To study library guides, as published on Springshare's LibGuides platform, new approaches are needed to expand the scope of the research, ensure comprehensiveness of data collection, and reduce bias for content analysis. Computational methods can be utilized to conduct a nuanced and thorough evaluation that critically assesses the resources promoted in library guides. Web-based library guides are curated by librarians to provide easy access to high-quality information and resources in a variety of formats to support the research needs of their users. Recent scholarship considers library guides as valuable resources and as de facto publications, highlighting the need for critical study. In this article, the authors present a novel model for comprehensively gathering data about a specific genre of books from individual LibGuide pages and applying computational methods to explore the resultant data. Beginning with a pre-selected list of 159 books, we programmatically queried the titles using the LibGuides Community search engine. After cleaning and filtering the resultant data, we compiled a list of 20,484 book references (of which 6,212 are unique) on 1,529 LibGuide pages. By testing against inclusion and exclusion criteria to ensure relevancy, we identified a total of 281 titles relevant to our topic. To gain insights for future study, citation analysis metrics are presented to reveal patterns of frequency, co-occurrence, and bibliographic coupling of books promoted in LibGuides. This proof-of-concept could be adopted for a variety of applications, including assessment of collections, public services, critical librarianship, and other complex questions to enable a richer and more thorough understanding of the information landscape of LibGuides. Despite ongoing efforts to improve database accessibility, aggregated database vendors concede that they do not have complete control over document accessibility. Instead, they point to the responsibility of journal publishers to deliver articles in an accessible format. This may increase the likelihood that users with disabilities will encounter articles that are not compatible with a screen reader. To better understand the extent of the problem, a document accessibility audit was conducted of randomly selected articles from EBSCO's Library & Information Source database. Full-text articles from 12 library science journals were evaluated against two measures of screen reader compatibility: HTML format (the optimal format for screen readers) and PDF accessibility conformance. Findings showed inconsistencies in HTML format availability for articles in the selected journals. Additionally, the entire sample of PDF articles failed to meet the minimum standard of PDF Universal Accessibility of containing a tagged structure. Technology in libraries has played an essential role in serving today's communities. This study provides an overview of the integrated library systems/software (ILSs) used in libraries in South Sulawesi, Indonesia. It aims to highlight the strengths and possibilities of ILSs and briefly explain their advantages and disadvantages along with the cost of implementation. The data was gathered from questionnaires sent via an online survey and from direct interviews with certain academic libraries over the period of 2019 to 2020. Fifty-three of 67 libraries that fulfilled the study have implemented an ILS. To deeply understand the application, a direct interview with some libraries was conducted to learn the advantages and disadvantages. The result of the study showed that the most used ILSs are SLiMS and INLISlite and other programs like Apollo, Athenium Light, Simpus, Spektra, Jibas, KOHA, and Openlibrary. The budget spent is an average of300 USD. While the ILSs have helped these libraries improve services, IT expertise and adequate resources are needed, especially when the systems present problems. An easy-to-use system that costs less will potentially be used in this area of research. This study will be particularly helpful for any library in Indonesia. This paper discusses how the William G. Squires Library conducted a usability study on their LibGuides to identify student design preferences. The results of the study support previous research concerning best practices for LibGuide creation. This paper offers insight into how librarian-centric design hinders student success and provides suggestions for how to best develop a student-centric template design that offers consistency and increased user proficiency across all guides. Universities and scientific educational institutions today need targeted information services to ensure that their user communities have the information they need. This study aims to design, develop, implement, and evaluate a mobile application for academic library services at Tarbiat Modares University (Tehran, Iran). A four-stage process was utilized to accomplish this aim. In the first phase, relevant literature was reviewed to obtain appropriate data requirements for the app. A questionnaire was designed and administered to survey expert librarians on the most suitable data requirements. The second phase involved the design of the user interface and user experience with the assistance of experts, followed by the evaluation of the experience. The third phase involved the development of the app in the Android Studio environment using the Java programming language, based on the requirements identified in the first and second phases. The app was then made available to the user community. Finally, the app was evaluated in the fourth phase using a questionnaire tool. Background: Cervical pedicle screw-rod fixation presents a complex approach in spinal surgery, offering enhanced spine stabilization in variable conditions considering traumatic injuries, degenerative changes, as well as orthopaedic and oncological ailments. This technique employs small diameter screw implants strategically placed to bolster the mechanical integrity of the spine. Notably, it involves minimally invasive procedures, resulting in smaller incisions and reduced patient discomfort. This study aims to assess the effects of trapezoidal thread screws in pedicle sections of the cervical spine during flexion-extension loadings, focusing on factors such as range of motion (ROM), implant stress, and stress on adjacent bone. Methods: Utilizing CT scan data, a finite element model of the cervical spine (C2-C7 vertebrae) was prepared. Trapezoidal thread screws were integrated into a single-level pedicle screw-rod fixation at the C5-C6 vertebrae. The C2 vertebra were given a compressive load of 50 N along with a moment of 1 Nm, resulting in the immobilization of the C7. Results and Discussion: The results indicate a reduction in ROM at the C5-C6 level by 69% to 77% compared to the intact spine during flexion-extension loading, with a slight increase in ROM observed at adjacent cervical spine levels. Stress analysis revealed that the trapezoidal thread screws induced stresses ranging from 24 MPa to 29 MPa in PEEK trapezoidal screw-rod implants, which fall below the material's yield stress. Conclusions: This suggests that the trapezoidal thread profile may be advantageous in minimizing stress concentration, attributed to its larger contact area with the vertebrae bone between the threads. Background: Optimal management and surgical planning of severe bony deformities and muscle tendon unit contractures demands comprehensive evaluation of all structures including the dynamic muscle tendon length of all muscles around the joint during gait. Objectives: Present study aimed to explore dynamic muscle–tendon length for all muscles around the knee joint along with pelvis, hip, and ankle joint kinematics among adolescent children with varying crouch angle. Methods: Muscle–tendon length of 29 adolescent children with cerebral palsy with varying crouch angles was computed using a full-body musculo-skeletal model and expressed as a percentage of muscle–tendon length during walking compared to resting condition. Results: Children with knee flexion angle greater than 25° demonstrated lower anterior pelvic tilt and 11% greater muscle–tendon length of semimembranosus and biceps femoris during stance phase of gait compared to children with knee flexion angle less than 25° and typically developing children (p < 0.01). Conclusions: The findings of present study reported that routine bedside clinical evaluation in adolescent children with knee flexion angle greater than 25° revealed moderate shortening of hamstring muscle in supine position. Whereas instrumented objective evaluation of gait demonstrated lengthened hamstring muscle and reduced hip extension and relatively lower anterior pelvic tilt. Therefore, it may be valuable to add objective assessment of dynamic muscle–tendon length to kinematics of all lower-extremity joint motion during gait, in order to understand the muscle–joint interactions; particularly in children with severe crouch and plan specific, tailor-made surgical and non-surgical interventions. Background: The most effective injective treatment approach for sacroiliac joint (SIJ) pain remains unclear. Aim of this study was to quantify the safety and effectiveness of the available injective strategies to address SIJ pain. Methods: A systematic review and meta-analysis of the literature was conducted on PubMed, Scopus, and Embase databases from inception until January 2023. Inclusion criteria were studies written in English, comparative and non-comparative studies regardless of the minimum follow-up, and case series on SIJ injections. Safety and efficacy of the different injection therapies for the SIJ were