Aim: The aim of this study was to identify charge nurses' experiences ensuring patient safety during the coronavirus disease 2019 (COVID‐19) pandemic. Methods: The research was conducted with charge nurses between July 2021 and February 2022. Research data were collected using a semi‐structured interview with two main questions, six sub‐questions and an audio recording. Interviews were conducted with 12 charge nurses and lasted ≈30 min. The content analysis method was used to transfer the collected data to written documents and define them to analyse the individual interview data. Content analyses were conducted independently by three different researchers. Inclusion and exclusion criteria for content were established to ensure consistency among all researchers. Researchers independently developed a coding scheme for content analysis based on the research objectives and applied it to the content. Intercoder reliability was assessed with Cohen's kappa coefficient to measure the consistency of coding among researchers. Results: Three main themes were identified as a result of the content analysis. The themes identified after independent examination by the three researchers were 'ensuring safety and protection', 'sustaining motivation and resilience' and 'organizational restructuring and challenges'. Conclusions: This study highlights the challenges faced by charge nurses during the COVID‐19 pandemic and their critical role in maintaining patient safety. The findings emphasize the importance of organizational adaptability, continuous motivation and comprehensive risk management strategies. Charge nurses played a key role in enhancing safety measures and fostering a culture of resilience among healthcare staff. Purpose: In this study, we aimed to examine the relationship between depression level, emotion regulation difficulties and social support in individuals with epilepsy. Materials and Methods: Neurological Disorders Depression Inventory-Epilepsy (NDDI-E), Difficulties in Emotion Regulation Scale (DERS), and Two-Way Social Support Scale (2- WSSS) were filled out by the patients followed up with a diagnosis of focal epilepsy. Results: One hundred nine patients were included in the study. Depression was found in 25% of the patients. The median DERS value of the patients with depression was 45 (28-55), and the group without depression was 24 (1833.5). It was determined that the DERS score of the depressed group was significantly higher than the group without depression. There is a negative correlation between social support scale (92 (69.5-102.5)) and emotion regulation difficulties scale (27 (19-41)) in the correlation analysis of the entire study population. According to the regression analysis, difficulties in emotion regulation are the main determinant of depression and explain depression at a rate of 16.8%. Conclusion: Emotion regulation difficulties are one of the main determinants of depression in individuals with epilepsy and inadequate social support is associated with emotion regulation difficulties. In order to reduce the frequency of depression in individuals with epilepsy, rehabilitation programs are needed to strengthen emotion regulation strategies.

Since December 2020, a significantly higher number of people worldwide have been vaccinated for coronavirus disease 2019 (COVID-19). Neurological complications have been reported after these vaccines, although a definitive causal relationship has not been proven in the available literature. We describe a 51-year-old man presenting with anti-glutamic acid decarboxylase (anti-GAD) antibody positive autoimmune encephalitis with progressive cognitive impairment and behavioral abnormalities, presenting shortly after the second dose of mRNA COVID-19 vaccine, possibly representing a serious vaccinerelated adverse event. Response to high-dose steroid and intravenous immunoglobulin treatment was positive. As many people around the world have been vaccinated against COVID-19, this case shows that autoimmune encephalitis and even anti-GAD antibody positive autoimmune encephalitis can develop as a side effect after this new vaccine, but with early diagnosis and appropriate treatment, the clinic can have a good prognosis. Observational studies with large numbers of patients are needed to explain causality. [ABSTRACT FROM AUTHOR]

