Background Testicular germ cell tumours (TGCT) are the most frequent cancers in young men in developed countries and their incidence rate has doubled worldwide over the past 40 years. Early life exposures to pesticides are suspected to increase TGCT risk. Our research aimed at estimating adult TGCT risk associated with parental domestic use of pesticides during early periods of child development. Methods We conducted a case-control study of 304 TGCT cases, aged 18–45 years old, recruited in 20 French university hospitals, and 274 controls frequency-matched on hospital and birth year. Participants’ mothers provided information on their domestic use of pesticides from 1 year before start of pregnancy to 1 year after their son’s birth, for gardening activities, treatment of indoor plants, pets, wood and mold, and pest control. Odds ratios (OR) for TGCT (overall and by histological subtype) and 95% confidence intervals (CI) were estimated using conditional logistic regression. Results Prevalence of reported domestic use of pesticides was 77.3% for insecticides, 15.9% for fungicides and 12.1% for herbicides. While no association was found for any use of insecticides (OR = 1.27, CI = 0.80–2.01) or herbicides (OR = 1.15, CI = 0.67–2.00), elevated risks of TGCT overall (OR = 1.73, CI = 1.04–2.87) and non-seminoma subtype (OR = 2.44, CI = 1.26–4.74) were observed for any use of fungicides. When specific purposes were examined, using fungicides and/or insecticides for woodwork (OR = 2.35, CI = 1.06–5.20) and using insecticides on cats and dogs (OR = 1.95, CI = 1.12–3.40) were associated with increased risk of non-seminoma subtype. We found no association for seminoma subtype. Conclusions Although recall bias may partially explain the elevated ORs, our study provides some evidence of a positive association between domestic use of pesticides during early periods of development, particularly fungicides and risk of adult TGCT and non-seminoma. Given the common domestic use of pesticides in France, further research on TGCT risk is warranted.

International audience; Background: In a previous study, we showed that the obstetric complication rate after in vitro fertilization (IVF) pregnancy was 40%. The main objective of our study was to determine maternal prognosis factors that influence the IVF pregnancy outcome.Methods: We conducted an observational retrospective monocentric study between January 2014 and January 2018. Pregnancy over 22 gestational weeks (GW) obtained after IVF in our infertility clinic was included. Maternal characteristics and pregnancy outcome were collected.Results: Data from 498 IVF pregnancies were analyzed. The most significant maternal prognosis factors for obstetric complications were maternal age above 40 years (OR 3,0 [95% IC 1,30-7,09], P = 0,010), twin pregnancies (3.8 [95% IC 1.49-9.99], P = .005), daily maternal smoking above 10 cigarettes (7.1 [95% IC 1.22-41.74], P = .029), maternal obesity (2.2 [95% IC 1.19-4.07], P = .012), endometriosis stages III and IV (6.4 [95% IC 1.52-27.04], P = .011), and history of ovarian hyperstimulation syndrome (OHSS) in early pregnancy (5.7 [95% IC 1.29-24.74], P = .021). Risk increase was independent of pregnancy type (singleton or twin) and allowed the elaboration of 2 nomograms.Conclusions: Our study showed a link between some maternal factors and increase in obstetric complications after IVF. Screening of these factors during preconceptional visit is essential to identify at high-risk pregnancies and adapt their monitoring.

Lead readily crosses the placenta and displays adverse effects on birth outcomes and neurodevelopment. Systematic identification of the risk of exposure during pregnancy is essential but rarely performed, probably due to hospital staff&rsquo, s workload and their lack of awareness. We aimed to evaluate the relevance of a questionnaire to screen pregnant women for lead exposure. A cross-sectional, multicentre study was carried out on a population of 792 pregnant women from February 2018 to May 2020. A total of 596 women had a blood lead test: 68.5% had blood lead levels below 10 &mu, g/L. The estimated prevalence above 25 &micro, g/L was 4% (95% confidence interval (CI) [2.6&ndash, 5.9]) and 1.3% had levels above 50 &micro, g/L (95% CI [0.6&ndash, 2.6]). Multivariate analysis showed that three risk factors significantly increased the probability of blood lead levels above 25 &micro, g/L: the use of traditional cosmetics (adjusted odds ratio [aOR]: 3.90, 95% CI [1.65&ndash, 9.21], p = 0.002), degraded old housing (aOR: 2.67, 95% CI [1.19&ndash, 6.038], p = 0.018), and (marginally) eating bread more than twice a day (aOR: 2.40, 95% CI [0.96&ndash, 6.11], p = 0.060). Our study reveals that a three-question tool can be used to quickly screen for the risk of lead exposure in our population and to trigger lead blood tests and special vigilance during pregnancy follow-up.

ObjectiveTo examine the impact of ethanol sclerotherapy (EST) for endometrioma on in vitro fertilization (IVF) cumulative live birth rates (CLBR) in women with moderate-severe endometriosis.MethodsThis retrospective cohort study included women with moderate-severe endometriosis (revised American Fertility Society stage III-IV) and endometrioma who underwent IVF with the ultra-long agonist protocol. We compared two groups: women undergoing EST for endometrioma before IVF (EST group), and women whose endometrioma was left in situ during IVF (No-EST group). The primary outcome was the CLBR per IVF cycle, including fresh and frozen embryo transfers. The secondary endpoints included the complication rate, number of mature oocytes retrieved, clinical pregnancy rate and pregnancy loss rate.ResultsSeventy-four women were included in the study, with 37 in the EST group and 37 in the No-EST group, representing 67 and 69 IVF cycles, respectively. The population and cycle characteristics were comparable between the two groups, especially the ovarian response to stimulation. The CLBR was significantly increased in the EST group compared to the No-EST group (31.3% vs. 14.5%, p = 0.03). The clinical and biochemical pregnancy rates were significantly increased in the EST group (37.3% vs. 15.9%, p = 0.01 and 43.3% vs. 23.2%, p = 0.01, respectively). Multivariate analysis revealed a significantly increased chance of live birth in women exposed to EST before IVF with an adjusted OR of 2.68 (95% confidence interval, CI: 1.13-6.36, p = 0.02). In the EST group, we reported one major complication Clavien and Dindo classification grade III, complication involving an ovarian abscess that required a laparoscopic drainage.ConclusionsEST is an interesting technique to improve IVF success rates in women with moderate-severe endometriosis. EST could be discussed before IVF in infertile women.

