Abstract

International audience; We report a case of a needlefish jaws retained near the C5-C6 joint that was associated with chronic pain and inflammation and seen confirmed by FDG-PET scan. Two unsuccessful surgeries using an anterior approach were complicated by vascular and nerve injuries. We used image-guided surgery with a posterior approach.

Abstract

Highlights: • Non-linear iterative structural normalization method focused on the deep brain. • Multi-modality image data from deep brain stimulation patients. • Comparison of ANTS, FNIRT and DRAMMS for the non-linear registrations using different settings for each. • Evaluation of the registration tools based on the analysis of 58 structures of the deep brain segmented manually by a single expert. • ANTS was identified as the best performing non-linear registration tool., Deep brain stimulation (DBS) therapy requires extensive patient-specific planning prior to implantation to achieve optimal clinical outcomes. Collective analysis of patient’s brain images is promising in order to provide more systematic planning assistance. In this paper the design of a normalization pipeline using a group specific multi-modality iterative template creation process is presented. The focus was to compare the performance of a selection of freely available registration tools and select the best combination. The workflow was applied on 19 DBS patients with T1 and WAIR modality images available. Non-linear registrations were computed with ANTS, FNIRT and DRAMMS, using several settings from the literature. Registration accuracy was measured using single-expert labels of thalamic and subthalamic structures and their agreement across the group. The best performance was provided by ANTS using the High Variance settings published elsewhere. Neither FNIRT nor DRAMMS reached the level of performance of ANTS. The resulting normalized definition of anatomical structures were used to propose an atlas of the diencephalon region defining 58 structures using data from 19 patients.

Abstract

Rationale Occult tumor cell invasion along white matter fibers is inapparent in conventional MRI. We aimed to precise glioblastomas extension on Diffusion Tensor Imaging (DTI) through a prospective and comparative study. Patients, material and methods Glioblastomas (n=15), metastases (n=9) and healthy controls (n=15) were included. DTI parameters were analyzed, particularly in the normal appearing white matter (NAWM) beyond peritumoral area. Results Specific DTI abnormalities were found in glioblastomas such as hyperanisotropia and morphological modifications in the NAWM.

Abstract

International audience; Mastering detailed anatomy of the human deep brain in clinical neurosciences is challenging. Although numerous pioneering works have gathered a large dataset of structural and topographic information, it is still difficult to transfer this knowledge into practice, even with advanced magnetic resonance imaging techniques. Thus, classical histological atlases continue to be used to identify structures for stereotactic targeting in functional neurosurgery. Physicians mainly use these atlases as a template co-registered with the patient's brain. However, it is possible to directly identify stereotactic targets on MRI scans, enabling personalized targeting. In order to help clinicians directly identify deep brain structures relevant to present and future medical applications, we built a volumetric MRI atlas of the deep brain (MDBA) on a large scale (infra millimetric). Twelve hypothalamic, 39 subthalamic, 36 telencephalic, and 32 thalamic structures were identified, contoured, and labeled. Nineteen coronal, 18 axial, and 15 sagittal MRI plates were created. Although primarily designed for direct labeling, the anatomic space was also subdivided in twelfths of AC-PC distance, leading to proportional scaling in the coronal, axial, and sagittal planes. This extensive work is now available to clinicians and neuroscientists, offering another representation of the human deep brain ([https:// hal.archives-ouvertes.fr/] [hal-02116633]). The atlas may also be used by computer scientists who are interested in deciphering the topography of this complex region.

Abstract

Background We report the first documented case of visual hallucinations with disembodiment and selfmotion illusion on eye closure. Clinical presentation A 71-year-old woman was operated on for a cyst of the left centrum semiovale, adjacent to the precuneus, the posterior cingulate cortex and the splenium of corpus callosum. Two days after, she complained of visual hallucinations occurring on eye closure. She reported reminiscences of landscapes, trees and animals. She had a sensation of floating and moving, flying over the landscapes or walking on lanes. She could stop the hallucinations by opening her eyes. The hallucinations lasted for six days. Pharmacological and neurological possible origins of the hallucinations were explored. Conclusion No direct relation could be found between the pharmacological treatment and the hallucinations. Cerebral volumetric analysis showed a structural reorganization following cyst drainage, which might have conducted to transient hyper excitability of the surrounding structures. The release phenomenon induced by eye closure might have triggered the activation of a distributed network within associative visual, parietal and temporal cortices. The involvement of the splenium and the cingulum is discussed. Furthermore, MRI- and DTI-follow-up suggests that structural reorganization after surgery can be spread over long periods until disappearance of cyst-related deformations. Stabilization of cyst volume, near to zero, was observed after about 2 years.

