Objective To compare lesion conspicuity on synthetic screening mammography (SM) plus digital breast tomosynthesis (DBT) versus full field digital mammography (FFDM) plus DBT. Materials and methods Seven breast imagers each prospectively evaluated 107–228 screening mammograms (FFDM, DBT, and SM; total 1206 examinations) over 12 weeks in sets of 10–50 consecutive examinations. Interpretation sessions alternated as follows: SM + DBT, then FFDM, or FFDM + DBT, then SM. Lesion conspicuity on SM versus FFDM (equal/better versus less) was assessed using proportions with 95% confidence intervals. DBT-only findings were excluded. Results Overall 1082 of 1206 (89.7%) examinations were assessed BI-RADS 1/2, and 124 of 1206 (10.3%) assessed BI-RADS 0. There were 409 evaluated findings, including 134 masses, 119 calcifications, 72 asymmetries, 49 architectural distortion, and 35 focal asymmetries. SM conspicuity compared to FFDM conspicuity for lesions was rated 1) masses: 77 (57%) equal or more conspicuous, 57 (43%) less conspicuous; 2) asymmetries/focal asymmetries: 61 (57%) equal or more conspicuous, and 46 (43%) less conspicuous; 3) architectural distortion: 46 (94%) equal or more conspicuous, 3 (6%) less conspicuous; 4) calcifications: 115 (97%) equal or more conspicuous, 4 (3%) less conspicuous. SM had better conspicuity than FFDM for calcifications and architectural distortion and similar conspicuity for most masses and asymmetries. Conclusion Compared to FFDM, SM has better conspicuity for calcifications and architectural distortion and similar conspicuity for most masses and asymmetries.

Background Mammography has been the mainstay for the detection of breast cancer over decades. It has gradually advanced from screen film to full-field digital mammography. Tomosynthesis has evolved as advanced imaging for early diagnosis of breast lesions with a promising role in both diagnostic and screening settings, particularly in dense and treated breasts. Results This study included 90 female patients according to our inclusion criteria. All patients perform full-field digital mammography (FFDM) and digital breast tomosynthesis (DBT) and were classified according to breast density and age groups. Breast imaging reporting and data system (BI-RADS) scoring was assigned for each case. This was correlated with the final diagnosis; the diagnostic indices of mammography were a sensitivity of 64.44%, a specificity of 77.78%, a positive predictive value (PPV) 74.63%, a negative predictive value (NPV) of 68.63%, and a diagnostic accuracy of 71.11%. Diagnostic indices of DBT were a sensitivity of 100%, a specificity of 97.77%, PPV 97.78%, NPV 100%, and diagnostic accuracy of 97.7%. In patients with dense breasts American College of Radiology (ACR) (c and d), 61% of cases had changed their BIRADS scoring with the addition of tomosynthesis. Yet, in non-dense breast ACR (a and b), 45% of cases had changed BIRADS scoring with the addition of DBT to FFDM. Conclusion DBT is a promising imaging modality offering better detection and characterization of different breast abnormalities, especially in young females, and those with dense breasts with an increase of sensitivity and specificity than FFDM. This leads to a reduction in the recalled cases, negative biopsies, and assessing the efficacy of therapy as it enables improving detection of breast cancer and different breast lesions not visualized by conventional mammography

Objective The aim: Comparing sensitivity and specificity of digital breast tomosynthesis and full-field digital mammography in breast cancer detection associated with four different types of asymmetries according to BI-RADS Atlas. Patients and methods Materials and methods: Study included 201 patients with four types of asymmetries according BI-RADS atlas (asymmetry - 81 (40,3%), focal asymmetry - 82 (40,8%), global asymmetry - 36 (17,9%) and developing asymmetry - 2 (1,0%)) who underwent full-field digital mammography, digital breast tomosynthesis and hand-held full breast ultrasound from January 2017 to June 2018. The general rate of breast cancer for the 201 patients with asymmetries was 8 cases (4,0%) (IBC, n=6 (3,0%); DCIS, n=2 (1,0%) other findings associated with asymmetries were non-malignant, n=10 (5,0%) (sclerosing adenosis, n=5 (2,5%); fibroadenomatosis, n=3 (1,5%); simple cyst, n=1 (0,5%); radial scar associated with papilloma, typical ductal hyperplasia and sclerosing adenosis, n=1 (0,5%). Results Results: Analysis of the results showed that sensitivity of digital breast tomosynthesis was 75.0% [95% CI, 34.91% to 96.81%] and specificity was 94.8% [95% CI, 90.68% to 97.49%] which was superior to full-field digital mammography sensitivity 50.0% [95% CI, 15.70% to 84.30%] and specificity 91.19% [95% CI, 86.27% to 94.78%] for breast cancer detection associated with different types of asymmetries. Conclusion Conclusions: Using of digital breast tomosynthesis in assessment of breast asymmetries can improve sensitivity and specificity in breast cancer detection and reduce number of unnecessary biopsies and short-interval follow-up examinations.

