Background. According to randomized clinical trial SOLO1 olaparib statistically significantly improves progression-free survival versus placebo as a maintenance monotherapy in patients aged 18 and over with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response to first-line chemotherapy. As the data on overall survival (OS) in this trial remains interim it is still uncertain whether treatment with olaparib can provide any benefits in terms of OS.Objective: to evaluate a long-term OS for olaparib versus placebo as a maintenance monotherapy in patients with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response to first-line chemotherapy.Materials and methods. A 10-year mathematic model of disease progression and survival on olaparib versus placebo was developed. Modelling was based on data on progression-free survival from SOLO1 trial and data on OS after platinum-sensitive and platinum-resistant relapses from OCEANS and AURELIA trials. Additionally, patients who haven’t been treated with olaparib after first-line therapy in base-case scenario were assumed to get olaparib as a second-line treatment after platinum-sensitive relapse; mortality modelling for these patients was based on data from SOLO2 trial.Results. Median OS for olaparib was 107 months versus 66 months for placebo. 46 % of patients treated with olaparib were alive by the end of 10-year modelling period, but only 28 % patients from the placebo group. Hazard ratio of death for olaparib versus placebo was 0.64 (95 % confidence interval 0.49–0.84). Probabilistic sensitivity analysis showed robustness of these results.Conclusion. Using olaparib as a maintenance therapy in patients with newly diagnosed advanced ovarian cancer with BRCA mutations, who had response on first line chemotherapy, statistically significantly reduces risk of death by 36 %, compared to placebo.

We analyzed associations of polymorphic markers of DNA repair genes (XRCC1, ERCC2), cell cycle control genes (TP53, MDM2, and CDKN1A), methylation of promoter region, and mutation 5382insC of BRCA1 gene in ovarian cancer with effectiveness of platinumbased chemotherapy assessed by the median of progression-free survival time for markers of DNA repair genes and by relapse risk for all studied markers. An increase in the median of progression-free survival time for carriers of the Gln allele (р=0.025) and Gln/Gln genotype (р=0.022) of the Gln399Arg XRCC1 was observed during the 19-months period after chemotherapy. In carriers of C/C genotype of 5382insC mutation of BRCA1 gene (n=6), no relapses were observed (р=0.035), while 17 of 49 patients without this mutation developed relapses. Of 14 patients with BRCA1 gene function inactivation due to promoter methylation or the presence of the C/C genotype of 5382insC, one relapse was observed (p=0.033). Multivariate analysis revealed an association of markers of the XRCC1, TP53, MDM2 genes, BRCA1 gene inactivation, and type of surgery with the risk of relapse during the follow-up period up to 19 months after the end of chemotherapy (р≤0.0007).

The Russian economy's long-term growth is inextricably linked to an appealing and efficient investment process, without which it is impossible to upgrade output structurally and qualitatively, build market infrastructure, and boost local competitiveness. The pressing issue of adequate investment tools necessitates an active search for viable sources for domestic economy demands, which concretizes the problem of mobilizing the state's internal resources - the population's savings. According to the experience of countries with a high degree of economic development, population savings are the primary source of establishing the resource basis for ensuring long-term economic growth and development. The function of the domestic financial market in the process of converting savings into investments is examined in this article. It has been established that there is a link between the financial market and the investment process. Theoretical components of the essence of savings and how they are transformed into investments are exposed. The impact of household financial investment volumes on the country's economic development indicators is studied and the dynamics and structure of population savings in Russia. The issues surrounding the conversion of savings into investments are recognized and the directions for intensifying it to maximize the financial potential of savings for the implementation of investment programs.

Objective. To study the accumulation of 11C-methionine in primary brain tumors of different types and intact brain tissue. Materials and methods. A total of 40 patients (21 men and 19 women) with gliomas with different levels of malignancy (Grade I–IV) were studied. All patients underwent PET-CT investigations with 11C-methionine and MRI scans with contrast enhancement. Standardized uptake values (SUV) were determined for tumors and intact tissue, along with the index of accumulation (IA) and tumor volume. Results. A large degree of individual variability was found in methionine uptake into unaffected brain tissue (SUV was 0.47–1.73 for the frontal lobe); metabolism of this amino acid was greater in women (SUV frontal lobe 1.05 ± 0.24 in men and 1.32 ± 0.22 in women). IA values gave better discrimination of gliomas in terms of the degree of anaplasia than SUV values; in particular, these were statistically significantly different for Grade III and Grade IV gliomas (2.64 ± 0.98 and 3.83 ± 0.75, p < 0.05). The lowest levels of methionine metabolism were noted in the subgroup with diffuse astrocytomas (IA = 1.52 ± 0.57), and this was lower than in anaplastic astrocytomas (IA = 2.34 ± 0.77, p < 0.05). Conclusions. Results obtained using PET-CT with 11C-methionine demonstrated high informativeness in the differential diagnosis of gliomas with low and high levels of malignancy (Grade I–II had IA = 1.66 ± 0.71 Grade III–IV had IA = 3.18 ± 1.06, p < 0.05) and in demarcating between diffuse astrocytomas with Grade II and III anaplasia. Furthermore, high individual variability was demonstrated in the parameter SUV in intact brain matter, along with differences in amino acid metabolism in different areas.

Within the framework of solving the fundamental issue of ensuring the safety of mining camps, mines, and vital functioning of the population, a methodology and a software algorithmic apparatus have been developed for processing and interpreting monitoring gravimetric observations which are aimed at identifying karst areas in the massif. A complex of prospective geophysical works aimed at detecting areas with intensive development of karst processes has been tested. The authors of the project carried out experimental methodological geophysical works including high-precision gravimetric observations on the territory of the Verkhnekamskoye potash deposit. These works were carried out to test various options for the field work methodology on specific geological objects. In a broader sence, the results of the work are an example of a wide approbation of an advanced research methodology aimed at predicting the destructive consequences of karst-suffusion process

In addition to the typical signs, what are the pallor of the skin and mucous membranes, frequent, weak pulse, fainting, etc., internal bleeding that occurs as a result of rupture of the fetus during an ectopic pregnancy is accompanied by other, no less characteristic phenomena from various systems organism, which sometimes make it possible to timely confirm or establish a diagnosis and differentiate this suffering from other similar painful conditions.

Breast angiosarcoma (BAS) is an extremely rare and poorly studied neoplasm, the etiology of primary BAS remains controversial, and secondary BAS is most often radio-induced. Radio-induced breast tumors usually appear 10–20 years after the initial treatment; however, for BAS this period is much shorter and is about 4–7 years. This review presents literature data on the features of the clinic, diagnosis and treatment of BAS, as well as own clinical case observation of radio-induced sarcoma of the left breast, that developed 4 years after the primary breast cancer treatment.