Abstract

BACKGROUND To date, no study has compared the evolution of the foramen magnum area (FMA) and the posterior cranial fossa volume (PCFV) with the degree of cranial base synchondrosis ossification. OBJECTIVE To illustrate these features in healthy children. METHODS The FMA, the PCFV, and the ossification of 12 synchondroses according to the Madeline and Elster scale were retrospectively analyzed in 235 healthy children using millimeter slices on a computed tomography scan. RESULTS The mean FMA of 6.49 cm in girls was significantly inferior to the FMA of 7.67 cm in boys (P < .001). In both sexes, the growth evolved in a 2-phase process, with a phase of rapid growth from birth to 3.75 years old (yo) followed by a phase of stabilization. In girls, the first phase was shorter (ending at 2.6 yo) than in boys (ending at 4.33 yo) and proceeded at a higher rate. PCFV was smaller in girls (P < .001) and displayed a biphasic pattern in the whole population, with a phase of rapid growth from birth to 3.58 yo followed by a phase of slow growth until 16 yo. In girls, the first phase was more active and shorter (ending at 2.67 yo) than in boys (ending at 4.5 yo). The posterior interoccipital synchondroses close first, followed by the anterior interoccipital and occipitomastoidal synchondroses, the lambdoid sutures simultaneously, then the petro-occipital and spheno-occipital synchondroses simultaneously. CONCLUSION The data provide a chronology of synchondrosis closure. We showed that FMA and PCFV are constitutionally smaller in girls at birth (P ≤ .02) and suggest that a sex-related difference in the FMA is related to earlier closure of anterior interoccipital synchondroses in girls (P = .01). Poster; International audience; Purpose : Saccadic eye movements (SCM) are under the influence of the basal ganglia (BG) and SCM circuitry within the BG represents a good model for studying pathology in this system. The diagnosis of Parkinson’s disease (PD) which affects SCM and its distinction from non-dopaminergic, essential tremor (ET) can be challenging and relies on clinical observations. Diffusion tensor imaging and fiber tractography (DTI-FT) a new MRI technology, can be used to evaluate the presence and integrity of white matter tracts using directional diffusion patterns of water. The purpose of this study is to use DTI-FT to analyze SCM networks within BG and compare PD and ET Methods : We analyzed DTI-MRI images of 30 human brains: 20 pre-operative patients undergoing deep brain stimulation treatment for tremors – 10 PD and 10 ET; and 10 healthy control subjects (CS). 1.5 and 3 Tesla MRI were used for the image acquisition and BrainLab iplan 3.0 software used for mapping regions of interest (ROI) and deriving color-coded white matter tracts or tractography. BG pathways known to be involved with SCM were identified and the volume and size of the bundles representing their probabilistic connections measured. For each subject the 4 ROIs of BG: caudate nucleus (CN), external globus pallidus (GPe), subtantia nigra (SN) and subthalamic nucleus (STN); pedunculopontine tegmental nucleus (PPN) and superior colliculus (SC) were outlined in both hemispheres. Abstract

We used the method of direct electrostimulation combined with navigated tractography to map the frontal “ aslant ” tract, enabling us to perform reliable anatomo – functional correlation in a patient undergoing awake surgery to remove a left frontal glioma. The fi ndings enhanced our understanding of the organization of language within the human brain.

