We present a two-band Bose-Hubbard model which is shown to be minimal in the necessary coupling terms at resonant tunneling conditions. The dynamics of the many-body problem is studied by sweeping the system across an avoided level crossing. The linear sweep generalizes Landau-Zener transitions from single-particle to many-body realizations. The temporal evolution of single- and two-body observables along the sweeps is investigated in order to characterize the non-equilibrium dynamics in our complex quantum system., Comment: invited paper for special issue of Annals of Physics

7. Manifold approach for a many-body Wannier-Stark system: localization and chaos in energy space

We study the resonant tunneling effect in a many-body Wannier-Stark system, realized by ultracold bosonic atoms in an optical lattice subjected to an external Stark force. The properties of the many-body system are effectively described in terms of upper-band excitation manifolds, which allow for the study of the transition between regular and quantum chaotic spectral statistics. We show that our system makes it possible to control the spectral statistics locally in energy space by the competition of the force and the interparticle interaction. By a time-dependent sweep of the Stark force the dynamics is reduced to a Landau-Zener problem in the single-particle setting.

A paradigm model of modern atom optics is studied, strongly interacting ultracold bosons in an optical lattice. This many-body system can be artificially opened in a controlled manner by modern experimental techniques. We present results based on a non-hermitian effective Hamiltonian whose quantum spectrum is analyzed. The direct access to the spectrum of the metastable many-body system allows us to easily identify relatively stable quantum states, corresponding to previously predicted solitonic many-body structures.

Hace algún tiempo tuve la oportunidad de asistir a una conferencia que dictaba el padre Benedictino HILARIO BARRAGUER sobre la importancia de los Salmos en la vida del consagrado y me pareció de bastante importancia la relación que hacía el Padre Barraguer de los Salmos con la palabra de Dios, y es éste el motivo por el cual me animé a escribir estas líneas. Además, fuera de comentar lo que escuché quiero plantear también mi propia experiencia de sacerdote que tiene un contacto permanente con la palabra de Dios en los Salmos que la liturgia de las horas selecciona para que ciertos grupos consagrados en la Iglesia alaben a su Señor. Una cosa que creo es importante aclarar, es que, en ningún momento pretende este artículo ser un análisis exegético de los Salmos, primero porque no soy Teólogo exégeta y segundo porque no podemos remitir a estudios de consagrados Biblístas que han dedicado la mayor parte de su vida a un estudio profundo de los Salmos. Los almacenes de cadena son organizaciones en donde el personal es numeroso, diverso, de alta rotación y alto retiro; además la gran variedad de negocios que tienen tales organizaciones, hacen que posean muchas aplicaciones a las cuales muchos usuarios deben ingresar y en la mayoría de veces éste acceso se vuelve administrativamente costoso. Por tanto se hace necesario el establecimiento de un procedimiento informático (administración de usuarios) que permita al propio usuario gestionar sus contraseñas y que además le permita a la organización controlar la desactivación automática de los accesos al momento del retiro (terminación de contrato) del usuario. Para lograr esto existen variedad de herramientas en el mercado que satisfacen dicha necesidad, pero su implementación y soporte es demasiado costoso; por eso este artículo pretende mostrar algunos conceptos importantes y la manera en que otras herramientas y metodologías pueden realizar un adecuado proceso de autogestión y retiro del usuario en los almacenes de cadena a un menor costo.

Culture environments, trays and doses of organic compost were evaluated in the formation of cucumber seedlings (Cucumis sativus L.). Five environmental conditions were tested: (A1) a greenhouse with height of 2.5 m, covered with polyethylene film, (A2) nursery with height of 2.5 m, monofilament fabric, 50% shading, (A3) nursery with height of 2.5 m, heat-reflective screen, 50% shading, (A4) nursery with a height of 1.8 m, covered with coconut tree straw and (A5) greenhouse with height of 4.0 m, covered with polyethylene film, with zenith opening and thermo-reflective cloth under the plastic. Trays of 72 (R1) and 128 (R2) cells were filled with 93% soil and 7% organic compound (S1), 86% soil and 14% organic compound (S2) and 79% soil and 21% organic compound (S3). It was used a randomized design in split-split-plot scheme, with five replicates (environments x trays x substrates). The greenhouses provide the best environments for the formation of cucumber seedlings. A tray of 72 cells is the best container, promoting more vigorous seedlings in substrate with soil and 7 or 14% organic compound.Ambientes de cultivo, bandejas e doses de composto orgânico foram avaliados na formação de mudas de pepino (Cucumis sativus L.). Cinco ambientes de cultivo foram testados: (A1) estufa agrícola com altura de 2,5 m coberta com filme de polietileno; (A2) viveiro com altura de 2,5 m, tela de monofilamento com 50% de sombreamento; (A3) viveiro com altura de 2,5 m, tela termorrefletora, com 50% de sombreamento; (A4) viveiro com altura de 1,8 m, coberto com palha de coqueiro, e (A5) estufa agrícola com altura de 4,0 m, coberta com filme de polietileno, com abertura zenital e tela termorrefletora sob o filme. Bandejas de 72 (R1) e 128 (R2) células foram preenchidas com 93% de solo e 7% de composto orgânico (S1); 86% de solo e 14% de composto orgânico (S2), e 79% de solo e 21% de composto orgânico (S3). Utilizou-se um delineamento inteiramente casualizado, em esquema de parcelas subsubdivididas, com cinco repetições (ambientes x bandejas x substratos). As estufas agrícolas propiciam os melhores ambientes para a formação das mudas de pepino. A bandeja de 72 células é o melhor recipiente, promovendo plântulas mais vigorosas no substrato com solo e 7 ou 14 % de composto orgânico.

