Extremely noisy low light enhancement suffers from high-level noise, loss of texture detail, and color degradation. When recovering color or illumination for images taken in a dark environment, the challenge for networks is how to balance the enhancement for noise and texture details for a good visual effect. A single network is not suitable for solving the ill-posed problem of mapping the input image's noise to the clear target in the ground truth. To solve the problems, we pro-pose perceptual-details GAN (PD-GAN) utilizing Zero-DCE to initially recover illumination and combine residual dense-block Encoder-Decoder structure to suppress noise while finely adjusting the illumination. Besides, fractional differential gradient masks are integrated into the discriminator to enhance details. Experiment results demonstrate that PD-GAN outperforms other methods on the extremely low-light image dataset.

ObjectiveThis study assessed the association between body composition and prognosis of patients with upper urinary tract urothelial carcinoma (UTUC) patients treated by radical nephroureterectomy.MethodsWe retrospectively collected baseline data on age, sex, body mass index (BMI), hypertension, diabetes, and tumor-related factors. Computed tomography (CT) scans were performed to measure body composition parameters such as muscle attenuation (MA), total abdominal muscle area (TAMA), visceral fat area (VFA), intermuscular fat area (IMF), and lateral/posterior perirenal fat thickness (L/P PNF), visceral fat density (VD), and subcutaneous fat density (SD). Patient follow-up was conducted via telephone or in the clinic. The endpoints of follow-up were all-cause death, local progression or distant metastasis. Survival analysis was analyzed using the Kaplan-Meier method, and risk factors associated with prognosis were identified using univariate and multivariate Cox proportional hazard analyses.ResultsAmong the 273 UTUC patients (median age, 68 years) enrolled in our study, 102 had a BMI > 24.0, 100 suffered from diabetes, and 120 had hypertension. A large proportion of patients (189) had high grade tumors. Across all patients, 1- and 3-year rates for overall survival were 86.45% and 75.55%; local progression-free survival, 92.11% and 89.67%; and distant metastasis-free survival, 85.23% and 80.17%. Based on the Cox regression analysis, MA, IMF, TAMA, TPA, TPT, APT, SMI and PMI significantly reduced the risk of local progression (p < 0.05), while PPNF = 1 point reduced the risk of distant metastasis (p < 0.05). Overall survival was significantly associated with MA, TAMA, and SMI (p < 0.05).ConclusionOur findings illustrate that body composition parameters can act as independent predictors of prognosis in UTUC patients who underwent RNU. These results can help improve stratification of patients and optimize postoperative treatment.

Background The role of a tourniquet is still controversial for patients undergoing total knee arthroplasty (TKA). Our current study was performed to determine whether the nonuse of the tourniquet combine with tranexamic acid (TXA) application in TKA patients with end-stage osteoarthritis would accelerate the perioperative recovery rate and provide enough cement mantle thickness for implant fixation. Methods In this prospective, randomized controlled trial, 150 end-stage knee osteoarthritis patients receiving TKA were divided into three groups: group A (tourniquet group), group B (non-tourniquet group), and group C (tourniquet in cementation group). All enrolled patients received 3 g of intravenous TXA and 1 g topical TXA. The primary outcomes included blood loss variables and transfusion values. The secondary outcomes included VAS pain score, inflammatory factors level, range of motion, HSS score, postoperative hospital stay, and complication. Furthermore, by using a digital linear tomosynthesis technique, tibial baseplate bone cement mantle thickness was measured in four zones based on the knee society scoring system. Results No significant difference was found among the three groups with regards to total blood loss, transfusion, and complication. However, patients in group B showed lower inflammatory factors levels, shorter length of hospital stay, better range of motion, and lower postoperative pain. No significant difference was found among the three groups in four zones in terms of bone cement mantle thickness. Conclusions For end-stage knee osteoarthritis patients, the absence of tourniquet did not appear to affect blood loss and cement penetration in TKA patients. Furthermore, less inflammation reaction and better knee function can be achieved without a tourniquet. We recommend no longer use a tourniquet in primary TKA for patients with end-stage osteoarthritis when TXA is administrated. Trial registration ChiCTR-INR-16009026. Long-term tracking algorithms need to track targets stably in long videos. Compared with short-term tracking, long-term tracking faces more complex challenges. Such as the background brightness distributed in the entire gray level, wide range of target scale changes, more frequent motion blur and occlusion and other challenges. Existing long-term tracking algorithms cannot solve these challenges well, and easily cause tracking box drift, which significantly decrease the accuracy and robustness of the algorithms. In this paper, we design a detection and tracking combined network that can implement a global instance search for the tracked target. We integrate deformable convolution into the siamese network to solve the target deformation problem during long-term tracking. Besides, guided anchor is adopted in RPN to generate more sparse and accurate proposals, which can decrease the interference of the background. We design the cascaded RCNN with template information to filter out proposals and refine coordinates. And finally, select the highest confidence proposal as the final tracking box. Experiments show that our method has a stronger discriminative ability and can get more accurate tracking boxes compared with other advanced trackers. On several long-term tracking benchmarks, our method has achieved excellent performance.