quantified. A meta-analysis was conducted on the available data of the documented injective therapies. The "Checklist for Measuring Quality" by Downs and Black was used to assess the risk of bias and the quality of papers. Results: The literature search retrieved 43 papers (2431 patients): 16 retrospective case series, 2 retrospective comparative studies, 17 prospective case series, 3 prospective comparative studies, and 5 randomized controlled trials. Of the selected studies, 63% examined the effect of steroid injections, 16% of PRP injections, while 21% reported other heterogeneous treatments. The failure rate was 26% in steroid injections and 14% in PRP injections. The meta-analysis showed a statistically significant reduction in pain with the VAS score for both steroids and PRP: steroids improvement at mid-term 3.4 points (p < 0.05), at long-term 3.0 (p < 0.05), PRP improvement at mid-term 2.2 (p = 0.007), at long-term 2.3 points of the VAS pain scale (p = 0.02). Conclusions: Steroids are the most documented injective approach, with studies showing an overall safety and effectiveness. Still, the high number of failures underlined by some studies suggest the need for alternative procedures. Early PRP data showed promise, but the limitations of the current literature do not allow to clearly define the most suitable injective approach, and further studies are needed to identify the best injective treatment for SIJ patients. Background: Delayed operative fixation of acetabular fractures remains a major problem in many parts of the world. No previous studies have reported the effect of fixation delay on health-related quality of life (HRQOL). We aimed to investigate the effect of delayed operative fixation of acetabular fractures on health-related quality of life, EuroQol-5 Dimension questionnaire (EQ-5D), and other related outcomes. Methods: We retrospectively analysed 117 patients who underwent open reduction internal fixation for displaced acetabular fractures between 2014 and 2021. Patients were divided into groups based on the admission-to-surgery time (interval between injury and definitive surgery): 1–14, 15–21, and >21 days. Patients were analysed for associations between admission-to-surgery time and postoperative outcomes, including operative time, estimated blood loss, blood transfusion, postoperative complication, and quality of reduction. Eighty-five patients with a mean follow-up time of 3.94 ± 1.84 years were analysed for the association between admission-to-surgery time and conversion to total hip arthroplasty, the Modified Merle d'Aubigné and Postel score, EQ-5D score, ability to sit cross-legged, and ability to sit squat. Multivariable linear regression was used for continuous outcomes and logistic regression for categorical outcomes associated with delayed operative fixation. Results: An admission-to-surgery time > 14 days was associated with significantly higher blood loss [785 mL (236–1335), p = 0.006]. For associated fractures, an admission-to-surgery time > 21 days increased the risk of poor reduction [odds ratio (OR), 5.21 (1.42–19.11), p = 0.013]. Further, admission-to-surgery time > 21 days was associated with poor Modified Merle d'Aubigné and Postel scores [OR, 8.46 (1.48–48.29), p = 0.016], EQ-5D pain domain [OR, 3.55 (1.15–11), p = 0.028], and EQ-5D usual activity domain [OR, 4.24 (1.28–14), p = 0.018]. Conclusion: Delayed operative fixation of acetabular fractures after 21 days affected the functional outcomes and HRQOL, independent of the reduction status. Background and Purpose: Scaphoid waist fractures are often stabilised with compression screws, Kirschner wires (K-wires), or a combination of both. While clinical and bio-mechanical studies evaluating their utility are available, the ideal configuration of implant that would provide adequate stability to permit early use of the hand is debatable. We examined configurations of a single screw, one screw along with a K-wire, and two K-wires used for a transverse scaphoid waist fracture fixation aiming to assess the stability provided by each in the immediate postoperative period. Methods: Computer-aided design (CAD) models of the scaphoid, K-wire, and headless compression screw were created. A transverse fracture was created at the scaphoid waist, and the CAD models of the screw and K-wire were used to fix the fracture in different configurations in a distal to proximal direction. Finite Element Analysis (FEA) was used to examine the strength of configurations when they were subjected to compression and distraction forces. The total maximum deformation (TDef) and factor of safety (FoS) for each configuration were calculated and used as indirect indicators of postoperative stability. Results: When a single screw was used, the configurations with the screw directed posteriorly from either centre or anterior had the best combined TDef and FoS values. For one screw and one K-wire, the configuration with screw and K-wire parallel to each other with the screw located along the long axis in the AP projection and anterior to the K-wire in the lateral projection had the best combined TDef and FoS values. When using two K-wires, configurations with the two wires diverging proximally on the lateral projection had the best combined TDef and FoS values. Conclusions: When fixing a transverse scaphoid waist fracture with a single screw, the screw directed posteriorly from either the centre or anterior aspect of the distal pole has the best stability, a parallel configuration has the best stability when fixing it using a screw and a K-wire, and divergent configuration has the best stability when fixing it with two K-wires only. Study Background: Mechanical alignment has always been considered as the gold standard in total knee arthroplasty (TKA), but various other coronal alignment strategies have been proposed to enhance native knee kinematics and thus elevate patient satisfaction levels. Coronal plane alignment of the knee (CPAK) classification introduced by MacDessi is a simple yet comprehensive system to classify knees based on their coronal plane alignment. It categorizes knees into nine phenotypes based on medial proximal tibial angle (MPTA) and lateral distal femoral angle (LDFA). Materials and Methods: This study investigates the distribution of classification of primary arthritic knees (CPAK) types among arthritic knees in the South Indian population and compares the functional outcomes following total knee arthroplasty (TKA) using traditional mechanical alignment among various CPAK types. The research, spanning from September 2021 to August 2023, encompasses a comprehensive analysis of 324 patients with 352 knees in the first part and 48 patients with 72 knees in the second part of the study who underwent TKA, incorporating demographic data and radiological evaluations. Results: Results indicate a predominant distribution of CPAK type 1, followed by type 2 and type 4 among the South Indian population. In the functional outcomes analysis, regardless of CPAK type, patients exhibited significant improvements in Knee Injury and Osteoarthritis Outcome Score (KOOS), Oxford Knee Score (OKS), and visual analog scale (VAS) scores post-operatively. Conclusion: CPAK distribution among the South Indian population is comparable to other Indian study and studies with an Asian population, but varies with studies among the White population. Significant improvement of functional outcome among all CPAK types signifies the robust nature of conventional mechanical alignment strategy. Thus, our study serves as an initial exploration into the knee phenotype of the South Indian population and findings contribute to ongoing research on optimal alignment strategies in knee arthroplasty, paving the way for future, more extensive studies in this dynamic field. Study design: The GAP score predicted post-operative mechanical complications more effectively whereas SRS-Schwab classification improved evaluation of postoperative PROMs. Objective: The study compared the GAP Score and SRS-Schwab Classification in predicting surgical outcomes for adult spinal deformity (ASD) and elucidated whether both systems should be included in the preoperative planning. Materials and methods: Radiographic measurements and health-related quality of life scores at baseline, 6 weeks after surgery, and the last follow-up were collected from a cohort of 69 ASD patients subjected to long segment spinal fusion surgery after they were grouped by GAP score and SRS-Schwab classification respectively. Fisher's exact test and receiver operator characteristic (ROC) curve analysis was used to compare the incidence of mechanical complications and the discriminant capacity during revision surgery between the two groups. Postoperative patient-reported outcomes measures (PROMs) were compared by one-way ANOVA, and the proportions of MCID achieved for PROMs compared by chi-square test between the two groups. Results: The overall incidence of mechanical complications and revision surgery were 42% and 8.7%. Both GAP score and its categories predicted mechanical complications and revision surgery, but the GAP score system could not predict the improvements of PROMs. The SRS-Schwab classification could predict the occurrence of postoperative mechanical complications and improvements of postoperative PROMs between the aligned, moderately misaligned and severely misaligned groups (P < 0.05). Objectives: The acetabular cup design plays a critical role in reducing contact stress between femur head acetabular cup. Many studies used ellipsoidal and spheroidal geometry in acetabular cup design to effectively reduce contact stress. The present study focuses on