Introduction To investigate compliance to the ‘30% rule’ and key factors which may influence visualisation of the pectoralis major muscle (PMM) on the craniocaudal (CC) view of the breast. Methods A retrospective review of 2688 paired full‐field digital mammography (FFDM) CC view mammograms of women attending BreastScreen NSW between August and October 2015 was undertaken. PMM visualisation and measurements of PMM width and length, compressed breast thickness, the posterior nipple line (PNL) and age were recorded. Statistical analysis was performed using descriptive and inferential statistics to investigate associations between key breast measurements, age and PMM visualisation. Results PMM visualisation was reported in 10.4% of images unilaterally (one breast, left or right only), 14.1% bilaterally (both left and right breasts) and 24.5% overall (unilateral and bilateral combined). There was little or no correlations between PMM length or width and age, breast compressed thickness or PNL. Multiple logistic regression analysis found that up to 15% of the variance in visualisation of the PMM was accounted for by the predictors overall. While some predictors provided a statistically significant contribution to the model, the contribution was small and the odds ratio for all predictors approximated 1. Conclusion This research could not replicate the ‘30% rule’, and visualisation of the PMM was determined not to be influenced by the variables investigated. The significance of the ‘rule’ itself must be challenged where the vast majority of images (70–85%) do not comply, and there is no requirement for repeat imaging if the ‘rule’ is not met. Further research should be undertaken to validate this study including analysis of diagnostic images for comparison., The ‘30% rule’ was first documented 27 years ago, in the film screen era, by Bassett et al., establishing that the pectoralis major muscle (PMM) was able to be visualised in 32% of craniocaudal breast images. Subsequently, a wide range of visualisation rates (11–48%) has been cited. This study reports that the ‘30% rule’ is not being achieved in the Australian digital screening setting. The overall visualisation rate of the PMM was 24.5% with only 14.1% of images depicting the PMM bilaterally. No significant associations between PMM visualisation and age, compressed breast thickness or breast size were identified.

Introduction There are increasing concerns about radiation exposure among women who undergo full‐field digital mammography (FFDM) and digital breast tomosynthesis (DBT). The main aim of this study was to compare the entrance surface dose (ESD) and average glandular dose (AGD) from FFDM and DBT for different breast thicknesses. Methods The ESD and AGD for FFDM in craniocaudal, mediolateral oblique and DBT in craniocaudal projection were recorded from a GE Senographe Essential FFDM unit. The accuracy of the ESD and AGD from the FFDM unit was verified during regular quality assurance programme. Patients were categorised according to their compressed breast thicknesses. X‐ray tube potential and target filter combinations were varied with ESD and AGD recorded directly from the FFDM unit. The non‐parametric Kruskal–Wallis, Mann–Whitney and Wilcoxon signed‐rank tests were performed. Results The median and interquartile range (IQR) age of the patients were 48 and 11 years, respectively. The highest median for ESD and median total AGD for different breast thicknesses were ranged from 3.3 to 9.1 mGy and 3.3 to 6.0 mGy, respectively, for two‐view FFDM. However, it ranged from 3.1 to 8.9 mGy and 1.8 to 4.0 mGy, respectively, for single‐view DBT. Both ESD and AGD were significantly lower for DBT (P, There are some concerns about increasing the radiation dose among women undergoing both full‐field digital mammography (FFDM) and digital breast tomosynthesis (DBT). In our study, we found that the entrance skin dose (ESD) and total average glandular dose (AGD) are lower in DBT than FFDM techniques. This offers opportunities for the implementation of DBT in the breast cancer detection.

Purpose The advent of three-dimensional breast imaging systems such as digital breast tomosynthesis (DBT) has great promise for improving the detection and diagnosis of breast cancer. With these new technologies comes an essential need for testing methods to assess the resultant image quality. Although randomized clinical trials are the gold standard for assessing image quality, phantom-based studies can provide a simpler and less burdensome approach. In this work, a complete framework is presented for task-based evaluation of microcalcification (MCs) detection performance for DBT imaging systems. Methods The framework consists of three parts. The first part is a realistic anthropomorphic physical breast phantom created through inkjet printing, with parchment paper and iodine-doped ink. The second is a method for inserting realistic MCs fabricated from calcium hydroxyapatite. The reproducibility and stability of the phantom materials were investigated through multiple samples of parchment and ink over 6 months. The final part is an analysis using a four-alternative forced choice (4AFC) reader study. To demonstrate the framework, a task-based 4AFC study was conducted using a clinical system to compare performance from DBT, synthetic mammography (SM), and full-field digital mammography (FFDM). Nine human observers read images containing MC clusters imaged with all three modalities and tried to correctly locate the MCs. The proportion correct (PC) was measured as the number of correctly detected clusters out of all trials. Results Overall, readers scored the highest with FFDM, (PC = 0.95 ± 0.03) then DBT (0.85 ± 0.04), and finally SM (0.44 ± 0.06). For the parchment and ink samples, the linear attenuation properties were very stable over 6 months. In addition, little difference was found between the various parchment and ink samples, indicating good reproducibility. Conclusions This framework presents a promising methodology for evaluating diagnostic task performance of clinical breast DBT systems.

