Abstract

As many coastal regions experience a rapid increase in offshore wind farm installations, inter-farm distances become smaller, with a tendency to install larger turbines at high capacity densities. It is, however, not clear how the wake losses in wind farm clusters depend on the characteristics and spacing of the individual wind farms. Here, we quantify this based on multiple COSMO-CLM simulations, each of which assumes a different, spatially invariant combination of the turbine type and capacity density in a projected, future wind farm layout in the North Sea. An evaluation of the modelled wind climate with mast and lidar data for the period 2008–2020 indicates that the frequency distributions of wind speed and wind direction at turbine hub height are skillfully modelled and the seasonal and inter-annual variations in wind speed are represented well. The wind farm simulations indicate that for a typical capacity density and for SW winds, inter-farm wakes can reduce the capacity factor at the inflow edge of wind farms from 59 % to between 54 % and 30 % depending on the proximity, size and number of the upwind farms. The efficiency losses due to intra- and inter-farm wakes become larger with increasing capacity density as the layout-integrated, annual capacity factor varies between 51.8 % and 38.2 % over the considered range of 3.5 to 10 MW km−2. Also, the simulated efficiency of the wind farm layout is greatly impacted by switching from 5 MW turbines to next-generation, 15 MW turbines, as the annual energy production increases by over 27 % at the same capacity density. In conclusion, our results show that the wake losses in future wind farm clusters are highly sensitive to the inter-farm distances and the capacity densities of the individual wind farms and that the evolution of turbine technology plays a crucial role in offsetting these wake losses., Atmospheric Remote Sensing

Item does not contain fulltext, BACKGROUND: Several reports have demonstrated the impact of the COVID-19 pandemic on the management and outcome of patients with ST-segment elevation myocardial infarction (STEMI). The aim of the current analysis is to investigate the potential gender difference in the effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI patients within the ISACS-STEMI COVID-19 Registry. METHODS: This retrospective multicenter registry was performed in high-volume primary percutaneous coronary intervention (PPCI) centers on four continents and included STEMI patients undergoing PPCIs in March-June 2019 and 2020. Patients were divided according to gender. The main outcomes were the incidence and timing of the PPCI, (ischemia time ≥ 12 h and door-to-balloon ≥ 30 min) and in-hospital or 30-day mortality. RESULTS: We included 16683 STEMI patients undergoing PPCIs in 109 centers. In 2020 during the pandemic, there was a significant reduction in PPCIs compared to 2019 (IRR 0.843 (95% CI: 0.825-0.861, p < 0.0001). We did not find a significant gender difference in the effects of the COVID-19 pandemic on the numbers of STEMI patients, which were similarly reduced from 2019 to 2020 in both groups, or in the mortality rates. Compared to prepandemia, 30-day mortality was significantly higher during the pandemic period among female (12.1% vs. 8.7%; adjusted HR [95% CI] = 1.66 [1.31-2.11], p < 0.001) but not male patients (5.8% vs. 6.7%; adjusted HR [95% CI] = 1.14 [0.96-1.34], p = 0.12). CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures similarly observed in both genders. Furthermore, we observed significantly increased in-hospital and 30-day mortality rates during the pandemic only among females. Trial registration number: NCT 04412655.

The aim of the current analysis is to evaluate the age-related effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI within the registry ISACS-STEMI COVID-19. METHODS: This retrospective multicenter registry was performed in high-volume PPCI centers on four continents and included STEMI patients undergoing PPCI in March-June 2019 and 2020. Patients were divided according to age (< or ≥75 years). The main outcomes were the incidence and timing of PPCI, (ischemia time longer than 12 h and door-to-balloon longer than 30 min), and in-hospital or 30-day mortality. RESULTS: We included 16,683 patients undergoing PPCI in 109 centers. In 2020, during the pandemic, there was a significant reduction in PPCI as compared to 2019 (IRR 0.843 (95%-CI: 0.825-0.861, p < 0.0001). We found a significant age-related reduction (7%, p = 0.015), with a larger effect on elderly than on younger patients. Furthermore, we observed significantly higher 30-day mortality during the pandemic period, especially among the elderly (13.6% vs. 17.9%, adjusted HR (95% CI) = 1.55 [1.24-1.93], p < 0.001) as compared to younger patients (4.8% vs. 5.7%; adjusted HR (95% CI) = 1.25 [1.05-1.49], p = 0.013), as a potential consequence of the significantly longer ischemia time observed during the pandemic. CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures, with a larger reduction and a longer delay to treatment among elderly patients, which may have contributed to increase in-hospital and 30-day mortality during the pandemic.

