This narrative review aims to describe the impact of coronavirus disease 2019 (COVID-19) in pregnant women and their children, as well as the immune response generated in the binomial with vaccination. For this purpose, a literature review was performed in different platforms. With the results obtained, it was observed that COVID-19 during pregnancy is associated with various maternal-fetal complications, which decrease with anti-COVID-19 vaccination. This immunization has shown, in addition to adequate safety, placental transfer of antibodies to the fetus and breast milk, offering protection to the newborn against hospitalization due to COVID-19. It is concluded that this data support anti-COVID-19 immunization during gestation. [ABSTRACT FROM AUTHOR]

Introduction: The vertical transmission rate (VTR) of HIV has decreased to less than 2% in high-income countries, in spite of which perinatal infections continue to occur. We present data from the national cohort of pregnant women living with HIV and their children in Spain. The objectives were to describe the characteristics of this population, evaluate the VTR of HIV, the safety of antiretroviral therapy (ART) and the prevalence of coinfection. Patients and methods: Multicentre prospective, observational and descriptive study with participation of 62 hospitals. The sample included pegnant women living with HIV whose children were born between January 2020 and December 2022. We collected prospective data on the characteristics of mothers and children using an online questionnaire (REDCap web application). Results: The study included 414 mother-child dyads. Most mothers were immigrants (227/349; 65.1%). The main route of HIV infection was heterosexual transmission (160/402; 39.8%), followed by vertical transmission (44/402; 10.9%). The diagnosis was made before conception in 313/389 women (80.4%), 394/402 (98%) received ART during pregnancy and 356/402 (89.3%) had an undetectable viral load at the time of delivery. The delivery was vaginal in 230/388 children (59.3%). The proportion of preterm birth was 11.1%. The most frequent neonatal prophylaxis approach was monotherapy with zidovudine (358/414; 86.5%). There were 3 cases of vertical transmission of HIV (95% CI, 0%–1.54%). Only one newborn was breastfed. Conclusions: At present, most women living with HIV in Spain receive the diagnosis before conception, are of foreign ancestry and achieve good control of the infection. Although the VTR is very low in Spain, there are still infections that could be prevented with early diagnosis and treatment. Resumen: Introducción: La tasa de transmisión materno-infantil (TMI) del VIH ha disminuido a menos del 2% en países desarrollados, a pesar de lo cual siguen produciéndose infecciones. Se presentan los datos de la Cohorte Nacional Española de mujeres embarazadas que viven con VIH y sus hijos. Los objetivos fueron describir sus características, evaluar la TMI de VIH, la seguridad del tratamiento antirretroviral (TAR) y la prevalencia de coinfecciones. Pacientes y métodos: Estudio descriptivo, observacional, prospectivo y multicéntrico con participación de 62 hospitales. Se incluyeron las embarazadas cuyos hijos nacieron entre enero 2020 y diciembre 2022. Se recogieron prospectivamente características de las madres y niños en REDCap. Resultados: Hubo 414 pares madre-niño. Las madres fueron migrantes 227/349 (65,1%). La principal vía de adquisición del VIH fue la heterosexual (160/402; 39,8%), seguido de la TMI (44/402; 10,9%). En 313/389 (80,4%) el diagnóstico fue previo al embarazo, 394/402 (98%) recibieron TAR durante la gestación y 356/402 (89,3%) presentaba carga viral indetectable al parto. Hubo 230/388 (59,3%) neonatos nacidos por parto vaginal. La tasa de prematuridad fue 11,1%. La profilaxis neonatal más empleada (358/414; 86,5%) fue monoterapia con zidovudina. Hubo 3 casos de TMI del VIH (0,72% (IC 95%; 0%–1,54%), todos ellos con infección intraútero. Todos menos 1 recibieron lactancia artificial. Conclusiones: Actualmente las mujeres que viven con VIH en nuestro medio son migrantes, diagnosticadas mayoritariamente antes de la gestación, y logran adecuada situación inmunovirológica. Aunque la tasa de TMI es muy baja en nuestro país continúan ocurriendo infecciones prevenibles con diagnóstico y tratamiento más precoces.

Trypanosoma cruzi es el agente causal de la enfermedad de Chagas. En Paraguay se estima que existen 165.000 personas infectadas y actualmente la principal vía de transmisión es la congénita, con una prevalencia del 5% y con alrededor de 400 niños infectados nacidos por año. El departamento de Cordillera es área endémica de la Región Oriental del país, donde se implementó el Programa de Control Prenatal de Chagas en el año 1995. El objetivo de este estudio fue inferir sobre la potencial transmisión congénita de la enfermedad de Chagas en jóvenes embarazadas de segunda generación de Cordillera que asistieron a su control prenatal en el período 2010-2012, 15 años después de la implementación del Programa de Control Prenatal. De 9.588 embarazadas; 482 fueron seropositivas (5%); 2,74% en el grupo de 12 a 19 años, 4,93% de 20 a 30 años, 8,03% de 31 a 40 años y 16,11% de 41 a 48 años. Para estimar el origen de la transmisión congénita de segunda generación se tuvo en cuenta a las 72 seropositivas de 12 a 19 años y según la base de datos de las respectivas madres 61 eran seropositivas. Con esta información se estima que la transmisión a embarazadas de segunda generación de 12 a 19 años fue congénita en un 85% indicando una transmisión congénita muy significativa en la población de estudio. Esta información ayudaría al Programa Nacional de Chagas en el fortalecimiento del control de la transmisión congénita de la enfermedad de Chagas en el país.

