Abstract

BACKGROUND: Supratentorial midline epidural hematoma is rare but challenging in diagnosis and management. Indication for surgery can arise even following hospital admission. Being familiar to the presentation and watching out for direct and indirect signs on axial computed tomography (CT) such as suture diastasis or fracture traversing midline are essential to plan multi-planar CT enabling exact diagnosis including form and mass effect of hematoma. METHODS: Nine patients with midline epidural hematoma including two pediatric patients underwent surgery between 2013 and 2018. Pre-operative and post-operative patient status, radiological features, and surgical technique were analyzed. RESULTS: Four patients had deteriorating consciousness levels and two patients had paraparesis. All had fractures traversing midline and epidural hematomas with significant mass effect. They were operated through separated craniotomies around the midline and midline bone strip was used for dural tenting and as support for natural closure of bone flaps. No post-operative complications were developed. All patients were discharged with Glasgow Outcome Score of 5. CONCLUSION: Because of the rarity of the lesion and small number of patients, definitive conclusions may be misleading but we think that, in experienced hands, midline epidural hematomas can safely be operated on and, preservation of midline bone strip pro-vides easier bleeding control.

1970’li yillar ekonomide onemli degisimlere sahne olmustur. Kar oranlarinda ve verimlilikte dusus olarak kendini gosteren ekonomik kriz Fordist uretim duzeninin sorgulanmasina yol acmistir. Fordist katiliklarin ortadan kaldirilarak esneklestirme yonunde donusumler krizden cikmanin recetesi olarak gorulmustur. Fordist katiliklarin nedeni olarak gorulen sendikalarin gucu kirilmis, ucret ve calisma kosullari esneklesmeye baslamis, Fordist uretim paradigmasi yerini Post-Fordist uretim paradigmasina birakmistir. Ekonomik alt yapida meydana gelen post-Fordist donusum siyasi ust yapilari da etkileyerek kamu yonetimini donusturmustur. Fordist kamu yonetiminin hiyararsik yapisi ve kadrolu istihdam rejimi buyuk degisimler gecirmistir. Taseron, gecici ve sozlesmeli personel gibi istihdam bicimleri kamu istihdam rejiminde guclenmeye baslamis, zaman icinde kadrolu ve guvenceli istihdamin ortadan kalkacagi bir surece girilmistir. Kamu kurumlarinin calisma duzeni Post-Fordizm’den etkilenerek yatay ve esnek is iliskileri guclenmeye baslarken yoneticilik ve liderlik gibi kavramlar kamu sektorunun icerisine girmistir. Post-Fordist donemde, yerellesme ve ozellestirme egilimleri yeni uretim duzeninin gereksinimleri dogrultusunda guclenmekte, buna paralel olarak kamu hizmetleri de ozellestirme ve yerellesme yonunde ilerlemektedir. Devletin uretici rolunun duzenleyici olarak gelistigi Post-Fordist donusum, devleti yonetisim mekanizmasina uygun bir sekilde tasarlamakta, uretimin talep yonunden uzaklastirarak sosyal devleti bitirmekte, arz yonlu sorunlarla ilgilenen, rekabeti ve yenilikci kapasiteyi gelistiren bir devlet modeli ortaya cikarmaktadir. Bu yeni devlet modelinin ulus-devletten uzaklastigi, ulusal kalkinmanin sona erdigi seklinde yorumlar yapilmaktadir. Bu tezde Post-Fordist donemin genel olarak kamu yonetimi ve ozellikle de kamu personel rejiminde ortaya cikardigi donusum ve sonuclar analiz edilmistir.

Abstract

Aim: to discuss seizure outcomes of patients with invasive electroencephalography (EEG) monitorization (IEM) following their epilepsy surgery at our centre. Material and Methods: forty-seven patients suffering from refractory epilepsy and who were evaluated by invasive EEG were included in this retrospective study at Istanbul Faculty of Medicine from 2003 to 2017. We examined the Video EEG and invasive EEG monitorization, cranial MRI, SPECT, PET and neuropsychological tests of all patients. Postoperative seizure outcome results were evaluated according to Engel classification. The factors affecting seizure outcomes were discussed. Results: twenty-six of the patients were female (55.3%), 21 were male (44.7). The average age was 32.0 (+/- 12.4). Forty-three patients had surgery and the average age of these patients was 26,6 (+/- 11.15). 38.3% of the patients had hippocampal sclerosis (HS), 23.4% had focal cortical dysplasia (FCD), 8.5% had a tumor, 14.9% had sequela lesion and 14.9% had unknown etiology. Postoperative seizure status according to the Engel classification showed that 81.6% of the patients were class I, 10.5% were class II, 2.6% were class III and 5.3% were class IV. Conclusion: a significant relation was statistically determined between structural MRI lesion and favorable seizure outcome (p<0.05). The most frequent etiology was HS in our patients. Of the patients with Engel I, the averages of their ages, ages at onset of epilepsy and ages at surgery were lower than other groups, but the difference was not statistically significant (p>0.05). Abstract