10. Validation of wind farm parameterisation in Weather Forecast Model HARMONIE-AROME: Analysis of 2019

Abstract

In the next few decades climate mitigation efforts will transform the North Sea into one of the most important energy sources. The present wind energy capacity on the North Sea is expected to increase by almost a factor 5 in 2030 and almost a factor 10 in 2050. It is therefore of paramount importance to know how wind farms influence the atmosphere. Wind farms extract kinetic energy from the atmosphere and in doing so decrease the wind speed and increase turbulence levels. More turbulence means more mixing of vertical layers in the atmosphere and a change in humidity and temperature profiles. This may lead to cloud forming or dissipation. Wind farms are also an obstacle to the flow, which is what is called the blockage effect, as opposed to the wake effect behind the wind farm. This report is about the wake effect, mainly on wind, but we also analysed temperature and humidity profiles. In order to assess and quantify the wake effect, we compared two high resolution re-analyses for the year 2019 on a 2000 by 2000 km North Sea domain. The high resolution re-analyses with a 2.5 km horizontal grid spacing is based on global re-analysis ERA5 and downscaled with mesoscale weather model HARMONIE-AROME which is used operationally at KNMI. One of the re-analyses is without the effect of wind farms (referred to as control or HarmCY43-CTL in this report) and one with the Fitch wind farm parametrization that was recently incorporated in HARMONIE-AROME (HarmCY43-WFP). From the differences between the two we can isolate the wind speed deficits, or wakes, from the wind farms. Earlier validation studies have shown that a previous version of the HARMONIE-AROME model (HarmCY40) produces accurate wind climatology for undisturbed wind fields (period 2008-2018) and validates well against disturbed tower, aircraft and lidar measurements from 2016. 12. Downscaling daily air-temperature measurements in the Netherlands

Abstract

High-resolution, regularly gridded air-temperature maps are frequently used in climatology, hydrology, and ecology. Within the Netherlands, 34 official automatic weather stations (AWSs) are operated by the National Met Service according to World Meteorological Organization (WMO) standards. Although the measurements are of high quality, the spatial density of the AWSs is not sufficient to reconstruct the temperature on a 1-km-resolution grid. Therefore, a new methodology for daily temperature reconstruction from 1990 to 2017 is proposed, using linear regression and multiple adaptive regression splines. The daily 34 AWS measurements are interpolated using eight different predictors: diurnal temperature range, population density, elevation, albedo, solar irradiance, roughness, precipitation, and vegetation index. Results are cross-validated for the AWS locations and compared with independent citizen weather observations. The RMSE of the reference method ordinary kriging amounts to 2.6 °C whereas using the new methods the RMSE drops below 1.0 °C. Especially for cities, a substantial improvement of the predictions is found. Independent predictions are on average 0.3 °C less biased than ordinary kriging at 40 high-quality citizen measurement sites. With this new method, we have improved the representation of local temperature variations within the Netherlands. 14. Automating air temperature Siting Classification of meteorological stations according to the World Meteorological Organization guidelines

Abstract

This research thesis investigated the automatisation of the air temperature Siting Classification (SC) according to its World Meteorological Organization (WMO) guidelines. Instead of going into the field, the Automated SC (ASC) developed in this research thesis uses the Actueel Hoogtebestand Nederland (AHN) and Basisregistratie Grootschalige Topografie (BGT) geospatial open data datasets to determine the classifications for air temperature. The algorithm was first tested at the Automatic Weather Station (AWS) in De Bilt, after which it was applied to all the other 33 AWS sites owned and maintained by the Royal Netherlands Meteorological Institute (KNMI). The ASC model was developed using the programming language of R and made use of a decision tree to determine an air temperature SC Class 1 through 5 for each AWS. The usability of the model was tested through visual validations based on photos or satellite imagery to check on its accuracy, and comparison validations based on the current Manual Siting Classification (MSC) procedure done by the KNMI. Results shows that an ASC model corresponds for 55.9% with the MSC values and that the model can be applied for a reproducible and consistent SC if the data is determined to be correct and if a manual validation of the outcomes is executed. The ability of performing an ASC can contribute for further research in improving the air temperature SC of existing, or determining completely new locations. Furthermore, the model can also be used to determine the air temperature SC of the vast of other sensors of third-party networks. 15. Sky view factor calculations and its application in urban heat island studies

