Fusionen und Ubernahmen in der Pharmaindustrie haben zu einer Konzentration gefuhrt, von der auch die Impfstoffproduktion mit nur noch wenigen global tatigen Herstellern betroffen ist. Die zur Durchfuhrung der in Deutschland von der Standigen Impfkommission (STIKO) empfohlenen Impfungen benotigten Impfstoffe werden in der Regel nur von einem oder 2 Unternehmen vertrieben, was quasi zu Monopolstellungen gefuhrt hat. Dies behindert den Wettbewerb und resultiert haufig in Lieferengpassen. Das Paul-Ehrlich-Institut (PEI) und die STIKO sind deshalb regelmasig gezwungen, zeitnah Handlungsempfehlungen fur die Arzteschaft zu erstellen. Die Deutsche Akademie fur Kinder- und Jugendmedizin (DAKJ) fordert deshalb die politisch Verantwortlichen erneut auf, sich mit der Verhinderung von Versorgungsengpassen zu beschaftigen und in Absprache mit den betroffenen Kreisen Losungsmoglichkeiten zu erarbeiten. Die aktuelle Entwicklung einer nationalen Strategie gegen Lieferengpasse ist sehr begrusenswert und ist Anlass zu Hoffnung auf Besserung. Eine ausreichende Versorgung mit MMR-Impfstoffen ist zudem unabdingbar, wenn das Masernschutzgesetz in Deutschland glaubwurdig und wirksam umgesetzt werden soll.

Die Ubertragung von Infektionskrankheiten wird in Gemeinschaftseinrichtungen aufgrund der engen Kontakte sehr begunstigt. Die Deutsche Akademie fur Kinder- und Jugendmedizin hat bereits 2009 und 2015 gefordert, die Aufnahme von Kindern in Krippen, Kindergarten und Schulen von einem kompletten, nach STIKO(Standige Impfkommission am Robert-Koch-Institut)-Empfehlungen durchgefuhrten Impfschutz abhangig zu machen, was jedoch bisher nur fur die Masern-Impfung umgesetzt wurde. Diese Forderung soll erneuert werden. Ein ungeimpftes Kind kann beim Besuch von Gemeinschaftseinrichtungen sich selbst, aber auch andere ungeschutzte Kinder anstecken. Dabei sind insbesondere aufgrund von Kontraindikationen nichtgeimpfte, aber auch noch unvollstandig geimpfte Kinder im 1. und 2. Lebensjahr gefahrdet. Die Kommission fordert: 1) eine Weiterentwicklung des Infektionsschutzgesetzes zur Gewahrleistung einer ausreichenden Durchimpfungsrate, 2) einheitliche Regelungen der Lander bezuglich des Impfstatus zu Aufnahme und Verbleib in Gemeinschaftseinrichtungen sowie zur Aufklarung der Eltern, 3) die systematische Starkung des OGD (Offentlicher Gesundheitsdienst), damit auch dort Impfungen angeboten werden konnen, 4) die kassenarztlichen Vereinigungen auf, die Arzte dazu anzuhalten, alle empfohlenen Impfungen anzubieten, 5) die Wahrnehmung arztlicher Aufklarungspflichten zum Impfen und 5) die Wahrnehmung der elterlichen Sorgepflicht, Kinder durch Impfen vor vermeidbaren schweren Infektionskrankheiten zu schutzen, sowie 6) Aufklarung uber Hygiene- und sonstige Praventionsmasnahmen zur Verhinderung der Ubertragung von Infektionskrankheiten.

Publizierte Daten zeigen, dass mit dem aus 4 Komponenten bestehenden Impfstoff 4CMenB und dem aus 2 Komponenten bestehenden Impfstoff MenB-fHbp zwei effiziente Impfstoffe zur Prophylaxe invasiver MenB-Erkrankungen zur Verfugung stehen. Es ist von einer moglichen Protektion vor invasiver Infektion fur etwa 80 % der Falle in Deutschland auszugehen. Die Kommission halt folgendes Vorgehen fur sinnvoll: Kindern ab dem Alter von 2 Monaten mit bekanntem erhohten Risiko fur eine invasive Meningokokkeninfektion ist analog der Empfehlung der Standigen Impfkommission (STIKO) die Impfung gegen die Serogruppen A, C, W und Y und die Impfung mit Bexsero® (4CMenB) zu empfehlen. Jugendliche ab dem vollendeten 10. Lebensjahr mit erhohtem Risiko fur eine invasive MenB-Infektion konnen mit Bexsero® (4CMenB) oder Trumenba® (MenB-fHbp) immunisiert werden. Solange keine MenB-Standardimpfempfehlung durch die STIKO vorliegt, kann das strategische Ziel einer Senkung der Krankheitslast in der Allgemeinbevolkerung nicht erreicht werden. Bis dahin sollte unter Berucksichtigung der Zielsetzung eines individuellen Impfschutzes gegen MenB-Erkrankungen die Impfung fruhestmoglich, also bereits ab einem Alter von 2 Monaten, durchgefuhrt werden. Gleichzeitig sollte allen alteren Sauglingen, Kindern und Jugendlichen im Sinne einer Nachholimpfung der MenB-Impfschutz individuell angeboten werden. Zum raschen Erreichen eines Impfschutzes kann die Immunisierung in Abhangigkeit vom Lebensalter zeitgleich mit oder getrennt von anderen Impfungen erfolgen. Um das bei der gleichzeitigen Impfung gegen MenB und anderen Routineimpfungen im Alter von 2 und 4 Monaten beschriebene erhohte Risiko fur das Auftreten von Fieber innerhalb von 48 h zu reduzieren, kann Paracetamol verabreicht werden.

