This study examines the clinical utility of an electronic diary (ED) in the monitoring of the clinical manifestations of fibromyalgia (FM), compared to traditional paper-pencil self-reports. Fourteen women with FM completed an interview, several questionnaires, and an ED for a week (several times a day) recording pain, fatigue, sleep, difficulty in thinking, emotional distress, difficulty in daily functioning, coping with the disease, and stress. There were no differences in symptoms throughout the different moments of the day. A sleep latency of 45.36 minutes and sleep duration of 6.25 hours was observed. Significant correlations were found between ED measures depending on the time of day, and between ED measures and questionnaires. The ED showed to be useful for the evaluation of FM symptomatology and could be a key component in psychological intervention programs. [ABSTRACT FROM AUTHOR]

Background: Fibromyalgia syndrome (FMS) is a chronic, generalized and diffuse pain disorder accompanied by other symptoms such as emotional and cognitive deficits. The FMS patients show a high prevalence of gastrointestinal symptoms. Recently it has been found that microbes in the gut may regulate brain processes through the gut-microbiota-brain axis, modulating thus affection, motivation and higher cognitive functions. Therefore, the use of probiotics might be a new treatment that could improve the physical, psychological and cognitive state in FMS; however, no evidence about this issue is available.Methods: This paper describes the design and protocol of a double-blind, placebo-controlled and randomized pilot study. We use validated questionnaires, cognitive task through E-Prime and biological measures like urine cortisol and stool fecal samples. The trial aim is to explore the effects of eight weeks of probiotics therapy in physical (pain, impact of the FMS and quality of life), emotional (depression, and anxiety) and cognitive symptoms (attention, memory, and impulsivity) in FMS patients as compared to placebo.Conclusion: This pilot study is the first, to our knowledge, to evaluate the effects of probiotics in FMS. The primary hypothesis was that FMS patients will show a better performance on cognitive tasks, and an improvement in emotional and physical symptoms. Fibromyalgia is a complex syndrome that includes biological, psychological and social factors with a great impact on the quality of life. The best way to address this complexity is through a multidimensional approach in which psychological aspects have a great relevance. In this regard, several cognitive-behavioral interventions have proven its effectiveness, although the results are still modest. The use of information and communication technologies (ICTs) can help to improve the effectiveness of some components of the treatment. The aim of this paper is to present the development and preliminary results of a virtual reality system designed to induce positive emotions and promote motivation and behavioral activation in patients with fibromyalgia. Introduction. The aim of this paper is to evaluate the efficacy of a model based on the Lazarus & Folkman's transactional theory of stress to explain perceived stress and symptoms in a sample of people with fibromyalgia. Methods. The design was an open, cross-sectional, uncontrolled study. Subjects were contacted through internet. A self-administered questionnaire was used that contained each of the variables included in the model. To perform the statistical analysis we used structural equation models. Results. We evaluated 165 people who reported to have fibromyalgia diagnosis, 93.9% were female. According to PHQ-15 categories, used to measure symptoms, 83% of the participants were in the highest level of somatic symptom severity. FIQ was used to evaluate fibromyalgia impact, its maximum score is 100 points; taking into account mean and standard deviation, the samples are severely affected by fibromyalgia. We estimated five structural models; fifth model confirmed that some personal and social resources (self-esteem, self-efficacy, social support) perform as predictors of perceived stress level explaining 53% of its