Abstract

Recent evidence suggests that during volitional stepping older adults control whole-body angular momentum (H) less effectively than younger adults, which may impose a greater challenge for balance control during this task in the elderly. This study investigated the influence of aging on the segment angular momenta and their contributions to H during stepping. Eighteen old and 15 young healthy adults were instructed to perform a series of stepping at two speed conditions: preferred and as fast as possible. Full-body kinematics were recorded to compute angular momenta of the trunk, arms and legs and their contributions to total absolute H on the entire stepping movement. Results indicated that older adults exhibited larger angular momenta of the trunk and legs in the sagittal plane, which contributed to a higher sagittal plane H range during stepping compared to young adults. Results also revealed that older adults had a greater trunk contribution and lower leg contribution to total absolute H in the sagittal plane compared to young adults, even though there was no difference in the other two planes. These results stress that age-related changes in H control during stepping arise as a result of changes in trunk and leg rotational dynamics.

Abstract

Cancer-related fatigue can continue long after curative cancer treatment. The aim of this study was to investigate sleep and rest–activity cycles in fatigued and non-fatigued cancer survivors. We hypothesized that sleep and rest–activity cycles would be more disturbed in people experiencing clinically-relevant fatigue, and that objective measures of sleep would be associated with the severity of fatigue in cancer survivors. Cancer survivors (n = 87) completed a 14-day wrist actigraphy measurement to estimate their sleep and rest–activity cycles. Fatigue was measured using the Functional Assessment of Chronic Illness Therapy Fatigue Scale (FACIT-F). Participants were dichotomised into two groups using a previously validated score (fatigued n = 51 and non-fatigued n = 36). The participant’s perception of sleep was measured using the Insomnia Severity Index (ISI). FACIT-F score was correlated with wake after sleep onset (r = −0.28, p = 0.010), sleep efficiency (r = 0.26, p = 0.016), sleep onset latency (r = −0.31, p = 0.044) and Insomnia Severity Index (ISI) score (r = −0.56, p &lt, 0.001). The relative amplitude of the rest–activity cycles was lower in the fatigued vs. the non-fatigued group (p = 0.017, d = 0.58). After treatment for cancer, the severity of cancer-related fatigue is correlated with specific objective measures of sleep, and there is evidence of rest–activity cycle disruption in people experiencing clinically-relevant fatigue.

Abstract

Some studies have suggested that postural balance improved after a single session of transcranial direct current stimulation (tDCS), whereas others have found minimal, if any, effects on postural performance. To address the issue of replication in tDCS studies, we re-tested the anodal tDCS effects of left dorsolateral prefrontal cortex while performing a dual-task by increasing the attentional demands associated with more challenging proprioceptive conditions. Twenty-four young adults (mean age: 21.3 ± 1.2 years) were randomly divided into two groups (a “real tDCS” vs. a “sham tDCS” group) were asked to maintain a quiet stance on a force platform. Eight trials were conducted, with eyes open and eyes closed, standing on a firm and foam surface and performing a simple and dual-task (backward counting). The postural performance was assessed by various centre-of-pressure parameters before and immediately after a 20-min tDCS session. No main effect of group and no interaction considering this factor were observed, regardless of the centre-of-pressure variables (all p values > 0.1). No evidence of a more efficient postural control emerged after a tDCS session. Beyond promising research on tDCS to maximize cognitive and behavioural enhancement, the current results indicate that caution needs to be taken when drawing firm conclusions, at least in young healthy adults.

Abstract

Background Chronic Kidney Disease (CKD) is associated with reduced muscular strength resulting in profound fatigue. The physiopathology of these changes, their prevalence and evolution are still debated. Moreover, we have little data on elderly CKD patients. The present study protocol aims to 1) quantify the prevalence of low muscle strength (dynapenia) in a cohort of elderly patients with advanced CKD and to 2) characterize their force production coupled with electromyographic features and the symptoms of fatigue compared to a matched control group. Methods This is a case-control, prospective, interventional study. Inclusion criteria: age ≥ 60 years; CKD Stage 3b-5; clinical stability (i.e. no hospitalization and ≤ 25% in creatinine increase in the previous 3 months). Controls with normal kidney function will be matched in terms of age, gender and diabetes mellitus (requisite: estimated glomerular filtration rate ≥ 60 ml/min/1.73m2 available in the last 6 months). Exclusion criteria for cases and controls: neuromuscular disease, life expectancy The handgrip strength protocol is an intermittent test consisting in 6 series of 9 repetitions of 3-s sub-maximum contractions at 40% of the maximum voluntary contraction (MVC) and 2 s of resting time between contractions. Each series is separated by one fast sub-maximum contraction and one MVC. Strength is assessed with a high-frequency handgrip dynamometer paired with surface electromyography. Symptoms of fatigue are assessed using MFI-20 and FACIT-F questionnaires. In order to reach a statistical power of 96%, we plan to enroll 110 subjects in each group. Discussion The novelty of this study resides in the application of an already validated set of tests in a population in which this combination (dynamometer, electromyography and questionnaires) has not previously been explored. We expect a high prevalence of dynapenia and a higher fatigability in CKD patients. A positive correlation is expected between reported fatigue and fatigability. Better appreciation of the prevalence and the relationship between fatigability and a sensation of fatigue can help us target interventions in CKD patients to improve quality of life and survival. Trial registration The study was approved by Ethical Committee EST III n°20.03.01 and was recorded as a Clinical Trial (NCT04330807) on April 2, 2020.

Abstract

International audience; Attention deficit with or without hyperactivity disorder (ADHD) is one of the most frequent neuropsychiatric disorders, and affects 2-4% of adults. In contrast with many European countries, the identification and management of adult ADHD remains underdeveloped in France, and a subject of controversy. This review provides a practical update on current knowledge about ADHD in adults for French-speaking professionals who have to detect or manage adult patients with ADHD. ADHD is classified as a neurodevelopmental disorder in the recent update of the international diagnostic classification. While symptoms and impairment due to ADHD are frequently severe during childhood, they often evolve as children grow older, with frequent persistent disabilities in adulthood. In adulthood, the clinical presentation, as in childhood, involves the symptom triad of inattention, hyperactivity and impulsivity. However, differences are noted: hyperactivity is more often internalized, symptoms of inattention may be masked by anxiety symptoms or obsessive-like compensation strategies. ADHD is often diagnosed during childhood, but it is not rare for the diagnosis to be made later. Failure to recognise symptoms resulting in misdiagnosis, or alternatively well-developed compensation factors could be two underlying reasons for the long delay until diagnosis. Other symptoms, such as emotional deregulation or executive function-related symptoms are also usually observed in adults. In addition, in adults, ADHD is often associated with other psychiatric disorders (in 80% of cases); this makes the diagnosis even more difficult. These disorders encompass a broad spectrum, from mood disorders (unipolar or bipolar), to anxiety disorders, and other neurodevelopmental disorders and personality disorders, especially borderline and antisocial personality disorder. Substance-use disorders are very common, either as a consequence of impulsivity and emotional dysregulation or as an attempt at self-treatment. Sleep disorders, especially restless leg syndrome and hypersomnolence, could share common pathophysiological mechanisms with ADHD. ADHD and comorbidity-related symptoms are responsible for serious functional impairment, in various domains, leading to academic, social, vocational, and familial consequences. The impact on other psychiatric disorders as an aggravating factor should also be considered. The considerable disability and the poorer quality of life among adults with ADHD warrant optimal evaluation and management. The diagnostic procedure for ADHD among adults should be systematic. Once the positive diagnosis is made, the evaluation enables characterisation of the levels of severity and impairment at individual level. A full examination should also assess medical conditions associated with ADHD, to provide personalized care. In recent years, a growing number of assessment tools have been translated and validated in French providing a wide range of structured interviews and standardized self-report questionnaires for the evaluation of core and associated ADHD symptoms, comorbidities and functional impairment. The treatment of ADHD in adults is multimodal, and aims to relieve the symptoms, limit the burden of the disease, and manage comorbidities. The most relevant and validated psychological approaches are psycho-education, cognitive-behavioural therapy and "third wave therapies" with a specific focus on emotional regulation. Cognitive remediation and neurofeedback are promising strategies still under evaluation. Medications, especially psychostimulants, are effective for alleviating ADHD symptoms with a large effect size. Their safety and tolerance are satisfactory, although their long-term clinical benefit is still under discussion. In France, methylphenidate is the only stimulant available for the treatment of ADHD. Unfortunately, there is no authorization for its use among adults except in continuation after adolescence. Hence the prescription, which is subject to the regulations on narcotics, is off-label in France. This article aims to provide practical considerations for the management of ADHD and associated disorders in adults, in this particular French context.