Objective: To analyze the incidence of congenital syphilis and to describe the epidemiological profile of mothers and newborns diagnosed in the city of Sorocaba, Southeastern Brazil. Methods: This was a descriptive and inferential study using data from the compulsory notification of confirmed cases of congenital syphilis. This information was collected from 2018 to 2022, using the database of the Municipal Center for Specialized Care. Results: 164 cases were reported and the incidence of congenital syphilis was 3.66 new cases. The sociodemographic profile of the mothers, who were predominantly young, of white ethnicity and with unknown schooling, reflects challenges in identification and treatment. The significant association between maternal treatment regimen and schooling, especially in higher education, highlights the importance of access to information in the prevention of mother-to-child transmission. Despite universal prenatal coverage, the majority of inadequate or non-performing treatments suggests practical and socioeconomic barriers. The significant proportion of untreated or unknown partners highlights the need for comprehensive approaches. Conclusion: The results highlight the importance of integrated strategies that go beyond the clinical scope. The implementation of preventive, educational and socioeconomic support measures is imperative to move towards the effective reduction of congenital syphilis in Sorocaba. [ABSTRACT FROM AUTHOR]

El virus de hepatitis B es un virus ADN de doble cadena, cuya infección es silenciosa en la mayor parte de los casos, sin síntomas, y que puede pasarse a otras personas en la misma forma. Su infección crónica afecta más de 350 millones de personas a nivel mundial. La transmisión materno infantil es la causa más frecuente de transmisión, en especial en el momento del parto y si la carga viral materna es alta. Por lo tanto, se debe incentivar que, a toda paciente embarazada se le realice el tamizaje de serología con HBsAg en su primera consulta prenatal para detectar de forma precoz la enfermedad, y así reducir las consecuencias a largo plazo como la hepatitis crónica, cirrosis, falla hepática y carcinoma hepatocelular; estas aumentan en forma precoz si la infección se da al inicio de la vida. Esta infección requiere manejo especializado durante y posterior al embarazo, ya que la terapia antiviral en mujeres embarazadas con alta virulencia puede reducir significativamente la transmisión vertical si se inicia en el tercer trimestre, siendo actualmente el tenofovir el primer tratamiento de elección, en vez de lamivudina, porque su uso se ha visto limitado por presentar altos niveles de resistencia. De igual forma, se recomienda que todo recién nacido de madres con hepatitis B crónica debe recibir inmunoglobulinas y la primera dosis de la vacuna de hepatitis B, en las primeras 12 horas después del parto y no está contraindicada la lactancia materna.

Introduction: The impact of maternal SARS-CoV-2 infection and its risk of vertical transmission is still not well known. Recommendations from scientific societies seek to provide safety for newborns without compromising the benefits of early contact. The aim of the study is to describe characteristics and evolution of newborns born to mothers with SARS-CoV-2 infection, as well as the implemented measures following recommendations from the Sociedad Española de Neonatología. Methods: Observational, prospective and single-center cohort study. A specific circuit was designed for mothers with SARS-CoV-2 infection and their newborns. Epidemiological and clinical data were collected. PCR were performed in newborns at delivery and at 14 days of age. Results: 73 mothers and 75 newborns were included in the study. 95.9% of maternal infections were diagnosed during the third trimester of pregnancy, 43.8% were asymptomatic. Median gestational age was 38 weeks (IQR: 37–40), 25.9% of newborns required admission to Neonatology. Skin-to-skin mother care was performed in 68% of newborns, 80% received exclusive maternal or donated breast milk during hospital stay. No positive PCR results were observed in newborns at delivery, one case of positive PCR was observed in an asymptomatic neonate at 14 days of age. Conclusions: Risk of SARS-CoV-2 transmission is low when complying to the recommendations issued by Sociedad Española de Neonatología, allowing rooming-in and promoting breastfeeding. Resumen: Introducción: La repercusión de la infección materna por SARS-Cov-2 y su riesgo de transmisión vertical es aún poco conocido. Las recomendaciones de las sociedades científicas buscan proporcionar seguridad para el recién nacido sin comprometer los beneficios del contacto precoz. El objetivo del estudio es describir las características y evolución de recién nacidos de madres infectadas por SARS-CoV2, así como las medidas implementadas siguiendo las recomendaciones de la Sociedad Española de Neonatología. Material y métodos: Estudio observacional, prospectivo y unicentro de cohortes. Se diseñó un circuito específico para madres con infección por SARS-CoV2 y sus hijos. Se recogieron datos epidemiológicos y clínicos de las madres y sus recién nacidos. Se realizó PCR al nacer y a los 14 días. Resultados: Se incluyeron 73 madres y 75 neonatos. El 95.9% de infecciones maternas fueron diagnosticadas durante el tercer trimestre, el 43.8% fueron asintomáticas. La mediana de edad gestacional fue 38 semanas (RIC:37–40), el 25.9% de los neonatos requirió ingreso. En el 68% se realizó contacto piel con piel en paritorio y el 80% recibieron lactancia materna o donada exclusiva durante su estancia. No se objetivaron resultados positivos de PCR en la primera muestra obtenida nada más nacer, se objetivó 1 caso de PCR positiva en un neonato asintomático a los 14 días. Conclusiones: El riesgo de transmisión de la infección por SARS-CoV2 es bajo cumpliendo los protocolos de la Sociedad Española de Neonatología, permitiendo el alojamiento conjunto de madre e hijo, el contacto precoz y el establecimiento eficaz de la lactancia materna.

