High static field MR scanners can produce human tissue images of astounding clarity, but rely on high frequency electromagnetic radiation that generates complicated in-tissue field patterns that are patient-specific and potentially harmful. Many such scanners use parallel transmitters to better control field patterns, but then adjust the transmitters based on general guidelines rather than optimizing for the specific patient, mostly because computing patient-specific fields was presumed far too slow. It was recently demonstrated that the combination of fast low-resolution tissue mapping and fast voxel-based field simulation can be used to perform a patient-specific MR safety check in minutes. However, the field simulation required several of those minutes, making it too slow to perform the dozens of simulations that would be needed for patient-specific optimization. In this paper we describe a compressed-perturbation-matrix technique that nearly eliminates the computational cost of including complex coils (or coils and shields) in voxel-based field simulation of tissue, thereby reducing simulation time from minutes to seconds. The approach is demonstrated on a wide variety of head+coil and head+coil+shield configurations, using the implementation in MARIE 2.0, the latest version of the open-source MR field simulator MARIE.

Diffusion-weighted magnetic resonance imaging (DW-MRI) has evolved to provide increasingly sophisticated investigations of the human brain's structural connectome in vivo. Restriction spectrum imaging (RSI) is a method that reconstructs the orientation distribution of diffusion within tissues over a range of length scales. In its original formulation, RSI represented the signal as consisting of a spectrum of Gaussian diffusion response functions. Recent technological advances have enabled the use of ultra-high b-values on human MRI scanners, providing higher sensitivity to intracellular water diffusion in the living human brain. To capture the complex diffusion time dependence of the signal within restricted water compartments, we expand upon the RSI approach to represent restricted water compartments with non-Gaussian response functions, in an extended analysis framework called linear multi-scale modeling (LMM). The LMM approach is designed to resolve length scale and orientation-specific information with greater specificity to tissue microstructure in the restricted and hindered compartments, while retaining the advantages of the RSI approach in its implementation as a linear inverse problem. Using multi-shell, multi-diffusion time DW-MRI data acquired with a state-of-the-art 3 T MRI scanner equipped with 300 mT/m gradients, we demonstrate the ability of the LMM approach to distinguish different anatomical structures in the human brain and the potential to advance mapping of the human connectome through joint estimation of the fiber orientation distributions and compartment size characteristics.

PURPOSE: We develop and test a parallel transmit (pTx) pulse design framework to mitigate transmit field inhomogeneity with control of local specific absorption rate (SAR) in 2D rapid acquisition with relaxation enhancement (RARE) imaging at 7T.METHODS: We design large flip angle RF pulses with explicit local SAR constraints by numerical simulation of the Bloch equations. Parallel computation and analytical expressions for the Jacobian and the Hessian matrices are employed to reduce pulse design time. The refocusing-excitation "spokes" pulse pairs are designed to satisfy the Carr-Purcell-Meiboom-Gill (CPMG) condition using a combined magnitude least squares-least squares approach.RESULTS: In a simulated dataset, the proposed approach reduced peak local SAR by up to 56% for the same level of refocusing uniformity error and reduced refocusing uniformity error by up to 59% (from 32% to 7%) for the same level of peak local SAR compared to the circularly polarized birdcage mode of the pTx array. Using explicit local SAR constraints also reduced peak local SAR by up to 46% compared to an RF peak power constrained design. The excitation and refocusing uniformity error were reduced from 20%-33% to 4%-6% in single slice phantom experiments. Phantom experiments demonstrated good agreement between the simulated excitation and refocusing uniformity profiles and experimental image shading.CONCLUSION: PTx-designed excitation and refocusing CPMG pulse pairs can mitigate transmit field inhomogeneity in the 2D RARE sequence. Moreover, local SAR can be decreased significantly using pTx, potentially leading to better slice coverage, enabling larger flip angles or faster imaging.

Powerful MRI gradient systems can surpass the International Electrotechnical Commission (IEC) 60601-2-33 limit for cardiac stimulation (CS), which was determined by simple electromagnetic simulations and electrode stimulation experiments. To extend the coverage of brain coil arrays to the neck and cervical-spine region to enable combined head and neck imaging at 7 Tesla (T) ultra-high field MRI.The coil array structures of a 64-channel receive coil and a 16-channel transmit coil were merged into one anatomically shaped close-fitting housing. Transmit characteristics were evaluated in a BIn the brain, the SNR matches the commercially available 32-channel brain array and showed improvements in accelerated imaging capabilities. More importantly, the constructed coil array improved the SNR in the face area, neck area, and cervical spine by a factor of 1.5, 3.4, and 5.2, respectively, in regions not covered by 32-channel brain arrays at 7 T. The interelement coupling of the 16-channel transmit coil ranged from -14 to -44 dB (mean = -19 dB, adjacent elements-18 dB). The parallel 16-channel transmit coil greatly facilitates BThis new head-neck array coil is the first demonstration of a device of this nature used for combined full-brain, head-neck, and cervical-spine imaging at 7 T. The array coil is well suited to provide large FOV images, which potentially improves ultrahigh field neuroimaging applications for clinical settings.

