Background Current ESC guidelines for non-ST-segment elevation myocardial infarction suggest the utilization of echocardiography in patients with inconclusive initial electrocardiography and cardiac enzymes. Besides detection of alternative pathologies associated with chest pain, echocardiography can screen for wall motion abnormalities (WMA) as sign of myocardial necrosis. Purpose We evaluated the ability of the assessment of regional WMA, detected via transthoracic echocardiography, to predict the presence of culprit lesions in patients presenting with acute chest pain to the emergency department. Methods In this prospective single-centre observational cohort study, we included consecutive patients presenting to the emergency department of our University Hospital with acute chest pain, suggestive of an acute coronary syndrome, between December 2018 and August 2020. Patients with ST-elevation myocardial infarction, hemodynamic instability, or known coronary artery disease were excluded. As part of initial workup, patients received bedside echocardiography for the assessment of regional WMA by a dedicated study physician, blinded to all patients' characteristics. The primary endpoint was defined as the presence of culprit lesions as detected in subsequent invasive coronary angiography, requiring coronary revascularization therapy. Logistic regression analysis was performed in different models adjusted for traditional cardiovascular risk factors, cardiac biomarkers as well as established risk scores. Area under the receiver operating characteristics curve (AUC) was calculated to assess a potential improvement in the prediction of culprit lesions. Results Overall, 657 patients (age 58.06±18.04 years, 53% male) were included in our study. WMA were detected in 76 patients (11.6%). Patients with WMA were older (66.92±13.85 vs. 56.90±18.21 years, p Conclusion WMA strongly and independently predict the presence of culprit lesions in patients presenting with acute chest pain to the emergency department. Our results suggest that routine bedside echocardiography for assessment of WMA in emergency department may improve diagnostic algorithms in suspected acute coronary syndrome. Background The amount of epicardial adipose tissue (EAT) is associated with prevalent and incident myocardial infarction. However, clinical studies, specifically designed to determine, how the assessment of EAT can affect patient management, are lacking. Within Epicardial adipose tissue thickness PredIcts obstructive Coronary artery disease in Acute Coronary Syndrome patients (EPIC-ACS) study we tested the hypothesis that EAT quantification improves the prediction of the presence of culprit lesions in patients presenting with acute chest pain to the emergency department. Methods In this observational cohort study, we prospectively included consecutive patients presenting to the emergency department with acute chest pain suggestive of acute coronary syndrome between December 2018 and August 2020. Patients with ST-elevation myocardial infarction, hemodynamic instability, or known coronary artery disease were excluded. As part of the initial workup, bedside echocardiography for quantification of EAT thickness was performed by a dedicated study physician, blinded to all patients' characteristics. Treating physicians remained unaware of the results of the EAT assessment. The primary endpoint was defined as presence of a culprit lesion, as detected in subsequent invasive coronary angiography within 90 days after initial presentation. Logistic regression analysis was performed in different models adjusted for traditional cardiovascular risk factors, cardiac biomarkers as well as established cardiovascular risk scores. Results Overall, 657 patients (mean age 58.06±18.04 years, 53% male) were included in our study. Die extrakorporale kardiopulmonale Reanimation („extracorporeal cardiopulmonary resuscitation“, eCPR) kann als Rettungsversuch fur hoch selektierte Patienten mit refraktarem Herz-Kreislauf-Stillstand und potenziell reversibler Atiologie erwogen werden. Aktuell fehlen randomisiert-kontrollierte Studien zur eCPR und es existieren keine validen Pradiktoren fur Nutzen und Risiko, die bei der Indikationsstellung hilfreich sein konnten. Die bisherigen Selektionskriterien und Ablaufe sind klinikspezifisch und ein (nationaler) standardisierter Algorithmus fehlt. Das vorliegende Konsensuspapier bietet basierend auf einer konsentierten Expertenmeinung den Vorschlag fur ein standardisiertes Vorgehen bei eCPR.