Abstract

The sky view factor (SVF) is essential to describe the urban climatology at scales below 100m. This proxy for net radiation depends on the height of the obstacles in its surroundings. The SVF was calculated from a rasterized point cloud height dataset (with 6 − 10 points per m2). The resulting SVF depends on grid-resolution, search radius and number of directions. Previous research related the diurnal maximum urban heat island (UHI) of the canopy layer to the diurnal temperature range, solar irradiance, wind speed, vegetation fraction and SVF. The goal of this study is to determine the sensitivity of the SVF and the impact on the UHI. Within the Netherlands a test area of 70km2 was selected, including: urban areas, meadows and forests. There is a high sensitivity for grid-resolution. Therefore the impact of the SVFs grid resolution on the maximum UHI is explored. Results show that the fourth largest city within the Netherlands, Utrecht, has a mean diurnal maximum UHI of 3.1 °C using a 1m SVF resolution. But, with a 3m SVF resolution the UHI is on average 0.6 °C lower. This highlights the significance of a fine grid resolution which can capture houses, alleys and trees.

A spontaneous coronary artery dissection as the sole presenting feature of vascular Ehlers-Danlos syndrome is an uncommon finding. We present a 33-year-old woman with sudden onset chest pain caused by a spontaneous coronary artery dissection. Genetic testing revealed vascular Ehlers-Danlos syndrome as the underlying cause. Abstract

The first 3D sonar field measurements of ripples in the intertidal area are presented. The measurements were conducted during a 6 week field campaign in Autumn 2014 at the Sand Motor near Den Hague, The Netherlands. Ripple profile, hydrodynamic and suspension data were collected along a cross-shore transect. By means of a 2D Fourier analysis the 3D sonar images of the bedforms were analysed. Key parameters such as ripple height, length and angle were obtained. These parameters correlate to one another as well as to hydrodynamic data. During the measurement period the wave conditions were mostly calm and most waves arrived shore normal. A correlation between relative wave height and ripple length is found. The estimated ripple index, of 17.1, corresponds well to values found in literature. The FFT-routines are a powerful tool for analysing the enormous amount of sonar data, 1100 3D-images, automatically. A rotational diagram method was used to analyse 3D sonar images. A ripple regularity index (RRI) is proposed, as an objective and quantitative way to classify a ripple pattern as ‘regular’ or ‘irregular’.

Item does not contain fulltext BACKGROUND: Concerns have emerged regarding a higher risk of stent thrombosis after drug-eluting stent (DES) implantation, especially in the setting of ST-segment elevation myocardial infarction (STEMI). Our objective was to perform a meta-analysis using individual patient data to evaluate the long-term safety and effectiveness of DES compared with bare-metal stents (BMS) in patients undergoing primary percutaneous coronary intervention for STEMI. DATA SOURCES: Formal searches of electronic databases (MEDLINE and CENTRAL) and scientific session presentations from January 2000 to June 2011. STUDY SELECTION: We examined all completed randomized trials of DES for STEMI. DATA EXTRACTION: Individual patient data. DATA SYNTHESIS: Individual patient data were obtained from 11 of 13 trials identified, including a total of 6298 patients (3980 [63.2%] randomized to DES [99% sirolimus-eluting or paclitaxel-eluting stents] and 2318 [36.8%] randomized to BMS). At long-term follow-up (mean [SD], 1201 [440] days), DES implantation significantly reduced the occurrence of target-vessel revascularization (12.7% vs 20.1%; hazard ratio [95% CI], 0.57 [0.50-0.66]; P < .001, P value for heterogeneity, .20), without any significant difference in terms of mortality, reinfarction, and stent thrombosis. However, DES implantation was associated with an increased risk of very late stent thrombosis and reinfarction. CONCLUSIONS: The present pooled patient-level meta-analysis demonstrates that among patients with STEMI undergoing primary percutaneous coronary intervention, sirolimus-eluting and paclitaxel-eluting stents compared with BMS are associated with a significant reduction in target-vessel revascularization at long-term follow-up. Although there were no differences in cumulative mortality, reinfarction, or stent thrombosis, the incidence of very late reinfarction and stent thrombosis was increased with these DES.

Abstract

Epstein-Barr-virus-transformed peripheral-blood B-lymphocytes were gamma-irradiated at 0 degree C at doses from 10 to 100 Gy. The cells were immediately lysed and the DNA was isolated. Subsequently, the DNA was hydrolysed to 2′-deoxyribonucleosides with a mixture of DNAase I, venom and spleen exonucleases and alkaline phosphatase. The hydrolysate was dried, trimethylsilylated and analysed by capillary gas chromatography-mass spectrometry with selected-ion monitoring. The (5′R)- and (5′S)-diastereomers of 8,5′-cyclo-2′-deoxyguanosine were observed in a ratio of 1:3, and their formation was dose-dependent. It was possible to detect and characterize one such lesion in approx. 4 X 10(4) guanine nucleotide subunits of DNA.