The quality of cervical spinal cord images can be improved by the use of tailored radiofrequency coil solutions for ultra-high field imaging; however, very few commercial and research 7 Tesla radiofrequency coils currently exist for the spinal cord, and in particular those with parallel transmit capabilities. This work presents the design, testing and validation of a pTx/Rx coil for the human neck and cervical/upper-thoracic spinal cord. The pTx portion is composed of 8 dipoles to ensure high homogeneity over this large region of the spinal cord. The Rx portion is made of 20 semi-adaptable overlapping loops to produce high Signal-to-noise ratio (SNR) across the patient population. The coil housing is designed to facilitate patient positioning and comfort, while being tight fitting to ensure high sensitivity. We demonstrate RF shimming capabilities to optimize B1+uniformity, power efficiency and/or specific absorption rate (SAR) efficiency. B1+homogeneity, SNR and g-factor was evaluated in adult volunteers and demonstrated excellent performance from the occipital lobe down to the T4-T5 level. We compared the proposed coil with two state-of-the-art head and head/neck coils, confirming its superiority in the cervical and upper-thoracic regions of the spinal cord. This coil solution therefore provides a convincing platform for producing the high image quality necessary for clinical and research scanning of the upper spinal cord.

Purpose: Peripheral nerve stimulation (PNS) modeling has a potential role in designing and operating MRI gradient coils but requires computationally demanding simulations of electromagnetic fields and neural responses. We demonstrate compression of an electromagnetic and neurodynamic model into a single versatile PNS matrix (P-matrix) defined on an intermediary Huygens’ surface to allow fast PNS characterization of arbitrary coil geometries and body positions. Methods: The Huygens’ surface approach divides PNS prediction into an extensive precomputation phase of the electromagnetic and neurodynamic responses, which is independent of coil geometry and patient position, and a fast coil-specific linear projection step connecting this information to a specific coil geometry. We validate the Huygens’ approach by performing PNS characterizations for 21 body and head gradients and comparing them with full electromagnetic-neurodynamic modeling. We demonstrate the value of Huygens’ surface-based PNS modeling by characterizing PNS-optimized coil windings for a wide range of patient positions and poses in two body models. Results: The PNS prediction using the Huygens’ P-matrix takes less than a minute (instead of hours to days) without compromising numerical accuracy (error ≤ 0.1%) compared to the full simulation. Using this tool, we demonstrate that coils optimized for PNS at the brain landmark using a male model can also improve PNS for other imaging applications (cardiac, abdominal, pelvic, and knee imaging) in both male and female models. Conclusion: Representing PNS information on a Huygens’ surface extended the approach’s ability to assess PNS across body positions and models and test the robustness of PNS optimization in gradient design.

BackgroundIn patients with severe traumatic brain injury (TBI), coma is associated with impaired subcortical arousal mechanisms. However, it is unknown which nuclei involved in arousal (arousal nuclei) are implicated in coma pathogenesis and are compatible with coma recovery.MethodsWe mapped an atlas of arousal nuclei in the brainstem, thalamus, hypothalamus, and basal forebrain onto 3 tesla susceptibility-weighted images (SWI) in 12 patients with acute severe TBI who presented in coma and recovered consciousness within 6 months. We assessed the spatial distribution and volume of SWI microbleeds and evaluated the association of microbleed volume with the duration of unresponsiveness and functional recovery at 6 months.ResultsThere was no single arousal nucleus affected by microbleeds in all patients. Rather, multiple combinations of microbleeds in brainstem, thalamic, and hypothalamic arousal nuclei were associated with coma and were compatible with recovery of consciousness. Microbleeds were frequently detected in the midbrain (100%), thalamus (83%), and pons (75%). Within the brainstem, the microbleed incidence was largest within the mesopontine tegmentum (e.g., pedunculotegmental nucleus, mesencephalic reticular formation) and ventral midbrain (e.g., substantia nigra, ventral tegmental area). Brainstem arousal nuclei were partially affected by microbleeds, with microbleed volume not exceeding 35% of brainstem nucleus volume on average. Compared to microbleed volume within nonarousal brainstem regions, the microbleed volume within arousal brainstem nuclei accounted for a larger proportion of variance in the duration of unresponsiveness and 6-month Glasgow Outcome Scale–Extended scores.ConclusionThese results suggest resilience of arousal mechanisms in the human brain after severe TBI.

PURPOSE Three 64-channel cardiac coils with different detector array configurations were designed and constructed to evaluate acceleration capabilities in simultaneous multislice (SMS) imaging for 3T cardiac MRI. METHODS Three 64-channel coil array configurations obtained from a simulation-guided design approach were constructed and systematically evaluated regarding their encoding capabilities for accelerated SMS cardiac acquisitions at 3T. Array configuration AUni-sized consists of uniformly distributed equally sized loops in an overlapped arrangement, BGapped uses a gapped array design with symmetrically distributed equally sized loops, and CDense has non-uniform loop density and size, where smaller elements were centered over the heart and larger elements were placed surrounding the target region. To isolate the anatomic variation from differences in the coil configurations, all three array coils were built with identical semi-adjustable housing segments. The arrays' performance was compared using bench-level measurements and imaging performance tests, including signal-to-noise ratio (SNR) maps, array element noise correlation, and SMS acceleration capabilities. Additionally, all cardiac array coils were evaluated on a healthy volunteer. RESULTS The array configuration CDense with the non-uniformly distributed loop density showed the best overall cardiac imaging performance in both SNR and SMS encoding power, when compared to the other constructed arrays. The diffusion weighted cardiac acquisitions on a healthy volunteer support the favorable accelerated SNR performance of this array configuration. CONCLUSION Our results indicate that optimized highly parallel cardiac arrays, such as the 64-channel coil with a non-uniform loop size and density improve highly accelerated SMS cardiac MRI in comparison to symmetrically distributed loop array designs.