elevated acetabular cup rim with round corner design to reduce contact stress with round corner geometry. Methods: The cobalt chromium femur head and cup are considered for finite element (FE) model of hip resurfacing. The gait loads of routine activities of humans like normal walking, stair ascending and descending and sitting down and getting up gait activities are applied to the developed 3D FE model. Five microseparations of 0.5, 1, 1.5, 2 and 2.5 mm are considered in the present study. The acetabular cup inclination angle considered for this study are 35°, 45°, 55°, 65° and 75°. The contact stress and von Mises stress plot for each gait activities under these microseparations are analyzed for betterment of longevity of implants. Results: Overall elevated cup rim design helped in reducing contact stress to a greater extent than conventional cup with different geometries. Also, the predicted von Mises stress for all the parameters considered in the current study are well within the yield strength of CoCr material. Background: Anterior fracture dislocation of the shoulder may have associated injury to the glenoid labrum and/or capsule. There is a need to look at the possible patterns of capsulo-labral injuries intra-operatively and correlate them with pre-operative radiographs and CT scans. It can enable surgeons to predict the patterns of soft-tissue injuries in a specific fracture type. Methods: Patients operated for ORIF of 3- and 4-part anterior fracture dislocation between January 2017 and December 2022 were included in the study. The following data were collected: demographic details, fracture morphology and classification on radiograph and CT scan, neurovascular injury, the subtype of anterior dislocation, and intra-operative identifiable injury to glenoid labrum and/or capsule. Results: A total of 24 cases (mean age 40 ± 10 years) of anterior fracture dislocation of the shoulder were included in the study and were grouped based on Neer's classification. 12 patients had 3-part anterior fracture dislocation (3-part Group) with a mean age of 34 ± 9 years, while 12 patients had 4-part anterior fracture dislocation (4-part Group) with a mean age of 46 ± 7 years (p = 0.001). 10 out of 12 patients in the 3-part Group had the subcoracoid position of the dislocated head, whereas the head was present in 9 out of 12 patients of the 4-part Group at the preglenoid position (p = 0.003). The head position was at a mean distance of 1.85 cm from the anterior glenoid rim in the 4-part Group, whereas the head was engaged at the anterior rim of the glenoid in the 3-part Group. The medial metaphyseal beak was present in 11 out of 12 patients in the 3-part Group, whereas it was absent in all the cases of the 4-part Group (p = 0.001). 92% of the included patients in our study had capsulo-labral injury. 11 out of 12 patients in the 3-part Group had labral injury compared to 10 out of 12 patients in the 4-part Group with severe capsular stripping (p = 0.001). Conclusion: The 3-part fracture dislocation with radiological findings such as the long medial metaphyseal beak and subcoracoid position of the dislocated head had a strong association with a glenoid labral injury, whereas the 4-part fracture dislocation type had a widely displaced preglenoid position of the dislocated head with an absent metaphyseal beak and had a strong association with a capsular type of soft-tissue injury. Background: Clubfoot, also known as congenital talipes equinovarus is a prevalent childhood ailment that, if untreated, can lead to long-term discomfort and impairment. The Ponseti technique, aimed at achieving corrected foot positioning, is widely accepted. This study investigated the influence of clubfoot severity (Pirani score), patient age, and initial manipulation and casting weight on the overall number of casts needed for effective treatment. Methods: A prospective study was carried out involving 40 idiopathic clubfoot patients where manipulation and casting were performed following the Ponseti method. Patient age, weight, and foot Pirani score were evaluated concerning the total number of casts needed to achieve 70° of abduction. Results: The majority of participants were male infants aged 4–6 months, with a median age of 4.5 months. Half of them weighed between 3.00 and 4.99 kg at their first hospital visit, with a median weight of 3.20 kg. Bilateral clubfoot involvement was common (57.5%). Children with Pirani HFCS 2.0–3 often required percutaneous tenotomy (87.5%, p value 0.05). On average, patients needed 5 casts (range 3–9). The mean total Pirani scores were 4.71 for the right foot and 4.61 for the left foot. The study reported a positive correlation between higher total Pirani scores and the required number of casts. Conclusions: The Pirani scoring system is highly accurate in assessing clubfoot severity and predicting treatment success. Purpose: This study examines the influence of preoperative fatty infiltration (FI) of the subscapularis tendon (SBS) on outcomes following reverse total shoulder arthroplasty (rTSA) with SBS repair. Methods: A cohort of 161 rTSA patients with SBS repair, followed for a mean of 45.3 months, was divided into three groups based on FI: Group A (intact upper and lower portions, n = 85), Group B (intact lower portions, n = 44), and Group C (fatty infiltrated in both portions, n = 32). The mean age was 74.5 years (range: 65–95). Results: Preoperative FI displayed significant disparity among the groups: Group A (1.18 ± 0.60), Group B (2.95 ± 0.56), and Group C (4.0 ± 0.00) (p <.001). Group A exhibited a more positive trend in activities of daily living, particularly in toileting ability (81% in Group A, 68% in Group B, and 72% in Group C), although without statistical significance (p = 0.220). Complication rates varied: Group A had seven acromial fractures (8%), three cases of instability (3%), and six instances of scapular notching (7%). Group B experienced four acromial fractures (9%) and four cases of scapular notching (9%), while Group C had only one case of scapular notching (3%) (p = 0.733). Conclusion: In cases characterized by favorable preoperative SBS quality, there was an elevation in functional internal rotation (IR) post-surgery, accompanied by an increased incidence of postoperative complications. Background: Drug resistant (DR) osteoarticular TB (OATB) is a challenge in view of it being deep seated lesion and paucibacillary disease. Case definition, investigation protocol, treatment of proven DR and those cases where DR could not be demonstrated lacks clarity and evidence. Hence, a series of studies were conducted to develop an algorithm to investigate and treat therapeutically refractory disease (TRD) or presumptive drug resistance (PDR) cases of OATB. Patients and methods: 6 studies were conducted. Study one and two evaluated criteria to label TRD/PDR. Three subsequent studies were conducted where TDR/PDR or fresh cases of OATB cases were investigated by AFB smear, Bactec/liquid culture, histology and genotypic DST by CBNAAT & LPA. Sixth study was a retrospective evaluation of all DR cases treated for proven or clinical drug resistance (CDR). Results: Patient of bone/spine TB on ATT for 5 months or more show poor clinico-radiological treatment response as worsening of lesion, increased spinal deformity, persistent discharging sinus/ulcer, appearance of fresh lesion, recurrence of previous lesion, wound dehiscence of post-operative surgical scar cab labelled as PDR cases. These cases on histology ascertained TB and were proven DR on genotypic and phenotypic DST and are treated successfully. The patients of histologically ascertained TB and no/indeterminate phenotypic and genotypic DST were successfully treated as clinical drug resistance on MDR protocol. Conclusions: We described an algorithm. We must suspect PDR(TRD) based on criteria described. The tissue must be procured and submitted for AFB smear, histology and phenotypic and genotypic DST for diagnosis of TB. Genotypic and phenotypic DST will be useful to prove (90% instances) type of drug resistance. Remaining on strong clinical suspicion of DR and yet inconclusive on phenotypic/genotypic DST (<10%), may be treated as CDR as MDR. Background: Hemiarthroplasty (HA) is a common form of treatment for displaced neck of femur fractures. There is ongoing debate as to whether cemented or uncemented HA is a more superior treatment modality. The aim of this study was to compare the outcomes between patients that underwent cemented HA to uncemented HA. Secondarily, we analysed the costs associated of each treatment option. Methods: This was a retrospective study conducted at a busy district general hospital. The study included 335 patients that were treated with either a cemented or uncemented HA for a displaced neck of femur fracture between January 2017 and December 2018. Data collected included age, sex, American Society of Anesthesiologist (ASA) score, treatment modality, length of stay (LOS) and general costs. Results: 197 (58.8%) of the cohort underwent cemented HA and 138 (41.2%) underwent uncemented HA. Mean age for the cemented cohort was 84.7 years and 85.9 years in the uncemented group (p = 0.31). There was no significant differences between the groups with regard to mean LOS and discharge disposition (p = 0.44). There were no significant difference in 30-day and 1-year mortality between the two groups (p = 0.2). We did find a statistically significant difference in the costings between the two procedures, with cemented HAs costing £66 more than uncemented HAs (p < 0.001). Conclusion: We found that both cemented and uncemented HAs produced comparable results. We found a