This study evaluated the effect of processing method and severity of hybrid fall rye on dry matter intake (DMI), ruminal fermentation, and apparent total tract nutrient digestibility for beef cattle. Ruminally cannulated Simmental heifers (n = 8; 327±33 kg) were used in a replicated 4×4 Latin square (21-d periods) balanced for carry-over effects with a 2×2 factorial treatment. Tempered rye [water added to achieve 80% dry matter (DM)] was compared with dry-rolled rye. Roller gap width of 1.23-mm and 0.8-mm were used for coarse and finely processed rye, respectively. This resulted in processing index values of: 80.9% for dry-rolled and coarse; 72.7% for dry-rolled and fine; 92.9% for tempered and coarse; and 88.2% for tempered and fine processed rye. Diets (DM basis) included hybrid rye grain (86%), barley silage (15%), mineral (6%), and dry distillers’ grain plus solubles (3%). Feeding tempered rye increased (P< 0.01) DMI by 0.87 kg compared with dry-rolled rye, and feeding coarse rye increased (P=0.04) DMI by 0.33 kg compared with fine rye. Cattle fed dry-rolled and finely processed rye had greater (P< 0.04) area that pH was < 5.5 compared with tempered and coarse rye, respectively. Ruminal short-chain fatty acid concentrations and ammonia were not affected (P≥0.12). Feeding dry-rolled rye increased DM digestibility by 2.4% (P=0.02), and crude protein digestibility in 3.0% (P=0.01) compared with tempered rye. Starch digestibility was not affected by processing severity of dry-rolled rye, but increased from 92.8 to 95.9% for tempered rye with increasing severity (interaction, P=0.03). Rolling tempered rye at the same roller gap as dry-rolled rye decreases processing severity but may allow for a greater severity of processing without marked reductions in DMI and ruminal pH, while achieving similar total tract starch digestibility. When increasing processing severity (81 vs. 73%), dry-rolling hybrid rye may reduce DMI and ruminal pH.

This study evaluated the impact of naturally occurring water with a high-sulfate concentration on dry matter intake (DMI), water intake, growth, and trace mineral status of yearling beef heifers. Angus-based heifers (n = 48; 398.4 ± 22.1 kg) were randomly assigned to 1 of 12 pens (4 heifers/pen) and fed for 100 d arranged into 25 d periods. Heifers were provided water containing 350 (CON); 2,300 (MEDS); or 4,300 mg/L (HIGHS) sulfate. Concentrated water (71,482 mg/L sulfate) from natural source was collected and blended to achieve the treatments. All data were analyzed using the fixed effects of treatment, period (d 1-25, 26-50, 51-75, and 76-100), and the treatment × period interaction, with period as a repeated measure. Average daily gain, and gain-to-feed were not affected (P>0.29). Provision of HIGHS decreased (interaction, P< 0.01) DMI from d 1-25, 26-50 and 51-75, with no effect from d 76-100 compared with CON. Heifers drinking CON and MEDS during d 26-50 had greater (interaction, P< 0.01) water intake than HIGHS with no differences among treatments otherwise. Total sulfate intake (g/d) was greater for HIGHS, followed by MEDS and CON with a greater difference between treatments during d 26-50 (interaction, P< 0.01). Heifers provided MEDS and HIGHS had a greater (interaction, P< 0.01) reduction in serum copper compared with CON. Liver copper concentration did not differ at the start of the study but was 74% and 62% of CON for MEDS and HIGHS, respectively at the end of the study (interaction, P< 0.01). Serum selenium was greater for MEDS and HIGHS than CON; while liver selenium was least for HIGHS heifers at the end of the study (interaction, P< 0.01). These data highlight that beef heifers were able to tolerate 213 g/d of sulfate intake with no negative impact on growth performance; however, MEDS and HIGHS decreased liver copper and liver selenium concentrations.

- Abstract
Background and Objective After whiplash injury, some patients develop chronic whiplash-associated disorders. The exact pathophysiology of this chronification is still unclear and more knowledge is needed regarding the different post-injury phases. Therefore, studies were searched that examined temporal changes in pain processing, measured by Quantitative Sensory Testing (QST). Databases and Data Treatment This systematic review searched three electronic databases (Medline, Web of Science and Embase) for articles meeting the eligibility requirements. Risk of bias was assessed according to a modified Newcastle-Ottawa Scale. Results The 12 included studies presented moderate to good methodological quality. These studies showed altered pain processing within the first month after injury and normalization within 3 months in 59%-78% of the patients. After 3 months, recovery stagnates during the following years. Thermal and widespread mechanical hyperalgesia occur already in the acute phase, but only in eventually non-recovered patients. Conclusions Differences in pain processing between recovering and non-recovering patients can be observed already in the acute phase. Early screening for signs of altered pain processing can identify patients with high risk for chronification. These insights in temporal changes show the importance of rehabilitation in the acute phase. Future research should target to develop a standardized (bed-site) QST protocol and collect normative data which could, in relation with self-reported pain parameters, allow clinicians to identify the risk for chronification. Significance Altered pain processing is present soon after whiplash injury, but usually recovers within 3 months. Non-recovering patients show little to no improvements in the following years. Differences between recovering and non-recovering patients can be observed by Quantitative Sensory Testing already in the acute phase. Therefore, it is considered a feasible and effective tool that