Young women with reproductive tract neoplasms who receive treatment leading to termination of ovarian function often suffer from menopause symptoms that contribute to dramatic drop in quality of life. Climacteric symptoms in women with iatrogenic menopause are more severe than in case of natural menopause, especially in women with reproductive tract neoplasms. They lead to dramatic drop in quality of life and are one of the main reasons to stop applying endocrine therapy in women with hormone-positive tumors, which leads to decrease in disease free survival and to decline the prognosis. The most effective treatment option for climacteric symptoms of moderate to severe degrees is menopause hormone therapy; however, such therapy is not suitable for patients with estrogen-dependent tumors in past medical history due to the likelihood risk of progression of cancer, as well as the risk of venous thrombosis, the frequency of which in cancer patients increases. Non-hormonal pharmacological and non-pharmacological correction methods are used as first-line therapy for menopause disorders in women with estrogen-dependent tumors of the reproductive system. Among non-hormonal non-pharmacological correction methods actively study such methods as acupuncture, yoga, exercise to control weight, and a diet rich in phytoestrogens. The most effective non-hormonal methods of correcting vasomotor symptoms are serotonin and norepinephrine reuptake inhibitors. However, currently in Russia these drugs can be prescribed only by a psychiatrist. The finding of effective and safe non-hormonal methods to correct menopause symptoms in women with hormone-positive reproductive tract tumors is the important task in practice among doctors in different specialties.

This study addresses investigations of dental tissue surfaces by in vitro shift interferometry. This work is relevant because use of this device for measuring the surface of dental enamel has high precision — to tenths of micrometers. The operation of a functional interferometer system is described. The criteria for adjustment of the device are given. Experimental results from determination of the surface roughness of a second molar treated with Tokuyama Bond Force II and in the natural state are presented. Measurement error of 0.1 μm was obtained for the treated sample, compared with 0.25 μm for the untreated sample. The measurement error due to the need to rotate the radiation polarization plane using a half-wave plate in one channel of the shift interferometer was estimated.

We studied the association of polymorphic markers of cell cycle control genes (Arg72Pro of the TP53 gene, T(-410)G of the MDM2 gene, and Ser31Arg of the CDKN1A gene) in ovarian cancer and progression-free survival following platinum-based chemotherapy. Tumor tissue samples obtained from 49 patients who had undergone chemotherapy were examined. Patients received standard platinum-based chemotherapy and were observed until disease progression. Polymorphic markers of genes were evaluated by PCR-RFLP and real-time PCR. In patients carrying the G allele of the T(-410)G marker of the MDM2 gene, a decreasing trend was observed in median progression-free survival. An increase in the median progression-free survival was observed in carriers of the Pro allele of the TP53 gene (p=0.045). Furthermore, a stronger association was noted with carriers of the minor Pro/Pro homozygous genotype relative to the Arg/Arg genotype (p=0.007). In the subgroup of patients who underwent optimal or complete cytoreductive surgery, carriage of the minor Arg allele of the Ser31Arg marker (CDN1A gene) was associated with a decrease in the median progression-free survival time (p=0.004).

In 2011, a standard approach to the treatment of primary ovarian cancer (OC) included a cytoreductive surgery, which could be performed after 2–3 cycles of neoadjuvant chemotherapy, and chemotherapy consisting of platinum and taxanes. Such approach was provided for all patients, regardless of tumour histology and any molecular biological and genetic factors. The most complete picture of management and therapy of patients can be made using the treatment of a specific patient as an example. After application to the N.N. Blokhin National Medical Research Center of Oncology in 2011, the patient with OC received standard primary therapy and subsequent treatment of the recurrent disease, which was accompanied by various types of adverse events resulting in the poor quality of life for the patient. The data that some patients with OC have a BRCA1/2 mutation that is significant for prognosis and treatment came to hand later and, unfortunately, the awareness of a significant germinal BRCA1 mutation was of no use to the woman any longer. The life expectancy of this patient was 47 months. This is the average life expectancy for patients with stage IIIC OC. Major changes have been brought in the primary therapy of OC. If a diagnosis of low-grade IIIC ovarian adenocarcinoma was established in this patient today, needless to say that the BRCA1 mutation would be identified during the first-line chemotherapy, and in case of full or partial treatment effect, we would prescribe olaparib as maintenance therapy to the patient. Considering the fact that the median progression-free survival has not yet been achieved in the patients of SOLO-1 study, who received olaparib therapy, and is only approaching 54 months, it can be assumed that even the first relapse could not have developed in this patient.

The involvement of axillary lymph nodes is one of the most important prognostic factors, significantly affecting the treatment strategy for early breast cancer (BC). The risk of axillary lymph node metastases depends directly on a number of factors (age of women, size of tumor, presence of lymphovascular invasion and biological characteristics of cancer). The evaluation of regional lymph node status in patients with early BC includes the clinical examination of regional zones and the ultrasound study (US), using these methods can help to study lymph nodes shape, borders, margins and structure. The sensitivity of ultrasound in the evaluation of regional lymph nodes status directly depends on the biological subtype of the tumor; the minimum level of ultrasound sensitivity in the evaluation of lymph nodes status is detected for luminal HER2-negative cancer (less than 40%), and maximum sensitivity is detected for triple negative and HER2-positive subtypes (6871%). Clinical examination and modern ultrasound are the most accessible methods for the evaluation of regional lymph nodes status, but the possibility to misjudge metastatic process can be detected in 1/4 of patients. Verification of the diagnosis in the preoperative phase (fine-needle aspiration biopsy/core-needle biopsy under ultrasound guidance) allows minimize the number of errors for the regional staging. The sentinel lymph node biopsy (SLNB) is the gold standard of regional treatment in patients with early stage BC, nowadays. The randomized trials (NSABP B-32, ACOSOG q0011) show the safety of recession of performing regional lymph node dissection in favor of SLNB not only in case of clinically negative lymph nodes, but also in patients with metastases in 2 sentinel lymph nodes, upon condition that organ-conservative treatment and subsequent radiation therapy will be used. High-quality regional staging, the choice of the therapeutic algorithm in accordance with the biological characteristics of carcinoma, the application of the most effective modern drug regimes, the optimal radiation therapy allow not only minimize the extent of surgery, but also achieve high long-term survival results, provide excellent functional results and high quality of life in patients with the involvement of axillary lymph nodes.

