Amaç: BK virus (BKV) ile ilişkili hemorajik sistit (HS), hematopoetik kök hücre transplantasyonu (HKHT) yapılan hastalarda yaygın görülen bir komplikasyondur. Bu çalışmanın amacı, HKHT yapılan çocuk hastalarda BKV enfeksiyonu insidansının araştırılmasıdır. Gereç ve Yöntemler: Çalışmaya Ekim 2015 ile Eylül 2017 tarihleri arasında izlenen, yaşları 16 ay ile 16 yıl arasında olan toplam 51 hasta dahil edilmiştir. Hastalar BKV DNA'nın idrar ve kanda tespiti için kantitatif gerçek-zamanlı polimeraz zincir reaksiyonu (Anaolia Geneworks, Türkiye) testiyle monitörize edilmiştir. Bulgular: Hastaların 46'sına allojenik HKHT ve 5'ine otolog HKHT yapılmıştır. Allojenik nakil yapılan 46 hastanın 26'sında (%56,5) ve otolog nakil yapılan 5 hastanın 1'inde (%20,0) olmak üzere toplam 27 (%52,9) hastanın idrar ve/veya kanında BKV DNA pozitifliği saptanmıştır. Allojenik HKHT yapılan 46 hastanın 12 (%26,1)'sinde preemptif tedavi için gereken idrarda >107 kopya/ml BKV viral yük düzeyi tespit edilmiştir. Preemptif tedavi uygulanan 12 hastanın 9'unda (%75,0) HS gelişmesi önlenirken 3'ünde (%25,0) HS gelişmiş ve tedaviyle iyileşmiştir. HS gelişmeden önceki iki hafta içinde BKV virurisi >109 kopya/ml olarak tespit edilmiş ve HS prediktif tanısı için prognostik bir gösterge olarak kabul edilmiştir. BKV viremisi 1 hastada sistit gelişmesinden önceki iki hafta içinde >104 kopya/ml olarak tespit edilmiştir. Sonuç: Yüksek riskli hastalarda HS prediktif tanısı için BKV enfeksiyonu, özellikle HKHT hastalarında BKV virüri taraması önerilir.

Second malignancies develop with increased incidence in patients with herediter retinoblastoma. The most common second malignancies in retinoblastoma survivors are soft tissue sarcoma and osteosarcoma. Lymphoma developing as a second malignancy has rarely been reported. Here we report a case of lymphoma diagnosed in a 13 year-old-boy, 12 years after the diagnosis of unilateral retinoblastoma having heterozygous p.R552 (c.1654C> T) mutation, treated with enucleation and radiotherapy. Introduction: The minute amounts of several trace elements including copper, zinc and magnesium play some roles as essential constituents of various biological organs. The serum concentrations these elements are modified in some malignancies. The aim of this study is to investigate the copper, zinc and magnesium levels in the serum of children with various malignant disorders. Material and Methods: The serum levels of copper, zinc and magnesium were measured in 82 children with various malignant disorders and in 21 age-matched healty controls using an atomic absorbtion spectrophotometer. Purpose: Some patients with cancer use herbal therapy as an adjunct while receiving conventional cancer treatments. Beta-glucans have direct cytotoxic effects on cancer cells as well as stimulatory effects on the immune system. In this study, a nutritional supplement containing 1,3-1,6 beta-glucan obtained from baker's yeast (Saccharomyces cerevisiae) was used by patients diagnosed with sarcoma, and this study aimed to retrospectively evaluate the effect of beta-glucan use on the incidence of neutropenic fever (NF) and survival rates. Materials and Methods: Among the patients diagnosed with sarcoma between 2014 and 2017, 27 patients who used beta-glucan were included in the beta-glucan group, and 31 patients who did not use beta-glucan were included in the control group. The clinical records of patients were retrospectively analyzed. Results: The number of patients who had NF and the mean length of hospital stay due to NF were higher in the beta-glucan group than those in the control group, but the results were not statistically significant. The overall survival rates at 5 years were 83.3% and 52.9% and event-free survival rates at 5 years were 48.1% and 71% in the beta-glucan and control groups, respectively. Conclusion: The effect of prophylactic beta-glucan use on the incidence of NF and survival rates in pediatric patients with sarcoma should be evaluated more reliably in further prospective studies on a large patient group.