Abstract

International audience; Aim: Brain magnetic resonance imaging (MRI) motor development score (MDS) correlations were used to analyze the natural time-course of hypomyelinating PLP1-related disorders (Pelizaeus-Merzbacher disease [PMD] and spastic paraplegia type 2).Method: Thirty-five male patients (ranging from 0.7-43.5y at the first MRI) with PLP1-related disorder were prospectively followed over 7 years. Patients were classified according to best motor function acquired before 5 years (MDS) into five categories (from PMD0 without motor acquisition to PMD4 with autonomous walking). We determined myelination and atrophy scores and measured corpus callosum area, volume of cerebellum, white matter and grey matter on 63 MRI.Results: Age-adjusted multivariate analysis revealed that patients with PMD0-1 had higherseverity atrophy scores and smaller corpus callosum area than did patients with PMD2 and PMD3-4. Myelination score increased until 12 years. There was evidence that the mean myelination differed in frontal white matter, arcuate fibres, and internal capsules among the groups. Most patients showed worsening atrophy (brain, cerebellum, corpus callosum), whereas grey matter and white matter proportions did not change.Interpretation: Brain atrophy and myelination of anterior cerebral regions appear to be pertinent biomarkers of motor development. The time-course of inter-and intra-individual cerebral white matter and grey matter atrophy suggests that both oligodendrocytes and neurons are involved in the physiopathology of PLP1-related disorders

Abstract

International audience; Objective: Deep brain mapping has been proposed for direct targeting in stereotactic functional surgery, aiming to personalize electrode implantation according to individual MRI anatomy without atlas or statistical template. We report our clinical experience of direct targeting in a series of 156 patients operated on using a dedicated Inversion Recovery Turbo Spin Echo sequence at 1.5-tesla, called White Matter Attenuated Inversion Recovery (WAIR).Methods: After manual contouring of all pertinent structures and 3D planning of trajectories, 312 DBS electrodes were implanted. Detailed anatomy of close neighbouring structures, whether gray nuclei or white matter regions, was identified during each planning procedure. We gathered the experience of these 312 deep brain mappings and elaborated consistent procedures of anatomical MRI mapping for pallidal, subthalamic and ventral thalamic regions. We studied the number of times the central track anatomically optimized was selected for implantation of definitive electrodes.Results: WAIR sequence provided high-quality images of most common functional targets, successfully used for pure direct stereotactic targeting: the central track corresponding to the optimized primary anatomical trajectory was chosen for implantation of definitive electrodes in 90.38%.Conclusion: WAIR sequence is anatomically reliable, enabling precise deep brain mapping and direct stereotactic targeting under routine clinical conditions

Abstract

The action of synthetic progestogens, prescribed at a conventional dose in women, for a meningioma, is still poorly understood, and could be related to progesterone receptors. We report two cases illustrating multiple meningiomas with stabilization or tumor reduction after withdrawal of cyproterone acetate originally prescribed for a long term period. We also review the influence of synthetic progestogens on meningiomas, particularly the impact of treatment withdrawal.

Abstract

Introduction Central nervous system tumors (CNST) are the most lethal of solid tumors in childhood cancer. Patients and methods We report incidence and survival data for all CNST (International Classification of Diseases for Oncology third edition, category III or Xa) recorded in children under 15 years of age by the Auvergne-Limousin cancer registry for the period 1986–2009. Results Annual incidence of all CNST was 3.27 per 100,000 and the male to female ratio was 0.95. Over 45.0% of CNST were glial. Astrocytomas (36.2%) showed the highest incidence for each age group except between 1 and 4 years where embryonal tumors were more common. For all CNST, no significant variation in incidence over time was observed for the evaluated period of 23 years (annual percent change: −0.4%, 95% CI, [−2.8–2.1]). Globally, 5 years overall survival was 67% [59–73] and had increased by more than 16% between 1986–1999 and 2000–2009, mainly due to better survival for astrocytomas, other gliomas, ependymomas and choroid plexus tumors (P = 0.01). Conclusion We report that the incidence of CNST in Auvergne-Limousin is similar to that in the literature and did not increase between 1986 and 2009. In addition, 5 years overall survival increased after 1999, especially for surgically treatable tumors.

