BACKGROUND: The practical training process represents a crucial step in the university process for students on degree courses in the health and medical professions. This discussion is emphasized for nursing students, who, for each stage of the internship, may have the opportunity to enrich themselves in practical terms and to be able to compare themselves with the reality of work physically. Based on this discussion, it is essential to carry out a periodic evaluation of the working realities. OBJECTIVES: The main objective of this work is to implement an evaluation tool for the clinical training environment to ensure feedback for both the healthcare facility under consideration and to improve the quality of university clinical training. METHOD: This research study protocol envisages an observational study as the type of study, with recruitment of nursing degree courses to subsequently include students from the various academic years. The study involves the application of the CLES+T questionnaire in participating degree courses, initially to be able to have an overall vision and subsequently to be able to implement it as a permanent evaluation tool. EXPECTED RESULTS: It is expected, from the study described in this protocol, that the participating population will consider the questionnaire as a way of critically analysing the clinical environment. A broad participation of the nursing student population and good feedback of the examined environments is expected.

Health-related quality of life (HRQoL) is often markedly impaired in patients with peristomal irritant dermatitis. This article reviewed the etiology, nursing evaluation and intervention measures of irritant contact dermatitis in patients with enterostomy, to provide targeted and scientific basis for clinical practice of prevention and treatment of irritant dermatitis after enterostomy, reduce patients' pain, and improve patients' compliance and self-care ability. The Philippines continues to produce globally competitive nursing graduates. However, it performs poorly in international standards for quality education in nursing. This paper investigates the objective parameters of quality assurance: Commission on Higher Education (CHED) accreditation, Philippine Regulation Commission (PRC) licensure examinations, and other international and local standards to measure the quality of nursing education inputs and outputs. Results showed an accreditation rate of 3% for nursing programs, less than the national average of 10% for all programs across all professions (CHED 2018; CHED n.d. -a; CHED n.d. -b). Some highly regarded programs have advanced to Level IV (seven baccalaureate programs and two master's programs). Although there is a link between graduates' performance and effective instruction and sound evaluation in university procedures (Amanonce & Maramag, 2020), written board exams cannot assess some performance indicators to determine if a student has met program objectives. Although the country has produced globally competitive nursing graduates, steps must be taken to improve the program's compatibility with the concept of globalization. Objective To investigate the application effect of the procedural sedation protocol for patients with traumatic brain injury, and summarized related nursing management. Methods Totally 64 patients with traumatic brain injury were randomly divided into the control and study group, with 32 cases in each group. Patient in the control group received conventional therapy for sedation, and those in the study group were given a procedural sedation protocol. The Glasgow Coma Scale (GCS), Sedation-agitation Scale(SAS), dosage of sedatives and length of hospital stays were measured and compared between two groups. Results There was no significant difference in the GCS and SAS scores between two groups before intervention(P＞0. 05). Patients in the study group had a higher score of GCS and lower score of SAS compared with those in the control group after intervention(P＜0. 01). Shorter length of hospital stay and lower dosage of sedatives were observed in the study group than those in the control group(P＜0. 01). Conclusion The procedural sedation protocol is potentially effective to reduce the dosage of sedatives and shorten the length of hospital stay in patients with traumatic brain injury. This paper summarized the application effect of cold compress of Traditional Chinese Medicine (TCM) in a patient with wind-warm and pulmonary heat syndrome. According to the outcomes of nursing evaluation and diagnosis, the TCM cold compress therapy was carried out in combination with nursing interventions such as daily care, nutrition guidance, emotion nursing, and so on. The TCM cold compress therapy has obvious effect on body cooling during fever caused by wind-warm and pulmonary heat syndrome, and it demonstrates the characteristics of easy-to-use, convenience and cost-effectiveness. Introduction: The Social Determinants of Health are defined by the living conditions of the population, how they are born, grow, live, work and age, and are considered as the main responsible for the health of populations. Objective: To evaluate the influence of social determinants of health on effectiveness of family functioning. Materials and Methods: A quantitative study was carried out with 200 participants between 2017 and 2018. To assess the correlation between proximal, intermediate and distal social determinants of health and the level of effectiveness of family functioning, home visits were made to 100 families who responded the Assessment of strategies in Family Effectiveness instrument and a sociodemographic questionnaire. Data were analyzed by simple frequency, Spearman's correlation coefficient (?) (p <0.05), and subjected to descriptive analysis from the perspective of systemic organization. Results: The social determinants that showed a positive correlation coefficient with the level of effectiveness of family functioning were the years of study and the fixed family income. There was a negative correlation with the main source of income (number of occupations) or source of income and the presence of other members or relatives in the family. Discussion: Investments in education can improve the family's ability to organize, solve or prevent adverse events, increase income and provide more time for family relationships and the achievement of congruence, as evidenced by other authors. To describe the nurses' views for consideration when designing a program to prevent adolescents' anxiety in the perioperative period. A qualitative descriptive case study