Chronic pain and sleep disturbance are highly comorbid disorders, which leads to barriers to treatment and significant healthcare costs. Understanding the underlying genetic and neural mechanisms of the interplay between sleep disturbance and chronic pain is likely to lead to better treatment. In this study, we combined 1206 participants with phenotype data, resting-state functional magnetic resonance imaging (rfMRI) data and genotype data from the Human Connectome Project and two large sample size genome-wide association studies (GWASs) summary data from published studies to identify the genetic and neural bases for the association between pain and sleep disturbance. Pittsburgh sleep quality index (PSQI) score was used for sleep disturbance, pain intensity was measured by Pain Intensity Survey. The result showed chronic pain was significantly correlated with sleep disturbance (r = 0.171, p-value rg = 0.598 using linkage disequilibrium (LD) score regression analysis. Polygenic score (PGS) association analysis showed PGS of chronic pain was significantly associated with sleep and vice versa. Nine shared functional connectivity (FCs) were identified involving prefrontal cortex, temporal cortex, precentral/postcentral cortex, anterior cingulate cortex, fusiform gyrus and hippocampus. All these FCs mediated the effect of sleep disturbance on pain and seven FCs mediated the effect of pain on sleep disturbance. The chronic pain PGS was positively associated with the FC between middle temporal gyrus and hippocampus, which further mediated the effect of chronic pain PGS on PSQI score. Mendelian randomization analysis implied a possible causal relationship from chronic pain to sleep disturbance was stronger than that of sleep disturbance to chronic pain. The results provided genetic and neural evidence for the association between pain and sleep disturbance, which may inform future treatment approaches for comorbid chronic pain states and sleep disturbance.

Background: Novel coronavirus disease 2019 (COVID-19) has spread around the world; therefore, more attention should be paid to the clinical features of COVID-19, with the aim of improving the diagnosis and treatment of patients. Methods: By 15 February 2020, 60 patients diagnosed with COVID-19 had been admitted to Nanjing Second Hospital. We analyzed the clinical features of different age segments infected with COVID-19 prospectively, including epidemiological, clinical, laboratory, and radiological characteristics; treatment, clinical outcomes, and prognosis of this cohort of patients. Results: The cohort comprised 29 male and 31 female patients (median age = 46.18 years old (range: 18–97). Fifty-five (91.7%) patients had a clear epidemiological contact history. The average incubation period was 7.92 days. The most common clinical manifestations were fever (85%) and cough (75%). Peripheral white blood cell counts were mostly normal at admission, 7 days, and 14 days, with no differences among patients of different ages. The lymphocyte counts of all patients were in the normal range on admission, and after 7 days and 14 days of treatment; however, the lymphocyte count in > 65-year-old patients was less than that in the < 40 and 40–65-year-old groups after 7 and 14 days of treatment (P < 0.05, respectively). At admission, the CD4 T lymphocyte count was within the normal range; however, the CD4 T lymphocyte count in >65-year-old group was less than that in the < 40 and 40–65-years-old groups after 14 days of treatment. The CD4 T lymphocyte counts were 723.46 ± 243.82/ml (< 40), 640.00 ± 242.30/ml (40–65), and 399.88 ± 256.16/ml (> 65) (P =0.0075). The > 65-years-old group had higher levels of lactate dehydrogenase (269.83 ± 73.36 vs. 208.52 ± 35.67 and 243.83 ± 76.66) after 14 days (P = 0.0496). Imaging revealed more lesions in the 40–65 and > 65-year-old groups (P < 0.0001). The days after the nucleic acid detection turned negative in the three age groups were: 9.19 ± 3.93 (< 40), 10.04 ± 4.10 (40–65), and 13.57 ± 2.76 (> 65) (P = 0.0373). After antiviral treatment, together with anti-infection regimen if the patient with lung infection and continuous oxygen inhalation if the patient is hypoxic, all patients achieved total recovery and were discharged with follow-up. Conclusion: Patients with COVID-19 pneumonia generally had an epidemiological history. Older patients showed more extensive lesions upon admission, more severe illness, slower recovery of immune function, the longer viral nucleic acid persistence.