Research question Is polycyclic aromatic hydrocarbon (PAH) exposure associated with the reproductive outcomes of IVF treatment? Design A prospective, small-scale monocentric cohort study of couples who underwent IVF treatment between January 2018 and June 2019. Both members of each couple answered a questionnaire on PAH exposure and provided urine samples to measure urinary 1-hydroxypyrene (1-OHP) the day before oocyte retrieval and semen collection for fertilization. To assess the specific PAH exposure of gamete cells, immunostaining was conducted on both spermatozoa and granulosa cells obtained during IVF with an anti-benzo(a)pyrene diol epoxide (BPDE) monoclonal antibody that recognizes BDPE-DNA adducts. To assess DNA damage, a comet assay on spermatozoa was conducted. The PAH exposure was compared between couples who had positive HCG and couples who had negative HCG on day 14 after embryo transfer. Results Eighteen couples were included. The mean 1-OHP level in women whose HCG tests were positive (n = 6) was significantly lower than that in women with negative HCG tests (0.098 [0.042–0.170] versus 0.177 [0.067–0.812] μg/g creatinine; P = 0.048). The presence of BPDE-DNA adducts in granulosa cells of women with a negative (29.7 [16.2–57.5] arbitrary units) or positive HCG test (20.3 [9.3–23.3] arbitrary units) were not significantly different (P = 0.092). The urinary 1-OHP levels of men and BPDE-DNA adducts in spermatozoa showed no differences between groups. Conclusions This exploratory research should encourage further studies to determine the effect of women's exposure to PAHs on reproductive outcomes of IVF treatment.

International audience; SARS-CoV-2 (severe acute respiratory syndrome coronavirus 2) metamorphosed our medical practice. In early June 2020, more than 6,400,000 COVID-19 (coronavirus-19 disease) cases were diagnosed across the world and more than 380,000 deaths were linked to COVID-19. Many medical symptoms of COVID-19 were reported. We will focus, here, on potential impacts of COVID-19 on men's andrological health. Our society (French-speaking society of andrology, SALF) also emitted some recommendations in the andrological management of men infected by SARS-CoV-2. First, considering the fever and the potential presence of SARS-CoV2 in semen, SALF recommends waiting for 3 months (duration of one spermatogenesis cycle and epididymal transit) before re-starting ART in the case of men diagnosed COVID-19 positive. Whatever the nature of testosterone and COVID-19 relationships, we recommend an andrological examination, sperm parameters, and hormonal evaluation at the time of the COVID-19 is diagnosed, and several months later. Furthermore, we are concerned by the potential morbid-mortality of the COVID-19, which mainly affects men. This "andrological bias", if proven, must be reduced by specific andrological diagnosis, therapeutic and prophylactic measures. Research in this direction must be substantiated and financially supported over the next few months (years).; Le SRAS-CoV-2 (nouveau coronavirus ou coronavirus numéro 2 responsable du syndrome respiratoire aigu sévère) a métamorphosé notre pratique médicale. De nombreux symptômes médicaux de cette infection virale ont été signalés. Nous nous concentrerons, ici, sur les impacts potentiels de COVID-19 sur la santé andrologique des hommes. Notre société (Société d’andrologie de langue Française, SALF) émet ici quelques recommandations dans la prise en charge andrologique des hommes infectés par le SRAS-CoV-2. Tout d’abord, compte tenu de la fièvre et de la présence potentielle du SRAS-CoV2 dans le sperme, la SALF recommande d’attendre 3 mois (durée d’un cyclede spermatogenèse et transit épididymaire) avant de recommencer les techniques d’assistance médicale à la procréation pour les hommes diagnostiqués COVID-19 positifs. Quelle que soit la nature des relations entre la testostérone et l’infection à SARS-CoV-2, nous recommandons un examen andrologique, un examen des paramètres du sperme et une évaluation hormonale au moment du diagnostic de l’infection, ainsi qu’à distance (3–6 mois plus tard). De plus, nous sommes préoccupés par la morbidité et la mortalité potentielles de l’infection COVID-19, qui touche principalement les hommes. Ce “biais andrologique”, s’il est. prouvé, doit être réduit par un diagnostic andrologique spécifique et des mesures thérapeutiques et prophylactiques.La recherche dans ce sens doit être étayée et soutenue financièrement au cours des prochains mois (années).

ObjectiveWe sought to determine whether the early luteal serum progesterone (P4) level predicts the success of IVF treatment with oral dydrogesterone for luteal support.MethodThis retrospective monocentric cohort study included 242 women who underwent IVF treatment with fresh embryo transfer (ET) between July 2017 and June 2018. The population was unselected, and women were treated according to our unit's usual stimulation protocols. For the luteal phase support (LPS), all women were supplemented with a 10 mg three-times-daily dose of oral dydrogesterone beginning on the day of oocyte pick-up (OPU). Blood sampling was performed on the day of ET (Day 2-3 after OPU) to determine the early luteal serum progesterone level.ResultsROC curve analysis allowed us to determine two thresholds for the prediction of live birth using the early P4 level. Women who had early luteal P4 levels greater than 252 nmol/l had a significantly higher live birth rate (27.1%) than women with early luteal P4 between 115 and 252 nmol/l (17.2%) and women with early luteal P4 below 115 nmol/l (6.0%; p = 0.011). After a multiple regression analysis, an early luteal P4 level greater than 252 nmol/l was still associated with a higher chance of a live birth than a P4 between 115 and 252 nmol/l (OR = 0.40 [0.18-0.91]; p = 0.028) or a P4 below 115 nmol/l (OR = 0.10 [0.01-0.52]; p = 0.006).ConclusionsOur study suggests a positive association between early P4 levels and reproductive outcomes in IVF using oral dydrogesterone for luteal support. The inconsistencies between our results and those of other studies suggest that extrapolation is impractical. Further larger prospective cohort studies should be conducted to determine reliable thresholds that could be used to personalize luteal phase support.