Autoimmunity reflects an altered immune status. The coexistence of three or more autoimmune diseases in a patient constitutes multiple autoimmune syndrome. Here, an interesting case with a history of Hashimoto's thyroiditis and got the diagnosis of systemic lupus erythematosus during her evaluation, in which the clinical signs of neuromyelitis optica spectrum disorder (NMOSD) and miyasthenia gravis co-occurred will be discussed. In addition to high-dose pulse steroid, plasmapheresis and intravenous immunoglobulin treatments, rituximab treatment was started and the patient is followed up without clinical worsening and disability in the three-year follow-up. Patients with diagnosis of NMOSD need special attention as multiple immune-mediated disorders may be present simultaneously or sequentially during the course of the disease process. This case report emphasizes the importance of a multidisciplinary team approach for a better understanding of disorders related to the breakdown of immune tolerance. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: To investigate the vagus nerve (VN) dimensional changes with Parkinson's disease (PD), compared with healthy subjects. Additionally, it is important to investigate whether there is any relationships between these changes and patient's motor and non-motor symptoms (NMS) of PD. Materials and Methods: A cohort of 43 patients with PD formed a group that was compared with 44 patients without PD, denoted as the healthy subject (HS) group. The diameter and areas of VN of study groups were measured using ultrasonography (US). The study groups were further divided into <65 and ≥65 subgroups, to evaluate the possible effect of age on the VN and evaluated relationships of VN dimensions, between subgroups. In the PD group, a correlational analysis was conducted between the diameter and area of the VN and the motor and NMS scores. Results: There was statistically significant difference in right (P =.002) and left VN diameters (P =.007) and in right (P =.001) and left VN areas (P =.007), between study groups. There was no significant difference in right and left VN diameters and the right and left VN areas, between subgroups. There was moderately negative correlation between gastrointestinal NMS scores and right VN area (r = −0.499, P =.002), left VN area (r = −0.499, P =.002), right VN diameter (r = −0.378, P =.023), left VN diameter (r = −0.385, P =.021), respectively. Conclusion: The US demonstrated that VN dimensions may possibly reduce in those patients affected by PD. In this cohort, it appears that an increase in gastrointestinal NMS scores may be explained by atrophy of the VN. [ABSTRACT FROM AUTHOR]

Introduction: The aim of this study was to form the Turkish adaptation of the SCOPA-SleepScale to be used in evaluating sleep quality in individuals with Parkinson's disease and to test its psychometric properties. Method: Data for this methodological study was collected between May and December 2017 in the neurology outpatient clinic of a hospital through face to face interviews with patients with a diagnosis of Parkinson's disease. The sample of the study consisted of 105 patients of 18 years of age and above with no additional neurological diseases who volunteered for the study. The SCOPA Sleep Scale was translated into Turkish through translation and back translation, and expert views were taken to test content validity. The reliability analyses of the scale were performed using item- total score correlations, test-retest correlations, and internal consistency. Exploratory factor analyses were conducted for construct validity and the Pittsburgh Sleep Quality Index (PSQI) and Epworth Sleepiness Scale (ESS) were applied for criterion validity. The appropriateness of data for factor analysis was examined using the Kaiser-Meyer-Olkin (KMO) and Bartlett's tests. Results: The factor loads of the SCOPA Sleep Scale varied between 0.743 and 0.901, and the Turkish version of the scale exhibited a two factor structure in compliance with the original scale. The Cronbach s alpha coefficient was found 0.907 for the nighttime sleep sub-dimension and 0.906 for the daytime sleepiness sub-dimension. In the test-retest evaluation, a correlation of 0.948 was obtained in the nighttime sleep sub-dimension of the scale and a correlation of 0.956 was obtained in the daytime sleepiness sub-dimension of the scale. The nighttime sleep sub-dimension of the scale showed a positive correlation with PSQI, while the daytime sleepiness sub-dimension showed a positive correlation with ESS. Conclusion: The Turkish form of the SCOPA Sleep Scale is a valid and reliable tool to evaluate the sleep quality of individuals with Parkinson's disease [ABSTRACT FROM AUTHOR]