BACKGROUND: Comparable global data on health and nutrition of school-aged children and adolescents are scarce. We aimed to estimate age trajectories and time trends in mean height and mean body-mass index (BMI), which measures weight gain beyond what is expected from height gain, for school-aged children and adolescents., METHODS: For this pooled analysis, we used a database of cardiometabolic risk factors collated by the Non-Communicable Disease Risk Factor Collaboration. We applied a Bayesian hierarchical model to estimate trends from 1985 to 2019 in mean height and mean BMI in 1-year age groups for ages 5–19 years. The model allowed for non-linear changes over time in mean height and mean BMI and for non-linear changes with age of children and adolescents, including periods of rapid growth during adolescence., FINDINGS: We pooled data from 2181 population-based studies, with measurements of height and weight in 65 million participants in 200 countries and territories. In 2019, we estimated a difference of 20 cm or higher in mean height of 19-year-old adolescents between countries with the tallest populations (the Netherlands, Montenegro, Estonia, and Bosnia and Herzegovina for boys; and the Netherlands, Montenegro, Denmark, and Iceland for girls) and those with the shortest populations (Timor-Leste, Laos, Solomon Islands, and Papua New Guinea for boys; and Guatemala, Bangladesh, Nepal, and Timor-Leste for girls). In the same year, the difference between the highest mean BMI (in Pacific island countries, Kuwait, Bahrain, The Bahamas, Chile, the USA, and New Zealand for both boys and girls and in South Africa for girls) and lowest mean BMI (in India, Bangladesh, Timor-Leste, Ethiopia, and Chad for boys and girls; and in Japan and Romania for girls) was approximately 9–10 kg/m2. In some countries, children aged 5 years started with healthier height or BMI than the global median and, in some cases, as healthy as the best performing countries, but they became progressively less healthy compared with their comparators as they grew older by not growing as tall (eg, boys in Austria and Barbados, and girls in Belgium and Puerto Rico) or gaining too much weight for their height (eg, girls and boys in Kuwait, Bahrain, Fiji, Jamaica, and Mexico; and girls in South Africa and New Zealand). In other countries, growing children overtook the height of their comparators (eg, Latvia, Czech Republic, Morocco, and Iran) or curbed their weight gain (eg, Italy, France, and Croatia) in late childhood and adolescence. When changes in both height and BMI were considered, girls in South Korea, Vietnam, Saudi Arabia, Turkey, and some central Asian countries (eg, Armenia and Azerbaijan), and boys in central and western Europe (eg, Portugal, Denmark, Poland, and Montenegro) had the healthiest changes in anthropometric status over the past 3·5 decades because, compared with children and adolescents in other countries, they had a much larger gain in height than they did in BMI. The unhealthiest changes—gaining too little height, too much weight for their height compared with children in other countries, or both—occurred in many countries in sub-Saharan Africa, New Zealand, and the USA for boys and girls; in Malaysia and some Pacific island nations for boys; and in Mexico for girls., INTERPRETATION: The height and BMI trajectories over age and time of school-aged children and adolescents are highly variable across countries, which indicates heterogeneous nutritional quality and lifelong health advantages and risks., Wellcome Trust, AstraZeneca Young Health Programme, EU., peer-reviewed

Dual-wavelength light sources with stroboscopic illumination technique were applied in a process of photoelastic modulated ellipsometry to retrieve two-dimensional ellipsometric parameters of thin films on a silicon substrate. Two laser diodes were alternately switched on and modulated by a programmable pulse generator to generate four short pulses at specific temporal phase angles in a modulation cycle, and short pulses were used to freeze the intensity variation of the PEM modulated signal that allows ellipsometric images to be captured by a charge-coupled device. Although the phase retardation of a photoelastic modulator is related to the light wavelength, we employed an equivalent phase retardation technique to avoid any setting from the photoelastic modulator. As a result, the ellipsometric parameters of different wavelengths may be rapidly obtained using this dual-wavelength ellipsometric system every 4 s. Both static and dynamic experiments are demonstrated in this work.

Ga and/or N doped TiO 2 nanoparticles synthesized by one pot hydrothermal method possess pure anatase phase with spherical morphology. Calculations based on density functional theory (DFT) explained the band gap narrowing, origin of optical absorption and the possible changes in the photocatalytic activity of synthesized nano-photocatalysts. Analysis of band structure and density of states revealed that N and Ga, N codoping introduced some partially occupied states above the valance band maximum which might influence the electron hole pair recombination. Calculations as well as experiments confirmed the shifting of absorption edge towards visible regime due to doping. Spectroscopic investigations confirmed the existence of dopant atoms in the doped samples. Ga doped TiO 2 samples provided the best photocatalytic activity compared to the synthesized samples attributed to the suitable band structure, enhanced optical absorption and effective separation between photoexcited carriers.

In this study, a polarization phase shifting multi-loop Sagnac interferometer is created in order to produce two sets of linear orthogonal polarizations, one of which acts as the system’s dynamic reference beam for recording dynamic uncertainty events. The other served as the test beam to determine phase difference as the light passed through a sample. All optical waves traveled through two main compartments that minimized external vibrations, and after accounting for dynamic uncertainty effects, the error of the unwrapped phase was significantly improved. We demonstrated phase distribution both with and without recording the dynamic uncertainty of a flat glass and lenses.