Introduction Ovarian reserve is a major prognosic factor for Medical Assisted Procreation. Tubal surgery, realised close to mesosalpinx and ovarian vascularization, could impare ovarian function. However, salpingectomy is currently used to treat ectopic pregnancies or hydrosalpinx before IVF attempt. Disponible studies on this subject are unclear. The aim of this study is to evaluate the impact of salpingectomy for ectopic pregnancy on the ovarian response during IVF attempt. Material and methods It was a single center comparative study. Included patients were the one receiving oocyte puncture for IVF attempt, with a history of unilateral tubal surgery: salpingectomy for ectopic pregnancy. We conducted a case-control study, comparing the sonographic parameters of the surgery ovary (case) to those of the safe ovary (control) during the first IVF attempt after salpingectomy. The ovarian sonographic response was evaluated according to the follicular antral count on day 3 and the sonographic follicular count on trigger day. Results 55 patients were included. There was no significant difference in the number of recruited follicles on the operated side versus control side (p = 0.85 for >14 mm follicles, p = 0,46 for 10 to 14 mm, p = 0,52 for total amount of recruited follicles). There was no significant difference for the follicular antral count neither (p = 0.79). Discussion In our population, there was no significant difference in the sonographic ovarian response to IVF stimulation between the ovary on the operated side and the control ovary among patients treated by unilateral salpingectomy for ectopic pregnancy.

International audience; Objectives: Pregnancy rate in in vitro fertilization (IVF) depends on many factors, such as the characteristics of the couple and the clinicobiological parameters. Interest in alternative and complementary medicine (ACM) for IVF is discussed because of the lack of scientific evidence. Energy resonance by cutaneous stimulation (ERCS), an acupuncture-like technique, consists of skin stimulation to transmit vibratory messages. The aim of this study was to evaluate the effect of ERCS on live birth rates (LBRs) in IVF. Design: A prospective observation study was performed in the Unit of Assisted Reproductive Technologies of a University Teaching Hospital. Every woman who agreed to participate in this study and received a fresh embryo transfer (ET) after IVF or Intracytoplasmic Sperm Injection (ICSI) was included. Patients randomly underwent an ERCS session on the day of ET according to the schedule of the midwife performing this technique. The control group consisted of women undergoing ET under usual conditions. The main outcome measure was the LBR per transfer. Results: Three-hundred-eighteen women were included, 120 in the ERCS + ET group and 198 in the ET without ERCS group. None of the women dropped out. The clinical characteristics in both groups were comparable. There was a significant difference in the clinical pregnancy rates, 31.7% in the ERCS group versus 21.7% in the No ERCS group (p = 0.037). The LBR in the ERCS group was nearly significantly higher, 29.2% versus 20.7% in the No ERCS group (p = 0,059). Conclusions: Women undergoing ERCS on the day of ET had a significantly higher IVF pregnancy rate. However, this methodology made not possible to draw conclusions about the mechanisms that induced the increase of IVF LBRs: placebo effect, ''cocooning,'' or ERCS self-effect? Further well-conducted studies are strongly needed to assess ERCS efficacy.

Alteration of sperm parameters related to occupational exposures is the subject of several studies, often on a case-control approach. The study populations usually comprise men consulting in infertility clinics for couple infertility. The objective of this review is to identify, from these case-control studies, the main occupational factors that may be associated with altered sperm parameters. We selected 13 articles in the PubMed database. Participation in these studies varied from 61 to 2619 subjects, with great methodological heterogeneity, particularly in the characterization of exposure. The main occupations that appear significantly associated with a risk of altered sperm parameters are workmen, painters, farriers, welders, plumbers and technicians. When analysis focuses on occupational exposures, a significant result is reported for solvents, heavy metals, heat, vibrations and non-ionizing radiation. International audience; Objectives: Solvent exposure is among the most common occupational exposures to chemical toxicants; data about the impact of such exposure on semen parameters are contradictory. We conducted the first meta-analysis to evaluate the risk of alteration in semen parameters related to occupational exposure to solvents. Methods: From the PubMed database, we selected studies analyzing the semen of subjects occupationally exposed to solvents, compared with unexposed controls. The meta-analysis was performed on the various semen parameters analyzed in both populations. Results: Seven studies were included in the study. The meta-analysis revealed a significant decrease in ejaculate volume [standardized mean difference (SMD) ¼ À0.35 (À0.63 to À0.07)] and sperm concentration [SMD ¼ À0.36 (À0.64 to À0.08)] in workers exposed to solvents compared with unexposed controls. Conclusion: Our results highlight the importance of preventing reprotoxic risks to male fertility in the workplace.