Ovarian cancer is the fifth leading cause of cancer-related death among women in the world. In spite of recent progress in treatment strategy, around 70 % of ovarian cancer patients relapse. The cytoreductive surgery followed by platinum-, taxane-containing chemotherapy is the standard approach to the primary treatment of ovarian cancer; a variant of neoadjuvant chemotherapy with interval cytoreduction may be used in patients with stage IIIC-IV disease.Given that angiogenesis plays a central role in progression of solid tumour growth and metastasis, recent studies have focused on anti-angiogenic treatment. Bevacizumab, a humanized IgG monoclonal antibody that inhibits the vascular endothelial growth factor receptor, is most promising antiangiogenic drug. Bevacizumab was approved on December 23, 2011 by the European Medicines Agency and on June 13, 2018 by the Food and Drug Administration as first-line treatment in epithelial ovarian, fallopian tube or primary peritoneal cancer stage III or IV in combination with carboplatin and paclitaxel. Based on sub-analyses, the recommended dosage of bevacizumab is 15 mg/kg every 3 weeks for a total of 22 cycles. Bevacizumab is a well-studied drug with a favourable safety profile that has been used in routine clinical practice for more than 10 years. However, the search for predictors to identify the category of patients, who will benefit most from bevacizumab therapy, is still in progress, and clinical trials that may improve the therapeutic potential in treating ovarian cancer in the near future due to introduction of new combinations of bevacizumab with PARP inhibitors and immuno-oncological drugs are under way. In view of acquisition of mature clinical trial data, a range of the most discussed issues regarding optimal use of bevacizumab in patients with ovarian cancer has been identified.

The trial objective was to determine the peripheral blood NK cells cytotoxic activity effect on trophoblast cells at recurrent pregnancy loss (RPL). The investigation involved non-pregnant women with PRL in proliferating and secretory menstrual cycle phases (PMCPh and SMCPh, respectively); women of 6-7 weeks pregnancy with RPL in past medical history; healthy fertile non-pregnant women in PMCPh and SMCPh, women of 6-7 weeks physiological pregnancy, nulliparity healthy women with regular menstrual function in PMCPh and SMCPh. NK cells cytotoxic activity was determined using peripheral blood mononuclear cells. The target cells were JEG-3 line trophoblasts. It has been established that NK cells cytotoxic activity effect on trophoblasts is lower in SMCPh than in PMCPh in non-pregnant fertile women. The NK cells cytotoxic activity was higher in SMCPh than in PMCPh in non-pregnant women with PRL and also higher than the same value in SMCPh in non-pregnant fertile women. The increased NK cells cytotoxic activity values in SMCPh in women with RPL may be the reason for miscarriage.

The aim of this research was to assess the proliferative activity of Natural Killer Cells (NK cells) from Peripheral Blood Mononuclear Cells (PBMCs) in the presence of trophoblast cells in women with a history of recurrent miscarriages. We examined the peripheral blood of women with recurrent miscarriage in the proliferative (n = 12) or secretory (n = 13) phase of their menstrual cycle, and pregnant women with a history of recurrent miscarriage at 6-7 weeks of their current pregnancy (n = 14). Controls were fertile non-pregnant women in the proliferative (n = 11) or secretory (n = 13) phase of their menstrual cycle, and pregnant women at 6-7 weeks of a physiologically normal pregnancy (n = 20). We used IL-2 as a factor maintaining PBMCs viability during long-term culturing. We established that culturing in the presence of IL-2 contributed to an increase in the number of CD56+CD16- NK cells and to a decrease in the number of CD56+CD16+ NK cells from PBMCs compared with these numbers before culturing in both healthy women and in women with recurrent miscarriage. After culturing of PBMCs in the presence of trophoblast cells and IL-2 (compared with culturing without trophoblast cells), the intensity of Ki-67 expression by NK cells was reduced in the whole NK cell population (CD3-CD56+), and in the CD56+CD16- and CD56+CD16+ populations of NK cells in women with recurrent miscarriage and in healthy controls. The intensity of CD56 expression was reduced in the presence of trophoblast cells and IL-2 in non-pregnant women with recurrent miscarriage in the secretory versus the proliferative phase of the menstrual cycle.

Aim. Eribulin is an active cytostatic, associated with a wide range of mechanisms of antitumor effects, but eribulin efficiency and safety in patients with breast cancer (BC), associated with cerebral metastases are still poorly understood. Materials and methods. We analyzed the combined Russian experience of eribulin application in BC patients associated with brain metastases; the analysis included 459 Russian women with advanced BC who had received at least 2 course of eribulin during the period from 2014 to 2018; 35 of 459 patients had brain metastases (40.0% - luminal HER2-negative subtype, 31.4% - triple negative subtype and 28.6%h - HER2-positive BC). The median age was 52 years (39 - 80 years of age). In most cases, the patient had two or more metastatic brain lesions (68.6%; the median was - 3); brain radiotherapy was used in 62.8% of patients before eribulin treatment and in 5.8% of patients was held stereotactic radiation therapy during eribulin chemotherapy. We analyzed the efficiency of eribulin application (the therapy continued until disease progression, the development of unacceptable toxicity, or impossibility to apply the drug for any other reason). Results. The results showed that clinical efficacy (objective response rate + stabilization of disease lasting for more than 6 months) was 48.6%: partial response - in 20% of patients and stabilization of disease - 62.9%; tumor growth control was in 82.9%. Median PFS in all group of patients with brain metastases was 4.1 months and was similar to median PFS in patients who received radiotherapy before eribulin treatment or without eribulin - 4.1 vs 3.47 months; p=0.798. Conclusions. The application of eribulin in BC patients with brain metastasis are absolutely justified, the drug demonstrates the efficiency in a retrospective analysis in a Russian population. The determination of the optimal algorithm for the treatment of patients with metastatic BC associated with brain metastasis requires a multidisciplinary approach and further research.