Immigrierende und adoptierte Kinder und Jugendliche weisen oft eine unvollstandige Krankheits‑, Sozial- und Impfanamnese auf. Bei ihnen konnen weitere anamnese- bzw. risikoassoziierte Untersuchungen auf Infektionskrankheiten, nichtubertragbare und chronische Vorerkrankungen sowie angeborene Krankheiten sinnvoll sein. In dieser Stellungnahme wird ein Leitfaden fur Routineuntersuchungen bei oftmals fehlenden Krankheitszeichen oder Symptomen vorgestellt, deren Umfang auf der Basis verfugbarer anamnestischer Angaben, des Herkunftslands bzw. der Herkunftsregion und der Befunde der korperlichen Untersuchung individuell festgelegt werden soll. Ebenso ist die Planung der Nachholimpfungen und Vorsorgeuntersuchungen wichtiger Bestandteil der kinderarztlichen Versorgung. Im Fall von Sprachbarrieren sollte ein professioneller Dolmetscher hinzugezogen werden.

Persistierende Schmerzen trotz adaquater Entzundungskontrolle stellen bei vielen rheumatologischen Erkrankungen eine grose Herausforderung fur Patient und Arzt dar. Der Fokus der Medikamentenentwicklung lag in den letzten Jahren auf der antientzundlichen Therapie. Hier sind enorme Fortschritte gemacht und viele Therapieoptionen geschaffen worden. Man beobachtet, dass entzundungsbedingte Schmerzen zwar gut uber antientzundliche Therapien gelindert werden konnen, die chronische Schmerzkomponente bleibt aber ein Problem. Schmerzen werden durch Entzundungsmediatoren beeinflusst. Durch Wirkung am peripheren Nozizeptor kommt es zur peripheren Sensibilisierung. Dauert die Entzundungsreaktion an, kann es auch zentral auf Ruckenmarksebene oder im Gehirn durch einwandernde Immunzellen und/oder lokale Aktivierung von z. B. Mikroglia zur zentralen Sensibilisierung und Chronifizierung kommen. Hierdurch kommt es zu einer zunehmenden Verselbststandigung und Abkopplung der Schmerzen vom eigentlichen Entzundungsprozess. Die vorliegende Ubersicht beschaftigt sich mit der Frage ob b(„biologic“)DMARDs („disease-modifying antirheumatic drugs“) oder ts(„targeted synthetic“)DMARDs neben der Entzundungshemmung auch gunstig in diese den Schmerz betreffenden Vorgange eingreifen konnen. Es gibt praklinische Daten, die einen Einfluss auf Sensibilisierungsvorgange bei Einsatz von Zytokininhibitoren nahelegen. Andererseits zeigen klinische Daten bisher nur, dass sowohl bDMARDs als auch tsDMARDs den nozizeptiven Entzundungsschmerz uber die Krankheitsentitaten hinweg zuverlassig und schnell reduzieren. Eine Wirkung auf die Vorgange der zentralen Sensibilisierung und damit auf die Chronifizierung von Schmerzen kann anhand der vorliegenden klinischen Daten nicht abschliesend beurteilt werden.

In Deutschland werden ca. 30 % aller Kinder per Kaiserschnitt entbunden. Der Kaiserschnitt zahlt damit zu den haufigsten operativen Prozeduren in Deutschland und weltweit, mit einem hohen Risiko fur postpartale infektiose Komplikationen. Deshalb ist unumstritten eine perioperative Antibiotikaprophylaxe indiziert, die nach Abklemmen der Nabelschnur oder 30 min vor Hautschnitt gegeben werden kann. Letzteres wird in den aktuellen Leitlinien empfohlen, kann aber zur Folge haben, dass jahrlich uber 200.000 Kinder schon bei ihrer Entbindung Antibiotika exponiert werden. Diese Stellungnahme befasst sich mit der Frage, ob die aktuelle Datenlage ausreicht, um diese Empfehlung aufrechtzuerhalten.

ZusammenfassungDie Lyme-Borreliose, übertragen durch die Zecke Ixodes ricinus und hervorgerufen durch die Spirochäte Borrelia burgdorferi, tritt in fast 90 % der Fälle mit Hauterscheinungen auf, dem Erythema migrans, manchmal auch multilokulär, dem Borrelien-Lymphozytom und der seltenen Acrodermatitis chronica atrophicans. Das typische Erythema migrans bedarf keiner Laborbestätigung, sondern wird sofort antibiotisch behandelt: bis 8 Jahre mit Amoxycillin, danach mit Doxycyclin. Die anderen Manifestationen bedürfen der serologischen Bestätigung. Die beste Prophylaxe ist die zügige Entfernung anheftender Zecken.