The aim of the current analysis is to investigate the potential gender difference in the effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI patients within the ISACS-STEMI COVID-19 Registry. METHODS: This retrospective multicenter registry was performed in high-volume primary percutaneous coronary intervention (PPCI) centers on four continents and included STEMI patients undergoing PPCIs in March-June 2019 and 2020. Patients were divided according to gender. The main outcomes were the incidence and timing of the PPCI, (ischemia time ≥ 12 h and door-to-balloon ≥ 30 min) and in-hospital or 30-day mortality. RESULTS: We included 16683 STEMI patients undergoing PPCIs in 109 centers. In 2020 during the pandemic, there was a significant reduction in PPCIs compared to 2019 (IRR 0.843 (95% CI: 0.825-0.861, p < 0.0001). We did not find a significant gender difference in the effects of the COVID-19 pandemic on the numbers of STEMI patients, which were similarly reduced from 2019 to 2020 in both groups, or in the mortality rates. Compared to prepandemia, 30-day mortality was significantly higher during the pandemic period among female (12.1% vs. 8.7%; adjusted HR [95% CI] = 1.66 [1.31-2.11], p < 0.001) but not male patients (5.8% vs. 6.7%; adjusted HR [95% CI] = 1.14 [0.96-1.34], p = 0.12). CONCLUSIONS: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures similarly observed in both genders. Furthermore, we observed significantly increased in-hospital and 30-day mortality rates during the pandemic only among females. Trial registration number: NCT 04412655.

Abstract

Background Chronic obstructive pulmonary disease (COPD) is projected to become the third cause of mortality worldwide. COPD shares several pathophysiological mechanisms with cardiovascular disease, especially atherosclerosis. However, no definite answers are available on the prognostic role of COPD in the setting of ST elevation myocardial infarction (STEMI), especially during COVID-19 pandemic, among patients undergoing primary angioplasty, that is therefore the aim of the current study. Methods In the ISACS-STEMI COVID-19 registry we included retrospectively patients with STEMI treated with primary percutaneous coronary intervention (PCI) between March and June of 2019 and 2020 from 109 high-volume primary PCI centers in 4 continents. Results A total of 15,686 patients were included in this analysis. Of them, 810 (5.2%) subjects had a COPD diagnosis. They were more often elderly and with a more pronounced cardiovascular risk profile. No preminent procedural dissimilarities were noticed except for a lower proportion of dual antiplatelet therapy at discharge among COPD patients (98.9% vs. 98.1%, P = 0.038). With regards to short-term fatal outcomes, both in-hospital and 30-days mortality occurred more frequently among COPD patients, similarly in pre-COVID-19 and COVID-19 era. However, after adjustment for main baseline differences, COPD did not result as independent predictor for in-hospital death (adjusted OR [95% CI] = 0.913[0.658–1.266], P = 0.585) nor for 30-days mortality (adjusted OR [95% CI] = 0.850 [0.620–1.164], P = 0.310). No significant differences were detected in terms of SARS-CoV-2 positivity between the two groups. Conclusion This is one of the largest studies investigating characteristics and outcome of COPD patients with STEMI undergoing primary angioplasty, especially during COVID pandemic. COPD was associated with significantly higher rates of in-hospital and 30-days mortality. However, this association disappeared after adjustment for baseline characteristics. Abstract

Background The fear of contagion during the coronavirus disease-2019 (COVID-19) pandemic may have potentially refrained patients with ST-segment elevation myocardial infarction (STEMI) from accessing the emergency system, with subsequent impact on mortality. Objectives The ISACS-STEMI COVID-19 registry aims to estimate the true impact of the COVID-19 pandemic on the treatment and outcome of patients with STEMI treated by primary percutaneous coronary intervention (PPCI), with identification of “at-risk” patient cohorts for failure to present or delays to treatment. Methods This retrospective registry was performed in European high-volume PPCI centers and assessed patients with STEMI treated with PPPCI in March/April 2019 and 2020. Main outcomes are the incidences of PPCI, delayed treatment, and in-hospital mortality. Results A total of 6,609 patients underwent PPCI in 77 centers, located in 18 countries. Abstract