Background: The use of tranexamic acid (TXA) in primary total hip arthroplasty is well documented. However, considering the potential side effects, including deep vein thrombosis and pulmonary embolism, the ideal method of providing TXA to patients undergoing total hip arthroplasty remains controversial. The objective of this trial was to assess the efficacy and safety of intravenous (IV) administration combined with topical administration of TXA regarding postoperative blood loss and transfusion rates in patients treated with primary unilateral total hip arthroplasty. Methods: In this prospective, randomized controlled trial, 150 patients were divided into three groups: the combined group (IV administration of 15 mg/kg of TXA combined with topical administration of 1 g/100 mL of TXA), the single IV group (IV administration of 15 mg/kg of TXA), and the placebo group. The primary outcomes included blood-loss variables (total, intraoperative, and drainage blood loss; changes in hemoglobin, hematocrit, and platelet concentration; and amount of IV transfusion fluid) and transfusion values (frequency of transfusion and number of transfused blood units). The secondary outcomes included the length of the hospital stay, range of hip motion, Harris hip score, and prevalences of deep vein thrombosis and pulmonary embolism. Results: The total blood loss in the combined group (mean and standard deviation, 835.49 ± 343.50 mL) was significantly reduced (p < 0.05) in comparison with that in the single IV group (1002.62 ± 366.85 mL) and placebo group (1221.11 ± 386.25 mL). The combined group also had fewer transfusions in comparison with the single IV and placebo groups (1, 8, and 19, respectively; p < 0.05). There was no difference among the 3 groups with regard to the rates of deep vein thrombosis or pulmonary embolism. Conclusions: Intravenous combined with topical administration of TXA in patients undergoing a primary unilateral total hip arthroplasty significantly reduced postoperative bleeding and the transfusion rate. Studies with more patients and longer follow-up are needed to confirm whether this promising combined strategy is safe with regard to thromboembolic complications. Band structure of anatase [Formula: see text] could be modified by doping foreign atoms. With ab-initio calculations, considering praseodymium, nitrogen and gadolinium as potential dopants, some mono-, co- and tri-doped models are introduced. The structure modification due to each model is compared with a standard [Formula: see text] system. Pr doping reduced the band gap of [Formula: see text] by introducing Pr 4f states below the conduction band, and the Fermi level appears below the conduction band minimum. The addition of nitrogen in Pr-doped [Formula: see text] pushed back the Fermi level to its intrinsic position while reducing the band gap. The tri-doped system comprising of Pr, N and Gd as dopants provided a populated band gap. The impurity states associated with Gd appeared in the middle of the band gap. The density of states analysis reveals that Pr 4f states change the location of N 2p states in the band structure. Instead of appearing as isolated states, the N 2p states are mixed with O 2p states while the Pr 4f states are coupled with the Ti 3d states in Pr, N-[Formula: see text]. The Pr, N, Gd tri-doped [Formula: see text] system provided strong visible light absorption due to the stepwise transition of electrons by Gd f states. While providing reasonable visible light absorption, the Pr, N-[Formula: see text] is expected to provide strong visible light photoactivity due to the clear band gap. Due to the absence of isolated states, the generated photo-excited carriers would be efficiently utilized in the oxidation/reduction processes.

The risk analysis on karst groundwater pollution is a research hotspot in current international hydrogeological field as well as the premise of preventing and controlling groundwater pollution. According to the characteristics of groundwater pollution in the typical study area, the study selected main-control factors of risk evaluation on karst groundwater pollution in mountainous areas at first. Based on this, the research determines the method for quantifying the factors and established a risk evaluation index system for karst groundwater pollution. To overcome drawbacks of the method for determining weights of factors in traditional evaluation method, the study determines the structure of the artificial neural network model by combining the selected evaluation factors. And also, the weight coefficients of evaluation factors on each layer are calculated. On this basis, the model for evaluating the risk of karst groundwater pollution is established. Moreover, the risk zoning evaluation map of groundwater pollution in the typical study area is prepared after conducting the weighted stacking of various sub-layers using the geographic information system. The method applied in the study can comprehensively and objectively reflect that the groundwater pollution is controlled by multiple factors and reveal the nonlinear characteristic of the pollution process. Additionally, the evaluation result is institutive and visible, which can provide a certain basis and reference for relevant researches.

In this paper, a new fast block-matching algorithm for video coding is presented. In the algorithm, the partial distance search (PDS) technique is performed in the wavelet domain to eliminate the undesired blocks. Since the energy of blocks is concentrated in few coefficients in the wavelet domain, by starting the PDS at these coefficients, the algorithm can significantly reduce computational time for block matching. The PDS can be performed over the subspace in the wavelet domain to further lower the computational time at the expense of possible slight degradation in performance. In addition, the simple Haar wavelet can be used to remove the need for multiplication for blocking matching. Simulation results show that our algorithm is very effective for video coding where real-time processing is required.

