4. Avaliação dos métodos de mensuração de cor dos dentes vitais

- Abstract
O objetivo deste trabalho foi avaliar dois métodos de escolha de cor de dentes vitais. Foram selecionados 50 pacientes seguindo os critérios pré-estabelecidos. A avaliação da cor foi realizada nos incisivos centrais superiores. Para a avaliação da cor dos dentes foram utilizados dois métodos: escala de cor Vita Clássica(Vita-Zahnfabrik, Alemanha) (avaliação visual) e o aparelho espectrofotômetro Easyshade (Vita-Zahnfabrik, Alemanha) (avaliação instrumental). A prevalência ocorreu na cor A2 tendo equivalência entre os dois aparelhos. Concluímos que não houve diferença entre os métodos usados para a mensuração de cor.Descritores: Cor. Dente. Esta ocorrência representa cerca de 10% dos casos de insucessos nos tratamentos endodônticos. A con- duta terapêutica pode variar e está relacionada com o local da perfuração endodôntica. O presente trabalho se propõe a relatar um caso de perfuração iatrogênica e uma das alternativas possíveis para o tratamento das perfurações endodônticas em nível de colo dentário. Durante a abertura coronária ocorreu perfuração cervical devido à incorreta inclinação de penetração da broca. Considerando o local da perfuração, optou-se pelo selamento com resina composta, após acesso cirúrgico e isolamento absoluto. Após o sela- mento, o canal foi instrumentado e preenchido com medicação à base de hidróxido de cálcio. Posteriormente o canal foi obturado pela técnica da condensação lateral ativa com cimento Sealapex. O acompanhamento radiográfico mostrou regressão da lesão e o silêncio clínico confirmou o su- cesso do tratamento. The endodontic perforations are iatrogenic communications that result in a communication between the endodonto and periodontium. This occurrence represents about 10 % of cas- es of failure in endodontic treatment. The management may vary and is related to the drilling site endodontic. This paper aims to report a case of iatrogenic perforation and one of the possible alternatives for the treatment of endodontic perforations -level cervical tooth. During the opening coronary perforation occurred due to improper slope cervical penetration drill. Considering the drilling site, we opted for sealing com- posite resin after surgical access and absolute isolation. After sealing, the channel was filled with medication and instru- mented based on calcium hydroxide. Later the canal was obturated by lateral condensation technique with Sealapex. The radiographic follow-up showed regression of the lesion and the clinical silence confirmed the success of the treatment. Many mouth rinses(MR) and toothpaste (TP) are released with the statement of whiten your teeth, but without specifying the percentage of hydrogen peroxide and pH. The aim of this study was to evaluate the concentration of hydrogen peroxide and pH of some MR and TP available on the market. 3 were evaluated trademarks of MR and TP divided into 6 groups (n = 5): EB: G1- Colgate Plax Whitening (CPW); G2 Listerine Whitening (LW); G3-Cepacol Whitening Plus (C) and TP: G4-Aquafresh Ultimate White (MF); G5-Close up White now (CU); G6-Smile Xtreme and White (SO) and group G7-Opalescence PF control 10% (OP). For the evaluation of hydrogen peroxide dosing was used titration method with potassium permanganate and the PH evaluation was through pHmeter. Data were submitted to analysis of variance. The average amount of hydrogen peroxide and pH detected, according to the methodology used, it may be concluded that both have low amounts of hydrogen peroxide in regarding the control group, acid pH to mouth rinses and basic pH for toothpastes. Clinical relevance: hardly the mouth rinses and toothpastes evaluated will provide teeth whitening due to low amount of presented hydrogen peroxide and limited contact time with teeth. O propósito desta investigação, in vitro, foi avaliar a resistência de união a microtração (µTBS) de 4 cimentos resinosos, nas interfaces adesivas entre cerâmica (IPS Empress 2) e dentina. Foram selecionados 16 molares humanos hígidos e divididos aleatoriamente em 4 grupos. Grupo1 - (RX) Single Bond + Rely X, CRA; Grupo 2 - (VL) Excite DSC + Variolink II ; Grupo 3 - (PN) ED Primer + Panavia ; e Grupo 4 - (UC) Unicem. Foram analisados os resultados através do teste estatístico de análise de variância, seguido pelo teste de Tukey post hot. Medias em MPa ± desvio-padrão: RX (17,90a ± 3,53); VL (15,95b ± 3,07); PN (15,58b ± 2,90); UC (8,41c ± 2,54). Letras subscritas indicam valores homogêneos através do teste de Tukey, que demonstraram diferença entre os grupos, onde o cimento resinoso Rely X CRA obteve maior resistência adesiva a microtração. Variolink II e Panavia F resultaram em valores similares e a menor resistência adesiva foi do cimento resinoso Unicem.