Background Pharmachild is a pharmacovigilance registry on children with JIA treated mainly with biologics±methotrexate (MTX). Little evidence exists in literature about the role of JIA or its immunosuppressive therapy in determining infections, especially caused by opportunistic pathogens. Objectives To provide an update on opportunistic infections (OI) revised by an independent Safety Adjudication Committee (SAC) (3 paediatric rheumatologists and 2 paediatric infectious disease specialists). Methods The participating centres were asked to report all infections encountered by their JIA patients. PRINTO and the medical monitor (MM) classified events based on MedDRA dictionary. Moderate/serious/severe/very severe infections were then revised blindly by the SAC, who was asked to answer 6 questions. The events with consensus of at least 3/5 experts on the first 3 questions (‘Is this an infection?’, ‘Is it common?’, ’Is it opportunistic?’) were retained for the analysis. With referral to the recommendations by Withrop et al.1, for the first time a list of OIs in children with JIA on immunosuppressive therapy was elaborated and approved by consensus, through three Delphi steps. Finally, we compared the OI list defined by the SAC to the list of OI approved by the specialists. Results A total of 772 safety events related to 634 patients were submitted to the SAC. 689 (89.2%) events received consensus among the experts on the 3 questions and, of these, 682 (99.0%) were considered as infections, corresponding to 153 different Preferred Terms (PT), according to MedDRA dictionary. Among the 682 infections, 603 (88.4%) were defined by the experts as common and 119 (17.4%) as opportunistic. For 92 (60%) of the 153 PT, the MM and SAC used the same PT, while the remaining 40% was adjudicated by a third examiner, who analysed again the case reports and assigned the PT which was the most appropriate taking into account the experts’ opinion. Among the final number of infections emerged that herpes viral, respiratory and EBV infections were the most frequent (Table). Analysing the infections by PT (n=149), the experts adjudicated: 22 as OI, 119 as not OI, 8 discordant. Comparing the experts’ adjudication with the approved list of OI by PT, there was full agreement for the 22 PT classified as OI, while 19/117 (16.2%) PT resulted in the list, but were not classified as OI by the experts. Conclusions Our analysis showed a significant number of OI in JIA patients on immunosuppressants and provided an approved document stating the most frequent OI in children with JIA. The most frequent opportunistic pathogens resulted herpes virus, excluding varicella, and mycobacteria, but the list of definite and probable OIs needs to be validated/revised with the analysis of future datasets. Chronic lymphoplasmacellular osteomyelitis may occur in children, adolescents and adults, but has not been found in newborns or babies either in our series or in the literature. Symptoms suggesting an acute disease like fever are uncommon, but a primary chronic course with symptomatic and asymptomatic periods is typical. Pain and swelling are the main symptoms; painless masses are rare. In children and adolescents the clavicle and metaphyseal regions of long bones are typical sites of chronic abacterial osteomyelitis. In adults the clavicles or the first two ribs are mainly affected with synovitis of the adjacent joints, but the long bones are rarely involved. Laboratory findings are non-specific but important for the differential diagnosis. The sedimentation rate and c-reactive protein might be elevated. The X-ray examination shows osteolytic, sclerotic or mixed bony changes and, in case of a diaphyseal involvement, onionskinlike periosteal bone formation may be present, suggesting a malignant process. In late stages sclerotic bone formations may be seen as a rest. Uni- or multifocal lesions can be detected by bonescan, as can asymptomatic lesions. Magnetic resonance imaging shows gross signal intensity differences both in the bone and perifocal soft tissue and involvement of the synovium with gadolinium DPTA enhancement in T1-weighted images. In early stages of the disease granulocytes, microabscesses and new bone formations might suggest bacterial osteomyelitis that cannot be differentiated by histology. In intermediate phases lymphocytic and plasma-cellular infiltrates are found, whereas in late phases sclerotic bone formations and fibrosis of the bone marrow are seen histologically. In chronic lymphoplasmacellular osteomyelitis, all clinical, radiological and histological findings, as well as negative bacteriological cultures, are mandatory and will allow a definitive diagnosis to be made. The disease may be uni- or multifocal, and new bone lesions may occur over time, as well as skin manifestations, which can be found years before or after bone involvement. The association with dermatological diseases and/or synovitis led to the acronym SAPHO syndrome (synovitis, acne, pustulosis, hyperostosis, osteomyelitis). For the treatment nonsteroidal anti-inflammatory drugs are effective for pain relief, reduction of swelling and dysfunction. Antibiotics have been used in several series and are not effective. Major surgery is not recommended even for recurrences, and the prognosis for growth and function is excellent in the long term despite recurrences over several years.

Der Verdacht auf Fieber erfordert eine akkurate Messung und die Suche nach der Ursache. Wenn sich mit Anamnese und korperlichem Untersuchungsbefund einschlieslich der obligaten Urinuntersuchung keine Ursache des Fiebers findet, spricht man von Fieber ohne Fokus. Fieber ohne Fokus liegt meist eine selbstbegrenzte Virusinfektion zugrunde, jedoch kann die Ursache auch eine okkulte Bakteriamie sein, die eine antibiotische Therapie erfordert. Aufgrund der breiten Anwendung von Konjugatimpfstoffen gegen die wichtigsten bakteriellen Erreger des Kindesalters ist die okkulte Bakteriamie heute allerdings selten, sodass die Empfehlungen zum Umgang mit Fieber ohne Fokus uberarbeitet werden mussen. Eine ausgedehnte Abklarung wie bei Verdacht auf Sepsis und eine probatorische antibiotische Therapie sind seltener notwendig als fruher, besonders bei Kindern nach Erreichen des 4. Lebensmonats. Aber auch jungere Sauglinge konnen unter bestimmten Voraussetzungen zunachst nur stationar uberwacht werden. Diese erleichterten Empfehlungen gelten nicht, wenn die Kinder fruhgeboren waren oder abwehrgeschwacht sind, bereits antibiotisch behandelt wurden oder auffallige Laborwerte zeigen. Hingegen kann man bei gutem Allgemeinzustand und einem Alter ab 3 Monaten auf spezifische Laboruntersuchungen auser dem initialen Urinstatus verzichten.

Durch pharmakolegale Masnahmen sind die ersten neuen Biologika auch bei Kindern und Jugendlichen mit rheumatischen Erkrankungen, besonders der juvenilen idiopathischen Arthritis (JIA), in kontrollierten Studien getestet und in die Therapie eingefuhrt worden. Dies war moglich in internationalen Forschungsverbunden und nach Einfuhrung von Kriterien fur gut definierte Krankheitsbilder, Krankheitsaktivitat, Besserung und Verschlechterung. Etanercept, Adalimumab, Abatacept, Tocilizumab und Canakinumab haben begrenzte Zulassungen erhalten. Etanercept ist das am langsten zugelassene Biologikum und wie Adalimumab Mittel der Wahl, wenn Methotrexat bei der Polyarthritis nicht ausreichend wirkt oder nicht vertragen wird. Tocilizumab kann bei der systemischen JIA angewandt werden, wenn Glukokortikoide nicht ausreichen oder nicht vertragen werden. Diese und andere Biologika wie Anakinra und Rituximab werden auch auserhalb der Zulassung mit Erfolg eingesetzt. Es fehlen allerdings Langzeitstudien. Diese Medikamente sollten nur von Kinderrheumatologen angesetzt werden.

