1. Feature Selection from Differentially Private Correlations

Abstract

Data scientists often seek to identify the most important features in high-dimensional datasets. This can be done through $L_1$-regularized regression, but this can become inefficient for very high-dimensional datasets. Additionally, high-dimensional regression can leak information about individual datapoints in a dataset. In this paper, we empirically evaluate the established baseline method for feature selection with differential privacy, the two-stage selection technique, and show that it is not stable under sparsity. This makes it perform poorly on real-world datasets, so we consider a different approach to private feature selection. We employ a correlations-based order statistic to choose important features from a dataset and privatize them to ensure that the results do not leak information about individual datapoints. 2. Queues with Delayed Information: Analyzing the Impact of the Choice Model Function

Abstract

In this paper, we study queueing systems with delayed information that use a generalization of the multinomial logit choice model as its arrival process. Previous literature assumes that the functional form of the multinomial logit model is exponential. However, in this work we generalize this to different functional forms. In particular, we compute the critical delay and analyze how it depends on the choice of the functional form. We highlight how the functional form of the model can be interpreted as an exponential model where the exponential rate parameter is uncertain. Furthermore, the rate parameter distribution is given by the inverse Laplace-Stieltjes transform of the functional form when it exists. We perform numerous numerical experiments to confirm our theoretical insights.

3. Mean Field Queues with Delayed Information

Abstract

In this paper, we consider a new queueing model where queues balance themselves according to a mean field interaction with a time delay. Unlike other work with delayed information our model considers multi-server queues with customer abandonment. In this setting, our queueing model corresponds to a system of mean field interacting delay differential equations with a point of non-differentiability introduced by the finite-server and abandonment terms. We show that this system of delay differential equations exhibits a change in stability when the delay in information crosses a critical threshold. In particular, the system exhibits periodic oscillations when the delay in information exceeds this critical threshold and we show that the threshold surprisingly does not depend on the number of queues. This is in stark contrast to other choice based queueing models with delayed information. We compute this critical threshold in each of the relevant parameter regions induced by the point of non-differentiability and show numerically how the critical threshold transitions through the point of non-differentiability.

Distributed delay equations have been used to model situations in which there is some sort of delay whose duration is uncertain. However, the interpretation of a distributed delay equation is actually very different from that of a delay differential equation with a random delay. This work explicitly highlights this distinction as it is an important consideration to make when modeling delayed systems in which the delay can take on several values.

5. Queues with Updating Information: Finding the Amplitude of Oscillations

Abstract

Many service systems provide customers with information about the system so that customers can make an informed decision about whether to join or not. Many of these systems provide information in the form of an update. Thus, the information about the system is updated periodically in increments of size $\Delta$. It is known that these updates can cause oscillations in the resulting dynamics. However, it is an open problem to explicitly characterize the size of these oscillations when they occur. In this paper, we solve this open problem and show how to exactly calculate the amplitude of these oscillations via a fixed point equation. We also calculate closed form approximations via Taylor expansions of the fixed point equation and show that these approximations are very accurate, especially when $\Delta$ is large. Our analysis provides new insight for systems that use updates as a way of disseminating information to customers.

6. Multi-Delay Differential Equations: A Taylor Expansion Approach

Abstract

It is already well-understood that many delay differential equations with only a single constant delay exhibit a change in stability according to the value of the delay in relation to a critical delay value. Finding a formula for the critical delay is important to understanding the dynamics of delayed systems and is often simple to obtain when the system only has a single constant delay. However, if we consider a system with multiple constant delays, there is no known way to obtain such a formula that determines for what values of the delays a change in stability occurs. In this paper, we present some single-delay approximations to a multi-delay system obtained via a Taylor expansion as well as formulas for their critical delays which are used to approximate where the change in stability occurs in the multi-delay system. We determine when our approximations perform well and we give extra analytical and numerical attention to the two-delay and three-delay settings.

7. Breaking the Symmetry in Queues with Delayed Information

Abstract

Giving customers queue length information about a service system has the potential to influence the decision of a customer to join a queue. Thus, it is imperative for managers of queueing systems to understand how the information that they provide will affect the performance of the system. To this end, we construct and analyze a two-dimensional deterministic fluid model that incorporates customer choice behavior based on delayed queue length information. All of the previous literature assumes that all queues have identical parameters and the underlying dynamical system is symmetric. However, in this paper, we relax this symmetry assumption by allowing the arrival rates, service rates, and the choice model parameters to be different for each queue. Our methodology exploits the method of multiple scales and asymptotic analysis to understand how to break the symmetry. We find that the asymmetry can have a large impact on the underlying dynamics of the queueing system.

