This paper investigates the paleoenvironmental evolution of a long core extracted in a small cove located in the Tinto-Odiel estuary (SW Spain). The inferred reconstruction is supported by sedimentological, geochemical, paleontological data and dating. Seven phases have been identified, with the transit from Late Neogene marine environments to a subrecent freshwater pond and a final anthropic filling. On the basis of these data, this area was flooded during the maximum of the MIS-1 transgression (6.5-5.2 cal. kyr BP), with the temporary presence of a subtidal channel with phanerogam meadows. During this evolution, three geochemical peaks have been detected, which correspond to 1) the first evidence of mining activities (~4.5 cal. kyr BP), 2) an interval of intensive mining (1850-1960) and 3) an industrial period (1966-1985), affected by the dumping of highly polluting waste into this estuary.

Portus was the main port of imperial Rome from the 2nd century AD to the 5th century AD, with an inner, hexagonal port (Trajan basin) and an outer port (Claudius basin) linked by a channel bounded by quays. Six sedimentary facies have been differentiated in the geological analysis of a core extracted in this channel, basically composed of poorly classified bioclastic muds typical of restricted environments, accumulations of Posidonia oceanica (Linnaeus) Delile, 1813 and a final anthropic fill. The subfossil record of bivalves, foraminifera and ostracods is characteristic of Mediterranean brackish environments (lagoons, estuaries, deltas) with significant environmental stress, due to changes in the physical-chemical parameters and probably the periodic dredging of this seaway during a period of about 300 years. The presence of “Neptune balls” points to the development of probable stormy periods that deposit these unique structures in the internal areas of this historic port.

Research related to food insecurity (FI) among European university student populations is currently limited, especially the studies carried out during the COVID-19 pandemic. The present study aimed to assess the prevalence and identify possible predictors of FI among students from a Spanish public university, the University of the Basque Country UPV/EHU, during the COVID-19 pandemic. A cross-sectional observational study design was used, in which a total of 422 students completed an online survey. Results were weighted according to age and field of education. Binary logistic regressions adjusted by sex, age, and campus were applied to identify FI predictors. FI in 19.6, 2.6, and 0.7% of the population was mild, moderate, and severe, respectively. The three main predictors of FI were a decrease in the main source of income (OR, 2.80; 95% IC, 2.57–3.06), not receiving scholarships during the pandemic (OR, 2.32; 95% IC, 2.18–2.47), and living arrangements before the pandemic (not living with parents/relatives) (OR, 2.03, 95% IC, 1.89–2.18). This study found a high prevalence of FI among the students surveyed, and the strongest predictors of this FI were related to socioeconomic status. A robust and comprehensive policy response is recommended to mitigate FI in this population. The malacological analysis of the Cañada Honda shell midden (Tinto-Odiel estuary, SW Spain) has allowed an approximation of the palaeo biodiversity of this sector during the Mesolithic. During this period, the progressive marine transgression caused the flooding of this estuary, coinciding with an initial intensive capture of the bivalves Lutraria and Ruditapes decussatus in the northern and southern sector of this site, respectively. In a second phase, Ruditapes decussatus was the main collected species in all sectors. This last period coincided with some diversification in catches, still dominated by Ruditapes decussatus but with significant proportions of Cerastoderma edule. This stage witnessed a progressive sedimentary filling of the estuary and the creation of marshes and barrier islands. The almost total fragmentation of the specimens would imply that the surface of this shell midden was exposed for a long time while in use. In addition, an occasional charring of some molluscs has been detected. The main molluscs of this site are also the most abundant in other later shell middens (∼cal 5.2–4 kyr BP), indicating an important perpetuation of the collecting and/or eating habits in this environment. All the species determined in them are also currently found in this estuary and its surroundings, where they are also captured.

Objective: The aim of this study was to investigate food purchasing behaviors, choice determinants, and opin-ions about on-campus food availability by a university community and to analyze differences in these aspects between students, education and/or research staff (ERS), and administrative and services staff (ASS), and between males and females.Methods: This was a cross-sectional study that involved a representative sample of students (n =1089), ERS (n = 396), and ASS (n = 300) who completed an anonymous online survey. A previously adapted version of the questionnaire was administered. The results were weighted to ensure representativeness of this commu-nity population using weighted coefficients.Results: The results showed that most of the participants purchased food on campus (91.6%), especially for lunch (67.4% of foods and 37.4% of drinks) and snack (65.4% of foods and 45.4% of drinks). Hot drinks (i.e., cof-fee, tea, hot chocolate etc.; 60.5%), bottled water (49.2%), and hot foods (i.e., small servings [38.2%] and sand-wiches/hamburgers [31.7%]) were the most purchased items. Taste (98.6%) was the most important determinant in choice, followed by price for students, nutritional value for ASS, and health value for ERS. The "top 5" opinions suggested for the campus food environment and potential changes were "greater capacity to access free filtered drinking water", "greater capacity to recycle food packaging," "more healthy options in vending machines", "discounts for healthy choices," and "allergen labeling."Conclusion: Interventions that improve sustainability and the affordability of products with high nutritional quality, price-manipulation directives, and allergen information on labeling would be well received among this community.

