This article discusses the management of acute type A aortic dissection (ATAAD) complicated by coronary malperfusion. The study reports on the experience and revascularization strategy of a single center for patients with ATAAD and coronary malperfusion. The patients were divided into different subgroups based on the pattern of coronary dissection. The study suggests that concomitant coronary artery bypass grafting (CABG) may not be necessary for Neri negative and Neri A patients, but should be considered for Neri C patients. Somatosensory evoked potentials are a well-established tool for assessing volley conduction in afferent neural pathways. However, from a clinical perspective, recording of spinal signals is still a demanding task due to the low amplitudes compared to relevant noise sources. Computer modeling is a powerful tool for gaining insight into signal genesis and, thus, for promoting future innovations in signal extraction. However, due to the complex structure of neural pathways, modeling is computationally demanding. We present a theoretical framework which allows computing the electric potential generated by a single axon in a body surface lead by the convolution of the neural lead field function with a propagating action potential term. The signal generated by a large cohort of axons was obtained by convoluting a single axonal signal with the statistical distribution of temporal dispersion of individual axonal signals. For establishing the framework, analysis was based on an analytical model. Our approach was further adopted for a numerical computation of body surface neuropotentials employing the lead field theory. Double convolution allowed straightforward analysis in the frequency domain. The highest frequency components occurred at the cellular membrane. A bandpass type spectral shape and a peak frequency of 1800 Hz was observed. The volume conductor transmitting the signal to the recording lead acted as an additional bandpass reducing the axonal peak frequency from 200 Hz to 500 Hz. The superposition of temporally dispersed axonal signals acted as an additional low-pass filter further reducing the compound action potential peak frequency from 90 Hz to 170 Hz. Our results suggest that the bandwidth of spinal evoked potentials might be narrower than the bandwidth requested by current clinical guidelines. The present findings will allow the optimization of noise suppression. Furthermore, our theoretical framework allows the adaptation in numerical methods and application in anatomically realistic geometries in future studies. [ABSTRACT FROM AUTHOR]

Background and purpose: Early pharmacological deep vein thrombosis (DVT) prophylaxis is recommended by guidelines, but rarely started within 48 h. We aimed to analyze the effect of early (within 48 h) versus late (>48 h) DVT prophylaxis on hematoma expansion (HE) and outcome in patients with spontaneous intracerebral hemorrhage (ICH). Methods: We analyzed 134 consecutive patients admitted to a tertiary neurointensive care unit with diagnosed spontaneous ICH, without previous anticoagulation, severe coagulopathy, hematoma evacuation, early withdrawal of therapy or ineligibility for DVT prophylaxis according to our institutional protocol. Significant late HE was defined as ≥6 mL increase of hematoma volume between neuroimaging within 48 h and day 3–6. Multivariate analysis was performed to identify risk factors for late HE, poor 3‐month outcome (modified Rankin Scale score ≥ 4) and mortality. Results: Patients had a median Glasgow Coma Scale score of 14 [interquartile range (IQR), 10–15], ICH volume of 11 (IQR, 5–24) mL and were 71 (IQR, 61–76) years old. A total of 56% (n = 76) received early DVT prophylaxis, 37% (n = 50) received late DVT prophylaxis and 8 (6%) had unknown bleeding onset. Patients with early DVT prophylaxis had smaller ICH volume [9.5 (IQR, 4–18.5) vs. 17.5 (IQR, 8–29) mL, P = 0.038] and were more often comatose (26% vs. 10%, P = 0.025). Significant late HE [n = 5/134 (3.7%)] was associated with larger initial ICH volume (P = 0.02) and lower thrombocyte count (P = 0.03) but not with early DVT prophylaxis (P = 0.36). Early DVT prophylaxis was not associated with worse outcome. Conclusion: Significant late HE is uncommon and DVT prophylaxis within 48 h of symptom onset may be safe in selected patients with ICH. The lack of easily measurable, objective physiological activity parameters of the masseter and temporalis muscle during jaw movements in humans has led to the consideration to revise data of surface electromyographies (EMGs) by applying a computerized quantification method. The aim of this follow-up analysis was to get quantitative data out of EMG-records of an earlier study. These records were obtained with two different splints, splint 1 providing an anterior front-canine guidance and splint 2 providing bilateral balanced occlusion. Utilizing a computer aided integration method led to numeric results which statistically proves the prediction of the previous investigation. Applying the integration method, the EMG raw signal was transformed into area-values which enabled a statistical work up of the data. Wilcoxon test statistics shows a significant (P<0·05) lower muscle activity in patients wearing dentures providing anterior front-canine guidance compared to those with balanced occlusion. It is concluded that the neuromuscular activity of the elevator muscles is highly reproducible and that the neuromuscular function is similar in edentulous subjects to that found in people with natural teeth. Furthermore, the study statistically proves earlier visual data that all those subjects, whose muscle activities were observed with anterior guidance (splint 1) compared to bilateral balanced occlusion (splint 2) showed significantly lower values with regard to subjects wearing splint 2. The activity of jaw elevator muscles (masseter and anterior temporal muscle) was studied by surface electromyography in 17 patients wearing complete dentures with anterior-canine guidance. Baseline electromyography was recorded with the dentures and compared to recordings obtained with two different splints adapted to the tipper denture, one providing anterior guidance leading to immediate discussion of the posterior teeth during any jaw movement, the other providing posterior guidance buccally on the working side and lingually on the non-working side (bilaterally balanced occlusion). Recordings were obtained during a sequence of different jaw movements: postural position, maximal tooth contact in intercuspal position, protrusive movement of the mandible (under tooth contact) and during lateral excursions. also carried out under tooth contact. Muscle activity was not significantly different as recorded with dentures only versus with the splints providing anterior guidance. However, a significant increase in activity was observed with bilaterally balanced occlusion. The results of this study in edentulous people are similar to those found in patients with natural teeth.