La lactancia materna es el estándar normativo para la nutrición infantil e ideal en pandemias como la ocasionada por SARS-CoV-2 por ser la forma más limpia y segura para nutrir bebés y niños pequeños. El objetivo de este escrito es develar la información existente sobre transmisión vertical materno-infantil del nuevo coronavirus SARS-CoV-2. Se trató de una revisión documental de información extraída de bases de datos digitales a partir de descriptores. Los datos obtenidos fueron analizados y mostrados en esta investigación en dos secciones: lactancia materna y SARS-CoV-2 y conclusiones. Se concluye que la lactancia materna debe iniciarse lo antes posible y que no debe interrumpirse el contacto piel a piel, pues no se ha podido demostrar la transmisión vertical.

La enfermedad conocida como COVID-19 es causada por el virus SARS-CoV-2 y constituye una emergencia de la salud pública a escala global. Desde el informe sobre el primer caso en diciembre de 2019, en Whuan, provincia china de Hubei, la enfermedad se ha expandido rápidamente por el mundo y fue calificada como pandemia en marzo del presente año. El embarazo es un estado fisiológico que predispone a las mujeres a la infección viral. Más allá del impacto de la infección por la COVID-19 en una embarazada, existen preocupaciones relacionadas con el posible efecto sobre el resultado fetal y neonatal; por tanto, las gestantes constituyen un grupo que requiere atención especial en cuanto a prevención, diagnóstico y atención. En este trabajo se abordan aspectos relacionados con la infección por la COVID-19 y el embarazo.

BACKGROUND: The neonatal receptor for the constant fraction (Fc) of IgG (FcRn), expressed in syncytiotrophoblast and endothelial cells of the fetal vessels of the placenta, has the function of transferring maternal IgG in order to provide immunity to the fetus. Despite its protective role, various infectious agents use this mechanism to cross the placenta and infect the product. During T. gondii infection, specific IgG antibodies are produced that coat the parasite, facilitating its transplacental passage. OBJECTIVE: To determine the association between infection with IgG-coated T. gondii, FcRn expression and placental alterations in mice in the last third of gestation. MATERIAL AND METHODS: BALB/c mice were used, inoculated with 5 million parasites (ME49 strain) on day 15 of gestation, through the coccygeal vein, with: 1) PBS; 2) uncoated tachyzoites and 3) IgG-coated tachyzoites. Three days post-infection, the placentas of one uterine horn were processed for histopathology and immunohistochemistry to detect FcRn and T. gondii. In the other horn, the levels of the receptor mRNA (RT-qPCR) and the presence of parasite DNA (qPCR) were determined. RESULTS AND DISCUSSION: Toxoplasma gondii infection caused placental insufficiency due to vascular changes (thrombosis, hemorrhage and congestion), with more severe alterations in the group inoculated with IgG-coated tachyzoites and with a greater number of fetuses that presented growth restriction. Placentas with uncoated parasites showed lower expression of FcRn at the protein level, not mRNA, which is contrary to what was expected. IgG-coating was related to an increase or decrease in FcRn expression in different placental areas. CONCLUSIONS: Infection with Toxoplasma gondii and its IgG-coating modifies the expression of FcRn in the placenta and induces greater placental damage, which induces fetal growth restriction. El embarazo en una mujer que vive con el VIH continúa siendo un problema importante de salud que involucra no solo a la gestante si no al producto de la gestación por el alto riesgo de transmitir del virus durante el embarazo, el momento del parto y la lactancia materna. A pesar de los años transcurridos, los estudios seleccionados siguen reportando que la reducción de la transmisión vertical del VIH se logra con el diagnóstico temprano de la infección en la gestante, disminuyendo la carga viral materna mediante el uso de terapia antirretroviral en el embarazo y en el momento del parto, el nacimiento por cesárea electiva, la supresión de la lactancia materna y la profilaxis antirretroviral en el recién nacido.

Introducción: la enfermedad por coronavirus (COVID-19) se identificó como causa de un brote de enfermedad respiratoria en Wuhan, China, en diciembre 2019. Más allá del impacto del COVID-19 en la mujer embarazada, existen inquietudes sobre sus efectos en el resultado fetal y neonatal. El objetivo de esta revisión fue analizar literatura actual sobre este aspecto. Materiales y método: se realizó una búsqueda exhaustiva en PubMed, usando una combinación de los términos: “COVID-19” y “vertical transmission”. Los criterios de elegibilidad fueron: casos publicados sobre el impacto del COVID-19 en el embarazo y su transmisión vertical, pacientes de 18 años o más, y cursando un embarazo en el segundo o tercer trimestre al momento del diagnóstico de la enfermedad, reportes publicados en inglés o español. Resultados: trece estudios fueron incluidos según los criterios de elegibilidad. Un total de 50 pacientes embarazadas positivas para COVID-19 y 51 neonatos fueron estudiados. De los 51 neonatos, 7 obtuvieron resultados positivos; de los cuales 4 fueron de hisopado nasofaríngeo y 3 correspondieron a muestras de placenta y membranas fetales. Conclusión: en esta revisión no encontramos evidencia definitiva que soporte la transmisión vertical de SARS-CoV-2 durante el segundo y tercer trimestre del embarazo.