Peripheral Nerve Stimulation (PNS) limits the acquisition rate of Magnetic Resonance Imaging data for fast sequences employing powerful gradient systems. The PNS characteristics are currently assessed after the coil design phase in experimental stimulation studies using constructed coil prototypes. This makes it difficult to find design modifications that can reduce PNS. Here, we demonstrate a direct approach for incorporation of PNS effects into the coil optimization process. Knowledge about the interactions between the applied magnetic fields and peripheral nerves allows the optimizer to identify coil solutions that minimize PNS while satisfying the traditional engineering constraints. We compare the simulated thresholds of PNS-optimized body and head gradients to conventional designs, and find an up to 2-fold reduction in PNS propensity with moderate penalties in coil inductance and field linearity, potentially doubling the image encoding performance that can be safely used in humans. The same framework may be useful in designing and operating magneto- and electro-stimulation devices.

Access to scanners for magnetic resonance imaging (MRI) is typically limited by cost and by infrastructure requirements. Here, we report the design and testing of a portable prototype scanner for brain MRI that uses a compact and lightweight permanent rare-earth magnet with a built-in readout field gradient. The 122-kg low-field (80 mT) magnet has a Halbach cylinder design that results in a minimal stray field and requires neither cryogenics nor external power. The built-in magnetic field gradient reduces the reliance on high-power gradient drivers, lowering the overall requirements for power and cooling, and reducing acoustic noise. Imperfections in the encoding fields are mitigated with a generalized iterative image reconstruction technique that leverages previous characterization of the field patterns. In healthy adult volunteers, the scanner can generate T1-weighted, T2-weighted and proton density-weighted brain images with a spatial resolution of 2.2 × 1.3 × 6.8 mm3. Future versions of the scanner could improve the accessibility of brain MRI at the point of care, particularly for critically ill patients. A portable prototype scanner for brain MRI that uses a compact and lightweight permanent rare-earth magnet with a built-in readout field gradient generates clinically relevant images of the brain, as shown in adult volunteers.

Metabolic imaging of the human brain by in-vivo magnetic resonance spectroscopic imaging (MRSI) can non-invasively probe neurochemistry in healthy and disease conditions. MRSI at ultra-high field (≥ 7 T) provides increased sensitivity for fast high-resolution metabolic imaging, but comes with technical challenges due to non-uniform B0 field. Here, we show that an integrated RF-receive/B0-shim (AC/DC) array coil can be used to mitigate 7 T B0 inhomogeneity, which improves spectral quality and metabolite quantification over a whole-brain slab. Our results from simulations, phantoms, healthy and brain tumor human subjects indicate improvements of global B0 homogeneity by 55%, narrower spectral linewidth by 29%, higher signal-to-noise ratio by 31%, more precise metabolite quantification by 22%, and an increase by 21% of the brain volume that can be reliably analyzed. AC/DC shimming provide the highest correlation (R2 = 0.98, P = 0.001) with ground-truth values for metabolite concentration. Clinical translation of AC/DC and MRSI is demonstrated in a patient with mutant-IDH1 glioma where it enables imaging of D-2-hydroxyglutarate oncometabolite with a 2.8-fold increase in contrast-to-noise ratio at higher resolution and more brain coverage compared to previous 7 T studies. Hence, AC/DC technology may help ultra-high field MRSI become more feasible to take advantage of higher signal/contrast-to-noise in clinical applications.

Purpose Cardiac stimulation (CS) limits to gradient coil switching speed are difficult to measure in humans; instead, current regulatory guidelines (IEC 60601-2-33) are based on animal experiments and electric field-to-dB/dt conversion factors computed for a simple, homogeneous body model. We propose improvement to this methodology by using more detailed CS modeling based on realistic body models and electrophysiological models of excitable cardiac fibers. Methods We compute electric fields induced by a solenoid, coplanar loops, and a commercial gradient coil in two human body models and a canine model. The canine simulations mimic previously published experiments. We generate realistic fiber topologies for the cardiac Purkinje and ventricular muscle fiber networks using rule-based algorithms, and evaluate CS thresholds using validated electrodynamic models of these fibers. Results We were able to reproduce the average measured canine CS thresholds within 5%. In all simulations, the Purkinje fibers were stimulated before the ventricular fibers, and therefore set the effective CS threshold. For the investigated gradient coil, simulated CS thresholds for the x-, y-, and z-axis were at least one order of magnitude greater than the International Electrotechnical Commission limit. Conclusion We demonstrate an approach to simulate gradient-induced CS using a combination of electromagnetic and electrophysiological modeling. Pending additional validation, these simulations could guide the assessment of CS limits to MRI gradient coil switching speed. Such an approach may lead to less conservative, but still safe, operation limits, enabling the use of the maximum gradient amplitude versus slew rate parameter space of recent, powerful gradient systems.

Purpose We propose a fast, patient-specific workflow for on-line specific absorption rate (SAR) supervision. An individualized electromagnetic model is created while the subject is on the table, followed by rapid SAR estimates for that individual. Our goal is an improved correspondence between the patient and model, reducing reliance on general anatomical body models. Methods A 3D fat-water 3T acquisition (~2 minutes) is automatically segmented using a computer vision algorithm (~1 minute) into what we found to be the most important electromagnetic tissue classes: air, bone, fat, and soft tissues. We then compute the individual's EM field exposure and global and local SAR matrices using a fast electromagnetic integral equation solver. We assess the approach in 10 volunteers and compare to the SAR seen in a standard generic body model (Duke). Results The on-the-table workflow averaged 7'44″. Simulation of the simplified Duke models confirmed that only air, bone, fat, and soft tissue classes are needed to estimate global and local SAR with an error of 6.7% and 2.7%, respectively, compared to the full model. In contrast, our volunteers showed a 16.0% and 20.3% population variability in global and local SAR, respectively, which was mostly underestimated by the Duke model. Conclusion Timely construction and deployment of a patient-specific model is computationally feasible. The benefit of resolving the population heterogeneity compared favorably to the modest modeling error incurred. This suggests that individualized SAR estimates can improve electromagnetic safety in MRI and possibly reduce conservative safety margins that account for patient-model mismatch, especially in non-standard patients.