ObjectiveThe initial data of the International Study on Acute Coronary Syndromes - ST Elevation Myocardial Infarction COVID-19 showed in Europe a remarkable reduction in primary percutaneous coronary intervention procedures and higher in-hospital mortality during the initial phase of the pandemic as compared with the prepandemic period. The aim of the current study was to provide the final results of the registry, subsequently extended outside Europe with a larger inclusion period (up to June 2020) and longer follow-up (up to 30 days).MethodsThis is a retrospective multicentre registry in 109 high-volume primary percutaneous coronary intervention (PPCI) centres from Europe, Latin America, South-East Asia and North Africa, enrolling 16 674 patients with ST segment elevation myocardial infarction (STEMI) undergoing PPPCI in March/June 2019 and 2020. The main study outcomes were the incidence of PPCI, delayed treatment (ischaemia time >12 hours and door-to-balloon >30 min), in-hospital and 30-day mortality.ResultsIn 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio 0.843, 95% CI 0.825 to 0.861, p75 years) (p=0.015), and was not related to the peak of cases or deaths due to COVID-19. The heterogeneity among centres was high (pConclusionPercutaneous revascularisation for STEMI was significantly affected by the COVID-19 pandemic, with a 16% reduction in PPCI procedures, especially among older patients (about 20%), and longer delays to treatment, which may have contributed to the increased in-hospital and 30-day mortality during the pandemic.Trial registration numberNCT04412655.

Abstract

Background and aims SARS-Cov-2 predisposes patients to thrombotic complications, due to excessive inflammation, endothelial dysfunction, platelet activation, and coagulation/fibrinolysis disturbances. The aim of the present study was to evaluate clinical characteristics and prognostic impact of SARS-CoV-2 positivity among STEMI patients undergoing primary percutaneous coronary intervention (PPCI). Methods We selected SARS-CoV-2 positive patients included in the ISACS-STEMI COVID-19, a retrospective multicenter European registry including 6609 STEMI patients treated with PPCI from March 1st until April 30th, in 2019 and 2020. As a reference group, we randomly sampled 5 SARS-Cov-2 negative patients per each SARS-CoV-2 positive patient, individually matched for age, sex, and hospital/geographic area. Study endpoints were in-hospital mortality, definite stent thrombosis, heart failure. Results Our population is represented by 62 positive SARS-CoV-2 positive patients who were compared with a matched population of 310 STEMI patients. No significant difference was observed in baseline characteristics or the modality of access to the PCI center. Abstract

Background: The constraints in the management of patients with ST-segment elevation myocardial infarction (STEMI) during the COVID-19 pandemic have been suggested to have severely impacted mortality levels. The aim of the current analysis is to evaluate the age-related effects of the COVID-19 pandemic on mechanical reperfusion and 30-day mortality for STEMI within the registry ISACS-STEMI COVID-19., Methods: This retrospective multicenter registry was performed in high-volume PPCI centers on four continents and included STEMI patients undergoing PPCI in March-June 2019 and 2020. Patients were divided according to age (< or ≥75 years). The main outcomes were the incidence and timing of PPCI, (ischemia time longer than 12 h and door-to-balloon longer than 30 min), and in-hospital or 30-day mortality., Results: We included 16,683 patients undergoing PPCI in 109 centers. In 2020, during the pandemic, there was a significant reduction in PPCI as compared to 2019 (IRR 0.843 (95%-CI: 0.825-0.861, p < 0.0001). We found a significant age-related reduction (7%, p = 0.015), with a larger effect on elderly than on younger patients. Furthermore, we observed significantly higher 30-day mortality during the pandemic period, especially among the elderly (13.6% vs. 17.9%, adjusted HR (95% CI) = 1.55 [1.24-1.93], p < 0.001) as compared to younger patients (4.8% vs. 5.7%; adjusted HR (95% CI) = 1.25 [1.05-1.49], p = 0.013), as a potential consequence of the significantly longer ischemia time observed during the pandemic., Conclusions: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures, with a larger reduction and a longer delay to treatment among elderly patients, which may have contributed to increase in-hospital and 30-day mortality during the pandemic.

We did not find a significant gender difference in the effects of the COVID-19 pandemic on the numbers of STEMI patients, which were similarly reduced from 2019 to 2020 in both groups, or in the mortality rates. Compared to prepandemia, 30-day mortality was significantly higher during the pandemic period among female (12.1% vs. 8.7%; adjusted HR [95% CI] = 1.66 [1.31-2.11], p < 0.001) but not male patients (5.8% vs. 6.7%; adjusted HR [95% CI] = 1.14 [0.96-1.34], p = 0.12)., Conclusions: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a 16% reduction in PPCI procedures similarly observed in both genders. Abstract