La pandemia causada por COVID–19 (Coronavirus Disease 2019), viene produciendo un numero exponencial de infectados y muertes en diversos países del mundo, poniendo a prueba el sistema de salud de las mayores potencias mundiales y también en países como en Perú. Numerosos reportes ponen en evidencia la susceptibilidad de aquellos pacientes con comorbilidades y algún grado de compromiso inmunológico. Asimismo, en varios países del mundo ha generado preocupación la población de mujeres gestantes que debido a sus cambios fisiológicos e inmunología alterada sugieren un mayor impacto en el binomio madre - hijo.

El artículo discute las especificidades del proceso de iniciación sexual (IS) de las jóvenes viviendo con HIV infectadas por transmisión vertical (TV) a partir de un estudio transversal, con muestreo probabilístico, conducido en el municipio de São Paulo entre 2013 y 2014. Se compararon las medias de la edad de la primera relación sexual de las jóvenes de 18 a 24 años, segmentadas entre infectadas por TV, infectadas por otras vías y aquellas que no viven con HIV/SIDA. Se observó asociación entre el aplazamiento de la primera relación sexual y la infección por TV, tener filiación religiosa, tener al menos la enseñanza media completa, no haber hecho uso de drogas en la vida y haber tenido la primera relación con pareja más joven o hasta dos años más viejo. La infección por el HIV confiere complejidad a los procesos de socialización para la sexualidad y, consecuentemente, de transición a la vida adulta. Los resultados indican que diferentes vías de infección por el HIV aparecen asociadas a modos peculiares de vivir la sexualidad.

Background: Chagas disease, caused by Trypanosoma cruzi (T. cruzi), is endemic in Latin-America and is emerging in Spain due to immigration. The vertical transmission rate is around 5%. A routine prenatal screening with serology of all pregnant women from endemic areas is recommended to identify infected newborns, allowing early treatment and cure. Objective: The aim of this study was to estimate the prevalence of positive Chagas serology in a cohort of pregnant women from Latin-America and its vertical transmission. Patients and methods: An observational, prospective, follow-up study was conducted on women with positive serology to T. cruzi, as well as their newborns, from January 2013 to April 2015. Congenital Chagas was ruled out using a PCR technique at birth and at 1 month, and with serology at 9–12 months old. A child was considered infected when PCR was positive, and uninfected when PCR was negative, and/or it had a negative serology. Results: Screening was performed on 1.244 pregnant women from Latin-America, and there were positive results in 40 (prevalence 3.2%, 95% CI: 2.4–4.4%), with 85% of them from Bolivia.There was only one infected newborn (rate of vertical transmission 2.8% (95% CI: 0–15%)), who had a positive PCR at birth. Relative studies enabled an 8-year-old sister with an asymptomatic disease to be diagnosed and treated. Both were treated successfully with benznidazole (later the PCR and serology were negative). Conclusion: Screening during pregnancy in Latin-American women helped to detect those with Chagas disease. The rate of vertical transmission was 2.8%, in keeping with literature. Screening led to the detection and treatment of previously unidentified familial cases. Resumen: Introducción: La enfermedad de Chagas, causada por Trypanosoma cruzi (T. cruzi), es endémica en Latinoamérica y emergente en España, ligada a inmigración. La transmisión vertical se estima de alrededor del 5%. Se recomienda cribado selectivo en el embarazo para identificar al recién nacido infectado, permitiendo tratamiento precoz y curación de la enfermedad. Objetivo: El objetivo de este estudio fue estimar la prevalencia de serología positiva para T. cruzi en una cohorte de gestantes latinoamericanas y la tasa de transmisión vertical de la misma. Pacientes y métodos: Estudio observacional prospectivo de gestantes con serología positiva para T. cruzi en hospital terciario, desde enero del 2013 hasta abril del 2015. El seguimiento de recién nacidos se realizó con PCR al nacimiento, repetida al mes, y serología a los 9-12 meses. Se consideró infectado al niño con PCR positiva y no infectado al niño con PCR negativa y/o negativización de anticuerpos. Resultados: Se realizó cribado en 1.244 gestantes latinoamericanas, siendo positivas 40 (prevalencia 3,2%, IC del 95%: 2,4-4,4%), 85% procedentes de Bolivia. Solo un niño resultó infectado (transmisión vertical 2,8%, IC del 95%: 0-15%) con PCR positiva al nacimiento. La detección de la embarazada permitió estudiar a los hermanos, detectándose caso asintomático en paciente de 8 años. Ambos tratados con benznidazol con buena tolerancia, evolución favourable y negativización de PCR y anticuerpos. Conclusión: El cribado de embarazadas latinoamericanas ha permitido la detección de gestantes con enfermedad de Chagas. La transmisión vertical fue del 2,3%, coincidente con la literatura. El cribado ha permitido la detección y el tratamiento de casos familiares no identificados previamente.