variance. This perceived stress was predictor of a latent variable indicated by symptoms and FIQ, and it explained 31% of its variance. Goodness of fit index was adequate. Conclusions. The study suggests the important role that perceived stress has in the fibromyalgia impact and the symptoms severity. The aim of this paper was to measure the psychometric properties of the Life Orientation Test (LOT-R) in a sample of people with fibromyalgia. To gather an adequate sample, we contacted people with a fibromyalgia diagnosis via the Internet (n=154). We measured reliability using Cronbach's alfa, explanatory and confirmatory factor analyses and correlation among optimism, stress and negative affect. Results showed adequate psychometric properties of the LOT-R; a Cronbach's alfa of .81, evidence of a second order-factor model with an adequate goodness-of-fit, and negative and significant correlations with stress and negative affect. Background. The aim of the present study was to perform a systematic review and metaanalysis comparing walking test performance and gait pattern between individuals with and without fibromyalgia (FM). Methodology. This systematic review was registered in PROSPERO with the following reference: CRD42018116200. The search for the scientific articles in this systematic review was carried out using the MEDLINE, SCOPUS, PEDRO, CINHAL and WEB OF SCIENCE databases. A combination of three conceptual groups of terms was used: (1) fibromyalgia; (2) walk (performance) tests; and (3) gait analysis. The included articles were analyzed for both functional and pattern of walking data of patients with FM. In order to provide a better estimate of the difference between individuals with and without FM on gait, a meta-analysis was performed on the 6MWT (6-minute walk test). Results. Thirty-six studies were analyzed, with a total population of 4.078 participants (3.369 FM and 709 individuals without FM). From a functional point of view, the 6MWT distance covered by the group of individuals without FM was significantly greater than that of the individuals with FM in all the analyzed studies. In addition, when comparing the results obtained in the gait pattern analysis, it was observed that individuals with FM walked slower, with a shorter stride length and lower cadence compare to individuals without FM. Conclusions. It is possible to affirm that individuals with FM perform walking tests differently than individuals without FM. It was observed that individuals with FM walk performing a cycle of shorter length and lower frequency, producing a slower gait, which results in a shorter distance traveled, in the same period of time, with respect to healthy subjects. Fibromyalgia can be clinically defined by widespread pain lasting for longer than 3 months with tenderness on palpation in 11 or more of 18 specified tender points. Many people with fibromyalgia are significantly disabled, and experience moderate to severe pain for many years, for which conventional analgesics are usually not effective. For these patients treatment options generally include antidepressants like tricyclic agents, serotonin and noradrenaline reuptake inhibitors, or anticonvulsants like pregabalin or gabapentin. Pregabalin is a drug licensed for the treatment of fibromyalgia in the United States of America, with a mechanism of action similar to gabapentin. This mode of action confers antiepileptic, analgesic, and anxiolytic effects. This Cochrane systematic review included 8 randomized, placebo-controlled trials with low risk of bias, which studied the effect of a daily dose of pregabalin for the treatment of moderate to severe pain in adult patients suffering from fibromyalgia. Of the main results of this systematic review we highlight the major effect that a daily dose of 300 to 600 mg of pregabalin had in the reduction of pain intensity over a follow-up period of 12 to 26 weeks, with tolerable adverse effects, for a minority of people with moderate to severe pain due to fibromyalgia. This paper aims to summarize and discuss the main results and conclusions of this systematic review, as well as its implications for the daily clinical practice.