Educational Objectives As a result of reading this article, physicians should be able to: 1. Explain why periprosthetic infection is the most devastating complication of primary total joint arthroplasty. 2. Recognize the available strategies for using antibiotic-impregnated bone cement in patients undergoing primary total joint arthroplasty. 3. Discuss the potential strengths and limitations of using antibiotic-impregnated bone cement in primary total joint arthroplasty. 4. Recall the available evidence regarding the use of antibiotic-impregnated bone cement in primary total joint arthroplasty. There has been much debate and controversy about the routine use of antibiotic-impregnated bone cement in primary total joint arthroplasty. The purpose of this study was to undertake a meta-analysis to determine whether the use of antibiotic-impregnated bone cement would reduce the incidence of infection after primary total joint arthroplasty. Of 313 citations identified for screening, 6 trials reporting 26,791 patients were eligible for data extraction and meta-analysis. The authors found no statistically significant difference between antibiotic-impregnated bone cement and plain bone cement in terms of the incidence of infection. The results indicated that the use of antibiotic-impregnated bone cement in primary total joint arthroplasty did not lead to a decrease in the rate of infection. [ Orthopedics. 2014; 37(12):839–845.]

We present a phase-shifting interferometric system that uses the photoelastic modulator (PEM) with the stroboscopic illumination technique. The laser diode is controlled by a programmable pulse generator for triggering four short pulses at their specific temporal phase angles of a modulation cycle for phase-shifting operation. Thereby, the intensity variation of the PEM modulated signal can be freezed and the interferograms captured by a charge-coupled device. In this configuration, no mechanical operations are required in phase-shifting by using the stroboscopic illumination technique, and fast interference fringe measurements are possible. We performed experiments to demonstrate the phase map of two static samples and confirmed that the method is applicable to imaging the complex phase distributions of dynamic objects at speed of 2 Hz. The thickness deviation is almost less than 5% for an ultra-thin transparent film.

Regarding the diversity and complexity of the road conditions on the soft sediment seabed, in order to improve the driving control precision of the caterpillar nodule collector, this study, focusing on the noise disturbance of the nodule collector body, adopts the methods of wavelet packet decomposition algorithm and Hilbert-Huang transformation algorithm to reconstruct the nodule collector body vibration signals, which are targeted by Hilber- Huang transformation algorithm to reach IMF fraction in the process of Empirical Mode Decomposition. Therefore, through the Hilbert spectrum analysis of IMF component, IMF component power characteristics are achieved, the available IMF component is optioned to reconstruct signals and the disturbance of the noise is eliminated. Comparing to the approach of wavelet decomposition, Hilbert-Huang transformation's analysis and algorithm of the collector body's vibration signals' reconstruction are more accurate, providing valid control parameter to control the drive of caterpillar nodule collector on the soft quality bottom of the sea more precisely.

With the advantages of fluorescence excitation and environmental adaptability simultaneously, Ultraviolet Image Spectroscopy has shown irreplaceable features in the field of latent target detection and become a current research focus. A design of Large Aperture Ultraviolet Spatially Modulated Imaging Spectrometer (LAUV-SMIS) based on image plane interferometer and offner system was first proposed in this paper. The data processing technology of time-spatial modulation FTIS in UV band has been studied. The latent fingerprint could be recognized clearly from the image since which is capable to meet the need of latent target detection. The spectral curve of the target could distinguish the emission peak at 253.7nm and 365nm when the low pressure and high pressure mercury lamp were used as the illuminator. Accurate spectral data of the target can be collected on the short and long wave ends of the working band.

Dual-wavelength light sources with stroboscopic illumination technique were applied in photoelastic modulated ellipsometry to retrieve two-dimensional ellipsometric parameters of thin films on a silicon substrate. Two laser diodes are alternatively switched on and modulated by a programmable pulse generator for triggering four short pulses at their specific temporal phase angle of a modulation cycle, and short pulses are used to freeze the intensity variation of the PEM modulated signal that images can be captured by a charge-coupled device. Although the phase retardation of a photoelastic modulator is related to the light wavelength, we employed an equivalent temporal phase shift technique to avoid any setting from the photoelastic modulator. As a result, ellipsometric parameters of different wavelengths can be rapidly obtained from this dual-wavelength ellipsometric system every 10s. Both static and dynamitic experiments were demonstrated in this work.

