1. Impact of avalanche type of transport on internal transport barrier formation in tokamak plasmas

Abstract: Abstract In magnetic fusion plasmas, a transport barrier is essential to improve the plasma confinement. The key physics behind the formation of a transport barrier is the suppression of the micro-scale turbulent transport. On the other hand, long-range transport events, such as avalanches, has been recognized to play significant roles for global profile formations. In this study, we observed the impact of the avalanche-type of transport on the formation of a transport barrier for the first time. The avalanches are found to inhibit the formation of the internal transport barrier (ITB) observed in JT-60U tokamak. We found that (1) ITBs do not form in the presence of avalanches but form under the disappearance of avalanches, (2) the surface integral of avalanche-driven heat fluxe is comparable to the time rate change of stored energy retained at the ITB onset, (3) the mean E × B flow shear is accelerated via the ion temperature gradient that is not sustained under the existence of avalanches, and (4) after the ITB formation, avalanches are damped inside the ITB, while they remain outside the ITB.

2. Observation of avalanche-like transport in Heliotron J and JT-60U plasmas

Abstract: The avalanche type of transport can induce a long-radial transport and thus can contribute to the global profile formation. In this study, we observed the heat perturbations exhibiting avalanche-like transport in the stellarator/heliotron device, Heliotron J, and the tokamak device, JT-60U. We found that the electron heat propagation in Heliotron J is mainly generated from the heating source region. The relatively high value of the Hurst exponent, which is a signature of avalanches, depends on the total heating power. On the other hand, the electron and ion heat avalanches measured in JT-60U tend to spread from the local peak of the temperature gradient and are not influenced by the heating source profiles. The contrasting features of avalanches in stellarator/heliotrons and tokamaks potentially imply the difference in the temperature profile formation, such as the presence of stiffness.

3. Experimental evaluation of avalanche type of electron heat transport in magnetic confinement plasmas

Abstract: Turbulent transport is undoubtedly important in magnetic confinement plasmas. It has been reported that a lot of transport dynamics are not satisfying the local diffusive models. Here, we report the quantitative measurement of electron heat flux associated with ballistic propagating long-range transport events, which is considered to be a component of avalanches. In addition, we show the first observations of the substantial impact of avalanche-driven transport on profile resilience (or profile stiffness) observed in JT-60U. We found that, in the channel of the electron heat flux, the ratio between the increment of the avalanche-driven component to that of the total plasma heating becomes dominant (∼80%) in the case of the high-heating limit. This suggests a possible role for avalanche-driven transport to induce profile resilience, which has been evidenced by flux-driven simulations.

Abstract: Alarge superconducting machine, JT-60SA has been constructed to provide major contributions to the ITER program and DEMO design. For the success of the ITER project and fusion reactor, understanding and development of plasma controllability in ITER and DEMO relevant higher beta regimes are essential. JT-60SA has focused the program on the plasma controllability for scenario development and risk mitigation in ITER as well as on investigating DEMO relevant regimes. This paper summarizes the high research priorities and strategy for the JT-60SA project. Recent works on simulation studies to prepare the plasma physics and control experiments are presented, such as plasma breakdown and equilibrium controls, hybrid and steady-state scenario development, and risk mitigation techniques. Contributions of JT-60SA to ITER and DEMOhave been clarified through those studies. 9. 3D metal powder additive manufacturing phased array antenna for multichannel Doppler reflectometer

Abstract: Measuring the time variation of the wavenumber spectrum of turbulence is important for understanding the characteristics of high-temperature plasmas, and the application of a Doppler reflectometer with simultaneous multi-frequency sources is expected. To implement this diagnostic in future fusion devices, the use of a phased array antenna (PAA) that can scan microwave beams without moving antennas is recommended. Since the frequency-scanning waveguide leaky-wave antenna-type PAA has a complex structure, we have investigated its characteristics by modeling it with 3D metal powder additive manufacturing (AM). First, a single waveguide is fabricated to understand the characteristics of 3D AM techniques, and it is clear that there are differences in performance depending on the direction of manufacture and surface treatment. Then, a PAA is made, and it is confirmed that the beam can be emitted in any direction by frequency scanning. The plasma flow velocity can be measured by applying the 3D manufacturing PAA to plasma measurement.