Piezosurgery has its own characteristics such as precise cuts, selective and millimetric and also a clean surgical area that is considerate a less invasive surgery and revolutionary in oral surgery. The pneumatization of the maxillary sinus tend to difficult the rehabilitation of the posterior part of the maxilla on edentulous patients, making it hard to correct the condition without bone graft. Surgeries of sinus lift, therefore has its risk of punctuating the Schneider membrane, using rotatory instruments can damage the membrane but due to the selectivity for mineralized tissues performed with the piezoelectric technique suits very well for the patient and also the dentist safety. Excessive gingival display during smile can result in compromised esthetics. This study aims to report a case of excessive gingival display with multiple etiologies treated by means of modified lip repositioning technique associated with esthetic crown lengthening. A 23-year-old female patient, with 5-mm gingival display during smile caused by altered passive eruption and hypermobility of the upper lip, underwent the modified lip repositioning technique associated with gingivectomy followed by flap elevation and ostectomy/osteoplasty. Seven months after the second procedure, the patient had her esthetic complaint solved appearing stable in the observation period. The modified lip repositioning technique is an effective procedure employed to reduce gingival display and when associated with esthetic clinical crown lengthening, can appropriately treat cases of gummy smile. - Abstract
The overall objective of this study was to study the Temporomandibular Disorders and Orofacial Pain and its associated factors among adults in the city of Maringá - PR. The specific objectives were: To describe the sample according to socio-demographic factors; estimate the prevalence of TMD and Orofacial Pain; know the factors associated with TMD and Orofacial Pain; describe the association between TMD and Orofacial Pain with psychosocial aspects and its relation to the quality of sleep. The method used was a cross-sectional population-based study, the population of the city of Maringá, Paraná. The information referred to were obtained through structured interviews conducted in households of the population studied, conducted from August 2011 to March 2014. This study was approved by the Ethics Committee of the Faculty Inga with the CAAE 0071.0.362.000-11 and the Opinion number 0071/11. The study population was comprised of people aged 20 to 65, Public Health Service users in Maringá - PR. The study population consisted of 568 people aged 20 to 65, Public Health Service users in Maringá - PR. For data collection was used a structured, standardized and pre tested, containing demographic, socioeconomic, behavioral, psychosocial, and sleep-related. Axis II of the application of the RDC / TMD (socioeconomic and psychosocial factors and positioning and joint movement); Axis I application of the RDC / TMD (issue 1-7) - (positioning factors and joint movement); Sleep Assessment Questionnaire (SAQ). With respect to the independent variables, demographic variables were analyzed: sex, age, color / race, ethnicity and marital status; socioeconomic variables: education, occupational status and family income; psychosocial variables; of health status and morbidity conditions. After the selection of patients, a lists of possession with containing the same address, structured interviews were conducted, following the sequence: a) Reading and Consent Form signature by the patient; b) Application of Axis II of the RDC/ TMD (socioeconomic and psychosocial factors and positioning and joint movement); c) Application of Axis I RDC/ TMD - (positioning factors and joint movement); d) Sleep Assessment Questionnaire (SAQ). After the questionnaires, these were tabulated in Microsoft Excel spreadsheets Package 2010. The results show that 77.5% of respondents had sleep disorders. Regarding quality of life, psychological and social relationships domains were those with the highest quality of life while the one with lower quality of life was the self-control evaluation of quality of life. This study is the result of an observational study of temporomandibular dysfunction and facial pain (TMD and OFP) in the city of Maringa, State of Parana in 2011 2014. The information was obtained through interviews structured for questionnaire Axis I and II of the RDC /TMD, conducted with 500 people between 20 and 65 years, users of the Public Health System. This research aimed to observe asymptomatic patients for TMD, who had signs of dysfunction, associated factors as the etiology, prevalence and facial pain. In the sample of 500 people, all subjects responded that they did not have pain in the face, in maxillary, her temple (head), in front of the ear or in the ear last month, of which, 59.2% (n = 296) of people were female. Although all patients didn't presented pain, 57.0% (n = 285) had sleep disorder, and 23.8% (n = 119) with disc displacement reduction in the right or left, 28.0% (n = 140) disk displacement with reduction in pain, 24.8% (n = 124) left lateral deviation / right (uncorrected). The result of this study on the correlation of the absence of symptoms in the presence of TMD and OFP signs, demonstrates the need for clinical examination for diagnosing TMD, since 100% had no pain symptoms, but had TMD. 18. Analysis of Bond Strength by Pull Out Test on Fiber Glass Posts Cemented in Different Lengths.