A mathematical model for dengue fever transmission is analyzed, which incorporates relevant biological and ecological factors: vertical transmission and seasonality in the interaction between the vector (Aedes aegypti females) and the host (human). The existence and uniqueness of a positive disease-free periodic solution is proved; the global stability of the disease-free solution and the effect of periodic migrations of mosquitoes carrying the virus on the transmission of dengue are analyzed utilizing the mathematical definition of the Basic Reproductive Number in periodic environments; finally, it is numerically corroborated with the help of the Basic Reproductive Number that dengue cannot invade the disease-free state if it is less than one and can invade if it is greater than one, however, in both threshold conditions when vertical transmission occurs, the number of infected people and carrier vectors rises, representing a mechanism for the persistence of dengue cases in a community throughout a natural year. The well-known disease COVID-19 is caused by SARS-CoV-2 virus and it constitutes an emergency of the public health at global scale. Since the report about the first case on December, 2019, in Whuan, Chinese province of Hubei, the disease has expanded quickly through the world and it was qualified as pandemic on March of the present year. Pregnancy is a physiological state that predisposes women to viral infection. Beyond the impact of the infection due to COVID-19 in a pregnant woman, there are concerns related to the possible effect on the fetal and neonatal result; therefore, the pregnant women constitute a group that requires special care as for prevention, diagnosis and care. The SARS-COV 2 virus has led to a rapidly transmitting pandemic with a high death rate. There is a subregistry of confirmed cases due to the wide spectrum of clinical manifestations and the possibility of dissemination through asymptomatic patients. Studies are still lacking to confirm whether there is vertical transmission, however, the newborn may be infected through the placenta, direct contact with the mother or during hospital stay, so protective measures must be taken during maternal, perinatal and neonatal care. La infección oculta por el virus de la hepatitis B (IOB), se caracteriza por la presencia en suero o plasma del genoma viral (ADN-VHB) y anticuerpos contra la proteína de la cápside (anti-HBc) en ausencia del antígeno de superficie (HBsAg), marcador que tradicionalmente se emplea para identificar la presencia del virus. Con el objetivo de caracterizar la presencia de IOB en hijos de madres positivas al HBsAg, se estudiaron 291 muestras séricas de niños con la condición de ser HBsAg (-) y anticuerpos anti-HBsAg (anti-HBs) menores de 50 UI/L, conservadas en la seroteca del Laboratorio de Referencia Nacional de Hepatitis Virales. Se realizaron ensayos para determinar la exposición al virus (anti-HBc), a los sueros anti-HBc (+) se les realizó Reacción en Cadena de la Polimerasa en Tiempo Real (RCP-TR) para determinar y cuantificar el ADN-VHB. La prevalencia de exposición al VHB (anti-HBc) fue 16,8% (49/291). El ADN viral se cuantificó en el 14% (6/43) de los casos anti-HBc (+), observándose cargas virales que oscilaban entre 2,15 x 101 hasta 3,42 x 101 UI/mL. La prevalencia de la IOB para el total de los pacientes analizados fue 2,1% (6/291), considerada relativamente baja. No se encontró asociación significativa entre las variables sociodemográficas analizadas tales como: edad, sexo y provincia de procedencia. La IOB está presente en hijos de madres positivas al HBsAg, a pesar de la profilaxis contra la hepatitis B. Por lo tanto, se requiere de pesquisajes adecuados para detectar dicha entidad. Las implicaciones clínicas y epidemiológicas de la misma, requieren de un estrecho monitoreo y atención de estos pacientes. Este estudio se realiza por primera vez en Cuba y aporta conocimientos útiles para el diagnóstico, prevención y control de esta enfermedad en niños.

Mother-to-child transmission (MTCT) is the main mode of HIV-1 acquisition among young children worldwide. The goals of this study were to estimate the proportion of HIV MTCT and to identify factors associated with transmission. We reviewed data for HIV-infected pregnant women that had been reported to the National Information on Reportable Diseases System (SINAN) in Espírito Santo state, Brazil, between January 2007 and December 2012. HIV cases in children were followed until age 18 months. The proportion of women who transmitted HIV to their babies was 14% (95% CI 11-17%). In a multivariate logistic regression model, pregnant women who had lower than primary school education (OR 2.74; 95% CI 1.31-5.71), had 2 or more pregnancies during the study period (OR 2.28; 95% CI 1.07-4.84), had emergency cesarean delivery (OR 4.32; 95% CI 1.57-11.9), and did not receive antiretroviral therapy during prenatal care (OR 2.41; 95% CI 1.09-5.31) had higher odds of HIV MTCT. La caries dental es una de las enfermedades más comunes en la infancia y las personas continúan siendo susceptible a través de la vida. Aunque actualmente puede ser detenida y potencialmente revertida en etapas tempranas, no es autolimitada, progresa en forma crónica si no existe un cuidado y control de los factores que la producen, llegando a la destrucción de dientes, dolor, alteraciones funcionales, sistémicas y consecuencias en la calidad de vida de las personas. La caries temprana de infancia, de inicio precoz en niños, es causada en forma frecuente por hábitos alimenticios inapropiados y la adquisición temprana de microorganismos como Streptococcus mutans. Se ha sugerido una transmisión vertical de madre a hijo como la vía principal de adquisición de Streptococcus mutans, y también se ha demostrado en la literatura, que existiría una transmisión horizontal entre niños y sus cuidadores, compañeros de jardín infantil y colegios. Por esta razón durante muchos años se ha definido la enfermedad caries como infecciosa y transmisible. Nuevos avances en técnicas moleculares han dado evidencia acerca de la microflora autóctona y cómo la placa dental o biofilm funciona como un sistema ecológico dinámico y complejo. Existe evidencia que la caries dental no es una enfermedad infecciosa clásica, como se creía hace unos años, por el contrario, esta enfermedad es el resultado de un cambio ecológico en la biopelícula adquirida en la superficie dental. Además la transmisión de Streptococcus mutans de la madre hacia el hijo no implica que la enfermedad se desarrolle, por el contrario, la caries dental hoy se describe como una enfermedad común, compleja y multifactorial, donde interactúan varios factores de riesgo, entre los más destacados conductuales, ambientales y genéticos.

