There is a growing shift from the use of conventional pharmaceutical oral care products to the use of herbal extracts and traditional remedies in dental caries prevention. This is attributed to the potential environmental and health implications of contemporary oral products. This comprehensive review aims at the analysis of plant-derived compounds as preventive modalities in dental caries research. It focuses on data collected from 2019 until recently, trying to emphasize current trends in this topic. The research findings suggest that several plant-derived compounds, either aqueous or ethanolic, exhibit notable antibacterial effects against Streptococcus mutans and other bacteria related to dental caries, with some extracts demonstrating an efficacy comparable to that of chlorhexidine. Furthermore, in vivo studies using plant-derived compounds incorporated in food derivatives, such as lollipops, have shown promising results by significantly reducing Streptococcus mutans in high-risk caries children. In vitro studies on plant-derived compounds have revealed bactericidal and bacteriostatic activity against S. mutans, suggesting their potential use as dental caries preventive agents. Medicinal plants, plant-derived phytochemicals, essential oils, and other food compounds have exhibited promising antimicrobial activity against oral pathogens, either by their anti-adhesion activity, the inhibition of extracellular microbial enzymes, or their direct action on microbial species and acid production. However, further research is needed to assess their antimicrobial activity and to evaluate the cytotoxicity and safety profiles of these plant-derived compounds before their widespread clinical use can be recommended.

The primary aim of this study was to provide a sustainable and systemic solution to the prevalent time management challenges within healthcare academia. The secondary aim was to explore the correlations between various factors influencing time management in a dental university setting. To achieve these objectives, a predictive model, the Timebooster Academic Systemic Model (TASM), was developed. This model was crafted through systemic analysis software and a systemic management approach, utilizing three fundamental pillars: teaching, research, and administrative tasks. Soft skill cultivation, automation implementation, the delegation of administrative responsibilities, and the role of resilient academic manager–leaders were also examined. The study found that prioritizing administrative tasks often consumes valuable academic time, resulting in excessive costs and lowered resilience levels among academicians. TASM proposes solutions such as soft skill cultivation, full automation, delegation, and the mitigation of time-consuming elements, emphasizing the role of resilient academic manager–leaders in enhancing collaboration and teamwork. Time management challenges significantly impact academic well-being and resilience. In conclusion, systemic models like TASM provide a valuable framework to address prioritization concerns, identify administrative obstacles, and manage low-value time-consuming tasks within academia in a systemic manner.

In the area of dental healthcare services, where patients and professionals deal with multiple auditory stimuli, this cross-sectional study reports on the various dynamics of sound preferences, noise obstacles, and the profound impact of music on anxiety reduction within dental office settings. The sample comprises 134 dental patients, primarily females (56.7%), with varying age groups, education levels, and therapy durations in the university clinics of the Department of Dentistry, National and Kapodistrian University of Athens, and four private dental offices at the metropolitan area of Athens, Greece. Notably, 92.5% of the participants reported no hearing loss, and 56.7% exhibited some level of noise sensitivity. This study reveals a moderate to good perceived health status (M = 3.84, SD = 0.95) among participants, with low perceived noise disturbance in university dental clinics. Preferences for instrumental music, foreign pop, and classical music are prominent during waiting times. Potential irritations include impersonal treatment (44%) and staff behavior (41.8%). Correlations and predictors indicate associations between age, gender, educational status, music preferences, and noise disturbances. Findings aim to optimize eco-soundscapes in dental offices, contributing to enhanced patient well-being. The research underscores the potential of music interventions to alleviate dental anxiety and improve the overall dental experience and quality care.