Important functions of the spleen include the sequestration of senescent and deformed erythrocytes as well as the detection of antigens, regulation of the immune response, and the defence against infections. A palpable, enlarged spleen is almost always pathological and a diagnostic work-up is imperative. Splenomegaly may be caused by a wide range of conditions including common causes such as portal hypertension due to liver disease, haematological diseases such as lymphoma, myeloproliferative diseases, haemolytic anaemia and infections (e. g., infectious mononucleosis or malaria). Enlargement of the spleen is an important cardinal symptom in Felty's syndrome and Still's disease. In patients with haemochromatosis or sarcoidosis, the concomitant splenomegaly is frequently only mild and in connective tissue diseases enlargement of the spleen is rare. Important differenzial diagnoses of splenomegaly in children and adolescents include infectious diseases, anaemia, leukaemia, and lymphomas. Storage diseases, which usually occur in conjunction with pronounced splenomegaly, may be accompanied by neuropathological symptoms, bone involvement, dysmorphias, and enlargement of inner organs such as heart and liver. This group of diseases should be diagnosed during childhood or adolescence, as early diagnosis has become more important due the introduction of new therapeutic options. This is especially relevant for Gaucher's disease, the most common lysosomal storage disease. Other storage diseases that are accompanied by hepatosplenomegaly include Niemann-Pick type C disease, sphingomyelinase deficiency, aspartylglucosaminuria, sialic acid storage diseases, and cholesteryl ester storage disease.

Borrelia burgdorferi is the causative agent of Lyme borreliosis (LB). Clinical manifestations of LB include chronic inflammatory disorders of skin, central nervous system, and joints. Although LB is infectious and B. burgdorferi is highly susceptible to antibiotic treatment, about 10% of cases of Lyme arthritis are refractory. Possible pathogenetic mechanisms might be persistent antigenic stimulation or autoimmunity or a combination. Thus persistent inflammation due to LB might be a useful model for chronic inflammatory joint diseases including rheumatoid arthritis or juvenile idiopathic arthritis.

In Europe, Lyme borreliosis is the most common disease communicated by ticks and especially affects the skin, nervous system, joints, and heart. It is caused by at least four species of the spirochete Borrelia burgdorferi. The various pathologies are classified as early forms (erythema migrans, borrelia lymphocytom, early neuroborreliosis, carditis) or late forms (arthritis, acrodermatitis chronica atrophicans, chronic neuroborreliosis). The accuracy of serodiagnosis is 20-50% with erythema migrans, 70-90% with early neuroborreliosis, and nearly 100% with Lyme arthritis. Following special indications, the agent is confirmed by skin biopsy or spinal or joint puncture. Oral therapy is performed with amoxicillin, doxycycline, and cefuroxime, and intravenous therapy uses ceftriaxone, cefotaxime, or penicillin G. All in all, the prognosis of treated Lyme borreliosis is good. In childhood permanent defects are extremely rare, even following long-term manifestation at an early age.

The objective of this study is to evaluate complications and changes in health status (disease activity and flare) in response to the AS03-adjuvanted H1N1 vaccine in children with rheumatic diseases. We conducted a nationwide survey addressing paediatric rheumatology sites who participated in the national paediatric rheumatology database. Ninety patients were documented-38 % under treatment with biologicals-of whom 18 % suffered from complications (10 % local and 8 % systemic) with no relevant changes in median disease activity or flare rate during 4 weeks following the vaccination. The adjuvanted H1N1 influenza vaccine seems to be adequately tolerated in children with rheumatic diseases.

Cardiac involvement is common in adult patients with the presence of HLA B27 with or without the HLA B27-associated spondyloarthropathy ankylosing spondylitis. Most patients with juvenile spondyloarthropathy, which begins at age 16 or younger, do not have spinal involvement and there are only few reports of cardiac involvement. This study sought to assess the prevalence of carditis in patients with HLA B27-associated juvenile arthritis (B27-JA). In a controlled study, 40 patients with B27-JA, among them only 1 with ankylosing spondylitis, were examined by electrocardiogram and echocardiography with pulsed and color-flow Doppler at rest and at the termination of a bicycle exercise and compared to an age- and sex-matched control group negative for HLA B27. Four patients with B27-JA, and none in the control group, had inflammatory aortic regurgitation. Late diastolic flow velocity was significantly increased in patients with B27-JA at the termination of exercise. HLA B27 is a risk factor for endo-/myocardial damage in patients with B27-JA, even in the presence of only short and mild articular disease. Patients with B27-JA should be screened for the presence of aortic regurgitation.

To assess the incidence of Lyme borreliosis in Central Europe, a 12-month, prospective, population-based surveillance study of Lyme borreliosis was conducted in the Wurzburg region of central Germany, following an aggressive awareness campaign. The diagnosis of Lyme borreliosis required the presence of (i) erythema migrans (diameteror =5 cm); (ii) lymphocytoma; or (iii) another specific manifestation including Lyme arthritis, neuroborreliosis, carditis or acrodermatitis chronica atrophicans in conjunction with serological confirmation. A total of 313 cases of Lyme borreliosis was diagnosed, giving an incidence of 111 cases/100000 inhabitants, the highest rates occurring in children and elderly adults living in wooded as opposed to agricultural areas. The incidence in city dwellers and inhabitants of rural areas was not significantly different. Erythema migrans was the only manifestation in 279 (89%) patients. Of the 34 patients with manifestations other than erythema migrans alone, 15 had arthritis, nine neuroborreliosis, six lymphocytoma, four acrodermatitis chronica atrophicans and one carditis. Children were more likely than adults to have manifestations other than erythema migrans alone. Lyme borreliosis was very common in central Germany, and one of the most frequent bacterial infections. The observation of more cases of arthritis than neuroborreliosis was similar to that in the USA. These results may be representative for many parts of central Europe and suggest the need for development of a vaccine against borreliosis caused by European strains of Borrelia species.