Abstract

BACKGROUND Nutrition plays an important role in the aging process. Adherence to the Mediterranean diet (MedDiet) has been shown to be associated with lower rates of diseases. Cognitive status seems to be strongly interrelated with physical well-being, so that one influences the other. Physical performance measures are not only associated with clinical and subclinical age-related modifications, but are also able to predict disability, institutionalization, and mortality. OBJECTIVE To evaluate prospectively the associations between Mediterranean-Style Dietary Pattern Score (MSDPS), clinical characteristics, and cognition of the population sample of The TREVISO LONGEVA (TRELONG) Study, in Treviso, Italy. METHODS Global cognition, physical performance measures, MSDPS, and other clinical features were detected in 2010 in 82 men and 108 women. These characteristics were evaluated in relation to the physical performance measures identified 3.8 years later in 2013 in the same subjects, using a semantic connectivity map, through Auto-CM system, to grasp further and non-linear associations between variables which might remain, otherwise, undetected. RESULTS The Auto-CM system's map showed a close association between better levels of global cognition and MSDPS in 2010 and higher physical performance in 2013. On the other hand, worse levels of global cognition and MSDPS in 2010 were associated with lower physical performance in 2013. CONCLUSION The prevention models for successful aging may benefit from integrated programs that include cognitive, physical, and dietary interventions, since these aspects are mutually interrelated.

Abstract

The ways in which locations of ischemia and ischemic pain affect spatiotemporal gait parameters and leg electromyographic activity during walking have never been investigated in patients with peripheral arterial disease presenting intermittent claudication. Two groups were classified according to unilateral location of ischemia (distal, n = 10, or proximo-distal, n = 12). Patients described pain and three gait phases—initial pain-free, onset of pain and maximum pain—were analyzed. Patients with proximo-distal ischemia walked less (230 ± 111 m vs 384 ± 220 m), with increased step length, step time (+ 5.4% and + 5.8%) and reduced cadence (− 8.2%), than patients with distal ischemia. In both, the peaks of vertical ground reaction force were reduced in maximum pain (Peak1-distal: − 11.4%, Peak1-proximo-distal: − 10.3%; Peak2-distal: − 11.8%, Peak2-proximo-distal: − 9.0%). In the proximo-distal group, tibialis anterior activation peak and time were lower than in the distal group (− 4.5% and − 19.7%). During the maximum pain phase, this peak decreased only in the proximo-distal group (− 13.0%), and gastrocnemius medialis activation peak and time decreased in both groups (− 2.5% in distal and − 4.5% in proximo-distal). Thus, proximo-distal ischemia leads to more adverse consequences in gait than distal ischemia only. Increasing ischemic pain until maximum, but not onset of pain, induced gait adaptations.

Abstract

Rest-activity rhythm (RAR) disruptions are frequently associated with chemotherapy in breast cancer (BC), but they are less known in BC with endocrine therapy (ET). The aim of this ancillary study was to characterize the RAR and estimated sleep characteristics from actigraphy in BC patients either treated (ET+) or untreated with ET (ET−), compared to healthy controls (HC) and using a cross-sectional design. Eighteen ET+, 18 ET−, and 16 HC completed questionnaires and wore wrist actigraphs at home for 2 weeks. Parametric and nonparametric RAR, sleep parameters, and quality of life were compared between groups (p < .05). BC groups presented lower daytime activity than HC according to RAR analysis (mesor and M10 parameters). Compared to HC, ET− had lower inter-daily stability and ET+ had greater sleep complaints. Compared to ET−, ET+ had lower sleep efficiency, more time awake, and higher activity levels at night, as assessed with actigraphy. Our results suggest an effect of cancer independent of treatment on RAR in BC, highlighting the need for further investigation of this topic. In contrast, sleep as assessed with actigraphy seems modified only during ET which matches with patients’ sleep complaints. Further longitudinal studies would aid in confirming the latter hypothesis.

Abstract

The functional difference between the medial gastrocnemius (MG) and lateral gastrocnemius (LG) during walking in humans has not yet been fully established. Although evidence highlights that the MG is activated more than the LG, the link with potential differences in mechanical behavior between these muscles remains unknown. In this study, we aimed to determine whether differences in activation between the MG and LG translate into different fascicle behavior during walking. Fifteen participants walked at their preferred speed under two conditions: 0% and 10% incline treadmill grade. We used surface electromyography and B-mode ultrasound to estimate muscle activation and fascicle dynamics in the MG and LG. We observed a higher normalized activation in the MG than in the LG during stance, which did not translate into greater MG normalized fascicle shortening. However, we observed significantly less normalized fascicle lengthening in the MG than in the LG during early stance, which matched with the timing of differences in activation between muscles. This resulted in more isometric behavior of the MG, which likely influences the muscle–tendon interaction and enhances the catapult-like mechanism in the MG compared with the LG. Nevertheless, this interplay between muscle activation and fascicle behavior, evident at the group level, was not observed at the individual level, as revealed by the lack of correlation between the MG–LG differences in activation and MG–LG differences in fascicle behavior. The MG and LG are often considered as equivalent muscles but the neuromechanical differences between them suggest that they may have distinct functional roles during locomotion.

Abstract

Recent evidence suggests that older adults may have difficulty controlling whole-body angular momentum (H) during volitional stepping, which could impose a major challenge for balance control and result in potential falls. However, it is not known if and how H is influenced by speed when stepping. This study aimed to investigate the effect on H of increasing speed during step initiation in older adults. Twenty-seven healthy individuals over 60 were enrolled in the current study and were instructed to perform a series of step initiations with their dominant leg under two speed conditions: at preferred speed and as fast as possible. Two force plates and a motion-capture system were used to record H and the components of the net external moment (moment arms and ground reaction forces) during the double support and step execution phases of stepping. Results revealed that increasing speed of stepping affected H differently in both stepping phases and in the different planes. H ranges in all three planes increased with speed during the double support phase. During the step execution phase, while H ranges in frontal and transversal planes decreased, sagittal plane H range significantly increased with speed. This increased H range in the sagittal plane, which may result from the task demands, could impose a greater challenge for balance control in the elderly.

Abstract

Videofluoroscopic Swallow studies (VFSS) are useful radiological examinations to explore swallowing disorders but which require ionizing radiation. The aim of our study was to evaluate the comparability of pediatric VFSS at 15 frames per second (fps) with 30 fps. Fifty-five loops including 190 swallowings of VFSS at 30 fps performed on 32 consecutive pediatric patients in a University Hospital Center were retrospectively modified by a software to delete one image out of two to obtain secondary loops with a frame rate of 15 fps. An otorhinolaryngologist-phonatrician and a radiologist reviewed all swallowings blindly and randomly using the penetration and aspiration scale (PAS). In case of discordance, they concluded a consensual interpretation. Fifteen girls and seventeen boys were included. The median age was 4 years and 8 months (range = 4 months-16 yr.). 144 swallowings were normal. Swallowing disorder was confirmed in 46 swallowings, (23 supraglottic penetrations and 23 aspirations). Considering each swallowing at 15 fps, sensitivity and specificity were, respectively, 93% (CI 0.82-0.98) and 98% (CI 0.94-0.99). The Cohen'Kappa coefficient between each interpretation at 15 and 30 fps was "almost perfect" (κ = 0.95; CI 0.88-0.99). Considering each loop, conclusion was identical. Reducing frame rate at 15 fps during pediatric VFSS seemed to be acceptable with comparable diagnostic performances without clinical impact compared to 30 fps, while being an efficient way to reduce the ionizing radiation exposition in children. We would suggest reconsidering the possibility of using VFSS with a 15 fps in a pediatric population.