The so-called “smoking paradox”, conditioning lower mortality in smokers among STEMI patients, has seldom been addressed in the settings of modern primary PCI protocols. The ISACS−STEMI COVID-19 is a large-scale retrospective multicenter registry addressing in-hospital mortality, reperfusion, and 30-day mortality among primary PCI patients in the era of the COVID-19 pandemic. Among the 16,083 STEMI patients, 6819 (42.3%) patients were active smokers, 2099 (13.1%) previous smokers, and 7165 (44.6%) non-smokers. Despite the impaired preprocedural recanalization (p < 0.001), active smokers had a significantly better postprocedural TIMI flow compared with non-smokers (p < 0.001); this was confirmed after adjustment for all baseline and procedural confounders, and the propensity score. Active smokers had a significantly lower in-hospital (p < 0.001) and 30-day (p < 0.001) mortality compared with non-smokers and previous smokers; this was confirmed after adjustment for all baseline and procedural confounders, and the propensity score. In conclusion, in our population, active smoking was significantly associated with improved epicardial recanalization and lower in-hospital and 30-day mortality compared with previous and non-smoking history.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is projected to become the third cause of mortality worldwide. COPD shares several pathophysiological mechanisms with cardiovascular disease, especially atherosclerosis. However, no definite answers are available on the prognostic role of COPD in the setting of ST elevation myocardial infarction (STEMI), especially during COVID-19 pandemic, among patients undergoing primary angioplasty, that is therefore the aim of the current study., Methods: In the ISACS-STEMI COVID-19 registry we included retrospectively patients with STEMI treated with primary percutaneous coronary intervention (PCI) between March and June of 2019 and 2020 from 109 high-volume primary PCI centers in 4 continents., Results: A total of 15,686 patients were included in this analysis. Of them, 810 (5.2%) subjects had a COPD diagnosis. They were more often elderly and with a more pronounced cardiovascular risk profile. No preminent procedural dissimilarities were noticed except for a lower proportion of dual antiplatelet therapy at discharge among COPD patients (98.9% vs. 98.1%, P = 0.038). With regards to short-term fatal outcomes, both in-hospital and 30-days mortality occurred more frequently among COPD patients, similarly in pre-COVID-19 and COVID-19 era. However, after adjustment for main baseline differences, COPD did not result as independent predictor for in-hospital death (adjusted OR [95% CI] = 0.913[0.658-1.266], P = 0.585) nor for 30-days mortality (adjusted OR [95% CI] = 0.850 [0.620-1.164], P = 0.310). No significant differences were detected in terms of SARS-CoV-2 positivity between the two groups., Conclusion: This is one of the largest studies investigating characteristics and outcome of COPD patients with STEMI undergoing primary angioplasty, especially during COVID pandemic. COPD was associated with significantly higher rates of in-hospital and 30-days mortality. However, this association disappeared after adjustment for baseline characteristics. 42. Multifunctional Core-Shell Microgels as Pd-Nanoparticle Containing Nanoreactors With Enhanced Catalytic Turnover.

Abstract

In this work, we present core-shell microgels with tailor-made architecture and properties for the incorporation of palladium nanoparticles. The microgel core consists of poly- N -isopropylacrylamide (PNIPAM) copolymerized with methacrylic acid (MAc) as anchor point for the incorporation of palladium nanoparticles. The microgel shell is prepared by copolymerization of NIPAM and the UV-sensitive comonomer 2-hydroxy-4-(methacryloyloxy)-benzophenone (HMABP). The obtained core-shell architecture was analyzed by means of photon correlation spectroscopy, while the incorporated amount of HMABP was further confirmed via Fourier transform infrared spectroscopy. Subsequently, the microgel system was used for loading with palladium nanoparticles and their size and localization were investigated by transmission electron microscopy. The catalytic activity of the monodisperse palladium nanoparticles was tested by reduction of 4-nitrophenol to 4-aminophenol. The obtained reaction rate constants for the core-shell system showed enhanced activity compared to the Pd-loaded bare core system. Furthermore, it was possible to recycle the catalyst several times. Abstract