statistically significant reduction in operative time and costs associated with uncemented HA. Objectives: The aim of this study was to investigate the intrarater and interrater reliability and validity of range of motion measurements obtained with a universal goniometer, digital inclinometer, and smartphone application in patients with total knee arthroplasty. Methods: Range of motion of the knee joint was measured by two examiners with a universal goniometer, digital inclinometer, and a smartphone application. Data were obtained from 51 knees of 27 patients at postoperative 6 months. Two measurements made by the first examiner were compared to assess interrater reliability, and measurements from both examiners were compared to assess intrarater reliability. Statistical analysis was performed using intraclass correlation coefficient (ICC) and Spearman's rho values. Results: With all three methods, active and passive knee flexion range of motion measurements showed high intrarater and interrater reliability (ICC = 0.749–0.949). Concurrent validity analysis also demonstrated statistically significant, moderate to strong correlation among the three methods (r = 0.775–0.941). Conclusion: The universal goniometer, digital inclinometer, and smartphone application were all found to be reliable and valid assessment tools in clinical practice for patients with total knee arthroplasty. Objectives: The aim of this study to investigate the safety and effectiveness of performing the hanging arm test during surgical treatment for elbow varus posteromedial rotatory instability (VPMRI) to assess elbow stability and determine whether to repair the lateral ulnar collateral ligament (LUCL). Methods: In a retrospective study from August 2014 to March 2019, 27 patients with VPMRI who had a negative result in the hanging arm test after fixation of coronoid fracture were selected. Intraoperative bleeding, operative time, elbow range of motion (ROM), and complications were recorded. Elbow function was evaluated with the Mayo elbow performance score (MEPS) and the disabilities of the arm, shoulder, and hand (DASH) score. Results: The operation time was 85.9 ± 11.06 min (range 65–110). The intraoperative blood loss was 70.7 ± 9.31 ml (range 60–100). At the last follow-up, the elbow joint averaged 73.8° ± 2.931° in pronation, 78.9° ± 2.941° in supination, 7.2° ± 3.207° in extension, and 123.3° ± 6.651° in flexion. The MEPS score was 90.7 ± 4.36 (range 74–95), and the DASH score was 9.8 ± 2.58 (range 6.67–13.3). One patient presented with symptoms of ulnar nerve entrapment 2 months after operation and was treated with ulnar nerve release. The symptom of numbness went away completely 1 week after operation. No complications such as wound infection, arthritis, or chronic instability of the elbow were found in the other patients. Conclusion: Our findings suggest that not all VPMRI patients need the LUCL to be repaired, and the hanging arm test is a safe and reliable method to assess whether to repair the LUCL in the treatment of elbow VPMRI. Background: Pes planovalgus is one of the most common pediatric skeletal deformities. There have been no studies to analyze in detail the spatiotemporal variables of gait following arthroereisis. Purpose of the study: The purpose of our study was to assess gait parameters in patients with symptomatic flexible flatfoot following treatment with the talus screw. Methods: This was a prospective study assessing the 22 patients treated surgically due to symptomatic flexible flatfoot with the talus screw. Patients underwent gait assessment with a G-Sensor. We analyzed the following gait parameters: gait cycle duration, step length, support phase duration, swing phase duration, double support duration, single support duration, cadence, velocity, step length. Results: The post-operative gait parameter assessment for the operated and non-operated foot showed a significant difference only in terms of step length. Cadence increased from the pre-operative mean of 82.29 steps/min to a post-operative mean of 102.94 steps/min. Gait velocity increased significantly from 0.81 m/s before to 0.96 m/s after surgery. Discussion: Arthroereisis with the talus screw helps improve gait parameters following surgery. Post-operatively, we observed increased gait velocity and cadence and decreased gait cycle duration in the operated limb. Introduction: This study presents a global research scenario in the broad domain of osteoarthritis (OA) research, using quantitative and qualitative publication and citation indicators. Methods: The study is based on 45,368 global publications, sourced from the Scopus bibliographical database, covering three decades (1994–2023). We studied the performance of the top 12 developed and top 12 developing countries. The key countries, organizations and authors at national and international levels were identified. The broad subject areas and key journals contributing to global OA research were delineated, besides identifying the broad characteristics of highly cited papers in the field. Results: The United States and China were the most productive countries, while the Netherlands and Canada made the largest citation impact. Harvard Medical School and the University of Sydney made the most contribution, while Boston University and Pfizer Inc., USA registered the highest citation impact. Hunter DJ and Guermazi A were the most productive authors, while Lohmander LS, and Hochberg MC registered the highest citation impact. Osteoarthritis and Cartilage (n = 4879) and Annals of the Rheumatic Diseases (n = 786) published the maximum papers, while Arthritis and Rheumatism and Nature Reviews Rheumatology registered the largest citation impact. The highly cited papers with 100 or more citations constituted 6.25% of the total publications. Conclusions: There has been a systematic growth of publications on OA. The research on OA was mainly done in developed countries, with the maximum publications coming from the United States of America, China and Canada. Objective: Body dysmorphic disorder (BDD) represents a bridge between the fields of psychiatry and cosmetic surgery. It is a psychiatric disorder involving altered body image and has been associated with cosmetic surgery. Many studies have investigated BDD following rhinoplasty. However, their findings were inconsistent, which prompted us to conduct this systematic review to obtain strong evidence about the prevalence of BDD among rhinoplasty candidates. Methods: We searched the literature using electronic databases such as PubMed, Scopus, Web of Science, and others using keywords relevant to the study outcomes. We also manually examined the references of the included studies and relevant reviews to look for articles that might have been missed during the electronic search. Results: Nine articles met the inclusion criteria. We identified 7 other relevant articles; since they had been included in a review conducted in 2016, they were not included in the final list of articles. However, they were included in the discussion, and their findings have been compared with ours. The included articles resulted in a total sample of 712 rhinoplasty candidates. The estimated prevalence rates of BDD in these articles ranged from 22% to 52%, which was considered high by all the authors of the included studies. Objectives: Predicting the need for surgical treatment among patients with chronic rhinosinusitis (CRS) is challenging. The delta neutrophil index (DNI) has been proposed as a useful laboratory marker of immature granulocytes, which indicates infection or severe inflammation in several diseases. This study evaluated DNI as an early predictor of the need for surgery in patients with CRS. Methods: A total of 117 patients diagnosed with CRS were enrolled in this retrospective and observational study. Medical records, including symptoms data, WBC count, ESR level, LUC count, Lund-Mackay scores, and DNI, were reviewed. The receiver operating characteristic (ROC) curves were analyzed to determine the optimal cut-off values for predicting surgery. Results: Among 117 patients, 49 patients (41.9%) needed surgical intervention. The areas under the WBC, ESR, LUC, and DNI ROC curves were.571,.600,.592, and.782, respectively. The optimal cut-off value of DNI to predict surgery was.9%. The prognostic precision of DNI showed that the sensitivity was 59.2% and the specificity was 98.5%. In the analysis of risk factors, DNI levels were significantly associated with surgical intervention (odds ratio, 2.22; 95% confidence interval, 1.48-3.34; P <.01). Conclusions: The level of DNI, which reflects the severity of the disease, may be a useful predictor for determining the need for surgical intervention in patients with CRS. Objective: The present study was designed to investigate whether acellular dermal matrix (ADM) grafts could prevent Frey's syndrome (FS) and improve esthetic scores following parotidectomy. Methods: From January 2015 to December 2019, 175 patients underwent parotidectomy. We divided the patients into two groups: the ADM group and the control group. We included in each group 30 patients according to a propensity score matched analysis. Results: FS was subjective in 1 patient (3%) from the ADM group and 9 patients (30%) from the control group (P =0.015). Patients in the ADM group had a subjective esthetic score of 6.1 + 1.7 compared with 5.2 + 1.7 in the control group. The subjective esthetic score for patients in the ADM group was higher than that for patients in the control group (P =0.040). Background: Fine needle aspiration (FNA) is a common diagnostic tool used in the initial evaluation of parotid masses. In