In den letzten Jahren sind die Techniken zur Uberwachung und Befunderhebung immer weiter ausgereift. Von einfachen Untersuchungen, wie der Erhebung des Blutzuckers bis hin zu komplexen wie der Bestimmung von Gerinnungsparametern konnen jetzt viele Untersuchungen in unmittelbarer Nahe zum Patienten erfolgen. Die rasche Verfugbarkeit von Ergebnissen kann dabei zu unmittelbaren Konsequenzen in der Diagnosefindung und Therapieentscheidung fuhren. Die Diagnostik direkt am Patientenbett ist Dank vieler mobiler Analysatoren mittlerweile auch auserhalb der Klinik im Rettungsdienst moglich. Eine Standarduntersuchung in der Intensiv- und Notfallmedizin, welche auch zunehmend in der Praklinik eingesetzt wird, ist die Blutgasanalyse. Durch die Feststellung der Blutgase konnen respiratorische Insuffizienzen differenziert und die Beatmung optimiert werden. Bei instabilen oder reanimationspflichtigen Patienten kann die Blutgasanalyse zur Ursachenfindungen genutzt werden und gleichzeitig entsprechende Therapien beinhalten.

Extracorporeal cardiopulmonary resuscitation (eCPR) may be considered as a rescue attempt for highly selected patients with refractory cardiac arrest and potentially reversible etiology. Currently there are no randomized, controlled studies on eCPR, and valid predictors of benefit and outcome which might guide the indication for eCPR are lacking. Currently selection criteria and procedures differ across hospitals and standardized algorithms are lacking. Based on expert opinion, the present consensus statement provides a proposal for a standardized treatment algorithm for eCPR.

Sauerstoff ist das mit Abstand am haufigsten eingesetzte Notfallmedikament und erfreut sich hoher Akzeptanz. Bei fast jeder Notfalltherapie wird Sauerstoff in z. T. hohen Dosen eingesetzt und besitzt nahezu keine akut sichtbaren Nebenwirkungen. In der jungeren Vergangenheit erscheinen zunehmend Daten uber mogliche negative Effekte einer Hyperoxie besonders nach erfolgreicher Reanimation sowie beim Myokardinfarkt. Fur einen rationalen Einsatz von Sauerstoff mussen diese Daten im Kontext der praklinischen Arbeitssituation beachtet und bewertet werden.

Die aktuellen Empfehlungen der Bundesarztekammer (BAK) sehen den Einsatz ausgewahlter Notfallmedikamente durch Rettungsfachpersonal unter bestimmten Voraussetzungen auch vor Eintreffen eines Arztes vor. Seit 2004 ist in den Empfehlungen der BAK zur Notkompetenz auch ein Analgetikum zur Behandlung bei Verletzungen und ausgewahlten Schmerzsymptomen vorgesehen. Aufgrund dieser Empfehlungen kommen derzeit einzelne Konzepte zur Analgesie durch Rettungsassistenten zur Anwendung. Zur Beurteilung der Sicherheit und Wirksamkeit solcher Konzepte liegen bis heute kaum aussagekraftige Daten vor. Im Zeitraum vom 01.01.2006 bis zum 31.10.2009 wurden in einem Projekt ausgewahlte und speziell geschulte Rettungsassistenten mit der Anwendung eines Analgesiealgorithmus mit Morphin beim isolierten Extremitatentrauma beauftragt. Uber ein „call-back“-System musste die telefonische Freigabe der Analgesie durch einen Arzt angefordert werden. In die Datenauswertung wurden alle Einsatze, bei denen ein „call-back“ zur Analgesieanforderung erfolgte, integriert. Es wurden die Initial- und Ubergabewerte der Vitalparameter und der Schmerzintensitat anhand einer „numeric rating scale“ (NRS) von 0–10, die Menge an appliziertem Morphin sowie das Auftreten von relevanten Komplikationen und unerwunschten Nebenwirkungen deskriptiv ausgewertet. In 172 Fallen wurde eine Analgesie mit Morphin durch Rettungsassistenten durchgefuhrt. Relevante Komplikationen mit Uberschreiten vordefinierter Sicherheitsgrenzen traten nicht auf. Unerwunschte Nebenwirkungen, wie Ubelkeit und Erbrechen, traten selten auf (5,2%). Im Mittel wurden 8,5±3,9 mg Morphin iintravenos appliziert, was zu einer mittleren Schmerzreduktion von 8,3±1,2 auf 2,9±1,5 Punkten auf der NRS-Schmerzskala fuhrte. Die vorliegenden Daten sprechen fur die Sicherheit und Wirksamkeit der Analgesie mit Morphin bei isoliertem Extremitatentrauma bei Anwendung durch geschulte Rettungsassistenten.