Background: During the coronavirus disease 2019 (COVID-19) pandemic, concerns have been arisen on the use of renin-angiotensin system inhibitors (RASI) due to the potentially increased expression of Angiotensin-converting-enzyme (ACE)2 and patient's susceptibility to SARS-CoV2 infection. Diabetes mellitus have been recognized favoring the coronavirus infection with consequent increase mortality in COVID-19. No data have been so far reported in diabetic patients suffering from ST-elevation myocardial infarction (STEMI), a very high-risk population deserving of RASI treatment., Methods: The ISACS-STEMI COVID-19 registry retrospectively assessed STEMI patients treated with primary percutaneous coronary intervention (PPCI) in March/June 2019 and 2020 in 109 European high-volume primary PCI centers. This subanalysis assessed the prognostic impact of chronic RASI therapy at admission on mortality and SARS-CoV2 infection among diabetic patients., Results: Our population is represented by 3812 diabetic STEMI patients undergoing mechanical reperfusion, 2038 in 2019 and 1774 in 2020. Among 3761 patients with available data on chronic RASI therapy, between those ones with and without treatment there were several differences in baseline characteristics, (similar in both periods) but no difference in the prevalence of SARS-CoV2 infection (1.6% vs 1.3%, respectively, p  = 0.786). Considering in-hospital medication, RASI therapy was overall associated with a significantly lower in-hospital mortality (3.3% vs 15.8%, p  < 0.0001), consistently both in 2019 and in 2010., Conclusions: This is first study to investigate the impact of RASI therapy on prognosis and SARS-CoV2 infection of diabetic patients experiencing STEMI and undergoing PPCI during the COVID-19 pandemic. Abstract

In this study we use poly( N -isopropylacrylamide) (PNIPAM) based copolymer microgels to create free-standing, transferable, thermoresponsive membranes. The microgels are synthesized by copolymerization of NIPAM with 2-hydroxy-4-(methacryloyloxy)-benzophenone (HMABP) and spin-coated on Si wafers. After subsequent cross-linking by UV-irradiation, the formed layers easily detach from the supporting material. We obtain free standing microgel membranes with lateral extensions of several millimetres and an average layer thickness of a few hundred nanometres. They can be transferred to other substrates. As one example for potential applications we investigate the temperature dependent ion transport through the membranes via resistance measurements revealing a sharp reversible increase in resistance when the lower critical solution temperature of the copolymer microgels is reached. In addition, prior to cross-linking, the microgels can be decorated with silver nanoparticles and cross-linked afterwards. Abstract

CBD is a promising candidate for treatment of many diseases and plays a major role in the growing trend to produce high-end drugs from natural, renewable resources. In the present work, we demonstrate a way to incorporate the anti-inflammatory drug CBD into smart microgel particles. The copolymer microgels that we chose as carrier systems exhibit a volume phase transition temperature of 39 ∘C, which is just above normal body temperature and makes them ideal candidates for hyperthermia treatment. While a simple loading route of CBD was not successful due to the enormous hydrophobicity of CBD, an alternative route was developed by immersing the microgels in ethanol. Despite the expected loss of thermoresponsive behaviour of the microgel matrix due to the solvent exchange, a temperature-dependent release of CBD was detected by the material, creating an interesting question of interactions between CBD and the microgel particles in ethanol. Furthermore, the method developed for loading of the microgel particles with CBD in ethanol was further improved by a subsequent transfer of the loaded particles into water, which proves to be an even more promising approach due to the successful temperature-dependent release of the drug above the collapse temperature of the microgels.

46. Importance of pH in Synthesis of pH-Responsive Cationic Nano- and Microgels.

Abstract

While cationic microgels are potentially useful for the transfection or transformation of cells, their synthesis has certain drawbacks regarding size, polydispersity, yield, and incorporation of the cationic comonomers. In this work, a range of poly( N -isopropylacrylamide) (PNIPAM) microgels with different amounts of the primary amine N -(3-aminopropyl)methacrylamide hydrochloride (APMH) as the cationic comonomer were synthesized. Moreover, the pH-value during reaction was varied for the synthesis of microgels with 10 mol% APMH-feed. The microgels were analyzed by means of their size, thermoresponsive swelling behavior, synthesis yield, polydispersity and APMH-incorporation. The copolymerization of APMH leads to a strong decrease in size and yield of the microgels, while less than one third of the nominal APMH monomer feed is incorporated into the microgels. With an increase of the reaction pH up to 9.5, the negative effects of APMH copolymerization were significantly reduced. Above this pH, synthesis was not feasible due to aggregation. The results show that the reaction pH has a strong influence on the synthesis of pH-responsive cationic microgels and therefore it can be used to tailor the microgel properties.