Se reporta el diagnóstico accidental de dos casos de hepatitis B crónica por probable transmisión vertical. El primer caso es una gestante de 32 años que es derivada al hospital para monitoreo fetal por oligoamnios; durante su internación entra en trabajo de parto, y es asistida por una profesional sin usar guantes y se produce un importante contacto con sangre. Se realiza a la paciente un control serológico revelando ser portadora de HBsAg pero negativa para HBeAg; positiva para anti-HBe Ac y HBc IgG, negativa para anti HBs y HBc Ac IgM. El segundo caso es un paciente portador de Lupus Eritematoso Sistémico bajo tratamiento inmunosupresor, con antecedentes de tener una madre fallecida y dos hermanos portadores de VHB. La serología reveló: HBsAg (+) con carga viral de 113 copias, HBeAg (-), anti-HBe (+), HBc IgG (+), HBc IgM (-), hepatitis C (-). El paciente recibió tratamiento con tenofovir. Ambos pacientes fueron diagnosticados en forma accidental y considerados ser portadores de hepatitis B crónica inactiva por probable trasmisión vertical por los antecedentes familiares. Estos dos casos constituyen una llamada de atención sobre la transmisión vertical del virus y la importancia de realizar el despistaje en la embarazada en el primer control prenatal.

La infección por el virus linfotrópico humano de células T tipo 1 (HTLV-1) ha sido descrita en muchas áreas del mundo, como en los países del Caribe, Japón, África, Oceanía y en Sudamérica. En la presente revisión definimos la endemicidad del HTLV-1 en el país, planteando cuatro criterios epidemiológicos. Luego discutimos el tema central de la revisión: la transmisión vertical del HTLV-1, que en nuestro país sería uno de los principales mecanismos de transmisión. Dentro del desarrollo de este aspecto en particular, presentamos una estimación de la tasa de transmisión vertical y los factores de riesgo asociados con la transmisión vertical sobre la base de una revisión exhaustiva de estudios nacionales y extranjeros. Con esta revisión pretendemos dar una primera aproximación al estudio de la trasmisión vertical de HTLV-1, un aspecto poco estudiado en nuestro medio.Human type 1 T-cell lymphotropic virus (HTLV-1) infection has been described in many areas of the world, including Caribbean countries, Japan, Africa, Oceania and South America. In this review we define the endemicity of HTLV-1 in the country proposing four epidemiological criteria. Then we discuss the core subject of the review, which is the vertical transmission of HTLV-1. This mechanism is one of the main forms of transmission in our country. Within the development of this particular topic, we present an estimated rate of vertical transmission and the risk factors associated to vertical transmission based on an exhaustive review of the national and international literature. This review pretends to provide a first approach to the vertical transmission of HTLV-1, an aspect poorly studied in our country.

Antecedentes: En embarazadas seropositivas sin profilaxis antirretroviral la transmisión vertical (TV) del VIH es de 30%, cifra que disminuye bajo al 2% con un manejo integral de prevención. Objetivo: Conocer el comportamiento epidemiológico de la TV en Chile desde la creación del programa nacional de prevención. Método: Datos aportados por el Departamento de Epidemiología del MINSAL y CONASIDA (1984-2006). Estudió de situación VIH/SIDA por año, regiones, grupo etario y sexo, con especial énfasis en TV. Resultados: Se notificaron 9.317 casos de VIH y 7.886 casos de SIDA (1984-2006), prevalencia en aumento hasta el 2003 con tendencia descendente posteriormente. La razón actual de VIH entre hombres y mujeres es de 4:1, con clara tendencia al aumento de notificación en mujeres. La principal vía de exposición sigue siendo la vía sexual (93,4%). El grupo más afectado está entre los 20-39 años (73,1% para VIH y 63,1% para SIDA), cabe destacar que entre 0-9 años se encuentra el 1,1% de los afectados por VIH y el 1,4% por SIDA, todos infectados por TV. Conclusiones: La transmisión vertical de VIH en Chile es responsable de un bajo porcentaje de las personas notificadas de VIH/SIDA, pero es la causa de la totalidad de los niños afectados por la infección en nuestro país. Se observa una importante reducción de la TV, llegando a niveles muy cercanos a los objetivos ministeriales.Background: The risk of transmitting HIV from mother to unborn child, without any antiretroviral prophylaxis, reaches 30%. It can be reduced to less than 2% by implementing integral preventive strategies. Objective: To assess the epidemiological profile of HIV vertical transmission in Chile since the implementation of the national AIDS transmission prevention program. Method: Data from the Epidemiology Department of the Health Ministry and the National Commission for AIDS grouped by year, location, ageandgender. Withspecial emphasis on vertical transmission. Results: 9317 cases of HIV infection and 7886 cases of AIDS have been reported in Chile (1984-2006), with a decreasing tendency since 2003. Sexual exposure continues to be the primary route with 93.4% of the cases. The male/female ratio for case notif¡cation is 4:1, with a rising tendency among women. The most affected is between 20-39 years old (73% HIV notification and 63.1% for AIDS). Among children between 0-9 years of age is 1.1% of the HIV and 1.4% of AIDS notifications, all of them for vertical transmission. Conclusions: HIV vertical transmission in Chile is responsible for a small percentage of HIV/AIDS notifications, but it accounts for all of the infected children. There's been an important reduction in this transmission route, reaching prevalence close to the aimed rates, which can explained by the expanding prevention programs of the health policies implemented by the Health Ministry.