Objective. The purpose of the paper is to study and compare the level of tumor-associated proteins CD44 and S100, indicators of protein and lipid metabolism in melanocytic skin tumors. Materials and Methods. The authors examined 100 samples of 10 % homogenates of skin melanoma tissue, nevi, perifocal zone and resection line. CD44 and S100 levels were determined by enzyme immunoassay using standard test systems on a TECAN analyzer (Austria). The levels of total protein, cholesterol, triglycerides were determined on a ChemWell biochemical analyzer (USA). Results. A sharp increase in S100B level was detected in melanoma tissues, 28 times as high as in the samples of healthy tissue and nevi, as well as a significant, but less evident increase in the CD44 level, which was also observed in nevi tissue. The ratio of albumin and gamma globulins in melanoma and nevi tissues was 3–6 times lower if compared with healthy tissue, and the levels of cholesterol and triglycerides in melanoma were only a little higher than in healthy tissues and nevi. A more than double increase in the γ globulin fraction in melanoma tumor tissue with a decrease in albumin level and the absence of changes in other globulins, as well as a moderate but statistically significant increase in the γ globulin fraction in nevus tissue suggest that the tumor-associated S100B and CD44 markers belong to the γ-globulin fraction. Conclusion. The highly specific increase in S100B level in the supernatant of melanoma tissue homogenates, as well as a less specific increase in CD44 combined with the γ-globulin fraction dominance, suggest that such a correlation is an adverse prognostic sign of tumor progression, which may be important while choosing personalized treatment strategies. Изучить в сравнительном аспекте уровень опухолеспецифических белков CD44 и S100, показателей белкового и липидного обмена в меланоцитарных новообразованиях кожи. Материалы и методы. Объектом исследования были 100 образцов 10 % гомогенатов ткани меланомы кожи, невусов, перифокальной зоны и линии резекции. Уровень CD44, S100 определяли методами иммуноферментного анализа с использованием стандартных тест-систем на анализаторе TECAN (Австрия). Содержание общего белка, холестерина, триглицеридов устанавливали на биохимическом анализаторе ChemWell (США). Результаты. В тканях меланомы выявлено резкое увеличение уровня S100B, в 28 раз превышающего его значение в образцах здоровой ткани и невусов, а также достоверное, но менее выраженное увеличение уровня CD44, которое также наблюдалось в ткани невусов. Соотношение альбуминов и гамма-глобулинов в ткани меланомы и невусов было снижено в 3–6 раз по сравнению со здоровой тканью, а содержание холестерина и триглицеридов в меланоме незначительно превышало их содержание в здоровых тканях и невусах. Более чем двукратное увеличение фракции γ-глобулинов в опухолевой ткани меланомы на фоне снижения уровня альбуминов и отсутствия изменений других глобулинов, а также умеренное, но статистически значимое увеличение фракции γ-глобулинов в ткани невусов позволяют предположить, что изученные нами в качестве онкомаркеров белки S100В и CD44 относятся к фракции γ-глобулинов. Выводы. Высокоспецифичное повышение уровня S100B в надосадочной жидкости гомогенатов ткани меланомы, а также менее специфичное увеличение CD44 в сочетании с доминированием фракции γ-глобулинов позволяют предположить, что подобное соотношение факторов является прогностически неблагоприятным признаком опухолевой прогрессии, что может быть важным при выборе персонализированной тактики лечения. We performed a retrospective analysis of ultrasound scans in 42 patients with cutaneous melanoma of the extremities in planning the closure of skin defects with skin fascial island flaps supplied by perforating vessels. First, the primary melanoma focus was scanned by ultrasound to reveal characteristics of the skin, tumor and adjacent tissues. Then we detected perforating vessels to mark them and to select the sites for the island flaps. Ultrasound examination was performed using the IU 22 PHILIPS, GE Logiq E9 and Supersonic imagine AIxPLORER MultiWave Systems with linear multi-frequency sensors (517 MHz) in the B-mode, color and power Doppler to visualize the blood flow. We clarified the criteria of the necessary and sufficient parameters according to Doppler ultrasound visualization for surgical treatment of cutaneous melanoma of the extremities by the suggested method in preoperative period, as well as monitoring of the flap state after surgery. The transfer of flaps on perforating vessels did not cause blood flow disorders in them: the mean arterial blood flow velocity was 13.1 ± 4.7 cm/sec before surgery and 12.8 ± 5.4 cm/sec after it. The maximal venous flow velocity was on average 7.0 ± 1.3 cm/sec before surgery and 6.2 ± 0.8 cm/sec after it. Thus, triplex ultrasound significantly facilitates the selection and individual design of the flap with the inclusion of feeding vessels of sufficient potential, helps in planning the operation, reduces the risk of failure and improves the results of treatment. This method contributes to the radicalization of surgical intervention with a simultaneous decrease in the risk of postoperative complications and acceleration of medical and social rehabilitation of patients.

More than 300 000 new cases of endometrial cancer (EC) are registered annually in the world. The prognosis for EC is determined by the stage of the disease. In Russia, stage III–IV EC is diagnosed in 16 % of patients, with the mortality rate in the first year of follow-up of 9.2 %. The management of recurrent/metastatic EC remains a challenging clinical problem. Hormone therapy and chemotherapy is effective in 30 % of patients, with median progression-free and overall survival rates of approximately 6 months and 12 months, respectively. The role of targeted therapy in the treatment of EC remains undefined. However, several studies demonstrated that combination of targeted therapy with chemotherapy resulted in significant improvement in overall survival of patients with advanced endometrial cancer. Published data on the study of checkpoint inhibitors for EC have shown promising results, however, further research is required to more completely understand how the immune system recognizes and eradicated cancer.

Aim: To perform a comparative analysis with simultaneous measurement of vascular endothelial growth factor (VEGF), insulin-like growth factor 1 (IGF1) and matrix metalloproteinase 7 (MMP7) in serum samples taken from healthy women and ovarian cancer patients; to perform association of these markers with their expression in primary tumors depending on clinical, morphological and biochemical characteristics of the disease and its prognosis. Materials and methods: We assessed 54 treatment-naïve patients with ovarian cancer aged from 23 to 74 years (mean ± SD, 53.2 ± 1.9), being at various FIGO stages of the disease. The control group consisted of 120 healthy women of matched age and reproductive status, in whom serum biomarker levels were studied. Patient survival was assessed by the Kaplan-Meier method, with survival curves compared with log-rank test. All analyses were done with “STATISTICA” and SPSS software. Results: Serum VEGF levels in ovarian cancer patients were significantly (p 505 pg/ml (upper quartile in the control). With 505 pg/ml taken as a threshold, the test had sensitivity of 79.6% and specificity of 75%. Another cut-off value of serum VEGF level between the patients with ovarian cancer and the control group (510 pg/ml) was derived from ROC curves and 75% sensitivity and 78.2% specificity. No acceptable cut-off value for serum IGF1 to differentiate between the patients with ovarian cancer and the controls could be obtained from the ROC curves. The metrological characteristics of an automated interferometric system for measuring the surface shape of diffusely reflecting objects have been experimentally studied. Changes in the amplitude of the output signal during modulation of the optical path difference were studied and visualization of the effect of introduced interference on the measurement error was made. It is established that as the angle of incidence of optical radiation increases, the duration of the interference signal increases and, correspondingly, the measurement error of the automated interferometric system. The dependence of the measurement range on the scanning frequency of the reference mirror is obtained. The measurement error of an automated interferometric system under normal illumination does not exceed 0.67 μm.

