5. Lifetime economic potential of mobile stroke units in acute stroke care: A model-based analysis of the drivers of cost-effectiveness.

Abstract

Background and purpose: To simulate patient-level costs, analyze the economic potential of telemedicine-based mobile stroke units for acute prehospital stroke care, and identify major determinants of cost-effectiveness, based on two recent prospective trials from the United States and Germany. Methods: A Markov decision model was developed to simulate lifetime costs and outcomes of mobile stroke unit. The model compares diagnostic and therapeutic pathways of ischemic stroke, hemorrhagic stroke, and stroke mimic patients by conventional care or by mobile stroke units. The treatment outcomes were derived from the B_PROUD and the BEST-mobile stroke unit trials and further input parameters were derived from recent literature. Uncertainty was addressed by deterministic and probabilistic sensitivity analyses. A lifetime horizon based on the US healthcare system was adopted to evaluate different cost thresholds for mobile stroke unit and the resulting cost-effectiveness. Willingness-to-pay thresholds were set at 1x and 3x gross domestic product per capita, as recommended by the World Health Organization. Results: In the base case scenario, mobile stroke unit care yielded an incremental gain of 0.591 quality-adjusted life years per dispatch. Mobile stroke unit was highly cost-effective up to a maximum average cost of 43,067 US dollars per patient. Sensitivity analyses revealed that MSU cost-effectiveness is mainly affected by reduction of long-term disability costs. Also, among other parameters, the rate of stroke mimics patients diagnosed by MSU plays an important role. Conclusion: This study demonstrated that mobile stroke unit can possibly be operated on an excellent level of cost-effectiveness in urban areas in North America with number of stroke mimic patients and long-term stroke survivor costs as major determinants of lifetime cost-effectiveness. Abstract

We introduce MINFLUX, a concept for localizing photon emitters in space. By probing the emitter with a local intensity minimum of excitation light, MINFLUX minimizes the fluorescence photons needed for high localization precision. A 22-fold reduction of photon detections over that required in popular centroid-localization is demonstrated. In superresolution microscopy, MINFLUX attained ~1 nm precision, resolving molecules only 6 nm apart. Tracking single fluorescent proteins by MINFLUX increased the temporal resolution and the localizations per trace by 100-fold, as demonstrated with diffusing 30S ribosomal subunits in living E. coli. Abstract

In order to overcome intercellular variability and thereby effectively assess signal propagation in biological networks it is imperative to simultaneously quantify multiple biological observables in single living cells. While fluorescent biosensors have been the tool of choice to monitor the dynamics of protein interaction and enzymatic activity, co-measuring more than two of them has proven challenging. In this work, we designed three spectrally separated anisotropy-based Förster Resonant Energy Transfer (FRET) biosensors to overcome this difficulty. We demonstrate this principle by monitoring the activation of extrinsic, intrinsic and effector caspases upon apoptotic stimulus. Together with modelling and simulations we show that time of maximum activity for each caspase can be derived from the anisotropy of the corresponding biosensor. Such measurements correlate relative activation times and refine existing models of biological signalling networks, providing valuable insight into signal propagation. Abstract

Resumo: Este estudo teve como objetivo avaliar as possíveis alterações histopatológicas hepática de tilápias do Nilo alimentadas com dietas contendo silagem biológica de pescado com diferentes concentrações protéicas. Foram utilizados 180 juvenis alimentados com dietas contendo três níveis de proteína (20, 24 and 28% PB), e duas proporções de silagem biológica (¼ e ½) durante 75 dias. Os fragementos de fígado foram fixados em Bouin e inclusos em Histosec®. Posteriorente foram cortados em microtomo com espessura de 2 a 5μm. O método utilizado para coloração foi hematoxilina/eosina e PAS. Os cortes histológicos foram analisados em microscopio de luz. O desarranjo na morfologia do fígado dos peixes alimentados com silagem biológica foi influenciado pelos altos níveis protéicos, e pelo aumento ½ de proporções de proteínas de origem animal das dietas. Foi observado que a variação dos hepatócitos está diretamente ligada com o tipo da dieta fornecida para os peixes. Nos peixes alimentados com as dietas contendo 28% PB, o fígado apresentou desarranjo da estrutura cordonal dos hepatócitos, pontos de necrose e deslocamento do núcleo para periferia. Níveis elevados de silagem biológica de pescado provoca alterações deletérias no fígado. O nível de proteína adequado para manter o desenvolvimento associado à saúde do peixe é de 24%PB.