the literature, variable diagnostic accuracy of FNA is reported. Therefore, when considering clinical management of these patients, the utility of FNA is unclear. The aim of this study was to determine the capability of ultrasound-guided FNA to differentiate between benign and malignant neoplasms. Further, the way in which FNA results affect clinical decision-making was assessed. Methods: Retrospective data were collected for all patients who underwent parotidectomy at a large Canadian tertiary care center between 2011 and 2016. Patient demographics, preoperative imaging reports, preoperative FNA results, and final pathological diagnosis were analyzed. Results: Of the 199 patients who underwent parotidectomy, 184 had preoperative ultrasound-guided FNA. There were a total of 13 non-diagnostic FNAs. In diagnosing malignancy, FNA had a sensitivity and specificity of 71.4% and 98.7%, respectively. The positive predictive value (PPV) was 83.3%. The negative predictive value was 97.5%. Of the non-diagnostic FNAs, 2 out of 13 (15.4%) were deemed malignant neoplasms on final pathology. Conclusion: FNA is a useful adjunct in the work-up of parotid masses, but it should be used with caution. Objectives : Petrous bone cholesteatoma (PBC) is a rare condition of the petrous portion of the temporal bone. Treatment of choice consists of radical surgical removal, paying attention to protect the facial nerve and inner ear as far as possible. The aim of the present study was to evaluate the efficacy of modified translabyrinthine techniques in preserving hearing function and the use of the adjuvant endoscopic techniques in a group of PBC patients. Methods : This study comprised 16 cases of PBCs surgically treated in our Department. Pre- and post-operative hearing status was assessed with pure tone audiometry and speech discrimination and graded according to the Gardner- Robertson classification system. Facial function was based on the House Brackman (HB) classification. PBCs were grouped using Sanna's classification. The choice of surgical technique was based on the above findings together with preoperative evidence. Post-operative follow-up ranged from 1 to 10 years and also included Computed Tomography and Magnetic Resonance Imaging assessment. Results : PBCs were classified as follows: 37.5% infralabyrinthine; 43.75% supralabyrinthine; and 18.75% massive. Preservation of the bone conduction threshold was feasible in 62.5% of patients. For supralabyrinthine PBCs a subtotal petrosetomy was performed in all cases and hearing preservation was possible in 57% of them: an adjuvant endoscopic approach was performed in 43%. Infralabyrinthine PBCs were treated using a modified translabyrinthine approach with preservation of bone conduction in 83% of patients; an adjuvant endoscopic approach was performed in 50% cases. One patient with a massive cholesteatoma was treated by modified translabyrinthine approach, preserving a serviceable level of hearing. In all massive cases, an adjuvant endoscopic approach was performed. In 2 patients with preoperative palsy, facial nerve function showed an improvement. The follow-up period revealed evidence of limited recurrence at CT imaging in 2 patients. Conclusions : The introduction of modified surgical approaches, able to preserve the anatomical-functional structures, have shown an improvement of post-operative hearing outcomes. Background: External ear canal exostoses are usually bilateral and broad-based, secondary to external ear canal chronic cold exposure, especially water. Objectives: The objectives were to analyze the influence of the surgeon's experience on the 3-year recurrence and on the complication. We also studied the influence of prolonged exposure to cold water on the incidence of recurrence. Material and Methods: This monocentric retrospective study included 98 ears operated for canalplasty between 2009 and 2016 by nine different operators including one senior, a junior, and seven beginner surgeons. Senior, Junior, and Beginner groups were compared. Results: 3-year recurrence rate was higher in the Beginner and Junior groups than in the Senior group (69% and 38% vs 18%, P =.001). Although there was a difference between the Beginner (69%) and Junior (38%) groups, it was not significant (P =.407). Among the recurrences, 48% had continued exposure to cold water while only 7% of the ears without recidivism were still exposed (P <.001, OR = 1.25 [4.4; 36.1]). The complication rates were similar between groups, concerning pain (8% vs 30% vs 12%), per procedure perforation (17% vs 10% vs 16%), scarring disorders (25% vs 20% vs 13%), and osteitis (8% vs 0% vs 1%). Hearing was unaffected. Conclusions: This is the first study evaluating the risk of recurrence of external ear canal exostoses after canalplasty based on the surgeon's experience. Introduction: Synkinesis refers to abnormal involuntary facial movements that accompany volitional facial movements. Despite a 55% incidence of synkinesis reported in patients with enduring facial paralysis, there is still a lack of complete understanding of this debilitating condition, leading to functional limitations and decreased quality of life. 1 This article reviews the diagnostic assessment, etiology, pathophysiology, rehabilitation, and nonsurgical and surgical treatments for facial synkinesis. Methods: A PubMed and Cochrane search was done with no date restrictions for English-language literature on facial synkinesis. The search terms used were "facial," "synkinesis," "palsy," and various combinations of the terms. Results: The resultant inability to control the full extent of one's facial movements has functional and psychosocial consequences and may result in social withdrawal with a significant decrease in quality of life. An understanding of facial mimetic musculature is imperative in guiding appropriate intervention. While chemodenervation with botulinum toxin and neurorehabilitation have continued to be the primary treatment strategy for facial synkinesis, novel techniques such as selective myectomy, selective neurolysis, free-functioning muscle transfer, and nerve grafting techniques are becoming increasingly utilized in treatment regimens. Facial rehabilitation, including neuromuscular retraining, soft tissue massage, and relaxation therapy in addition to chemodenervation with botulinum toxin, remains the cornerstone of treatment. In cases of severe, intractable synkinesis and non-flaccid facial paralysis, surgical interventions, including selective neurectomy, selective myectomy, nerve grafting, or free muscle transfer, may play a more significant role in alleviating symptoms. Discussion: A multidisciplinary approach involving therapists, clinicians, and surgeons is necessary to develop a comprehensive treatment regimen that will result in optimal outcomes. Ultimately, therapy should be tailored to the severity and pattern of synkinesis, and each patient approached on a case-by-case basis. Objective: This study aims to compare the efficacy of the potassium-titanyl-phosphate (KTP) laser and cold steel surgery in treating oropharyngeal papilloma. Methods: Between 2017 and 2020, we enrolled 242 patients with oropharyngeal papilloma who were treated with either the KTP laser (n = 160) or cold steel surgery (n = 82). Patient charts were reviewed for demographic data (age and gender), pathology, anatomical location of lesions, operative duration, pain rating, residual disease, and recurrence. Results: The oropharyngeal papillomas were successfully removed in all patients, except one with a significant pharyngeal reflex. There was no significant difference in the average time for lesion resection between KTP laser and cold steel group (18.11 ± 13.96 s vs 19.43 ± 16.91 s, P >.05). However, all patients who underwent cold steel surgery experienced bleeding during the operation and required postoperative observation (about 20 min), making the total procedure time longer than that of the KTP laser procedure, which did not cause any intraoperative bleeding or require postoperative observation. After KTP laser treatment, the pain rating was.49 ±.98, whereas after cold steel surgery, it was.74 ± 1.12 (P =.058). Twenty-five samples were sent for human papillomavirus (HPV) testing, and one tested positive for both HPV 6 and 11 strains, while another tested positive for HPV 16. No residual disease or recurrence was observed at the treatment sites after a long period of follow-up (M = 15.35 ± 10.79 mo; range = 6-39 mo). Conclusion: The KTP laser provided a better hemostasis effect and a good surgical field of vision during the operation, allowing the surgeon to complete the procedure in less time. Objective: Our previous study revealed that serum amyloid A (SAA) levels in polyp tissues could serve as a biomarker for the prediction of corticosteroid insensitivity in patients with chronic rhinosinusitis with nasal polyps (CRSwNP). However, their association with disease control status in the patients after endoscopic sinus surgery remains to be assessed. Methods: Polyp tissues and control uncinate process mucosa were collected from 48 patients with CRSwNP and 10 healthy control subjects. SAA expression was examined using immunohistochemistry and enzyme-linked immunosorbent assay. Receiver operating characteristic (ROC) curves were performed to determine the predictive value of SAA in nasal polyps. The clinical characteristics of 2 CRSwNP subtypes (SAAhigh and SAAlow) were evaluated. Results: The SAA expression levels in polyp tissues were significantly elevated both in non-eosinophilic and eosinophilic CRSwNP as compared to the healthy controls. In patients with CRSwNP, the tissue SAA level