New and renewed medical health issues have appeared in the last years. Their main characteristic is the absence of an organic cause, in bio-psycho-medical terms. This paper tackles some of this issues, focusing specially in patients with chronic pain and a diagnose of fibromyalgia. Its main objective is to analyze the way in which people with this diagnose and that participate in patients community organizations, transform their corporal discomfort in psychosocial discomfort. This has been done using qualitative research methods, specifically focus groups and content analysis. The main results of this research show that people that are able to do this show improvement in their quality of life. At the same time, it shows the necessity of adding gender, social inequities and power relationships as important elements in public health.

OBJETIVO: avaliar anormalidades bioquímicas que possam estar relacionadas à fibromialgia, mais especificamente o metabolismo oxidante de granulócitos de pacientes com fibromialgia primária (FMP). MÉTODOS: foram selecionados 10 pacientes (9 mulheres e 1 homem) portadores de FMP, com idade entre 20 e 40 anos, de acordo com os critérios do Colégio Americano de Reumatologia, que não apresentavam qualquer outra doença de curso agudo ou crônico e estavam sem utilizar medicamentos no mínimo durante duas semanas. Controles normais (n = 19) pareados por sexo e idade foram selecionados. Os granulócitos de pacientes e controles foram separados do sangue periférico e utilizados como célula de trabalho. A produção de espécies reativas de oxigênio (ERO) foi determinada por meio de ensaio de quimioluminescência dependente de luminol. RESULTADOS: demonstramos, pela primeira vez, uma produção significantemente elevada de ERO (3,2 vezes maior) em granulócitos de sangue periférico de pacientes com FMP (p < 0,05). CONCLUSÕES: estes resultados demonstraram um comportamento anormal dos granulócitos de pacientes com FMP, em que uma produção aumentada de espécies reativas do oxigênio foi observada. Estudos futuros serão necessários para melhor esclarecer estes resultados, delineando uma nova perspectiva na compreensão e abordagem destes pacientes.OBJECTIVE: Our aim in this paper was to evaluate the biochemical abnormalities that may be correlated with fibromyalgia, such as the ROS production by granulocytes from patients with fibromyalgia in comparison to cells from healthy subjects. METHODS: Ten patients (9 women and 1 man, age ranging from 20 to 40 years old) with primary fibromyalgia, according to the American College of Rheumatology criteria, were selected. None of them were using medications 2 weeks before starting the study. Patients with rheumatic and non-rheumatic associated disease were excluded. Sex and age-matched healthy controls (n = 19) were simultaneously selected. The granulocytes from patients and controls were collected from peripheral blood. The quantification of reactive oxygen species was performed in a luminol-dependent chemiluminescence assay. RESULTS: We have demonstrated, for the first time, the increase of ROS production (3.2 times) in peripheral blood granulocytes from patients with fibromyalgia in comparison to the same cells from healthy subjects. CONCLUSIONS: The present result may indicate a possible role of free radicals in fibromyalgia subjects, once they have already been shown in other human diseases, including painful states. We suggest that further studies are needed to understand the importance of these findings, delineating a new perspective in understanding and approach of fibromyalgia patients.

This article discusses the history of and relationship between health, well-being, and human emotions. The authors comment on the role of emotions in Spanish and Latin American societies and cultural histories. They also consider the clinical, psychiatric, and psychological research conducted regarding diseases affecting well-being, with a particular focus on the study and treatment of fibromyalgia. The interaction between the humanities and medical sciences in the early 21st century is also examined.

La fibromialgia es considerada actualmente una enfermedad multisistémica con una posible patogenia disautonómica. El objetivo de este trabajo fue determinar, en las pacientes del área de salud, la relación entre la fibromialgia (FM) y las alteraciones del sistema nervioso autónomo. Se realizó un estudio prospectivo de sección transversal de casos y controles, en el período de 1 año, se estimaron porcentajes, análisis univariante para medidas de centralización y de dispersión, riesgo relativo (RR) e IC 95 %. Para estudiar la asociación entre las variables fibromialgia y disautonomía se hicieron pruebas de significación estadística Chi cuadrado X2 exacta de Fisher y la t de Student, todas para grupos independientes. Las pacientes con FM tuvieron una media de edad de 46,1 años y ± 1 DE 10,6 años, con predominio entre los 30 y los 50 años. Se presentó hipotensión ortostática en 31 de ellas (62 %), este hallazgo fue altamente significativo al compararlas con el grupo control, en el cual solamente fue apreciada en 11 de los 50 casos (22 %). Se concluyó que la hipotensión ortostática podría ser un factor de riesgo para padecer FM lo cual explicaría además el posible papel patogénico de la disautonomía planteado en esta enfermedad.Fibromyalgia is considered at present a multisystemic disease with a possible dysautonomic pathogeny. The objective of this paper was to determine in the patients from the health area the relation between fibromyalgia (FM) and the alterations of the autonomous nervous system. A one-year prospective cross-sectional case-control study was conducted, percentages were estimated, and an univariant analysis for centralization and dispersion measures, relative risk (RR) and CI 95 %, was made. Chi square tests of statistical significance, exact test (Fisher) and Student's t test were made to study the association between fibromyalgia and dysautonomia in different groups. The mean age of the patients with FM was 46.1 years old and ± 1 aged 10.6 , with predominance between 30 and 50 years old. 31 of them (62 %) presented orthostatic hypotension. This finding was highly significant on comparing them with the control group, where it was only observed in 11of the 50 cases (22 %). It was concluded that orthostatic hypotension could be a risk factor for suffering from FM, which would explain the possible pathogenic role of dysautonomia stated in this disease.