Objetivo: Determinar los costes mediante la medición directa, caso a caso, de la infección del sitio quirúrgico en la apendicectomía y la colectomía. Métodos: Diseño: estudio de casos y controles con base poblacional; los casos fueron todos los pacientes diagnosticados de infección de sitio quirúrgico tras apendicectomía o colectomía en SCIAS-Hospital de Barcelona, y los controles una muestra seleccionada aleatoriamente entre los pacientes no infectados que reunían características comunes para el apareamiento. Período de estudio: del 1 de enero de 1998 al 31 de diciembre de 1999. Tipos de apareamiento: a) individual de pacientes agrupados según el diagnóstico principal, el procedimiento quirúrgico principal, la edad y el sexo, y b) individual de pacientes agrupados según el diagnóstico principal, la edad y el sexo, pero utilizando además para la selección de los controles el diagnóstico secundario. Fuentes de información: sistema informático de registros e historias clínicas en soporte electrónico, incluyendo los costes registrados en tiempo real. Contabilidad analítica: programa ADS plus®, que calculó costes reales (paciente por paciente) a partir de datos como el consumo de farmacia, el material sanitario, las pruebas adicionales de diagnóstico y las estancias generadas. Los costes estructurales se imputaron en la matriz de costes a partir del cálculo contable y su impacto en los costes directos. Se consideró coste atribuible a la infección al coste incremental o adicional resultante de la diferencia entre casos y controles. Análisis estadístico: los cálculos estadísticos se realizaron mediante el paquete estadístico SPSS, versión 9.0. Se utilizaron pruebas no paramétricas teniendo en cuenta el tamaño de la muestra. Se aplicó el test de Wilcoxon para variables continuas con el fin de comparar la edad, la duración de las intervenciones quirúrgicas, la estancia hospitalaria y los costes. Los resultados se expresaron como media aritmética. Se consideró un nivel de significación estadística un valor de p < 0,01. Resultados: En el primer apareamiento se incluyeron 23 casos y 23 controles para los pacientes operados de apendicectomía, y 20 casos y 20 controles de colectomía. En el apareamiento 2 la muestra fue de 17 pares de casos y controles en apendicectomía, y 14 pares de casos y controles en colectomía. La infección comportó un aumento de la estancia media entre 7,7 días (apareamiento 1) y 7,3 días (apareamiento 2) en apendicectomía infectada, y entre 17,6 días (apareamiento 1) y 15,4 días (apareamiento 2) en colectomía. Los pacientes con infección de apendicectomía presentaron un coste medio tres veces superior a los no infectados tanto en el apareamiento 1 (2.998,60 frente a 941,89 ?, respectivamente; p < 0,0001) como en el apareamiento 2 (2.751,70 frente a 870,81 ?, respectivamente; p < 0,0001). Similar hallazgo se observó en la colectomía, tanto en el apareamiento 1 (10.705,34 frente a 2.600,55 ?, respectivamente; p < 0,0001) como en el apareamiento 2 (9.081,12 frente a 2.621,39 ?, respectivamente; p < 0,0001). El coste medio atribuible a la infección de sitio quirúrgico osciló entre 2.056,71 ? (apareamiento 1) y 1.880,89 ? (apareamiento 2) en el caso de la apendicectomía, y 8.140,79 ? (apareamiento 1) y 6.405,65 ? (apareamiento 2) en la colectomía. Conclusiones: En las condiciones del estudio, la infección del sitio quirúrgico determina la prolongación de la estancia hospitalaria de más de una semana en la apendicectomía y de más de dos en la colectomía, con un incremento superior al 300% en el coste directo total.

This study evaluated the effects of undigested neutral detergent fiber (uNDF) concentration and forage inclusion (FI) rate on dry matter (DM) intake, ruminal pH, reticular contractions, and gastrointestinal permeability for finishing beef cattle. Five ruminally cannulated Hereford′Simmental heifers (699±69.1 kg) were used in an incomplete 6×6 Latin square (26-d periods) with a 2×3 factorial treatment arrangement. Barley grain-based diets were formulated using barley silage or wheat straw to provide low or high uNDF (7.1 vs. 8.5% DM) with forage proportions of 5, 10, or 15% of dietary DM. Dry matter intake (P ≥ 0.10) and eating time (P ≥ 0.13) were not affected by uNDF, FI, or uNDF′FI. With low uNDF diets, increasing FI numerically (P = 0.02) increased rumination time (min/d); while, with high uNDF diets, rumination time increased with 5 to 10% FI, but not thereafter (P = 0.03). Mean ruminal pH was not affected by uNDF (6.17 vs. 6.19; P = 0.08), but increased with increasing FI (6.04b, 6.23a, and 6.28a; P = 0.02). Duration of ruminal pH < 5.5 was not affected by uNDF but tended (P = 0.07) to be reduced with increasing FI. High uNDF diet tended to increase the frequency of reticular contractions (1.43 vs. 1.51 contractions/min; P = 0.07) but decreased the contraction duration (13.2 vs. 14.1 sec; P = 0.04). Increasing FI increased contraction frequency (1.39b, 1.50a, and 1.53a contractions/min; P = 0.03) and tended to reduce contraction duration as forage increased from 5 to 10 and 15% (14.3, 13.1, and 13.6 sec; P = 0.07). Feeding high uNDF decreased (P = 0.05) permeability of the gastrointestinal tract based on the appearance of Cr-EDTA in urine following an intra-ruminal dose. Increasing FI tended to reduce gastrointestinal tract permeability (P = 0.06). Limited interactions indicate that uNDF and FI act independently suggesting that increasing dietary uNDF, without increasing FI rate, can stimulate frequency of reticulo-ruminal contractions and reduce gastrointestinal permeability for finishing cattle.

This study evaluated the effects of silage source, physically effective neutral detergent fiber (peNDF), and undigested neutral detergent fiber (uNDF) concentrations on eating behavior, ruminal pH, and reticular contractions for finishing beef heifers. Six Hereford′Simmental heifers (579±51.2 kg) were used in a replicated 6×6 Latin square (21-d periods) balanced for carry-over effects with 2×2 + 2 factorial treatment arrangement. Barley grain-based diets included barley (BarS) or wheat silage (WhS) at 10% of dietary DM. Silages were harvested at two chop lengths to yield low (LpeNDF) vs. high peNDF (HpeNDF) silage. For both BarS and WhS, chopped wheat straw was included (5% of DM) by replacing an equal proportion of LpeNDF silages to evaluate low vs. high uNDF (7.3 vs. 8.2% dietary DM) concentrations. Heifers fed HpeNDF WhS had greater DMI (silage′peNDF; P < 0.01) than those fed HpeNDF BarS, but DMI did not differ for LpeNDF treatments. Ruminating and eating time increased for heifers fed HpeNDF WhS relative to LpeNDF WhS, but were not different for BarS (silage′peNDF; P ≤ 0.02). Increasing uNDF increased eating and ruminating times for heifers fed WhS, but not BarS (silage′uNDF; P ≤ 0.03). Mean ruminal pH averaged 5.97 and was not affected by silage, peNDF, uNDF, or the interactions (P ≥ 0.12). Feeding BarS increased the duration (384 vs. 256 min/d; P = 0.02) and area (84.5 vs. 43.8 pH×min/d; P = 0.02) that pH was < 5.5 relative to WhS. Increasing peNDF numerically reduced the frequency of reticular contractions when fed BarS but numerically increased them when fed WhS (silage′peNDF; P < 0.01). Reticular contractions were not affected by dietary uNDF concentration (P ≥ 0.13). These data highlight that at a constant inclusion rate, silage source and chop length interact to affect DMI, eating behavior, ruminal pH, and reticular motility indicating the need to account for silage type, peNDF, and uNDF in diet formulation for finishing cattle.