24. Impact of the surgical strategy on the incidence of C5 nerve root palsy in decompressive cervical surgery.

Abstract

Our aim was to identify the impact of different surgical strategies on the incidence of C5 palsy.Degenerative cervical spinal stenosis is a steadily increasing morbidity in the ageing population. Postoperative C5 nerve root palsy is a common complication with severe impact on the patients´ quality of life.We identified 1708 consecutive patients who underwent cervical decompression surgery due to degenerative changes. The incidence of C5 palsy and surgical parameters including type and level of surgery were recorded to identify predictors for C5 nerve palsy.The overall C5 palsy rate was 4.8%, with 18.3% of cases being bilateral. For ACDF alone the palsy rate was low (1.13%), compared to 14.0% of C5 palsy rate after corpectomy. The risk increased with extension of the procedures. Hybrid constructs with corpectomy plus ACDF at C3-6 showed significantly lower rates of C5 palsy (10.7%) than corpectomy of two vertebrae (p = 0.005). Foi realizado estudo experimental de intoxicação por Brachiaria decumbens em ovinos confinados que receberam apenas esta forrageira como alimento. Os animais foram avaliados em três períodos do ano, com duração de 60 dias cada. As análises incluíram exame clínico e amostras de sangue para mensuração da atividade sérica das enzimas gama glutamiltransferase e aspartato aminotransferase com o objetivo de avaliar sua importância e utilidade no diagnóstico de fotossensibilização hepatógena causada pela planta. Foram descritos ainda, achados de necropsia e histopatológicos. Dos vinte e quatro animais confinados, cinco morreram no primeiro período (fevereiro a abril), quatro no segundo (junho a agosto) e dois no terceiro (outubro a dezembro), tendo como principais sinais clínicos anorexia, icterícia, fotofobia, dermatite leve, além de desidratação e apatia. Nos achados de necropsia foram constatados icterícia generalizada, vesícula biliar repleta e distendida, padrão lobular hepático evidente e, em dois casos, opacidade de córnea. As alterações microscópicas foram mais significativas no fígado com bilestase, tumefação e vacuolização de hepatócitos, sinusóides com macrófagos, proliferação de ductos e dúctulos biliares com infiltração linfocítica, que variaram na sua severidade conforme o tempo decorrido da ingestão da planta, além de imagens negativas de cristais no citoplasma de macrófagos presentes nos sinusóides hepáticos e no espaço periportal e cristais refringentes oticamente ativos, ocluindo a luz de ductos biliares. Os achados foram característicos de fotossensibilização hepatógena por B. decumbens, principalmente pela presença de fotofobia, mesmo quando os animais não apresentaram fotodermatite. O aumento da atividade sérica da gama glutamiltransferase teve alta correlação com a morte dos animais. O surgimento da elevação da atividade sérica de gama glutamiltransferase foi, em média, 11 dias antes da constatação dos sinais clínicos, validando sua importância no diagnóstico e na prevenção. A aspartato aminotransferase teve resultados inconsistentes. Treze animais não apresentaram sinais clínicos, apesar de serem constatadas significativas elevações nos níveis séricos das enzimas em questão, sugerindo a existência de indivíduos tolerantes/resistentes dentro do intervalo estabelecido. O modelo experimental foi adequado na caracterização da intoxicação de ovinos por B. decumbens, oferecendo subsídios para estimar preventivamente o risco de morte de ovinos intoxicados pela planta, além de ser útil para a realização de novos estudos da intoxicação por B. decumbens.Brachiaria decumbens was fed as exclusive source of food to feedlot sheep. Sheep were evaluated in three periods of the year, of 60 days each, regarding their clinical status and was performed collection of blood samples to measure enzymatic activity of gamma glutamyltransferase and aspartate aminotransferase to evaluate their diagnostic importance and usefulness besides the clinical signs, necropsy and histopathology finds in hepatogenous photosensitization Brachiaria related. Out of 24 feedlot sheep, five died in the first period (February-April), four in the second (June-August) and two in the third (October-December). Main clinical signs were anorexia, jaundice, photophobia and two cases of slight dermatitis, besides dehydration, apathy and corneal opacity. Necropsy revealed generalized jaundice, distended gall bladder and liver with evident lobular pattern. The histological changes varied in severity according the time after the beginning of the plant ingestion; they were more significant in the liver and included bilestasis, swelling and vacuolation of hepatocytes, foammy macrophages, bile duct proliferation and mononuclear inflammatory cell infiltration, further crystal negative images were found within cytoplasm of macrophages present inside of hepatic sinusoid and in the periportal space and also optically active refringent crystal in the lumen of bile tubules. These finds are characteristic of hepatogenous photosensitization, mainly by presence of photophobia, even when the sheep did not present photodermatitis. The results of serum biochemistry activity, during the course of poisoning with strong relation between increased gamma glutamyltransferase activity, and lethality. Onset of arising gama glutamyltransferase activity was 11 days, in average, before beginning of clinical signs. Aspartate aminotransferase had increased activity only close to the beginning of clinical signs. Serum biochemistry results of GGT suggest that they are useful for a precocious diagnosis preventing deaths caused by ingestion of B. decumbens. Thirteen sheep didn't show clinical signs despite significant increases on serum enzyme activity, suggesting the existence of tolerant/resistant individuals. The experimental model of feedlot was adequate to study B. decumbens toxicosis in sheep.