Open Access article distributed under the terms of CC BY., Background: Underweight and severe and morbid obesity are associated with highly elevated risks of adverse health outcomes. We estimated trends in mean body-mass index (BMI), which characterises its population distribution, and in the prevalences of a complete set of BMI categories for adults in all countries. Methods: We analysed, with use of a consistent protocol, population-based studies that had measured height and weight in adults aged 18 years and older. We applied a Bayesian hierarchical model to these data to estimate trends from 1975 to 2014 in mean BMI and in the prevalences of BMI categories (, Wellcome Trust, Grand Challenges Canada

Objectives: To develop a new classification method to describe incidence and prevalence of function and disability and their predictors in Chinese older adults. Methods: Data were obtained from 16,020 older adults in the 2002 wave of Chinese Longitudinal Healthy Longevity Survey (CLHLS). The subtypes of function and disability were defined by combining physical performance (PP) tests and self-reported assessments of activities of daily living (ADL). Results: The most prevalent subtype involved PP limitations without ADL dependency. Rural living made it more likely that an individual with physical performance limitations would be independent in ADL, whereas city residence made dependence in ADL more likely, regardless of whether performance limitations were present. Discussion: Self-reported and performance-based tools can be used in combination to estimate conceptually different subtypes of function and disability, with different epidemiological rates of incidence and different social, medical, and environmental predictors.

OBJECTIVES: To examine a new method of classifying disability subtypes by combining self-reported and performance-based tools to predict mortality in older Chinese adults. DESIGN: Prospective cohort study. SETTING: Community-dwelling older adults. PARTICIPANTS: Sixteen thousand twenty Chinese adults aged 65 and older from the Chinese Longitudinal Healthy Longevity Survey (CLHLS). MEASUREMENTS: Self-reported activities of daily living (ADLs) and physical performance (PP) tests (chair standing, lifting a book from floor, turning 360°) cross-classified to create mutually exclusive disability subtypes: subtype 0 (no limitations in PP or ADLs), subtype 1 (limitations in PP, no limitations in ADLs), subtype 2 (no limitations in PP, limitations in ADLs), and subtype 3 (limitations in PP and ADLs). Outcome was mortality over 3 years. RESULTS: Cox proportional hazard models, controlling for sociodemographic variables, living situation, healthcare access, social support, health status, and life-style, showed that older adults without any limitations in ADLs or PP had significantly lower mortality risk than those with other disability subtypes and that there was a graded pattern of greater mortality according to subtype 1 (hazard ratio (HR)=1.31, 95% confidence interval (CI)=1.20–1.42), 2 (HR=1.39, 95% CI=1.23–1.59), and 3 (HR=1.88, 95% CI=1.72–2.05). When compared with the average survival curve in the cohort, subtypes of isolated performance deficits or self-reported disability did not substantially discriminate risks of death over 3 years. CONCLUSION: Combined use of self-reported and PP tools is necessary when screening for mutually exclusive disability subtypes that confer significantly higher or lower mortality risks on a population of older adults.

The purpose of the study was to investigate the association between birth weight and number of metabolic syndrome (MetS) components in an urban Chinese cohort. Individuals (N = 2019) who were born between 1921 and 1954 at the Peking Union Medical College Hospital and who had detailed obstetric records volunteered to take part and were examined by medical personnel in a clinical setting between May 2003 and April 2005. Data of birth outcome and results on clinic examination in adulthood were analyzed using analysis of variance and multivariate ordinal regression to estimate the association between birth weight and MetS. Metabolic syndrome was defined as per the National Cholesterol Education Program Adult Treatment Panel III. The prevalence of MetS was 26.74%, whereas 55.43% of the subjects had at least 2 components of MetS. Subjects who presented with all 5 components of MetS exhibited a significantly lower birth weight and higher age, body mass index, and waist circumference at follow-up. Multivariate ordinal regression analysis revealed that, as compared with those with birth weights of 3000 to 3500 g, subjects who had birth weights of less than 2500 g were 66% more likely to develop a greater number of MetS components in adulthood (95% confidence interval, 1.18-2.34; P = .004), whereas those with birth weights between 2500 and 3000 g were 33% more likely to develop a greater number of MetS components as adults (95% confidence interval, 1.09-1.63; P = .005). The present study demonstrated the relationship between low birth weight and increased presentation of MetS components in Chinese adults.