Debido a la rara presentación simultánea entre cáncer y embarazo y a que la afectación de la unidad fetoplacentaria por dicha patología es aún más exótica, la presente revisión busca lograr un mejor entendimiento sobre la afectación placentaria y fetal por patologías malignas que pueden comprometer el embarazo. Se realizó una búsqueda en diferentes bases de datos electrónicas como Pubmed, Ovid, Elsevier, Cochrane y libros de texto impresos. Los hallazgos en la literatura médica son bastante limitados y básicamente se trata de reportes de casos o series de casos, encontrados mediante búsqueda bibliográfica. Desde el primer reporte de metástasis de carcinoma hepático a placenta y feto, descrito en 1866, se han documentado solamente 14 casos de metástasis transplacentaria materno-fetal y poco más de 50 casos de enfermedades malignas maternas con metástasis a placenta.Due to the rare simultaneous presentation of cancer during pregnancy and a fetoplacental unit being affected by such pathology is even rarer then this present review sought for better understanding regarding placental and foetal units being affected by malign pathologies which could compromise pregnancy. Different databases were searched (i.e. Pubmed, Ovid, Elsevier and Cochrane) as were medical texts and books. Findings in the medical literature were extremely limited, basically dealing with case-reports and case series found by a bibliographic search. Only 14 cases of maternal-foetal trans-placenta metastasis have been documented since the first report of hepatic carcinoma metastasis of the placenta and foetus described in 1866 and only just over 50 cases of malign maternal diseases involving metastasis of the placenta.

Objetivos: Identificar y describir los factores que han hecho posible la existencia de nuevas infecciones de VIH por transmisión vertical desde la implementación del tratamiento antirretroviral en la gestante seropositiva (1997-2001) en Cataluña. Métodos: Revisión sistemática de casos identificados en servicios de pediatría de todos los hospitales de Cataluña. Resultados: Se identificaron 28 casos de infección pediátrica por VIH: 9, 9, 8, 2 y 0 por año de nacimiento de 1997 a 2001, respectivamente. De 16 madres con diagnóstico de infección por VIH conocido antes o durante el embarazo, 9 realizaron profilaxis antirretroviral durante éste (5 con buena adhesión, uno desconocido y el resto con mala adhesión) y 7 no realizaron profilaxis (6 por rechazo y uno no se conoce). De 12 diagnosticadas después del parto, 5 fueron embarazos no controlados y el resto poco o bien controlados. De estos últimos, en 6 no se practicó serología para VIH y en uno fue negativa en el primer trimestre. Conclusiones: La transmisión vertical del VIH en Cataluña ha disminuido en los últimos años, pero se han producido infecciones por la mala implementación en algún caso de las medidas preventivas conocidas. Debería ofrecerse la prueba diagnóstica para VIH a toda gestante no sólo en el primer trimestre de embarazo sino también al final, si se sospecha exposición al virus, y en caso de gestaciones no controladas hay que usar pruebas diagnósticas de lectura rápida en la sala de partos.Objectives: To identify and describe the factors that have led to new cases of HIV infection through mother-to-child transmission since the introduction of antiretroviral therapy in HIV-seropositive pregnant women (1997-2001) in Catalonia. Methods: Systematic review of cases identified in the pediatric services of all the hospitals in Catalonia. Results: Twenty-eight cases of pediatric HIV infection were identified: 9, 9, 8, 2 and 0 per year of birth from 1997 to 2001, respectively. Of 16 mothers with a diagnosis of known HIV infection before or during pregnancy, nine underwent antiretroviral prophylaxis during pregnancy (compliance was good in five, unknown in one and poor in one) and seven did not undergo prophylaxis (six refused it and no information was available in one). Of 12 mothers diagnosed after delivery, pregnancy was not monitored in five and was little or well-monitored in the remaining seven. Of mothers with well-monitored pregnancy, a serological HIV test was not performed in six and was negative in the first trimester in one. Conclusions: Mother-to-child transmission of HIV has decreased in the last few years in Catalonia, but infections have sometimes occurred through poor implementation of preventive measures. Pregnant women should be offered an HIV diagnostic test not only in the first trimester but also at the end of pregnancy if HIV exposure is suspected. In women with unmonitored pregnancies, rapid diagnostic tests for HIV should be used in the delivery room.

Neste estudo, buscamos investigar o conhecimento que gestantes possuem sobre a transmissão vertical, o comprometimento do feto e o significado do resultado soropositivo que a identifica como infectada pelo HIV. A pesquisa exploratória, de natureza qualitativa, foi desenvolvida em Unidades Sanitárias de São Leopoldo/RS. A análise permitiu captar a percepção de 63 gestantes entre 16 e 40 anos sobre os motivos e os significados para realização do teste, os conhecimentos e vivências do cotidiano e as perspectivas e cuidados com o bebê. Os resultados trazem um alerta aos profissionais que atuam no pré-natal, visto que necessitam atender uma complexidade de situações que emergem quando se vincula gestação e AIDS.