Abstract

Objective: To report an outbreak of follicular conjunctivitis in a group of sheep diagnosed with Anaplasma spp., without any other co‐infection. Animals Studied: In all, 18 animals from a sheep head, males and females, from eight months to four years of age, were assessed for follicular conjunctivitis. Procedures: The procedures performed included general physical and ophthalmological examinations; PCR evaluation for infectious agents; analysis of hematological parameters, microbiological tests of ocular swabs, coproparasitological examination, histopathological examination of conjunctival biopsy. Results: All 18 animals had uni‐ or bilateral follicular conjunctivitis, and one animal also had unilateral uveitis. The results of microbiological analyzes were negative for Moraxella spp., Staphylococcus spp., and Pseudomonas spp., and PCR analysis results were negative for Chlamydia spp., Mycoplasma spp., and Toxoplasma gondii. Anemia, thrombocytopenia, lymphocytosis, and an inclusion body in some erythrocytes, compatible with Anaplasma and PCR analysis for Anaplasma spp. were positive. Conclusion: Anaplasmosis may be associated with follicular conjunctivitis in sheep and should be included in the differential diagnosis list and investigated in cases of conjunctivitis in herds. Abstract

Germany has been campaigning strongly for Deputy Finance Minister Caio Koch-Weser to become the next International Monetary Fund (IMF) MD. However, he has been widely criticized in the Anglo-Saxon media. Some observers feel that he lacks experience in finance and central banking and that he would lack the authority of somebody who had been a finance minister or central banker. Despite this, a consensus seems to be emerging that his experience as a leading manager and a global banker will balance out his lack of central banking ability. He has denied that he would not be a tough negotiator as head of the IMF.

Abstract

Mixed views continue to be expressed about the Basel Committee on Banking Supervision's recommendations for revising the 1988 Accord on capital standards. The new Basel II agreement would maintain in principle the 8% risk-weighted capital requirement, but would alter the risk weights allocated to asset categories. This would give bank managers and supervisors greater flexibility in setting adequate capital. The key element of the Basel Committee consultation paper is the new minimum capital requirement, which adds further risk brackets.

German finance minister Hans Eichel strongly believes that Russia should be given international aid to cover some of its debts. Germany has credited more to Russia than any other nation. It is opposed to waving all debts because it will lose significantly more than other countries including the US. Germany favours a debt burden relief system called the finesse approach over alternative systems named Grand Restructuring and Default. The Finesse approach would be more effective at relieving Russia of its debt problem than any other economic measures, says Eichel.