In the face of 21st-century environmental challenges, including climate change, migration pressures, and waste disposal issues, certain healthcare sectors, notably dentistry, pose a significant global environmental footprint with concerns about carbon emissions and waste production. This scoping review searches the paradox that healthcare, while dedicated to safeguarding health, inadvertently contributes to environmental degradation through waste accumulation and disposal. The analysis of the relevant literature emphasizes the imperative for an environmentally sustainable approach to dental waste measurement, disposal methods, and comprehensive education for stakeholders. Aligned with the World Conference on Sustainable Development and the United Nations’ 2030 Agenda, the study adhered to PRISMA-ScR systematic review guidelines and the Institute of Medicine’s recommendations. The study utilized Medline/PubMed and international organization websites for data retrieval, employing Boolean operator queries in Medline. The systematic literature analysis identified six cross-sectional studies (2004–2023) highlighting variations in dental waste composition due to the lack of standardized identification methods. This heterogeneity underscores the need for a proposed research model, emphasizing comprehensive methodologies and healthcare staff training in waste management, thus transforming the analysis into a scoping review. The study advocates for environmentally conscious dental practices, contributing to broader sustainable healthcare goals through sharing of a research protocol for dental solid waste management.

Nutrition and dietetic care span various life stages, addressing nutritional needs and exploring factors that shape dietary habits. Globally, an alarming disproportion in food resource distribution, coupled with substandard nutritional intake, underscores profound implications for oral health. This is evident in the facilitation of dental caries development and its repercussions on oral soft tissue integrity. While modern markets offer advanced edible products, their effects on oral health, especially in relation to dental caries, remain uncharted. This narrative literature review pursues a four-fold objective: (a) scrutinizing the repercussions of inadequate nutrition on dental caries, (b) exploring psychological determinants influencing dietary habits contributing to caries development, (c) investigating potential implications of newly devised food products on caries formation, and (d) elucidating the role of dentists as facilitators in promoting oral health practices. A comprehensive search spanning PubMed, Web of Science, and Cochrane Library yielded 46 cohort, cross-sectional research articles and systematic reviews adhering to standardized diagnostic criteria for dental caries evaluation. The results highlight contemporary lifestyles as potent contributors to heightened risk of caries due to suboptimal nutritional quality and nutrient insufficiency. Additionally, concerns have arisen with the advent of synthetically engineered food products, warranting exploration of potential implications for future caries development and global oral health status. This review emphasizes the pivotal role of dietitians–nutritionists and dental professionals in advocating sound nutritional practices specifically geared towards preventing dental caries. Ultimately, this review contributes to the understanding of the intricate interplay between nutrition, oral health, and the imperative role of healthcare professionals in fostering preventive measures.

The rising demand for novel antibiotic agents prompts an investigation into natural resources, notably plant-derived compounds. In this study, various extracts (aqueous, ethanolic, aqueous-ethanolic, and enzymatic) of Rosa damascena and Hypericum perforatum were systematically evaluated against bacterial strains isolated from dental lesions (n = 6) and food sources (raw milk and broiler carcass, n = 2). Minimal inhibitory concentration (MIC), minimal bactericidal concentration (MBC), antibiofilm activity, and time-kill kinetics were assessed across a range of extract concentrations, revealing a dose-responsive effect. Notably, some extracts exhibited superior antibacterial efficacy compared to standard clinical antibiotics, and the time-kill kinetics demonstrated a rapid elimination of bacterial loads within 24 h. The susceptibility pattern proved strain-specific, contingent upon the extract type, yet all tested pathogens exhibited sensitivity. The identified extracts, rich in phenolic and polyphenolic compounds, as well as other antioxidant properties, contributed to their remarkable antibiotic effects. This comprehensive investigation not only highlights the potential of Rosa damascena and Hypericum perforatum extracts as potent antibacterial agents against diverse bacterial strains including caries pathogens, but also underscores their rapid action and dose-dependent efficacy. The findings suggest a promising avenue for harnessing plant-derived compounds in the development of novel antimicrobial strategies against dental caries and other oral inflammations, bridging the gap between natural resources and antibiotic discovery.