using focus group and thematic analysis was conducted. Three face-to-face focus group interviews were conducted in October and November 2019 in the pediatric department of a university hospital. A purposive criterion method was applied to achieve a sample of 19 pediatric nurse specialists. Data were organized and systematized in the professional software for qualitative and mixed methods data analysis software (MAXQDA) and treated through the thematic analysis method. The COREQ checklist was used to report data collection, analysis, and results. Four major themes and 14 subthemes regarding the perioperative period were generated. The first, adolescent evaluation, included the knowledge evaluation about procedures, signs and symptoms, and desire to be engaged in care. The second, caring adolescents and parents, means that nurses must be ready to care for both, use the opportunities to implement the nursing interventions, and manage physical teen space to accommodate adolescents in the ward. The third, nurses' challenges in the perioperative period, comprise the lack of time and trained nurses to work with adolescents, and the absence of prior adolescents' preparation and postoperative feedback. The fourth, nursing consultation, consists in promoting interdisciplinarity, developing the nursing interventions, and the main content to be included in the program's design. Given the challenges experienced by nurses when caring for adolescents in the perioperative period, nurses suggested a systematized assessment of the adolescent at an early stage of the perioperative caring process. Added to this is the nurse's readiness for the adolescent and parents, as well as the existence of trained nurses to evaluate adolescents and to implement non-pharmacological interventions in the perioperative period. A nursing consultation emerges as the most suitable solution to include in a program to prepare adolescents for the surgical procedure and help them to manage anxiety. This paper reviewed the studies on peripherally inserted central catheter(PICC)-associated thrombosis in cancer patients, and briefly discussed the period of high incidence of thrombosis, clinical features, risk factors and nursing interventions. Related evaluation analysis was carried out to provide reference for the prevention of control of PICC- associated thrombosis in cancer patients. Objective: This cross-sectional study was planned to evaluate the self-efficacy and symptom control of cancer patients and to determine the factors affecting them. Methods: The sample of the study consisted of 329 cancer patients who were treated in the Medical Oncology Clinic of a university hospital between April and June 2019 and accepted to participate in the study. Data were collected using the Patient Characteristics Information Form, Cancer Behavior Inventory-Short Version (CBI-SV), and Edmonton Symptom Assessment Scale (ESAS). Percentage, mean, Mann–Whitney U test, and Kruskal–Wallis analysis of variance were used in the analysis of the data. Results: The mean CBI-SV score of the patients was 79.10 ± 17.55. It was determined that the highest mean score of ESAS of the patients was in the symptom of fatigue (3.53 ± 2.81). Some symptoms were statistically lower in patients with good income, who are working, and who are non-smokers. Also it was determined that the self-efficacy levels of the patients with good income and quitting smoking were higher (p < 0.05). At the same time, as the patients' self-efficacy scores increased, the severity of the symptoms they experienced decreased statistically (p < 0.05). Conclusion: It was found that the patients' self-efficacy score was above the moderate level, the most intense symptom experienced by the patients was fatigue, and the severity of the symptoms decreased as the patients' self-efficacy level increased. In line with these results; the symptoms, self-efficacy perceptions, and affecting factors of cancer patients should be evaluated by nurses at regular intervals, and care and consultancy services should be provided. Objective To investigate the effect of Objective Structured Clinical Examination (OSCE) in orientation training of night shift among newly recruited nurses. Methods Totally 26 registered nurses who required orientation training of night shift were divided into the observation group and control group. Routine theory and practice tests were carried out in the control group, and OSCE was performed in the observation group. The nurse’s satisfaction with training, comprehensive skills assessment outcome and exam result were observed and compared between two groups. Results The satisfaction rate of observation group was higher than that of control group (P＜0. 05). The scores of comprehensive skills assessment and theory test in the observation group were higher than those in the control group (P＜0. 05). Conclusion The OSCE in orientation training of night shift is potentially effective to enhance the comprehensive skills of nurses and improve their satisfaction. The urinary retention after mixed hemorrhoids surgery is associated with multiple factors including anesthesia, surgery, pain, overinfusion, body position, mental status and so on. This paper summarized the nursing experience in a patient undergoing massage combined with traditional Chinese medicine hot compress in treating urinary retention after mixed hemorrhoids surgery. Except for Traditional Chinese Medicine nursing techniques such as massage and Traditional Chinese Medicine hot compress, nursing interventions also included evaluation, environment optimization and psychological intervention, in order to promote the Traditional Chinese Medicine external application treatment in the clinical setting. Objective: To evaluate the quality of nursing care in a hospital using quality indicators. Method: observational, prospective, cross-sectional study conducted at the Inpatient Medical and Surgical Units of a philanthropic hospital in Minas Gerais, Brazil for 14 consecutive days in 2017, using the Active Search Registration instrument, developed and validated by Vituri. Result: 185 adult patients were evaluated, totaling 650 observations. The overview of the indicators of both units showed that, of the 14 indicators, seven (50%) were classified as “insufficient” in both places. Conclusion: systematic evaluation processes make it possible to identify the technical and relational performance of the professionals as well as their knowledge and skills for the care practices. The adoption of care indicators can help the management process of the nursing service.