Hintergrund: Die Lyme-Borreliose ist die haufigste durch infizierte Zecken ubertragene Infektionskrankheit des Menschen in Europa. Erscheinungsformen: Verschiedene klinische Erscheinungsformen konnen auftreten. Die Krankheitsbilder Erythema migrans, Meningitis, Hirnnervenlahmung und Arthritis dominieren im Kindesalter. Das Erythema migrans ist gewohnlich leicht diagnostizierbar und eine Antikorperbestimmung ist nicht indiziert. Die Neuroborreliose manifestiert sich meist als eine serose Meningitis mit oder ohne Hirnnervenparese, hauptsachlich periphere Fazialisparese. Der Liquorbefund bietet haufig das gleichzeitige Vorliegen von lymphozytarer Pleozytose, IgM-Dominanz in der intrathekalen Immunantwort und eine Schrankenstorung. Serodiagnostisch ist die Bestimmung des spezifischen Antikorperindex nach Reiber der beste Parameter zum Nachweis einer intrathekalen, erregerspezifischen Antikorperbildung. Die Lyme-Arthritis imponiert anfangs als episodische Oligoarthritis, meist unter Einschlus des Kniegelenks, und kann in eine chronische Monarthritis des Knies ubergehen. Fast immer werden hohe IgG-Antikorper-Titer gefunden. Die seltenen Manifestationen Enzephalomyelitis, chronische Arthritis, Karditis und entzundliche Augenerkrankungen sind klinisch vieldeutig, bieten selten eindeutige Befunde und sind in der Interpretation der serologischen Ergebnisse problematisch. Die Spezifitat der durch ELISA oder IFT ermittelten Antikorper mus im Immunoblot bewiesen werden. Diskussion: Therapeutisch sind Amoxicillin oder Doxicyclin beim Erythma migrans und Zephalosporine der 3. Generation parenteral bei den ubrigen Manifestationen als Mittel der Wahl anzusehen. Trotz klinisch erfolgreicher antibiotischer Therapie konnen sowohl die IgM- als auch die IgG-Antikorpertiter uber Monate und Jahre persistieren, ohne das sich daraus eine neue Behandlungsindikation ergibt, wenn nicht gleichzeitig noch eine weitere klinische Symptomatik besteht. Die Unterscheidung zwischen einer persistierenden Infektion und einer Seronarbe kann nur unter Berucksichtigung der klinischen Symptomatik, unspezifischen Laborparametern und Antikorpertitern im Verlauf getroffen werden.

Schmerzen am Bewegungsapparat gehoren zu den haufigsten Vorstellungsgrunden beim Kinderarzt. Da pathognomonische Befunde meist fehlen und Laboruntersuchungen und apparative Diagnostik oft nur zum Ausschlus anderer Erkrankungen beitragen, kommt der korrekten Anamneseerhebung und Beschreibung des physikalischen Befunds besondere Bedeutung zu. Es lassen sich entzundliche und nicht entzundliche Ursachen von Gelenkbeschwerden unterscheiden. Das klassische entzundliche Symptom ist die Arthritis. Wahrend bei den echten Arthritiserkrankungen wie der Coxitis fugax oder dem „kindlichen Rheuma” (juvenile rheumatoide Arthritis) die Arthritis im Zentrum des Interesses steht, gibt es viele Erkrankungen, die mit einer Arthritis einhergehen konnen: Dabei kann die Arthritis, wie z.B. bei der Purpura Schonlein-Henoch oder dem systemischen Lupus erythematodes, klinisch im Vordergrund stehen und so die Grundkrankheit moglicherweise schwer erkennbar sein. Nicht entzundliche Ursachen von Schmerzen am Bewegungsapparat sind wesentlich haufiger, wobei Traumen uberwiegen. Praktisch wichtig ist die Erkennung von sog. „Wachstumsschmerzen”. Obwohl viele Ursachen von Schmerzen am Bewegungsapparat im Kindes- und Jugendalter harmlos sind, mussen immer Erkrankungen ausgeschlossen werden, die eine sofortige Intervention erfordern. Dazu gehoren die septische Arthritis oder Osteomyelitis und Malignome wie die akute lymphatische Leukamie oder solide Tumoren. Die erfolgreiche Betreuung von Kindern mit chronischen Erkrankungen des Bewegungsapparats erfordert viel Erfahrung und bei nicht entzundlichen Erkrankungen oft die interdisziplinare Zusammenarbeit mit einem mit Kindern erfahrenen Orthopaden und bei entzundliche Erkrankungen mit einem mit diesen Erkrankungen speziell erfahrenen Kinderarzt.