Purpose: The aim of this study was to investigate the effect of telomerase activity on clinical characteristics and survival of patients with neuroblastoma. Materials and Methods: This study was carried out by examining telomerase activity in previously taken pathology preparations of 87 patients who were diagnosed with neuroblastoma between 2011 and 2019 in the Pediatric Oncology Department of Çukurova University Faculty of Medicine. Results: Totally 87 patients, 46 males (52.9%) and 41 females (47.1%), were included in our study. Median age at the time of diagnosis was 3.0 (0.08-15) years. Twenty-three (26.4%) patients included in the study had telomerase activity, 64 (73.6%) had no telomerase activity. Overall survival (OS) of patients with telomerase activity up to 8 years were 84%, while event-free survival (EFS) was 56%. Patients without telomerase activity had a 93% OS for up to 8 years, while EFS was 70%. There was no significant relationship between patients with or without telomerase activity in terms of sex, age, stage, risk group, relapse, myc- n amplification, OS and EFS. Conclusions: In this study, no significant difference was found between telomerase activity and clinical features and survival of neuroblastoma patients. Prospective studies involving larger numbers of patients will more clearly demonstrate the impact of telomerase activity on the prognosis of patients with neuroblastoma.

Background: Treatment of Ewing sarcoma (ES) requires multidisciplinary approach and deficiencies in treatment adversely affect the results. This study included patients diagnosed with ES and aimed to determine the factors affecting prognosis and investigate the efficacy of replacing actinomycin-D with carboplatin in consolidation treatment. Methods: Eighty-two pediatric ES patients diagnosed at a single institution between 2005 and 2020 were retrospectively evaluated. Clinical and epidemiological features, treatment modalities, prognostic criteria, and overall survival (OS) rates of patients revieved. In consolidation treatment, 22 patients were treated with actinomycin-D and 32 patients with carboplatin (500 mg/m2/dose), 24 patients could not receive consolidation treatment. The 5- and 10-year OS rates of the patients were compared. Results: The 5- and 10-year OS rates of the 82 patients with ES were 46% and 40%, respectively. The 5-year OS rates in the group with localized disease (n = 55) and metastasis (n = 27) at diagnosis were 54% and 26%, respectively (p = 0.006). When evaluated according to the consolidation treatment administered both the 5- and 10-year OS rates of the patients receiving actinomycin-D were 50%. The 5-year OS rate was 58% in the carboplatin group, and the 5- and 10-year OS rates of patients that did not receive consolidation treatment was 20%. Conclusions: Survival was significantly worse in the group that did not receive consolidation treatment. Furthermore, our results suggested that carboplatin could be used effectively as an alternative to actinomycin-D in ES consolidation treatment.

Purpose: In this study, the outcomes and flow cytometry results of childhood acute leukemia patients and their relationship with the prognosis and other clinical conditions was investigated. Materials and Methods: A total of 93 patients, including 69 acute lymphoblastic leukemia (ALL) and 24 acute myeloid leukemia (AML) patients diagnosed at our hospital between January 2008 and November 2013, were included. Results: 5-year overall survival (OS) was 66% in ALL patients and 80% in AML patients. When leukemia patients were classified flow cytometrically according to their cell surface antigens (CD); 5-year OS was 77% in CD2(+) patients and 58% in CD2(-) patients. Five-year OS was 82% in patients with CD10(+) and 61% in patients with CD10(-). However, the results were not statistically significant (p>0.05). ALL patients were divided into two groups as ALL with positive T cell markers and ALL with positive B cell markers in the foreground and evaluated accordingly, and the OS of ALL patients with positive T cell markers in the foreground was lower than ALL patients with positive B cell markers in the 5 year (59% and 75%, respectively) (p =0.518). On the other hand, AML patients were divided into two groups as AML with positive lymphoid markers and AML with negative lymphoid markers, and the 5 year OS of AML patients with positive lymphoid markers was higher than AML patients with negative lymphoid markers (83% and 70%, respectively) (p = 0.638). These results were not statistically significant. Conclusion: In the flow cytometric evaluation of our patients with leukemia, it was found that patients with CD10(+) had a better 5-year OS and patients with CD2(+) had a lower 5-year OS. The results of patients with positive B cell markers were better in ALL patients, and the results of patients with positive lymphoid markers were better in AML patients.