10. Laser transfer technique using wavefront correction and beam homogenizers in Thomson scattering diagnostics

Abstract: This paper suggests the use of two optical elements for laser transfer in Thomson scattering diagnostics, which are widely used for other laser applications. YAG lasers amplified by flash lamps (repetition frequency of 50–100 Hz having a few Joules output) can sometimes have adverse effects such as peaked beam profiles and wavefront distortion. We used a beam homogenizer, which is a refractive optical element, to change the beam profile from peaked to non-peaked at a designated position. Using a super-Gaussian output profile when amplifying lasers facilitated longer—by a factor of 1.5 (=300 mm/200 mm) compared with top-hat beam profile—sustained distances, i.e. the distance along the laser path having a non-peaked profile. In terms of beam quality, wavefront distortion broke the beam profile in the far field. We used a deformable mirror to correct and flatten the wavefront of the beam. A YAG laser having a wavefront distortion of 0.46λ peak-to-valley was used to test the application of the deformable mirror, the results of which improved wavefront distortion to a wavelength of 0.1λ peak-to-valley.

11. Real-time measurement and feedback control of ion temperature profile and toroidal rotation using fast CXRS system in JT-60U

Abstract: A fast charge exchange recombination spectroscopy (CXRS) system has been developed for the real-time measurement and feedback control of ion temperature ( T i ) profile and toroidal rotation velocity ( V t ) in JT-60U. In order to control T i and V t in real-time, the charge exchange recombination spectroscopy with high time resolution, the real-time processor system, and the real-time control system have been developed. Utilizing this system, real-time control of the T i gradient between r / a ∼ 0.25–0.5 has been demonstrated with neutral beams at high beta plasmas (normalized beta β N ∼ 1.6–2.8). The strength of the internal transport barrier is controlled. Moreover, the real-time control of V t has been demonstrated from counter (anti-parallel to the plasma current, I p ) to co (parallel to the I p ) direction. Then the behavior of edge localized mode (ELM) is changed by controlling the V t .

Abstract: 沖縄産ヒメサザンカ野生種13系統，芳香性ツバキの花粉親の一つであるヒメサザンカの系統1118（海外経由系統），芳香性ツバキ4品種の香気成分の比較を行った．ヒメサザンカの香気成分について，新たにリモネンおよび6種の芳香族化合物，安息香酸ベンジル，オイゲノール，サリチル酸メチル，o-アニス酸メチル，フェニルアセトアルデヒド，ベンズアルデヒドを同定した．沖縄産野生種13系統の香気成分量は，ほとんどのものが海外経由系統より多く，特に系統3と36が多かった．この二つを比較すると，花様の香調の2-フェニルエタノールやフェニルアセトアルデヒドの割合が多い系統36の香りの方が強く感じられた．ヒメサザンカを花粉親とする芳香性ツバキ‘姫の香’，‘港の曙’，‘春風’，‘フレグラントピンク’の香気成分の組成もヒメサザンカとほぼ同じであったが，組成比は品種ごとに大きく異なり，花様の香調を持つ成分の割合の多い‘姫の香’，‘港の曙’で香りが強く感じられた．

Abstract: Several small/no ELM regimes such as EDA, grassy ELM, HRS, QH-mode, type II and V ELMs with good confinement properties have been obtained in Alcator C-Mod, ASDEX-Upgrade, DIII-D, JET, JFT-2M, JT-60U and NSTX. All these regimes show considerable reduction of instantaneous ELM heat load onto divertor target plates in contrast to conventional type I ELM, and ELM energy losses are evaluated as less than 5% of the pedestal stored energy. These small/no ELM regimes are summarized and widely categorized by their pedestal conditions in terms of the operational space in non-dimensional pedestal parameters and requirement of plasma shape/configuration. The characteristics of edge fluctuations and activities of ideal MHD stability leading to small/no ELMs are also summarized.