Background: The aim of this study was to evaluate, by means of pull-out test, the bond strength of fiberglass posts when cemented with different lengths in endodontically treated teeth. Materials and Methods: Sixty single-rooted bovine roots were cut in the cementoenamel junction with 21 mm length. They were endodontically treated and randomly divided into three groups (n = 20). Group 1 - Preparation of ⅞of the remaining roots; Group 2 - Preparation of ½ of the remaining roots and Group 3 - Preparation of ¼ of remaining roots. For all groups it were used posts n = 3 (Exacto, Angelus, Brazil), and cemented with self-etching resin cement (RelyXU200). After cementing posts, the samples were thermocycled (10.000 cycles/5°C and 55°C). The pull-out test was performed on a universal testing machine (EMIC - DL500) and the values obtained were statistically analyzed by analysis of variance (one-factor ANOVA) and multiple comparison test of Tukey, with level of significance of 5%. Results: The mean values ± standard deviation in Newtons (N) were: Group 1 = 120.5 (±42.8) A, Group 2 = 103.1 (±31.2) AB, Group 3 = 41.2 (±22.4) C, P < 0.005. Conclusion: The preparation of ½ of remaining root appears to be a viable alternative when ⅞of the preparation of the remaining root is not possible, but more results are needed for clinical validation. In this manuscript, we present a rare case report of giant complex odontoma in the maxillary sinus, where the applied therapy included complete excision of the lesion with a conservative approach. Odontomas are also called benign growth abnormalities or hamartomas. They represent a more common type of odontogenic tumor and are related to various disorders such as bad dental placements, expansion, increased volumetric bone, and no eruption of permanent teeth. Usually they have an asymptomatic evolutionary course. The etiologic factors, although obscure, are related to local trauma, infection, and genetic factor. The structural composition of an odontoma consists of mature dental tissues. Odontomas can be differentiated according to their anatomical presentations: Compound odontoma-clusters of several denticles and complex odontoma-well defined tumefaction mass. Os objetivos deste trabalho foram avaliar a distribuição de tensões (DT), resistência à fratura (RF), resistência adesiva (RA) e seus respectivos padrões de fratura de raízes bovinas fragilizadas, reabilitadas com pinos personalizados com diferentes materiais resinosos. Além disso, objetivou-se avaliar a profundidade de polimerização dos materiais resinosos por meio do método de microdureza Knoop (MD). Oito grupos experimentais foram criados (n=10 para RF e RA; n=5 para MD), tendo como fator em estudo: 1. o tipo de pino utilizado (pino de fibra de vidro industrializado reembasado (I) e pino personalizado (P)); 2. o material de reembasamento apenas para os pinos industrializados (compósito experimental (e) e resina composta (r)); 3. o material utilizado para confecção apenas dos pinos personalizados (compósito experimental (e) e resina bulk fill (b)); e 4. a quantidade de dentina remanescente (fragilização radicular por desgaste interno nos comprimentos de 5 mm ou 10 mm). Cento e quarenta raízes bovinas com dimensões semelhantes foram selecionadas e restauradas de acordo com o grupo ao qual pertenciam. Para RF, ligamento periodontal e osso alveolar foram simulados e os espécimes posicionados em máquina Instron 5965, em um ângulo de 135° em relação ao longo eixo do dente de modo que recebessem carregamento à velocidade constante de 0,5mm/min em compressão até a fratura. Em seguida, o padrão de fratura foi classificado em: F1- coroa; F2- raiz; F3- coroa/raiz; F4- fratura com deslocamento do conjunto coroa/pino. Para RA, os espécimes foram seccionados em fatias 1 mm perpendicularmente ao seu longo eixo. Cada espécime foi posicionado em máquina de ensaio de modo que recebessem a carga no sentido apical-coronal, à velocidade constante de 0,5mm/min, empurrando o pino através do maior lado da amostra até a fratura. O padrão de fratura foi classificado em: F1 - coesiva do pino e/ou material de reembasamento; F2 - coesiva da dentina; F3 - adesiva pino/material de