Nanoparticles (NPs) are sized between 1 and 100 nm. Their size allows new nanoscale properties of particular interest for industrial and scientific purpose. Over the past twenty years, nanotechnology conquered many areas of use (electronic, cosmetic, textile...). While, human is exposed to an increasing number of nanoparticles sources, health impacts and, particularly on reproductive function, remains poorly evaluated. Indeed, traceability of nanoparticles use is lacking and nanotoxicology follows different rules than classical toxicology. This review focuses on the impact of NPs on health and particularly on fertility and addresses potential risks of chronic exposure to NPs on human fertility. (C) 2014 Elsevier Masson SAS. BACKGROUND: The purpose of this retrospective observational study was to identify prognostic factors that lead to a live birth (LB) in couples with unexplained infertility in order to define the best assisted-reproductive technique (ART) strategy. METHODS: Prognostic factors of couples with unexplained infertility managed initially with gonadotropin intrauterine inseminations (IUI) at a single university fertility center were analyzed. Infertility was not considered “unexplained” in case of mild male infertility and suspicion of diminished ovarian reserve (FSH>10 IU/L). ART management consisted to start with IUI cycles and then, if failure, to propose in vitro fertilization (IVF). Couples were compared according to the results of IUI cycles in terms of LB. RESULTS: Between January 2011 and July 2015, 133 couples with unexplained infertility were included (320 IUI cycles). The average age of women was 31.6±4.6 years and the average number of IUI per couple was 2.4±1.2. The IUI live birth rate (LBR) was 37.6%, with an average of 2 cycles to obtain a pregnancy. For 63 couples, no pregnancy occurred after IUI cycles. The prognostic factors of the two groups “LB after IUI” vs. “no LB after IUI” were not statistically different. The remaining 20 couples had a spontaneous pregnancy with a LB. Cumulative LBR, including spontaneous and ART pregnancies, was 65.7 %. Of the 63 couples with no LB after IUI, 33.3% dropped-out from infertility treatments before starting IVF. CONCLUSIONS: To avoid couple’s drop-out, we advise to start infertility treatment for unexplained infertility with two IUI before undergoing IVF if IUI failure.

Objectifs L’objectif de cette etude etait de presenter la prise en charge globale des blesses medullaires dans notre centre, depuis le recueil de sperme pour preservation de la fertilite jusqu’au resultats de l’utilisation des paillettes en aide medicale a la procreation (AMP). Methodes Il s’agissait d’une etude descriptive retrospective monocentrique incluant tous les hommes blesses medullaires adresses dans notre Centre d’etude et de conservation des œufs et du sperme humains (CECOS) entre 2002 et 2018 pour preservation de la fertilite et/ou desir de paternite immediat. Les caracteristiques generales de la population etaient analysees ainsi que le detail des lesions medullaires. Le sperme etait recueilli par voie anterograde et/ou retrograde par technique de Hotchkiss modifiee soit apres masturbation, soit apres stimulation vibratoire penienne (SVP) soit enfin apres prelevement chirurgical. Les tentatives d’AMP etaient ensuite realisees en fonction du projet parental, des parametres spermatiques et des facteurs feminins. Resultats Cent vingt-deux patients ont ete inclus dans l’etude avec un âge median de 31 ans au moment de la prise en charge. Le delai median entre la lesion medullaire et le debut de la prise en charge etait de 3,09 ans. Vingt-sept couples (22 %) avaient des antecedents de fertilite avec 29 grossesses et 19 parites. Soixante-six (54 %) patients consultaient pour preservation de leur fertilite alors que 52 d’entre eux (43 %) avaient deja un desir d’enfant. Au total, 346 tentatives de recueil de sperme ont ete realisees conduisant a 183 (53 %) essais fructueux (18/26 par masturbation, 143/297 par SVP et 22/23 par chirurgie). Au total, 1028 paillettes ont pu etre congelees. Trente-huit couples ont pu beneficier de tentatives d’AMP ayant conduit a 25 grossesses et 18 naissances vivantes. Conclusion La prise en charge des blesses medullaires a permis de preserver la fertilite de 122 patients et d’obtenir un taux de naissance cumule de pres de 50 %. Une collaboration formalisee entre centres de reeducation, urologues et biologistes de la reproduction permettrait de proposer une information systematique et une prise en charge en AMP adaptee au niveau lesionnel.

International audience; Purpose Does the type of anesthesia (paracervical block (PCB) or general anesthesia (GA)) impact live birth rate, pain, and patient satisfaction? Methods A non-randomized prospective cohort study was conducted in women treated for IVF. Two groups of patients were prospectively included: the PCB group (n = 234) and the GA group (n = 247). The type of anesthesia was determined by the patients. The primary endpoint was cumulative live birth rate by OR. Secondary endpoints were self-assessment of the patients' peri-operative abdominal and vaginal pain vs the doctors' evaluations during PCB, post-operative abdominal and vaginal pain level, and patient satisfaction in both groups. Pain levels were assessed with a numerical rating scale (NRS). Results The live birth rate was similar in both groups (19.8% in the GA group vs 20.9% in the PCB group, P = 0.764). During oocyte retrieval in the PCB group, the physicians significantly under-estimated the vaginal pain experienced by the patients (3.04 +/- 0.173 for patients vs 2.59 +/- 0.113 for surgeons, P = 0.014). Post-operative vaginal and abdominal pain were significantly greater in the PCB group compared to the GA group (2.26 +/- 0.159 vs 1.66 +/- 0.123, respectively, P = 0.005, and 3.80 +/- 0.165 vs 3.00 +/- 0.148, respectively, P < 0.001). Patients were more significantly satisfied with GA than with PBC (P < 0.001). Conclusion Because the LBR was similar in both groups and patient satisfaction was high, the choice of anesthesia should be decided by the patients.

International audience; Objective: To evaluate the percentage of parents in one French center for the study and preservation of eggs and sperm who disclose their use of donated spermatozoa to their children.Design: A questionnaire survey of couples who had a child using donated spermatozoa.Setting: University hospital laboratory.Patient(s): One hundred five couples.Intervention(s): Questionnaire sent by mail.Main Outcome Measure(s): The percentage of parents who disclose their use of donated spermatozoa to their child.Result(s): Among the 157 questionnaires sent, 105 couples answered, which corresponded to 138 children. There were 40 (38%) couples who had already disclosed the donor origin to their child and 65 (62%) who had not. Of the 40 couples who disclosed the donor origin, 37 (93%) had intended to do so before making use of assisted reproductive techniques (ART), but two (5%) had not wanted to do so before ART. Among the 65 couples who did not inform their child, 42 (65%) planned to inform their child soon, but 20 (31%) wanted to keep the sperm origin secret. Of the 20 couples who wanted to keep the origin secret, nine couples had told other persons about the gamete donation but had not informed their child and do not intend to inform their child in the future.Conclusion(s): This first report about disclosure attitude in a large cohort of parents of donor-conceived offspring in France showed that most parents had already disclosed their use of donated spermatozoa to their children or intended to disclose it soon and had an attitude after birth consistent with their intentions prior to ART.