Currently in diagnostic setting for breast cancer, FFDM and DBT are performed conjunctively. However, performing two imaging modalities may increase radiation exposure by double. Two-dimensional reconstructed images created from DBT with 2DSM, has a potential to replace conventional FFDM in concerning both radiation dosage and image quality. With increasing concerns for individual radiation exposure, studies analyzing radiation dosage in breast imaging modalities are needed. This study compared radiation dosage and image quality between DBT + 2DSM versus FFDM.374 patients (mean age 52 years) who underwent both DBT and FFDM were retrospectively reviewed. Radiation dosage data were obtained by radiation dosage scoring and monitoring program Radimetrics (Bayer HealthCare, Whippany, NJ). Entrance dose and mean glandular doses in each breast were obtained for both modalities. To compare image quality of DBT + 2DSM and FFDM, a 5-point scoring system for lesion clarity was assessed. The parameters of radiation dosage (entrance dose, mean glandular dose) and image quality (lesion clarity scoring) were compared.For entrance dose, DBT had lower mean dosage (14.8 mGy) compared with FFDM (21.8 mGy, p-value 0.0001). Mean glandular doses for both breasts were lower in DBT (Left 1.74, Right 2.1) compared with FFDM (Left 2.85, Right 2.74, p-value 0.0001). Lesion clarity score was higher in DBT with 2DSM (mean score 4.03) compared with FFDM (3.82, p-value 0.0001).DBT showed lower radiation entrance dose and mean glandular doses to both breasts compared with FFDM. DBT + 2DSM had better image quality than FFDM, suggesting that DBT with 2DSM has potential as an alternative to FFDM.

Synthesized 2D images can be reconstructed from tomosynthesis images in breast imaging. This study aims to investigate the diagnostic efficacy of synthesized 2D images (C-View) in comparison to full field digital mammography (FFDM) when used with digital breast tomosynthesis (DBT) in multi-ethnic Malaysian population. FFDM and C-View images (n = 380) were independently evaluated by three readers through Breast Imaging Reporting and Data System (BI-RADS) categorisation, breast density and lesion characterisation. Statistical analysis was done comparing sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of C-View + DBT with FFDM + DBT as standard of reference. Very good interreader agreement in BI-RADS category and density assessment between C-View + DBT and FFDM + DBT, with high sensitivity, specificity, PPV and NPV of C-View + DBT when compared with FFDM + DBT. There was comparable PPV between C-View + DBT and FFDM + DBT, with histopathology as gold standard. High level of interreader agreement in BI-RADS category and density assessment for FFDM + DBT and C-View + DBT. There was good agreement between FFDM + DBT and C-View + DBT in mass characterization, and almost perfect agreement in calcification and asymmetric density. 52.2% lower radiation dose incurred when using C-View + DBT. Hence, synthesized 2D images are comparable to FFDM with reduction in radiation dose within the limits of Malaysian multi-ethnic population.

Randomized, Controlled Trials (RCT), the most rigorous tests of efficacy, had proven that mammography screening reduces deaths by early detection. This had been validated in studies that showed that screening in the community also resulted in fewer deaths. Film mammography (FM) had been replaced by Xeroradiography (XM) which had been replaced by Screen/Film Mammography (SFM) which was being replaced by Full Field Digital Mammography (FFDM). The FDA required FFDM to undergo a Premarket Approval (PMA) instead of the usual 510 K required for conversion for other x-ray studies to digital imaging. In addition, the FDA was going to require that even after a PMA, the FFDM systems were going to have to undergo a post approval RCT comparison to SFM. Experience and science were able to convince the FDA that a post-approval trial was not needed, and the requirement was dropped. Congress, however, had earmarked $25 million to support the trial that was no longer required. It appears that the Digital Mammographic Imaging Screening Trial (DMIST) was undertaken to take advantage of the earmarked money and was used to compare SFM to FFDM for cancer detection. The historical issues involved with DMIST provide an important background for the Tomosynthesis Mammographic Imaging Screening Trial (TMIST). Just as the monies for DMIST could have been better spent on an RCT of MRI for screening, the monies for TMIST could be better spent to improve our ability to detect more breast cancers at a time in their growth when cure may be possible.