Abstract

Background and aims The purpose of this study was to present data on the safety of anti-severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) vaccination in a cohort of inflammatory bowel disease (IBD) patients of an ongoing multicenter study (ESCAPE-IBD) sponsored by the Italian Group for the study of Inflammatory Bowel Disease (ClinicalTrials.gov Identifier: NCT04769258). Methods Anti-SARS-CoV-2 vaccination was administrated to 809 IBD patients. Interviews were conducted to report adverse events related to vaccination. Of these 809, 346 patients were surveyed on the pandemic burden and the main reason for hesitancy in coronavirus disease 2019 vaccination. The chi-square test was used to compare categorical variables. Logistic regression was used to assess the relationship between disease-related characteristics and the onset of adverse events. Results About 45% of patients had at least one side effect, following the first dose (10%), the second (15%), and both doses (19%). All the adverse events were mild and lasted only a few days. Logistic regression analysis revealed that female sex (P < 0.001), younger age (P = 0.001), seroconversion (P = 0.002), and comorbidity (P < 0.001) were significantly associated with adverse events. The survey showed that the main concerns were the possibility of adverse event (33%). Almost all patients (99%) felt safer having been vaccinated at their IBD reference center. Conclusion The vaccine reactions experienced in IBD patients were mostly self-limited. We found high acceptance and good safety of SARS-CoV-2 vaccination in our cohort.

Abstract

Background: Patients on immunosuppressive drugs have been excluded from COVID-19 vaccines trials, creating concerns regarding their efficacy. Aims: To explore the humoral response to COVID-19 vaccines in patients with inflammatory bowel disease (IBD) Methods: Effectiveness and Safety of COVID-19 Vaccine in Patients with Inflammatory Bowel Disease (IBD) Treated with Immunomodulatory or Biological Drugs (ESCAPE-IBD) is a prospective, multicentre study promoted by the Italian Group for the study of Inflammatory Bowel Disease. We present data on serological response eight weeks after the second dose of COVID-19 vaccination in IBD patients and healthy controls (HCs). Results: 1076 patients with IBD and 1126 HCs were analyzed. Seropositivity for anti-SARS-CoV-2 IgG was reported for most IBD patients, even if with a lesser rate compared with HCs (92.1% vs. 97.9%; p<0.001). HCs had higher antibody concentrations (median OD 8.72 [IQR 5.2-14-2]) compared to the whole cohort of IBD patients (median OD 1.54 [IQR 0.8-3.6]; p<0.001) and the subgroup of IBD patients (n=280) without any treatment or on aminosalicylates only (median OD 1.72 [IQR 1.0–4.1]; p<0.001). Conclusions: Although most IBD patients showed seropositivity after COVID-19 vaccines, the magnitude of the humoral response was significantly lower than in HCs. Differently from other studies, these findings seem to be mostly unrelated to the use of immune-modifying treatments (ClinicalTrials.govID:NCT04769258).

Abstract

Background: The prevalence of anxiety is three times greater in patients with chronic diseases than in the general population. There has recently been growing interest in the roles of inflammation in contributing to the development of anxiety in people with Immune-mediated inflammatory diseases (IMID). Patient-reported outcome measures can facilitate the assessment of physical and psychological functioning. In recent years various patient outcome measures (PROMs) have been implemented. The National Institutes of Health (NIH)'s Patient-Reported Outcomes Measurement Information System (PROMIS®) is a set of publicly available and standardized Patient-Reported Outcomes (PROs) that cover physical appearance, mental health, and social health. The PROMIS® has been built through an Item Response Theory approach (IRT), a model-based measurement in which trait level estimates depend on both persons' responses and on the properties of the items that were administered. The aim of this study is to test the psychometric properties of the original anxiety items of the PROMIS® in a cohort of outpatients with IMIDs.Methods: We translated the original eight anxiety items from the PROMIS into Italian and administered them to consecutive outpatients affected by Inflammatory Bowel disease (n = 246), rheumatological (n = 100) and dermatological (n = 43) diseases, and healthy volunteers (n = 280). We analyzed the data through an Item Response Theory (IRT) analysis to evaluate the psychometric properties of the Italian adaptation of the PROMIS® anxiety short form.Results: Taken together, Confirmatory Factor Analysis and Exploratory Factor analysis suggested that the unidimensionality assumption if the instrument holds. The instrument has excellent reliability from a Classical Theory of Test (CTT) standpoint (Cronbach's ⍺ = .94, McDonald's ω = .95). The 1PL Graded Response Model (GRM) model provided a good balance between the goodness of fit and parsimony (BIC = 9099.82) as compared to the 2PL GRM model (BIC = 9111.02). We did not find signs of Differential Item Functioning (DIF), but we found that the local independence assumption was violated for some items. The analysis of the test reliability curve suggested that the instrument is most reliable for higher levels of the latent trait of anxiety.Discussion: The Italian adaptation of the PROMIS® anxiety short form shows acceptable psychometric properties both from a CTT and an IRT standpoint. Its compatibility with the 1 parameter logistic (1PL) model implies that its total score is a sufficient statistic for the latent anxiety trait. The TRC shows that this instrument is mostly informative for people with higher levels of anxiety, making it particularly suitable for clinical populations such as IMID patients.