International audience; Objective: The aim of this study was to evaluate the information pregnant women received regarding possible exposures to five recognized reprotoxic agents during their pregnancy. Study design: A cohort study was conducted using two postnatal units in France. Women hospitalized in postnatal units were requested to complete a self-administered two part questionnaire. The first part gathered information about the patient's socio-professional level and the type of pregnancy follow-up. The second part examined the information the patient received regarding daily products containing the following known reprotoxic agents: bisphenol A, toluene, n-hexane, cis-chloroallyl-triaza-azonia-adamantane-chloride and O-phenyl-phenol. The women cited the sources of information. We combined the employment status and educational level to separate the women into two groups. The groups were then compared using the Chi Square test or Fisher's exact test. Result(s): There were 390 women in this study. Our results showed the women received information regarding the following: 21.6% (n=84) regarding tin cans, 21.9% (n =85) concerning plastic meal boxes when heated in microwave ovens, 8.8% (n=32) about water in gas-bottles, 27.4% (n=106) about non organic foods, 39.3% (n= 152) about hair dyes, 17% (n = 66) about nail polishes, 23.4% (n =103) about insect repellents, 34.4% (n=133) about “do-it-yourself” products, 2.1% (n=8) about gardening products, 26.7% (n=103) about electric plug-in repellents, 21.1% (n= 81) about housekeeping products, and 6.8% (n= 26) about register receipts. Women with a higher level of education and a qualified occupation were better informed about these daily products. These women were more likely to learn the information on their own (internet, media). Conclusion(s): Our study showed French women did not receive sufficient information regarding potential exposures to reprotoxic agents during pregnancy. (C) 2016 Elsevier Ireland Ltd. International audience; Beside cytotoxic drugs, other drugs can impact men's fertility through various mechanisms. Via the modification of the hypothalamic-pituitary-gonadal axis hormones or by non-hormonal mechanisms, drugs may directly and indirectly induce sexual dysfunction and spermatogenesis impairment and alteration of epididymal maturation. This systematic literature review summarizes existing data about the negative impact and associations of pharmacological treatments on male fertility (excluding cytotoxic drugs), with a view to making these data more readily available for medical staff. In most cases, these effects on spermatogenesis/sperm maturation/sexual function are reversible after the discontinuation of the drug. When a reprotoxic treatment cannot be stopped and/or when the impact on semen parameters/sperm DNA is potentially irreversible (Sulfasalazine Azathioprine, Mycophenolate mofetil and Methotrexate), the cryopreservation of spermatozoa before treatment must be proposed. Deleterious impacts on fertility of drugs with very good or good level of evidence (Testosterone, Sulfasalazine, Anabolic steroids, Cyproterone acetate, Opioids, Tramadol, GhRH analogues and Sartan) are developed.

With the improvement of the anticancerous treatments, the preservation of the feminine fertility before gonadotoxic treatment tends at present to stand out as a legal obligation, with a duty of information to patients. When emergency IVF can be performed, the cryopreservation of embryos is the best mastered method which offers most chances to patients to obtain a pregnancy after cancer remission thanks to the transfer of frozen embryos. This article proposes an overview about the indications, the feasibility and the ethical and practical limitations of IVF emergency for embryo freezing before gonadotoxic anticancerous treatment.

Resume But Decrire et evaluer une nouvelle technique de recueil des spermatozoides (SPZ) en cas d’ejaculation retrograde refractaire au traitement medical. Methode Etude prospective comparant une technique de recueil des SPZ apres ajustement per os des parametres physicochimique des urines (U) a une technique d’instillation endovesicale de milieu de conservation des gametes (Ferticult ® ) avant ejaculation (F). Chaque patient etait son propre temoin. L’evaluation a porte sur la mobilite et la vitalite des SPZ. Les couples ont ete suivis au moins un an quant aux resultats de leur recours a l’aide medicale a la procreation (AMP). Resultats Le passage a la technique F permettait d’ameliorer significativement mobilite et vitalite chez tous les patients (8/8). Sept patients sur huit (87 %) passaient d’un test de decongelation negatif avec prelevement U a un test de decongelation positif et une possibilite d’AMP apres prelevement F. Quatre couples ont utilise les SPZ preleves durant l’etude 3 ont obtenu une a trois grossesses donnant naissance a cinq enfants. Conclusion L’instillation endovesicale de milieu de conservation des gametes avant ejaculation a ete une technique efficace de recueil des SPZ en cas d’hypofertilite par ejaculation retrograde refractaire au traitement medical. Entre nos mains et sur un effectif restreint, elle a permis d’obtenir de meilleurs resultats que l’ajustement per os des parametres physicochimique des urines.

Men with spinal cord injury present a unique infertile population. Only 10 % of them can father children without medical assistance, owing to potential impairments in erection, ejaculation and semen quality. The algorithm typically followed is to retrieve semen by Penile Vibratory Stimulation, in case of failure by Electro Ejaculation. Most of these patients have normal sperm concentrations but abnormally low sperm motility and vitality in the ejaculate. The reasons for poor semen quality in spinal cord injured men are reviewed. If semen cannot be obtained by Electro Ejaculation, or if the ejaculate from Penile Vibratory Stimulation or Electro Ejaculation contains an insufficient quantity or quality of sperm for in vitro fertilization with intracytoplasmic sperm injection, then retrieval of sperm from reproductive tissues is attempted. Despite abnormal semen quality, successful pregnancies with sperm from spinal cord injured male partners have occurred by intravaginal insemination, intrauterine insemination, and in vitro fertilization with intracytoplasmic sperm injection. The prevailing pregnancy and fecundity rates in couples with a spinal cord injured male partner are reviewed.