Chaos is a seemingly random and irregular movement, happening in a deterministic system without random factors. Chaotic theory has promising applications in various areas (e.g., communication, image encryption, geophysics, weak signal detection). However, observed chaotic signals are often contaminated by noise. The presence of noise hinders the chaos theory from being applied to related fields. Therefore, it is important to develop a new method of suppressing the noise of the chaotic signals. Recently, the denoising algorithm for chaotic signals based on collaborative filtering was proposed. Its denoising performance is better than those of the existing denoising algorithms for chaotic signals. The denoising algorithm for chaotic signals based on collaborative filtering makes full use of the self-similar structural feature of chaotic signals. However, in the parameter optimization issue of the denoising algorithm, the selection of the filter parameters is affected by signal characteristic, sampling frequency and noise level. In order to improve the adaptivity of the denoising algorithm, a criterion for selecting the optimal filter parameters is proposed based on permutation entropy in this paper. The permutation entropy can effectively measure the complexity of time series. It has been widely applied to physical, medical, engineering, and economic sciences. According to the difference among the permutation entropies of chaotic signals at different noise levels, first, different filter parameters are used for denoising noisy chaotic signals. Then, the permutation entropy of the reconstructed chaotic signal corresponding to each of filter parameters is computed. Finally, the permutation entropies of the reconstructed chaotic signals are compared with each other, and the filter parameter corresponding to the minimum permutation entropy is selected as an optimal filter parameter. The selections of the filter parameters are analyzed in the cases of different signal characteristics, different sampling frequencies and different noise levels. Simulation results show that this criterion can automatically optimize the filter parameter efficiently in different conditions, which improves the adaptivity of the denoising algorithm for chaotic signals based on collaborative filtering.

Objectives. This study investigates the factors associated with frailty and the association of frailty with mortality in a national sample of adults aged 65 ‐ 109 in China. Methods. Using the 2002 wave of the Chinese Longitudinal Healthy Longevity Survey, we construct a frailty index (FI) based on 39 measures available in the data set. We use ordinal logistic regressions to examine the factors associated with the FI and use Weibull hazard regression to examine the association between frailty and 3-year mortality from 2002 to 2005. Results. Age, sex, ethnicity, urban ‐ rural residence, economic condition, religious involvement, and daily exercise are signifi cantly associated with levels of frailty. Hazard analyses further reveal that the FI is a robust predictor of mortality at advanced ages and that the relationship between frailty and mortality is independent of various covariates. Discussion. The measurement and analysis of frailty have broad implications for public health initiatives designed to target individuals with the diminished capacity to effectively compensate for external stressors and to prevent further declines associated with aging and mortality. A key to healthy longevity is the prevention, postponement, and potential recovery from physical and cognitive defi cits at advanced ages through enhanced medical interventions and treatments.

This paper introduces application and hydraulic system's working principle of the Multi-DOF hydraulic arm, adopts the power bond graph to establish the mathematical model of the hydraulic system, and uses the Matlab software to draw the Bode diagram of the open-loop system. The stability and other time domain performance of the hydraulic system are analyzed by using the engineering control theory, the results show that, on the premise of guaranteeing the system stability, the system could obtain good time domain performance, through increasing the open-loop gain coefficient properly. The results of simulation are useful for the design and dynamic analysis of the hydraulic system.

MarkII method is a function point method. It is primarily based on Reporting Requirements, but the MarkII method exists measuring problems: less data and poor accuracy. After analyzing the MarkII method and semantics of the main components of UML model, a method which applies MarkII method to measure the scale of the object-oriented system is proposed by establishing correspondence relationship and mapping rules between them. To verify the feasibility of this method, we design and develop a measure tool. Experimental results show that the proposed method can effectively measure the scale of the object-oriented system, and achieve significantly improved accuracy and automation.

By pooling the data from the three waves (1998, 2000, and 2002) of the Chinese Longitudinal Health and Longevity Survey, this study examines the association of sociodemographic factors with the onset and recovery of ADL disability including changes in functional status before dying. The results show that the sociodemographic factors play some specific roles in disability dynamics at very high ages even after controlling for a rich set of confounders. Our results also point out that the conventional method, which excludes the information of ADL changes before dying due to unavailability of the data, overestimates the effects of age, gender, ethnicity, and living alone on disability transitions whereas it underestimates the effects of SES, although such discrepancies are not very big compared with the results including information of ADL changes before dying.

Statistical analysis of a large and unique longitudinal data-set demonstrates that childbearing after age 35 or 40 is associated with survival and healthy survival among very old Chinese women and men. The association is stronger for women than for men. The estimates are adjusted for a variety of confounding factors: demographic characteristics, family support, social connections, health practices, and health conditions. Further analysis based on an extension of the Fixed-Attributes Dynamics method shows that late childbearing is positively associated with long-term survival and healthy survival from ages 80-85 to 90-95 and 100-105. This association exists among oldest-old women and men, but, again, the effects are substantially stronger for women than for men. We discuss four possible factors that may explain why late childbearing affects healthy longevity at advanced ages: (1) social factors; (2) biological changes caused by late pregnancy and delivery; (3) genetic and other biological characteristics; and (4) selection.

