1. Alcoholism in homeless men in the mid-nineties: results from the Bavarian Public Health Study on homelessness.

Abstract

Parallel to structural economic changes homelessness has become publicly more visible and has received increased media attention in Western industrialized countries. Most studies on mental illness and homelessness in recent years were carried out in North America but only few studies in Europe have dealt with these issues. The goals of the present study were (1) to assess alcohol abuse and dependence as well as other mental disorders in a representative sample of homeless men in Munich using reliable methods of case identification (Structured Clinical Interview for DSM-IV (SCID)), (2) to compare homeless alcoholics with homeless non-alcoholics in our sample on relevant variables, and (3) to compare our data from the Munich sample with data obtained by others. According to our results, the life-time prevalence of any SCID-DSM-IV Axis I diagnoses was 93.2% and the lifetime prevalence of substance use disorder was 79.6%. The single most prevalent diagnosis among homeless males in Munich was alcohol dependence (life-time 72.7%); alcohol abuse (life-time 5.5%) and drug abuse/dependence were considerably lower in prevalence (life-time 19.1%) (weighted data). A higher rate of psychotic disorders was found for non-alcoholic homeless men. Data show that alcoholism and its consequences were more severe in the Munich as compared to North American samples. Homeless alcohol dependent men showed a high comorbidity with other mental disorders (life-time) such as mood disorders (36.4%), anxiety disorders (16.4%), drug abuse/dependence (18.9%) and psychotic disorders (4.5%). Of those with alcohol dependence at some time during their life 59.1% had experienced at least one other life-time mental disorder. Alcohol-related behavioral patterns and symptoms as well as general social functioning are described. Considering the very high prevalence of alcohol dependence (frequently in combination with other mental disorders), the participation in alcohol rehabilitation and other services as well as self-help groups was rather low among Munich homeless alcoholics. Currently homeless men in Germany are adequately supplied with food, clothes, and shelter but psychiatrically (and medically) neglected. Existing concepts and programs for dealing with these problems need to be implemented.

Family-based predictors of relapse were examined in 100 alcoholics who participated in a 12-week treatment program with 6-month and 18-month followups. "Expressed Emotion" (EE), or attitudes of relatives toward the alcoholic as measured by the Camberwell Family Interview, scales measuring rejection of the alcoholic by relatives, and self-reports of partner interaction were evaluated as possible predictors of abstinence. During therapy, partnership interactions showed a transient deterioration with increased temporary friction. Based on conservative criteria, the abstinence rate was 40% at 6-month followup and 30% at 18-month followup. An association with the relapse at followup could be obtained for the Patient Rejection Scale (PRS) and, using empirically derived classification rules, for the main three variables of the Camberwell Family Interview (CFI) conducted on admission: "Critical Comments," "Emotional Overinvolvement," and "Warmth." A low number of Critical Comments and a high score in Warmth were associated with a lower risk of relapse; however, contrary to expectations, Emotional Overinvolvement of the significant other was associated with more abstinence. In addition, the number of Critical Comments made by relatives about the alcoholic, a major component of high EE as measured by the CFI, had a statistically significant impact on the "survival function" of abstinence, and thus contributed to the prediction of the course of alcoholism in the expected direction.

Abstract

One hundred alcoholics participated in a 6-week inpatient and a 6-week outpatient therapy programme and a 6-month and 18-month follow-up. Major goals of the study were a descriptive analysis of the course of illness over time, the evaluation of the effects of therapy in general and concerning the specific effects of additional (A) systematic inclusion of the key relative versus (B) systematic involvement in self-help, and identification of predictors for relapse. While therapy as such was quite effective and resulted in significant changes in symptomatology and the scores on personality scales, the additional involvement of the key relative showed no substantial additional treatment effects. Partnership interactions showed a deterioration during therapy, with temporary increased frictions. Based on conservative calculations, 40% of all patients remained abstinent until the 6-month follow-up and 30% until the 18-month follow-up.

Abstract

The study analyses the risks of mortality associated with alcohol consumption and smoking, as well as possible counteracting effects of physical activity and social support through close personal relationships. Data are based on the Upper Bavarian Study, a longitudinal epidemiological study of a representative community sample (n = 1668) in a rural area. Extensive semistructured psychiatric interviews by research physicians were conducted between 1975 and 1977 (n = 1536). Thirteen years after psychiatric assessment, information was obtained from the community register concerning death in the interval, data of death and cause of death according to ICD 9. This information could be ascertained for 93.1% (n = 1430) of those who had been interviewed, thus providing a good basis for generalizing the findings. Results indicate that alcohol intake and cigarette smoking increased mortality while physical activity and the availability of a steady partner had protective effects. There were no interactive effects between the four variables studied, except for a dramatically increased risk for women drinking more than 20 ml of pure alcohol a day and reporting no physical exercise at wave one assessment. The relative risks of alcohol intake and smoking, and the counteracting effects of physical activity and partnership, are exemplified in the cases of a 40-year-old female and a 40-year-old male. Specific analyses of the relationship between alcohol consumption, smoking, physical exercise and personal relationships, on the one hand, and, on the other, different causes of death, are presented.