International audience; OBJECTIVE To compare the options available for prostate cancer management and their consequences on fertility for men, in order to best advise these men to choose their treatment, especially if they are young, with no child with their current partner. METHODS A literature review on prostate cancer and fertility over the last 26 years was carried out on PubMed database. The literature was based on evidence and practical considerations. Twenty-nine articles were selected according to their relevance. RESULTS After prostatectomy, there is an obstructive infertility in 100% of the cases. In external radiotherapy, doses more than 15 cGy induced reduction in sperm count. Direct irradiation between 15 and 35 cGy caused oligozoospermia and doses between 35 and 50 cGy caused reversible azoospermia. The calculated projected doses on testicles were 196 cGy (+/- 145 cGy). The brachytherapy effects on fertility seems to be less harmful. The irradiation dose received by testicles is less important (less than 20 cGy) after brachytherapy than after external radiotherapy. Infertility induced by hormonal therapy alone should be reversible. Fertility and focal therapy have not been well evaluated yet. Active surveillance is the management of prostate cancer which allows to keep at best men's fertility. CONCLUSION Urologists should consider approaching the topic of infertility when discussing the pros and cons of various prostate cancer management with their younger patients. If a patient with prostate cancer expresses interest in future fertility, a semen analysis should be performed. Cryopreservation represents the only preemptive accompanying possibility to preserve fertility in young cancer patients. (C) 2015 Elsevier Inc.

International audience; Objectives: The breastfeeding initiation rate is extremely disparate worldwide and remains low in France, with a breastfeeding initiation rate of 60.2%. Some studies have reported increased early parenting difficulties in women who conceived with in vitro fertilization (IVF). Our study had two main objectives: (1) to determine by a retrospective chart review the demographic and medical factors associated with breastfeeding versus formula feeding in women who had undergone IVF and (2) to assess by survey study the reasons for the infant feeding decision (breastfeeding versus formula feeding) for women who conceived by IVF. Subjects and Methods: By telephone interviews, we conducted a longitudinal nonexperimental cohort study of French women who had a live birth after IVF. Fertility data and obstetric outcomes were collected from individual medical records. Results: Among women who had a live birth after IVF (n=73), 63% (n=46) initiated breastfeeding for a mean duration of 6.2 +/- 6.2 months. In our IVF population, the breastfeeding initiation rate was similar to that in the general French population. The following three factors were found to have a significant negative influence on breastfeeding initiation: duration of infertility greater than 2 years, cesarean delivery, and history of formula feeding when the women themselves were newborns. Conclusions: Long duration of infertility and history of cesarean delivery after IVF negatively influenced the breastfeeding initiation rate. For this subgroup of patients, postpartum care should be enhanced to support the early mother-baby relationship and promote breastfeeding.

Resume Objectif. – Evaluer les resultats d'un protocole d'insemination avec sperme de donneur, associant stimulation et monitorage de l'ovulation, preparation du sperme par migration ascendante et insemination intra-uterine, propose aux patientes de moins de 39 ans. Apprecier l'influence de differents parametres cliniques et biologiques sur les taux de grossesses. Patients et methodes. – Analyse retrospective de 249 cycles (106 couples) realises sur une periode de quatre ans. Resultats. – Le taux de grossesses par cycle est de 28,1 %, le taux de grossesses multiples de 11,4 %/cycle et d'accouchement/cycle de 22 %. La majorite des grossesses (84 %) est obtenue avant la quatrieme tentative. Parmi les facteurs etudies, seule une relation entre le nombre de spermatozoides mobiles progressifs insemines et les chances de grossesse a ete mise en evidence : le taux de grossesses/cycle est de 40,4 % si plus d'1,5 million de spermatozoides mobiles progressifs est insemine contre 24,7 % si ce nombre est inferieur ou egal a 1,5 million (p Discussion et conclusion. – Les inseminations avec sperme de donneur realisees dans des conditions precises permettent d'obtenir des taux de grossesses satisfaisants et de repondre a l'attente des couples ayant souvent un parcours difficile vis-a-vis de l'infertilite.

Cervical dislocation is a commonly used method of mouse euthanasia. Euthanasia by isoflurane inhalation is an alternative method which allows the sacrifice of several mice at the same time with an anaesthesia, in the aim to decrease pain and animal distress. The objective of our study was to assess the impact of these two methods of euthanasia on the quality of mouse oocytes. By administering gonadotropins, we induced a superovulation in CD1 female mice. Mice were randomly assigned to euthanasia with cervical dislocation and isoflurane inhalation. Oviducts were collected and excised to retrieve metaphase II oocytes. After microscopic examination, oocytes were classified into three groups: intact, fragmented/cleaved and atretic. Intact metaphase II oocytes were employed for biomedical research. A total of 1442 oocytes in the cervical dislocation group were compared with 1230 oocytes in the isoflurane group. In the cervical dislocation group, 93.1% of the oocytes were intact, versus 65.8% in the isoflurane group ( P ≤ 0.001). In light of these results, we conclude that cervical dislocation is the best method of mouse euthanasia for obtaining intact oocytes for biomedical research.