Objectives: We aimed to compare the recall rate (RR) and the cancer detection rate (CDR) of combined full field digital mammography and digital breast tomosynthesis (FFDM + DBT) to those of full field digital mammography (FFDM) alone in breast cancer survivors. Methods: We enrolled 146 female breast cancer survivors schedule. All patients underwent FFDM and DBT in the same setting. Results of FFDM alone were compared to those of FFDM + DBT regarding patients' RR and CDR.Sensitivity, specificity, accuracy, positive and negative predictive values were also calculated for FFDM alone and for FFDM + DBT in detecting breast cancer lesions. Results: Our results showed that FFDM + DBT decreased patients' RR by 3.4% and increased the CDR by 4.1%. Reduction in RR was evident in higher breast densities. FFDM mammography had 18 false negative lesions and 29 false positives. Sensitivity, specificity, accuracy, NPV and PPV in detecting breast lesions were: 84.2%, 53.1%, 64.0%, 86.7% and 48.9% for FFDM compared to 100%, 92.1%, 95.3%, 100% and 89.7% for FFDM + DBT. Conclusion: Combined FFDM + DBT in the post breast cancer surveillance regimen has shown to reduce the patients' RR and to increase the CDR. FFDM + DBT had higher diagnostic accuracy than FFDM alone. FFDM + DBT ought to be a standard combination in the breast cancer surveillance in treated patients. This pictorial essay demonstrates the variable appearances of ductal carcinoma in situ on full-field digital mammography, synthesized mammography, and digital breast tomosynthesis. The spectrum of intercase and intracase variability suggests further refinement of reconstruction algorithms for synthesized mammography may be necessary to maximize early detection of ductal carcinoma in situ.

Mobile mammography can improve access to screening mammography in rural areas and underserved populations. We evaluated the frequency of incomplete reports in mobile mammography screening and the relationships between prior mammograms and recall rates. The frequency of incomplete mammogram reports, the subgroups of those needing prior comparison mammograms, recalls for additional imaging, and availability of prior mammograms of a mobile screening mammography unit were compared with fixed site mammography from January 1, 2007 through December 31, 2009. All mobile unit mammograms were full field digital mammography (FFDM). Differences between rates of recall, incomplete reports, and availability of prior mammograms were calculated using the Chi-Square statistic. Of 2640 mobile mammography cases, 21.9% (578) reports were incomplete, versus 15.2% (7653) (p ≤ 0.001) of 50325 fixed site reports. Of incomplete cases, recall for additional imaging occurred among 8.3% (218) of mobile mammography reports versus 11.3% (5708) (p ≤ 0.001) of fixed site reports. Prior mammograms were needed among 13.6% (360) of mobile mammography versus 3.9% (1945) (p ≤ 0.001) of fixed site reports. Mobile mammography recall rate varied with availability of prior mammograms: 16.0% (54) when no prior mammograms, 7.6% (127) when prior mammograms were elsewhere but unavailable and 5.9% (37) when prior FFDM were immediately available (p ≤ 0.001). Incomplete reports were more frequent in mobile mammography than the fixed site. The availability of prior comparison mammograms at time of interpretation decreased the rate of incomplete mammogram reports. Recall rates were higher without prior comparison mammograms and lowest when comparison FFDM mammograms were available.

Objective To determine what percentage of cancers, detected by screening ultrasonography (US), were detectable by full-field digital mammography (FFDM) and digital breast tomosynthesis (DBT). Materials and methods Eighty-four consecutive women in whom mammography was negatively interpreted and supplementary screening US initially detected breast cancers at outside hospitals underwent both FFDM and DBT. We excluded cases with overt suspicious findings on repeat mammography and ineligible cases. In the remaining 41 cases, three radiologists who were blinded to tumor location, even though they were aware that they had breast cancers independently reviewed both FFDM and DBT. The reference standard was the reference FFDM made by two unblinded reviewers who were aware of the tumor location and shape on DBT, US, and magnetic resonance imaging (MRI). The visibility score based on the correct marking was compared between FFDM and DBT. Results Among the 41 cases, the cancers were visible in 25 (61.0%) on FFDM and in 34 (82.9%) on DBT ( P = 0.047) by the unblinded review. In the blinded analysis, the cancers were significantly more "constantly visible" in the three radiologists on DBT than on FFDM [53.7% (22/41) vs. 26.8% (11/41), respectively, P = 0.013]. The dominant lesion type was "focal asymmetry" on DBT (39.0%) and "asymmetry" on FFDM (31.7%). Conclusions Our analysis suggests that 54% of cancers that were detected by US and were not evident on 2D mammography were detectable by screening using DBT. Additional 29% of cancers were visualized on DBT, when the area of concern was known.