Abstract

Objective Studies investigating the effects of subthalamic deep-brain stimulation (DBS-STN) on restless legs syndrome (RLS) in Parkinson's disease (PD) are limited and report conflicting results, with some describing the emergence of RLS after DBS-STN, while others report postoperative improvement of this disorder. Severe decrease in postoperative dopaminergic medications dose, which may unmask RLS symptoms, has been proposed to explain the emergence of RLS after surgery. We aimed to specifically identify factors associated with the risk of developing RLS after DBS-STN in order to enhance our comprehension of the underlying mechanisms contributing to the development of RLS in PD. Patients In this observational prospective study, we evaluated the occurrence of RLS in 31 patients with PD originally free from RLS symptoms, six months after bilateral chronic DBS-STN, and compared clinical and treatment parameters of patients who developed postoperative RLS with those of patients without postoperative RLS. Results Six patients out of 31 reported post-operative emergence of RLS. There was no between-group difference in demographic data, pre-operative treatment parameters or clinical improvement measures after DBS-STN. However, PD patients with emergence of RLS after DBS-STN had a higher dose of dopamine agonists at postoperative evaluation compared to PD patients without emergence of RLS ( p = 0.040) and a lower percentage of decrease in dopamine agonists ( p = 0.043). Conclusion Overstimulation resulting from cumulative effects of dopamine agonists and STN-DBS may induce changes in excitability of the dopaminergic system, leading to an emergence of RLS. Clinicians should take into account this phenomenon while adjusting pharmacological treatment after surgery.

Abstract

The incidence of glioblastoma increases with age, with a median age, at diagnosis, of 65 years. Indeed, the optimization of standard of care of elderly glioblastoma patients in an aging population in Western countries becomes crucial. The age remains the main prognostic factor of glioblastoma. Survival among elderly patients is significantly less than among younger patients. The median survival of elderly glioblastoma patients is generally inferior to 6 months. More aggressive tumor behavior, less aggressive treatments, increased toxicity of therapies and more unfavorable clinical factors and comorbidities could explain a higher severity of the disease in the elderly. The balance between treatment efficacy and quality of life is a major focus because of the shorter life expectancy of patients. The standard of care of glioblastoma in elderly patients remains controversial. Large optimal resection, when achievable, should be preferred to biopsy. Survival is longer after adjuvant radiotherapy, either normofractionated over 6-weeks course or hypofractionated over 3-weeks course, for patients with good clinical status. Hypofractionation is often preferred because of shorter procedure. Chemotherapy alone with temozolomide can be proposed to patients with methylated MGMT promoter. A phase III randomized study, testing short-course adjuvant radiotherapy with or without temozolomide in elderly patients with good clinical status, is ongoing.

Abstract

Introduction Notre but etait d’etudier l’influence de parametres de tractographie d’une structure simple, le nerf trijumeau, possedant une microarchitecture myelinique heterogene. Materiels et methode Nous avons realise des tractographies des deux nerfs trijumeaux de 8 sujets, apres acquisition d’une sequence d’imagerie par resonnance magnetique nucleaire de tenseur de diffusion (DTI) a 20 directions sur une machine 3 Teslas a partir de 3 regions d’interets (ROIs) : au niveau de la portion cisternale du nerf, la myeline d’origine centrale (CenMye) et la myeline d’origine peripherique (PerMye), et au niveau du ganglion de Gasser (GgGasser). La distance minimale de suivi etait fixee a 5 mm et le seuil d’anisotropie fractionnelle (FA) variait de 0,07 a 0,18. Dans un premier temps nous avons etudie les caracteristiques des ROIs pour valider l’analyse ulterieure tractographique. Les produits derives de la tractographie, fibres et objets (issus des fibres) ont ete etudies ainsi : fibres, valeur de FA et la longueur ; objets, volume et valeur de diffusion apparente (ADC). Resultats Nous avons observe une augmentation de la valeur de FA et une stabilite de l’ADC quand le seuil de FA augmentait pour CenMye ; et inversement pour GgGasser. La valeur de FA etait superieure pour CenMye. Conclusion La region optimale pour tracer le nerf trijumeau est sa portion centrale ; le seuil optimal de FA etant compris entre 0,10 et 0,15. Il semble exister une difference de valeur de FA entre la region centrale et la region peripherique qui pourrait eclairer l’influence de la myeline dans l’anisotropie.