This study was designed to evaluate the effect of silage source (barley vs. wheat silage) when harvested at two chop lengths (low vs. high physically effective neutral detergent fiber [peNDF]) and when barley silage was partially replaced with straw to increase the undigested neutral detergent fiber (uNDF) concentration on performance and carcass characteristics of finishing steers. Four hundred and fifty yearling commercial crossbred steers with an initial body weight (BW) of 432 ± 30.5 kg were allocated to 30 pens and fed diets containing 90% concentrate:10% forage for 123 d in a completely randomized block design with a 2 × 2 + 1 factorial arrangement. Treatments included 1) barley silage (BarS) with low peNDF (LpeNDF); 2) BarS with high peNDF (HpeNDF); 3) BarS with straw to yield a diet with LpeNDF + uNDF; 4) wheat silage (WhS) LpeNDF; and 5) WhS HpeNDF. There were no silage × peNDF interactions for dry matter intake (DMI), average daily gain (ADG), or gain to feed ratio (G:F), but cattle fed WhS LpeNDF had a lower (P < 0.01) proportion of yield grade 3 and a greater proportion in yield grade 2 carcasses than cattle fed BarS LpeNDF or HpeNDF and WhS HpeNDF. Cattle fed WhS LpeNDF had greater (P = 0.02) incidence of severe liver abscesses when compared with cattle fed BarS LpeNDF or HpeNDF and WhS HpeNDF. Cattle fed BarS consumed less (P < 0.01) uNDF as a percentage of BW, had increased (P = 0.02) ADG, heavier (P = 0.02) hot carcass weight, with greater (P = 0.01) back fat thickness, and (P < 0.01) incidence of minor liver abscesses when compared with cattle fed WhS. Feeding HpeNDF did not affect DMI, ADG, or G:F, but increased (P = 0.02) marbling score and reduced (P < 0.01) the proportion AA quality grade and increased (P < 0.01) those classified as AAA when compared with cattle fed LpeNDF. Cattle fed low uNDF had lesser (P < 0.01) uNDF intake as a percentage of BW, greater dressing percentage (P = 0.01), had a lower (P < 0.01) proportion of carcasses in yield grade 2, and a greater (P < 0.01) proportion of carcasses in yield grade 3 when compared with cattle fed high uNDF. Thus, silage source, peNDF, and uNDF content do not impact DMI or G:F when diets contain 10% forage, but BarS relative to WhS as well strategies increasing the peNDF concentration may increase ADG, HCW, back fat thickness, dressing percentage, marbling score, and carcasses classified as quality grade AAA. Future research is needed to evaluate the usefulness of peNDF and uNDF in rations for finishing cattle.

- Abstract
Treatment of patients with cancer in hospitals or clinics is resource-intensive and imposes a burden on patients. ‘Flexible care’ is a term that can be used to describe treatment administered outside the oncology ward, oncological outpatient clinic or office-based oncologist setting. Programmes that reduce travel burden by bringing cancer treatment to the patient's home, workplace or closer to the patient's home, in the form of satellite clinics or mobile cancer units, expand treatment capacity and are well received. Clinical trial data show that, compared with intravenous administration, subcutaneous (s.c.) administration of trastuzumab is preferred by patients with breast cancer (BC), saves healthcare professionals' (HCPs) time, reduces drug preparation and administration time and reduces direct and indirect costs. As such, s.c. trastuzumab is well suited to flexible care. The results of a Belgian study (BELIS) show that home administration of s.c. trastuzumab is feasible and preferred by patients with BC. Numerous programmes and pilot studies in Europe show that s.c. trastuzumab can be administered effectively in the patient's home, in primary care settings or local hospitals. Such programmes require planning, training, careful patient selection and technology to link patients, caregivers and specialists in oncology clinics. Once these elements are in place, flexible care offers patients with BC a choice of how treatment may be delivered and lead to improved quality of life, while reducing pressure on HCPs and hospitals. BACKGROUND: Early spontaneous preterm delivery is often associated with microbial invasion of the amniotic cavity and/or intraamniotic inflammation. OBJECTIVE: The objective of the study was to develop and validate clinically feasible multivariable prediction models of spontaneous delivery within 7 days and microbial invasion of the amniotic cavity in women admitted with diagnose of preterm labor and intact membranes below 34 weeks. STUDY DESIGN: We used data from a cohort of women admitted from 2012 to 2018 with diagnosis of preterm labor below 34 weeks who had undergone amniocentesis to rule out microbial invasion of the amniotic cavity. The main outcome was spontaneous delivery within 7 days from admission. The secondary outcome was microbial invasion of the amniotic cavity, defined by a positive culture and/or 16S ribosomal RNA gene in the amniotic fluid. The sample (n = 358) was divided into derivation (2012-2016) and validation cohorts (2017-2018). Logistic regression models using a stepwise selection of variables were developed for the outcomes evaluated. We explored as predictive variables ultrasound cervical length measurement at admission, maternal C-reactive protein, gestational age, amniotic fluid glucose, and interleukin-6 (expressed as log units). Models were developed in the derivation cohort and applied to the validation cohort and diagnostic performance was calculated. RESULTS: The derivation cohort included 263 women and the validation cohort 95 women. One hundred five of the women (39%, 105 of 268) spontaneously delivered in the following 7 days and 68 (19%, 68 of 358) had microbial invasion of the amniotic cavity. For spontaneous delivery within 7 days after admission, 4 predictors were identified: cervical length at admission, gestational age, amniotic fluid glucose, and interleukin-6. The diagnostic performance of the model was assessed in the validation cohort using the receiver operating characteristic curve and showed an area under curve of 0.86 (95% confidence interval, 0.77-0.95) with a detection rate of spontaneous delivery within 7 days of 87%, a false-positive rate of 33%, a negative predictive value of 80%, and a negative likelihood ratio of 0.1908. For microbial invasion of the amniotic cavity, 2 independent predictors of the amniotic cavity were identified: amniotic fluid glucose and maternal C-reactive protein. The receiver operating characteristic curve and an area under curve in the validation cohort was 0.83 (95% confidence interval, 0.70-0.96) with a detection rate of 76%, a false-positive rate of 8%, a negative predictive value of 93%, and a negative likelihood ratio of 0.2591. CONCLUSION: In women with preterm labor, we propose 2 clinically feasible prediction models to classify as low vs high risk of spontaneous delivery within 7 days and of microbial invasion of the amniotic cavity. The models showed a high diagnostic performance and could be of value to optimize clinical management.