Die Atemwegssicherung stellt eine zentrale Masnahme in der Notfall- und Intensivmedizin dar, um jederzeit eine ausreichende Oxygenierung zu gewahrleisten. Aufgrund des meist kritischen Patientenzustandes, der oft eingeschrankten Umgebungsbedingungen und oft auch unzureichender Erfahrung aller Beteiligten ist sie sehr risikobehaftet. Oberstes Ziel ist deshalb die Umgehung einer Intubation durch konsequente Anwendung nichtinvasiver Beatmung gerade auch im praklinischen Bereich. Bei bestehender Notwendigkeit zur Intubation mussen ausreichende Ruckfallebenen bzw. Alternativkonzepte zur Beherrschung des moglicherweise schwierigen Atemweges vorhanden sein. Als moderne, unterstutzende Verfahren stehen hier die fiberoptische Wachintubation und Videolaryngoskope zur Verfugung, mit denen die Sicherheit und Erfolgsrate deutlich verbessert werden konnen. Aufgrund der ubiquitaren Anwendbarkeit erscheint der Routineeinsatz der Videolaryngoskopie in der Notfall- und Intensivmedizin empfehlenswert.

Das akute kardiogene Lungenodem ist einer der Hauptgrunde fur die Hospitalisierung von Patienten. Die Mortalitat ist hoch, und viele Patienten benotigen eine Beatmung. Eine endotracheale Intubation jedoch hat – gerade in der praklinischen Situation – spezifische Komplikationen und birgt Risiken fur den Patienten. Systematische Untersuchungen legen nah, dass die „continuous positive airway pressure“ (CPAP) -Therapie die Notwendigkeit der Beatmung und auch die Mortalitat bei Patienten mit kardiogenem Lungenodem reduziert. Dennoch wird die Therapie praklinisch bisher nur selten eingesetzt. Wir therapierten insgesamt 108 Patienten mit vermutetem kardiogenen Lungenodem im Rahmen einer Beobachtungsstudie mit einem einfachen „flow-CPAP“-System (Fa. Vygon, Aachen) im Rettungswagen. Sowohl die Sauerstoffsattigung (von 82% auf 95%) als auch die Atemfrequenz (von 29/min auf 21/min) besserten sich hochsignifikant. Nur 8 Patienten (7,4%) mussten intubiert werden. Es wurden keine relevanten Komplikationen beobachtet. Diese Ergebnisse sprechen dafur, dass eine CPAP-Therapie sicher und erfolgreich auch in der praklinischen Notfallmedizin genutzt werden kann.