Abstract

Abstract Background Jejunal diverticulosis is an uncommon disease and usually asymptomatic. It can be complicated not only by diverticulitis, but by hemorrhage, perforation, intussusception, volvulus, malabsorption and even small bowel obstruction due to enteroliths formed and expelled from these diverticula. Methods We describe a case of an occult bleeding jejunal diverticulum, casually discovered in a patient that was taken to surgery for a Dieulafoy’s lesion after unsuccessful endoscopic treatment. We performed a gastric resection together with an ileocecal resection. Macroscopic and microscopic examinations confirmed the gastric Dieulafoy’s lesion and demonstrated the presence of another source of occult bleeding in asymptomatic jejunal diverticulum. Discussion The current case emphasizes that some gastrointestinal bleeding lesions, although rare, can be multiple and result in potentially life-threatening bleeding. The clinician must be mindful to the possibility of multisite lesions and to the correlation between results of the investigations and clinical condition of the bleeding patient.

Abstract

Purpose: The aim of this study was to compare the influence of the number of coronal walls and post-endodontic restorations on the mechanical strength of 165 recently extracted endodontically treated maxillary premolars.Methods: The teeth were divided into 3 control (no post) and 3 test groups according to the number of residual walls. Each test group was divided into subgroups according to the type of post-endodontic restoration (single, oval, and multi-post techniques). Samples were prepared conforming to the assigned subgroup. A universal loading machine applied a load parallel to the longitudinal axis of the teeth, thus simulating physiological occlusion. ANOVA and the Kruskal Wallis test were used for comparisons (P ≤ 0.05), and Tukey's test for multiple comparisons.Results: For intact teeth, significant differences were found among all 3 subgroups, with single post showing the highest values. For 3 residual walls, oval post resulted in significantly lower values than single and multi-post systems. For 2 residual walls, the multi-post technique showed almost twice the resistance of oval post restorations.Conclusion: In oval canals the use of a single or multi-post technique increased post-and-core resistance even in intact teeth, whereas oval fiber posts showed no improvements. Abstract

Familial adenomatous polyposis (FAP) is one of the most important clinical hereditary forms of inherited susceptibility to colorectal cancer and is characterized by a high degree of phenotypic heterogeneity. We used a mass spectrometry driven-proteomic strategy to identify serum molecules differently expressed in FAP patients. The data obtained were subsequently processed by bioinformatic analysis and confirmed by Western blotting. Significant differences were highlighted in the expression of serum proteins of FAP patients. In particular, two proteins (alpha-2-HS-glycoprotein and apoliprotein D) were down-regulated (about 0.5- and 0.7-fold, respectively) in carpeting versus diffuse FAP patients and healthy donors, while alpha-2-antiplasmin was up-regulated (about 1.4-fold). Moreover, mass spectrometry approach enabled us to identify serum biomarkers specific for two distinct clinical form of FAP, i.e. carpeting and diffuse FAR In particular, vitronectin was tip-regulated (more than 1.4-fold) in diffuse FAP patients versus carpeting FAP and versus healthy donors, and two additional proteins (Haptoglobin and alpha-1-acid glycoprotein 1) were up-regulated in 2 out of 3 carpeting FAP patients. Our study suggests that mass spectrometry combined to a strong bioinformatics analysis is a valuable tool for the identification ofquali/quantitative differences in the serum proteome of otherwise indistinguishable FAP phenotypes. Moreover, the definition of a proteomic profile, supported by the supervised classification, is a powerful and highly sensitive approach for the identification molecular signatures that are able to outperform the traditional disease markers and can therefore be efficiently applied for the diagnosis and clinical management of FAP patients. Abstract

