29. Causes of hospitalization and predictors of in-hospital mortality among people living with HIV in Sicily-Italy between 2010 and 2021.

Abstract

Despite the rising number of people living with human immunodeficiency virus (HIV), there is a lack of knowledge about the factors that lead to PLWHs being hospitalized in worldwide literature. Our study aimed to investigate PLWH admissions in Sicily (Italy) between January 2010 and September 2021 and to analyze the characteristics and risk factors for in-hospital mortality and differences between Italians and foreigners. Data from the hospital discharge forms of all people living with HIV (PLWH) hospitalized in Sicilian hospitals were retrospectively collected. Age, sex, nationality, length of stay, acquired immunodeficiency syndrome (AIDS), and non-AIDS-related diseases were evaluated using univariate analysis according to in-hospital mortality rates. The factors associated with mortality were included in the logistic regression model. In total, 5281 admissions from 2726 PLWHs occurred, most of which were related to non-AIDS diseases. Approximately 20 % regarded foreign patients, mainly from Africa. Logistic regression analysis revealed an association between in-hospital mortality and some AIDS- and non-AIDS-related diseases (wasting syndrome, lymphomas, Kaposi sarcomas, progressive multifocal leukoencephalopathy, cryptococcosis, abscesses, sepsis, cardiovascular disease, nephropathy, and respiratory diseases). African patient admissions were significantly associated with tuberculosis, toxoplasmosis, Burkitt lymphoma, and hepatitis B diagnoses. Our study showed that most hospitalizations were related to non-AIDS-defining diseases, with differences between Italian and foreign patients, mainly from Africa. • Few data are in the literature about PLWH admissions. • The average age of PLWHs has increased. • Most hospitalizations were related to non-AIDS-defining diseases. Abstract

• SARS-CoV-2 RNA was detected on 32.1% of all surfaces in COVID-19 patient rooms of an acute-care hospital following terminal cleaning. • The prevalence of SARS-CoV-2 RNA contamination was highest among floor surfaces (78.7%). • Odds of SARS-CoV-2 surface contamination increased with month. We evaluated the effect of terminal cleaning on SARS-CoV-2 RNA contamination of COVID-19 isolation rooms in an acute care hospital. SARS-CoV-2 RNA was detected on 32.1% of room surfaces after cleaning; the odds of contamination increased with month. The prevalence of elevated high-touch surface contamination was lower in terminally cleaned rooms than patient-occupied rooms. [ABSTRACT FROM AUTHOR]

44. Lifetime brain structural trajectories in TAUPS2APP mouse model of Alzheimer's disease: Neuroimaging / animal imaging.

Abstract

Background: The TAUPS2APP Alzheimer's disease (AD) mouse model expresses mutant APPSWE, PSEN2N141I and TAUP301L mutations and displays age‐dependent accumulation of Aβ plaques and neurofibrillary tangles. In humans, abnormal protein accumulation and spreading are related to neurodegenerative processes, most stereotypically in the hippocampus and medial temporal lobe. Prior reports in different AD models have demonstrated various brain structural phenotypes (Grandjean et al.,2016). Here we assess neuroanatomical trajectories in the TAUPS2APP model of AD using longitudinal MRIs. Method: Thirteen C67BL/6 (11f/2m) and seventeen TAUPS2APP (10f/7m) mice underwent T2‐weighted MRI (voxel size 117x147x147 μm, matrix 256x102x120, FoV 3x1.5x1.5 cm, TR=2500 ms TE=50 ms) at 4,9,13,18, and 24 months of age. Data were analyzed following two‐level deformation‐based morphometry pipeline that uses ANTs SyN nonlinear registration. At the first level within subjects' minimum deformation averages are created and estimates of volume change are computed (log Jacobian determinants). Next, a second level average is created out of unbiased within‐subject averages and the Jacobians are resampled into the common space Result: A linear mixed‐effect model with subject as random effect (second order spline fit, sex as a covariate) revealed significant differences in interaction between age and genotype in neuroanatomical trajectory (absolute Jacobian) of bilateral hippocampus, lateral septal area (LSA) and isocortical areas (at False Discovery Rate of 5%, fig.1). TAUPS2APP showed an accelerated overgrowth of hippocampus at young age but larger volumetric decline with age in comparison to non‐TG animals (fig.2a). A similar pattern was observed in LSA (fig.2b). On the contrary in the isocortex, where non‐TG declined with age, TAUPS2APP mice showed volume enlargement (fig. 3c). Conclusion: These data suggest that TAUPS2APP model is characterized by abnormal neuroanatomical trajectory throughout its lifespan relative to controls. While hippocampal atrophy recapitulates clinical markers of late‐onset AD, enlarged cortex that had been reported earlier in some AD mouse models (TgCRND8, APP/J20 and PSAPP) does not. It was suggested that elevated APP levels may affect the rate of local region growth in adult mice ‐ due to space occupancy by amyloidosis and astrogliosis. 48. Factors associated with persistent colonisation with methicillin-resistant Staphylococcus aureus.