Background: Osteoporosis and cardiovascular diseases share risk factors and pathophysiological mechanisms. Hip fracture and cardiovascular diseases are very common in older people. Objectives: Evaluate clinical and subclinical arteriosclerotic disease in older patients with hip fracture compared with patients without fracture in order to increase knowledge about the relation between both diseases in old patients. Method: Age- and sex matched case-control study of octogenarians with and without recent hip fracture. Vascular risk factors, subclinical vascular diseases (assessed by carotid plaques, carotid intima media thickness and arterial stiffness) as well as cardiovascular diseases were analyzed. Univariate and multivariate logistic models were used to estimate Odds Ratios (OR) with their 95% confidence intervals (CI) to assess the association arteriosclerosis and hip fracture. Results: We analyzed 95 patients per group with a median age of 82 [79-87] years of whom 77.9% were female. Patients in both groups have elevated rates of vascular disease (25%) without differences between them. Patients with hip fracture had higher subclinical arteriosclerotic alterations with higher percentage of carotid plaques (OR 3.25 [1.06 - 9.97]) compared with the control group. Conclusions: Old patients with hip fracture had significantly higher presence of subclinical alterations but not increase on rate of cardiovascular arteriosclerotic disease compared with those without hip fracture. Is mandatory to increase our knowledge in shared risk factors for both diseases to enhance fracture prevention or vice versa.

El analisis sedimentologico y paleontologico permite definir cuatro facies sedimentarias, caracteristicas de depositos aluviales (F1: 6,5-6,2 ka BP), la presencia de facies estuarinas durante la inundacion marina del sector coincidente con el maximo transgresivo holoceno y el periodo inmediatamente posterior (F2: 6,2-5,2 ka BP), el transito a marismas (F3: 5,2 ka BP-siglo XIX) y un relleno antropico de finales del siglo XX. EnglishThis paper analyzes the vertical evolution of facies and the Ag and Bi contents of a continuous core extracted in the middle estuary of the Tinto river (S.O. Spain). The sedimentological and paleontological analysis allows to define four sedimentary facies, characteristics of alluvial deposits (F1: 6.5-6.2 ka BP), the marine flooding of the sector during the maximum of the Holocene transgression and the period immediately after (F2: 6.2 -5.2 ka BP), the transition to marshes (F3: 5.2-19th century) and an anthropic landfill since the end of the 20th century. The concentrations of Ag and Bi make it possible to specify three main periods of historical contamination: i) the start of mining activities and the exploitation of Ag during the Bronze Age (~ 4.5-3.2 ka BP); ii) the period of maximum intensity of Roman mining (1st-2nd centuries DC); and iii) recent intensive mining (since 1870).

Introducción y objetivo: Dentro de las posibles causas de AOS, aunque poco frecuentes, están las alteraciones de la epiglotis. El objetivo de este trabajo es destacar su importancia e insistir en su diagnóstico activo ya que en casos bien seleccionados se puede tratar quirúrgicamente. Síntesis: Se detalla la importancia de la epiglotis como causa de AOS, su exploración y las alternativas quirúrgicas posibles. Conclusiones: La epiglotis es una estructura anatómica que generalmente pasa desapercibida como causa de AOS en adultos y que puede provocar una AOS de carácter incluso severo. Debe ser estudiada de una manera activa. Es probable que su participación en la AOS no se revele durante la exploración con el paciente despierto y sea necesario recurrir a la DISE para apreciar el modo en que colapsa durante la inspiración del paciente dormido. Se describen algunas técnicas quirúrgicas ya que supone una buena elección en muchos pacientes debidamente seleccionados.