Objectifs Les traumatismes medullaires surviennent le plus souvent chez des hommes jeunes a l’apogee de leur sante reproductive et sexuelle. Le projet parental est une part importante du processus de readaptation. L’objectif de notre etude etait de faire un etat des lieux de pratiques techniques et strategie de recueil du sperme des hommes blesses medullaires (HBM). Methodes Un questionnaire comportant 12 items sur la prise en charge de la fertilite de l’HBM a ete adresse aux cliniciens et biologistes specialises en Assistance medicale a la procreation (AMP), via les listes de diffusion de quatre societes savantes : Societe d’andrologie de langue francaise, Association francaise d’urologie, Societe de medecine de la reproduction, Association de biologistes d’etudes de la fecondation et de la conservation de l’œuf (BLEFCO). Resultats Soixante-dix-huit questionnaires complets ont ete retournes. Seuls 15 % des praticiens declaraient faire un prelevement chirurgical en premiere intention. Soixante et onze pour cent des praticiens proposaient en premiere intention un recueil de sperme par stimulation vibratoire penienne (SVP) et/ou electro-ejaculation (EEJ) (26 %). Les raisons pour ne pas proposer une SVP ou une EEJ etaient le manque de formation et l’absence de materiel. Cinquante-huit pour cent des praticiens faisaient une congelation de gametes en vue d’une AMP asynchrone. Le seuil de spermatozoides mobiles necessaire a une IIU etait majoritairement de 1million. Soixante-seize pour cent des praticiens utilisant la SVP proposaient aux couples une utilisation a domicile. Conclusion La SVP et l’EEJ permettent le recueil de sperme pour 95 % des HBM. Ce taux de succes, la facilite de mise en œuvre de la SVP, l’alteration du spermogramme a la phase chronique d’un traumatisme medullaire, incitent a proposer un recueil de sperme pour congelation aux HBM dans la premiere annee de reeducation.

In spinal cord injured (SCI) patients, three main factors may cause infertility: erectile dysfunction, ejaculatory dysfunction and impaired semen quality. This letter aims to discuss how we can manage SCI patients’ fertility in accordance with patient-centred care. For such SCI patients aged 20 to 40, having children represents hope for the future. Furthermore, it is a way to rebuild a life after the spinal injury and must be seen as an important part of the rehabilitation program. We suggest that sperm cryopreservation may contribute to patient-centred care management of SCI patients’ fertility, although there is no scientific evidence that cryopreservation will improve fertility outcome after SCI. Indeed, sperm cryopreservation is an affordable and simple technique in specialised centres with trained staff. Here, a protocol to manage SCI patients’ fertility is discussed: we propose PVS for sperm banking to all SCI patients after the phase of spinal shock during the rehabilitation program. If live sperm are retrieved, they are frozen and stored; however, if no live sperm are retrieved, electroejaculation and/or surgical sperm extraction are proposed only for patients who desire biological fatherhood. Prospective studies on the evolution of semen parameters, ejaculatory dysfunction, post-infectious obstructions and spermatogenesis impairment in chronic SCI patients are urgently needed to provide robust data for the evidence-based management of SCI patients’ fertility. Even if use rates are expected to be low, sperm banking may be a simple and affordable preventative measure for selected male SCI patients.

In industrial countries, evidence suggests that semen quality has been steadily decreasing over the past 5 decades. We employed a short questionnaire to examine the association between self-reported physical or chemical occupational exposures and semen quality. The study included 402 men consulting for couple infertility (314 with oligospermia, asthenospermia, or teratospermia and 88 with normal semen; World Health Organization criteria). Exposure effects on global sperm quality and total sperm count, sperm motility, and sperm morphology were investigated. We found significant associations between semen impairment and occupational risk factors such as exposure to heavy metals (adjusted odds ratio [OR] = 5.4; 95% confidence interval [CI], 1.6-18.1), solvents (OR = 2.5; 95% CI, 1.4-4.4), fumes (OR = 1.9; 95% CI, 1.1-3.4), and polycyclic aromatic hydrocarbons (OR = 1.9; 95% CI, 1.1-3.5). Exposure to pesticides or cement was nearly significant (OR = 3.6; 95% CI, 0.8-15.8, and OR = 2.5; 95% CI, 0.95-6.5, respectively). Physical risk factors were associated with some sperm anomalies, such as mechanical vibrations with oligospermia and teratospermia as well as excess heat and extended sitting periods with impaired motility. Exposure to ionizing radiation and electromagnetic fields was not associated with semen impairment; these results, however, may be skewed, because very few subjects reported such exposure. Despite the small dataset, self-reported exposures were correlated with semen impairment. This approach may be recommended in routine clinical practice to seek relationships between occupational exposures to reprotoxic agents and impaired semen parameters. This knowledge would allow preventive measures in the workplace to be established and could be complemented by the use of biomarkers to better characterize exposure to chemical substances and their spermiotoxic effects.