was significantly higher in the disease-controlled patients than those of the partly controlled and uncontrolled. ROC curve analysis revealed that a cut-off value of 114.9 ng/mL for the tissue SAA level predicted the patients with disease-controlled status with 93.33% sensitivity and 63.64% specificity (AUC =.8727, P <.001). Furthermore, The SAAhigh subgroup showed higher tissue eosinophil numbers and percentage of the disease-controlled patients compared to the SAAlow subgroup. Conclusions: Our findings suggest that measurements of SAA in polyp tissues may provide useful information for evaluating CRSwNP conditions, especially identifying the CRSwNP patients with disease-controlled status after endoscopic sinus surgery. Objective: To explore whether the different phases of the nasal cycle have a significant effect on nasal temperature, the nasal mucosal clearance rate, and levels of nasal nitric oxide (nNO) and to investigate the correlation between these nasal conditions. Methods: The study participants were divided into 2 groups: the control group and the rhinitis group. The participants' nasal temperature, cilia clearance rate, and nNO levels were measured during different phases of the nasal cycle (the congestion phase and decongestion phase) in the control group and before and after undergoing inferior turbinate ablation in the rhinitis group. Results: The temperature of the nasal cavity in the control group was significantly higher in the congestion phase than in the decongestion phase (P =.0025), while in the rhinitis group, the temperature of the nasal cavity decreased significantly after inferior turbinate ablation (P =.001). In the control group, the nasal mucosa clearance time was significantly shorter in the congestion phase than in the decongestion phase (P =.001), and in the rhinitis group, the clearance time of the nasal mucosa was significantly shortened after the operation (P =.0025). In the control group, the levels of nNO were significantly higher in the congestion phase than in the decongestion phase (P =.025), while in the rhinitis group, nNO levels decreased significantly after the operation (P =.005). Conclusion: The function of the nasal cavity changes in different phases of the nasal cycle. Therefore, when evaluating the impact of various factors on nasal function, factors associated with the nasal cycle should also be considered. Objective: The Arab Gulf is highly vulnerable to T2DM and its serious consequences. The manner in which these populations respond to such alterations in their surroundings may largely be governed by their genetic makeup. This review aimed to screen the genetic loci candidates that are associated with T2DM to assess the most prominent one in the early diagnosis of this chronic dysfunction. Methods: Variable pieces of literature were searched to assess the association between pathogenic single-nucleotide polymorphisms (SNPs) and the onset of T2DM in the Arab Gulf countries. The effects of odd ratio (OR), sample sizes, and collaborations of the captured genes of the eligible studies were analyzed. The protocol was registered in National Institute for Health Research. Results: Twenty-seven pathogenic SNPs were identified in 16 genes that were reported in 31 articles encompassing 15,982 patients and 11,976 controls. The highest numbers of conducted research were localized in Iraq and Saudi Arabia with 39% and 32%, respectively. HNF4A and TCF7L2 genes represent the most extensively studied pathogenic genes in terms of the number of individuals included and the number of T2DM-related loci, respectively. Intron SNPs exhibited the highest percentages of pathogenic loci associated with T2DM with 61%. Moderate association between the pathogenic SNPs and disease outcome was observed, but strengths and weights of association vary across studies. Conclusion: For a better understanding of the molecular etiology of T2DM, finding SNPs, and establishing a meaningful genotype-phenotype connection for complicated diabetic disorders, the cumulative relevance of identified pathogenic SNPs in Arab Gulf was shown. Objective: The relationships between carotid intima-media thickness (C-IMT) and β cell function and insulin resistance in patients with type 2 diabetes (T2D) have not been fully elucidated. This study is to investigate whether impaired glucose metabolism is etiologically associated with C-IMT in patients with T2D. Methods: The study group consisted of 490 (284 men, 206 women) participants. Venous blood specimens were obtained from all subjects for biochemical profiles after an >8-h overnight fast. C-IMT was measured as the distance between the luminal-intimal leading edge (I-line) and the medial-adventitial leading edge (M-line) on the far wall. Insulin resistance was estimated with the homeostasis model assessment of insulin resistance (HOMA-IR). The acute insulin response to arginine was calculated as the mean of the three plasma insulin levels obtained within 2, 4, and 6 min after the arginine bolus minus the pre-stimulus plasma insulin levels. Results: There was a graded increase in C-IMT values according to tertiles of HOMA-IR in men; the values of C-IMT were significantly decreased across the tertiles of acute insulin and C-peptide responses in women. Multivariate analysis revealed that HOMA-IR and age were positively associated with C-IMT among men participants, and acute insulin response and current smoking were the independent determinants of C-IMT in women. Conclusion: Early insulin response stimulated by arginine is independently associated with C-IMT in women T2D individuals, whereas insulin resistance is positively correlated with C-IMT in men T2D subjects. Background: Laparoscopic mini gastric bypass (LMGB) bariatric surgery, characterized by a single ante colic gastro-jejunostomy (GJ) anastomosis procedure, requires specialized post-surgery care. Objective: The aim of the study is to evaluate the impact of personalized counseling using nutrition health education (NHE) material, developed based on the Bariatric Analysis and Reporting Outcome System (BAROS), as compared to standard hospital care, on post-operative patient's quality of life (QoL). Methodology: A prospective randomized control trial was conducted, wherein 120 patients registered for the LMGB surgery were enrolled and followed up over 3 months after surgery. Based on alternate allocation randomization, odd number patients were allocated to the experimental group (Group E n = 60) receiving the personalized NHE material, and even number patients were allocated to the control group (Group C n = 60) receiving the standard hospital care. QoL and health outcomes data were analyzed pre- and post-surgery using SPSS-23 software. Results: Post-operative findings indicated better weight loss and improved quality of life scores in group E patients. The mean %excess weight loss at 3 months post-surgery of group E patients was 18% more compared to group C patients. Notably, the BAROS scores for group E patients were within the good category (4.80 ± 1.63) versus group C patients' scores being in the fair category (3.00 ± 1.64), representing a statistically significant difference (p < 0.001). Conclusion: Based on the type of bariatric surgery (be it restrictive, mal-absorptive, or combined procedure), a focused bariatric surgery-specific nutrition and education leads to better weight loss, resolution of comorbidity, improved quality of life, prevention of weight regains, and minimal post-surgical complications as compared to general counseling. Background: There is no randomized study comparing the isocaloric (ICHD) and hypocaloric diet (HCD) in PCOS subjects. Objective: To compare ICHD and HCD in weight-matched PCOS subjects. Methods: PCOS subjects were randomized to receive either a HCD or ICHD. Clinical, biochemical, hormonal and dietary assessments were performed at baseline, 3 months and at one year. Results: There were 168 PCOS subjects randomized to receive either ICHD (n = 84) or HCD (n = 84). We observed that while the hypocaloric diet was more effective in reducing weight, both the diets were effective in improving clinical symptoms. Around 50% patients showed improvement in clinical symptoms at 3 months and 30% were on diet alone therapy at one year in both the groups. The effect on hirsuitism was modest by both the diets at one year. The effectiveness of both the diets was similar with both the per protocol and the Intention to treat analysis; nevertheless, there was a greater loss to follow-up in patients on ICHD with high baseline caloric intakes. Conclusion: HCD and ICHD are equally effective in as many as 30% patients with mild symptoms over long term. Patients having higher caloric intakes at baseline should not be offered isocaloric diets. Objective: The present systematic review was aimed to explore the possible relationship between enterovirus (EV) infection and type 1 diabetes (T1D) as an autoimmune disease. Methods: The major international databases including PubMed, ISI Web of Science, Scopus, and Embase were searched for human and animal studies published until November 2020. Results: After comprehensive search of databases, 29 eligible studies were included in the meta-analysis. We found that all studies used at least one of the autoimmune antibodies including ICA, GADA, IA2A, and IAA to identify the pre-diabetic subjects. According to the results, the estimated odds ratio of the risk of T1D associated with EV was 1.19 (95% CI, 1.10–1.29, I2 = 80.5). Our finding also revealed that there was a positive association between EV positivity and patients with a higher risk of T1D (OR = 1.35, 95% CI = 1.29 to 1.41). Conclusion: Our