Abstract: Comparisons of H-mode regimes were carried out on the Alcator C-Mod and JFT-2M tokamaks. Shapes were matched apart from aspect ratio, which is lower on C-Mod. The high recycling steady H-mode on JFT-2M and enhanced D-alpha (EDA) regime on C-Mod, both of which feature very small or no ELMs, are found to have similar access conditions in q 95 - v* space, occurring for pedestal collisionality v* ? 1. Differences in edge fluctuations were found, with lower frequencies but higher mode numbers on C-Mod. In both tokamaks an attractive regime with small ELMs on top of an enhanced D α baseline was obtained at moderate v* and higher pressure. The JFT-2M shape favoured the appearance of ELMs on C-Mod and also resulted in the appearance of a lower frequency component of the quasicoherent mode during EDA.

Abstract: Three options of fusion DEMO plant are proposed characterized by functions of the center solenoid (CS). The prime option uses a downsized CS, which does not provide sufficient V-s for plasma current ramp-up but supplies enough coil current for plasma shaping. This option produces a fusion output of 3 GW with a major radius of 5.5 m, aspect ratio of 2.6, normalized beta of 4.3 and maximum field of 16.4 T. The estimated reactor weight is lighter than that of other conventional tokamak reactors, suggesting an economic advantage. The plant uses rather conservative technologies such as Nb3Al superconductor, water-cooled solid breeder blanket, low activation ferritic steel as the structural material and tungsten monoblock divertor plate. The design philosophy and key issues related to the constituent technologies of the plant are described in the present paper.

Abstract: In GAMMA 10, the radiation pattern of a heating wave of fundamental ECRH for potential formation was not axi-symmetric but vertically elongated on the resonance surface. The non-axisymmetric radiation pattern caused a non-axisymmetric potential profile and likely induced a radial loss. Then, the radiation pattern has been axi-symmetrized. Accordingly, an almost axi-symmetric potential profile has been obtained and a large density increase during ECRH has been observed in the central cell. This implies that radial losses associated with ECRH have been substantially reduced. A new antenna has recently been installed for additional plug electron heating (second ECRH). With this antenna, a larger power and/or a longer pulse width have become available. In a most recent experiment, we have succeeded in axial plugging with a duration up to 0.15 s by sequential use of the usual ECRH and the second ECRH. Moreover, there is an indication that the second ECRH is more desirable for further reduction of the radial loss.

Abstract: Required basic features as to 30 Gbpsi recording density for write head are good overwrite O/W, good nonlinear transition shift NLTS, and high frequency performance in half-micron track width. To achieve these, we have developed a new structure, planar yoke write head, combined with SV read head, which features narrow track (physical write track width=0.35 /spl mu/m), narrow coil pitch (1.3 /spl mu/m coil pitch) for short yoke length (16 /spl mu/m yoke length), one-layer coil design, and used flat yoke pole while succeeding in the advantages of our current stitched pole write head. Regarding the write performance, we compared planar yoke head with current stitched pole head by simulation and actual measurement.

Abstract: A millimetre wave two-dimensional detector array has been newly developed for application to a phase-imaging interferometer installed in the GAMMA 10 tandem mirror. The interferometer consists of two sources and a quasi-optical transmission system. The transmitted wave is mixed with the local oscillator (LO) wave to provide a 150 MHz intermediate frequency (IF) signal. The detector has 4 × 4 channels consisting of beam-lead GaAs Schottky barrier diodes bonded to bow-tie antennas fabricated using photolithographic techniques on a fused quartz substrate of size 38.1 mm × 38.1 mm. The interferometer successfully measures the time evolution of line density profiles with a phase resolution of less than 1 200 fringe and a spatial resolution of 16 mm in the vertical direction and 5 mm in the horizontal direction. The system is also applied to the measurement of electron cyclotron emission (ECE) as a heterodyne radiometer. The axial and radial dependence of the second-harmonic ECE is observed simultaneously. The fluctuation spectra of the ECE signals are compared with those obtained from the interferometer.