Clinical studies using different imaging protocols to perform digital breast tomosynthesis (DBT) were reviewed (2008-14) to assess interpretive accuracy. Descriptive pooled statistics were used to estimate and summarise accuracy measures for each type of imaging protocol in relation to that of two-view full-field digital mammography (FFDM). In studies comparing multiple DBT imaging protocols, a trend of increased performance was often seen when including both the mediolateral oblique and craniocaudal views for DBT alone and even more so for DBT adjunct to FFDM. Overall, the average ΔAUC (%; sd) across studies for stand-alone DBT (relative to FFDM), in one and in two views, were 2.2 (± 3.7) and 5.9 (± 4.6), and when used together with FFDM, 3.9 (± 2.0) and 6.7 (± 0.9). With respect to individual studies, improvements in accuracy using DBT were present for different types of imaging protocols although the magnitude of the impact varied between studies, and some studies did not show significant improvements in comparison with FFDM. The most consistent effect of improvement in breast cancer detection was seen across studies for two-view DBT with FFDM. These summary findings may depend on the sampling constraints present in tomosynthesis imaging and on other factors discussed in this paper. In order to investigate these effects more thoroughly and how they might impact outcomes, comparative or randomized-controlled trials are warranted.

We aimed to investigate the effects of mammary gland density and average glandular dose (AGD) on contrast-to-noise ratio (CNR) of breast-equivalent phantoms with different mammary gland/fat tissue ratios. Full-field digital-mammography breast X-rays were performed on breast-equivalent phantoms with three different mammary gland/fat tissue ratios (Phantom A [30/70], Phantom B [50/50], and Phantom C [70/30]) and seven thicknesses ranging from 10 mm to 70 mm. The prediction formula for the CNR was calculated by multivariate analysis and the effects of the various parameters on CNR were evaluated using a multiple regression analysis model. Higher CNR values were obtained with lower mammary gland/fat tissue ratios and lower phantom thicknesses. Variation in CNR among the three breast models was low (coefficient of variation, 3.4–8.7%) at lower phantom thicknesses (10–30 mm) and high (coefficient of variation, 10.5–16.8%) at higher phantom thickness (50–70 mm). CNR showed a strong negative correlation (r = -0.8989) with AGD across all three mammary gland ratios. A predictive formula for CNR using AGD and mammary gland density was developed. CNR can be predicted with high precision using AGD and mammary gland density. The predicted CNR could be used to measure the diagnostic reliability of mammography in breast cancer.

Objectives Contrast-enhanced spectral mammography (CESM) shows promising initial results but comes at the cost of increased dose as compared with full-field digital mammography (FFDM). We aimed to quantitatively assess the dose increase of CESM in comparison with FFDM. Materials and methods Radiation exposure-related data (such as kilovoltage, compressed breast thickness, glandularity, entrance skin air kerma (ESAK), and average glandular dose (AGD) were retrieved for 47 CESM and 715 FFDM patients. All examinations were performed on 1 mammography unit. Radiation dose values reported by the unit were validated by phantom measurements. Descriptive statistics of the patient data were generated using a statistical software package. Results Dose values reported by the mammography unit were in good qualitative agreement with those of phantom measurements. Mean ESAK was 10.5 mGy for a CESM exposure and 7.46 mGy for an FFDM exposure. Mean AGD for a CESM exposure was 2.80 mGy and 1.55 mGy for an FFDM exposure. Conclusions Compared with our institutional FFDM, the AGD of a single CESM exposure is increased by 1.25 mGy (+81%), whereas ESAK is increased by 3.07 mGy (+41%). Dose values of both techniques meet the recommendations for maximum dose in mammography.

Objective To evaluate the impact of adding 3D Tomosynthesis to Full Field Digital Mammography (FFDM) in the detection and diagnosis of breast lesions. Subjects and methods The study included 166 mammograms with indeterminate findings selected from 1600 mammograms. They were classified into two groups: group 1 ‘Diagnostic mammograms’ of symptomatic women and group 2 ‘Screening mammograms’. Dense breasts assigned as ACR3 and ACR4 presented 69% ( n = 114/166) of the studied cases. FFDM and 3D tomosynthesis examination was done and imaging findings were evaluated before and after the use of 3D tomosynthesis images. Results Both modalities were compared regarding detection and diagnosis, each individually assessed, using the Pearson Chi Square tests. Detection ( P value: 0.006) and diagnosis ( P value: 0.048) of breast lesions dramatically improved when 3D tomosynthesis images were considered in the evaluation. The sensitivity, specificity, and accuracy of digital mammography was 60%, 20.7% and 48% have significantly enhanced on applying tomosynthesis to be 94.5%, 74% and 89.7%. Conclusion Three-dimensional tomosynthesis significantly enhanced the detection and characterization of breast lesions on digital mammography especially in the context of dense breast parenchyma (ACR 3&4).