Mother-baby attachment is a process that starts from the pregnancy period and continues in the birth and postpartum period. Nurses working in the field of women's health and midwives have important roles and responsibilities in supporting prenatal attachment. In this review, it is aimed to present the measurement tools, influencing factors and supporting evidence-based applications that evaluate prenatal attachment in the light of literature. As a result of the literature review; "Maternal-Fetal Attachment Scale", "Prenatal Attachment Inventory", "Maternal Prenatal Attachment Scale" and "Prenatal Attachment Scale", which evaluated mother-infant attachment in pregnant women and whose validity and reliability were made in Turkish, were reached. In these studies, it was revealed that many individual and environmental factors of the pregnant woman affected prenatal attachment. In experimental studies available, it was shown that fetal movement count, leopold maneuver practice, pregnancy training and counseling, father training, birth preparation training, relaxation exercise and guided imagery, cognitive behavioral therapies, yoga and meditation practices support prenatal attachment, while listening to music does not affect it. Nurses are recommended to provide training and counseling to pregnant women with low prenatal attachment risk on these cheap and non-invasive practices, and to support the pregnant woman to use the most appropriate method for her. In addition, it was found that studies testing the effectiveness of evidence-based practices in prenatal attachment are limited in our country and more randomized controlled studies are needed. - Abstract
The present study focuses on the quantitative phase imaging of erythrocytes with the aim to compare the morphological differences between epilepsy patients under antiepileptic treatment, who have no other disease which may affect the erythrocyte morphology, and the healthy control group. The white light diffraction phase microscopy (WDPM) has been used to obtain the interferogram of the erythrocyte surfaces. The continuous wavelet transform with Paul wavelet has been chosen to calculate the surface profiles from this interferogram image. For the determination of alteration in morphology, besides WDPM, erythrocyte surfaces have been investigated by light microscope and scanning electron microscope. In this way, it has been possible to see the difference in terms of precision and implementation between the most commonly used methods with regard to the quantitative phase imaging. Erythrocytes from all the samples have been examined and displayed in both two‐ and three‐dimensional way. We have observed that erythrocytes of patients with effective antiepileptic blood levels were more affected in morphology than healthy subjects. When we compared the erythrocyte morphological changes of patients who received monotherapy or polytherapy, no difference was observed. In conclusion, antiepileptic drugs (AEDs) cause red blood cell (RBC) morphological changes and a combined usage of WDPM with Paul wavelet and light microscopy methods are very convenient for studying the erythrocyte morphologies on multiple patients. Background: It is well known that axonal degeneration plays a role in disability in patients with multiple sclerosis, and synaptopathy has recently become an important issue. Aims: To investigate the possible roles of selected synaptic and presynaptic membrane protein genetic polymorphisms (VAMP2, SNAP-25, synaptotagmin, and syntaxin 1A) in patients with multiple sclerosis. Study Design: Case-control study. Methods: A total of 123 patients with multiple sclerosis and 192 healthy controls were included. The functional polymorphisms of specific SNARE complex proteins (VAMP2, synaptotagmin XI, syntaxin 1A, and SNAP-25) were analyzed by polymerase chain reaction. Results: Significant differences were detected in the genotype and allele distribution of 26-bp Ins/Del polymorphisms of VAMP2 between patients with multiple sclerosis and control subjects; Del/Del genotype and Del allele of VAMP2 were more frequent in patients with multiple sclerosis (p=0.011 and p=0.004, respectively). Similarly, Ddel polymorphism of SNAP-25 gene C/C genotype (p=0.059), syntaxin 1A T/C and C/C genotypes (p=0.005), and synaptotagmin XI gene C allele (p=0.001) were observed more frequently in patients with multiple sclerosis. CC, syntaxin rs1569061 1A gene for 33-bp promoter region TC haplotypes, and synaptotagmin XI gene were found to be associated with an increased risk for multiple sclerosis (p=0.012). Similarly, GC haplotype for rs3746544 of SNAP-25 gene and rs1051312 of SNAP-25 gene were associated with an increased risk for multiple sclerosis (p=0.022). Conclusion: Genetic polymorphisms of SNARE complex proteins, which have critical roles in synaptic structure and communication, may play a role in the development of multiple sclerosis. For the treatment of T2DM; cure can not be achieved with conventional methods. The degree of increase in the epidemic of diabetes; together with the research of new treatments will provide a better understanding of patophysiology necessitates the development. Preferred drugs in cases that do not respond to metformin; hypoglycemia, can cause weight gain and are undesirable cardiac event. Today; incretin-based therapies have been developed that provide effective glucose level and weight control. Incretins while increasing glucose dependent insulin secretion in the pancreas, suppress glucagon secretion. Glucagon-like peptide-1 receptor agonists, and dipeptidyl peptidase-4 inhibitors, are incretin based treatment. Glucagon-like peptide-1 receptor agonists is the most important treatment can be used with monotherapy or combination therapy. Glucagon-like peptide-1 agonists, besides providing good glycemic control, the low rate of hypoglycemia and weight loss are causing significant advantage. Dipeptidyl peptidase-4 inhibitors; leads to rapid inactivation of incretin hormones such as glucagon-like peptide-1 agonists and glucose dependent insulinotropic polypeptide. Dipeptidyl peptidase-4 inhibitors; they extend the duration of action of glucagon-like peptide-1 by inhibiting the degradation of incretin hormones in circulation. [ABSTRACT FROM AUTHOR]