© 2020 Elsevier Ltd Introduction: Before using blood-oxygen-level-dependent magnetic resonance imaging (BOLD MRI) during maternal hyperoxia as a method to detect individual placental dysfunction, it is necessary to understand spatiotemporal variations that represent normal placental function. We investigated the effect of maternal position and Braxton-Hicks contractions on estimates obtained from BOLD MRI of the placenta during maternal hyperoxia. Methods: For 24 uncomplicated singleton pregnancies (gestational age 27–36 weeks), two separate BOLD MRI datasets were acquired, one in the supine and one in the left lateral maternal position. The maternal oxygenation was adjusted as 5 min of room air (21% O2), followed by 5 min of 100% FiO2. After datasets were corrected for signal non-uniformities and motion, global and regional BOLD signal changes in R2* and voxel-wise Time-To-Plateau (TTP) in the placenta were measured. The overall placental and uterine volume changes were determined across time to detect contractions. Results: In mothers without contractions, increases in global placental R2* in the supine position were larger compared to the left lateral position with maternal hyperoxia. Maternal position did not alter global TTP but did result in regional changes in TTP. 57% of the subjects had Braxton-Hicks contractions and 58% of these had global placental R2* decreases during the contraction. Conclusion: Both maternal position and Braxton-Hicks contractions significantly affect global and regional changes in placental R2* and regional TTP. This suggests that both factors must be taken into account in analyses when comparing placental BOLD signals over time within and between individuals.

Purpose Multichannel transcranial magnetic stimulation (TMS)1 is an emerging technology that allows multiple sites to be stimulated simultaneously or sequentially under electronic control without movement of the coils. A multichannel TMS/MRI head coil array for 3 Tesla is currently under development to mitigate challenges of concurrent TMS/fMRI as well as enable potential new applications. The influence of the multichannel TMS system on the MR image quality and safety must be carefully investigated. Methods A standard birdcage volume coil for 3 Tesla systems was simulated using a commercial numerical electromagnetic solver. Two setups, consisting of 1) a MR-compatible TMS coil, and 2) a 3-axis TMS coil array, were simulated to quantify changes in the transmit field B 1 + and the SAR. A realistically shaped homogeneous head model was used in the computations. Results The stimulation coils produced enhancements and attenuations on the transmit field with effects greater than 5% up to 2.4 cm and 3.3 cm under the scalp for the MR-compatible TMS coil and 3-axis TMS coil array, respectively. The 10 g-SAR distribution did not change significantly in either of the cases; however, the nominal SAR maximum locus was shifted between existing hot spots. Conclusion The simulated B 1 + variations found near the TMS coils indicate the possibility of inducing sequence-dependent image artefacts predominatly limited to the vicinity of the coil(s). However, we conclude that neither the MR-compatible commercial TMS coil nor the 3-axis TMS coil array siginificantly elevate SAR in the head or neck beyond accepted safety limits.

PURPOSE: To introduce a computationally efficient approach to optimizing the data acquisition parameters of MR Fingerprinting experiments with the Cramér-Rao bound. METHODS: This paper presents a new approach to the optimal experimental design (OED) problem for MR Fingerprinting, which leverages an early observation that the optimized data acquisition parameters of MR Fingerprinting experiments are highly structured. Specifically, the proposed approach captures the desired structure by representing the sequences of data acquisition parameters with a special class of piecewise polynomials, i.e., B-splines. This incorporates low-dimensional spline subspace constraints into the OED problem, which significantly reduces the search space of the problem, thereby improving the computational efficiency. With the rich B-spline representations, the proposed approach also allows for incorporating prior knowledge on the structure of different acquisition parameters, which facilitates the experimental design. RESULTS: The effectiveness of the proposed approach was evaluated using numerical simulations, phantom experiments, and in vivo experiments. The proposed approach achieves a two-order-of-magnitude improvement of the computational efficiency over the state-of-the-art approaches, while providing a comparable signal-to-noise ratio efficiency benefit. It enables an optimal experimental design problem for MR Fingerprinting with a typical acquisition length to be solved in approximately one minute. CONCLUSIONS: The proposed approach significantly improves the computational efficiency of the optimal experimental design for MR Fingerprinting, which enhances its practical utility for a variety of quantitative MRI applications.

Accurate spatial alignment of MRI data acquired across multiple contrasts in the same subject is often crucial for data analysis and interpretation, but can be challenging in the presence of geometric distortions that differ between acquisitions. It is well known that single-shot echo-planar imaging (EPI) acquisitions suffer from distortion in the phase-encoding direction due to B0 field inhomogeneities arising from tissue magnetic susceptibility differences and other sources, however there can be distortion in other encoding directions as well in the presence of strong field homogeneities. High-resolution ultrahigh-field MRI typically uses low bandwidth in the slice-encoding direction to acquire thin slices and, when combined with the pronounced B0 inhomogeneities, is prone to an additional geometric distortion in the slice direction as well. Here we demonstrate a presence of this slice distortion in high-resolution 7T EPI acquired with a novel pulse sequence allowing for the reversal of the slice-encoding gradient polarity that enables the acquisition of pairs of images with equal magnitudes of distortion in the slice direction but with opposing polarities. We also show that the slice-direction distortion can be corrected using gradient reversal-based method applying the same software used for conventional corrections of phase-encoding direction distortion.