e21581 Background: Overexpression of the Bcl-2 protein inhibits apoptosis and promotes carcinogenesis. Stress causes signaling leading to cell buffering with Bcl-2 protein above acceptable levels. The purpose of the study was to analyze the influence of comorbidity – chronic neurogenic pain (CNP) – on the Bcl-2 levels in mitochondria of cells of melanoma, the heart, skin and brain in female mice with growing tumors. Methods: Female С57ВL/6 mice were divided into groups: intact group (n = 21); control group with a CNP model – bilateral sciatic nerve ligation (n = 21); group M – B16/F10 melanoma (n = 63); CNP+M group – B16/F10 melanoma was transplanted 3 weeks after the CNP model creation (n = 63). The concentration of Bcl-2 (ng/mg of protein) was determined in mitochondrial samples by ELISA (Thermo Fisher Scientific, Austria). Statictical analysis of results: Statistica 10.0. Results: CNP decreased the Bcl-2 level in heart mitochondria by 1.3 times (p < 0.05), but increased it in skin and brain mitochondria by 5.8 and 1.3 times, respectively. Similar changes were observed in melanoma growth 1 week after its transplantation: Bcl-2 levels decreased in heart mitochondria by 1.3 times, and increased in the skin and brain by 8.9 and 1.3 times, respectively. After 2 weeks of the tumor growth, Bcl-2 in brain mitochondria decreased by 1.7 times, and it started declining in the skin by the 3rd week – by 4 times, compared to intact females. Bcl-2 in tumor mitochondria exceeded the values in the skin by more than 4 times throughout the experiment. Tumor growth in presence of CNP caused a decrease in Bcl-2 in brain mitochondria by 2.4 times after 3 weeks, and in the heart and skin – by 2 and 1.7 times, respectively, after 2 weeks. Bcl-2 in tumor mitochondria in presence of CNP was lower than in the intact skin on average by 1.8 times throughout the experiment. Conclusions: CNP as a comorbidity caused a modulating effect on the mechanisms of survival and apoptosis of cells both in the tumor and in the main organs providing the vital functions of the body - the brain and heart, and also affects the target organ of melanoma - the skin. The results demonstrated the ability of comorbidity to change levels of Bcl-2 in mitochondria depending on the stage of tumor development.

e21580 Background: The thyroid gland is the most important regulator of the body's response to stressful influences, including malignant growth. Iodine is a constituent of thyroid hormones, and it plays an important role in anti-tumor protection. The purpose of the study was to analyze the levels of thyroid hormones in the thyroid gland of male and female mice with B16/F10 melanoma, as well as the effect of an organic iodine-based preparation on the survival of animals with transplanted melanoma. Methods: Levels of total (T3, T4) and free (FT3, FT4) thyroid hormones were measured by RIA in the thyroid of male and female С57ВL/6 mice (n = 80) with transplantable B16/F10 melanoma. The effect of an organic iodine-based preparation on the dynamics of the melanoma growth was studied in the main group of animals (n = 42) receiving per os 1,3-diethylbenzimidazolium triiodide in a single dose of 0.4 mg/kg throughout the experiment; the control group (n = 20) received distilled water. The tumor volume and survival of mice were measured. Results: The lowT3/lowT4 syndrome developed in females with transplantable B16/F10 melanoma starting from week 2 of the experiment, diagnosed by decreasing levels of FТ3 and FТ4 by 3 times and lower and normal levels of T3 and T4. In males, melanoma growth caused the thyroid hypofunction expressed in a decrease in the levels of T3 and T4 by 2 times, and FT3 and FT4 by 1.5 times. The latent period prior to the tumor onset was 10-12 days in females and 5-7 days in males. The survival of females was 5-6 weeks, males – 3-4 weeks. The administration of the organic iodine-based preparation extended the latent period in females up to 14-18 days, in males up to 10-14 days; the survival of females increased by 1.8 times, males by 1.5 times. 9% of females in the main group showed complete tumor resorption without relapses throughout their lives. Conclusions: The growth of transplantable B16/F10 melanoma in male and female С57ВL/6 mice was accompanied by suppression of the thyroid function with the development of uncontrolled hypothyroidism in males and the lowT3/lowT4 syndrome in females, which correlates with the survival of animals.

e15045 Background: Berberine is an alkaloid compound with a structure that is highly similar to that of intercalating agents. It affects numerous cell signaling pathways and is widely studied as potential anticancer drug. It is known that berberine affects cancer cells migration through metalloproteinase-2 inhibition, but this effect was never studied on glioma cells. Anti-migratory drugs are of special interest in brain cancer therapy since glioma's highly invasive nature makes total surgical removal of tumor practically impossible. The aim of the study was to evaluate berberine anti-migratory activity on glioma cells. Methods: Cell migration capacity of T98G and U87MG cell lines, as well as primary glioma cell culture established in our laboratory, was assessed via standard wound healing assay with automated image acquisition and analysis on Lionheart FX (BioTek) cell imager. Prior to assay setting up cell cultures were maintained in DMEM medium with L-glutamine (1 μM) (Gibco) and 10% FBS (Gibco) at 37C0 and 5.0% CO2. Cells were seeded at 250 000 cells per well on 24-well plates and incubated overnight in order to attach to plate bottom. After that a vertical wound was made manually in each well, and berberine was added to experimental wells to final concentration 50 mg/L. Plates with cells were continuously incubated and photographed in cell imager at 37C0 and 5.0% CO2. The extent of cells migration was measured as the percent of wound area decrease after 24 hours of incubation in relation to starting time point. Data are given as: Mean ± 95% confidence interval. Results: In our study we berberine exhibited anti-migratory activity in all cell cultures under study. In rather fast growing primary cell culture wound area decrease was 99.23%±0.62% in control sample and 91.75%±0.28% in experimental sample. The difference was small but significant at p < 0.001 level (df = 30). Popular permanent glioma cell lines T98G and U87MG showed more prominent decrease in studied parameter with higher degree of variance at the same time. In T98G wound area decrease was 71.6%±12.3% in control and 48.8%± 7.6% in experimental samples after 24 hours of cultivation in presence of 50 mg/L berberine. While U87MG demonstrated 60.28%±5.13% and 37.5%± 8.34% wound area decrease accordingly. The obtained difference between control and experimental groups in permanent cell cultures was statistically significant at the 0.05 level (df = 30). Conclusions: Our preliminary research proved berberine to be potent anti-migratory agent in glioma treatment. Further investigations are needed to evaluate its ability to inhibit glioma cell expansion in vivo.