Abstract

Background: Research has shown important between-individual variations in physical activity (PA) during the COVID-19 lockdown.Objectives: The objectives of this is study are to examine the individual, sociodemographic, and environmental factors related to PA during the spring 2020 COVID-19 lockdown in France and to explore the mediating and moderating role of intention and self-efficacy toward PA in the relationships between sociodemographic/environmental variables and PA.Design: In this cross-sectional study, participants living in France (N = 386) completed an online survey between March 30 and April 10, 2020.Method: Minutes per week of moderate-to-vigorous PA during the lockdown; usual physical activity before the lockdown; and psychological (e.g., intention, self-efficacy, and autonomous and controlled motivation), sociodemographic (gender, age, and number of children), and environmental (habitat surface area and type of housing) factors were measured in the survey. Multiple linear regressions were used to investigate the role of these predictors on PA. Intention and self-efficacy were also examined as moderators and mediators of the association between sociodemographic/environmental factors and PA.Results: Usual physical activity before the COVID-19 lockdown, intention toward PA, habitat surface area, and controlled motivation significantly predicted PA during the lockdown. No mediating effects of intention or self-efficacy were found. Intention significantly moderated the association between gender and PA and the association between part-time work and PA.Conclusions: PA during the COVID-19 lockdown was mainly predicted by individual factors and notably usual PA. These results highlight the important role of habits in a highly changing context.

Abstract

International audience; The manual segmentation of multiple organs in 3D ultra-sound (US) sequences and volumes towards their quantitative analysis is very expensive and time-consuming. Fully supervised segmenta-tion methods still require the collection of large volumes of annotated data while unlabeled images are abundant. In this work, we propose a semi-automatic deep learning approach modeled as a weak-label learning problem: given a few 2-D incomplete annotations for selected slices, the goal is to propagate the masks to the entire sequence. To this end, we make use of both positive and negative constraints induced by incomplete labels to penalize the segmentation loss function. Our model is composed of one encoder and two decoders to model the segmentation and an auxiliary reconstruction task. Moreover, we consider the spatio-temporal information by deploying a Convolutional Long Short Term Memory module. Our findings suggest that the reconstruction decoder and the spatio-temporal information lead to a better geometrical estimation of the mask shape. We apply the model to the task of low-limb muscle segmentation in a dataset of 44 patients and 6160 images.

Abstract

The purposes of this study were to investigate the combined effects of age and obesity on gait and to analyze the relationship between age and obesity on ankle muscle activities during walking.4 groups; the young non-obese control group (CG, n = 50, age = 31.8 ± 4.5 years; BMI = 21.4 ± 2.2 kg/mCompared to OB, OBOA walked with higher CoP velocity, shorter stride, spending more time in support phase (p .05). These manifestations were associated with higher TA and SOL activities during the 1st double support (1st DS) and higher TA activity during the single support (SS) (p .05). Compared to OA, OBOA walked with lower GRFv, shorter and wider stride and spend more time in SU (p .05). Moreover, SOL, TA and GM activities of OBOA were higher compared to OAG during 1st DS, SS and 2nd Double support (2nd DS), respectively (p .05). During the 1ST DS, the stepwise multiple regression revealed that age accounted for 87% of the variance of TA activity. The addition of age contributed a further 16% to explain the variance TA activity. During the SS, age accounted for 64% and 46% of the variance of SOL and TA activity respectively. The addition of the body weight added further 15% and 66% of the variation of SOL and TA activity respectively. During the 2nd DS, body weight accounted for 86% of the variance and the addition of the body weight added a further 17% to explain the high level of GM.Age in obese adults and obesity in older adults should be considered separately to evaluate neuromuscular responses during walking and, subsequently, optimize the modality of treatment and rehabilitation processes in obese individuals in order to reduce and/or prevent the risk of falls.

Abstract

International audience; Obesity is known to have a detrimental effect on balance and motor performance during daily motor tasks. However, it remains unclear whether these obesity-related impairments are due to deficient anticipatory postural adjustments (APA) that precede voluntary movement. The objective of this study was to examine the effects of obesity on APA and the impacts related on motor performance and mediolateral postural stability during gait initiation. Fifteen obese and ten normal-weight young participants performed a series of gait initiation at their preferred speed. Our results showed that the durations and amplitudes of APA along both anteroposterior and mediolateral directions did not differ between the two groups (P > 0.05). In contrast, compared to normal-weight participants, mechanical effectiveness of APA was reduced in obese participants (P < 0.05). As a result, we observed a decreased motor performance (P < 0.05), in terms of peak anteroposterior center-of-mass velocity at the end of the first step, and a reduced mediolateral stability at swing foot contact in obese participants compared to normal-weight participants (P < 0.05). These findings suggest that APA effectiveness during gait initiation is reduced in obese adults, resulting in a decrease of both mediolateral stability and motor performance compared to their lean counterparts.

Abstract

It is well established that obesity is associated with deterioration in postural control that may reduce obese adults' autonomy and increase risks of falls. However, neuromuscular mechanisms through which postural control alterations occur in obese adults remain unclear.To investigate the effects of obesity on muscle coactivation at the ankle joint during static and dynamic postural control.A control group (CG; n = 20; age = 32.5 ± 7.6 years; BMI = 22.4 ± 2.2 Kg/m²) and an obese group (OG; n = 20; age = 34.2 ± 5.6 years; BMI = 38.6 ± 4.1 Kg/m²) participated in this study. Static postural control was evaluated by center of pressure (CoP) displacements during quiet standing. Dynamic postural control was assessed by the maximal distance traveled by the CoP during a forward lean test. Electromyography activity data for the gastrocnemius medialis (GM), soleus (SOL) and tibialis anterior (TA) were collected during both quiet standing and forward lean tests. Muscle activities were used to calculate two separate coactivation indexes (CI) between ankle plantar and dorsal flexors (GM/TA and SOL/TA, respectively).CoP displacements were higher in the OG than in the CG for quiet standing (p0.05). When leaning forward, the maximal distance of the CoP was higher in the CG than in the OG (p0.05). Only the CI value calculated for SOL/TA was higher in the OG than in the CG for both static and dynamic tasks (p0.05). The SOL/TA CI value in the OG was positively correlated with CoP displacements during quiet standing (r = 0.79; p0.05).Obesity increases muscle coactivation of the soleus and tibialis anterior muscles at the ankle joint during both static and dynamic postural control. This adaptive neuromuscular response may represent a joint stiffening strategy for enhancing stability. Consequently, increased ankle muscle coactivation could not be considered as a good adaptation in obese adults.

Abstract

Intermittent Claudication due to Peripheral Arterial Disease (PAD-IC) induces ischemic pain in exercising muscles, and therefore impaired gait. In a pathological context, the analysis of the Vertical component of Ground Reaction Force (VGRF) is frequently used to describe gait pattern. This paper aims to define gait profiles according to the relative difference between peaks of VGRF; a Rearfoot and a Forefoot profile revealing a more loading or push-off strategy. We evaluated 70 participants (24 with unilateral disease (Unilat-IC), 22 with bilateral disease (Bilat-IC) and 24 Controls) during a walk test on an instrumented treadmill. Results indicate that Unilat-IC patients present a Rearfoot-profile in both legs during the pain-free gait period, likely to stabilize their gait. With the onset and increase of pain, the asymptomatic leg changes for a Forefoot-profile. This asymmetrical pattern suggests that a compensatory mechanism occurred to unload the symptomatic (painful) leg, possibly creating an imbalance. In Bilat-IC and Controls subjects, a Forefoot-profile is found, with a symmetrical pattern. However, there is a trend for lower propulsive capacity in case of Bilat-IC due to ischemic pain, but patients did not have the ability to compensate as in Unilat-IC. Therefore, Bilat-IC should not be considered as a "double" Unilat-IC. This study highlights the existence of gait profiles based on VGRF in PAD-IC patients. These profiles are dependent on the type of disease. Analysis of these gait profiles can 1) provide a simple way to identify gait alterations and 2) participate in improving physical rehabilitation strategies in PAD-IC patients.