This work shows recent theoretical contributions in the area of competition and strategy which have been expressed by a mathematical model. Taking as its starting point the theory of decision making, so that may be used for empirical research. The development objective of the model is to explain the possible outcomes of the strategic management of a firm, having as theoretical base the theory of competition and strategy. Finally, using the model it is concluded that competitive advantage is not exploitable feature, but is the result of the deployment of skills that are imperative for the best performance and business, understanding the latter as the supply of attributes of customer value. In this sense, competitive advantage is a condition with extrinsic in relation to other competitors. Furthermore, it is considered that the quality that enables a company to get better returns is the distinctive competence, whose value is evaluated in the operation of the company. El presenta trabajo muestra los últimos aportes teóricos en el área de competencia y estrategia los cuales han sido expresados mediante un modelo matemático. Tomando como punto de partida la teoría sobre toma de decisiones, a fin de que pueda emplearse para investigaciones empíricas. El objetivo del desarrollo del modelo es explicar los posibles resultados de la gestión estratégica de una empresa, teniendo como base teórica la teoría de la competencia y estrategia. Finalmente, con ayuda del modelo se concluye que la ventaja competitiva no consiste característica explotable, sino que es resultado del despliegue de competencias que son imperativas para el mejor desempeño y del negocio; entendido este último como la oferta de atributos de valor al cliente. En ese sentido, la ventaja competitiva consiste en una condición extrínseca en relación a con otros competidores. Además, se considera que la cualidad que permite a una empresa obtener mejores rendimientos es la competencia distintiva, cuyo valor se evalúa en la operación de la empresa.

OBJECTIVE: To evaluate the influence of perinatal inflammation on neurodevelopmental outcome of premature infants. STUDY DESIGN: From a retrospective cohort study of women with preterm labor with intact membranes or preterm prelabor rupture of membranes (PPROM) with an amniocentesis to rule out intra-amniotic inflammation (IAI) and microbial invasion of the amniotic cavity (MIAC), we evaluated neurodevelopmental outcome of their infants born between 24.0 and 34.0 weeks gestation. Women with clinical chorioamnionitis at admission were excluded. Neurodevelopmental outcome was screened with the Ages & Stages Questionnaire (ASQ)-3. We analyzed the relationship between an altered ASQ-3 and antenatal, intra-partum and post-partum factors related to perinatal inflammation. Objective To determine the incidence and risk factors of first cardiovascular event (CVE) in patients with chronic inflammatory rheumatic diseases (CIRD). Methods Analysis of data after 2.5 years of follow-up from the prospective study CARMA project, that includes patients with CIRD [rheumatoid arthritis (RA), ankylosing spondylitis (AS), and psoriatic arthritis (PsA)] and matched individuals without CIRD from 67 hospitals in Spain. CVE cumulative incidence per 1000 patients was calculated after 2.5 years from the start of the project. Weibull proportional hazard model was used to calculate hazard ratio (HR) and 95% confidence interval (95% CI) of the risk factors. Results 2595 (89.1%) patients completed the 2.5 years of follow-up visit. Cumulative incidence of CVE in patients with CIRD was 15.30 cases per 1000 patients (95% CI: 12.93-17.67), being higher in the subgroup with AS; 22.03 (95% CI: 11.01-33.04). Patients with AS (HR: 4.11; 95% CI: 1.07-15.79), those with older age (HR: 1.09; 95% CI: 1.05-1.13), systolic hypertension (HR: 1.02; 95% CI: 1.00-1.04) and long duration of the disease (HR: 1.07; 95% CI: 1.03-1.12) were at higher risk of first CVE during the 2.5 years of follow-up. In contrast, female gender was a protective factor (HR: 0.43; 95% CI: 0.18-1.00). Conclusion Among CIRD patients prospectively followed-up at rheumatology outpatient clinics, those with AS show higher risk of first CVE. Objective Cardiovascular (CV) disease is one of the main causes of morbi-mortality in spondyloarthritis (SpA), partially explained by traditional CV risk factors. Information on lipoprotein(a) [Lp(a)], a non-conventional risk factor, in SpA is scarce. In this study we assessed the prevalence of hyperlipoproteinaemia(a) in SpA patients and analysed the possible related factors. Methods A baseline analysis was made of ankylosing spondylitis (AS) and psoriatic arthritis (PsA) patients and controls included in the CARMA project (CARdiovascular in RheuMAtology), a 10-year prospective study evaluating the risk of CV events in chronic inflammatory rheumatic diseases. A multivariate logistic regression model was performed using hyperlipoproteinaemia( a) (Lp(a) >50 mg/dl) as a dependent variable and adjusting for confounding factors. Results 19.2% (95% CI: 16.80-22.05) of the SpA patients [20.7% (95% CI: 16.91-24.82) of those with AS and 17.7% (95% CI: 14.15-21.75) of those with PsA] and 16.7% (95% CI: 13.23-20.86) of the controls had hyperlipoproteinaemia(a) (p=0.326). Adjusting for age and sex, SpA patients were more likely to have hyperlipoproteinaemia(a) than controls (OR: 1.43, 95% CI: 1.00-2.04; p=0.05), especially those with AS (OR: 1.81, 95% CI: 1.18-2.77; p=0.007). In the adjusted model, apolipoprotein B in all patients, non-steroidal anti-inflammatory drugs in AS, and female sex in PsA, were associated with hyperlipoproteinaemia(a). No disease-specific factors associated with hyperlipoproteinaemia(a) were identified. Conclusion SpA patients show a moderately increased risk of hyperlipoproteinaemia(a) compared to controls, especially those with AS. Lp(a) determination may be of interest to improve the CV risk assessment in SpA patients.