Las caráceas constituyen un grupo importante de macrófitos acuáticos en el Parque Nacional de Doñana. En este artículo, se estudia la distribución de sus girogonitos en diversos medios de este espacio protegido (lagunas temporales, surgencias, caños, márgenes del río Guadalquivir, etc), relacionándola tanto con las variables físico-químicas como con la textura y mineralogía del sedimento. Se efectúa una revisión del registro geológico de este grupo en diversos testigos compuestos por sedimentos pleistocenos y holocenos.

Objective:To evaluate diets in terms of nutritional characteristics and quality from the perspectives of health, greenhouse gas emissions (GHGE) and possible associations with each other in a representative sample of students at a public university.Design:Cross-sectional. Dietary intake was evaluated with a validated FFQ, and diet quality was assessed through the Healthy Eating Index (HEI-2010) and MedDietScore (MDS). GHGE data were obtained from the literature. In addition, sex, socio-economic status (SES) and body fat (BF) status were analysed as covariates.Setting:Basque Autonomous Community, Spain.Participants:Totally, 26 165 healthy adults aged 18–28 years.Results:Student diets were characterised by low consumption of carbohydrates (38·72 % of total energy intake (TEI)) and a high intake of lipids (39·08 % of TEI). Over half of the participants had low dietary quality. The low-emitting diets were more likely to be consumed by subjects with low HEI-2010 scores (β: 0·039 kg eCO2/1000 kcal/d) and high MDS scores (β: −0·023 kg eCO2/1000 kcal/d), after controlling for sex, SES and BF status. Both the low-emitting and healthy diets were more likely to be consumed by women and by those with normal BF percentage.Conclusions:UPV/EHU university students’ diets were characterised by moderate quality from a nutritional perspective and moderate variation in the size of carbon footprints. In this population, diets of the highest quality were not always those with the lowest diet-related GHGE; this relationship depended in part on the constructs and scoring criteria of diet quality indices used.