Multiple kartilaginare Exostosen sind eine seltene autosomal-dominant vererbte Erkrankung. Wir berichten uber eine Familie, in der der Vater des Vaters, der Vater und alle 3 Kinder betroffen sind. Bei dem 7 7/12 Jahre alten ersten Kind waren im 12. Lebensmonat erste Exostosen an beiden Ellbogen und Handgelenken, spater auch an den Fingern, den Unterschenkeln und den Fusen aufgefallen. Das Exostosenwachstum an der Ulna fuhrte zu einer Ulnaverkurzung mit angularer Fehlstellung des rechten Ellbogens und Luxation des Radiuskopfchens sowie einer Ulnardeviation im Handgelenk. Die Funktion des rechten Arms war deutlich eingeschrankt. Zur Korrektur der Unterarmdeformitat wurde im 5. Lebensjahr eine Osteotomie der Ulna, gefolgt von einer Kallusdistraktion mittels Fixateur externe fur die Dauer von 8 Monaten durchgefuhrt. Dadurch konnten das Radiuskopfchen reponiert und die Funktion von Hand und Unterarm wieder hergestellt werden. Sechs Monate spater wurde die Exostose der rechten distalen Ulna entfernt. Zwei Jahre nach der ersten Operation ist die Funktion der rechten Hand und des Ellbogens regelrecht. Das kosmetische Ergebnis ist gut.

A 13-year-old male adolescent suddenly developed unilateral cervical lymphadenopathy which resulted in the excision of the enlarged lymph nodes. After the intervention his general state deteriorated and the patient was transferred to our hospital. The patient presented with high fever, bilateral non-purulent conjunctivitis, strawberry tongue and fissured lips, a maculopapular rash and severe polyarthritis including all peripheral joints. We treated him for Kawasaki disease with single high-dose intravenous gammaglobulin. Within 16 hours he became afebrile and all other symptoms disappeared rapidly. A few days later, he developed thrombocytosis and desquamation of the tips of fingers and toes. The immunohistological examination of the lymph nodes demonstrated interfollicular hyperplasia of the T-zones and in the peripheral blood an expansion of the Vβ2-positive T-cell subset was found. These findings support recent aspects concerning the pathogenesis of Kawasaki disease. Discussion: Although Kawasaki disease is mainly observed in preschoolchildren, older children and adolescents may be affected. Extreme forms of cervical lymphadenopathy may occur and in the case of a strictly unilateral lymphadenopathy the diagnosis may be difficult.

Non-steroidal antiinflammatory drugs (NSAIDs) are routinely used in the therapy of chronic inflammatory joint diseases in childhood. Recently the NSAID naproxen was recognized to induce pseudoporphyria. This rare photodermatitis is characterized by skin fragility and vesiculation, resulting in shallow scarring. We report 4 children with juvenile rheumatoid arthritis who developed naproxen-induced pseudoporphyria. All children had received naproxen for more than 5 months when pseudoporphyria occurred. A disorder of porphyrin metabolism was excluded by analysis of the urine, serum and erythrocytes. Previous reports on naproxen-induced pseudoporphyria described a rapid disappearance of blisters after discontinuation of treatment. However, in our patients, new lesions appeared for up to 5 weeks after discontinuation of the therapy and skin fragility was apparent for up to 6 months after cessation of treatment. Since naproxen is a widely used drug in the treatment of children with juvenile rheumatoid arthritis parents of fair-skinned children should be alerted to the possibility of this rare adverse effect.

Diagnosis of neuroborreliosis is often difficult since history and clinical presentation may be non-specific and serological tests may initially be negative. We therefore tested the polymerase chain reaction (PCR) for the detection of borrelial sequences in CSF and urine samples of consecutive children with neuroborreliosis seen in a single summer season. Four of eight children were negative in serum for antibodies to Borrelia burgdorferi. Two of eight children were PCR-positive in CSF and one other child was positive in urine. In two out of four children PCR was the only laboratory test confirming the clinical diagnosis. All children recovered after treatment with third generation cephalosporins. When seven of eight children were re-examined 6 months later all were healthy and antibodies to B. burgdorferi were detected in their serum. PCR may assist the paediatrician in establishing a diagnosis of neuroborreliosis; however, a negative result does not rule out neuroborreliosis. PCR is an adjunct, but no substitute for clinical judgement and serology.

In developed countries, acute gastroenteritis (AGE) is a major source of morbidity. However, only a few studies have estimated its incidence and the associated medical burden. This population-based study determined the incidence of community-acquired AGE patients seeking medical care and the relative role of various pathogens. Stool samples from patients with AGE presenting to a general practitioner (GP), pediatrician, or specialist in internal medicine for that reason were screened for various bacterial and viral enteropathogens. A control group was established as well. Incidences were calculated by the number of positive patients divided by the general population. The study was performed in north-west Germany in 2004. We report on a 30-month-old previously healthy Turkish boy who presented with fever, hepatosplenomegaly and generalized lymphadenopathy. He died 4 months after admission in spite of treatment with steroids, acycloguanosine and cyclophosphamide. Epstein-Barr virus (EBV) DNA was detected in the patient's bone marrow and in a lymph node biopsy. Cells from the lymph node biopsy showed monoclonal rearrangements of immunoglobulin heavy chain genes but no rearrangements of T-cell receptor beta-chain genes or immunoglobulin kappa chain genes. Serological data indicated chronic active EBV infection. There was a slight increase of CD8 positive cells in peripheral blood and a normal response to T-cell mitogens. However, T-cell lines established with interleukin 2 from lymph node biopsy completely failed to kill autologous EBV-transformed B-cells and K 562 target cells. Moreover, in regression tests the patient's peripheral blood mononuclear cells completely failed to limit outgrowth of autologous EBV infected B-cells. We conclude that the patient's selective immuno-deficiency had led to the rapid development of EBV-associated monoclonal lymphoproliferation.