Objective: To assess maxilla and mandibular arch widths' response to Haas-type rapid maxillary expansion (RME) anchored to deciduous vs permanent molars on children with unilateral posterior crossbite. Materials and Methods: Seventy patients with unilateral posterior crossbite recruited at the Universities of Genova, Siena, and Insubria (Varese) were randomly located into GrE (RME on second deciduous molars) or Gr6 (RME on first permanent molars) and compared. Results: Upper intermolar distance and permanent molar angulation increased significantly in Gr6 vs GrE at T1. Upper intercanine distance increased significantly in GrE vs Gr6 at T1 and T2. GrE showed significant increases for upper intermolar and upper intercanine widths. Gr6 showed statistically significant increases for upper intermolar widths, for upper and lower intercanine widths, and for increases of angulation of upper and lower permanent molars. Conclusions: GrE showed reduced molar angulation increases at T1 and reduced molar angulation decreases at T2 when compared with Gr6. At T2, the net increase of the upper intercanine distance in GrE was still significant compared with Gr6, indicating a more stable expansion in the anterior area. [ABSTRACT FROM AUTHOR]

25. Loss of CRABP-II Characterizes Human Skin Poorly Differentiated Squamous Cell Carcinomas and Favors DMBA/TPA-Induced Carcinogenesis

Abstract

Retinol and its derivatives play an important role in epidermal growth and differentiation and represent chemopreventive agents in nonmelanoma skin cancer. Retinoic acid binding protein II (CRABP-II) is a cytoplasmic receptor that critically regulates all-trans-retinoic acid (ATRA) trafficking. We documented the marked reduced expression of CRABP-II and its promoter methylation in human poorly differentiated squamous cell carcinomas. To investigate the role of CRABP-II in skin carcinogenesis we used skin lesion induction by dimethylbenz[a]anthracene/12-O-tetradecanoyl-phorbol-13-acetate in CRABP-II–knockout C57BL/6 mice. We observed earlier and more diffuse epidermal dysplasia, greater incidence and severity of tumors, reduced expression of cytokeratin 1/cytokeratin 10 and involucrin, increased proliferation, and impaired ATRA inhibition of tumor promotion compared with wild-type animals. CRABP-II–transfected HaCaT, FaDu, and A431 cells showed expression of differentiation markers, retinoic acid receptor-β/-γ signaling, ATRA sensitivity, and suppression of EGFR/v-akt murine thymoma viral oncogene homolog 1 (AKT) pathways in a fatty acid binding protein 5/peroxisome proliferator–activated receptor-β/-δ–independent manner. The opposite was true in keratinocytes isolated from CRABP-II–knockout mice. Finally, CRABP-II accumulation induced ubiquitination-associated reduction of EGFR. Our results showed reduced CRABP-II expression in human poorly differentiated squamous cell carcinomas, and its gene deletion favored experimental skin carcinogenesis and impaired ATRA antitumor efficacy, likely modulating EGFR/AKT pathways and retinoic acid receptor-β/-γ signaling. Therapeutic interventions aimed at restoring CRABP-II–mediated signaling may amplify therapeutic retinoid efficacy in nonmelanoma skin cancer.

Abstract

Aim: The aim of this work is to evaluate whether some facial components have a stronger impact than others on the perception of beauty and to determine whether classical aesthetical standards are still valid for the current face types. Methods and materials: 58 students aged 18-30 years, 32 male and 26 female were analized. Face photos in a rest position were loaded on "Point. tool" software and some facial landmarks were loaded on "Venus" software to simulate aesthetical improvements. The square Pöch-Perseo mesh was used to develop our own mesh and each face was associated with a reference geometrical shape and a mesh. We carried out a quantitative and a qualitative evaluation of the face after and before the improvement of 75%. The data was subdivided into four categories. Statistical analyses: "Kolmogorov-Smirnov", "Kruskal-Wallis" and "Man-Whitney U" tests were used. Data distribution in each group is not standard. There is a statistically significant difference in the four categories in each group. Result: There are no facial components having a stronger impact on the beauty and attractiveness of a face. No geometrical shape prevails in our sample, although the oval shape is more represented than the others. Conclusion: Classical beauty norms (oval shaped) can no longer be the only appropriate aesthetical reference for the variety of face types we examine in our daily practice. [ABSTRACT FROM AUTHOR]