PurposeTo develop an efficient acquisition technique for distortion-free diffusion MRI and diffusion-relaxometry.MethodsA new ACcelerated Echo-train shifted Echo-Planar Time-resolved Imaging (ACE-EPTI) technique is developed to achieve high-SNR, distortion- and blurring-free diffusion and diffusion-relaxometry imaging. ACE-EPTI employs a newly designed variable density spatiotemporal encoding with self-navigation capability, that allows submillimeter in-plane resolution using only 3-shot. Moreover, an echo-train-shifted acquisition is developed to achieve minimal TE, together with an SNR-optimal readout length, leading to ~30% improvement in SNR efficiency over single-shot EPI. To recover the highly accelerated data with high image quality, a tailored subspace image reconstruction framework is developed, that corrects for odd/even-echo phase difference, shot-to-shot phase variation, and the B0 field changes due to field drift and eddy currents across different dynamics. After the phase-corrected subspace reconstruction, artifacts-free high-SNR diffusion images at multiple TEs are obtained with varying T2* weighting.ResultsSimulation, phantom and in-vivo experiments were performed, which validated the 3-shot spatiotemporal encoding provides accurate reconstruction at submillimeter resolution. The use of echo-train shifting and optimized readout length improves the SNR-efficiency by 27-36% over single-shot EPI. The reconstructed multi-TE diffusion images were demonstrated to be free from distortion (susceptibility and eddy currents) and phase/field variation induced artifacts. These improvements of ACE-EPTI enable improved diffusion tensor imaging and rich multi-TE information for diffusion-relaxometry analysis.ConclusionACE-EPTI was demonstrated to be an efficient and powerful technique for high-resolution diffusion imaging and diffusion-relaxometry, which provides high SNR, distortion- and blurring-free, and time-resolved multi-echo images by a fast 3-shot acquisition.

PURPOSE To demonstrate a navigator/tracking-free retrospective motion estimation technique that facilitates clinically acceptable reconstruction time. METHODS Scout accelerated motion estimation and reduction (SAMER) uses a single 3-5 s, low-resolution scout scan and a novel sequence reordering to independently determine motion states by minimizing the data-consistency error in a SENSE plus motion forward model. This eliminates time-consuming alternating optimization as no updates to the imaging volume are required during the motion estimation. The SAMER approach was assessed quantitatively through extensive simulation and was evaluated in vivo across multiple motion scenarios and clinical imaging contrasts. Finally, SAMER was synergistically combined with advanced encoding (Wave-CAIPI) to facilitate rapid motion-free imaging. RESULTS The highly accelerated scout provided sufficient information to achieve accurate motion trajectory estimation (accuracy ~0.2 mm or degrees). The novel sequence reordering improved the stability of the motion parameter estimation and image reconstruction while preserving the clinical imaging contrast. Clinically acceptable computation times for the motion estimation (~4 s/shot) are demonstrated through a fully separable (non-alternating) motion search across the shots. Substantial artifact reduction was demonstrated in vivo as well as corresponding improvement in the quantitative error metric. Finally, the extension of SAMER to Wave-encoding enabled rapid high-quality imaging at up to R = 9-fold acceleration. CONCLUSION SAMER significantly improved the computational scalability for retrospective motion estimation and correction.

PURPOSE: This study investigates whether two-channel radiofrequency (RF) shimming can improve imaging without increasing specific absorption rate (SAR) for fetal MRI at 3T. METHODS: Transmit field [Formula: see text] average and variation in the fetus was simulated in seven numerical pregnant body models. Safety was quantified by maternal and fetal peak local SAR and fetal average SAR. The shim parameter space was divided into improved [Formula: see text] (magnitude and homogeneity) and improved SAR regions, and an overlap where RF shimming improved both classes of metrics compared with birdcage mode was assessed. Additionally, the effect of fetal position, tissue detail, and dielectric properties on transmit field and SAR was studied. RESULTS: A region of subject-specific RF shim parameter space improving both [Formula: see text] and SAR metrics was found for five of the seven models. Optimizing only [Formula: see text] metrics improved [Formula: see text] efficiency across models by 15% on average and 28% for the best-case model. [Formula: see text] variation improved by 26% on average and 49% for the best case. However, for these shim settings, fetal SAR increased by up to 106%. The overlap region, where both [Formula: see text] and SAR metrics improve, showed an average [Formula: see text] efficiency improvement of 6% on average across models and 19% for the best-case model. [Formula: see text] variation improved by 13% on average and 40% for the best case. RFS could also decrease maternal/fetal SAR by up to 49%/58%. CONCLUSION: RF shimming can improve imaging compared with birdcage mode without increasing fetal and maternal SAR when a patient-specific SAR model is incorporated into the shimming procedure.