Objective: In this study, we aimed to assess the presence of minor complications in patients with type 2 diabetes mellitus (DM) referred to electroneurography (ENG) laboratory for evaluation of neuropathy. In addition, the relationship between duration of disease, electrophysiologic findings, neurological examination findings, plasma glycated hemoglobin A1C levels, glucose levels and microvascular complications were examined. Materials and Methods: One hundred seventy patients with type 2 diabetes were included. Age, gender, duration of disease, results of ophthalmological examinations, neuropathic complaints, electrophysiological examination results, neurological examination findings and laboratory examination results including blood glucose and A1C levels, presence of albuminuria were recorded retrospectively. Results: Abnormal ENG findings were found in one hundred forty patients (82.4%). In all patients, the rate of retinopathy was 34.1% and the rate of albuminuria was 19.4%. The rate of polyneuropathy (PNP( was 78.2% in patients with abnormal neurologic examination. Neuropathic complaints and PNP were found to be related with the presence of retinopathy. The rate of retinopathy was higher in patients with albuminuria. Significant relationship was found between disease duration and retinopathy, PNP and neuropathic complaints. Conclusions: This study emphasizes that detailed history, neurological and ophthalmological examinations are easy, cost-effective and reliable examinations in follow-up of type 2 diabetics. ENG examination, which needs time and money, should be done in asymptomatic cases or for differential diagnosis of neuropathies with clues not related with diabetic neuropathy. [ABSTRACT FROM AUTHOR]

Aim: To review the clinical, electrophysiological and neuroimaging data of eight adult patients (4F/4M) with epilepsy and bilateral periventricular nodular heterotopia (PNH) after a long duration of follow-up. Materials and Methods: The clinical charts were reviewed for demographic and clinical features, seizure types and frequency, treatment and prognosis of all eight patients who were under follow-up by one of the authors (SS). The recordings of video-EEG monitoring with scalp electrodes in five patients and routine EEGs in all patients were reviewed. Results: The clinical semiology was in accord with seizures originating from temporal lobe region in four patients, while an extratemporal onset was assumed in the others (in one with additional temporal seizures). Interictal EEGs were normal in one patient, who was diagnosed as having psychogenic nonepileptic seizure. Abnormalities seen in interictal EEGs were bilateral independent temporal focus in three, unilateral epileptiform abnormalities in two, and generalized 3 cyc/sec discharges mimicking idiopathic generalized epilepsies in two patients. Only two patients' MRI revealed bilaterally contiguous heterotopic nodules (symmetric and asymmetric type) and, interestingly, these two patients did not have intractable seizures while the other six patients with bilateral, asymmetric and noncontiguous heterotopic nodules suffered from intractable seizures. Conclusions: Patients with bilateral PNH have different clinical features, EEG findings and extension of the heterotopic nodules. These patients may be misdiagnosed as having idiopathic generalized epilepsy, temporal lobe epilepsy or even psychogenic nonepileptic seizures without high quality MRI because of the misleading seizure semiology and interictal-ictal EEG findings. Seizures are usually drug-resistant but the patients who have diffuse symmetric or asymmetric contiguous heterotopic nodules may have good prognosis. [ABSTRACT FROM AUTHOR]