International audience; ObjectiveThe aim of this study was to develop specific prediction equations based on acceleration data measured at three body sites for estimating energy expenditure (EE) during static and active conditions in middle-aged and older adults with and without type 2 diabetes (T2D).Research methodsForty patients with T2D (age: 40–74 yr, body mass index (BMI): 21–29.4 kg·m−2) and healthy participants (age: 47–79 yr, BMI: 20.2–29.8 kg·m−2) completed trials in both static conditions and treadmill walking. For all trials, gas exchange was monitored using indirect calorimetry and vector magnitude was calculated from acceleration data measured using inertial measurement units placed to the participant's center of mass (CM), hip and ankle. Stepwise multiple regression analyses were conducted to select relevant variables to include in the three EE prediction equations, and three Monte Carlo cross-validation procedures were used to evaluate each separate equation.ResultsVector magnitude (p < 0.0001) and personal data (gender, diabetes status and BMI; p < 0.0001) were used to develop three linear prediction equations to estimate EE during static conditions and walking. Cross-validation revealed similar robust coefficients of determination (R2: 0.81 to 0.85) and small bias (mean bias: 0.008 to −0.005 kcal·min−1) for all three equations. However, the equation based on CM acceleration exhibited the lowest root mean square error (0.60 kcal·min−1 vs. 0.65 and 0.69 kcal·min−1 for the hip and ankle equations, respectively; p < 0.001).ConclusionThe three equations based on acceleration data and participant characteristics accurately estimated EE during sedentary conditions and walking in middle-aged and older adults, with or without diabetes.

Abstract

Objective: To evaluate the role of obesity in the effects of physical activity (PA) on postural control and functional and physical capacities in the older adults and to assess the effectiveness of a PA program on these capacities. Methods: Six obese (age = 78.8 [3.7] y; body mass index > 30 kg/m2), 7 overweight (age = 80.9 [2.8] y; 25 2), and 6 normal weight (age = 80.8 [5.7] y; body mass index 2) older adults performed the time up and go test, the 6-minute walk test, and the Tinetti test. Static and dynamic (forward leaning) postural control tests were also assessed. All these tests were similarly assessed 4 months later, during which only the obese group and overweight group participated in a PA program. Results: Before PA, results of the time up and go test, 6-minute walk test, Tinetti test, quiet standing, and forward lean tests revealed that physical capacities and static and dynamic postural control were impaired in the obese group when compared to the normal weight group. After PA, results of quiet standing, physical and functional tests were improved for obese group. Conclusions: Obesity is an additional constraint to age-related postural control and functional and physical capacities deteriorations. Nevertheless, a PA program is effective in improving balance and functional capacities in obese older adults.

Abstract

IntroductionDepression is among the most widespread psychiatric disorders in France. Psychiatric disorders are associated with considerable social costs, amounting to €22.6 billion for treatment and psychotropic medication in 2011. Treatment as usual (TAU), mainly consisting of pharmacotherapy and psychotherapy, is effective for only a third of patients and in most cases fails to prevent treatment resistance and chronicity. Transcranial direct current stimulation (tDCS) consists in a non-invasive and painless application of low-intensity electric current to the cerebral cortex through the scalp. Having proved effective in depressed patients, it could be used in combination with TAU to great advantage. The objective is to compare, for the first time ever, the cost-utility of tDCS-TAU and of TAU alone for the treatment of a depressive episode that has been refractory to one or two drug treatments.Methods and analysisThis paper, based on the DISCO study protocol, focuses on the design of a prospective, randomised, controlled, open-label multicentre economic study to be conducted in France. It will include 214 patients with unipolar or bipolar depression, assigning them to two parallel arms: group A (tDCS-TAU) and group B (TAU alone). The primary outcome is the incremental cost-effectiveness ratio, that is, the ratio of the difference in cost between each strategy to the difference in their effects. Their effects will be expressed as numbers of quality-adjusted life-years, determined through administration of the EuroQol Five-Dimension questionnaire over a 12-month period to patients (EQ-5D-5L). Expected benefits are the reduction of treatment resistance and suicidal ideation as well as social and professional costs of depression. Should depression-related costs fall significantly, tDCS might be considered an efficient treatment for depression.Ethics and disseminationThis protocol has been approved by a French ethics committee, the CPP-–Est IV (Comité de Protection des Personnes–Strasbourg). Abstract

International audience; This article presents an analysis of students’ activity in step. Referring to the theoretical and methodological framework “course of action” (Theureau, 2006), it describes their sensory learnings in step. It is focused on the way students learn to pay attention to their body’s sensations, and how they interpret them. The aim of this study is centered on the construction processes of “perceptive know how” by practitioners, by referring to their experiences. Indeed, building perceptual and sensitive competences seems fundamental to the practitioner, in order to regulate his actions, to better regulate his effort, and for enhancing health and well-being. Finally, this research explores how perceptive know-hows can be developed. From this point onwards, some interventions strategies and singular accompaniment postures can be proposed for actors of education through physical practices.; Cet article présente une analyse de l’activité de lycéens en step. En référence au cadre du cours d’action (Theureau, 2006), il décrit les apprentissages sensoriels de lycéens débutants en step, et plus précisément la manière dont ils apprennent à être attentifs à leurs sensations corporelles et à les interpréter. L’enjeu est de mieux comprendre les processus de construction de « savoir-faire perceptifs » chez les pratiquants, en nous référant à leur vécu. En effet, la construction de compétences perceptives et sensibles apparaît fondamentale chez le pratiquant pour réguler les actions motrices, apprendre à gérer l’effort, mais aussi dans une logique de développement du bien-être et de la santé. Aussi, il s’agit d’explorer le développement de savoir-faire perceptifs, afin de proposer des stratégies d’intervention et des postures d’accompagnement singulières pour des acteurs de l’éducation par les APSA.

Abstract

The objective of this study was to investigate the effect of obesity on markers of neuromuscular fatigability of ankle muscles and their potential relationships with postural control capacities. Two groups: non-obese control group (CG; n = 15; age = 40.1 ± 13.9 years; BMI = 23.9 ± 2.8 kg/m2) and obese group (OG; n = 15; age = 44.2 ± 12.4 years; BMI = 42.2 ± 5.2 kg/m2), performed two postural tasks with and without vision in order to collect parameters of center of pressure (CoP) displacements; area (cm2), velocity (mm/s), length (cm) and the ratio length/area. Abstract

Although it is known that the muscle activation patterns used to produce even simple movements can vary between individuals, these differences have not been considered to prove the existence of individual muscle activation strategies (or signatures). We used a machine learning approach (support vector machine) to test the hypothesis that each individual has unique muscle activation signatures. Eighty participants performed a series of pedaling and gait tasks, and 53 of these participants performed a second experimental session on a subsequent day. Myoelectrical activity was measured from eight muscles: vastus lateralis and medialis, rectus femoris, gastrocnemius lateralis and medialis, soleus, tibialis anterior, and biceps femoris -long head. The classification task involved separating data into training and testing sets. For the within-day classification, each pedaling/gait cycle was tested using the classifier, which had been trained on the remaining cycles. For the between-day classification, each cycle from day 2 was tested using the classifier, which had been trained on the cycles from day 1. When considering all eight muscles, the activation profiles were assigned to the corresponding individuals with a classification rate of up to 99.28% (2,353/2,370 cycles) and 91.22% (1,341/1,470 cycles) for the within-day and between-day classification, respectively. When considering the within-day classification, a combination of two muscles was sufficient to obtain a classification rate >80% for both pedaling and gait. When considering between-day classification, a combination of four to five muscles was sufficient to obtain a classification rate >80% for pedaling and gait. These results demonstrate that strategies not only vary between individuals, as is often assumed, but are unique to each individual. NEW & NOTEWORTHY We used a machine learning approach to test the uniqueness and robustness of muscle activation patterns. We considered that, if an algorithm can accurately identify participants, one can conclude that these participants exhibit discernible differences and thus have unique muscle activation signatures. Our results show that activation patterns not only vary between individuals, but are unique to each individual. Individual differences should, therefore, be considered relevant information for addressing fundamental questions about the control of movement.