Purpose: B1+ and T1 corrections and dynamic multi-coil shimming approaches were proposed to improve the fidelity of high isotropic resolution Generalized slice dithered enhanced resolution (gSlider) diffusion imaging. Methods: An extended reconstruction incorporating B1+ inhomogeneity and T1 recovery information was developed to mitigate slab-boundary artifacts in short-TR gSlider acquisitions. Slab-by-slab dynamic B0 shimming using a multi-coil integrated {\Delta}B0/Rx shim-array, and high in-plane acceleration (Rinplane=4) achieved with virtual-coil GRAPPA were also incorporated into a 1 mm isotropic resolution gSlider acquisition/reconstruction framework to achieve an 8-11 fold reduction in geometric distortion compared to single-shot EPI. Results: The slab-boundary artifacts were alleviated by the proposed B1+ and T1 corrections compared to the standard gSlider reconstruction pipeline for short-TR acquisitions. Dynamic shimming provided >50% reduction in geometric distortion compared to conventional global 2nd order shimming. 1 mm isotropic resolution diffusion data show that the typically problematic temporal and frontal lobes of the brain can be imaged with high geometric fidelity using dynamic shimming. Conclusions: The proposed B1+ and T1 corrections and local-field control substantially improved the fidelity of high isotropic resolution diffusion imaging, with reduced slab-boundary artifacts and geometric distortion compared to conventional gSlider acquisition and reconstruction. This enabled high-fidelity whole-brain 1 mm isotropic diffusion imaging with 64 diffusion-directions in 20 minutes using a 3T clinical scanner.

Objective To evaluate alterations in apparent axon diameter and axon density obtained by high‐gradient diffusion MRI in the corpus callosum of MS patients and the relationship of these advanced diffusion MRI metrics to neurologic disability and cognitive impairment in MS. Methods Thirty people with MS (23 relapsing‐remitting MS [RRMS], 7 progressive MS [PMS]) and 23 healthy controls were scanned on a human 3‐tesla (3T) MRI scanner equipped with 300 mT/m maximum gradient strength using a comprehensive multishell diffusion MRI protocol. Data were fitted to a three‐compartment geometric model of white matter to estimate apparent axon diameter and axon density in the midline corpus callosum. Neurologic disability and cognitive function were measured using the Expanded Disability Status Scale (EDSS), Multiple Sclerosis Functional Composite (MSFC), and Minimal Assessment of Cognitive Function in MS battery. Results Apparent axon diameter was significantly larger and axon density reduced in the normal‐appearing corpus callosum (NACC) of MS patients compared to healthy controls, with similar trends seen in PMS compared to RRMS. Larger apparent axon diameter in the NACC of MS patients correlated with greater disability as measured by the EDSS (r = 0.555, P = 0.007) and poorer performance on the Symbol Digits Modalities Test (r = ‐0.593, P = 0.008) and Brief Visuospatial Memory Test–Revised (r = −0.632, P

Continued improvement in MRI acquisition technology has made functional MRI (fMRI) with small isotropic voxel sizes down to 1 mm and below more commonly available. Although many conventional fMRI studies seek to investigate regional patterns of cortical activation for which conventional voxel sizes of 3 mm and larger provide sufficient spatial resolution, smaller voxels can help avoid contamination from adjacent white matter (WM) and cerebrospinal fluid (CSF), and thereby increase the specificity of fMRI to signal changes within the gray matter. Unfortunately, temporal signal-to-noise ratio (tSNR), a metric of fMRI sensitivity, is reduced in high-resolution acquisitions, which offsets the benefits of small voxels. Here we introduce a framework that combines small, isotropic fMRI voxels acquired at 7 T field strength with a novel anatomically-informed, surface mesh-navigated spatial smoothing that can provide both higher detection power and higher resolution than conventional voxel sizes. Our smoothing approach uses a family of intracortical surface meshes and allows for kernels of various shapes and sizes, including curved 3D kernels that adapt to and track the cortical folding pattern. Our goal is to restrict smoothing to the cortical gray matter ribbon and avoid noise contamination from CSF and signal dilution from WM via partial volume effects. We found that the intracortical kernel that maximizes tSNR does not maximize percent signal change (ΔS/S), and therefore the kernel configuration that optimizes detection power cannot be determined from tSNR considerations alone. However, several kernel configurations provided a favorable balance between boosting tSNR and ΔS/S, and allowed a 1.1-mm isotropic fMRI acquisition to have higher performance after smoothing (in terms of both detection power and spatial resolution) compared to an unsmoothed 3.0-mm isotropic fMRI acquisition. Overall, the results of this study support the strategy of acquiring voxels smaller than the cortical thickness, even for studies not requiring high spatial resolution, and smoothing them down within the cortical ribbon with a kernel of an appropriate shape to achieve the best performance—thus decoupling the choice of fMRI voxel size from the spatial resolution requirements of the particular study. The improvement of this new intracortical smoothing approach over conventional surface-based smoothing is expected to be modest for conventional resolutions, however the improvement is expected to increase with higher resolutions. This framework can also be applied to anatomically-informed intracortical smoothing of higher-resolution data (e.g. along columns and layers) in studies with prior information about the spatial structure of activation.