The occult hepatitis B infection (OBI) is defined as the presence of hepatitis B virus (HBV) DNA and antibodies to core antigens of the HBV (anti-HBc) in the sera or in the plasma and the absence of hepatitis B surface antigen (HBsAg). The current study aimed to characterize the OBI in children born of HBsAg-positive mothers. Serum samples of 291 children with negative HBsAg and anti-HBs <50UI/L collected from all over the country with active-pasive immunization, were screened for anti-HBc antibodies. Those anti-HBc positive sera were subsequently tested by Real-Time Polymerase Chain Reaction to determine and quantify HBV-DNA levels and its correlation with socio-demographics data. The prevalence of anti-HBc positivity was 16.8% (49/291). HBV-DNA was detected in 14% of the examined cases with a low HBV viral load ranging from 2.15 to 3.42 x 10¹ UI/mL. The overall OBI prevalence rate was 2.1% (6/291). There were no statistically significant differences between the sociodemographic variables studied (age, sex and location). OBI is present among Cuban children born of HBsAg-positive mothers despite prophylaxis against hepatitis B. Therefore, sensitive screening assays for occult HBV infection must be considered and deserves an adequate clinical monitoring of these patients. This study is carried out for the first time in Cuba and makes a useful contribution to prevention and control of hepatitis B in children. [ABSTRACT FROM AUTHOR]

ResumenEn un estudio transversal descriptivo realizado de mayo a noviembre de 2009, se estudiaron 108 gestantes atendidas tanto en consulta externa, como en el servicio de urgencias y en la sala de maternidad del hospital San Cristóbal de Ciénaga, hallándose 32 pacientes con VDRL reactivo (vereal disease research laboratory) (29,6 %), de las cuales 7 presentaban inmunofluorecencia indirecta con absorción (FTA-ABS) positiva, correspondiendo a una seroprevalencia de 6,4 % para sífilis. En cuanto a Toxoplasmosis se confirmaron 4 casos (3,7 %) en mujeres procedentes de áreas rurales.El factor común en estas mujeres, es la pobre adherencia al control prenatal y la falta de conocimiento acerca de las infecciones de transmisión vertical (sífilis, toxoplasmosis y VIH) aunado esto a diversas prácticas y conductas de riesgo que ponen en peligro el bienestar materno-fetal. (DUAZARY 2011, 42 - 47)AbstractIn a descriptive study conducted from May to November 2009, we studied 108 pregnant women served both outpatient and in the emergency department and the hospital maternity ward of San Cristobal of cienaga. Being found 32 patients with reactive VDRL (29.6%), of which seven had positive FTA-ABS corresponding to a seroprevalence of 6.4% for syphilis. Toxoplasmosis As confirmed four cases (3.7%) in women from rural areas. The common factor in these women is the poor adherence to prenatal care and lack of knowledge about vertical transmission of infections (syphilis, toxoplasmosis and HIV) coupled this to various practices and risk behaviors that endanger the welfare maternal fetal.Keywords: vertical transmission; prenatal care; knowledge; attitude, practice.

Introduction. Dengue is the most important mosquito-borne viral disease. The World Health Organization in 1994 estimated that 2.000 million people worldwide are exposed to contagion. This study aimed to characterize dengue virus (DV) infection in pregnant women with or without acute febrile syndrome and the impact on their newborns (NB). Methods: Blood samples were collected from the binomial mother -NB for hematological analysis and specific diagnosis by qPCR -DV -DV - NS5, NS1 antigen and IgM / IgG antiDV. Results: 30 pregnant women (14-36 years old) attending the Obstetric Emergency of Instituto Autónomo Hospital Universitario de Los Andes between June 2010 and July 2011 were included. Twenty six percent of mothers were suspected dengue cases and 73.3 % were not. Twenty three percent of pregnant women were reactive to IgM - antiDV, 6 of 8 suspected dengue patients and 1 of 22 non suspected. One of 7 infected mothers presented with nonspecific symptoms, 4 with fever, chills, myalgia, arthralgia , nasal congestion, jaundice and retro -ocular pain, and 2 with signs of shock characterized by severe genital bleeding and hypovolemic shock. Vertical transmission occurred in 2/7 NB of mothers with dengue shock. These NB presented with fetal distress, with thrombocytopenia and mild jaundice as the predominant findings. IgG antibodies were detected in 9/30 pregnant mothers and their NB, suggesting secondary infection. Conclusions: In endemic and epidemic areas DV infection occurs transplacentally during the peri -partum period. AIDS is a disease that represents one of the biggest health problems of today because of his character and pandemic severity. The rate of AIDS cases is increasing in women, causing an increase in children infected by vertical transmission: transmission of the HIV virus from mother to child during pregnancy. The Brazilian government has been striving to reduce vertical transmission rates, by deploying the Sunrise Project, based in Protocol 076. This study aims to demonstrate the effectiveness of the protocol for the prevention of vertical HIV+ pregnant women, highlighting the important role of nurses en the puerperal pregnancy. This is an exploratory and systematic, made from a literature review. We concluded that the Sunrise Project is effective, however, faces some difficulties in implementation. The nurse, as part of the team, has a key role in the pregnant woman's accession to the Protocol. [ABSTRACT FROM AUTHOR]