Abstract

Background: Malignant pleural mesothelioma is highly aggressive and recurs rapidly despite radical multimodality treatment. Progression of mesothelioma is thought to be governed by various growth factors, including vascular endothelial growth factor (VEGF). Placenta growth factor (PlGF) belongs to the VEGF family, although no study has yet investigated its expression in mesothelioma. We hypothesized that PlGF is overexpressed in mesothelioma and could have prognostic value in patients treated by extrapleural pneumonectomy. Methods: We assessed by immunohistochemistry with semiquantitative classification (0 = no staining; 3 = strong staining), the expression levels of PlGF and its cognate receptors VEGF receptor 1, neuropilin-1, and neuropilin-2 in 27 patients with malignant pleural mesothelioma undergoing extrapleural pneumonectomy, in 14 patients with reactive mesothelium, and in 10 patients with normal mesothelium. Results: Whereas PlGF was not expressed in normal mesothelium, it was overexpressed (grade 3) more frequently in mesothelioma than in reactive mesothelium specimens (11 or 41% versus 1 or 7%, respectively, p = 0.03). Furthermore, in mesothelioma, VEGF receptor 1 and neuropilin-1 and -2 were overexpressed in 18 specimens (67%), 8 specimens (30%), and 9 specimens (33%), respectively. Mean survival after extrapleural pneumonectomy was 17 months. An inverse relationship was found between the degree of PlGF expression and survival in months (R = −0.45, p = 0.01). No correlation was found between tumor stage and survival (R = −0.33) and between tumor stage and PlGF expression (R = 0.07). Conclusions: We have shown that PlGF can be overexpressed in malignant pleural mesothelioma. In addition, the finding of an inverse relationship between PlGF expression levels and survival suggests a pivotal role of this factor in the recurrence and progression of mesothelioma after extrapleural pneumonectomy. Abstract

Recent findings have disclosed the role of UDP-glucuronosyltransferase (UGT) 1A1*28 on the haematological toxicity induced by irinotecan (CPT-11), a drug commonly used in the treatment of metastatic colorectal cancer (mCRC). We investigated the pharmacogenomic profile of irinotecan-induced gastrointestinal (GI) toxicity by the novel drug-metabolizing enzyme and transporter (DMET) microarray genotyping platform. Twenty-six mCRC patients who had undergone to irinotecan-based chemotherapy were enrolled in a case (patients experiencing > grade 3 gastrointestinal, (GI) toxicity) - control (matched patients without GI toxicity) study. A statistically significant difference of SNP genotype distribution was found in the case versus control group. The homozygous genotype C/C in the (rs562) ABCC5 gene occurred in 6/9 patients with GI toxicity versus 1/17 patients without GI toxicity (P=0.0022). The homozygous genotype G/G in the (rs425215) ABCG1 was found in 7/9 patients with GI toxicity versus 4/17 patients without GI toxicity (P=0.0135). The heterozygous genotype G/A in the 388G>A (rs2306283) OATP1B1/SLCO1B1 was found in 3/9 patients with grade >3 GI toxicity versus 14/17 patients without GI toxicity (P=0.0277). DNA extracted from peripheral blood cells was genotyped by DMET Plus chip on Affymetrix array system. Genotype association was calculated by Fisher's exact test (two tailed) and relevant SNPs were further analyzed by direct sequencing. We have identified 3 SNPs mapping in ABCG1, ABCC5 and OATP1B1/SLCO1B1 transporter genes associated with GI toxicity induced by irinotecan in mCRC patients expanding the available knowledge of irinogenomics. The DMET microarray platform is an emerging technology for easy identification of new genetic variants for personalized medicine.

Since GAGs are involved in the regulation of renal protein filtration and GAGs disruption is associated with anionic proteinuria, we examined whether changes in the selectivity of renal protein filtration occur in IBD.
Methods.From 46 patients with IBD (17 with Crohn's disease (CD), and 29 with ulcerative colitis (UC)) and 21 healthy subjects, urine and serum samples were obtained. Albumin, total IgG and IgG4 clearances were measured using sensitive methods. Serum p-ANCA and TNF-α were tested.
Results.Median IgG4 clearance was 0.041 ml/ min/10-3 in patients with UC and 0.10 ml/ min/10-3 in CD patients, both significantly higher than in controls (0.03 ml/min/10-3) (P&lt;0.03). IgG4 clearance was above the upper normal limit in 9/17 CD (53%) and in 10/29 UC (34.5%). Eighteen of 19 patients showing abnormal IgG4 clearance were taking mesalazine. In patients on maintenance oral mesalazine, IgG4 clearance was higher than that in patients off treatment (0.12 vs 0.03 ml/min/10-3, P=0.04). No clinical/laboratory sign of renal dysfunction was documented in patients with altered IgG4 clearance and maintained on mesalazine treatment.
Conclusion.Renal protein charge permselectivity is impaired in 40% of patients with IBD with no overt proteinuria. Our data suggest that altered IgG4 clearance may represent a subclinical marker of renal involvement in IBD.