Die Versorgung polytraumatisierter Patienten in der Akutphase ist ein Musterbeispiel fach- und berufsgruppenubergreifender Zusammenarbeit unter besonders zeitkritischen Bedingungen. Mit der Leitlinie Polytrauma der Deutschen Gesellschaft fur Unfallchirurgie (DGU) sind umfassende Anforderungen formuliert, deren Ubertragung in die Praxis zusatzliche Prozessbeschreibungen erfordert. Auf der Basis der Leitlinie und anhand validierter Qualitatsindikatoren wurde die optimale Versorgungsqualitat als Soll-Zustand definiert.Unter Berucksichtigung lokaler Ressourcen wurden im Konsens aller an der Versorgung Beteiligten die wichtigsten Interventionen und Entscheidungsebenen in logischer Sequenz dargestellt (klinischer Algorithmus) und personelle Zustandigkeiten mit Zeitbezug festgelegt. Optimierungspotenziale wurden in Bereichen identifiziert, fur die in der Leitlinie keine expliziten Empfehlungen gemacht werden. Diese bilden die zentralen Elemente des Algorithmus. Zur Gewahrleistung der Kontinuitat des Qualitatsmanagements wurden Indikatoren, Zeitfenster und Verantwortlichkeit fur eine Reevaluation des Ist-Zustands und die Uberarbeitung des Behandlungspfads definiert. Die Implementierung von Leitlinien erfordert auf lokaler Ebene eine problem- und prozessorientierte, arbeitsplatzspezifische Ausarbeitung im Konsens aller Beteiligten. Diese Ausarbeitung der Leitlinie zum Behandlungspfad ist die Basis der quantitativen Prozess- und Kostenanalyse.

A total of 243 patients were entered at 20 general practice centres in the UK in this 6 week, double-blind, randomised, parallel-group study, which was designed to compare the safety and efficacy of nabumetone (1g at night) and slow-release diclofenac (100mg at night) in the treatment of osteoarthritis. Seven parameters of efficacy were measured, adverse events were recorded and laboratory monitoring was carried out. At the end of the study, both treatment groups showed highly significant clinical improvement with no significant intergroup differences. 24 patients (20%) in the nabumetone group and 21 (17%) in the slow-release diclofenac group withdrew before the end of 6 weeks, mainly because of adverse events. Although overall the incidences of adverse events were similar in both treatment groups, significantly fewer patients complained of digestive system side effects in the nabumetone group (21) than in the slow-release diclofenac group (35) [p = 0.03]. The study was also intended to evaluate the effectiveness of study monitoring by a clinical research associate who attended study centres with a laptop microcomputer to enter trial data. Immediate correction by the trialist of errors and omissions in the data, with on-site data entry on the laptop computer, contributed to trialist motivation and resulted in rapid recruitment of patients, collection of high quality data and speedy analysis at the end of the trial.

The food manufacturer is in business to make and sell good food at a profit. Consumers will only buy and make repeat purchases if the food is perceived as being good value for money and generally only if they can rely on the product being of a consistent standard. A firm needs therefore to establish a reputation for making a product to a certain standard and to maintain it. The quality standard that the manufacturer wishes to maintain should be clearly defined, preferably in the form of a quality policy statement made at a high level of management. To maintain the highest quality standards requires defined high quality of raw materials, careful supervision of manufacture and a sufficiently high selling price. This will not always be the appropriate objective, as in the majority of situations a lower but nonetheless acceptable standard of quality will be required for the realistically attainable market.

However, the food or drinks factory has a key role in determining the profitability and success of any food or drinks company. It is important to design a factory which is clean and safe in addition to producing a high quality product at a low cost.

In the two centuries since Nicholas Appert filed his revolutionary patent for the preservation of food by heating it in closed containers, the food industry has made enormous progress towards two objectives: (i) To conserve individual items of food so that they may be kept in good or perfect condition from the times and places where they are relatively abundant until they can be consumed at other times and in other places where they would not otherwise be available; (ii) To maximize the commercial efficiency of production.

We have performed a detailed pharmacokinetic study of the plasma concentrations of the major active metabolite of nabumetone, 6-methoxy-2-naphthylacetic acid (6 MNA), attained after a single dose and during chronic administration comparing the results of a group of young healthy volunteers with those of a group of elderly arthritic patients. The latter had higher peak plasma concentrations of 6 MNA and slower rates of elimination but there is no tendency for the drug to accumulate unpredictably in the old. Disease activity also influences plasma concentration, those with more active disease, and lower serum albumin concentrations had lower AUC values.