Comprendre pourquoi une solution est sélectionnée parmi d’autres est une étape incontournable si l’on veut optimiser le mouvement, que ce soit chez des sujets pathologiques ou des sportifs. Abstract

International audience; Purpose : This study aimed to determine whether an increase in muscle shear modulus measured 30 min after eccentric exercise (30 min) reflects the magnitude of force deficit measured 48-h post-exercise (48 H).Methods : A total of 53 healthy participants were distributed in five groups. Four groups performed either repeated eccentric elbow flexions or knee extensions at either a low or high load. A fifth group performed repeated concentric elbow flexions (control load).Results : A significant decreased peak torque was found for elbow flexors and knee extensors 48 h after the eccentric exercises (all P values < 0.001). A significant increase in shear modulus was found at 30 min for the elbow flexors for low (+70.5 ± 44.3%, P < 0.001) and high load (+153.9 ± 192.4%, P < 0.001). Similarly, the shear modulus of knee extensors increased for low (+26.7 ± 19.1%, P < 0.001) and high load (+79.4 ± 67.1%, P < 0.001). The relative increase in shear modulus measured at 30 min was significantly correlated to the relative decrease in peak torque measured at 48 H for both elbow flexors (r = −0.80) and knee extensors (r = −0.82). A further analysis suggested that biceps brachii and rectus femoris were more affected by muscle damage than their synergists.Conclusion : This study shows that an increase in muscle shear modulus measured 30 min after a damaging exercise reflects the decrease in peak torque measured at 48 H. Abstract

Although cold application (ie, cryotherapy) may be useful to treat sports injuries and to prevent muscle damage, it is unclear whether it has adverse effects on muscle mechanical properties. This study aimed to determine the effect of air-pulsed cryotherapy on muscle stiffness estimated using ultrasound shear wave elastography. Myoelectrical activity, ankle passive torque, shear modulus (an index of stiffness), and muscle temperature of the gastrocnemius medialis were measured before, during an air-pulsed cryotherapy (-30 degrees C) treatment of four sets of 4minutes with 1-minute recovery in between and during a 40minutes postcryotherapy period. Abstract

The present study aimed to assess the ability of muscle stiffness (shear modulus) and response to electrically-induced muscle contraction to detect changes in muscle properties over a 12-month period in children with Duchenne muscular dystrophy (DMD). Ten children with DMD and nine age-matched healthy male controls participated in two experimental sessions (T 0 and T +12months ) separated by 12.4 ± 0.9 months. Two contractions of the biceps brachii were electrically-induced during which an ultrasound probe was placed over the muscle. The resting shear modulus was measured using elastography from six muscles. Evoked maximal torque was increased at T +12months in controls (+11.2 ± 7.6%, P 0.001) but was not modified in children with DMD ( P = 0.222). Electromechanical delay (+12.9 ± 11.3%, P 0.001) and its force transmission component (+10.1 ± 21.6%, P = 0.003) were significantly longer at T +12months than T 0 for children with DMD. The results revealed an increase in muscle stiffness at T +12months in children with DMD for tibialis anterior (+75.1 ± 93.5%, P = 0.043), gastrocnemius medialis (+144.8 ± 180.6%, P = 0.050) and triceps brachii (+35.5 ± 32.2%, P = 0.005). This 12-month follow-up study demonstrates that electromechanical delay and elastography may help detect subtle muscle impairments in patients with DMD. These sensitive outcomes may improve the follow-up of innovative therapeutic interventions within the field of DMD.

Abstract

During stretching studies, surface electromyography (sEMG) is used to ensure the passive state of the muscle, for the characterization of passive muscle mechanical properties. Different thresholds (1%, 2% or 5% of maximal) are indifferently used to set "passive state". This study aimed to investigate the effects of a slight activity on the joint and muscle mechanical properties during stretching. The joint torque and muscle shear modulus of the triceps surae muscles were measured in fifteen healthy volunteers during ankle dorsiflexions: (i) in a "fully relaxed" state, (ii) during active conditions where participants were asked to produce an sEMG amplitude of 1%, 2% or 5% of their maximal sEMG amplitude of the triceps surae. The 1% condition was the only that did not result in significant differences in joint torque or shear modulus compared to the relaxed condition. In the 2% condition, increases in joint torque were found at 80% of the maximal angle in dorsiflexion, and in the shear modulus of gastrocnemius medialis and gastrocnemius lateralis at the maximal angle in dorsiflexion. During the 5% condition, joint torque and the shear modulus of gastrocnemius medialis were higher than during relaxed condition at angles larger than 40% of maximal angle in dorsiflexion. The results provide new insights on the thresholds that should be considered for the design of stretching studies. A threshold of 1% seems much more appropriate than a 2% or 5% threshold in healthy participants. Further studies are required to define similar thresholds for patients.

Abstract

International audience; Purpose: Muscle–tendon units are forcefully stretched during rapid deceleration events such as landing. Consequently, tendons act as shock absorbers by buffering the negative work produced by muscle fascicles likely to prevent muscle damage. Landing surface properties can also modulate the amount of energy dissipated by the body, potentially effecting injury risk. This study aimed to evaluate the influence of three different surfaces on the muscle–tendon interactions of gastrocnemius medialis (GM), and vastus lateralis (VL) during single- and double-leg landings from 50 cm.Methods: Ultrasound images, muscle activity and joint kinematics were collected for 12 participants. Surface testing was also performed, revealing large differences in mechanical behavior.Results: During single-leg landing, stiffer surfaces increased VL fascicle lengthening and velocity, and muscle activity independent of joint kinematics while GM length changes showed no difference between surfaces. Double-leg landing resulted in similar fascicle and tendon behavior despite greater knee flexion angles on stiffer surfaces.Conclusion: This demonstrates that VL fascicle lengthening is greater when the surface stiffness increases, when performing single-leg landing. This is due to the combination of limited knee joint flexion and lower surface absorption ability which resulted in greater mechanical demand mainly withstood by fascicles. GM muscle–tendon interactions remain similar between landing surfaces and types. Together, this suggests that surface damping properties primarily affect the VL muscle–tendon unit with a potentially higher risk of injury as a result of increased surface stiffness when performing single-leg landing tasks.

Abstract

Background: Obstetrical analgesia remains a matter of controversy because of the fear of neurotoxicity of local anesthetics on demyelinated fibers or their potential relationship with subsequent relapses. Objective: To assess the impact of neuraxial analgesia on the risk of relapse during the first 3 months post-partum, with a focus on women who experienced relapses during pregnancy. Methods: We analyzed data of women followed-up prospectively during their pregnancies and at least 3 months post-partum, collected in the Pregnancy in Multiple Sclerosis (PRIMS) and Prevention of Post-Partum Relapses with Progestin and Estradiol in Multiple Sclerosis (POPARTMUS) studies between 1992–1995 and 2005–2012, respectively. The association of neuraxial analgesia with the occurrence of a post-partum relapse was estimated by logistic regression analysis. Results: A total of 389 women were included, 215 from PRIMS and 174 from POPARTMUS. In total, 156 women (40%) had neuraxial analgesia. Overall, 24% experienced a relapse during pregnancy and 25% in the 3 months post-partum. Women with a pregnancy relapse were more likely to have a post-partum relapse (odds ratio (OR) = 1.83, p = 0.02), independently of the use of neuraxial analgesia. There was no association between neuraxial analgesia and post-partum relapse (OR = 1.08, p = 0.78). Conclusion: Neuraxial analgesia was not associated with an increased risk of post-partum relapses, whatever multiple sclerosis (MS) activity during pregnancy.