Objective: To analyze times of nursing activities obtained with the Pediatric Patient Classification Instrument (PPCI) and Nursing Activities Score (NAS). Methodology: Transversal, descriptive and exploratory study performed in a pediatric surgical hospitalization unit. Data collection was performed using data from patients' charts obtained through daily visits for three fortnight. Organized data were analyzed using descriptive statistics. Results: The sample had 155 charts and the patients were predominantly boys aged 1 to 6 years, hospitalized for hepatic and gastrointestinal or genitourinary splenectomy. Regarding the PPCI, the score revealed patients requiring nursing care, predominantly at an intermediate level. Regarding NAS, the score represented the need for intensive nursing care. Conclusion: The two classification models were not equivalent in terms of the number of hours needed for care in pediatric surgical patients. [ABSTRACT FROM AUTHOR]

Abstract Background Assessing the clinical ability of nursing students continues to be a fundamental challenge within the nursing degree. Designing innovative teaching strategies while ensuring an educational experience that results in safe nursing practices is an additional challenge for all educators. Objectives To design, develop and implement a tool to evaluate the clinical skills of nursing students. Design A two-phase study. In the second phase, a pre-experimental study was conducted. Settings Health Sciences Department of the University of Almería, Spain. Participants 250 Nursing students in the second year of their degree. Methods The study was divided into 2 phases: 1st) Web - based tool design and development and 2nd) Usability testing. In the second phase, a pre-experimental study was conducted both with an experimental group and a control group. Results In the first phase, a panel of 15 experts confirmed that the content and technical aspects of the tool were adequate (scores of 4–5/5). In the second phase, the participants of the experimental group showed higher final scores in the evaluated skills section, as well as shorter evaluation time and a greater number of observations and registered incidents than the participants in the control group. Finally, the students of the experimental group indicated a greater degree of satisfaction with the evaluation system used. Conclusions The web-based tool designed is an effective strategy to evaluate clinical skills. It is important to have strategies and instruments that allow us to demonstrate the development and acquisition of the skills required for future nursing professionals. [ABSTRACT FROM AUTHOR]

Quantitative descriptive study aimed to identify the opinion of nursing managers of teaching hospitals on the relevance of quality indicators in the management of nursing services. The population was composed of nine nursing managers of teaching hospitals of the state of Paraná. Data were collected in the months of August and September 2012, through a tool - a questionnaire on quality indicators made available electronically to be completed by the nurses. The participants showed greater appreciation of quality indicatorsfor the management of nursing services and evaluation of health care than to general indicators and personnel management indicators.For the management of health institutions, professional must be able to analyze work processes and make decisions aimed to the continuous improvement of care and competitiveness of institutions.