Amaç: Bu çalışmada seri halde supin ve pron pozisyonlarda T1 ve T2 ağırlıklı aksiyal ve sagital manyetik rezonans görüntüleme yapılan 40 sağlıklı insanda meduller konusun supin-pron pozisyon değişikliği ile uyarılan hareket aralığı değerlendirildi. Yöntem: Her bir denekte konus medullarisin ucu seviyesinde elektronik ölçekler kullanılarak konus medullaris ve ön dural yüzey arasındaki mesafeyi ve dural kesenin genişliğini ölçtük. Elde edilen veriler SPSS 15 kullanılarak değerlendirildi. Bulgular: Konus ucunun, denekler supin pozisyondan pron pozisyona getirildiğinde ortalama değer olarak spinal kanal genişliğinin %30±14’ü oranında yer değiştirdiği saptandı (p:0,0001). Yaş (0-18 ve >19 yaş) ve cinsiyet farklılığı konus ucunun hareketliliğinde istatistiksel olarak anlamlı faktörler değildi. Sonuç: Çalışmamız klinik olarak gergin omurilik şüphesi bulunan normal uzanımlı konus vakalarında güvenilir bir diyagnostik referans aralığı sunmaktadır. Objective: In this study, using a series of supine and prone positions T1 and T2 with weighted axial and sagittal magnetic resonance imaging, the movement interval of the conus medullaris stimulated by the supine-prone position change was assessed in 40 healthy subjects. Method: We measured the distance between the conus medularis and the front dural surface and the width of the dural sac using electronic scales at the tip level of the conus medullaris in each subject. The data obtained was evaluated using SPSS 15. Results: It was determined that the spinal canal width was decreased by 30% (p:0.0001) as the average value when the conical tip was brought to the prone position from supine. Age (0-18 and > 19 years) and gender diversity were not statistically significant factors in the movement of the conus medullaris. Conclusion: Our study offers a reliable diagnostic reference range in cases of normal extension conus with clinically tense spinal cord suspicion. WOS:000463368800003

29. Special designed routing device to ease endoscopic transforaminal lumbar disc surgery: A cadaveric study

Abstract

Background Data: Fully endoscopic lumbar disc (FELD) surgery via transforaminal (TF) approach may be more demanding to adopt for surgeons experienced with conventional microdiscectomy, due to the necessity of a new anatomic orientation and understanding. We designed a routing device to facilitate access to safe anatomic triangle defined by Kambin at the level of foramen in cadavers. Purpose: To show that the transforaminal route for endoscopic lumbar disc herniations is safely applicable with the aid of a new routing device. Materials and Methods: Ten cadavers between the ages 18-75, with no history of lumbar spinal surgery or trauma, with previous abdominal computed tomography (CT) scans included in our study. Postmortem abdominal CT scans were performed. Images were examined and transforaminal entrance angles without causing damage to retroperitoneal structures for each lumbar disc space and anatomical differences were recorded. TF approach was performed in cadavers using the angles measured from abdominal CTs and the entry point determined with the help of routing device. Results: L1-L2, L2-L3, L3-L4, L4-L5 disc spaces were operated in ten cadavers. Kambin’s triangle was successfully reached with help of routing device using data obtained from CT (X’, α) and C-arm fluoroscopy (X, Y, Y’). Y’ marker with protractor on routing device, and the metal rod on this Y’ marker with an opening through which only the punction needle could pass were very important in reaching the target. The metal bar horizontal movement and fixation to this mechanism contributed to operation of device. Entrance points and angles calculated with the help of CT scans were found to be compatible with the images obtained from fluoroscopy and endoscopy during operation. Conclusions: In this study, it has been showed that TF approach can be safely performed with help of the new designed routing device.