BACKGROUND The results obtained to date concerning food groups, diet quality and colorectal cancer (CRC) risk vary according to criteria used and the study populations. AIM To study the relationships between food groups, diet quality and CRC risk, in an adult population of the Basque Country (North of Spain). METHODS This observational study included 308 patients diagnosed with CRC and 308 ageand sex-matched subjects as controls. During recruitment, dietary, anthropometric, lifestyle, socioeconomic, demographic and health status information was collected. Adherence to the dietary recommendations was evaluated utilizing the Healthy Eating Index for the Spanish Diet and the MedDietScore. Conditional logistic regressions were used to evaluate the associations of food group intakes, diet quality scores, categorized in tertiles, with CRC risk. RESULTS The adjusted models for potential confounding factors showed a direct association between milk and dairy products consumption, in particular high-fat cheeses [odds ratio (OR) third tertile vs first tertile = 1.87, 95% confidence intervals (CI): 1.11-3.16], and CRC risk. While the consumption of fiber-containing foods, especially whole grains (OR third tertile vs first tertile = 0.62, 95%CI: 0.39-0.98), and fatty fish (OR third tertile vs first tertile = 0.53, 95%CI: 0.27-0.99) was associated with a lower risk for CRC. Moreover, higher MD adherence was associated with a reduced CRC risk in adjusted models (OR third tertile vs first tertile = 0.40, 95%CI: 0.20-0.80). CONCLUSION Direct associations were found for high-fat cheese, whereas an inverse relation was reported for fiber-containing foods and fatty fish, as well as adherence to a Mediterranean dietary pattern. This paper analyzes the first systematic faunal record of the 1755 Lisbon tsunami in the Mediterranean. On the basis of sedimentological and paleontological features, the sedimentary record of a core collected in Gibraltar was divided into six sedimentary facies, with a paleoenvironmental evolution from a shallow marine paleoenviroment to an increasingly restricted lagoon. This record includes a bioclastic layer deposited by the 1755 Lisbon tsunami and characterized by an erosive base, presence of basal rip-up clasts and abundant shell debris composed by marine and brackish molluscs. The paleoenvironmental reconstruction derived from the foraminiferal analysis is congruent with that inferred from the sedimentary and the macrofaunal reconstructions, with the introduction of brackish species into the innermost, intertidal areas of a confined lagoon. The Tinto and Odiel rivers (SW Spain) drain from a vast sulfide mining district and join at a 20-km-long estuary that enters the Atlantic Ocean. In this work, the contents of rare earth elements (REE) and fractionation in Neogene–Holocene sediment cores from La Fontanilla cove (Tinto estuary) were studied. The sediments were collected from a depth of 18 m at different distances from the recent river flow and were analyzed for new information on the temporal development of the REE load in the sediment column. Results show that the å REE is higher in the finer sediments and during periods of mining activity from prehistoric to recent times. Marine influence appears to increase the light REE (LREE) relative to the heavy REE (HREE). The REE patterns of these estuarine sediments show convex curvatures in the MREE relative to the LREE and HREE, indicating the presence of acid-mixing processes between the fluvial waters affected by acid mine drainage (AMD) and seawater, as well as the precipitation of poorly crystalline mineral phases. Significant positive Eu anomalies were found in ebb-tide channels and marsh deposits, which can reflect the mineralogical composition and/or a strong localized salinity gradient combined with organic matter degradation. This paper presents the easternmost mineralogical and geochemical evidence of the 1755 Lisbon tsunami found in the Western Mediterranean. This multidisciplinary analysis of a sediment core obtained in Gibraltar (southern Iberian Peninsula) has allowed us to differentiate a tsunamiite from an old lagoon (The Inundation). This tsunamigenic layer has increased levels of calcite and aragonite and higher concentrations of Ba and ferromagnesian elements in comparison with the underlying lagoonal sediments of this core. This layer is also differentiated by its paleontological record, with the introduction of marine species within this lagoon. The uppermost part of the core includes a transition from swampy/marsh paleoenvironments to terrestrial scenarios, with a final anthropogenic filling occurring during the last century, This work was carried out through the following projects: a) DGYCIT project CTM2006- 06722/MAR; b) DGYCIT project CGL2006-01412; and c) FEDER 2014-2020 project UHU-1260298. Background: To better understand Mexican adolescent’s body self-perception, this study aimed to analyze their body dissatisfaction (BD) levels according to sex and place of residence. We also aimed to explore differences in body composition (BC), physical fitness (PF), and physical activity (PA) between satisfied and dissatisfied adolescents and to find the associations between BD and these parameters. Methods: Cross-sectional, observational study carried out in Jalisco (Mexico) in which 451 adolescents (43.68% males, 43.90% rural) aged 13–17 years were evaluated. BD and self-perceived PF and PA were assessed with validated questionnaires, while objectively measured PF (strength, explosive strength, speed-agility, and cardiovascular fitness) was assessed using field tests. To evaluate BC, anthropometric measures and bioelectric impedance analysis were carried out. Regression analysis was used to ascertain the associations between health-related factors and body dissatisfaction. Results: Girls obtained higher scores on the BD questionnaire than boys (11.12 ± 3.13 vs. 10.33 ± 2.73, p &lt, 0.05), whereas no geographical differences between rural and urban adolescents were found. BD was positively associated with higher fat mass (β = 0.15, 0.001), and negatively associated with muscle mass and PF (β = −0.24 and β = −0.23, 0.001). However, PA was not associated with any of the measured parameters. Conclusions: selected parameters of BC, PF, and sex have an impact on the Mexican adolescent’s body satisfaction levels and should be considered when designing future health policies.

As a consequence of population aging, we have witnessed in internal medicine hospital wards a progressive shift from a population of in-patients relatively young and mainly affected by a single ailment to one of ever older and more and more complex patients with multiple chronic diseases, followed as out-patients by many different specialists with poor integration andinevitably treated with multiple medications. Polypharmacy (defined as the chronic intake of five or more drugs) is associated with increased risks of drug-drug interactions and related adverse effects, prescription and intake errors, poor compliance, re-hospitalization and mortality. With this background, the Italian Society of Internal Medicine chose to start in 2008 a prospective register called REPOSI (REgistro POliterapie SIMI, Società Italiana di Medicina Interna) in internal medicine and geriatric hospital wards. The country wide register is an ongoing observatory on multimorbidity and polypharmacy in the oldest old, with the goal to improve prescription appropriateness and, thus to avoid potentially inappropriate medications. The main findings of the register, that has accrued so far, 7005 older patients throughout a 10year period, are summarized herewith, with special emphasis on the main patterns of poor prescription appropriateness and related risks of adverse events.