Overdiagnosis of breast cancer, i.e. the detection of slow-growing tumors that would never have caused symptoms or death, became more prevalent with the implementation of population-based screening. Only rough estimates have been made of the proportion of patients that are overdiagnosed and identification of those patients is difficult. Therefore, the aim of this study is to evaluate whether tumor biology can help identify patients with screen-detected tumors at such a low risk of recurrence that they are likely to be overdiagnosed. Furthermore, we wish to evaluate the impact of the transition from film-screen mammography (FSM) to the more sensitive full-field digital mammography (FFDM) on the biology of the tumors detected by each screening-modality. All Dutch breast cancer patients enrolled in the MINDACT trial (EORTC-10041) accrued 2007–2011, who participated in the national screening program (biennial screening ages 50–75) were included (n = 1,165). We calculated the proportions of high-, low- and among those the ultralow-risk tumors according to the 70-gene signature for patients with screen-detected (n = 775) and interval (n = 390) cancers for FSM and FFDM. The diagnostic reference level (DRL) has generally been defined as the 75th percentile of the distributions of mean doses observed on a sample of standard-sized patients, but for mammography, the 95th percentile has been used. In this study, the 75th and 95th percentiles are estimated for 26 full-field digital mammography units, representing six models from four manufacturers. Systematic differences between categories of manufacture/models are investigated with respect to mean glandular dose (MGD) and figure of merit (FOM), defined as signal difference to noise ratio squared divided by the MGD. The MGDs per unit range from 0.7 to 2.1 mGy, with overall 75th and 95th percentiles of 1.4 and 2.0 mGy, respectively. The different manufacture/models show differences in both dose distributions and FOMs. As national DRL, the 95th percentile is proposed to determine which units can be accepted for use. To identify the need for optimisation, it is proposed to use the 75th percentile for the different manufacture/models, along with the FOM.

Aim: This prospective clinical study aimed to evaluate whether it would be possible to reduce the rate of re-excisions using CMOS technology, a specimen radiography system (SRS) or digital breast tomosynthesis (DBT) compared to a conventional full field digital mammography (FFDM) system. Material and Method: Between 12/2012 and 2/2013 50 patients were diagnosed with invasive breast cancer (BI-RADS™ 5). After histological verification, all patients underwent breast-conserving therapy with intraoperative imaging using 4 different systems and differing magnifications: 1. Inspiration™ (Siemens, Erlangen, Germany), amorphous selenium, tungsten source, focus 0.1 mm, resolution 85 µm pixel pitch, 8 lp/mm; 2. BioVision™ (Bioptics, Tucson, AZ, USA), CMOS technology, photodiode array, flat panel, tungsten source, focus 0.05, resolution 50 µm pixel pitch, 12 lp/mm; 3. the Trident™ specimen radiography system (SRS) (Hologic, Bedford, MA, USA), amorphous selenium, tungsten source, focus 0.05, resolution 70 µm pixel pitch, 7.1 lp/mm; 4. tomosynthesis (Siemens, Erlangen, Germany), amorphous selenium, tungsten source, focus 0.1 mm, resolution 85 µm pixel pitch, 8 lp/mm, angular range 50 degrees, 25 projections, scan time > 20 s, geometry: uniform scanning, reconstruction: filtered back projection. We analysed 300 patients X rayed with digital breast tomosynthesis (DBT), full-field digital mammography (FFDM) and 'COMBO' (single view both in FFDM and DBT in a single breast compression)-Hologic Selenia Dimension-W/Rh-Ag-Al: three different anode-filter combinations-automatic exposure control modalities. Examination parameter data collection (EPDC) and 'in vivo' dosimetry using metal oxide semiconductor field effect transistor (MOSFET) were utilised to determine and compare entrance skin air kerma (ESAK) and average glandular dose (AGD) from a radioprotection viewpoint in the DBT and COMBO modalities. MOSFET has been tested to be introduced in clinical routine. EPDC of DBT underlines increase in ESAK and AGD compared with FFDM (p0.05). The mean percentage increase was 34 % (+17 %) in ESAK and 46 % (+16 %) in AGD. In the COMBO modality, the mean percentage increase in ESAK was 162 % (+41 %) and in AGD was 202 % (+61 %). Differences between MOSFET measurement and calculated values were8 % for breasts thicker than 30 mm; otherwise, the errors are15 %. DBT increases ESAK and AGD, due to the 3D acquisition modality. MOSFET may be considered in DBT as a routine check.

Computer-aided detection (CAD) systems have been developed for interpretation to improve mammographic detection of breast cancer at screening by reducing the number of false-negative interpretation that can be caused by subtle findings, radiologist distraction and complex architecture. They use a digitized mammographic image that can be obtained from both screen-film mammography and full field digital mammography. Its performance in breast cancer detection is dependent on the performance of the CAD itself, the population to which it is applied and the radiologists who use it. There is a clear benefit to the use of CAD in less experienced radiologist and in detecting breast carcinomas presenting as microcalcifications. This review gives a detailed description CAD systems used in mammography and their performance in assistance of reading in screening mammography and as an alternative to double reading. Other CAD systems developed for MRI and ultrasound are also presented and discussed.