Abstract

High-frequency oscillations in local field potentials recorded with intracranial EEG are putative biomarkers of seizure onset zones in epileptic brain. However, localized 80-500 Hz oscillations can also be recorded from normal and non-epileptic cerebral structures. When defined only by rate or frequency, physiological high-frequency oscillations are indistinguishable from pathological ones, which limit their application in epilepsy presurgical planning. We hypothesized that pathological high-frequency oscillations occur in a repetitive fashion with a similar waveform morphology that specifically indicates seizure onset zones. We investigated the waveform patterns of automatically detected high-frequency oscillations in 13 epilepsy patients and five control subjects, with an average of 73 subdural and intracerebral electrodes recorded per patient. The repetitive oscillatory waveforms were identified by using a pipeline of unsupervised machine learning techniques and were then correlated with independently clinician-defined seizure onset zones. Consistently in all patients, the stereotypical high-frequency oscillations with the highest degree of waveform similarity were localized within the seizure onset zones only, whereas the channels generating high-frequency oscillations embedded in random waveforms were found in the functional regions independent from the epileptogenic locations. The repetitive waveform pattern was more evident in fast ripples compared to ripples, suggesting a potential association between waveform repetition and the underlying pathological network. Our findings provided a new tool for the interpretation of pathological high-frequency oscillations that can be efficiently applied to distinguish seizure onset zones from functionally important sites, which is a critical step towards the translation of these signature events into valid clinical biomarkers.awx374media15721572971001. 38. Subarachnoid-pleural fistula in a child: The cause and treatment

Abstract

Hydrothorax of the cerebrospinal fluid after a subarachnoid-pleural fistula is a rare condition. Subarachnoid-pleural fistula may appear after a trauma at the thoracolumbar vertebral column or iatrogenically after thoracotomy. A two years and four months old boy who was operated because of ganglioneuroblastoma was admitted to hospital due to respiratory distress. The chest roentgenogram obtained two months after thoracotomy, showed a pleural effusion at the left side and a chest tube was inserted. The craniospinal magnetic resonance imagining revealed subarachnoid-pleural fistula and lumbar external cerebrospinal fluid drainage was performed. The chest tube was removed by application of tetracycline between pleural layers. After the patient was discharged, respiratory distress reoccured after 3 weeks and a chest tube was reinserted due to fluid at the left hemithorax. An external lumbar drainage was reapplied. The location of the fistula was determined by craniospinal magnetic resonance imagining and myelography. The fistula was surgically restored by a posterior approach and laminectomy. The cerebrospinal fluid drainage and chest tube was removed three days and seven days after the operation respectively. The patient was discharged at the 13 days after the operation. During periodical outpatient follow up the patient has no symptoms and is neurologically intact. Subarachnoid-pleural fistulas, usually do not regress spontaneously or respond to conservative methods. Invasive approaches including surgery may be needed to treat patients with subarachnoid-pleural fistulae.

Abstract

Background: In the present study, we hypothesized that a low gray matter-white matter ratio (GWR) is associated with poor cognitive function and low quality of life in patients with mild to moderate (WFNS ≤3) aneurysmal subarachnoid hemorrhage (aSAH). Methods: All patients with aSAH who were admitted to the neurointensive care unit (Neuro ICU) and whose WFNS score was ≤ 3 were enrolled in the study. During the Neuro ICU follow-up period, the following variables were recorded: demographics, neurological status, comorbidities, time elapsed between onset of bleeding and Neuro ICU admission, treatment method, number of days with vasospasm symptoms (DVS) and vasopressor usage. One year after bleeding, all patients except those who could not answer the questionnaires appropriately were administered the MoCA and SF-36 tests, and brain magnetic resonance imaging and then volumetric brain analysis were performed. Results: Eighty-two patients completed the study. One year after aSAH, cognitive dysfunction and low quality of life were observed in 59.8% and 25.6% of patients, respectively. Among the variables obtained during Neuro ICU follow-up, DVS was found to be a major risk factor for cognitive dysfunction (OR: 3.9, 95% CI: 1.9-7.8; p < 0.001), poor quality of life (OR: 2.8, 95% CI: 1.4-5.3, p = 0.002) and a lower GWR value ( p < 0.001, correlation coefficient = −0.410, R2 = 0.234). One year after aSAH, higher GWR values were associated with higher MoCA (R2 = 0.506 for males, R2 = 0.413 for females) and SF-36 (R2 = 0.270 for males, R2 = 0.364 for females) scores in both genders. Also, GWR ≤ 1.35 in males and GWR ≤1.33 in females indicated MoCApoor patients with over 80% specificity and sensitivity. Conclusion: GWR has good correlation with the MoCA and SF-36 score, and a low GWR can indicate cognitive dysfunction.