Abstract: A broad band frequency modulation reflectometer system has been installed and operated on the GAMMA 10 tandem mirror. A fast sweep hyperabrupt varactor-tuned oscillator (HTO) is used as a source, which can be swept over the full frequency band (12–18 GHz) in less than 5 μs. The high frequency fringes due to the change in the cut-off layer can be distinguished from those due to the existence of density fluctuations, since the fluctuation level with frequency components larger than the ion cyclotron range of frequency (10 MHz) is much smaller than the average density (10−4 or less). The reliability of density profile measurement is investigated for various sweep times of HTO from 5 μs to 5 ms. It is observed that the reliability is greatly improved and the reconstructed density profiles agree well with that obtained from an interferometer when the sweep time is faster than 10 μs. The present reflectometer is routinely applied to determine the density profiles of the central cell plasma in GAMMA 10.

Abstract: Reflectometric techniques have been applied to the GAMMA 10 tandem mirror to measure density and magnetic fluctuations associated with electromagnetic plasma waves in the ion cyclotron range of frequency (ICRF). The excitation and propagation of ICRF-driven waves are studied by reflectometers in order to achieve efficient heating of plasmas. The radial profile of the fluctuation level is expected to give the deposition profile of the heating power. A cross-polarization scattering diagnostic system is also installed in the central cell of GAMMA 10. An incident ordinary (extraordinary) wave is converted to an extraordinary (ordinary) wave by magnetic fluctuations. The converted wave can propagate through a cut-off layer and reach the detector. The level of magnetic fluctuation (about 10 −4 ) is evaluated for the ICRF-driven waves.

Abstract: Transfer molding is a typical method for packaging integrated circuits (ICs). Transfer molding is very difficult to do at heights less than 1 mm, and has a limited surface mounting area. Recent research of high density surface mounting for thinner and lighter weight packaging has progressed for both IC cards and liquid crystal displays (LCDs). However, the dispensing method has many problems, such as the height variations of the coating, difficulties of uniform spreading on large size ICs, and one-by-one encapsulation. A unique printing method is developed to solve such problems. Through this method it is possible to achieve a height less than 1 mm and a uniform thickness on an integrated system. On reaching these parameters, the surface mount ICs replace molded packaged devices that are manufactured using expensive equipment and molding dies. The method requires only a small amount of epoxy resin and is, therefore, more economical than the conventional transfer molding process. A one-component epoxy resin NPR-150 suitable for printing is developed. >

Abstract: Dodecanoyl amidoalkylguanidine hydrochlorides (C(12)A(m)G, m = 2, 3, 4, 6) are cationic surfactants that have an amidoalkyl group (A(m)) as spacer between the cationic guanidine and hydrophobic groups in the molecule. The effect of the A(m) group on the aggregation properties of the surfactants was evaluated through measurements of their critical micelle concentration (cmc) value, Krafft point, phase behavior, area occupied by one molecule at the air/water interface, and micellar aggregation number. Dodecylguanidine hydrochloride (C(12)A(0)G) with no A(m) group is a unique cationic surfactant because it exhibits a strong tendency for self-assembly when compared with common ionic surfactants, due to the hydrogen bonding between its guanidine groups in addition to the hydrophobic interaction between its alkyl chains [M. Miyake, K. Yamada, N. Oyama, Langmuir 24 (2008) 8527-8532]. In contrast, C(12)A(m)G showed a decreasing tendency for self-assembly with increasing alkyl chain length, m, of the A(m) group up to m = 3, above which the tendency increased. Such changes in aggregation tendency of the surfactants were suggested to arise from an increased bulkiness of the hydrophilic part caused by the A(m) group, resulting in a decrease in the hydrogen bonding between the guanidine groups and an increase in micellization through the cooperative hydrophobic interaction between the hydrophilic groups. From the balance of these effects, the area of the hydrophilic part of C(12)A(4)G was the largest and the hydrogen bonding between the guanidine groups in C(12)A(4)G was weakened. It is suggested in guanidine-type surfactant that A(4) gave a similar aggregation tendency to traditional ionic surfactants and a weak effect for skin.