Abstract

Immune checkpoint inhibitors (ICIs) represent an important advance in the adjuvant treatment of patients with high-risk melanoma. Although the safety profile of anti-programmed cell death protein-1 (PD-1) is fairly acceptable, different immune-related adverse events (irAEs) are described. Herein we report for the first time a notably multidisciplinary combined approach on a malignant melanoma (MM) patient treated with anti-PD-1 antibody in adjuvant setting. In this novel approach, corticosteroid-refractory immune-mediated colitis (IMC) was effectively treated with Vedolizumab, a selective blockade of the α4β7 integrin and corticosteroids were successfully administered for autoimmune neutropenia. Notably, our patient also express HLA-B*35, a potential biomarker for predicting a genetic basis of autoimmune susceptibility. Our experience offers a possible future perspective about the use of Vedolizumab together with immunotherapy in a strategic early approach for high-risk patients genotyped for HLA.

Abstract

Recent findings have disclosed the role of UDP-glucuronosyltransferase (UGT) 1A1*28 on the haematological toxicity induced by irinotecan (CPT-11), a drug commonly used in the treatment of metastatic colorectal cancer (mCRC). We investigated the pharmacogenomic profile of irinotecan-induced gastrointestinal (GI) toxicity by the novel drug-metabolizing enzyme and transporter (DMET) microarray genotyping platform. Twenty-six mCRC patients who had undergone to irinotecan-based chemotherapy were enrolled in a case (patients experiencing > grade 3 gastrointestinal, (GI) toxicity) - control (matched patients without GI toxicity) study. A statistically significant difference of SNP genotype distribution was found in the case versus control group. The homozygous genotype C/C in the (rs562) ABCC5 gene occurred in 6/9 patients with GI toxicity versus 1/17 patients without GI toxicity (P=0.0022). The homozygous genotype G/G in the (rs425215) ABCG1 was found in 7/9 patients with GI toxicity versus 4/17 patients without GI toxicity (P=0.0135). The heterozygous genotype G/A in the 388G>A (rs2306283) OATP1B1/SLCO1B1 was found in 3/9 patients with grade > 3 GI toxicity versus 14/17 patients without GI toxicity (P=0.0277). DNA extracted from peripheral blood cells was genotyped by DMET Plus chip on Affymetrix array system. Genotype association was calculated by Fisher's exact test (two tailed) and relevant SNPs were further analyzed by direct sequencing. We have identified 3 SNPs mapping in ABCG1, ABCC5 and OATP1B1/SLCO1B1 transporter genes associated with GI toxicity induced by irinotecan in mCRC patients expanding the available knowledge of irinogenomics. The DMET microarray platform is an emerging technology for easy identification of new genetic variants for personalized medicine.

Abstract

Objective To evaluate the effectiveness of gonadotropin-releasing hormone agonists (GnRH-a) with or without coadministration of tibolone in women with menstrual cycle–related irritable bowel syndrome (IBS). Design Prospective, randomized, placebo-controlled clinical trial. Setting Universities of Catanzaro and Naples. Patient(s) One hundred twenty young premenopausal women with menstrual cycle–related IBS (Rome II criteria). Intervention(s) Administration of leuprolide acetate depot (LAD, 11.25 mg IM/3 months) plus tibolone (group A), LAD plus placebo tablets (group B), and injection of a placebo solution plus placebo tablets (group C). Main outcome measure(s) Severity of bowel symptoms or signs of IBS and quality of life (QoL), at baseline and after 6 months of treatment. Result(s) In all groups, the mean scores for each symptom or sign of IBS and for QoL were significantly improved after treatment. A significant difference was observed between group C and groups A and B. No difference between these last groups was detected in symptoms or signs of IBS. The QoL scores were significantly higher in group A than in group B. Conclusion(s) Gondotropin-releasing hormone agonist administration is effective in women with menstrual cycle–related IBS. The addition of tibolone does not reduce effectiveness compared with agonist alone and increases QoL.