Abstract

Purpose:The aim of the current study was to determine whether the anaerobic performance assessed by a modified Wingate test could account for the 1,500-m rowing ergometer performance in young competitive rowers.Methods:Fourteen national-level adolescent rowers performed on a rowing ergometer i) a 30-s allout exercise (i.e., modified Wingate test) and ii) a 1,500-m all-out exercise (i.e., rowing performance). For each of these two exercises, the mean power output was considered as the main performance criterion (PWIN and P1500, respectively).Results:PWIN was correlated to P1500 (r2 = .83, p < .0001, β = 100%). Furthermore, the estimated total muscle mass was correlated to PWIN and P1500 (r2 = .72 and r2 = .83, p < .0001, β = 100%, respectively). The allometric scaling also indicated that total muscle mass accounts 62% for the relationship between PWIN and P1500.Conclusion:Our results highlight the importance of considering the contribution of total muscle mass and anaerobic energy pathways to 1,500-m rowing performance in competitive adolescent rowers. Therefore, the modified Wingate test could be used by rowing coaches to potentially identify talented young rowers.

Abstract

International audience; OBJECTIVE: The current study aimed to examine whether specific features of psychomotor retardation (PMR) and cognitive functioning established different profiles in unipolar (UD) and bipolar depression (BD). METHODS: Two groups of age-matched patients with UD (n=54) and BD (n=20) completed the Montgomery-Asberg Depression Rating Scale (MADRS/60), the Montreal Cognitive Assessment (MoCA/30), and the Salpêtrière Retardation Rating Scale (SRRS/60). We analyzed the group effect and then performed intra-group analyses. RESULTS: The BD patients have higher SRRS score, and lower MoCA score than UD despite no difference on the level of depression between UD and BD. Our results show that PMR can be predicted by the level of depression in UD and by the cognitive alteration and onset of disease in BD. CONCLUSION: PMR is a relevant marker of depression. Our results highlight the importance of concomitant evaluation of psychomotor and cognitive functions in the distinction of UD and BD symptoms.

International audience; PurposeThis study aimed to assess muscle coactivation in quadriceps and hamstring muscles using ultrasound shear wave elastography.MethodsDuring maximal voluntary isometric contractions (MVC), both myoelectrical activity and shear modulus of antagonist muscles were measured (i.e., rectus femoris, vastus lateralis and vastus medialis during knee flexions and semitendinosus, semimembranosus and biceps femoris long head during knee extensions). To account for changes induced by inevitable joint rotation during MVC, the shear modulus values were compared to those measured at the same knee angle during a passive cycle. Abstract

Summary Objectives Due to its ease of use, tolerance, and cost of acquisition, transcranial direct current stimulation (tDCS) could constitute a credible therapeutic option for non-resistant depression in primary care, when combined with drug management. This indication has yet to receive official recognition in France. The objective of this study is to evaluate the production cost of tDCS for the treatment of depression in hospitals, under realistic conditions. Methods The methodology adopted is based on cost accounting and was validated by a multidisciplinary working group. It includes equipment, staff, and structural costs to obtain the most realistic estimate possible. We first estimated the cost of producing a tDCS session, based on our annual activity objective, and then estimated the cost of a 15-session treatment program. This was followed up with a sensitivity analysis applying appropriate parameters. Results The hospital production cost of a tDCS depression treatment program for a single patient was estimated at €1555.60 euros: €99 in equipment costs, €1076.95 in staff costs, and €379.65 in structural costs. Conclusion This cost analysis should make it possible to draw up pricing proposals in compliance with regulations and health policy choices and to develop health-economic studies. This would ultimately lead to official recognition of tDCS treatment for depression in France and pave the way for studying various scenarios of coverage by the French national health insurance system.

Abstract

Objective The aim of the study was to investigate the influence of age and/or obesity on postural control, ankle muscle activities during balance testing and force production capacities. Materials and methods 4 groups; control group (CG; n = 25; age = 31.8 ± 7.5 years; BMI = 21.4 ± 2.5 kg/m2), obese group (OG; n = 25; age = 34.4 ± 9.5 years; BMI = 39.6 ± 5.4 kg/m2), elderly group (EG; n = 15; age = 77.1 ± 8.4 years; BMI = 24.4 ± 1.3 kg/m2) and obese elderly group (ObEG; n = 12; age = 78.6 ± 6.6 years; BMI = 34.5 ± 3.1 kg/m2) performed maximal voluntary contraction (MVC) before testing to calculate the maximal relative force of ankle plantar flexor (PF) and dorsal flexor (DF) muscles. Center of pressure (CoP) parameters and the electromyography (EMG) activity of PF and DF muscles were collected during MVC, quiet standing and limit of stability (LoS) testing along antero-posterior and medio-lateral axes. Results Maximal relative force was higher in EG and ObEG than CG and OG, respectively (p 0.6; p −0.6; p Conclusion Obesity-related postural control alteration is associated with increased activity of PF. This neuromuscular adaptation may reflect deteriorations of the proprioceptive system and is likely additional to age-related muscular impairments. This may be a mechanism by which obesity increases postural control alterations in elderly.

Abstract

The torque-sharing strategies between synergistic muscles may have important functional consequences. This study involved two experiments. The first experiment ( n = 22) aimed 1) to determine the relationship between the distribution of activation and the distribution of torque-generating capacity among the heads of the hamstring, and 2) to describe individual torque-sharing strategies and to determine whether these strategies are similar between legs. The second experiment ( n = 35) aimed to determine whether the distribution of activation between the muscle heads affects endurance performance during a sustained submaximal knee flexion task. Surface electromyography (EMG) was recorded from biceps femoris (BF), semimembranosus (SM), and semitendinosus (ST) during submaximal isometric knee flexions. Torque-generating capacity was estimated by measuring muscle volume, fascicle length, pennation angle, and moment arm. The product of the normalized EMG amplitude and the torque-generating capacity was used as an index of muscle torque. The distributions of muscle activation and of torque-generating capacity were not correlated significantly (all P > 0.18). Thus, there was a torque imbalance between the muscle heads (ST torque > BF and SM torque; P < 0.001), the magnitude of which varied greatly between participants. A significant negative correlation was observed between the imbalance of activation across the hamstring muscles and the time to exhaustion ( P < 0.001); i.e., the larger the imbalance of activation across muscles, the lower the muscle endurance performance. Torque-sharing strategies between the heads of the hamstrings are individual specific and related to muscle endurance performance. Whether these individual strategies play a role in hamstring injury remains to be determined. NEW & NOTEWORTHY The distribution of activation among the heads of the hamstring is not related to the distribution of torque-generating capacity. The torque-sharing strategies within hamstring muscles vary greatly between individuals but are similar between legs. Hamstring coordination affects endurance performance; i.e., the larger the imbalance of activation across the muscle heads, the lower the muscle endurance.