Aim To measure the change in diagnostic accuracy of conventional film-screen mammography and full-field digital mammography (FFDM) with the addition of digital breast tomosynthesis (DBT) in women recalled for assessment following routine screening. Materials and methods Ethics approval for the study was granted. Women recalled for assessment following routine screening with screen-film mammography were invited to participate. Participants underwent bilateral, two-view FFDM and two-view DBT. Readers scored each lesion separately for probability of malignancy on screen-film mammography, FFDM, and then DBT. The scores were compared with the presence or absence of malignancy based on the final histopathology outcome. Results Seven hundred and thirty-eight women participated (93.2% recruitment rate). Following assessment 204 (26.8%) were diagnosed as malignant (147 invasive and 57 in-situ tumours), 286 (37.68%) as benign, and 269 (35.4%) as normal. The diagnostic accuracy was evaluated by using receiving operating characteristic (ROC) and measurement of area under the curve (AUC). The AUC values demonstrated a significant ( p = 0.0001) improvement in the diagnostic accuracy with the addition of DBT combined with FFDM and film-screen mammography (AUC = 0.9671) when compared to FFDM plus film-screen mammography (AUC = 0.8949) and film-screen mammography alone (AUC = 0.7882). The effect was significantly greater for soft-tissue lesions [AUC was 0.9905 with the addition of DBT and AUC was 0.9201 for FFDM with film-screen mammography combined ( p = 0.0001)] compared to microcalcification [with the addition of DBT (AUC = 0.7920) and for FFDM with film-screen mammography combined (AUC = 0.7843; p = 0.3182)]. Conclusion The addition of DBT increases the accuracy of mammography compared to FFDM and film-screen mammography combined and film-screen mammography alone in the assessment of screen-detected soft-tissue mammographic abnormalities.

To assess the effect of breast density, fibroglandular tissue volume, and breast volume on the rate of false-positive marks of a computer-assisted detection software in digital mammography.222 patients with normal digital mammograms and a minimum follow-up of 22 months were retrospectively identified. MLO and CC views were analyzed using a CAD software with three operating points ('specific', 'balanced', 'sensitive'). False-positive marks were recorded. Images were analyzed by a volumetric breast density assessment software, yielding estimates of percentage density, fibroglandular tissue volume, and breast volume. Statistical analysis was performed using the Mann-Whitney U-test, the t-test for independent samples and the Poisson regression model.Patients with high fibroglandular tissue volumes had a higher mean number of false-positive mass marks than patients with low fibroglandular tissue volumes (specific setting: 0.50 vs. 0.35, respectively; balanced setting: 0.70 vs. 0.40, respectively, p0.05; sensitive setting: 0.89 vs. 0.58, respectively, p0.05). Relative risk for a false-positive mass marker increased by 1.43 (p0.05), 1.63 (p0.001) and 1.50 (p0.01) per 100ml of fibroglandular tissue for the specific, balanced and sensitive settings, respectively. No significant effects of percentage density or breast volume on the number or the relative risk of false-positive mass marks were observed.The volume of fibroglandular tissue present, but not the percentage density of the breast, affected the specificity for masses of the CAD software investigated. This may have implications for improving the performance of CAD systems, as the specificity of CAD may be improved by adjusting the algorithm threshold depending on the volume of fibroglandular tissue present. Considering both factors, fibroglandular tissue volume and percentage density, independently, could improve overall CAD performance in subgroups of patients, e.g. those with small, dense breasts or large breasts with low density.

Now, full field digital mammography (FFDM) is widely used in diagnosis of breast cancer. With the development of FFDM, the radiation dose delivered to the patients involved in an imaging protocol is of utmost concern. Here, we analyzed the average glandular dose (AGD) and entrance surface exposure (ESE) of 1397 patients (6008 images) who underwent mammographic examinations by FFDM in three modes with automatic optimization of parameters (AOP), namely STD for standard mode, CNT for contrast mode and DOSE for dose mode. In addition, exposure factors including kVp, tube loading (mAs), and target/filter combination were evaluated. As a result, the patient dose was sorted as CNT > STD > DOSE. The dose difference among the three AOP modes was mainly attributed to the selection of mAs. The AGD and breast compressed thickness were well correlated in STD and DOSE modes. However, the correlation between CNT-delivered AGD and breast compressed thickness was dependent on the range of the breast thickness and patient age. The findings on dose and exposure characteristics of the three AOP modes get useful message of patient dose in the acquisition of FFDM.