Abstract

100 male and female patients, aged 20 to 60 years with the diagnosis alcohol-addiction (ICD 9 No 303.0) took part in a longitudinal treatment evaluation study. Consecutive admissions from May '83 until July '85 were assessed. All patients participated in a 6-week inpatient group therapy and a 6-week outpatient treatment for alcohol-dependent patients at the Psychiatric University Hospital in Munich. Assessments were performed at the following times: 1. on admission, 2. at the end of inpatient therapy, 3. at the end of outpatient therapy, 4. at a 6-month follow-up and 5. at an 18-month follow-up. The major goals of the study were as follows: 1. Descriptive analysis of the course of illness from admission to the 18-month follow-up; 2. Analysis of the influence of the social setting at home (relationship, attitude of the family towards the patient) (Kreisman's Patient Rejection Scale) and expression of emotions in the family according to the "Camberwell Family Interview" (CFI); 3. Evaluation of the effect of the additional treatment component (A) "Systematic involvement of the significant other" as compared to an alternative component (B) "Systematic encouragement of the patient in self-help initiatives"; 4. Identification of risk factors for relapse and predictors of good prognosis that can be used to improve the therapeutic concept. The sample consisted of 42% women and 58% men, with a mean age of 37.9 years. Only patients who had a spouse or another close relative were accepted for participation in the study. At the beginning of the study 48% of the patients were married, 18% divorced, 27% unmarried and 6% lived separately or were widowed. 95 of the 100 patients could be reassessed at discharge from inpatient therapy, 92 at the end of 6-week outpatient treatment phase, 91 at the 6-month follow-up and 90 at the 18-month follow-up. The additional treatment components (A versus B) showed no major differences in effect. Significant changes in the patients attitude, behaviour and psychopathology were observed for patients receiving either one of the additional treatment components (A or B). The lack of differing effects from the additional treatment components is plausible, because the main part of the therapy (without the additional therapy component) was the same in both groups; this basic therapy took up most treatment time and by itself brought about major changes in personality scales, psychopathology and partnership interactions.(ABSTRACT TRUNCATED AT 400 WORDS)

Abstract

Several endocrine parameters were assessed in 35 alcoholic inpatients after admission to hospital, and 17 of the 35 were retested after several weeks of sobriety. No difference was found in clonidine-stimulated growth hormone (GH) secretion between male alcoholics and male healthy controls, but significant positive correlations of GH secretion and alcohol content in expired breath on admission and gamma-GT values after abstinence were observed. Nonsuppression in the dexamethasone suppression test was found in 17% of the patients on admission, which seemed to be due to alcohol withdrawal. Postdexamethasone cortisol levels were significantly positively correlated with the "apathic syndrome" (r = 0.40; p less than or equal to 0.05). About one-third of the patients showed a blunted response in the TRH-test both on admission and after abstinence. No significant influence of alcohol intake, withdrawal or familial disposition on prolactin values could be detected. The results of the TRH test and the DST point to similar endocrinological patterns in alcoholics as in depressive patients and thus support the hypothesis of a link between alcoholism and depression.

8. Intervention effects of supplying homeless individuals with permanent housing: a 3-year prospective study.

Abstract

Objective: To describe the intervention effects of supplying homeless individuals with permanent housing. Method: In a prospective study, 109 male and 20 female homeless individuals were assessed at baseline and at 1- and 3-year follow-up concerning mental illness (SCID-I), psychopathology, global assessment of functioning, emotional lability and alcohol consumption. Results: A high proportion (86%) of the individuals was able to maintain or improve stability of housing. Only minor changes were observed concerning mental illness and global functioning. Extensive alcohol consumption and high psychopathology increased the risk of losing the stable housing. Conclusion: The placement of homeless individuals in board and care homes or community housing after social counselling seems to be a necessary measure to remedy homelessness. However, supplying more permanent housing is not sufficient to decisively improve mental health status. [ABSTRACT FROM AUTHOR]

Abstract

Eating disorders and substance use disorders frequently co-occur. Twin studies reveal shared genetic variance between liabilities to eating disorders and substance use, with the strongest associations between symptoms of bulimia nervosa (BN) and problem alcohol use (genetic correlation [rg], twin-based=0.23-0.53). We estimated the genetic correlation between eating disorder and substance use and disorder phenotypes using data from genome-wide association studies (GWAS). Four eating disorder phenotypes (anorexia nervosa [AN], AN with binge-eating, AN without binge-eating, and a BN factor score), and eight substance-use-related phenotypes (drinks per week, alcohol use disorder [AUD], smoking initiation, current smoking, cigarettes per day, nicotine dependence, cannabis initiation, and cannabis use disorder) from eight studies were included. Significant genetic correlations were adjusted for variants associated with major depressive disorder (MDD). Total sample sizes per phenotype ranged from ~2,400 to ~537,000 individuals. We used linkage disequilibrium score regression to calculate single nucleotide polymorphism-based genetic correlations between eating disorder and substance-use-related phenotypes. Significant positive genetic associations emerged between AUD and AN (rg=0.18; false discovery rate q=0.0006), cannabis initiation and AN (rg=0.23; qwith binge-eating (rg=0.27; q=0.0016).