PURPOSE To compare the performance of an 8-channel surface coil/clamshell transmitter and 32-channel head array coil/birdcage transmitter for hyperpolarized 13 C brain metabolic imaging. METHODS To determine the field homogeneity of the radiofrequency transmitters, B1 + mapping was performed on an ethylene glycol head phantom and evaluated by means of the double angle method. Using a 3D echo-planar imaging sequence, coil sensitivity and noise-only phantom data were acquired with the 8- and 32-channel receiver arrays, and compared against data from the birdcage in transceiver mode. Multislice frequency-specific 13 C dynamic echo-planar imaging was performed on a patient with a brain tumor for each hardware configuration following injection of hyperpolarized [1-13 C]pyruvate. Signal-to-noise ratio (SNR) was evaluated from pre-whitened phantom and temporally summed patient data after coil combination based on optimal weights. RESULTS The birdcage transmitter produced more uniform B1 + compared with the clamshell: 0.07 versus 0.12 (fractional error). Phantom experiments conducted with matched lateral housing separation demonstrated 8- versus 32-channel mean transceiver-normalized SNR performance: 0.91 versus 0.97 at the head center; 6.67 versus 2.08 on the sides; 0.66 versus 2.73 at the anterior; and 0.67 versus 3.17 on the posterior aspect. While the 8-channel receiver array showed SNR benefits along lateral aspects, the 32-channel array exhibited greater coverage and a more uniform coil-combined profile. Temporally summed, parameter-normalized patient data showed SNRmean,slice ratios (8-channel/32-channel) ranging 0.5-2.00 from apical to central brain. White matter lactate-to-pyruvate ratios were conserved across hardware: 0.45 ± 0.12 (8-channel) versus 0.43 ± 0.14 (32-channel). CONCLUSION The 8- and 32-channel hardware configurations each have advantages in particular brain anatomy.

Objective: The purpose of this paper is to prove that computer-vision techniques allow synthesizing water-fat separation maps for local specific absorption rate (SAR) estimation, when patient-specific water-fat images are not available. Methods: We obtained ground truth head models by using patient-specific water-fat images. We obtained two different label-fusion water-fat models generating a water-fat multiatlas and applying the STAPLE and local-MAP-STAPLE label-fusion methods. We also obtained patch-based water-fat models applying a local group-wise weighted combination of the multiatlas. Electromagnetic (EM) simulations were performed, and B1+ magnitude and 10 g averaged SAR maps were generated. Results: We found local approaches provide a high DICE overlap (72.6 ± 10.2% fat and 91.6 ± 1.5% water in local-MAP-STAPLE, and 68.8 ± 8.2% fat and 91.1 ± 1.0% water in patch-based), low Hausdorff distances (18.6 ± 7.7 mm fat and 7.4 ± 11.2 mm water in local-MAP-STAPLE, and 16.4 ± 8.5 mm fat and 7.2 ± 11.8 mm water in patch-based) and a low error in volume estimation (15.6 ± 34.4% fat and 5.6 ± 4.1% water in the local-MAP-STAPLE, and 14.0 ± 17.7% fat and 4.7 ± 2.8% water in patch-based). The positions of the peak 10 g-averaged local SAR hotspots were the same for every model. Conclusion: We have created patient-specific head models using three different computer-vision-based water-fat separation approaches and compared the predictions of B1+ field and SAR distributions generated by simulating these models. Our results prove that a computer-vision approach can be used for patient-specific water-fat separation, and utilized for local SAR estimation in high-field MRI. Significance: Computer-vision approaches can be used for patient-specific water-fat separation and for patient specific local SAR estimation, when water-fat images of the patient are not available.

BACKGROUND Rapid volumetric imaging protocols could better utilize limited scanner resources. PURPOSE To develop and validate an optimized 6-minute high-resolution volumetric brain MRI examination using Wave-CAIPI encoding. STUDY TYPE Prospective. POPULATION/SUBJECTS Ten healthy subjects and 20 patients with a variety of intracranial pathologies. FIELD STRENGTH/SEQUENCE At 3 T, MPRAGE, T2 -weighted SPACE, SPACE FLAIR, and SWI were acquired at 9-fold acceleration using Wave-CAIPI and for comparison at 2-4-fold acceleration using conventional GRAPPA. ASSESSMENT Extensive simulations were performed to optimize the Wave-CAIPI protocol and minimize both g-factor noise amplification and potential T1 /T2 blurring artifacts. Moreover, refinements in the autocalibrated reconstruction of Wave-CAIPI were developed to ensure high-quality reconstructions in the presence of gradient imperfections. In a randomized and blinded fashion, three neuroradiologists assessed the diagnostic quality of the optimized 6-minute Wave-CAIPI exam and compared it to the roughly 3× slower GRAPPA accelerated protocol using both an individual and head-to-head analysis. STATISTICAL TEST A noninferiority test was used to test whether the diagnostic quality of Wave-CAIPI was noninferior to the GRAPPA acquisition, with a 15% noninferiority margin. RESULTS Among all sequences, Wave-CAIPI achieved negligible g-factor noise amplification (gavg ≤ 1.04) and burring artifacts from T1 /T2 relaxation. Improvements of our autocalibration approach for gradient imperfections enabled increased robustness to gradient mixing imperfections in tilted-field of view (FOV) prescriptions as well as variations in gradient and analog-to-digital converter (ADC) sampling rates. In the clinical evaluation, Wave-CAIPI achieved similar mean scores when compared with GRAPPA (MPRAGE: OW = 4.03, OG = 3.97; T2 w SPACE: OW = 4.00, OG = 4.00; SPACE FLAIR: OW = 3.97, OG = 3.97; SWI: OW = 3.93, OG = 3.83) and was statistically noninferior (N = 30, P