BreastCheck, the Irish Breast Screening Programme, has employed three different models of a full field digital mammography (FFDM) system since its transition to a digital service in 2007. The three models from GE Healthcare, Hologic and Sectra exhibit differences in their design and function, the most significant of which include anode target/filter choice, detector technology and the type of exposure automation.The aim of this study was to use the results from a clinical breast dose survey to examine the differences between three different FFDM models in terms of exposure selection, breast mean glandular dose (MGD) and automatic exposure control (AEC) dose contribution.The accuracy of the dose estimation was improved by inclusion of the AEC pre-exposure dose contribution. The photon-counting system demonstrated the lowest average MGD. The GE Healthcare and Hologic flat-panel detector systems demonstrated a small but statistically significant dose difference. The pre-exposure dose contribution did not exceed 13% of the total exposure dose for any system in the survey. A comparison of the system calculated organ dose estimate from each machine with the corresponding MGD calculated from medical physics measurements indicated reasonably accurate organ dose estimates for most systems in the survey.The results of this study provide a comprehensive assessment of the breast dose performance of current digital mammography systems in a clinical screening setting.

The object of this study was to compare of full-field digital mammography (FFDM) workstation and conventional picture archiving and communication systems (PACS) in image quality and diagnostic performance. We assembled 80 masses and 80 microcalcifications. Images were displayed on workstation, 5M, and 3M PACS monitors. The image quality for mammograms on workstation was significantly better than that for mammograms on PACS monitors. The sensitivity and NPV for microcalcifications on workstation were higher than those on PACS monitors. The conventional PACS cannot substitute for a FFDM workstation for mammographic evaluation.

Aim To estimate the acceptable compression ratio of full-field digital mammography (FFDM) using the Joint Photographic Experts Group (JPEG) 2000 compression algorithm. Materials and methods Eighty cases that included images of 40 masses (20 benign, 20 malignant) and 40 microcalcifications (20 benign, 20 malignant) were collected. The images were compressed to five different lossy ratios: 20:1, 40:1, 60:1, 80:1, and 100:1, and four radiologists independently determined whether the compressed group was distinguishable from the control group. The ratio of the compressed group that was rated indistinguishable from the control group was compared for each reviewer, and the results were analysed for agreements of three or more reviewers. Results The ability to distinguish the compressed image from the control group is given as a range across the four reviewers: 0–1.3% (0/80 to 1/80) of the 20:1, 0–2.5% (0/80 to 2/80) of the 40:1, 5–7.5% (4/80 to 6/80) of the 60:1, 10–37.5% (8/80 to 30/80) of the 80:1, and 30–87.5% (24/80 to 70/80) of the 100:1. For three compression groups (20:1, 40:1, and 60:1), three or more reviewers agreed that there was a distinguishable difference for 0/80, 0/80, and 3/80 images, respectively. Thus, the compressed images do not differ significantly from the control group ( p >0.05). However, the 80:1 and 100:1 compressed images were different for 9/80 and 29/80 images, respectively, which is significantly different from the control group ( p Conclusion The lossy 60:1 compression ratio for FFDM is visually identical to the control image and, therefore, potentially acceptable for primary interpretation.

To assess time expenditure using the influence of computer-assisted detection (CAD) system in the interpretation of the dependence of early research and benign and malignant mammograms on readers' experience.CAD (Image Checker V2.3; R2 Technology, Los Altos, CA, USA) was prospectively applied on digital mammograms of 303 patients [early research (n=103), benign (n=102), and malignant group (n=98)]. Mammograms were analyzed by three readers with varying experience in evaluating mammograms (medical student, resident and attending) according to the BI-RADS classification. Time was stopped and recorded. All images were presented randomly with and without the influence of CAD and from the different patient groups. To evaluate the statistical significance, the corresponding P value for time to read the mammograms in addition to different patient groups, application of CAD, readers' experience, and interaction of reader was calculated.The attending needs, independent of CAD application, the least time, followed by the medical assistant and the student. In all three patient groups, CAD adoption elongates reading time of the student and the resident. The medical specialist needs with and without CAD median the same time. In the early research group, no significant differences were registered (P=.1343). Concerning readers' experience, there is an explicit significant difference (P.0001). The application of CAD correlates with the corresponding readers' experience and also provides a not significant result. In comparison, the P value for the malignant and benign groups shows significant interactions between the readers' experiences as well as CAD application.The future role of CAD application depends on whether sensitivity can be increased and time expenditure caused by false-positive marks can be decreased. In the future, second reading could be substituted by a CAD system if the reader has a wide professional experience.

This study was aimed at evaluating the mean glandular dose (MGD) and affecting factors during mammography examinations by a full-field digital mammography unit. An extensive quality control program was performed to assure that the unit is properly working. Required information including compressed breast thickness (CBT), breast parenchymal pattern and technical factors used for imaging were recorded. An entrance skin exposure measurement was also performed using slabs of polymethylmethacrylate with 2-8 cm thickness. On the basis of recorded information and measured data, the MGD was estimated for 1145 mammography examinations obtained from 298 patients. Mean CBTs of 4.9 and 5.8 cm and MGDs of 2 and 2.4 mGy were observed for craniocaudal and mediolateral oblique views, respectively. Significant correlation was seen between MGD and CBT, breast parenchymal pattern and applied kVp and mAs.