Abstract

We conducted a prospective cohort study between 1 January 2010 and 31 December 2012 at five adult and paediatric academic medical centres to identify factors associated with persistent methicillin-resistant Staphylococcus aureus (MRSA) colonisation. Adults and children presenting to ambulatory settings with a MRSA skin and soft tissue infection (i.e. index cases), along with household members, performed self-sampling for MRSA colonisation every 2 weeks for 6 months. Clearance of colonisation was defined as two consecutive negative sampling periods. Subjects without clearance by the end of the study were considered persistently colonised and compared with those who cleared colonisation. Of 243 index cases, 48 (19·8%) had persistent colonisation and 110 (45·3%) cleared colonisation without recurrence. Persistent colonisation was associated with white race (odds ratio (OR), 4·90; 95% confidence interval (CI), 1·38–17·40), prior MRSA infection (OR 3·59; 95% CI 1·05–12·35), colonisation of multiple sites (OR 32·7; 95% CI 6·7–159·3). Conversely, subjects with persistent colonisation were less likely to have been treated with clindamycin (OR 0·28; 95% CI 0·08–0·99). Colonisation at multiple sites is a risk factor for persistent colonisation and may require more targeted decolonisation efforts. 49. Standardizing Care and Parental Training to Improve Training Duration, Referral Frequency, and Length of Stay: Our Quality Improvement Project Experience.

Abstract

Objectives At our institution, there is a six bed Pediatric Respiratory Care Unit for technology dependent infants and children with a tracheostomy tube. A lack of consistency in patient care and parent/guardian education prompted our group to critically evaluate the services we provided by revisiting our teaching protocol and instituting a new model of care in the Unit. The aims of this quality improvement (QI) project were to standardize care and skills proficiency training to parents of infants with a tracheostomy tube in preparation for discharge to home. Methods: After conducting a current state survey of key unit stakeholders, we initiated a multidisciplinary, QI project to answer the question: ‘could a standardized approach to care and training lead to a decrease in parental/guardian training time, a decrease in length of stay, and/or an increase in developmental interventions for infants with tracheostomy tubes’? A convenience sample of infants with a tracheostomy tube admitted to the Pediatric Respiratory Care Unit were included in the study. Descriptive statistics were used to analyze the results. Results Through this QI approach, we were able to decrease the time required by parents to achieve proficiency in the care of a technology dependent infant, the length of stay for these infants, and increase referral of the infants for developmental assessment. Abstract

Background This study aimed to have international experts converge on a harmonized definition of whole hippocampus boundaries and segmentation procedures, to define standard operating procedures for magnetic resonance (MR)-based manual hippocampal segmentation. Methods The panel received a questionnaire regarding whole hippocampus boundaries and segmentation procedures. Quantitative information was supplied to allow evidence-based answers. A recursive and anonymous Delphi procedure was used to achieve convergence. Significance of agreement among panelists was assessed by exact probability on Fisher's and binomial tests. Results Agreement was significant on the inclusion of alveus/fimbria ( P = .021), whole hippocampal tail ( P = .013), medial border of the body according to visible morphology ( P = .0006), and on this combined set of features ( P = .001). This definition captures 100% of hippocampal tissue, 100% of Alzheimer’s disease-related atrophy, and demonstrated good reliability on preliminary intrarater (0.98) and inter-rater (0.94) estimates. Discussion Consensus was achieved among international experts with respect to hippocampal segmentation using MR resulting in a harmonized segmentation protocol. [ABSTRACT FROM AUTHOR]