This white paper discusses prospects for advancing hyperpolarization technology to better understand cancer metabolism, identify current obstacles to HP (hyperpolarized) 13C magnetic resonance imaging’s (MRI’s) widespread clinical use, and provide recommendations for overcoming them. Since the publication of the first NIH white paper on hyperpolarized 13C MRI in 2011, preclinical studies involving [1-13C]pyruvate as well a number of other 13C labeled metabolic substrates have demonstrated this technology's capacity to provide unique metabolic information. A dose-ranging study of HP [1-13C]pyruvate in patients with prostate cancer established safety and feasibility of this technique. Additional studies are ongoing in prostate, brain, breast, liver, cervical, and ovarian cancer. Technology for generating and delivering hyperpolarized agents has evolved, and new MR data acquisition sequences and improved MRI hardware have been developed. It will be important to continue investigation and development of existing and new probes in animal models. Improved polarization technology, efficient radiofrequency coils, and reliable pulse sequences are all important objectives to enable exploration of the technology in healthy control subjects and patient populations. It will be critical to determine how HP 13C MRI might fill existing needs in current clinical research and practice, and complement existing metabolic imaging modalities. Financial sponsorship and integration of academia, industry, and government efforts will be important factors in translating the technology for clinical research in oncology. This white paper is intended to provide recommendations with this goal in mind.

Access to MRI is limited for patients with deep brain stimulation (DBS) implants due to safety hazards, including radiofrequency (RF) heating of tissue surrounding the leads. Computational models provide an exquisite tool to explore the multi-variate problem of RF heating and help better understand the interaction of electromagnetic fields and biological tissues. This paper presents a computational approach to assess RF-induced heating, in terms of specific absorption rate (SAR) in the tissue, around the tip of bilateral DBS leads during MRI at 64MHz/1.5 T and 127 MHz/3T. Patient-specific realistic lead models were constructed from post-operative CT images of nine patients operated for sub-thalamic nucleus DBS. Finite element method was applied to calculate the SAR at the tip of left and right DBS contact electrodes. Both transmit head coils and transmit body coils were analyzed. We found a substantial difference between the SAR and temperature rise at the tip of right and left DBS leads, with the lead contralateral to the implanted pulse generator (IPG) exhibiting up to 7 times higher SAR in simulations, and up to 10 times higher temperature rise during measurements. The orientation of incident electric field with respect to lead trajectories was explored and a metric to predict local SAR amplification was introduced. Modification of the lead trajectory was shown to substantially reduce the heating in phantom experiments using both conductive wires and commercially available DBS leads. Finally, the surgical feasibility of implementing the modified trajectories was demonstrated in a patient operated for bilateral DBS.

Spin-echo (SE) BOLD fMRI has high microvascular specificity, and thus provides a more reliable means to localize neural activity compared to conventional gradient-echo BOLD fMRI. However, the most common SE BOLD acquisition method, SE-EPI, is known to suffer from T2′ contrast contamination with undesirable draining vein bias. To address this, in this study, we extended a recently developed distortion/blurring-free multi-shot EPI technique, Echo-Planar Time-resolved Imaging (EPTI), to cortical-depth dependent SE-fMRI at 7T to test whether it could provide purer SE BOLD contrast with minimal T2′ contamination for improved neuronal specificity. From the same acquisition, the time-resolved feature of EPTI also provides a series of asymmetric SE (ASE) images with varying T2′ weightings, and enables extraction of data equivalent to conventional SE EPI with different echo train lengths (ETLs). This allows us to systematically examine how T2′-contribution affects different SE acquisition strategies using a single dataset. A low-rank spatiotemporal subspace reconstruction was implemented for the SE-EPTI acquisition, which incorporates corrections for both shot-to-shot phase variations and dynamic B0 drifts. SE-EPTI was used in a visual task fMRI experiment to demonstrate that i) the pure SE image provided by EPTI results in the highest microvascular specificity; ii) the ASE EPTI series, with a graded introduction of T2′ weightings at time points farther away from the pure SE, show a gradual sensitivity increase along with increasing draining vein bias; iii) the longer ETL seen in conventional SE EPI acquisitions will induce more draining vein bias. Consistent results were observed across multiple subjects, demonstrating the robustness of the proposed technique for SE-BOLD fMRI with high specificity.

Multi-parametric quantitative MRI has shown great potential to improve the sensitivity and specificity of clinical diagnosis and to enhance our understanding of complex brain processes, but suffers from long scan time especially at high spatial resolution. To address this long-standing challenge, we introduce a novel approach, termed 3D Echo Planar Time-resolved Imaging (3D-EPTI), which significantly increases the acceleration capacity of MRI sampling, and provides high acquisition efficiency for multi-parametric MRI. This is achieved by exploiting the spatiotemporal correlation of MRI data at multiple timescales through new encoding strategies within and between efficient continuous readouts. Specifically, an optimized spatiotemporal CAIPI encoding within the readouts combined with a radial-block sampling strategy across the readouts enables an acceleration rate of 800 folds in thek-tspace. A subspace reconstruction was employed to resolve thousands of artifact-free high-quality multi-contrast images spaced at a time interval of ~1 ms. We have demonstrated the ability of 3D-EPTI to provide robust and repeatable whole-brain simultaneous T1, T2, T2*, PD and B1+mapping at high isotropic resolution within minutes (e.g., 1-mm isotropic resolution in 3 minutes), and to enable submillimeter multi-parametric imaging to study detailed brain structures.HighlightsUltra-fast acquisition for 3D multi-parametric quantitative MRI.Simultaneous T1T2T2* PD and B1+mapping.3-minute scan at 1-mm isotropic resolution with whole-brain coverage.Multi-parametric mapping at 700-μm isotropic resolution in 10 minutes.Repeatable quantification and cortical-depth analysis.