40. Shear wave elastography investigation of multifidus stiffness in individuals with low back pain

© 2019 Elsevier Ltd The purpose of this study was to investigate differences in passive muscular stiffness between the superficial multifidus (SM)and deep multifidus (DM), and to compare their passive and active stiffness in individuals with low back pain (LBP) and asymptomatic individuals. Fifteen LBP individuals and 15 asymptomatic individuals were recruited. Passive stiffness of the SM and DM was measured bilaterally using sheer wave elastography (SWE) with participants lying prone. Active stiffness was measured for the SM during trunk extension, and the contraction ratio was calculated. DM displayed higher passive muscular stiffness than SM in both the asymptomatic and LBP groups (14.41 ± 2.62 and 15.40 ± 2.77 kPa respectively; t = 7.765 and t = 3.864, p < 0.05). Individuals with LBP exhibited higher passive muscular stiffness of SM (LBP: 10.15 ± 4.21, asymptomatic: 6.84 ± 1.69 kPa; t = 3.002, p < 0.05)and a lower contraction ratio (LBP: 1.54 ± 0.47, asymptomatic: 2.65 ± 1.36 kPa; p < 0.05) compared to the asymptomatic group. The findings support a differentiation in passive muscular stiffness between SM and DM and provide evidence for an alteration in muscular stiffness at rest in individuals with LBP. The lower increase of muscular stiffness with contraction observed for those with LBP may reflect a deficit in activation of the multifidus.

41. Variation in the spatial distribution of erector spinae activity during a lumbar endurance task in people with low back pain

© 2019 Anatomical Society This study aimed to investigate the spatial distribution and redistribution of lumbar erector spinae (ES) activity during a lumbar extension endurance task in pain-free participants and how this is modified in people with low back pain (LBP). High density surface electromyography (HDEMG) was recorded using 13 × 5 electrode grids placed over the lumbar ES in 13 LBP and 13 control participants while completing an Ito test to task failure. The root mean square of the HDEMG signals was computed, a topographical map of the EMG amplitude generated and the centre of the activity (centroid) determined throughout the task. The centroid of the EMG amplitude map was systematically more cranial (F = 6.09, P = 0.022) for the LBP participants compared with the control subjects. Regression analysis showed that the extent of redistribution of ES activity was associated with longer endurance. These results show that LBP participants utilised a different motor strategy to perform the endurance task, characterised by greater activation of more cranial regions of the ES and less redistribution of ES activity throughout the task. This study provides new insight into the functional activation of the lumbar ES and how it is modified when people have pain.

- Abstract
Skull Base osteomyelitis is a rare clinical entity, even more in the pediatric population, with a high overall mortality. Typical cases appear as a complication of malignant otitis external, compromising the temporal bone. We present a case of a male 11 month old infant that was admitted at the intensive care unit of the Hospital Universitario "José Eleuterio González " located in Monterrey, México, because of a history of 7 days of fever, cough, otorrhea, bipapebral edema with right side conjuntival erythema and septic shock with altered consciousness. After the initial assesment and treatment a non contrasted head CT was made that showed gas density in the skull base, the cavernous sinus and the right transverse sinus topography, right stranding of intraconal fat with a hyperdense a thick right superior ophthalmic vein and hipodense lesions on the brain. Even though the magnetic resonance (MRI) has the higher sensibility and specificity for this image diagnosis, recognizing through a simple head tomography this entity and possible complications is important because of the need of urgent adequate and directed treatment., La osteomielitis craneal es una entidad clínica poco común y aún mas rara en pediátricos, que puede afectar la bóveda craneal o la base del cráneo, con alta tasa de mortalidad. Presentamos un caso de un lactante masculino de 11 meses de edad que fue admitido a la unidad de cuidados intensivos del Hospital Universitario " José Eleuterio González" en Monterrey, México, por una historia de 7 días de fiebre, tos, otorrea, edema bipalpebral con eritema conjuntival derecho y choque séptico con alteración del estado de conciencia. Después de la valoración y tratamiento inicial se solicitó una tomografía computarizada no contrastada del cráneo, que demostro material con densidad de gas en la base del cráneo, en las topografías del el seno cavernoso y seno transverso derecho, borramiento de planos grasos de la grasa intraconal derecha con una vena oftalmica superior hiperdensa y gruesa, asi como lesiones hipodensas en el cerebro. Aunque la resonancia magnética simple tiene la mayor sensibilidad y especificidad para este diagnóstico por imagen, saber reconocer a través de una tomografía simple esta entidad y las complicaciones se vuelve importante por la necesidad de un tratamiento dirigido y adecuado de forma urgente.