Objective: To identify the dependency levels of hospitalized patients in surgical clinical units. Method: It is a cross-sectional study. A total of 5,083 evaluations were performed through the User Dependency Evaluation Tool between May and October 2015, totaling 120 days of collection in two surgical hospitalization units of a university hospital in the southern region of Brazil. The User Dependency Evaluation Tool is a new Patient Classification System developed in Brazil to access the patients' dependency levels and the demand of the nursing team. Results: It was evidenced that in 2,452 (48.2%) of the evaluations the patients required intermediate care, followed by 1,913 (37.6%) requiring minimal care, 652 (12.9%) requiring high dependency care and 59 (1.1%) requiring semi-intensive care. No patients were classified as requiring intensive care. Conclusion: The results identified the degree of patient dependency in relation to nursing care, providing support to the management practice in nursing, especially for staff dimensioning in surgical units. Objectives: cross-culturally adapt to the Spanish context and make a new proposal for the nursing outcome, Physical Condition (2004), of the Nursing Outcomes Classification (NOC) for its precise use in clinical practice. Method: a cross-cultural adaptation study and a proposal for the nursing outcome, Physical Condition, was conducted and supported by the opinion of 26 experts. The data was obtained through an electronic form, and a quantitative analysis was conducted, using the SPSS software. Results: the version adapted to the Spanish context was obtained and the proposal of the outcome, Physical Condition, received agreement from 26 experts, with a mean score greater than 7.6 for adequacy of the outcome definition and its indicators, and 8.5 for the relevance of the indicators. Conclusions: the version adapted to the Spanish context and a new proposal for Physical Condition were obtained. The results obtained indicate a high level of adequacy and relevance, an instrument of great utility in the clinic, and research was obtained to evaluate the interventions directed to the improvement of the physical condition. This paper critically analyses realist evaluation, focussing on its primary analytical concepts: mechanisms, contexts, and outcomes. Noting that nursing investigators have had difficulty in operationalizing the concepts of mechanism and context, it is argued that their confusion is at least partially the result of ambiguities, inconsistencies, and contradictions in the realist evaluation model. Problematic issues include the adoption of empiricist and idealist positions, oscillation between determinism and voluntarism, subsumption of agency under structure, and categorical confusion between context and mechanism. In relation to outcomes, it is argued that realist evaluation's adoption of the fact/value distinction prevents it from taking into account the concerns of those affected by interventions. The aim of the paper is to use these immanent critiques of realist evaluation to construct an internally consistent realist approach to evaluation that is more amenable to being operationalized by nursing researchers. The initial evaluation of the child, when admitted on the Paediatric Intensive Unit, is a essential tool and key piece as a starting point on the development of a specific Care Plan for each child. Therefore, it is necessary the existence of a protocol, according to a rigorous methodology, so that cares will have quality and thus, it will be avoided that each nursing professional will act in a different way, according to his intuition, beliefs or improvisation capacity.The initial evaluation of the child will allow us not only to coordinate the interventions, but also to give continuity to the cares.The initial evaluation of the child document is a nursing register that is part of the clinical register of the paediatric patient. Nursing registers turn to be the best approximation of what nowadays is our job’s practice, and they are, therefore, necessary for us to be judged by a professional perspective and to make it possible to classify the services we carry out to the society.

ABSTRACT Objectives: to translate and cross-culturally adapt the Leg Ulcer Measurement Tool to Brazilian Portuguese. Methods: methodological study involving the steps of initial translation, synthesis of translations, back-translation, expert panel and pretest. For analysis by the expert committee, the content validity index was calculated and in the pretest for practicality, the agreement rate was calculated. Satisfactory agreement was considered when > 0.8 and 80%, respectively. Results: the initial steps of translation were satisfactorily developed and there was little disagreement between the translators. In the expert panel, was obtained significant concordance of 0.97. The pretest was performed with ten nurses and 30 patients. The feasibility of the translated version was evaluated with 100% agreement. Final Considerations: the instrument presented a high level of concordance among the experts during all steps and showed content validity thereby making the adaptation appropriate for the Brazilian context.

ABSTRACT Objective: To validate the content of the indicators of the nursing outcome Dry eye severity, of the Nursing Outcomes Classification, in adult patients admitted to the intensive care unit. Methods: Methodological study of content validity, developed in two stages: the first, consisting of 23 specialists; and the second, of a consensus group formed by ten nurses. For analyzing the data in the first stage, we used the content validity index and binomial test for each indicator; in the second stage, the discussions of each meeting were transcripted, and the indicators that achieved 100% consensus were validated. Results: Of the 14 indicators evaluated by the specialists, 7 presented a cut-off point below 0.80, but did not show statistical significance in the binomial test. In the consensus group’s validation, six indicators were reformulated. Conclusion: This study considered the 14 indicators valid for evaluating patients admitted to the intensive care unit.