Abstract

International audience; Aim: Defining the origins of muscle injury has important rehabilitation and exercise applications. However, current knowledge of muscle damage mechanics in human remains unclear in vivo. This study aimed to determine the relationships between muscle–tendon unit mechanics during maximal eccentric contractions and the extent of subsequent functional impairments induced by muscle damage.Methods: The length of the muscle–tendon unit, fascicles and tendinous tissues was continuously measured on the gastrocnemius medialis using ultrasonography, in time with torque, during 10 sets of 30 maximal eccentric contractions of plantar flexors at 45°s -1, in seventeen participants.Results: Muscle–tendon unit, fascicles and tendinous tissues were stretched up to 4.44 ± 0.33 cm, 2.31 ± 0.64 cm and 1.92 ± 0.61 cm respectively. Abstract

International audience; Automation in industries reduced the human effort, but still there are many manual tasks in industries which lead to musculo-skeletal disorder (MSD). Muscle fatigue is one of the reasons leading to MSD. The objective of this article is to experimentally validate a new dynamic muscle fatigue model taking co-contraction factor into consideration using electromyo-graphy (EMG) and Maximum voluntary contraction (MVC) data. A new model (Seth's model) is developed by introducing a co-contraction factor 'n' in R. Ma's dynamic muscle fatigue model. The experimental data of ten subjects are used to analyze the muscle activities and muscle fatigue during extension-flexion motion of the arm on a constant absolute value of the external load. The findings for co-contraction factor shows that the fatigue increases when co-contraction index decreases. The dynamic muscle fatigue model is validated using the MVC data, fatigue rate and co-contraction factor of the subjects. It has been found that with the increase in muscle fatigue, co-contraction index decreases and 90% of the subjects followed the exponential function predicted by fatigue model. The model is compared with other models on the basis of dynamic maximum endurance time (DMET). The co-contraction has significant effect on the muscle fatigue model and DMET. With the introduction of co-contraction factor DMET decreases by 25.9% as compare to R. Ma's Model.

Abstract

Purpose:To evaluate the effects of muscle fatigue on badminton performance during a smash stroke.Methods:In total, 17 young, well-trained players completed 20 forehand smashes twice (prefatigue and postfatigue protocol), and both speed and precision of the strokes were measured. The fatigue protocol consisted of 10 series of 10 maximal countermovement jumps (3-s rest in between) followed by 8 lunges. Perception of effort and countermovement-jump performance during each series were also measured to assess fatigue.Results:Shuttlecock speed decreased moderately (−3.3%) but significantly after the fatigue protocol (P ). Precision significantly decreased after the fatigue protocol (−10.3%,P = .001,). The decrease in precision was mainly due to an increased number of faults (P = .006,,dz = 0.756) and to a decrease in accuracy (P = .066,,dz = 0.478).Conclusion:The present study showed that fatigue impairs performance during specific badminton skills. Moreover, by showing a slight decrease in speed and a large decrease in accuracy of the shuttlecock when fatigue is experienced, the present study suggested that, as previously observed in other racket sports, the speed of the missile appears to be the key factor used by the players to win the rally. Coaches and physical trainers should therefore develop interventions aiming to limit the negative impact of fatigue on badminton strokes.

Abstract

Upper limb abilities can be assessed from different kinds of exercises (e.g., cranking, push-up, and medicine ball put test). Since the bench press is a very common exercise used in training routines by most athletes in many sports, its interest in the scientific literature raised in the past two decades. As presented in previous chapters during jumping or cycling, from several bench presses performed against different loads, coaches and athletes can simply and accurately define their upper limb force-velocity (F-v) profile. They can estimate their theoretical maximal force (F 0), velocity (V 0) and power (P max ). The aim of this chapter is to present the important points that must be taken into account to optimize the use of the bench press as a routine testing. In a first part, this chapter will focus on the importance of taking into consideration all the mechanical inertia involved in the bench press exercise. Not considering the upper limb mass in the calculation of the force produced during the exercise implies an underestimation in the F-v profile that can reach 30% for P max . This could conduct to incorrect choices in the optimal load during training and thus limit the performance improvement. In the second part of this chapter, we present a simple mechanical model of the bench press to study the importance of the upper limb acceleration in the estimation of the force produced. The moving system (i.e., lifted mass and upper limbs) is modeled from rigid segments and the force can be determined thanks to four simple measurements: the vertical displacement of the lifted load, the elbow angle measured using a goniometer, the arm and forearm lengths and the constant horizontal position of the hand on the barbell. The validity of this model has been confirmed through experimental data obtained from a force platform. An important point is that the kinematics and kinetics of this model allow demonstrating that the acceleration of the moving system is similar to the one of the barbell. Finally, based on the previous statement, the last part of this chapter presents a simple method for assessing force, velocity and power during a ballistic bench press performed on a traditional guided barbell, based on the Newtonian laws and only three simple parameters: (i) upper limb mass estimated as 10% of the body mass; (ii) barbell flight height recorded with a nylon cable tie and (iii) push-off distance measured with a measuring tape. Consequently, coaches and athletes could accurately determine their F-v profile and extrapolate reliable mechanical parameters (F 0 , v 0, Sfv and P max ) in order to maximize upper limbs performance and manage training programs in field conditions.

Abstract

International audience; This study introduces local pattern texture anisotropy as a novel parameter to differentiate healthy and disordered muscle and to gauge the severity of muscle impairments based on B-mode ultrasound images. Preliminary human results are also presented. A local pattern texture anisotropy index (TAI) was computed in one region of interest in the short head of the biceps brachii. The effects of gain settings and box sizes required for TAI computation were investigated. Between-day reliability was studied in patients with sporadic inclusion body myositis (n = 26). The ability of the TAI to discriminate dystrophic from healthy muscle was evaluated in patients with Duchenne muscular dystrophy and healthy controls (n = 16). TAI values were compared with a gray-scale index (GSI). TAI values were less influenced by gain settings than were GSI values. TAI had lower between-day variability (typical error = 2.3%) compared with GSI (typical error = 2.3% vs. 8.3%, respectively). Abstract

The application of a series of extremely high accelerative motor-driven quick releases while muscles contract isometrically (i.e. slack test) has been proposed to assess unloaded velocity in human muscle. This study aimed to measure gastrocnemius medialis fascicle (VF) and tendinous tissues shortening velocity during motor-driven quick releases performed at various activation levels to assess the applicability of the slack test method in human. Maximal fascicle shortening velocity and joint velocity recorded during quick releases and during fast contraction without external load (ballistic condition) were compared. Gastrocnemius medialis fascicle behaviour was investigated from 25 participants using high-frame rate ultrasound during quick releases performed at various activation levels (from 0% to 60% of maximal voluntary isometric torque) and ballistic contractions. Unloaded joint velocity calculated using the slack test method increased whereas VF decreased with muscle activation level (P≤0.03). Passive and low-level quick releases elicited higher VF values (≥ 41.4±9.7 cm.s−1) compared to ballistic condition (36.3±8.7 cm.s−1), while quick releases applied at 60% of maximal voluntary isometric torque produced the lowest VF. These findings suggest that initial fascicle length, complex fascicle-tendon interactions, unloading reflex and motor-driven movement pattern strongly influence and limit the shortening velocity achieved during the slack test. Furthermore, VF elicited by quick releases is likely to reflect substantial contributions of passive processes. Therefore, the slack test is not appropriate to assess maximal muscle shortening velocity in vivo.

International audience; Introduction Because the hand joints possess a broad range of motion, the muscle length can vary importantly which might result in significant variations of the muscle force-generating capacities. However, facing the complexity of this musculoskeletal system, no study has examined the effect of hand muscle length change on muscle force. This study aimed to characterize the force-length relationship of muscles involved in wrist and metacarpophalangeal flexion.Methods Eleven participants performed two sessions: (i) one for the wrist flexor muscles and (ii) one for the finger flexor muscles. For each session, the participants performed two maximal voluntary contractions and then two progressive isometric ramps from 0% to 100% of their maximal force capacity at five different wrist/metacarpophalangeal angles. An ultrasound scanner was used to measure the myotendinous junction displacement of flexor carpi radialis (FCR) and flexor digitorum superficialis (FDS) during isometric contractions. A three-dimensional relationship between muscle length, force, and activation level was modeled using optimization procedure.Results Globally, the FCR was stronger and shorter compared with FDS. The results showed that the three-dimensional relationships fitted well the experimental data (mean R-2 = 0.92 0.07 and 0.87 +/- 0.11 for FCR and FDS, respectively). Using joint angle and EMG data, this approach allows to estimate the muscle force with low estimation errors (9% of F-max).Conclusions This study proposes a new method to investigate the force-length relationship by combining ultrasound measurement, musculoskeletal modeling and optimization procedures. The data and relationships provide a new insight into hand biomechanics and muscle function that could be useful for designing hand tools or surgical operations.