The EHU12/24 (code of a survey from the University of the Basque Country/Euskal Herriko Unibertsitatea, UPV/EHU) study was designed to investigate the risk factors of overweight/obesity-related lifestyles, particularly those associated with diet, their psychosocial influences and the interactions among these factors. This observational cohort study was carried out according to a standardised protocol and involved a representative sample of the UPV/EHU student population. Anthropometric measurements, direct behavioural determinants, such as physical activity and diet, and indirect determinants, such as social/psychological factors, are considered. In this paper, we present the survey design, instruments, measurements and related quality management. We describe the study sample in terms of its socio-economic and demographic factors and knowledge area and summarise the methodology used to collect the data and obtain the anthropometric measurements. The participants were 603 students (59·5 % female) aged 18–28 years. The crude participation proportion was 53·5 %. Regarding the knowledge area, the lowest response proportions were obtained from the Health Sciences (38·6 %) compared with the Non-Health Sciences (48·3 %) (P = 0·003). The mean age was 20·9 years, and 83·1 % of the sample were from Basque Country. Regarding the socio-economic characteristics, there were significant differences by sex and knowledge area in most studied variables. Moreover, the Health Sciences students were more likely younger, from outside Basque Country, to have parents with university degrees and to have a higher social status. In conclusion, the EHU12/24 cohort provides valuable data for analysing the complexity and multidimensionality of obesity in university students.

This paper investigates the paleoenvironmental evolution of a core extracted in the middle sector of the Tinto River estuary, SW Spain, one of the most polluted areas in the world due to mining over thousands of years (>4 kyr BP) and recent industrial discharges. This evolution includes alluvial sands (>6.4 cal kyr BP), bioclastic sands and silts deposited in subtidal and intertidal channels during and after the Holocene transgression maximum (6.4-4.3 cal kyr BP), the sedimentation of clayey-sandy silts in low and high marshes during the last 2.4 kyr BP and a final anthropic filling. Three sharp peaks of pollution have been detected, representing a) a natural origin during the Holocene transgression; b) the impact of the first mining activities (~4.5 cal kyr BP); and c) the effect of industrial discharge and a new period of mining activity throughout the 19th and 20th centuries. Foraminifera, ostracods and molluscs disappeared during these last two peaks., Este trabajo investiga la evolución paleoambiental inferida a partir del estudio de un testigo extraído en el sector medio del estuario del río Tinto (SO de España), una de las zonas más contaminadas del mundo por actividades mineras milenarias (>4 kyr BP) y descargas industriales recientes. Esta evolución incluye arenas aluviales (>6.4 cal kyr BP), arenas bioclásticas y limos depositados en canales submareales e intermareales durante y después del máximo transgresivo holoceno (6.4-4.3 cal kyr BP), la sedimentación de limos arcillosos-arenosos marismeños durante los últimos 2.4 kyr BP y un relleno antrópico final. Se han detectado tres picos de contaminación, que proceden de: a) un origen natural durante el máximo transgresivo holoceno; b) el impacto de las primeras actividades mineras (~ 4.5 kyr BP); y c) el efecto conjunto de los vertidos industriales y un nuevo período de actividad minera durante los siglos XIX y XX. Los foraminíferos, ostrácodos y moluscos desaparecieron durante estos dos últimos picos.

Background. Given the lack of data about the nutritional value and other determinants of the consumption of foods and drinks sold in vending machines in European universities and the relevance of this sector in Spain, it is necessary to obtain scientific data on this topic. The present study aimed to assess the availability, nutritional profile and processing level of food products from vending machines at a Spanish public university and to investigate differences in nutritional profile according to the cost and promotion. Methods. Cross-sectional descriptive study. Data from all products available (3894) were collected and analysed using the criteria of the Spanish Agency for Consumption, Food Safety and Nutrition and the United Kingdom nutrient profiling model. The items were also classified according to the degree of industrial processing through the NOVA system. Promotion was assessed, taking into account where products were displayed in vending machines. Results. The most common products were sweets (23.4% of the total options), coffee (20.3%) and salty snacks (11.7%). According to the combination of the two criteria used to assess nutritional profile, 48.6% of the products were classified as with low nutritional quality (LNQ). In addition, 73.8% of the items were categorised as “ultra-processed”. Foods (β = 0.31, 95% CI 0.24, 0.39, p &lt, 0.001) and hot drinks (β = 0.46, 95% CI 0.39, 0.52, p &lt, 0.001) with high nutritional quality (HNQ) were more likely to have higher prices than alternatives with LNQ. Both foods and cold drinks that support healthy dietary recommendations were promoted to a lesser extent than those with LNQ (p &lt, 0.001). Conclusion. Almost half of the products were of LNQ and three-quarters had a high level of processing. Moreover, foods and cold drinks with LNQ were less expensive and more often promoted than alternatives with HNQ.