findings suggest that EV infections can be a major risk factor for T1D and EV positivity is importantly associated with β-cell autoimmunity and a higher risk of T1D. Objective: Pancreatic beta cell destruction is a hallmark of type 1 diabetes (T1D), a heterogeneous disorder with a wide range of potential causes. T cell activation molecules have been shown to play an important role in the development of T1D, according to the majority of studies. Some autoimmune diseases have been linked to SNPs in the 4q27 region, specifically in the KIAA1109-interleukin 2 (IL2)-IL21 block. The purpose of this research was to look into how certain polymorphic variants in the 4q27 region are linked to T1D. Methods: We investigated whether variants in the 4q27 region could be a causal factor in T1D susceptibility. Polymorphisms of ten single-nucleotide polymorphisms (SNPs) belonging to the KIAA1109/IL21/IL2 block were studied in 255 individuals from 59 families using the Sequenom MassARRAY platform. Results: The IL21/IL2 region was found to have a significant association with T1D in Tunisian cohorts. We found that the T allele of the rs2221903 marker is disproportionately passed down from parents to their children. In addition, haplotype analyses encompassing all of the SNPs under consideration show that the GACAGGA and the shortly TT haplotypes were significantly over-transmitted from parents to their children, suggesting they may be a T1D genetic susceptibility factor in our population. Conclusion: Several autoimmune disorders (ADs) have been linked to the IL2/IL21 genes, suggesting that there is a shared genetic background that confers a common genetic predisposition across ADs. Objective: Increasing evidence revealed the role of adipokines in carbohydrates and fat metabolism. The present study was designed to evaluate the adiponectin, leptin, and resistin levels in prediabetes subjects and evaluate the relationship between these adipokines and insulin resistance. Methods: A hospital-based descriptive study was conducted for one year. 200 individuals who met the inclusion criteria were enrolled in the study. Based on the oral glucose tolerance test, the study subjects were grouped into healthy controls (n = 100), prediabetic non-obese (n = 61), and prediabetic obese (n = 39). Blood glucose estimation was done by the glucose oxidase peroxidase method. Chemiluminescent immunoassay was used for the measurement of insulin level and homeostasis model assessment-estimated insulin resistance was used for the assessment of insulin resistance. Serum adipokines levels were determined by ELISA. Statistical analysis was performed by using SPSS Software. Results: Serum adiponectin levels decreased significantly in obese prediabetes (7.21 ± 2.15 µg/ml) when compared to non-obese prediabetes (7.28 ± 2.41 µg/ml) and healthy control subjects (13.64 ± 2.88 µg/ml, p < 0.001). Serum leptin levels increased significantly in obese prediabetes (13.59 ± 2.59 ng/ml) when compared to non-obese prediabetes (9.84 ± 2.66 ng/ml) and healthy control subjects (12.28 ± 2.65 ng/ml, p < 0.001). Serum resistin levels increased significantly in obese prediabetes (17.67 ± 3.60 ng/ml) when compared to non-obese prediabetes (17.2 ± 3.93 ng/ml) and healthy control subjects (14.46 ± 4.16 ng/ml, p < 0.001). Adiponectin-leptin ratio decreased significantly in obese prediabetes (0.56 ± 0.22) when compared to non-obese prediabetes (0.79 ± 0.4) and healthy control subjects (1.15 ± 0.37; p < 0.001). Fasting insulin resistance was statistically significant (p < 0.001) in all groups. Conclusion: The present study strongly suggests that the adipokine profile is an ideal diagnostic tool to predict prediabetes and metabolic syndrome, especially among those with insulin resistance. Background: Diabetes mellitus is an immune compromised state and Covid-19 an infection associated with immune dysregulation. This study was conducted to appreciate the factors which may contribute to increased morbidity and mortality in people with Diabetes. Objective: To understand the profile of diabetes mellitus in COVID-19 patients and to ascertain their risk factors for mortality. Methods: This was a single-center, retrospective observational study conducted in a tertiary care hospital. The data of adults with established or newly diagnosed diabetes mellitus admitted with COVID-19 between April 2020 and January 2021 was analyzed in relation to their age, sex, duration of hospitalization, systolic blood pressure (SBP) at admission, presence of other comorbidities, initial fasting plasma glucose, oxygen therapy, CT severity, biochemical parameters, inflammatory markers, and hyperglycemia management, and compared between survivors and non-survivors, to ascertain the risk factors for mortality. Results: A total of 2640 adults, above 18 years of age with diabetes mellitus and COVID-19, were included. Among them, 2229 (84.4%) survived, and 411 (15.6%) died. Preexisting diabetes mellitus was recorded in 2246 patients (85.1%) and newly diagnosed in 394 patients (14.9%) with mortality of 16.8% and 8.4% respectively. Multivariate logistic regression analysis showed odds ratio (OR) of 4.33 (95% CI= 2.533–7.402) for severity in CT chest and 3.9 (95% CI= 3.108–4.895) for use of oxygen therapy, which were independently associated with in-hospital mortality in the study population and the risk was higher in adults aged more than 45 years, OR 2.035 (95% CI = 1.379–3.003). The subgroup of patients with risk factors like multiple comorbidities, fasting plasma glucose >140 mg/dl, and abnormal SBP also had higher levels of inflammatory markers and poorer outcomes. Conclusion: Based on our study, advanced age, extremes of SBP, uncontrolled fasting plasma glucose, and presence of other comorbidities like hypertension, coronary artery disease, chronic kidney disease, thyroid disorders, bronchial asthma, or COPD at admission appeared as risk factors for mortality in people with diabetes and COVID-19. Such patients had higher likelihood of elevated renal parameters, liver enzymes, C reactive protein, and more severe lung involvement, necessitating supplemental oxygen therapy. Objective: Atherogenic index of plasma (AIP) and triglyceride-glucose (TyG) index are inexpensive and non-invasive markers with high predictive value for early detection of cardiovascular disease in DM patients. Herein, dapagliflozin reduced the AIP and TyG and caused positive cardiovascular effects in patients with type 2 diabetes mellitus (T2DM). Methods: We retrospectively evaluated the data of patients aged >18 years with T2DM (n = 348; 210 [60.3%] women and 138 [37.7%] men; mean age = 59.24, standard deviation [SD] = ±10.44 years) who presented to a single-center internal medicine outpatient clinic between June 01, 2017, and December 30, 2020, and who were started on dapagliflozin as part of their treatment. Demographic data and clinical data of the patients at 0, 6, 12, and 24 months were retrieved from the electronic medical records of the hospital. Results: Hypertension was the most common comorbidity (n = 155 [48.9%] patients). AIP values measured before dapagliflozin initiation (mean = 0.68; SD, 0.33) and at 6 months (mean = 0.62; SD, 0.30) were significantly different (p < 0.00). Furthermore, TyG index values measured before initiation of medication (mean = 9.98; SD, 0.76) and at 6 months (mean = 9.73; SD, 0.71) were significantly different (p < 0.00). These differences persisted until 12 and 24 months after treatment initiation. Conclusions: Dapagliflozin administration lowered the AIP and TyG index in patients with T2DM; this may slow the atherosclerotic process and prevent the associated macrovascular complications. Objective: MODY (maturity onset diabetes of the young) is a rare, monogenic, autosomal dominant form of diabetes occurring in young (< 25 years) individuals. It has a wide phenotypic variability and can be misdiagnosed as type 1 diabetes mellitus (T1DM). Appropriate detection of MODY has therapeutic and genetic implications. This study aimed to detect MODY among long standing, clinically diagnosed T1DM patients with preserved C-peptide levels. Methods: This was a hospital-based cross-sectional study from central South India which included 100 clinically diagnosed T1DM patients (according to ADA criteria), with a duration of > 3 years. MODY probability score, urinary C-peptide to creatinine ratio (UCPCR), and relevant biochemical investigations were performed. Targeted next-generation sequencing (NGS) for MODY-related genes (13 genes) was done for individuals with UCPCR > 0.2. Results: A UCPCR value of > 0.2 (suggestive of preserved endogenous insulin secretion) was observed in eight individuals. The mean HbA1c values were lower, and the MODY probability score was higher in individuals with preserved endogenous insulin (8.07% vs 9.53% and 2.8% vs 1.5%; p-value: 0.005 and 0.004 respectively). One of the eight individuals (12.5%) had a non-pathogenic gene variant in KLF11. Conclusion: In a South Indian cohort of T1DM with preserved C-peptide, we could not find any case of MODY through targeted NGS. UCPCR can be used as a screening tool to identify cases needing genetic testing for MODY. Objective: There is scarcity of data on diabetic ketoacidosis (DKA) in patients with diabetes mellitus (DM) due to diseases of exocrine pancreas (T3DM). Methods: We did a retrospective record-review of patients admitted with DKA from 2017 to 2022. Clinical and biochemical characteristics were compared among DKA cases in type 1 DM (T1DM), type 2 DM (T2DM), and T3DM. Logistic regression was performed to find independent predictors of death and DKA complications. Results: Out of 107 