Abstract: The elastic neutron-scattering experiments were carried out on the solid solutions ${\text{CeRh}}_{1\ensuremath{-}x}{\text{Co}}_{x}{\text{In}}_{5}$ to clarify the nature of the antiferromagnetic (AF) state in the vicinity of the quantum critical point: ${x}_{c}\ensuremath{\sim}0.8$. The incommensurate AF order with the wave vector of ${q}_{h}=(1/2,1/2,\ensuremath{\sim}0.3)$ observed in pure ${\text{CeRhIn}}_{5}$ is weakly suppressed upon doping with Co, and a commensurate ${q}_{c}=(1/2,1/2,1/2)$ and an incommensurate ${q}_{1}=(1/2,1/2,\ensuremath{\sim}0.42)$ AF structures evolve at intermediate Co concentrations. The AF orders with ${q}_{c}$ and ${q}_{1}$ are enhanced at $x=0.7$, and furthermore, the ${q}_{h}$ AF order vanishes. These results suggest that the AF correlations with the ${q}_{c}$ and ${q}_{1}$ modulations are significantly enhanced in the intermediate $x$ range, and may be connected with the evolution of the superconductivity observed above $x\ensuremath{\sim}0.3$.

Abstract: A spontaneous transition phenomena between two states of a plasma with an internal transport barrier (ITB) is observed in the steady-state phase of the magnetic shear in the negative magnetic shear plasma in the JT-60U tokamak. These two ITB states are characterized by different profiles of the second radial derivative of the ion temperature inside the ITB region (one has a weak concave shape and the other has a strong convex shape) and by different degrees of sharpness of the interfaces between the L mode and the ITB region, which is determined by the turbulence penetration into the ITB region.

Abstract: BACKGROUND AND PURPOSE: To investigate associations between cerebral ischemic events and signal hyperintensity in T1-weighted MR imaging (T1WI) of carotid plaque according to stenosis severity and to estimate persistence of T1WI signal hyperintensity. METHODS: A total of 222 patients (392 atherosclerotic carotid arteries) underwent plaque imaging using 3D inversion-recovery-based T1WI (magnetization-prepared rapid acquisition with gradient-echo [MPRAGE]). Carotid plaque with intensity on MPRAGE of >200% that of adjacent muscle was categorized as “high signal intensity” and correlated with ipsilateral ischemic events within the previous 6 months. A total of 58 arteries (35 patients) underwent repeat MR imaging a total of 70 times at a median interval of 279 days (range, 10–1037 days). RESULTS: Ipsilateral ischemic events were more frequent in patients with MPRAGE high signals than in patients with low signals in the 0%–29%, 30%–69%, and 70%–99% stenosis groups: Relative risk (95% confidence interval) was 2.50 (0.96–6.51), 7.55 (1.84–31.04), and 1.98 (1.01–3.90), respectively. In the 70 cases of repeat MR imaging, 29 of 30 cases with high signals on the preceding MR imaging maintained high signals. Of the 58 arteries that underwent repeat MR imaging, 4 of 22 carotid arteries with high signals developed ipsilateral subsequent ischemic events within 1 year, whereas none with low signals developed subsequent events. CONCLUSIONS: Carotid plaque signal hyperintensity on T1WI is strongly associated with previous ipsilateral ischemic events, persisting over a period of months, and may indicate risk of subsequent events. Larger clinical trials are warranted to clarify associations between signal hyperintensity and risk of subsequent cerebral ischemic events.