Abstract

Library of Congress Control Number: 2014953249; International audience; Deciphering the connectivity supporting brain function in psychiatric disorders is one of the major challenges in clinical neurosciences. Neural correlates of psychiatric disorders are not well-known because experimental research is extremely difficult to carry on facing the complexity of biological, medical and socio-psychological concepts. Although far from an extensive knowledge of such complex issue, one can summarize most main macroscopic or microscopic circuits known in human and higher species, but also in rodents. After a reminder of scales and functionality of neurobiological circuits, anatomo-functional correlates of the executive-behavioral system and psychiatric disorders are exposed, focusing on most frequent domains of psychiatry, anxiety, mood, substance disorders and memory.

Annual NeuroDevNet Brain Development Conference; Montréal (CAN) - (2014-07-19 - 2014-07-24); International audience; Objective:This study aims to determine the natural history of genetic hypomyelinating disorders Pelizaeus–Merzbacher disease (PMD) and Spastic Paraplegia type 2 (SPG2) on magnetic resonance imaging (MRI) and to correlate MRI results with clinical severity.Methods:A prospective study in 35 patients with variable forms of PMD or SPG2 was performed on a total of 66 cerebral MRI. A myelination score and a global atrophy score were determined for each MRI. We also performed image segmentation to calculate corpus callosum area, cerebellum, white matter and gray matter volumes.Results:Multivariate analysis with adjustment to the age revealed a significant difference for global atrophy score and corpus callosum area in different subgroups of severity at the first MRI acquisition. Evolution of MRI over time showed an increasing score of myelination until the age of 12 years, followed by stabilization. On the contrary, global atrophy, atrophy of corpus callosum and cerebellum worsened in the majority of patients with aging. No change in gray matter and white matter proportions was noticed over time.Discussion:This study points out a correlation between cerebral atrophy and clinical severity in patients with PMD and SPG2 whereas degree of myelination was not clearly discriminating. It also highlighted, for the first time in a large cohort of patients, a clear inter- and intra-individual cerebral atrophy over time involving white and gray matter.

Abstract

Objective: To assess the efficacy of epidural motor cortex stimulation (MCS) on dystonia, spasticity, pain, and quality of life in patients with dystonia secondary to a focal basal ganglia (BG) lesion. Methods: In this double-blind, crossover, multicenter study, 5 patients with dystonia secondary to a focal BG lesion were included. Two quadripolar leads were implanted epidurally over the primary motor (M1) and premotor cortices, contralateral to the most dystonic side. The leads were placed parallel to the central sulcus. Only the posterior lead over M1 was activated in this study. The most lateral or medial contact of the lead (depending on whether the dystonia predominated in the upper or lower limb) was selected as the anode, and the other 3 as cathodes. One month postoperatively, patients were randomly assigned to on- or off-stimulation for 3 months each, with a 1-month washout between the 2 conditions. Voltage, frequency, and pulse width were fixed at 3.8 V, 40 Hz, and 60 μs, respectively. Evaluations of dystonia (Burke-Fahn-Marsden Scale), spasticity (Ashworth score), pain intensity (visual analog scale), and quality of life (36-Item Short Form Health Survey) were performed before surgery and after each period of stimulation. Results: Burke-Fahn-Marsden Scale, Ashworth score, pain intensity, and quality of life were not statistically significantly modified by MCS. Conclusions: Bipolar epidural MCS failed to improve any clinical feature in dystonia secondary to a focal BG lesion. Classification of evidence: This study provides Class I evidence that bipolar epidural MCS with the anode placed over the motor representation of the most affected limb failed to improve any clinical feature in dystonia secondary to a focal BG lesion.

Abstract

Extremes of eating disorders (ED) have become prevalent in both developed and developing countries. Available therapies, though largely effective, fail in a substantial number of patients and carry considerable side effects. Morbid obesity and anorexia nervosa (AN) represent important causes of morbidity and mortality among young adults. Morbid obesity affects disproportionate numbers of children. AN is also important for its high mortality in young adults. The challenges of effectively treating AN are well recognized. In this article, important aspects of ED are reviewed in detail and novel approaches to the treatment of ED are proposed.