Noise levels in a dental office can be produced by different specialty instruments. Exposure to high levels of noise (unwanted sounds) may cause auditory and non-auditory health problems in dentists. The aim of this study was to (a) measure the noise levels within different clinics and laboratories of the Department of Dentistry, School of Health Sciences of the National and Kapodistrian University of Athens, (b) promote information sharing on this serious health issue among stakeholders, and (c) collect data to organize preventive measures for students and personnel (faculty members, collaborators, administrative, and technical staff). Since the study did not apply to acoustics and acoustic measurements, a digital sound level meter and noise-integrating dosimeters with an analogue electronic transducer were used to collect data from certain postgraduate (PG) and undergraduate (UG) clinics and laboratories (LAB) during peak working periods and with a duration of 1 h per clinic/lab. Both personal (dosimeters) and static (area monitoring) noise exposure assessments were evaluated, resulting in various teaching-related activities in dental clinics. At all locations, the maximum exposure limit value of 87 dB(A) was not exceeded. However, chairside personal measurements during ultrasonic work revealed that the lower exposure action value of 80 dB(A) was exceeded. PG clinics were noisier than UG. LAB training settings, even with the new equipment, were close to the upper exposure limit due to the simultaneous use of airotors. In this context, targeted research and investigations into measures are proposed to safeguard the health and safety of students during their duties at the dental school.

Dental caries is a multifactorial disease with multiple risk factors. Vitamin D levels (VDLs) and vitamin D receptor polymorphisms (VDRPs) have been investigated for this reason. The aim of this narrative review is to investigate the relation and the factors affecting vitamin D deficiency (VDD), VDRP, Early Childhood Caries (ECC) and Severe Early Childhood Caries (S-ECC) in children (primary and mixed dentition) and dental caries risk in adults (permanent dentition). Additionally, we present a model incorporating factors and interactions that address this relationship. Methods: Three databases (PubMed/MEDLINE, Web of Science, Cochrane Library) were comprehensively searched until 17 January 2023 using the following keywords: “vitamin D”, “vitamin D receptor polymorphism”, “dental caries”, and “dental caries risk”, finding 341 articles. Two reviewers searched, screened, and extracted information from the selected articles. All pooled analyses were based on random-effects models. Eligibility criteria were articles using dmft/DMFT diagnostic criteria with calibrated examiners, probability sampling, and sample sizes. We excluded studies conducted on institutionalized patients. A total of 32 studies were finally used. Results: In most studies, TaqI, FokI, and BsmI polymorphisms affected the prevalence of dental caries. A strong correlation between ECC, S-ECC, and the prevalence of dental caries was reported in association with VDD and maternal intake of VD in primary dentition. Regarding the influence in mixed dentition, the results were found to be inconclusive. A slight positive influence was reported for permanent dentition. Conclusions: Factors affecting caries risk were maternal intake, socioeconomic factors, and level of VD. There is a certain need for more well-conducted studies that will investigate the association between VDR gene polymorphisms and the prevalence of dental caries in mixed and permanent dentition, specifically in adult patients.

Dental and oral health are considered among the main health issues for migrants and refugees, as access to dental health care services is often expensive and difficult. The study investigates dental and oral health determinants among migrants and refugees in 10 European countries (Austria, Bulgaria, Cyprus, France, Germany, Greece, Italy, Malta, Spain, and Sweden), examining how mental health, legal status, discrimination issues and dental services’ use frequency affect dental health. Methods: A cross sectional study using a purpose-made questionnaire was carried out to assess health status and access, with a dedicated section to measure self-perceived dental health, prevalence of caries, last visit to dentist and anticipated access to dental health services. Multivariable logistic regression models were performed to investigate the impact of quality of life, discrimination, immigration status, and other demographic factors on dental health. Results: About half of the sample suffered from poor dental condition and 22% had never visited a dentist. Migrants with higher educational levels had higher odds of having good dental health (OR = 1.08; 95%CI (1.03, 1.12)) and brushing their teeth daily (OR = 1.1; 95%CI (1.04, 1.17)). Higher general and mental health scores were indicative of better dental condition (general health: OR = 1.02; 95%CI (1.01, 1.03); mental health: OR = 1.01; 95%CI (1.004, 1.02)) and higher probability of daily teeth brushing (general health: OR = 1.02; 95%CI (1.01, 1.03); mental health: OR = 1.02; 95%CI (1.01, 1.03)). The possession of any kind of legal immigration permission and not having any children showed similar results. Age and discrimination were correlated with decreased likelihood for good dental conditions. Gender was correlated with daily teeth brushing, as female migrants had higher odds of brushing their teeth daily. Conclusions: Many migrants report poor dental health. Nonetheless, migrants with higher education levels, legal immigration status, better general and mental health, no children, lower sense of discrimination, younger age, and regular dental visits were positively correlated to good dental health (perceived as no dental caries).