e21557 Background: 10% of cancer patients have comorbidities accompanied by chronic pain. Neurotrophins and fibrinolytic system are involved in carcinogenesis and pain pathogenesis. The purpose of the study was to measure levels of neurotrophins in white matter of the brain of urokinase-deficient (uPA–) mice with B16/F10 melanoma growing in presence of chronic neuropathic pain (CNP). Methods: The study included female mice С57ВL/6 (normal genome uPA+, n = 40) and C57BL/6-Plautm1.1BugThisPlauGFDhu/GFDhu (urokinase gene-knockout uPA–, n = 28) with B16/F10 melanoma (M) implanted subcutaneously 2 weeks after bilateral sciatic nerve ligation (CNP model). Intact mice (I) were controls. Levels of brain-derived neurotrophic factor BDNF, nerve growth factor NGF-β and neurotrophins 3 and 4 (NT3, NT4) were measured by ELISA in white matter of the brain after 3 weeks of tumor growth in presence of CNP. Results: Tumor volume in (uPA–) females by week 3 of carcinogenesis was 0.04 cm3, which was 70 times smaller than in (uPA+) females. Tumor volume in (uPA–) females with CNP was 5.76 cm3, which was 144 times larger than in (uPA–) females without CNP, and in (uPA+) females – 2.5 cm3. The brain of I (uPA–) showed higher levels of NT3 (by 1.3 times, p < 0.05), NT4 (by 2.6 times) and NGF-β (by 1.9 times, p < 0.05) and lower BDNF (by 1.7 times, p < 0.05), compared to I (uPA+). Both strains of mice with M or CNP demonstrated decreased levels of NGF-β, more pronounced in animals with a combination of these factors. (uPA–) females with CNP+M showed a decrease of NT3 and BDNF by 2 times, with NGF-β 2.2 times higher than in (uPA+) mice. Conclusions: The study revealed underlying differences in levels of neurotrophins in the brain of (uPA–) females which could contribute to the creation of conditions for the inhibition of tumor growth. Changes in the levels of NGF-β in mice with melanoma or CNP were nonspecific. Changes in the BDNF, NT3 and NGF-β balance in the brain of (uPA–) mice may be part of the mechanism of greater stimulation of melanoma growth under the influence of CNP.

The development of cytostatic therapy in the treatment of ovarian and cervical cancer has resulted in a deadlocked situation. Thus, the basic aim of the investigations was to examine the biological basis of these diseases, last years. Angiogenesis is one of the main factors of the tumor microenvironment, associated with tumor growth and metastasis. In this regard, new drugs that work by inhibiting angiogenesis have started to appear. One of the first drugs studied in randomized phase III studies was bevacizumab. The application of bevacizumab demonstrated the significant increase both in progression free-survival and life expectancy in high-risk group of patients with ovarian cancer and in patients with recurrent and advanced cervical cancer.

The treatment of patients with ovarian cancer remained unchanged over the past years: cytoreductive surgery and platinum-based chemotherapy regardless of the histological type of the tumor. A deeper understanding of ovarian cancer biology has paved the way to developing targeted drugs; the most studied are bevacizumab and olaparib (Lynparza) - the first among PARP inhibitors, which has been recently approved in the world and in Russia. Olaparib demonstrated a statistically significant efficacy in the treatment using supportive regimen in patients with recurrent platinum-sensitive ovarian cancer after successful platinum-based chemotherapy and with the presence of BRCA1/2 mutations and all these characteristics would significantly change the course of the disease in this group of patients.

Preeclampsia still remains one of the most severe pregnancy complications and is an actual problem in the obstetrics practice. At present, the joint impact of cytokines and other placenta secreted factors on trophoblast cell functional activity during preeclampsia complicated pregnancy remains unclear. The aim of the study is to estimate the surface receptors expression by trophoblast cells in the presence of placenta secreted factors during physiological pregnancy and at preeclampsia. Trophoblast cells of the JEG-3 line were incubated in the presence of supernatants obtained by cultivation of placentas from women with physiological pregnancy and with preeclampsia. Surface receptors expression by trophoblast cells was estimated by FACS Canto II flow cytometer. It was established that in the third trimester both under normal and pathological conditions, the placenta secreted factors impact on the cytokine receptor expression by trophoblast differs while the trophoblast response capacity to the migration and proliferation stimulating and inhibiting signals remains stable. JEG-3 line cells enhanced the expression of CD186, CD140a, Integrin β6, VE-cadherin, CD29, and CD140a in the case of incubation in the presence of placenta supernatants from the third-trimester pregnancy complicated with preeclampsia compared to incubation in the presence of placenta supernatants form the third trimester of physiological pregnancy.

Aims The aim hereof is to study the cardiovascular system disorders in the early post-castration period in patients with locally advanced cervical cancer and a possibility of correction of such disorders with the low-dose xenon therapy. Materials and methods The study involved 28 female patients of reproductive age (49.8 ± 0.5 years) diagnosed with locally advanced cervical cancer. The main test group consisted of 16 patients, who received the xenon therapy in addition to the main treatment in the early post-castration period. The reference group included 12 female patients without xenon therapy. The cardiovascular system functional state was evaluated with cardiac analyzer Cardiocode at three stages: before surgical intervention, on postsurgery day 3 and upon the completion of the therapy. Results and conclusions Early manifestations of the post-ovariectomy syndrome in the postsurgery period in female patients with locally advanced cervical cancer were accompanied by a disorder in the cardiovascular bio-adaptive and metabolic regulatory mechanisms of the uncompensated distress type, an excess of the phosphocreatine concentration and a decrease in the oxygen level in the myocardium. Upon the completion of the therapy in the reference group patients, changes in the coronary vessels hemodynamics, the dominance of the anaerobic energy exchange in the myocardium, pronounced neurovegetative and psychoemotional disorders have been revealed. The use of the low-dose xenon therapy in the main test group patients has contributed to normalization of myocardial metabolism, recovery of the aerobic energy exchange, and neutralization of neurovegetative and psychoemotional manifestations of the post-ovariectomy syndrome. Thus, the diagnostics of cardiovascular disorders in the early postsurgery period as well as the correction of the disorders and abnormalities with the low-dose xenon therapy are of great importance in the prevention of cardiovascular diseases in oncogynecologic patients with the post-ovariectomy syndrome.