Abstract

For gliomas, the goal of surgery is to maximise the extent of resection (EOR) while minimising the postoperative morbidity. The purpose of this study was to evaluate the benefits of a protocol developed for the surgical management of gliomas located in language areas, where tractography-integrated navigation was used in conjunction with direct electrical stimulations (DES). The authors included ten patients suffering of gliomas located in language areas. The preoperative planning for multimodal navigation was done by integrating anatomical magnetic resonance images and subcortical pathway volumes generated by diffusion tensor imaging. Six white matter fascicles implicated in language functions were reconstructed in each patient, including fibres for phonological processing (i.e. the arcuate fasciculus), fibres for lexical-semantic processing (i.e. the inferior frontooccipital fasciculus, inferior longitudinal fasciculus and uncinate fasciculus), and two premotor fasciculi involved in the preparation of speech movements (the subcallosal medialis fasciculus and cortical fibres originating from the medial and lateral premotor areas). During surgery, language fascicles were identified by direct visualisation on tractography-integrated navigation images and by observing transient language inhibition after subcortical DES. Language deficits were evaluated preoperatively and postoperatively, and compared with the EOR. Tractography was successfully performed in all patients, preoperatively demonstrating the relationships between the tumours to resect and the language fascicles to preserve from injury. With the use of the tractography-integrated navigation system and intraoperative DES, language functions were preserved in all patients. The mean volumetric resection was 93.0 ± 10.4 % of the preoperative tumour volume, with a gross total resection in 60 % of patients. The intraoperative combination of tractography and DES contributed to maximum safe resection of gliomas located in language areas.

Abstract

Despite a better understanding of brain language organization into large-scale cortical networks, the underlying white matter (WM) connectivity is still not mastered. Here we combined diffusion tensor imaging (DTI) fiber tracking (FT) and language functional magnetic resonance imaging (fMRI) in twenty healthy subjects to gain new insights into the macroscopic structural connectivity of language. Eight putative WM fascicles for language were probed using a deterministic DTI-FT technique: the arcuate fascicle (AF), superior longitudinal fascicle (SLF), uncinate fascicle (UF), temporo-occipital fascicle, inferior fronto-occipital fascicle (IFOF), middle longitudinal fascicle (MdLF), frontal aslant fascicle and operculopremotor fascicle. Specific measurements (i.e. volume, length, fractional anisotropy) and precise cortical terminations were derived for each WM fascicle within both hemispheres. Connections between these WM fascicles and fMRI activations were studied to determine which WM fascicles are related to language. WM fascicle volumes showed asymmetries: leftward for the AF, temporoparietal segment of SLF and UF, and rightward for the frontoparietal segment of the SLF. The lateralization of the AF, IFOF and MdLF extended to differences in patterns of anatomical connections, which may relate to specific hemispheric abilities. The leftward asymmetry of the AF was correlated to the leftward asymmetry of fMRI activations, suggesting that the lateralization of the AF is a structural substrate of hemispheric language dominance. We found consistent connections between fMRI activations and terminations of the eight WM fascicles, providing a detailed description of the language connectome. WM fascicle terminations were also observed beyond fMRI-confirmed language areas and reached numerous cortical areas involved in different functional brain networks. These findings suggest that the reported WM fascicles are not exclusively involved in language and might be related to other cognitive functions such as visual recognition, spatial attention, executive functions, memory, and processing of emotional and behavioral aspects.

Abstract

National audience; Introduction. – La segmentation manuelle du thalamus sur l'imagerie par résonance magnétique nucléaire (IRM) en clinique à 1,5 Tesla reste difficile. Nous avons réalisé une étude anatomique morphologique à 4,7 Tesla du thalamus pour revisiter sa systématisation et utiliser cette information pour l'identification (segmentation) des noyaux en routine clinique.Matériel - Méthode. – Deux cent cinquante-six coupes centrées sur les ganglions de la base d'un spécimen anatomique ont servis pour l'étude morphologique (voxel = 0,253 mm3). La segmentation intra-thalamique a été réalisée manuellement en fonction des contrastes spontanés et de la position relative des structures. Les connaissances issues d'atlas stéréotaxiques et de livres d'anatomie ont servi à l'identification primaire qui a été finalisée grâce au suivi tridimensionnel.Résultats ou Cas rapporté. – L'ensemble des noyaux intra-thalamiques a pu être segmenté : la région ventro-latérale, centrale et dorsale, le thalamus postérieur et médian, ainsi que des structures liées (zona incerta, substance grillagée d'Arnold, champs de Forel). L'information 3D infra-millimétrique a permis une meilleure compréhension de la systématisation, directement exploitable, car les contrastes sont identiques, pour la reconnaissance des noyaux en conditions cliniques en IRM dès 1,5 Tesla.Conclusion. – Ce travail a permis de revisiter l'anatomie complexe du thalamus. Il a été appliqué pour la cartographie morphologique IRM chez 12 patients. La segmentation (son principe et ses résultats) et l'application clinique sont discutées.