THE NUMBER OF oldest old persons in China, defined here as aged 80 and older, is calculated by the United Nations to climb from about 11.5 million in the year 2000 to 27 million in 2020, 39 million in 2030, 64 million in 2040, and 99 million in 2050. 1 Under the UN’s medium fertility assumption, 2 the percent aged 65 and older is expected to increase from 6.9 in 2000 to 15.7 in 2030 and 22.7 in 2050, while the share of the oldest old which in 2000 is 13 percent of the elderly population will be some 30 percent in 2050. The main reason the number of oldest old will climb so quickly after 2030 is that China’s “baby boomers,” who were born in the 1950s and 1960s, will fall into the category of “oldest old” at that time. During 2000‐50 the population of the oldest old will grow faster than any other age group in China. This phenomenon is not limited to China. The average annual rates of increase of oldest old persons between 2000 and 2050 are expected to be around 4.3 percent in China, India, and Mexico and 2.2‐2.8 percent in Canada, Japan, the United States, Germany, and France (UN 2001b). The rate of increase of the Chinese oldest old subpopulation is much faster (4.4 percent vs. 2.7 percent) than that of the entire elderly population aged 65 and older. The oldest old are much more likely to need help in daily living than the younger elderly. The oldest old consume medical care and other services and benefits of government and private transfers far out of proportion to their numbers. As evidenced later in this article, the proportion of individuals leading active daily lives declines and the disability rate increases dramatically with age among the Chinese oldest old. Torrey (1992: 382) estimated that in the United States, the Medicare costs of those aged 80 and older are 77, 60, and 36 percent higher than those of the younger elderly aged 65‐69, 70‐74, and

Background The aim of our study was to investigate the clinical results and serum metal ion levels in a cohort of patients who received total hip arthroplasty (THA) with ceramic-on-metal (CoM) bearings. Methods From September 2009 to December 2011, 78 patients (89 hips) who underwent THA with CoM bearings were involved in this study. Harris Hip Score, Short-Form 12, and Western Ontario and McMaster Universities Osteoarthritis Index scores were measured and radiographs were taken for radiographic analysis. Serum metal ion levels of cobalt (Co), chromium (Cr), molybdenum (Mo), and titanium (Ti) were measured using high-resolution inductively coupled plasma-mass spectrometry. Results Severy-four patients (85 hips) were followed up at a mean of 50 months. At the end of follow-up, HSS, Short-Form 12, and Western Ontario and McMaster Universities Osteoarthritis Index scores were improved significantly compared with preoperative values. No intraoperative and postoperative complications occurred, and no radiolucency, osteolysis, and loosening was found from radiographic examination. Metal ion analysis showed that serum metal ions levels were significantly elevated compared with normal values. Spearman correlation analysis revealed that there was a correlation between 3 metal ion levels and body mass index(Co: r = 0.49, P r = 0.47, P r = 0.36, P = .04). No correlation was found between metal ion levels and age, cup abduction angle, cup anteversion angle, acetabular version, bilateral arthroplasty, cup screw used, hip stem implant type, or femoral head size. Conclusion Our study concluded that the use of a CoM THA is effective clinically, but the systemic metal ion levels are significantly elevated at midterm follow-up. Whether the elevated metal ion levels will induce an adverse reaction is unknown and long-term follow-up is need.

In this article it is shown that the extremely low fertility rates reported in China – well below replacement level – derived from the Chinese Survey in 1992 are false. Serious under-reporting of most recent births in China was caused by various factors, among them high pressure on officials to achieve the birth control targets set, the design of the questionnaire, and the employment of family planning workers as enumerators. The most likely value of total fertility in 1991–92 was at or slightly below replacement level, i.e. between 2.1 and 2.2 children per woman. Even after adjustment for serious under-reporting, marital fertility fell substantially between 1990 and 1992, mainly as a consequence of tighter implementation of the strict family planning programme. Rapid economic development also contributed to the fall, as many young people in the country left farming to engage in non-agricultural activities locally, or migrated to urban areas, particularly in the southern part of the country, where economi...

This study analyzes the unique datasets of the Chinese Longitudinal Healthy Longevity Survey using logistic regression and controlling for various covariates. Our analyses clearly demonstrate that disabled older parents are more satisfied with care provided by daughters than sons and that older parents enjoy greater filial piety from and better relationships with daughters than sons. The daughter-advantages of enjoying greater filial piety from and better relationships with children are stronger among the oldest-old aged 80+ than the young-old aged 65-79, and surprisingly more profound in rural areas than urban areas, while son-preference is much more prevalent among rural residents. We also discuss why China's rigorous fertility policy until October-2015 and much less-developed pension system in rural areas substantially contribute to sustaining traditional son-preference and a high sex ratio at birth (SRB) when fertility is low. We recommend China take integrative public health policy actions of informing the public that having daughter(s) is beneficial for old age care, developing the rural pension system and implementing the universal two-child policy as soon as possible. We believe that these policy actions would help to reduce son-preference, bring down the high SRB, and enable more future elderly parents to enjoy better care from their children and healthier lives.