reembasmento; F4 - adesiva material de reembasamento/cimento e F5 – mista. Para MD, os espécimes foram seccionados paralelamente ao seu longo eixo. Os testes foram realizados com um penetrador Knoop sob uma carga estática de 50 gramas por 10 segundos. MD foi calculada como o valor médio para cada terço. E por fim, o método de elementos finitos foi empregado para analisar a DT. Modelos virtuais 3D representativos de cada grupo experimental foram criados e analisados sob critério de tensões equivalentes de von Mises, simulando carregamento de 100N em um ângulo de 45o com o longo eixo. O compósito experimental demostrou ser uma alternativa para a reabilitação de raízes fragilizadas quando utilizado para criar pinos personalizados ou quando utilizado como material de reembasamento de pinos, com valores de resistência à fratura semelhantes à técnica tradicional porém, com distribuição de tensões mais favoráveis. Além disso, apresentou excelente resistência adesiva à estrutura dentinária, sendo comparável à resina composta convencional e à resina bulk fill. Já a resina bulk fill resultou em baixos valores de resistência à fratura associado ao deslocamento do conjunto pino/coroa protética. Valores de microdureza inferiores foram observados no terço apical para a bulk fill. The objectives of this work were to evaluate the stress distribution (SD), fracture resistance (RF), adhesive strength (AS) and their respective fracture patterns of weekeaned bovine roots, rehabilitated with customized pins with different resinous materials. In addition, the objective of this study was to evaluate the polymerization depth of the resinous materials by Knoop microhardness method (MH). Eight experimental groups were established (n = 10 for FR and AS; n = 5 for MH), with the factor under study: 1. The post type (industrialized glass fiber post relined (I) and personalized post (P), 2. the relining material only for industrialized glass fiber post (experimental composite (e) and composite resin (R)); 3. the material used only for personalized post (experimental composite (e) and bulk fill resin (b), and 4. the amount of remaining dentin (root embrittlement inner wear, in lengths of 5 mm or 10 mm). One hundred and forty bovine roots of similar size were selected and restored according to the group to which they belonged. For FR, Pperiodontal and alveolar bone were simulated and the specimens were placed in an Instron 5965 machine at a 135 ° angle to the long axis and loaded at a constant velocity of 0.5 mm/min in compression until the fracture. The fracture pattern was classified as: F1- crown; F2- root; F3- crown/root; F4- fracture with displacement of the crown/post. For AS, the specimens were sectioned in slices of 1 mm perpendicular to their long axis. Each specimen was placed in a test machine to received the load in the apical-coronal direction, at a constant velocity of 0.5mm / min, pushing the pin through the largest side of the sample until fracture. The fracture pattern was classified as: F1- cohesive of the pin and/or reline material; F2- cohesive of dentin; F3- adhesive of pin/reline material; F4 - Adhesive bonding cement/ eline material and F5 - mixed. For MH, the specimens were sectioned parallel to their long axis. The tests were performed with a Knoop penetrator under a static charge of 50 grams per 10 seconds. MH was calculated as the average value for each third. Finally, the finite element method was used to analyze the DT. 3D virtual models representative of each experimental group were created and analyzed under von Mises equivalent stresses criterion, simulating loading of 100N at a 45o angle with the long axis. The experimental composite proved to be an alternative for the rehabilitation of weakened roots when used to create personalized posts or when used as a relining material, with FR values similar to the traditional technique, but with more favorable stress distributions. Moreover, it presented excellent adhesive strength to the dentin structure, being comparable to conventional composite resin and bulk fill resin. The bulk fill resin resulted in low values of fracture resistance associated with the fracture and displacement of post/crown.