Amaç: Medulloblastom, çocuklarda en sık görülen malign beyin tümörüdür. Medulloblastom için mevcut tedavi stratejisi cerrahi, ardından radyoterapi ve kemoterapiyi içerir. Bu çalışmada kurumumuzda son 15 yılda tedavi edilen medulloblastomlu hastaların klinik özellikleri, risk kategorileri, tedavileri ve sonuçlar analiz edilmiştir.Gereç ve Yöntem: Çalışmamıza Ocak 2004 - Aralık 2018 tarihleri arasında medulloblastom tanısı alan 84 hasta dahil edildi. Hastaların hastane dosyaları incelendi ve klinik ve epidemiyolojik özellikler, histopatoloji, tedavi yöntemleri, prognostik kriterler ve sağkalım oranları veri formunda not edildi.Bulgular: Tanı anında ortanca yaşı 8 olan 45 erkek ve 39 kadın vardı. Hastaların yaklaşık % 12'si Suriyeli mültecilerdi. Tanı sırasındaki yaşa, cerrahi sonrası rezidüel tümörün boyutuna ve Chang evreleme sistemine bağlı olarak standart risk grubuna 54, yüksek risk grubuna 30 hasta dahil edildi. Beş yıllık genel sağkalım ve olaysız sağkalım oranları sırasıyla% 58.1 ve% 57.6 idi. Sağkalım üzerinde istatistiksel olarak anlamlı bir etkiye sahip prognostik faktörler histopatolojik grup, cerrahi tip ve risk grubuydu. Çok değişkenli analizde, sadece risk grubunun sağkalımı etkilediği bulundu.Sonuç: Hastanın yaşını, cerrahi rezeksiyon derecesini ve metastaz durumunu kullanan risk grubu parametresi medulloblastomlu hastaların sağkalımına etki eden en önemli faktördür.

Amaç: Bu çalışmanın amacı bölgemizdeki pediatrik lenfomaların histopatolojik alt tiplerinin oranını ve bunların Ebstein-Barr virüs enfeksiyonu ile olan ilişkisini incelemektir.Gereç ve Yöntem: Hodgkin lenfoma tanısı alan 36 olgu ve non-hodgkin lenfoma tanısı alan 51 olgu retrospektif arşiv taraması yapılarak çalışmaya dahil edildi. Olgulara immünohistokimyasal olarak Ebstein-Barr virüs latent membran proteini-1 ve in-situ hibridizasyon uygulandı.Bulgular: Hodgkin lenfoma olgularında; en sık rastlanan histopatolojik alt tipler sırası ile mikst sellüler klasik hodgkin lenfoma (%55,6) idi. Non-Hodgkin lenfoma olgularında en sık rastlanan histopatolojik alt tip Burkitt lenfoma (%51) idi. Ebstein-Barr virüs latent membran proteini-1 ile 29 hodgkin lenfoma olgusunda pozitif iken non-hodgkin lenfoma olgularında immünohistokimya ile pozitif boyanma saptanmadı. Hodgkin lenfoma olgularında in situ hibridizasyon ile boyanma oranı % 83.,3 iken non-Hodgkin lenfoma olgularında bu oran % 27.5 idi.. Her iki boyama yöntemi için gruplar arasındaki fark istatistiksel olarak anlamlıydı.Sonuç: Bu çalışmada, olgularımızın histopatolojik dağılımlarının gelişmekte olan ülkelerdeki pediatrik lenfoma olguları ile benzer olduğunu göstermektedir. Aynı zamanda in situ hibridizasyon’da Ebstein-Barr virüs pozitifliği daha fazla olması nedeniyle in situ hibridizasyonun ebstein barr virüs tespitinde immünohistokimyadan daha spesifik olduğunu düşünmekteyiz.

Amaç: Vitamin B12 ve folik asit DNA sentezi, tamiri ve ekspresyonunda önemli görevler alırlar. Bu çalışmada çocukluk çağı kanserleri ile vitamin B12 ve folik asit düzeyleri arasındaki ilişki araştırıldı.Gereç ve Yöntem: Kasım 2013 ve Aralık 2015 arasında yeni tanı alan solid tümörlü 125, lenfoproliferatif/miyeloproliferatif malign hastalığı olan 113 hasta ve 63 kontrol çalışmaya dahil edildi. Vitamin B12 ve folik asit düzeyleri tanısal çalışmaların bir parçası olarak tanı anında ölçüldü.Bulgular: Vitamin B12 ve folik asit düzeyleri malign hastalığı olan çocuklarda kontrol grubuna oranla anlamlı olarak düşük bulundu. Homosistein düzeyleri ise kontrol grubuna göre istatistiksel olarak daha yüksekti. Folik asit düzeyleri lenfoproliferatif/miyeloproliferatif malign hastalığı olanlarda solid tümör grubuna oranla daha düşüktü.Sonuç: Düşük vitamin B12 ve folik asit düzeyleri kanser tanısında destekleyici belirteçler olarak kullanılabilir. Bu ilişkinin gelecekte daha geniş serilerle ortaya konması gerekmektedir.