cases, n = 79 were included (T1DM = 30, T2DM = 37, T3DM = 12). All cases of DKA in T3DM were of mild or moderate severity. The time to resolution of DKA was shortest for T3DM (p = 0.04). Death rates were similar in all groups but in T3DM, all deaths occurred after resolution of DKA. There was a precipitating factor for DKA in 11/12, 91.7% cases in T3DM group, most common being infections (58.3%) followed by glucocorticoid use (16.7%). T3DM cases had shorter duration of DM but higher CRP, procalcitonin, and qSOFA scores for infections precipitating DKA (pall < 0.03). CRP was an independent predictor for DKA-related complications (OR: 1.01, 1.001–1.01, p = 0.04). Conclusion: This is the largest single-center cohort of T3DM with new-onset DKA, suggesting severe infections and glucocorticoid usage as major precipitating factors, while clinical course and outcomes were distinctly different from T2DM and T1DM patients with DKA. Objective: Dyslipidemia has been implicated in the development and progression of renal disease. To our knowledge, no reports have demonstrated an association between blood lipid level variability and diabetic kidney disease (DKD) in China. Our objective is to investigate the influence of variability in triglyceride levels on DKD incidence in a middle-aged to elderly Chinese Zhuang population with type 2 diabetes mellitus. Methods: In all, 276 participants with type 2 diabetes mellitus aged ≥ 45 years were followed up for 2 ~ 5 years and the results were analyzed. Variability in their triglycerides was evaluated using standard deviation, coefficient of variation, and variability independent of the mean, and the mean was calculated, and the outcome was DKD. We applied a Cox proportional hazard model to determine the relationship between variability TG levels and DKD. Results: During the mean 3-year follow-up, 74 participants developed DKD. In a multivariable cox regression model, triglyceride variability was a significant risk factor for DKD. The hazard ratios (HRs) (95% confidence intervals [CI]) for each increase in SD, CV, and VIM of triglycerides by 1 SD were 1.257 (1.038–1.522), 1.525 (1.059–2.195), and 1.007 (1.004–1.011), respectively. Compared to the lowest quartiles of SD of triglycerides, the HRs (95%CI) were 1.858 (1.359–2.542), 1.881 (1.354–2.612), and 1.858 (1.343–2.570) in Q2, Q3, and Q4. Consistency was seen when CV and VIM were used for calculating variability. Conclusion: High TG variability in middle-aged and elderly Chinese Zhuang patients with type 2 diabetes mellitus was associated with a significantly increased risk of developing DKD. Objective: To investigate the prevalence, risk factors, and healthcare-seeking patterns of hypertension and diabetes in Karnataka, India, and to offer knowledge that might guide public health initiatives intended to lessen the burden of these illnesses. Methods: In order to examine the prevalence, risk factors, and healthcare-seeking behaviour of hypertension and diabetes in Karnataka, India, a cross-sectional study is carried out using the information gathered from 26,574 households on 30,455 women and 4516 men (who were in their reproductive period) from the National Family Health Survey (2019–20). The information was summarised using descriptive statistics, which included frequencies and percentages. The association between different risk variables and the likelihood of getting diabetes and hypertension was examined using the chi-squared test and a logistic regression model. Data were analysed using STATA software version 16. Results: The study found that age, gender, education level, religion, and BMI are all significantly associated with hypertension and diabetes (p < 0.001). Tobacco use and alcohol consumption were not significantly associated with hypertension, but tobacco use was significantly associated with diabetes (p < 0.001). However, alcohol consumption was not found to be significantly associated with diabetes whereas the older age groups, males, underweight, overweight and obese, and tobacco use were all associated with increased odds of diabetes. On the other hand, females, secondary education or higher, and alcohol consumption were associated with decreased odds of diabetes. Conclusion: In conclusion, the study found a high prevalence of hypertension and diabetes in Karnataka, India, and identified several risk factors associated with these diseases. The study also highlighted the need for improved healthcare-seeking behaviour among people with hypertension and diabetes. Objective: This study aimed to identify reliable predictors of haemoglobin A1c (HbA1c) reduction and weight loss within 6 months after treatment with exenatide. Methods: A total of 343 patients with type 2 diabetes mellitus were examined and followed up for 12 months. The study patients were divided into two groups: responders and non-responders, which were defined based on their glycemic control (responders: HbA1c reduction of ≥ 1.0%) and weight response (responders: weight loss of ≥ 3%) within 6 months after exenatide administration. Binary logistic regression analysis was performed to identify the predictors associated with exenatide response, and a receiver operating characteristic (ROC) curve was plotted to assess the predictive ability of the identified factors. Results: Of the 148 patients who met the inclusion criteria, 53 (35.81%) were responders and 95 (64.19%) were non-responders. Binary logistic regression analysis revealed that baseline HbA1c, baseline weight, and duration of diabetes were significant predictors of glycemic and weight responses to exenatide. The area under the curve of the ROC for the predictors of HbA1c and weight responses within 6 months after exenatide initiation was 0.765 (95% confidence interval: 0.686–0.845). Objective: Statin treatment compliance and achieving target levels differ between populations. This study aimed to determine the status of achieving the lipid targets and evaluate patients' compliance to statin treatment in type 2 diabetic patients with dyslipidemia. Method: This cross-sectional study included type 2 diabetes mellitus (T2DM) patients with dyslipidemia who applied for treatment at our polyclinics. Statin compliance was evaluated using the Modified Morisky Adherence Scale (MMAS-8) through a face-to-face interview. Cardiovascular (CV) risk was calculated according to the 2019 ESC (European Society of Cardiology) criteria by evaluating the patients' individual risk factors. LDL targets were determined according to risk categories with the same criteria. Results: A total of 504 patients (F/M: 274/230) were included. The serum LDL levels were 102.6 ± 39.2 mg/dL. Of the patients, 56.1% were under statin treatment. CV risk was very high in 73.6% of patients. LDL levels were significantly lower in users than in non-users (91.2 ± 26.0, 117.3 ± 38.4, p < 0.0001). The rate of reaching the LDL target was 14.8% in statin users. Treatment compliance was low for 40.6% of statin users. Discontinuation of statin treatment due to side effects was 15.7% (n = 14). N = 49 patients willingly discontinued statin treatment. They reported that 40.8% considered the treatment unnecessary. Conclusion: It was observed that 56.1% of type 2 diabetic patients were on statin therapy. A small percentage of them 14.8% (n = 42) reached the LDL target. Objective: Diabetes retinopathy (DR) is one of the top listed world health issues. Screening is usually advised for diabetic cases on a routine basis. However, there is only partial access to ophthalmologists in all parts of the world. Irisin is a myokine that is being investigated for this reason. Our study aims to assess the role of irisin in detecting DR. Methods and materials: In a cross-sectional study, 140 diabetes cases, including 70 patients with diabetic retinopathy and 70 patients free from diabetic retinopathy, were enrolled. Demographic and anthropometric data were gathered, and the patients underwent physical examination. Also, microalbuminuria and serum irisin levels were assessed in both groups. The results were compared between the two study groups. Results: Among the enrolled cases, 55 (39.28%) were men, and 85 (60.72%) were women. The disease duration (p = 0.016) and the serum level of HBA1c (p = 0.014) were significantly higher in the diabetic retinopathy group compared to the group free from diabetic retinopathy. Moreover, irisin was significantly higher in the patients free from diabetic retinopathy (p = 0.007). Background: Type 2 diabetes exposes the body to a state of high blood sugar for a long time and causes varying degrees of hardening of the arteries, making it more prone to cardiovascular emergencies. Objective: The aim of the study was to explore the risk factors of early arterial stiffness in patients with type 2 diabetes mellitus (T2DM). Methods: A retrospective study was conducted on 316 T2DM patients without macroangiopathy in The First Hospital of Qinhuangdao. Early arterial stiffness was evaluated by brachial-ankle pulse wave velocity (baPWV). Results: Ninety patients (28.5%) had baPWV≥1800cm/s. baPWV showed positive correlation with systolic blood pressure (r=0.456, p<0.001), diastolic blood pressure (r=0.133, p=0.018), urine albumin-creatinine ratio (UACR) (r=0.232, p<0.001), neutrophil lymphocyte ratio (NLR) (r=0.185, p=0.001), and visceral fat area (r=0.139, p=0.014). In multiple linear regression analysis, systolic blood pressure (β=6.240, p<0.001), UACR (β=3.805, p=0.019), NLR (β=43.722, p=0.013), and visceral fat area (β=0.778, p=0.030) were significant independent predictors for baPWV. Conclusion: The decline of arterial elasticity was common in T2DM patients without macroangiopathy.