Abstract: Giant aneurysms of the anterior communicating artery (AComA) are rare. The clinical presentation of giant AComA aneurysms is usually associated with the mass effect of the space-occupying lesion or with subarachnoid haemorrhage. A giant AComA aneurysm presenting with a seizure has only been reported twice previously. We report a 70-year-old female patient, in whom a single seizure was the only symptom of a giant AcomA aneurysm, with no neurological deficit. The diagnosis of unruptured giant AComA aneurysm was made with cranial CT, MRI and angiography. The patient refused surgical intervention, was treated with anti-epileptic therapy and has been asymptomatic for 7 months. We suggest that elderly patients presenting with a first seizure need detailed evaluation and giant aneurysms, which may be confused with other intracranial space-occupying lesions, need to be considered in the differential diagnosis. Optic perineuritis is an uncommon variety of orbital inflammatory disease. A case with a 45-day history of headache and vomiting who was diagnosed with fibrillary astrocytoma of the right frontal lobe is reported. Progressive bilateral visual loss developing postoperatively was considered as bilateral optic perineuritis based on the pathological and radiological findings. However, extensive medical and surgical therapy failed to recover visual loss. By this case, the mechanisms and difficulties in the diagnosis were discussed. [ABSTRACT FROM AUTHOR]

Aim: Silent brain ischemia (SBI), defined as ischemic changes and infarcts without neurologic signs, is an established marker of poor survival. Magnetic resonance imaging (MRI) is useful to define SBI and white matter hyperintensities that correspond to microangipathic ischemic disease. This study aimed to investigate the relationship among SBI, white matter lesions and the extent of coronary artery disease (CAD), and to determine possible predictors of SBI. Material and Methods: A total 10640 patients who underwent coronary angiography were retrospectively screened to reveal 312 patients who had been evaluated with a subsequent cranial MRI within 6 months. CAD severity was established with Gensini score and MRIs were evaluated to determine presence of SBI and white matter hyperintensities scored by Fazekas. Finally, 58 patients with SBI and 254 without SBI consisted SBI and non-SBI groups. Results: Patients with SBI were significantly older with higher prevalence of male gender than the non-SBI patients. Both Gensini and Fazekas scores were higher in SBI-group (p<0.001). Fazekas score was positively correlated with Gensini score (r=0.219, p<0.001) and age (r=0.465, p<0.001). In the logistic regression analysis; age, male gender and Gensini score were identified as the independent predictors of SBI. Conclusion: Although SBIs don't present neurological symptoms they are associated with poor survival and future stroke. Our data suggest that cranial MRI may be a screening tool in risk stratification, particularly in elderly male patients with multivessel CAD. Our study also depicted that age, male gender and high Gensini scores are the independent predictors of SBI. [ABSTRACT FROM AUTHOR]

Objective: In neurological clinical practice screening test for ANA (Anti-nuclear antibody) is helpful in the diagnosis of autoimmune diseases which have neurological involvement. ANA investigation tests might be positive both in connective tissue disorders with neurobgical symptoms, such as Systemic Lupus Erythematosus, Sjogren's disease, Rheumatoid Arthritis and also in neurological diseases in the which autoimmune system is responsible such as Multiple Sclerosis (MS) and Myastenia Gravis. In addition drug usage and older age are the factors which might cause positive results in ANA screening tests. In this study we aimed to investigate the neurologjcd diseases which are associated with postive ANA tests and to understand the efficiency of this screening test in neurological practice. Material and Method: The patients who were followed up in neurology outpatient clinic with positive ANA test were investigated. The medical records of 309 patients with positive ANA results were evaluated. The most common neurological diseases with positive ANA and ANA patterns were determined. Results: The patients who were thought to be demyelinating disease had the highest number of positive ANA results. However, positive ANA results were also found with different and non specific neurological symptoms. Conclusion: As a result of the diagnosis of neurological diseases, positive ANA results might be beneficial in patients with supportive medical history and laboratory findings. [ABSTRACT FROM AUTHOR]

Isolated hypoglossal nerve palsy is an uncommon condition and it may be due to damage to the hypoglossal nucleus in the brain stem or to damage to the nerve along its course. Isolated hypoglossal nerve palsies may indicate the presence of an intracranial or extracranial space—occupying lesion, head and neck injury, carotid artery dissection, vascular abnormality, idiopathic, infection, autoimmune disease, or neuropathy in most cases. In this article, we present two cases of isolated hypoglossal nerve palsy and discuss other possible causes. [ABSTRACT FROM AUTHOR]
