have been reported in which only systolic murmurs not continuing into diastole were found. Why does a patent ductus arterious occasionally fail to give rise to a continuous murmur? Burchell5 suggested that in these cases the usual aortic-pulmonary pressure gradient may be absent during diastole or during the entire cardiac cycle. However, no data bearing on this point have thus far been available. In this paper we present two cases of patent ductus arteriosus in children who did not exhibit the classic machinery murmur. In the preoperative evaluation of these patients, cardiac catheterization was helpful in making the diagnosis and, in one of the cases, furnished data which satisfactorily explained the lack of typical physical signs.