This paper investigates the prevalence and interaction of overweight/obesity-related lifestyles (specifically, examining whether subjects meet the recommended criteria), in a representative sample of university students. This study is part of the project EHU12/24, an observational cross-sectional study, designed to assess the prevalence of excess body fat (BF) and major risk of developing overweight/obesity, according to a standardised protocol. In a cohort of 603 students, aged between 18 and 28 years, of the University of the Basque Country, information about meal patterns, diet quality, physical activity, sitting time, sleeping time, toxic habits and anthropometric measurements were collected. Sampling took place from February 2014 to May 2017. Binary logistic regression models adjusted for the covariates were utilised to test the association between lifestyles and risk of excess adiposity. The prevalence of overweight/obesity, according to BF percentage, was 14·4 %. The variables analysed related to meal pattern were associated with diet quality, and less healthy food habits were associated with other less health-related behaviours. Among men, moderate/low physical activity, breakfast skipping, non-adequate breakfast duration, number of eating occasions and eating breakfast alone/depending on the occasion were associated with excess BF, while among women, low MedDietScore, moderate/high alcohol consumption, non-adequate sleep duration, eating breakfast and lunch alone/depending on the occasion. Results suggest that certain unhealthy lifestyle behaviours coexist, interact with one another and increase the risk of overweight/obesity in this population. Sex-specific differences in risk factors of obesity have implications for interventions for primary prevention of obesity within this environment.

Resumen: Introducción: El objetivo de nuestro trabajo es valorar si la estabilidad biomecánica que presenta la reconstrucción anatómica del ligamento talofibular anterior (LTFA) es similar a la que exhibe LTFA sano. Material y método: Realizamos un estudio biomecánico en cadáver con una muestra de 18 tobillos. Tras la aplicación de las maniobras de cajón anterior (CA) y estrés en varo (EV), medimos con un artrómetro, el movimiento angular en los tres planos espaciales (axial, coronal y sagital) registrado por un sensor localizado en el astrágalo. Resultados: Existen diferencias estadísticamente significativas en el plano axial entre el comportamiento biomecánico del LTFA intacto y del LTFA seccionado para la maniobra de CA con p = 0.012 y para la maniobra EV con p = 0.013. Por lo que respecta al plano coronal también objetivamos una diferencia estadísticamente significativa con la maniobra EV con p = 0.016. En el plano sagital no existen diferencias estadísticamente significativas con ninguna de las maniobras.Para finalizar, no existen diferencias estadísticamente significativas, si comparamos la reconstrucción anatómica con injerto del LTFA frente al LTFA intacto con ninguna de las maniobras. Conclusión: La reconstrucción anatómica con injerto del LTFA permite reproducir la estabilidad biomecánica del LTFA íntegro, en un modelo cadavérico a tiempo cero. Abstract: Introduction: The purpose of this work is perform a biomechanical comparison of anatomic reconstruction of the anterior talofibular ligament (ATFL) with the intact ATFL. Materials and methods: We studied 18 fresh cadaveric ankles with intact ATFL. Each specimen was clinically assessed with the anterior drawer (AD) and varus tilt (VT) tests and the angular movement in the three spatial planes (axial, coronal and sagittal) was measured with an arthrometer using a sensor located in the talus. Results: Statistically significant differences were found in the axial plane, between the intact ATFL versus the sectioned ATFL for AD test with p = 0.012, and for VT test with p = 0.013. Regarding the coronal plane, we also observed a statistically significant difference for VT test with p = 0.016. In the sagittal plane, there are no statistically significant differences in both maneuvers.No statistically significant differences were found when comparing the biomechanics of anatomic ligament reconstruction versus the intact ATFL. Conclusion: Autograft anatomic reconstruction of the ATFL showed biomechanical properties similar to those of the native ATFL, at the zero moment in a cadaveric model.

Introduction: The oldest-old population (80 years or older) has the highest lethality from COVID-19. There is little information on the clinical presentation and specific prognostic factors for this group. This trial evaluated the clinical presentation and prognostic factors of severe disease and mortality in the oldest-old population. Methods: This is an ambispective cohort study of oldest-old patients hospitalized for respiratory infection associated with COVID-19 and with a positive test by RT-PCR. The clinical presentation and the factors associated with severe disease and mortality were evaluated (logistic regression). All patients were followed up until discharge or death. Results: A total of 103 patients (59.2% female) were included. The most frequent symptoms were fever (68.9%), dyspnoea (60.2%), and cough (39.8%), and 11.7% presented confusion. Fifty-nine patients (57.3%) presented severe disease, and 59 died, with 43 patients (41.7%) presenting both of these. In the multivariate analysis, female sex (odds ratio [OR] 0.31, 95% confidence interval [95% CI] 0.13–0.73, p 0.0074) and serum lactate dehydrogenase (LDH) (OR 2.55, 95% CI 1.21–5.37, p 0.0139) were associated with severe disease, and serum sodium was associated with mortality (OR 3.12, 95% CI 1.18–8.26, p 0.0222). No chronic disease or pharmacological treatment was associated with worse outcomes. Conclusions: The typical presenting symptoms of respiratory infection in COVID-19 are less frequent in the oldest-old population. Male sex and LDH level are associated with severe disease, and the serum sodium level is associated with mortality in this population.