Abstract

International audience; Electric chronic stimulation of the human motor cortex (ECSM) has been reported to alleviate chronic severe pain. However the mechanism of action of ECSM is still hypothetical. This is due mainly to the poor knowledge of, 1) the electric diffusion through the multiple structures beneath the epidural contacts (i.e. dura matter, cerebrospinal fluid space, arachnoid membrane, grey and white matter layers, pie mere and vascular tree), 2) the absence of consensus concerning the stimulation parameters (mono versus bipo-lar stimulation, cathodic or anodic current) and 3) the detailed cortical topography of the contacts. In this study we focused on the precise identification of the cortical areas covered by the electric contacts in a series of twelve patients operated on for ECSM. We propose a new automatic tool for topographic analysis able to compute 2D maps from the 3D anatomic MRI with bijective transformation (point-to-point correspondance). Anatomical regions of interest (AROIs) were visually identified, manually outlined and extracted (Iplan, BrainLab, Germany) for further analysis: 1) for the anatomic structures, on pre operative T1-weigthed magnetic resonance imaging (MRI), the frontal (superior or F1, intermediate or F2 and inferior or F3), the pre central and the post central gyrus; 2) for the electrode contacts (Resume, Medtronic, USA), on post operative computerized tomography (CT). After getting white and gray matter membership maps by automatic segmentation, we produced a cortical mask to build a triangular mesh. We defined a homeomorphism between the 3D mesh and a subset of R 2 and could apply in consequence the circle packing algorithm. We built depth maps (distance to the skull), distance-to-contact maps (distance to a given electrode contact) and anatomic structure maps. Results showed that it was easier to accurately define the location of the contact projection on the cortex allowing physicians to correlate the benefit with the topography. In particular, because of the unfolding , it was easier to integrate the cytoarchitectonics (i.e. the manually identified AROIs) knowledge in the analysis. Beyond the better understanding of ECSM and indirectly of the pathophysiologic process of chronic pain, this new tool might be used in the future for image guided electrode positioning .

Abstract

International audience; Aims: We performed quantitative diffusion tensor imaging and brain tractography to distinguish clinical severity in a series of 35 patients with hypomyelinating PLP1-related disorders classified using the Motor Developmental Score according to the best motor function acquired before the age of 5 years and the gross motor function measure (GMFM) at the time of magnetic resonance imaging acquisition. Methods: We calculated fractional anisotropy and diffusivity values in 26 regions of interest and the numbers of fibers and volumes of hemisphere tractograms. Fiber bundles on tractograms were characterized according to 3 criteria: size, direction of main-stream fibers, and connectivity of bundles (extratelencephalic projections, commissural fibers, and intrahemispheric connections). Results: Age-adjusted multivariate analysis in 3 severity groups revealed increased isotropic diffusion in the superior cerebellar peduncle and grey matter in the most severe group, and larger tractogram volumes and increased numbers of fibers in the least severely affected group. Tractogram patterns showed preserved extratelencephalic projections and a main anterior-posterior aspect of intrahemispheric fibers in most patients, whereas interhemispheric connectivity was variable. The most severely affected and intermediate patients had less intrahemispheric connectivity, with a frequent predominant anterior-posterior direction of main-stream fibers. Interpretation: Diffusion tensor imaging and tractographic parameters can operate as biomarkers to distinguish clinical severity in PLP1-related disorders and could improve our understanding of hypomyelinating leukodystrophies.