- Abstract
[Background]: Inflammation is known to be related to the leading causes of death including cardiovascular disease, several types of cancer, obesity, type 2 diabetes, depression-suicide and other chronic diseases. In the context of whole dietary patterns, the Dietary Inflammatory Index (DII®) was developed to appraise the inflammatory potential of the diet. [Objective]: We prospectively assessed the association between DII scores and all-cause mortality in two large Spanish cohorts and valuated the consistency of findings across these two cohorts and results published based on other cohorts., [Design]: We assessed 18,566 participants in the “Seguimiento Universidad de Navarra” (SUN) cohort followed-up during 188,891 person-years and 6790 participants in the “PREvencion con DIeta MEDiterránea” (PREDIMED) randomized trial representing 30,233 person-years of follow-up. DII scores were calculated in both cohorts from validated FFQs. Higher DII scores corresponded to more proinflammatory diets. A total of 230 and 302 deaths occurred in SUN and PREDIMED, respectively. In a random-effect meta-analysis we included 12 prospective studies (SUN, PREDIMED and 10 additional studies) that assessed the association between DII scores and all-cause mortality., [Results]: After adjusting for a wide array of potential confounders, the comparison between extreme quartiles of the DII showed a positive and significant association with all-cause mortality in both the SUN (hazard ratio [HR] = 1.85; 95% CI: 1.15, 2.98; P-trend = 0.004) and the PREDIMED cohort (HR = 1.42; 95% CI: 1.00, 2.02; P-trend = 0.009). In the meta-analysis of 12 cohorts, the DII was significantly associated with an increase of 23% in all-cause mortality (95% CI: 16%–32%, for the highest vs lowest category of DII)., [Conclusion]: Our results provide strong and consistent support for the hypothesis that a pro-inflammatory diet is associated with increased all-cause mortality. The SUN cohort and PREDIMED trial were registered at clinicaltrials.gov as NCT02669602 and at isrctn.com as ISRCTN35739639, respectively.

In this paper, the influence of pseudohalide ligands coordinated to the ruthenium center on the hydrogenation of cyclohexene was investigated. The pseudohalide complexes containing ruthenium were synthesized from the cis-[RuCl2(dppb)(N-N)] {where N-N = 2,2′-bipyridine (bipy) (1A), 4,4′-dimethyl-2,2′-bipyridine (Me-bipy) (2), or 4,4′-dimethoxy-2,2′-bipyridine (MeO-bipy) (3), dppb = 1,4-bis(diphenylphosphino)butane}, by chloride exchange to provide complexes with general formula cis-[RuL2(dppb)(N-N)] {where L2 = N3− (4), (7) and (9) CN− (5), (8) and (10) or SCN− (6) and (11)}. All complexes were characterized using spectroscopic, electrochemical techniques, as well as elemental analyses to check their molecular formulation and purity. Furthermore, crystal structure of the complexes cis-[Ru(N3)2(dppb)(bipy)] (4), cis-[Ru(CN)2(dppb)(bipy)] (5), cis-[Ru(SCN)2(dppb)(bipy)] (6), cis-[Ru(CN)2(dppb)(Me-bipy)] (8), and cis-[Ru(SCN)2(dppb)(MeO-bipy)] (11) were determined by single-crystal X-ray diffraction, as well as the X-ray structure of the solvate complex [RuCl(CH3CN)(dppb)(bipy)](PF6) (1B). Dichloride and pseudohalide complexes containing ruthenium were applied as pre-catalysts in the hydrogenation of cyclohexene, after pre-established conditions using the cis-[RuCl2(dppb)(bipy)] (1A) as a target complex. The quantitative conversion of cyclohexene to cyclohexane was obtained in 5 h at 25 °C using 0.1 mol% of (1A) in methanol solution at 15 atm H2. The substituted bipy derivatives (2) and (3) were also effective pre-catalysts, but pseudohalides were not under these conditions.

5 Tablas, Background: Potatoes have a high glycemic load but also antioxidants, vitamins, and minerals. It is unclear what mechanisms are involved in relation to their effect on blood pressure (BP) and hypertension. Objectives: This study aimed to assess the association between potato consumption, BP changes, and the risk of hypertension in 2 Spanish populations. Methods: Separate analyses were performed in PREDIMED (PREvención con DIeta MEDiterránea), a multicenter nutrition intervention trial of adults aged 55-80 y, and the SUN (Seguimiento Universidad de Navarra) project, a prospective cohort made up of university graduates and educated adults with ages (means±SDs) of 42.7±13.3 y for men and 35.1± 10.7 y for women. In PREDIMED, generalized estimating equations adjusted for lifestyle and dietary characteristics were used to assess changes in BP across quintiles of total potato consumption during a 4-y follow-up. Controlled BP values (systolic BP < 140 mm Hg and diastolic BP < 90 mm Hg) during follow-up were also assessed. For SUN, multivariateadjusted HRs for incident hypertension during a mean 6.7-y follow-up were calculated. Results: In PREDIMED, the total potato intake was 81.9 ± 40.6 g/d. No overall differences in systolic or diastolic BP changes were detected based on consumption of potatoes. For total potatoes, the mean difference in change between quintile 5 (highest intake) and quintile 1 (lowest intake) in systolic BP after multivariate adjustment was 20.90 mm Hg (95% CI: -2.56, 0.76 mm Hg; P-trend = 0.1) and for diastolic BP was 20.02 mm Hg (95% CI: -0.93, 0.89 mm Hg; P-trend = 0.8). In SUN, the total potato consumption was 52.7 ± 33.6 g/d, and no significant association between potato consumption and hypertension incidence was observed in the fully adjusted HR for total potato consumption (quintile 5 compared with quintile 1: 0.98; 95% CI: 0.80, 1.19; P-trend = 0.8). En 2014 se ha celebrado en Reus el Campeonato Mundial de Patinaje Artístico. Este evento acoge las distintas modalidades de patinaje. Participaron 1.200 deportistas y cerca de 20 mil espectadores. La satisfacción entre el público alcanzó la puntuación de 8,5, sobre 10, y 8,1 entre los deportistas. La fidelización se expresó favorablemente puesto que el 95% del público asistente y el el 90% de los deportistas encuestados recomendarían a sus amigos y familiares la asistencia a un espectáculo parecido. El gasto directo estimado que ha revertido en el territorio es de 2,1 millones (euros). El multiplicador de la inversión es 3,22, mientras que el multiplicador relativo al consumo alcanza la cifra de 2,29 para el gasto de la organización y 2,71 para el gasto de asistentes. El efecto total se estima, para el conjunto de la economía de Cataluña, en 5,5 millones de euros. Los sectores más beneficiados son el turismo (22,8%), comercio (14,8%), servicios a empresas (14,2%) y transporte y comunicaciones (10%). ABSTRACT: The last days of September and beginning of October 2014 was held in the city of Reus Artistic Skating World Championships. This event welcome the differents types of skating and brought together abouth 1.200 athletes and 20 thousand spectators. The degree of satisfaction among athletes reached the score of 8.5 out of 10, while among the audience was 8.1 note. The loyalty expressed favorably since 95% of respondents would recommend attending a show of similar characteristics, while 90% of athletes are also evident in this sense. The estimated direct spending that has reversed in the territory is estimated at 2.1 million euros. The investment multiplier was 3.22, while the multiplier on the consumption reached the figure of 2.29 for the expenditure of the organization and spending 2.71 assistants. In overall, it is estimated for the whole economy of Catalonia a positive effect by 5.5 million euros. The most benefited sectors from these effects are tourisme (22.8%), trade (14.8%), business services (14.2%) and transport and communications (10%).