Lyme borreliosis is a multisystem disorder common in childhood. It is an acute and persistent anthropozoonotic infection caused by the spirochete Borrelia burgdorferi (Bb) which is transmitted by Ixodes ticks. After the tick bite in summer, erythema migrans, meningoradiculoneuritis, or carditis may develop within the same season. Later manifestations may be oligo-arthritis, progressive encephalomyelitis, or acrodermatitis chronica atrophicans. The most common course is probably asymptomatic. Connatal infection is possible. Diagnosis is established mainly by history and clinical manifestations. The antibody response to Bb can be measured in serum and cerebrospinal fluid. Tests may be false-negative early in the course of the disease or after early treatment. False-positive results may be caused by cross-reactions. Interpretation of test results must also consider unrelated anamnestic titres or asymptomatic infection. Treatment with appropriate antibiotics cures the disease in most patients, however some patients may not respond. The optimal drug has not yet been found. Best prophylaxis is by early removal of the tick from the skin.

Es werden 2 Kinder vorgestellt (7jahriges Madchen und 11jahriger Junge) mit einer seltenen Form der Neuroborreliose. Betroffen waren N. opticus und N. abducens. Klinisch fanden sich eine beidseitige Papillenprominenz sowie ein Strabismus convergens bedingt durch eine Parese des VI. Hirnnerven. Zusatzlich bestanden Kopfschmerzen, so das primar eine intrakraniale Raumforderung mittels Kernspintomographie ausgeschlossen wurde. Die weitere Diagnostik ergab eine mononukleare Liquorpleozytose, und es liesen sich in Serum und Liquor spezifische Antikorper gegen Borrelia burgdorferi nachweisen. Somit ergab sich die Diagnose einer Neuroborreliose mit Neuritis und Papillitis nervi optici und Neuritis nervi abducentis. Wenige Tage bis Wochen nach einer i.v.-Therapie mit Ceftriaxon war der klinische Befund unauffallig. Diskussion: Bei ungeklarten Hirnnervenparesen und Papillenprominenz mus nach Ausschlus einer intrakranialen Raumforderung an eine Neuroborreliose gedacht werden. Davon zu trennen ist ein isolierter Befall des Auges im Sinn einer okularen spaten Lyme-Borreliose.

Die juvenile Dermatomyositis (JDM) ist eine entzundliche Multisystemerkrankung unklarer Atiologie mit besonderer Beteiligung der Haut und der quergestreiften Muskulatur. Nach einem Prodromalstadium tritt eine progressive proximale Muskelschwache auf. An der Haut sind die klassischen Zeichen der heliotrope Ausschlag, Wangenerythem, Gottron-Zeichen und Nagelbettgefasveranderungen. Zur Diagnose werden modifizierte Kriterien nach Bohan und Peter verwendet, die neben der klinischen Haut- und Muskelbeteiligung erhohte Muskelenzymwerte und typische Befunde in Elektromyographie, Muskelbiopsie und neuerdings auch Magnetresonanztomographie (MRT) einbeziehen. Steroide werden klassisch hochdosiert oral gegeben. Eine intravenose Pulstherapie mit niedriger dosierter oraler Intervalltherapie und weitere immunsuppressive Medikamente wie Methotrexat konnen Steroidnebenwirkungen vermindern. Die Prognose der JDM hat sich verbessert, und die meisten Patienten erholen sich, aber einige Patienten versterben und eine Minderheit hat Defektheilungen mit persistierender Muskelatrophie oder ausgepragter Kalzinose.

Following the clinical observation of a peculiar joint pattern in children with Lyme arthritis, we classified the pattern of joint involvement in children with oligoarticular arthritis (2-4 joints) of different causes, including Lyme arthritis, juvenile chronic arthritis and juvenile spondyloarthropathy, as: (1) symmetrical: arthritis of the same joints on both sides of the body; (2) unilateral: all involved joints on the same side; (3) predominantly unilateral: joints involved on the contralateral side also showed ipsilateral involvement; (4) oblique: involvement on the contralateral side of one or more joints that did not show ipsilateral involvement. The oblique pattern was found in 13 of 42 children with early-onset pauciarticular juvenile chronic arthritis and in 17 of 32 patients with juvenile spondyloarthropathy, but not in 32 of 32 children with Lyme arthritis. If confirmed, these results may have implications for the diagnosis and pathogenesis of Lyme arthritis.

Demonstration of myositis in juvenile dermatomyositis (JDM) has traditionally required the presence of two out of three objective findings including muscle enzyme studies, electromyography, and muscle biopsy. Magnetic resonance imaging (MRI) is considered another objective criterion and has been recommended for follow-up studies as well. We report on a 6-year-old girl with JDM, presenting with facial rash and proximal muscle weakness, in whom MRI, in conjunction with elevated creatinine kinase (CK) levels, was diagnostic, with an increased T2 signal of proximal muscles. MRI reflected exacerbation of myositis in spite of steroid therapy 2 weeks later, but failed to mirror normalization of muscle strength and CK values 10 weeks later. In fact, improvement of MRI followed clinical and laboratory normalization with a delay of 2 months. This possible delay should be considered when MRI is used to monitor the response to therapy.

Diagnosis of Lyme arthritis (LA) in children and adolescents may be difficult due to non-specific clinical manifestations and unreliable serological tests for antibodies to Borrelia burgdorferi. In a national prospective study, 186 children with arthritis were examined in whom the attending physicians had considered the diagnosis of LA. Ultimately, LA was confirmed in 87 patients and these were compared with the remaining 99 children in whom arthritis was attributable to other causes. In comparison to patients with other causes of arthritis, patients with LA had a higher frequency of episodic arthritis and initial knee joint arthritis, reported tick bites more frequently, were older, had a lower frequency of initial arthralgias, and there were fewer large joints involved. A score was developed in a group of these patients and tested in a second group. It enabled patients with LA to be distinguished from those with other causes of arthritis: within a range from 12 to −7 points, a score of 2.5 or less excluded LA whereas 6 or more points were highly indicative of LA. If only those children with a score result between 2.5 and 6 had been tested for antibodies to B. burgdorferi, the number of tests would have been reduced by 63%. Conclusion Careful analysis of clinical presentation and use of a clinical score may help in distinguishing LA from other causes of arthritis and thus reduce unnecessary and expensive testing and uninterpretable test results.