Amaç: Bu çalışmanın amacı çocukluk çağı akut myeloid lösemilerinde (AML) hücre çoğalması ve apoptozis ile ilgili yollarla bağlantılı olduğu düşünülen lamin A/C, lamin B1 ve lamin B2 proteinlerinin gen ekspresyon durumunun saptanması ve prognoz ile olan ilişkilerinin değerlendirilmesidir.Gereç ve Yöntem: AML tanısı alan 25 olgu ve 35 kontrol olgu çalışmaya alındı. Tanı anında ve indüksiyon tedavisi bitiminde lamin A/C, lamin B1 ve lamin B2 protein gen ekspresyonları Real Time-Polimeraz Zincir Reaksiyonu (RT-PCR) yöntemi ile çalışıldı. Bulgular: AML hastalarında, tanı anında bakılan lamin B1 protein gen ekspresyonu kontrol grubuna göre belirgin olarak düşük bulundu. İndüksiyon tedavisi sonrası bakılan lamin A/C ve B2 protein gen ekspresyonu, tanı anına göre anlamlı olarak düşük saptandı. Lamin B1’de istatistiksel olarak anlamlı değişiklik saptanmadı. Hepatomegalisi olan AML hastalarında, hepatomegalisi olmayanlara göre lamin A/C, splenomegalisi olan AML hastalarında, splenomegalisi olmayanlara göre hem lamin A/C, hem de lamin B2 protein gen ekspresyonu artmış bulundu. Sonuç: Çocukluk çağı akut myeloid lösemilerinde, lamin B1 protein gen ekspresyonu, tanısal bir belirteç olarak, indüksiyon tedavisi sonrasında lamin A/C ve B2’nin azalması ise tedaviye alınan yanıtın bir göstergesi olarak kullanılabilir.

In this study, we aimed to prospectively analyze the emergency diagnosis and clinical characteristics of the oncologic emergency patients admitted, as well as to evaluate the approach considerations to the cases, their treatment, patient outcomes and the prognostic factors. A total of 88 oncologic emergency patients (90 oncologic emergency hospitalizations), except for febrile neutropenia, were admitted to the pediatric oncology department between July 2014 and December 2015. A total of 30 cases of tumor lysis syndrome, 19 hyperleukocytosis, 11 convulsion, 7 spinal cord compression, 7 hemorrhagic cystitis, 9 pleural effusion, 6 vena cava superior syndrome, 4 typhlitis, 3 anuria, 4 gastrointestinal bleeding, 3 hypercalcemia, 4 acute respiratory distress syndrome, 2 acute massive hepatomegaly and 1 increased intracranial pressure were identified. Sixty-five (72.2%) of the study patients had a primary oncologic disease, whereas 25 (27.8%) had a recurring condition. The most common presenting complaint was fever (23.3%) followed by breathing difficulties (22.2%). Fifty-nine patients (65.6%) recovered and were discharged home, while 31 cases (34.4%) died. Tumor lysis syndrome was the most frequently encountered emergency condition in the pediatric oncology patient group. Oncologic emergencies need to be approached quickly and systematically in the light of current scientific knowledge.

İkincil malign hastalıklar herediter retinoblastomlu hastalarda artmış bir sıklıkta gelişebilmektedir. Retinoblastom yaşayanlarında en sık rastlanan malign hastalıklar yumuşak doku sarkomları ve osteosarkomdur. İkincil tümör olarak lenfoma gelişmesi ise nadiren bildirilmiştir. Bu yazıda ünilateral retinoblastom nedeniyle enükleayon ve radyoterapi ile tedavi edilen, 12 yıl sonra lenfoma tanısı alan ve heterozigot p.R552 (c.1654C> T) mutasyonu saptanan 13 yaşında bir hasta rapor edilmiştir. Giriş: Bakır, çinko, magnezyum dahil olmak üzere birçok iz elementlerin küçük miktarları dahi birçok biyolojik organların temel bileşenleri olarak rol oynamaktadır. Bu elementlerin serum konsantrasyonları bazı malign durumlarda değişiklik gösterirler. Bu çalışmanın amacı, çeşitli malign hastalığı olan çocukların serumlarındaki bakır, çinko ve magnezyum düzeylerini araştırmaktır. Materyal ve Metot: Çeşitli malign hastalıklara sahip 82 çocuğun ve kontrol grubunu oluşturan yaş olarak denk 21 sağlıklı çocuğun bakır, çinko ve magnezyum düzeyleri atomik absorpsiyon spektrofotometre kullanılarak ölçüldü.