Objective:To assess the food environment at OsloMet, through the nutritional profile and processing level of available commercial foods and drinks, as well as to determine food-purchasing behaviours, preferences and opinions on the food environment, in order to identify whether interventions on campus need to be conducted.Design:Cross-sectional descriptive study.Setting:Pilestredet and Kjeller campus of OsloMet (Norway).Participants:To analyse the nutritional profile of products offered at all food outlets (seven canteens, three coffee shops and two vending machines) at the main campuses three criteria were applied: those proposed by the Spanish Agency for Food Safety and Nutrition, the UK nutrient profiling model and those of the Food and Drink Industry Professional Practices Committee Norway. In addition, products were classified by processing level, using the NOVA system. Food purchasing, food choice behaviours and opinions were analysed through a survey online, in which 129 subjects participated.Results:With regard to the first of the objectives, the combination of the above-mentioned criteria showed that 39·8 % of the products were ‘unhealthy’ and 85·9 % were ‘ultra-processed’. Regarding the second objective, the most important determinants of food choice were taste, convenience, and cost and nutrition/health value. The most common improvements suggested were lowering the cost, improving the allergen information on labelling and increasing the variety of fresh and healthy foods.Conclusions:A high proportion of the products offered were considered ‘unhealthy’ and highly processed. Interventions that improve food prices, availability and information on labelling would be well-received in this community.

Decarboxylation of fatty acids is an interesting route for the production of renewable long-chain hydrocarbons that could replace fossil resources in the formulation of diesel and jet range fuels. The results reported in the present work demonstrate that Pd supported on hierarchical ZSM-5 zeolite allows for the decarboxylation of different fatty acids (stearic, oleic, and palmitic acids) to proceed under mild operation conditions (T = 250 °C and PH2 = 6 bar) with over 90% selectivity toward middle distillate hydrocarbons and conversions in the range of 60–85%. This outstanding performance has been attributed to a combination of high accessibility and tailored acidity in the zeolitic support that favors the metal dispersion and promotes the interaction between Pd and acid sites. The presence of a hydrogen atmosphere, under low-pressure conditions, is required for activating the Pd sites. It is also noteworthy that the catalyst could be reused after washing with n-dodecane, retaining 74% of its initial stearic acid conversion with just a minor variation in the yield of C17 hydrocarbons.

Epidemiologic studies have revealed inconsistent evidence of gene-diet interaction in relation to colorectal cancer (CRC). The aim of this study was to analyze them in a sample of cases and controls from the population-based bowel cancer screening program of the Osakidetza/Basque Health Service. This study analyzed dietetic, genetic, demographic, socioeconomic factors and lifestyles. In the present manuscript, the survey design, sampling, instruments, measurements and related quality management were presented. Moreover, we analyze differences between cases and controls in some data, especially those related to diet. The participants were 308 cases and 308 age- and sex-matched subjects as controls. Cases were more likely than controls to have overweight/obesity (67.5% vs. 58.1%, p &lt, 0.05), a lower intake of vitamin B2 (0.86 &plusmn, 0.23 vs. 0.92 &plusmn, 0.23 mg/1000 kcal, p &lt, 0.01) and calcium:phosphorus ratio (0.62 &plusmn, 0.12 vs. 0.65 &plusmn, 0.13, p &lt, 0.01). A higher proportion of cases than controls did not meet the Nutritional Objectives for saturated fatty acids (85.7% vs. 67.5%, p &lt, 0.001) or cholesterol (35.4% vs. 25.0%, p &lt, 0.01). In conclusion, the present study provides valuable data for analyzing the complexity of gene-diet interaction in relation to CRC. The results presented here suggest that overweight/obesity and a high intake of certain dietary components, especially saturated fatty acids and cholesterol, are more frequent in cases than in controls.