In recent years, the main scientific directions concerning ovarian cancer have been associated with the attempt to individualize the treatment based on prognostic and predictive molecular biological markers. The damage in BRCA1 and BRCA2 genes is identified in 10-15% of ovarian cancer cases. The main function of BRCA proteins is responsibility for repairing double-strand break in DNA by a mechanism of homologous recombination. The result of BRCA gene dysfunction is BRCA-associated DNA damage repair mechanism. As a result, these tumors can be extremely sensitive to DNA-damaging cytostatic agents - such as platinum-based drugs. Several studies show the better survival rates of patients with BRCA mutations in comparison with sporadic ovarian cancer. The information concerning the course of disease and treatment characteristics of patients with ovarian cancer associated with BRCA1, 2 mutations in the Russian population of patients is extremely limited.

e22097 Background: Chronic neurogenic pain (CNP) as a comorbid disease is quite frequent, but its effect on malignant diseases is poorly studied. The risk of cutaneous melanoma in women is higher, but the disease course in men is more severe. The purpose of the study was to reveal the effect of CNP on the levels of sex hormones in the blood of patients with melanoma depending on their gender. Methods: Blood levels of estradiol (E2), estrone (E1), progesterone (P4), testosterone (T) and prolactin (PRL) were measured by ELISA in patients with Т3-4NxM0 melanoma (M): 21 women with CNP (pelvic pain - 7, osteochondrosis – 14), mean age 67.2±2.7 years; 17 men with CNP (osteochondrosis), mean age 65.6±3.1 years. The control group included patients with melanoma similar in age, gender and disease stages without CNP. Results: In women with M+CNP, E2 and P4 were decreased by 1.8 times, while E1 was 1.4 times, T – 2 times, PRL – 1.5 times higher, compared to controls. In men with M+CNP, blood levels of estrogens were unchanged, PRL was 2.5 times lower and P4 and T – 1.3 times higher, compared to the corresponding control levels. Conclusions: CNP upregulated blood levels of androgens in both male and female melanoma patients, and caused the inversion of estrogens only in women with the prevalence of E1 over E2. Gender differences in the CNP influence included the elevation of progesterone and a decrease of prolactin in the blood of men, and P4 deficiency with increased prolactin in women with M+CNP. The specific effect of CNP on the hormonal profile in patients with cutaneous melanoma should be considered in choosing antitumor treatment.

e22082 Background: Clusters of tumor cells in blood vessels secrete VEGF family factors necessary for the formation of premetastatic niches. TGF-β activates the epidermal-mesenchymal transition (EMT). The purpose of the study was to analyze levels of angiogenesis and EMT factors in the blood of patients with cutaneous melanoma (M) and chronic neurogenic pain (CNP). Methods: Blood levels of VEGF-A, VEGF-C, VEGFR-1, VEGFR-3, TGF-β1 and TGF-βR2 were measured by ELISA in patients with Т3-4NxM0 melanoma: 21 women with CNP (pelvic pain - 7, osteochondrosis – 14), mean age 67.2±2.7 years; 17 men with CNP (osteochondrosis), mean age 65.6±3.1 years. The control group included patients with melanoma similar in age, gender and disease stages without CNP. Results: VEGF-A in women and men with M+CNP was higher than in controls by 2.7 and 24.9 times respectively; VEGFR-1 was decreased in women by 1.8 times and increased in men by 1.8 times. VEGF-С was unchanged in women and 1.5 times higher in men, and VEGFR-3 was increased by 2.2 times in women and unchanged in men. TGF-β1 was elevated in women and men with M+CNP by 1.4 and 1.8 times, compared to controls, TGF-βR2 – by 1.9 and 2 times respectively. Conclusions: In the blood of women with M+CNP, CNP activates the blood vascular endothelial growth factor VEGF-A and the factor of epidermal-mesenchymal transition TGF-β, while in the blood of men with M+CNP it activates the blood vascular endothelial growth factor VEGF-A, the lymphatic vascular endothelial growth factor VEGF-C and the factor of epidermal-mesenchymal transition TGF-β. VEGF mediates vascular permeability and is associated with the mobilization of endothelial progenitor cells from the bone marrow into premetastatic niches. Activation of TGF-β signaling promotes the induction of the epidermal-mesenchymal transition and supports the properties of cancer stem cells.

OBJECTIVE To investigate the variations in 11C-methionine uptake in the intact brain tissue and in glial brain tumors of different types. MATERIAL AND METHODS Forty patients (21 men, 19 women) with gliomas, Grade I-IV, underwent 11C-methionine PET-CT and contrast-enhanced MRI. Standardized uptake value (SUV), tumor-to-normal (T/N) ratios and tumor volume were analyzed. RESULTS The high inter-subject variability was detected in the intact brain tissue (SUV in the frontal lobe (FL) varies from 0.47 to 1.73). Amino acid metabolism was more active in women than in men (FL SUV 1.32±0.22 and 1.05±0.24, respectively). T/N ratio better differentiates gliomas by the degree of anaplasia compared to SUV. e21035 Background: Lack of prognostic markers of melanoma to assess its progression, metastasis (MTS), and treatment effectiveness, determines the feasibility of studying biophysical aspects of the transmission of events from cellular to extracellular environment. Receipt and circulation of pathological proteins of melanoma (M) in the blood carries important information, and its trap is the phase transition of biofluids into a solid state. The purpose of the study was to determine markers of melanoma at the supramolecular level in solid-state samples of the blood serum (BS). Methods: 240 samples of BS obtained prior to the surgery from 60 patients with M were studied. The preparations were prepared by the Shatokhina-Shabalin method by creating a closed "analytic cell": dehydration of a BS drop (10 mcL) under a cover glass at 20–240С, humidity 65–70%, for 72 hours. The folding of M proteins was assessed by a polarized light microscopy (Leica DM LS2 microscope, x400-640) using photo and video control and the Morphotest program. Results: The initial stages of M development were characterized by the early appearance of single combined melanoform crystals — anisotropic microspherolites (Mi) embedded in the center of a large anisomorphone (lAM). As M progressed, Mi aggregation with basic lAMs was enhanced. Anisotropy of Mi increased, revealing clear sectors filled with melanin. In the later stages of M growth, melanoform aggregations dominated with point, filamentous, and chained melanin structures. The manifestation of MTS was accompanied by the destruction of the aggregations and the filling of the analytic cell space with numerous Mi. Conclusions: Correspondence of crystallogenesis of M melanomorphic markers in BS in tumor progression allows using the studied supramolecular factors to predict the development of melanoma.

e21036 Background: The high metastatic potential of melanoma and the need for long-term patient monitoring causes the search for tumor markers of this malignant neoplasm. Our aim was a comparative analysis of levels of tumor-specific proteins CD44 and S100 and protein composition in melanocytic lesions of the skin. Methods: We studied 86 samples of cutaneous melanoma and nevus tissues, their perifocal tissues and resection line tissues obtained during tumor excision from 23 patients with cutaneous melanoma pT1-4N0-1M0 and 14 patients with nevi. Intact skin samples obtained from non-cancer patients during reconstructive plastic surgery were used as the comparison group. All patients gave their written informed consent. Levels of CD44 (BenderMedSystems, USA) and S100 (Fujirebio, Sweden) were determined by ELISA in 10% homogenates of all tissues; total protein levels were determined by standard spectrometry and fractional composition of proteins were studied by turbidimetric method. Statistical processing of results was performed using the Statistika 6.0 program with Student’s t-test for two independent groups. Results: Melanoma was characterized by a sharp increase in S100B levels, 28 and 7 times exceeding the levels in intact tissues and nevi. The level of CD44 in melanoma tissue was increased only by 2 times, in nevus tissue - by 48%. The ratio of albumin and gamma globulins in the tissue of melanoma and nevi was 79% and 29% lower than in healthy skin. A more than twofold increase in the gamma globulin fraction in the melanoma tumor tissue against a decrease in albumin and the absence of changes in other globulins, as well as a moderate but statistically significant increase in the gamma globulin fraction in nevi tissue, indicates that S100B and CD44 proteins belong to the gamma globulin fraction. Conclusions: A highly specific increase of S100 levels and a less specific increase of CD44 levels in supernatant liquid of melanoma tissue homogenates, together with the predominance of the gamma globulin fraction, allow considering such factors as a prognostically unfavorable sign of tumor progression, which can be important when choosing a personalized treatment strategy.