In couples presenting with retrograde ejaculation refractory to medical treatment, the first choice of fertility treatment should be Assisted Reproductive Techniques using rapidly purified spermatozoa retrieved from post-ejaculatory urine. The Hotchkiss technique and modified variants are simple and efficient for retrieving sperm from the bladder. We developed a new protocol, including a novel modified Hotchkiss technique involving sperm cryopreservation. The aim was to study the pregnancy rate and birth rate achieved by intra cytoplasmic sperm injection (ICSI) using frozen-thawed sperm retrieved from the bladder with this novel modified Hotchkiss technique in patients with refractory retrograde ejaculation.In this descriptive retrospective, single-center study, we analyzed the local database of all patients who banked sperm at the CECOS Laboratory Biology of Reproduction of La Conception University Hospital, Marseille, France, between 2004 and 2014. A total of 2171 patients banked sperm during this time, including 63 patients with retrograde ejaculation, of whom ten patients banked sperm that had been retrieved by the modified Hotchkiss technique. These ten couples underwent 26 ICSI cycles: nine clinical pregnancies were achieved in six couples, including eight after fresh embryo transfer and one after thawed embryo transfer, resulting in seven live births. The average live birth rate per transfer was 28 %.We report the largest series of births using frozen-thawed spermatozoa retrieved from post-ejaculatory urine by a modified Hotchkiss technique. This series of births demonstrates that this new modified Hotchkiss technique allows for successful association with sperm cryopreservation, leading to an efficient and easy management of couples with refractory retrograde ejaculation.Chez les couples dont l’homme présente une éjaculation rétrograde réfractaire au traitement médical, la première ligne de traitement de l’infertilité est le recours à l’aide médicale à la procréation (AMP) utilisant des spermatozoïdes purifiés des urines après éjaculation. Nous avons développé un nouveau protocole avec une nouvelle technique de Hotchkiss modifiée impliquant une cryoconservation des spermatozoïdes obtenus. L’objectif de cette étude était d’étudier les taux de grossesses et de naissances obtenues après injection intra cytoplasmique de spermatozoïdes (ICSI) provenant de paillettes congelées à partir des spermatozoïdes purifiés des urines avec la nouvelles méthode de Hotchkiss modifiée, chez les patients atteints d’éjaculation rétrograde réfractaire.Dans cette étude descriptive monocentrique rétrospective, nous avons analysé la base de données locale de tous les patients ayant conservé du sperme au CECOS du laboratoire de biologie de la reproduction de l’hôpital universitaire de la Conception à Marseille, France, entre 2004 et 2014. Un total de 2171 patients a conservé du sperme pendant cette période. Soixante trois présentaient une éjaculation rétrograde, parmi lesquels 10 ont conservé du sperme qui avait été purifié des urines selon la méthode de Hotchkiss modifiée. Ces 10 couples ont réalisé 26 tentatives d’ICSI. 9 grossesses cliniques ont été obtenues chez 6 couples; 8 après transfert d’embryon frais, et une après transfert d’embryon congelé, ayant conduit à 7 naissances. Le taux moyen de naissance par transfert était de 28 %.Nous rapportons ici la plus large série de naissances utilisant des paillettes de spermatozoïdes purifiés des urines après éjaculation rétrograde selon une méthode de Hotchkiss modifiée. Cette série de naissances démontre que cette nouvelle technique de Hotchkiss permet une association réussie avec la cryoconservation du sperme, permettant une prise en charge facile et efficace des couples avec éjaculation rétrograde réfractaire.

Male infertility related to professional reprotoxic exposure has been assessed in several studies. Collaboration between occupational physicians and patients can yield information about the preventive measures that can be taken to avoid such exposure. The use of preventive measures is determined by the collaboration between reproductive medicine and occupational medicine and also by the patient's awareness of reprotoxic occupational exposures. Our andrology laboratory developed a systematic environmental interview that an occupational physician administers before semen analysis to assess patients' occupational reprotoxic chemical and physical exposures. This observational prospective study evaluated patients' feelings regarding this interview. The main outcome measure was the participants' score to determine their general reprotoxicant knowledge. The study also evaluated the patients' satisfaction about the interview with occupational physician and their attitude about reproductive toxicants.The mean score for general knowledge of reprotoxicants was 9.6 ± 2.7/16. The most frequently underestimated reprotoxic factor was excessive heat (34.7 % correct responses). In cases of semen parameter abnormalities AND recognized occupational reprotoxic exposure, 63.2 % of the patients said they would use individual protective devices, and 55.1 % said they would temporarily adapt their workstation. Regarding the interview with the laboratory's occupational physician, 80.7 % considered it moderately or very useful. Of the interviewed patients, 46.2 % reported having changed their living habits 2 months after the interview, and 88.5 % were satisfied or very satisfied with the care they received. All of the respondents said it would be useful to extend the interview to include their wives.The data suggest that patients' knowledge about reprotoxic exposures can be improved, particularly knowledge related to physical exposure. The vast majority of patients were satisfied with the introduction of this new collaboration between reproductive and occupational medicine.Plusieurs études ont mis en évidence des infertilités masculines en lien avec une exposition reprotoxique professionnelle. L’interaction entre médecin du travail et patient peut faire émerger des informations sur les mesures preventives à mettre en place pour éviter ces expositions. L’utilisation de ces mesures préventives est déterminée par la collaboration entre médecin de la reproduction et médecin du travail et également par la conscience du patient de subir une exposition professionnelle reprotoxique. Notre laboratoire de spermiologie a mis en place un entretien systématique environnemental, assuré par un médecin du travail avant l’analyse de sperme, dans le but d’établir les expositions professionnelles reprotoxiques chimiques et physiques de chaque patient. Cette étude observationnelle prospective évalue la perception des patients de cet entretien. Le critère d’évaluation principal est le score de chaque patient évaluant ses connaissances générales sur les reprotoxiques. L’étude évalue également la satisfaction des patients à propos de cet entretien avec le médecin du travail et leur attitude vis à vis des reprotoxiques.Le score moyen de connaissances générales sur les reprotoxiques était de 9.6 ± 2.7/16. Le facteur reprotoxique le plus fréquemment sous estimé était la chaleur excessive (34.7 % de bonnes réponses). En cas d’altération des paramètres spermatiques associée à des expositions professionnelles reprotoxiques, 63.2 % des patients se disaient prêts à utiliser des dispositifs de protection individuelle, et 55.1 % à accepter une adaptation temporaire de leur poste de travail. Concernant l’entretien avec le médecin du travail du laboratoire de spermiologie, 80.7 % le considéraient modérément ou très utile. Parmi les participants, 46.2 % rapportaient un changement dans leurs habitudes de vie 2 mois après l’entretien, et 88.5 % étaient satisfaits ou très satisfaits des soins reçus. Tous les participants trouveraient utile d’étendre cet entretien à leur compagne.Nos résultats suggèrent que les connaissances des patients sur les expositions reprotoxiques peuvent être améliorées, particulièrement en ce qui concerne les expositions physiques. La grande majorité des patients était satisfaite de l’introduction de cette nouvelle collaboration entre médecine de la reproduction et médecine du travail.