Abstract: We performed elastic neutron scattering experiments on solid solution CeRh1-xCoxIn5 with x=0.4 to clarify the nature of the antiferromagnetic (AF) phase in the vicinity of the quantum critical point. We observed the incommensurate AF order below T_Nh=3.5 K. The structure of the incommensurate AF order is basically unchanged from that for pure CeRhIn5. We further found the evolution of a new commensurate AF order with the modulation of q_c=(1/2,1/2,1/2) below T_Nc=2.9 K. The volume-averaged moments for the incommensurate and commensurate AF phases are 0.38 mu_B/Ce and 0.21 mu_B/Ce, respectively, which are reduced from the incommensurate AF moment (0.75 mu_B/Ce) for pure CeRhIn5. We suggest from these results that the commensurate magnetic correlation may be tightly coupled with the superconductivity observed below T_c=1.4 K., 5 pages, 4 figures, to appear in J. Phys. Soc. Jpn

Abstract: An alternative method to conserve water and produce crops in arid regions is through hydroponics. Application of treated wastewater for hydroponics will help in stripping off nutrients from wastewater, maximising reuse through reduced evaporation losses, increasing control on quality of water and reducing risk of pathogen contamination. This study focuses on the efficiency of treated wastewater from an on-site aerobic wastewater treatment unit. The experiment aimed to investigate 1) nutrient reduction 2) microbial reduction and 3) growth rate of plants fed on wastewater compared to a commercial hydroponics medium. The study revealed that the chemical and microbial quality of wastewater after hydroponics was safe and satisfactory for irrigation and plant growth rate in wastewater hydroponics was similar to those grown in a commercial medium.

Abstract: The roles of momentum transport on the toroidal rotation velocity (Vt) profile and the properties of transport coefficients are found by transient momentum transport analysis. The perturbation technique enables us to evaluate the momentum diffusivity (χφ) and the convection velocity (Vconv), and to calculate Vt profiles driven by external torque input by neutral beams (NBs). The measured Vt profiles with and without the external torque input are almost reproduced by χφ and Vconv in low-β (βN < 0.4) plasmas. At higher β, the local pressure gradient plays a role in determining the local value of intrinsic rotation velocity. Concerning the momentum transport, χφ increases with increasing heating power, and decreases with increasing plasma current (Ip). In H-mode plasmas, χφ is smaller than that in L-mode plasmas under similar experimental conditions. It is found that χφ, which is separated from the non-diffusive term increases with increasing heat diffusivity (χi), χφ/χi ∼ 1-3, and -Vconv increases with increasing χφ, Vconv/χφ ∼ -2.5 to -0.7 m-1 , in H-mode plasmas.

Abstract: Since the last IAEA conference, the scientific programme of JET has focused on the qualification of the integrated operating scenarios for ITER and on physics issues essential for the consolidation of design choices and the efficient exploitation of ITER. Particular attention has been given to the characterization of the edge plasma, pedestal energy and edge localized modes (ELMs), and their impact on plasma facing components (PFCs). Various ELM mitigation techniques have been assessed for all ITER operating scenarios using active methods such as resonant magnetic field perturbation, rapid variation of the radial field and pellet pacing. Abstract: Background/aim: In prostate cancer, robotic total prostatectomy is a popular treatment modality. However, prostate-specific antigen (PSA) recurrence after prostate cancer surgery remains a concern. Salvage radiotherapy is commonly used to treat PSA recurrence, but the recurrence rate after salvage radiotherapy is high, highlighting the need for better predictive markers. This study aimed to retrospectively evaluate the association between cribriform pattern and PSA recurrence in patients receiving radiotherapy after radical prostatectomy., Patients and Methods: Data of 50 patients who underwent radiotherapy after total prostatectomy between January 2010 and May 2020 were retrospectively evaluated. The median age was 67 years. Among these patients, two cases involved postoperative irradiation, while 48 cases involved salvage irradiation after postoperative PSA recurrence. The median time from surgery to PSA recurrence was 38.3 months. The median radiation dose was 64 Gy in 32 fractions. Three-dimensional conformal radiation therapy was administered in 38 cases and intensity-modulated radiation therapy was used in 12 cases. Combined hormone therapy was administered in 21 cases. PSA levels were measured every 3 months after treatment. Statistical analysis between groups was performed by a t-test., Results: The median follow-up period after radiotherapy was 31 months. No local recurrences were observed at the prostate bed, and no deaths related to prostate cancer were recorded during follow-up. However, 18 patients (36.0%) had PSA recurrence. The PSA recurrence rate based on the cribriform pattern was 17.6% in the none to moderate group (34 patients) and 75.0% in the severe cribriform pattern group (16 patients). The PSA recurrence rate was significantly higher in patients with a severe invasive cribriform pattern (p=0.001). 46. Incidental finding of aberrant left common carotid artery during intracerebral hemorrhage examination.