OBJECTIVE To study the effect of metabolic characteristics of the tumor determined by 99mTc-MIBI single-photon emission computed tomography (SPECT) and various molecular genetic features on the outcomes of combination treatment of hemispheric glioblastomas. MATERIAL AND METHODS This single-center prospective cohort study involved 68 patients aged 25-78 years (38 males and 30 females) with primary glioblastomas. Hypermetylation of the promotor region of the MGMT gene was observed in 24 (42%) out of 57 patients. The IDH1 mutation was revealed in two (3.5%) patients. The catamnestic data were available for 66 out of 68 patients. The first SPECT/CT study was carried out before chemoradiation therapy; the second SPECT/CT study was performed after the chemoradiation therapy. In each study, quantitative measures were calculated for the early (15-30 min after the patient had received a radiopharmaceutical) and late (after 45-60 min) phases. RESULTS The actuarial survival rates after 12 and 24 months were 69.6 and 29.1%, respectively. The median overall survival rate was 17.5 months (95% CI 12.9-20.3). Non-compaction of the ventricular myocardium (NCM) is a genetic cardiomyopathy usually due to mutationof the G4.5 gene located in the Xq28 chromosomal region. This congenital disorder is characterized by pronounced trabeculations and intertrabecular recesses resulting from abnormal embryogenesis between the fifth and eighth fetal weeks. The reported prevalence in the general population is between 0.014% and 1.3%. The classic triad of complications includes heart failure, ventricular arrhythmias and systemic embolic events, although some patients have an asymptomatic form. NCM is commonly diagnosed by echocardiography, but contrast ventriculography, CT and MRI can also be used. Here we present a case of left ventricle NCM, manifested after respiratory infection, in a pregnant patient with congenital thrombophilia and a history of myocardial infarction. LEARNING POINTS Non-compaction myocardium (NCM) in pregnant women has been associated with a poor prognosis. We should avoid routinely recommending young women with NCM to refuse pregnancy. A decision to continue pregnancy should be made by the patient in discussion with specialists.

HMG-CoA inhibitors have been widely used in primary and secondary prevention of atherosclerosis for more than 30 years. The evidence base for statins includes dozens of randomized clinical trials (RCT), involving hundreds of thousands patients. According to the results of the latest meta-analyses, reducing low-density lipoprotein cholesterol (LDL-CH) level by 1 mmol/l in statin-treated patients could decrease cardiovascular risk by 0,8 %. Atorvastatin (Liprimar®) is a modern synthetic statin, which has been thoroughly studied in several RCTs over the last 15 years. These trials demonstrated its effectiveness and tolerability in patients with chronic coronary heart disease, acute coronary syndrome (ACS), Type 2 diabetes mellitus, and arterial hypertension. In the studies comparing atorvastatin (10-80 mg/d) to other statins, baseline LDL-CH levels were reduced by 53 % in atorvastatintreated patients. Atorvastatin therapy was also well tolerated. Compared to placebo, atorvastatin therapy in the dose of 10 and 80 mg/d was associated with the incidence of hepatic transaminase elevation of 0,1 % and 0,6 %, respectively. This evidence base (in particular, the results of the trials on 80 mg/d atorvastatin therapy in ACS patients) has been a cornerstone of modern clinical guidelines, recommending target LDL-CH levels of 2 mmol/l. Currently, atorvastatin is the most widely prescribed statin in the majority of both developed and developing countries. Increasing initial dose and prescribing high-dose atorvastatin therapy (40-80 mg/d) could facilitate an improvement in treatment quality and a reduction in high levels of cardiovascular mortality inRussia.

The evidences for statin use in cardiology are presented. Especially hypolipidemic efficacy of atorvastatin is demonstrated in according to comparative studies. Results of clinical studies on atorvastatin efficacy and safety in different groups of patients are also presented.

The theoretical approach based on photonic gap-map (GM) presentation is elaborated for the design of one-dimensional (1D) photonic crystal structures based on Si with (i) omnidirectional reflection and (ii) tunable defect modes. The features of the omnidirectional band gap formation for photonic crystals of medium optical contrast and low number of periods have been explained. The formation of a 1D photonic structure of limited number of periods with omnidirectional band gap close to that for an infinite periodic structure has been discussed. The possibility for tuning the resonance peak position of a photonic crystal with a microcavity depending on its refractive index and thickness has been demonstrated for light at different incident angles and polarizations.

Safety and efficiency of intracoronary transventricular transplantation of autologous mononuclear bone marrow cells in rats with postinfarction cardiosclerosis were studied. The cells migrated to the damaged area and were detected only in the cicatricial tissue; they have fibroblast-like phenotype and some of them were stained with Fapα (marker of reactive fibroblasts). More active proliferation of non-muscular cells and formation and maturation of collagen fibers in the cicatricial tissue were observed after transplantation of mononuclear cells. This led to thickening of the cicatricial wall, but the size of the scar and index of dilatation of the left ventricle remained unchanged. The number and volume density of newly formed blood vessels in the damaged area increased after transplantation, but no labeled cells were seen in the vascular walls. It can be hypothesized that stimulation of neoangiogenesis is mediated by paracrine mechanisms, which also explains improvement of global contractility of the left ventricle (increased contractility index in functional tests). Thus, transplantation of mononuclear bone marrow cells leads to thickening and strengthening of the cicatricial wall, stimulates angiogenesis, and improves global myocardial contractility. However, no morphological signs of reverse remodeling of the left-ventricular myocardium were revealed.

We report here on optical and magneto-optical spectra investigations of nanostructured Co and Ni films deposited on top of polymethyl methacrylate (PMMA) colloidal crystal. Resonant peculiarities have been observed in both spectra; their positions are scaled with the PMMA sphere diameter and depend on incident angle. Asymmetry of the resonance lineshapes as well as change of the direction of magneto-optical rotation has been observed. Both the surface plasmon resonance and the interference between reflections from the colloidal crystal and from the nanostructured film should be considered to explain the obtained results.