Objective:To adapt a short FFQ (SFFQ) and evaluate its relative validity and reproducibility to assess food group intake in a population resident in the Basque Country. Moreover, the possible influence of associated variables (such as education level) on its validity and reproducibility was determined.Design:Nine-day 24-h recalls (24HR) were used as a reference to explore validity over the course of 1 year. The degree of misclassification in the SFFQ was evaluated by a contingency table of quartiles and by Bland–Altman plots comparing SFFQ2 and 24HR. SFFQ was administered twice to explore reproducibility at 1 year.Setting:Basque Autonomous Community.Participants:Adults aged ≥21 years (n 99). The sample was randomly selected and representative of the target population.Results:For validity, statistically significant correlations were observed for more than half of the food groups, with the lowest correlations (r or ρ) for fat (–0·008) and the highest for other foods (0·963). The mean percentage of the subjects’ food intake that was classified into the same or adjacent quartile in both methods was 75·2 %. Reproducibility was explored by the correlation coefficient and ranged from 0·201 to 0·809, and 82·6 % of participants were in the same or adjacent quartile in both SFFQ. The associated variables did not seem to influence the validity and reproducibility of the SFFQ.Conclusions:An adapted SFFQ presented good reproducibility and validity for measuring most food groups in the target population, and these results did not seem to be influenced by the associated variables.

Resumen Introduccion Los esguinces de tobillo son una de las patologias traumatologicas mas frecuentes. A pesar de un tratamiento conservador adecuado, este fracasa en el 20-40% de los casos. Estos pacientes suelen desarrollar una inestabilidad cronica de tobillo. En el 75-80% de las ocasiones existe una rotura aislada del ligamento talofibular anterior. Material y metodo Estudio retrospectivo observacional de 21 pacientes intervenidos quirurgicamente mediante tecnica de reparacion anatomica artroscopica, por inestabilidad lateral cronica de tobillo, entre mayo de 2012 y enero de 2013. Se realizo un seguimiento medio de 29 meses (rango de 25-33). La distribucion por sexos fue de 15 hombres y 6 mujeres; la edad media fue de 30,43 anos (rango de 18-48). En 4 pacientes (19%), se diagnostico una deformidad en varo del retropie asociada. En 15 pacientes (71,4%), se diagnosticaron lesiones intraarticulares: 6 pacientes presentaron lesiones osteocondrales (LOC) talares, 3 presentaban un pinzamiento posterior de tobillo y 6 pacientes presentaban distintas patologias de los tendones peroneos. Todas estas lesiones fueron abordadas quirurgicamente en el mismo acto quirurgico. Resultados Se llevo a cabo una revision clinico-funcional, utilizando la escala American Orthopaedic Foot and Ankle Society (AOFAS). La media preoperatoria fue de 66,12 puntos (rango de 60-71) y la postoperatoria se incremento hasta una media de 96,95 puntos (rango de 90-100). Todos los pacientes pudieron retomar sus actividades deportivas previas en una media de 21,5 semanas (rango de 17-28). Encontramos complicaciones en 3 pacientes (14%). Conclusiones La reparacion anatomica del ligamento talofibular anterior mediante esta tecnica presenta unos resultados clinico-funcionales excelentes, con una temprana reincorporacion del paciente a sus actividades deportivas y un bajo indice de reintervenciones y complicaciones.

© 2020 Elsevier Ltd Background: Observational studies have suggested that accelerated surgery is associated with improved outcomes in patients with a hip fracture. The HIP ATTACK trial assessed whether accelerated surgery could reduce mortality and major complications. Methods: HIP ATTACK was an international, randomised, controlled trial done at 69 hospitals in 17 countries. Patients with a hip fracture that required surgery and were aged 45 years or older were eligible. Research personnel randomly assigned patients (1:1) through a central computerised randomisation system using randomly varying block sizes to either accelerated surgery (goal of surgery within 6 h of diagnosis) or standard care. The coprimary outcomes were mortality and a composite of major complications (ie, mortality and non-fatal myocardial infarction, stroke, venous thromboembolism, sepsis, pneumonia, life-threatening bleeding, and major bleeding) at 90 days after randomisation. Patients, health-care providers, and study staff were aware of treatment assignment, but outcome adjudicators were masked to treatment allocation. Patients were analysed according to the intention-to-treat principle. This study is registered at ClinicalTrials.gov (NCT02027896). Findings: Between March 14, 2014, and May 24, 2019, 27 701 patients were screened, of whom 7780 were eligible. 2970 of these were enrolled and randomly assigned to receive accelerated surgery (n=1487) or standard care (n=1483).