Abstract: We report a case involving an aortic arch anomaly that has not been previously documented. The patients were a 76-year-old female who was urgently transported to the hospital because of a sudden disturbance of consciousness. Neurological symptoms indicated impending brain herniation, and the patient was diagnosed with hypertensive intracerebral hemorrhage (left putamen-thalamus) and acute hydrocephalus, thereafter she died approximately 6 hours after arrival. Head and neck-to-chest computed tomography angiography revealed an atypical aortic arch branching pattern. From the proximal side, the left common carotid artery, right common carotid artery, right subclavian artery, and left subclavian artery originated sequentially, suggesting an aberrant left common carotid artery. In the context of acute stroke management, understanding such aortic arch variations is crucial for planning access routes and treatment strategies for neuroendovascular procedures., (© 2024 The Authors. Abstract: Background: In 2018, diagnostic criteria were introduced for IgG4-related periaortitis/periarteritis and retroperitoneal fibrosis (PA/RPF). This study assessed the existing criteria and formulated an improved version., Methods and Results: Between August 2022 and January 2023, we retrospectively analyzed 110 Japanese patients diagnosed with IgG4-related disease (IgG4-RD) involving cardiovascular and/or retroperitoneal manifestations, along with 73 non-IgG4-RD patients ("mimickers") identified by experts. Patients were stratified into derivation (n=88) and validation (n=95) groups. Classification as IgG4-RD or non-IgG4-RD was based on the 2018 diagnostic criteria and various revised versions. Sensitivity and specificity were calculated using experts' diagnosis as the gold standard for the diagnosis of true IgG4-RD and mimickers. In the derivation group, the 2018 criteria showed 58.5% sensitivity and 100% specificity. The revised version, incorporating "radiologic findings of pericarditis", "eosinophilic infiltration or lymphoid follicles", and "probable diagnosis of extra-PA/-RPF lesions", improved sensitivity to 69.8% while maintaining 100% specificity. In the validation group, the original and revised criteria had sensitivities of 68.4% and 77.2%, respectively, and specificities of 97.4% and 94.7%, respectively., Conclusions: Proposed 2023 revised IgG4-related cardiovascular/retroperitoneal disease criteria show significantly enhanced sensitivity while preserving high specificity, achieved through the inclusion of new items in radiologic, pathological, and extra-cardiovascular/retroperitoneal organ categories.

48. Metastatic Pleomorphic Carcinoma of the Lung with Extensive Chromosomal Rearrangements: An Autopsy Case with a Literature Review.

Abstract: We herein report a 47-year-old man who presented with progressive paraparesis. Imaging revealed a right upper pulmonary nodule, massive bilateral adrenal metastases, thoracolumbar vertebral osteolysis, and subcutaneous nodules. A biopsy of the right buttock nodule revealed a poorly differentiated metastatic carcinoma with high programmed cell death-ligand 1 expression and extensive chromosomal rearrangements. The patient died 10 days after the initiation of pembrolizumab treatment. Autopsy findings confirmed pulmonary pleomorphic carcinoma with extensive metastases. Quantification of chromosomal rearrangements revealed a jump-up mutation from the normal karyotype, followed by a further incremental increase in the degree of deviation.