Abstract

Background: It has been suggested the COVID pandemic may have indirectly affected the treatment and outcome of STEMI patients, by avoidance or significant delays in contacting the emergency system. No data have been reported on the impact of diabetes on treatment and outcome of STEMI patients, that was therefore the aim of the current subanalysis conducted in patients included in the International Study on Acute Coronary Syndromes-ST Elevation Myocardial Infarction (ISACS-STEMI) COVID-19., Methods: The ISACS-STEMI COVID-19 is a retrospective registry performed in European centers with an annual volume of > 120 primary percutaneous coronary intervention (PCI) and assessed STEMI patients, treated with primary PCI during the same periods of the years 2019 versus 2020 (March and April). Main outcomes are the incidences of primary PCI, delayed treatment, and in-hospital mortality., Results: A total of 6609 patients underwent primary PCI in 77 centers, located in 18 countries. Diabetes was observed in a total of 1356 patients (20.5%), with similar proportion between 2019 and 2020. During the pandemic, there was a significant reduction in primary PCI as compared to 2019, similar in both patients with (Incidence rate ratio (IRR) 0.79 (95% CI: 0.73-0.85, p < 0.0001) and without diabetes (IRR 0.81 (95% CI: 0.78-0.85, p < 0.0001) (p int = 0.40). We observed a significant heterogeneity among centers in the population with and without diabetes (p < 0.001, respectively). The heterogeneity among centers was not related to the incidence of death due to COVID-19 in both groups of patients. Interaction was observed for Hypertension (p = 0.024) only in absence of diabetes. Furthermore, the pandemic was independently associated with a significant increase in door-to-balloon and total ischemia times only among patients without diabetes, which may have contributed to the higher mortality, during the pandemic, observed in this group of patients., Conclusions: The COVID-19 pandemic had a significant impact on the treatment of patients with STEMI, with a similar reduction in primary PCI procedures in both patients with and without diabetes. Hypertension had a significant impact on PCI reduction only among patients without diabetes. We observed a significant increase in ischemia time and door-to-balloon time mainly in absence of diabetes, that contributed to explain the increased mortality observed in this group of patients during the pandemic., Trial Registration Number: NCT04412655.

Background: The fear of contagion during the coronavirus disease-2019 (COVID-19) pandemic may have potentially refrained patients with ST-segment elevation myocardial infarction (STEMI) from accessing the emergency system, with subsequent impact on mortality., Objectives: The ISACS-STEMI COVID-19 registry aims to estimate the true impact of the COVID-19 pandemic on the treatment and outcome of patients with STEMI treated by primary percutaneous coronary intervention (PPCI), with identification of "at-risk" patient cohorts for failure to present or delays to treatment., Methods: This retrospective registry was performed in European high-volume PPCI centers and assessed patients with STEMI treated with PPPCI in March/April 2019 and 2020. Main outcomes are the incidences of PPCI, delayed treatment, and in-hospital mortality., Results: A total of 6,609 patients underwent PPCI in 77 centers, located in 18 countries. In 2020, during the pandemic, there was a significant reduction in PPCI as compared with 2019 (incidence rate ratio: 0.811; 95% confidence interval: 0.78 to 0.84; p < 0.0001). The heterogeneity among centers was not related to the incidence of death due to COVID-19. A significant interaction was observed for patients with arterial hypertension, who were less frequently admitted in 2020 than in 2019. 49. Resolving the internal morphology of core-shell microgels with super-resolution fluorescence microscopy.

Abstract

We investigate the internal morphology of smart core-shell microgels by super-resolution fluorescence microscopy exploiting a combination of 3D single molecule localization and structured illumination microscopy utilizing freely diffusing fluorescent dyes. This approach does not require any direct chemical labeling and does not perturb the network structure of these colloidal gels. Hence, it allows us to study the morphology of the particles with very high precision. We found that the structure of the core-forming seed particles is drastically changed by the second synthesis step necessary for making the shell, resulting in a core region with highly increased dye localization density. 50. Tuning the Swelling Properties of Smart Multiresponsive Core-Shell Microgels by Copolymerization.

Abstract

The present study focuses on the development of multiresponsive core-shell microgels and the manipulation of their swelling properties by copolymerization of different acrylamides-especially N -isopropylacrylamide (NIPAM), N -isopropylmethacrylamide (NIPMAM), and NNPAM-and acrylic acid. We use atomic force microscopy for the dry-state characterization of the microgel particles and photon correlation spectroscopy to investigate the swelling behavior at neutral (pH 7) and acidic (pH 4) conditions. A transition between an interpenetrating network structure for microgels with a pure poly- N,-n -propylacrylamide (PNNPAM) shell and a distinct core-shell morphology for microgels with a pure poly- N -isopropylmethacrylamide (PNIPMAM) shell is observable. The PNIPMAM molfraction of the shell also has an important influence on the particle rigidity because of the decreasing degree of interpenetration. Furthermore, the swelling behavior of the microgels is tunable by adjustment of the pH-value between a single-step volume phase transition and a linear swelling region at temperatures corresponding to the copolymer ratios of the shell. This flexibility makes the multiresponsive copolymer microgels interesting candidates for many applications, e.g., as membrane material with tunable permeability.