To study the antibacterial effect of different Greek honeys, samples of citrus honey, Saturja spp. Honey, and oregano and sage honey were collected directly from producers. Manuka honey and artificial honey were used as controls. The honeys were diluted in various concentrations to determine the minimum inhibitory concentration (MIC) and were also placed in agar wells to determine the inhibitory zones of growth. The bacteria tested were two reference strains and five pathogens isolated from patients with various dental ailments. A series of samples were diluted with artificial saliva instead of distilled water to simulate the conditions in the oral cavity. The results show that in most cases the Greek honeys, and particularly the citrus honey and the oregano and sage honey, outperformed the antibacterial activity of manuka honey against all tested bacteria. This performance was due to the hydrogen peroxide as well as to other components of the honeys, that is, peptides and other substances such as phenolic compounds and flavonoids. Artificial saliva enhanced the antibacterial effect of the honeys in comparison to distilled water.

Probiotic therapy forms a new strategy for dental caries prevention. Probiotic microorganisms possess the ability to displace cariogenic microorganisms and colonize the oral cavity. They can produce various antimicrobial substances such as bacteriocins, bacteriocin-like peptides, lactic acid, and hydrogen peroxide. Dairy products may be ideal for probiotic administration in dental patients. Many other means have been proposed, primarily for those allergic to dairy components, such as capsules, liquid form, tablets, drops, lozenges, sweetened cakes, and ice creams. The last two forms can be used in a coaching approach for children and elderly patients who find it difficult to avoid sugary beverages in their daily routine and benefit from the suggestion of easy, cheap, and common forms of delicacies. In caries prevention, the concept of the effector strain is already considered an integral part of the contemporary caries cure or prevention strategy in adults. Adults, though, seem not to be favored as much as children at early ages by using probiotics primarily due to their oral microbiome’s stability. In this non-systematic review we describe the modes of action of probiotics, their use in the cariology field, their clinical potential, and propose options to prevent caries through a patient coaching approach for the daily dental practice.

Over the last decades, the incidence of diabetes has increased in developed countries and beyond the genetic impact, environmental factors, which can trigger the activation of the gut immune system, seem to affect the induction of the disease process. Since the composition of the gut microbiome might disturb the normal interaction with the immune system and contribute to altered immune responses, the restoration of normal microbiota composition constitutes a new target for the prevention and treatment of diabetes. Thus, the interaction of gut microbiome and diabetes, focusing on mechanisms connecting gut microbiota with the occurrence of the disorder, is discussed in the present review. Finally, the challenge of functional food diet on maintaining intestinal health and microbial flora diversity and functionality, as a potential tool for the onset inhibition and management of the disease, is highlighted by reporting key animal studies and clinical trials. Early onset of the disease in the oral cavity is an important factor for the incorporation of a functional food diet in daily routine.

Health-related behavior based on diet is an important determinant of oral health in independent elderly. Aging impairs senses, mastication, oral status, and function, causing nutritional needs and diet insufficiencies that contribute to a vicious circle of impairment. But the present needs of independent older adults suggest that health research and oral health care should shift from disease management and therapy to integral customized and personal treatment plans, including lifestyle, psychological, nutritional, and oral health coaching approaches. In this paper health coaching approaches in medical and dental settings are valued as to their effectiveness for older adults. Furthermore, coaching approaches for seniors are discussed and coaching models for better senior patient-dentist cooperation on the diet issue are suggested. Diet and oral health coaching is proven to be a modern senior patient-centered approach that needs to be incorporated at all relevant settings. It should aim to empower older adults in co-management of their oral diseases or bad diet habits affecting their oral health. This can be carried out through an incorporated educational plan for dentists either at the postgraduate or professional level since advantages seem to enhance the quality of life of the independent elderly.