Abstract

International audience; We investigated the effects of the initial length of the muscle- tendon unit (MTU) and muscle pre- activation on muscle- tendon interactions during plantarflexion performed at maximal velocity. Ultrasound images of gastrocnemius medialis were obtained on 11 participants in three conditions: (a) active plantarflexion performed at maximal velocity from three increasingly stretched positions (10°, 20°, and 30° dorsiflexion), (b) passive plantarflexion induced by a quick release of the ankle joint from the same three positions, and (c) pre- activation, which consisted of a maximal isometric contraction of the plantarflexors at 10° of dorsiflexion followed by a quick release of ankle joint. During the active condition at maximal velocity, initial MTU stretch positively influenced ankle joint velocity (+15.3%) and tendinous tissues shortening velocity (+37.6%) but not the shortening velocity peak value reached by muscle fascicle. The muscle fascicle was shortened during the passive condition; however, its shortening velocity never exceeded peak velocity measured in the active condition. Muscle pre- activation resulted in a considerable increase in ankle joint (+114.7%) and tendinous tissues velocities (+239.1%), although we observed a decrease in muscle fascicle shortening velocity. During active plantarflexion at maximal velocity, initial MTU length positively influences ankle joint velocity by increasing the contribution of tendinous tissues. Although greater initial stretch of the plantarflexors (ie, 30° dorsiflexion) increased the passive velocity of the fascicle during initial movement, its peak velocity was not affected. As muscle pre- activation prevented reaching the maximal muscle fascicle shortening velocity, this condition should be used to characterize tendinous tissues rather than muscle contractile properties.

Abstract

International audience; This case study examined how two rowers adapted their rowing patterns following crew training as a newly formed coxless pair. The two participants were expert (double-oar) single scull-boat rowers. Performing as a crew in the coxless-pair's sweep-boat, where each rower operates a single oar, on-the-water data were collected before and after a 6-week intensive team-training program. Rowing patterns were characterized by the horizontal oar angle, oar angular velocity and linear oar-water velocity profiles during the catch (minimal oar angle) to finish (maximal oar angle) half-cycles of the propulsive water phase. After crew training, rowers demonstrated a tighter synchronization and a closer correspondence in oar angle at the moment of catch, together with a closer matching of the evolution over time of their subsequent oar movements. Most likely due to the inherent asymmetries involved in sweep-boat rowing, the stroke rower also developed a somewhat longer-duration larger-amplitude oar movement than the bow rower. Remarkably, both rowers revealed changes in the inter-cycle variability of their individual patterns of rowing. While the initially more variable stroke rower improved the consistency of his rowing pattern over practice, the initially highly consistent bow rower on the contrary relaxed his tendency to always perform in the same way. We discuss how the crew performance changed over training and to what extent it was associated with changes in individual behaviors. Along the way we demonstrate that the often-used measure of average continuous relative phase does not adequately capture the particularities of the coordination pattern observed. Overall, the results obtained at the individual level of analysis suggest that team benefits were obtained through distinct adaptations of the rowers' individual rowing patterns.

Abstract

International audience; Reaction time (RT) is classically divided into premotor time (PMT) and electromechanical delay (EMD). However, the determination of the onset of electromyographic activity (EMG) during voluntary contraction remains questionable. In addition, the reliability of RT, PMT and EMD needs to be determined. Twelve participants performed two sessions of RT trials, separated by 5 min. RT was evaluated during voluntary isometric contractions of the elbow flexors, i.e., time between a light signal (stimulus) and the onset of the mechanical response. To assess EMD, an electrode array (64 channels) was used to accurately detect the onset of EMG activity. PMT represented the major part of the RT (~88%). Coefficients of variation were reasonably satisfactory for all parameters (range: 11·9-13·4%). The use of electrode array appears to be a relevant method to measure EMD. Moreover, sessions based on two trials are reliable enough to detect changes in RT components.

Abstract

Muscle fascicles–tendon interactions are the main determinant in production of high joint velocity. Currently, no study has investigated the muscle fascicles behaviour of knee extensor muscles until the highest reachable velocity (e.g., unloaded knee extension). We aimed to track the changes in vastus lateralis fascicles length during knee extensions to quantify muscle fascicles and tendinous tissues contributions to muscle–tendon unit shortening and to determine maximal muscle fascicles shortening velocity. Fifteen participants performed isokinetic and isoinertial knee extensions, and ultrafast ultrasound imaging was used to observe the vastus lateralis fascicles from low to very high joint velocity. The muscle fascicles shortening velocity increased linearly with the increase in knee joint velocity up to the maximal joint velocity (mean R 2 = 0.93 ± 0.08). Muscle fascicles contribution to muscle–tendon unit shortening velocity was almost constant regardless of the condition (83 ± 23%). Using Hill’s equation, the maximal velocity of knee joint and muscle fascicles was determined at 1000 ± 489°s−1 and 5.1 ± 2.0 L0 s−1 (47.4 ± 18.7 cm s−1), respectively. Contribution of muscle fascicles to the muscle–tendon unit shortening velocity was much higher for the vastus lateralis in this study compared to the gastrocnemius medialis in two previous studies. Moreover, this contribution of muscle fascicles shortening velocity was constant whatever the velocity condition, even at the highest reachable velocity. Thus, the vastus lateralis fascicles shortening velocity increases linearly with the knee joint velocity until high velocities and its behaviour strongly accorded with the classical Hill’s force–velocity relationship.

Abstract

This study examined the age-related decline in endurance running performance of one of the greatest master runners ever, Ed Whitlock, who died recently. His running performances from 1500 m to marathon were analyzed for 5 periods of 5 years from 65–69 years to 85–89 years. Despite exceptional running performances for his advanced age, the rate of decline in his performances increased after 80 years and was drastically amplified after 85 years.

Abstract

International audience; This study investigated the effects of active and/or passive warm-up tasks on the hamstring muscles stiffness through elastography and passive torque measurements. On separate occasions, fourteen males randomly completed four warm-up protocols comprising Control, Cycling, Foam rolling, or Cycling plus Foam rolling (Mixed). The stiffness of the hamstring muscles was assessed through shear wave elastography, along with the passive torque-angle relationship and maximal range of motion (ROM) before, 5, and 30 minutes after each experimental condition. At 5 minutes, Cycling and Mixed decreased shear modulus (−10.3% ± 5.9% and −7.7% ± 8.4%, respectively; P≤.0003, effect size [ES]≥0.24) and passive torque (−7.17% ± 8.6% and −6.2% ± 7.5%, respectively; P≤.051, ES≥0.28), and increased ROM (+2.9% ± 2.9% and +3.2% ± 3.5%, respectively; P≤.001, ES≥0.30); 30 minutes following Mixed, shear modulus (P=.001, ES=0.21) and passive torque (P≤.068, ES≥0.2) were still slightly decreased, while ROM increased (P=.046, ES=0.24). Foam rolling induced “small” immediate short-term decreases in shear modulus (−5.4% ± 5.7% at 5 minutes; P=.05, ES=0.21), without meaningful changes in passive torque or ROM at any time point (P≥.12, ES≤0.23). These results suggest that the combined warm-up elicited no acute superior effects on muscle stiffness compared with cycling, providing evidence for the key role of active warm-up to reduce muscle stiffness. The time between warm-up and competition should be considered when optimizing the effects on muscle stiffness.