Fundamento. El objetivo de este estudio es estimar la prevalencia de insomnio en pacientes que precisaron ingresar en unidades psiquiátricas hospitalarias de corta estancia. Metodología. Se diseñó un estudio observacional descriptivo en el que tomaron parte 16 unidades de hospitalización psiquiátrica breve de adultos de España. El instrumento utilizado fue la Escala Atenas de Insomnio. Resultados. Se reclutó a 425 pacientes, obtuvieron un valor medio de la puntuación global de la Escala Atenas de Insomnio de 8,56 (DE: 5,78). El ítem para el que se obtuvo mayor puntuación media fue el relativo a la presencia de despertares durante la noche, seguido del relativo al tiempo que lleva quedarse dormido. La prevalencia de insomnio estimada es del 53% (IC 95%). Conclusiones. El insomnio es un problema de elevada prevalencia en las unidades de hospitalización psiquiátricas. Las mayores dificultades se dan en la inducción y el mantenimiento del sueño.

- Abstract
Background: Potatoes have a high glycemic load but also antioxidants, vitamins, and minerals. It is unclear what mechanisms are involved in relation to their effect on blood pressure (BP) and hypertension. Objectives: This study aimed to assess the association between potato consumption, BP changes, and the risk of hypertension in 2 Spanish populations. Methods: Separate analyses were performed in PREDIMED (PREvención con DIeta MEDiterránea), a multicenter nutrition intervention trial of adults aged 55-80 y, and the SUN (Seguimiento Universidad de Navarra) project, a prospective cohort made up of university graduates and educated adults with ages (means±SDs) of 42.7±13.3 y for men and 35.1± 10.7 y for women. In PREDIMED, generalized estimating equations adjusted for lifestyle and dietary characteristics were used to assess changes in BP across quintiles of total potato consumption during a 4-y follow-up. Controlled BP values (systolic BP < 140 mm Hg and diastolic BP < 90 mm Hg) during follow-up were also assessed. For SUN, multivariateadjusted HRs for incident hypertension during a mean 6.7-y follow-up were calculated. Results: In PREDIMED, the total potato intake was 81.9 ± 40.6 g/d. No overall differences in systolic or diastolic BP changes were detected based on consumption of potatoes. For total potatoes, the mean difference in change between quintile 5 (highest intake) and quintile 1 (lowest intake) in systolic BP after multivariate adjustment was 20.90 mm Hg (95% CI: -2.56, 0.76 mm Hg; P-trend = 0.1) and for diastolic BP was 20.02 mm Hg (95% CI: -0.93, 0.89 mm Hg; P-trend = 0.8). In SUN, the total potato consumption was 52.7 ± 33.6 g/d, and no significant association between potato consumption and hypertension incidence was observed in the fully adjusted HR for total potato consumption (quintile 5 compared with quintile 1: 0.98; 95% CI: 0.80, 1.19; P-trend = 0.8). - Abstract
The single ventricle is a congenital heart defect in which cannot be distinguished in ventricular mass more than a well-developed camera so this gets most of the atrioventricular connection. This complex congenital malformation occurs in 5 out of 100,000 live births representing 1% of congenital heart disease in infants. We report the case of a nursing female gender 4 months old, who was admitted to the "Hospital Materno Infantil" located in the city of Tegucigalpa, Honduras, for presenting 3 Months cyanosis in foot plantar region, nail and peribucal that deepened with crying, tachypnea and oxygen saturation of 54%. Performing a chest radiograph showed dextrocardia, confirmed by echocardiography, which also showed: common input single ventricle, severe pulmonary stenosis, mild ductus arteriosus persistence. The realization of a multislice tomography of abdomen with intravenous contrast reported suitable right spleen with homogeneous size and increased size left liver confirming situs inversus totalis. While situs inversus totalis is not a threat to life, the single ventricle has a poor prognosis if not treated in time, being very important the early diagnosis for early surgical intervention and thus further improve the quality of life of patients., El ventrículo único es un defecto congénito del corazón en el cual no se puede distinguir en la masa ventricular más de una cámara bien desarrollada por lo que esta recibe la mayor parte de la conexión atrioventricular. Esta malformación congénita compleja se manifiesta en 5 de cada 100,000 nacidos vivos representando el 1% de cardiopatías congénitas en lactantes. Presentamos el caso de una lactante del género femenino de 4 meses de edad, que fue ingresada en el “Hospital Materno Infantil” ubicado en la ciudad de Tegucigalpa, Honduras, por presentar cianosis de 3 meses de evolución en región palmo plantar, ungueal y peribucal que se agudizaba con el llanto, taquipnea y saturación de oxígeno de 54%. La realización de una radiografía simple de tórax mostró una dextrocardia, confirmándose por ecocardiografía, la cual también evidenció: ventrículo único de entrada común, estenosis pulmonar severa, persistencia del conducto arterioso leve. La realización de una tomografía multicorte de abdomen con medio de contraste intravenoso reportó bazo de lado derecho adecuado tamaño homogéneo e hígado aumentado de tamaño a lado izquierdo confirmando situs inversus totalis. Si bien el situs inversus totalis no es una amenaza a la vida, el ventriculo unico tiene mal pronostico si no se trata a tiempo, siendo de gran importancia el diagnóstico precoz para la intervención quirúrgica temprana y así además mejorar la calidad de vida de los pacientes.