49. Examination of iatrogenic FDG accumulation after COVID-19 vaccination.

Abstract: Purpose: This study aimed to investigate the frequency of COVID-19 vaccine-induced reactive change and potential factors including blood type correlated with increased FDG uptake on positron emission tomography (PET)/computed tomography (CT)., Materials and Methods: We evaluated 284 patients who underwent PET/CT between June and September 2021 and had a known history of COVID-19 vaccination. Information on the injection site, vaccine type, and adverse reactions was obtained. We visually assessed the presence or absence of accumulation in the axillary and supraclavicular lymph nodes and the deltoid muscles. We measured the maximum standardized uptake value (SUVmax) using semi-quantitative analysis., Results: Our study included 158 males and 126 females aged 16-94. The median time between vaccination and PET/CT was 9 and 42 days for patients who had received their first and second doses, respectively. We observed axillary lymph node accumulation, supraclavicular lymph node accumulation, and deltoid muscle accumulation in 98 (SUVmax 1.07-25.1), nine (SUVmax 2.28-14.5), and 33 cases (SUVmax 0.93-7.42), respectively. In cases with axillary lymph node (P = 0.0057) or deltoid muscle (P = 0.047) accumulation, the shorter the time since vaccination, the higher the FDG accumulation. Patients with axillary lymph node accumulation were significantly younger (P < 0.0001) and had a significantly higher frequency of adverse reactions such as fever (P < 0.0001) and myalgia (P = 0.002). No significant relationship was observed between blood type and the frequency of FDG accumulation. Logistic regression analysis also showed that age, gender, days since vaccination, and adverse reactions such as fever and myalgia were important factors for axillary lymph node accumulation., Conclusion: Our study found that FDG accumulation in the axillary lymph nodes and deltoid muscle was higher within a shorter time after vaccination, and axillary lymph node accumulation was higher in young patients, females, and those with adverse reactions of fever and myalgia. No significant relationship was observed between blood type and the frequency of FDG accumulation. Abstract: Background: Systemic sclerosis (SSc) is an autoimmune disease characterized by vascular injury and inflammation, followed by excessive fibrosis of the skin and other internal organs, including the lungs. CX3CL1 (fractalkine), a chemokine expressed on endothelial cells, supports the migration of macrophages and T cells that express its specific receptor CX3CR1 into targeted tissues. We previously reported that anti-CX3CL1 monoclonal antibody (mAb) treatment significantly inhibited transforming growth factor (TGF)-β1-induced expression of type I collagen and fibronectin 1 in human dermal fibroblasts. Additionally, anti-mouse CX3CL1 mAb efficiently suppressed skin inflammation and fibrosis in bleomycin- and growth factor-induced SSc mouse models. However, further studies using different mouse models of the complex immunopathology of SSc are required before the initiation of a clinical trial of CX3CL1 inhibitors for human SSc., Methods: To assess the preclinical utility and functional mechanism of anti-CX3CL1 mAb therapy in skin and lung fibrosis, a sclerodermatous chronic graft-versus-host disease (Scl-cGVHD) mouse model was analyzed with immunohistochemical staining for characteristic infiltrating cells and RNA sequencing assays., Results: On day 42 after bone marrow transplantation, Scl-cGVHD mice showed increased serum CX3CL1 level. Intraperitoneal administration of anti-CX3CL1 mAb inhibited the development of fibrosis in the skin and lungs of Scl-cGVHD model, and did not result in any apparent adverse events. The therapeutic effects were correlated with the number of tissue-infiltrating inflammatory cells and α-smooth muscle actin (α-SMA)-positive myofibroblasts. RNA sequencing analysis of the fibrotic skin demonstrated that cGVHD-dependent induction of gene sets associated with macrophage-related inflammation and fibrosis was significantly downregulated by mAb treatment. In the process of fibrosis, mAb treatment reduced cGVHD-induced infiltration of macrophages and T cells in the skin and lungs, especially those expressing CX3CR1., Conclusions: Together with our previous findings in other SSc mouse models, the current results indicated that anti-CX3CL1 mAb therapy could be a rational therapeutic approach for fibrotic disorders, such as human SSc and Scl-cGVHD., (© 2024.