Summary Background Enteroaggregative Escherichia coli (EAggEC or EAEC) can spread and cause disease in developing countries, but it is not presently known whether it spreads disease in industrialised countries. Therefore, we did a prospective study to assess the incidence and the clinical manifestations of infections due to EAEC in children in Germany. Methods 798 children with diarrhoea, admitted to hospital within a defined geographical area during a 24-month period, were included in the trial. EAEC were cultured from stool specimens, screened by PCR, and identified by colony hybridisation from DNA sequences found on the virulence plasmid. The findings were confirmed by aggregative adherence to HEp-2 cells. Stool samples from 580 children admitted to hospital without diarrhoea were also studied as controls. Findings EAEC were found in the stools of 16 (2%) of 798 children with diarrhoea, but in none of 580 children without diarrhoea. Only four of the EAEC-infected children had travelled to developing countries. Most EAEC infections were acquired in the summer months. Infection with EAEC was associated with acute, watery diarrhoea in 12 children, and with chronic diarrhoea of up to 5 months' duration in four. Five children had abdominal colic that lasted for 2–4 weeks as their main symptom. The incidence of EAEC infection was 7·7 patients admitted to hospital per 100 000 children in the general population aged younger than 16 years. Interpretation EAEC infection is associated with acute, watery diarrhoea and may be acquired in industrialised countries. Chronic diarrhoea or abdominal colic of unknown aetiology in young children may also be caused by EAEC infection.

The objective of this study was to assess the validity of the lymphocyte proliferation assay in the diagnosis of Lyme arthritis (LA). We analyzed peripheral blood mononuclear cells of 11 patients with LA and 5 healthy controls for proliferative responses to live Borrelia burgdorferi by 3H-thymidine uptake. Before and after proliferation, the total number of cells was estimated and the relative amount of lymphocyte subsets was determined by FACS Lymphoproliferative responses to B. burgdorferi in patients with LA were predominantly due to CD4+ cells and in a small part due to CD8+ cells and were antigen-dependent in comparison to healthy donors. Considering the number of cells before proliferation, responses of TCR gamma/delta + cells and CD56+ cells were higher than responses to TCR alpha/beta+ and CD3+ cells in LA patients, but not in healthy donors. There were no demonstrable antigen-dependent CD19+ cell responses. Our results support the validity of lymphocyte proliferation assay in the diagnosis of LA.

We report on three children with pauciarticular arthritis in whom the clinical picture and serology were compatible with both arthritis reactive to infection with Yersinia or Salmonella and with Lyme arthritis. Results of analysis of synovial fluid by polymerase chain reaction for enterobacterial or borrelial sequences were negative. Immunofluorescence with specific antibodies revealed the presence of amorphous enterobacterial antigens in synovial fluid cells. Since this staining did not reveal enterobacterial morphology, we infected synovial fluid cells of two children with juvenile rheumatoid arthritis in vitro with Yersinia or Salmonella. After 24 h typical rods were observed, but after about 1 week amorphous antigen similar to what had been found in the three patients was seen. In cases of reactive arthritis with ambiguous results of serological testing the diagnosis may be confirmed by demonstration of enterobacterial antigens in synovial fluid.

In a boy with neurofibromatosis type 1 (NF-1), hypertension, septic infection of an aneurysm in the deltoid muscle, bowel infarction, multiple arterial aneurysms and venous thrombosis occurred within a period of 6 weeks. Histologically, vascular neurofibromatosis of the small vessels of the gut was found. This unusual occurrence of a multitude of clinical features within a few weeks was caused by vascular neurofibromatosis. Awareness of this condition in patients with NF-1 may help the paediatrician to avoid unnecessary diagnostic procedures and to initiate appropriate symptomatic therapy.

It has been hypothesized that a persistent intraarticular viral infection might play an important part in the pathogenesis of chronic arthritis. However, it remains unclear how such an infection could survive in synovial cells that express large amounts of HLA-DR and inter-cellular adhesion molecule-1 (ICAM-1) by which they communicate with immunocompetent cells. In an in vitro model of persistent mumps virus infection of synovial cells, results suggested that, in contrast to mock-infected cells, cells containing viral antigen did not express HLA-DR in response to interferon-gamma and that they did not up-regulate ICAM-1 expression under these conditions. Previously it has been shown that infected synovial cells do not express viral surface antigens. By these mechanisms, infected cells, interspersed among a large majority of uninfected cells, might evade recognition and eradication by the the immune system. Lack of neoantigen expression on infected cells might be an important viral strategy to maintain a persistent infection and to initiate and perpetuate joint inflammation.

We report the case of a 12-year-old boy suffering from severe low-titer cold-hemagglutinin disease with excruciating colicky abdominal pain, jaundice, and acute hemolytic anemia requiring transfusion. Cold hemagglutinins of the IgM type and a positive direct antiglobulin test, predominantly against C3d, were found. Steroid pulse therapy with 20 mg/kg body wt. methylprednisolone for 3 consecutive days was given. Abdominal pain disappeared within 12 h of the first steroid infusion and hemolysis was halted. We conclude that a therapeutic trial with steroid pulse therapy in severe low-titer cold-hemagglutinin disease is warranted.

Borrelia burgdorferi (B.b.) was isolated from cerebrospinal fluid from two children with aseptic meningitis. Serology for B.b. was initially negative or borderline and became positive during the subsequent course. Children with aseptic meningitis of unknown origin should be re-evaluated within 1 month to detect possible increasing antibody titres to B.b. Aseptic meningitis may be a stage 1 manifestation of Lyme disease.
