We describe a novel patient-positioning device for dynamic CT myelography. Dynamic CT myelography requires angling the patient's spine to distribute dense contrast along the dependent thecal sac. The proposed device is constructed of a low-density reinforced polymer frame and can be raised or lowered to various heights with a hand-operated mechanism, allowing precise adjustment of the spinal angle and control of the contrast bolus, increasing the safety, reproducibility, and sensitivity of dynamic CT myelography., (© 2023 by American Journal of Neuroradiology.)

Background: Aneurysmal subarachnoid hemorrhage (SAH) is a catastrophic disease with devastating consequences, including a high mortality rate and severe disabilities among survivors. Inflammation is induced following SAH, but the exact role and phenotype of innate immune cells remain poorly characterized. We investigated the inflammatory components of the early brain injury in an animal model and in SAH patients., Method: SAH was induced through injection of blood in the subarachnoid space of C57Bl/6 J wild-type mice. Prospective blood collections were obtained at 12 h, days 1, 2, and 7 to evaluate the systemic inflammatory consequences of SAH by flow cytometry and enzyme-linked immunosorbent-assay (ELISA). Brains were collected, enzymatically digested, or fixed to characterize infiltrating inflammatory cells and neuronal death using flow cytometry and immunofluorescence. Phenotypic evaluation was performed at day 7 using the holding time and footprint tests. We then compared the identified inflammatory proteins to the profiles obtained from the plasma of 13 human SAH patients., Results: Following SAH, systemic IL-6 levels increased rapidly, whereas IL-10 levels were reduced. Neutrophils were increased both in the brain and in the blood reflecting local and peripheral inflammation following SAH. More intracerebral pro-inflammatory monocytes were found at early time points. Astrocyte and microglia activation were also increased, and mice had severe motor deficits, which were associated with an increase in the percentage of caspase-3-positive apoptotic neurons. Similarly, we found that IL-6 levels in patients were rapidly increased following SAH. ICAM-1, bFGF, IL-7, IL-12p40, and MCP-4 variations over time were different between SAH patients with good versus bad outcomes. Moreover, high levels of Flt-1 and VEGF at admission were associated with worse outcomes., Conclusion: SAH induces an early intracerebral infiltration and peripheral activation of innate immune cells. Furthermore, microglia and astrocytic activation are present at later time points. Our human and mouse data illustrate that SAH is a systemic inflammatory disease and that immune cells represent potential therapeutic targets to help this population of patients in need of new treatments.

Background and Purpose: Moyamoya disease is a chronic neurovascular steno-occlusive disease of the internal carotid artery and its main branches, associated with the development of compensatory vascular collaterals. Literature is lacking about the precise description of these compensatory vascular systems. Usually, the posterior circulation is less affected, and its vascular flow could compensate the hypoperfusion of the ICA territories. The aim of this study was to describe these natural connections between the posterior cerebral artery and the anterior cerebral artery necessary to compensate the lack of perfusion of the anterior cerebral artery territories in the Moyamoya population., Materials and Methods: All patients treated for Moyamoya disease from 2004 to 2018 in 4 neurosurgical centers with available cerebral digital subtraction angiography were included. Forty patients (80 hemispheres) with the diagnosis of Moyamoya disease were evaluated. The presence of anastomoses between the posterior cerebral artery and the anterior cerebral artery was found in 31 hemispheres (38.7%)., Results: Among these 31 hemispheres presenting with posterior cerebral artery-anterior cerebral artery anastomoses, the most frequently encountered collaterals were branches from the posterior callosal artery (20%) and the posterior choroidal arteries (20%). Another possible connection found was pio-pial anastomosis between cortical branches of the posterior cerebral artery and the anterior cerebral artery (15%). We also proposed a 4-grade classification based on the competence of these anastomoses to supply retrogradely the territories of the anterior cerebral artery., Conclusions: We found 3 different types of anastomoses between the anterior and posterior circulations, with different abilities to compensate the anterior circulation. Inflammatory myofibroblastic tumors (IMT) of the central nervous system (CNS) are rare entities that have a predilection for local recurrences. Approximately half of the inflammatory myofibroblastic tumors contain translocations that result in the over-expression of the anaplastic lymphoma kinase (ALK) gene. We hereby present the case of a patient diagnosed with a left parieto-occipital IMT that recurred after multiple surgeries and radiotherapy. Immuno-histochemical examination of the tumor demonstrated ALK overexpression and the presence of an ALK rearrangement observed in lung cancers. The patient was subsequently started on an ALK inhibitor. A response evaluation criteria in solid tumors (RECIST) partial response was observed by the seventh month of ALK inhibition and the tumor remained in control for 14 months. The current case reiterates the activity of ALK inhibitors within the CNS and suggests that radiotherapy may potentiate the permeability of ALK inhibitors in CNS tumors addicted to ALK signalling., Competing Interests: The authors have declared financial relationships, which are detailed in the next section.

We describe a misleading case of a partially occluded A1 segment duplication that mimicked an ACoA aneurysm on computed tomography angiography and conventional angiography and led to surgical intervention. The location of such an anomaly at the ACoA on the side of least hemodynamic stress may provide a clue to recognizing this variant. [ABSTRACT FROM AUTHOR]

Research on social dynamics and social networks has long informed how researchers and practitioners make sense of positive and negative influences on individuals' behaviors. Research on supportive network members and the role dynamics shared with parents, peers, and romantic partners have been central to empirical and theoretical contributions. Notably, the operationalization of social relationships has not changed much over the years. Scholars have primarily relied upon measures embedded in risk assessment tools or interview data about supportive ties. However, these approaches are not designed to assess individuals' social capital, clarify mobilization strategies for support, or inform whether compositional and structural network characteristics (and changes) affect correctional experiences. This essay highlights the contributions of extant research on social relationships during probation and parole supervision and identifies ways that advancements in personal (egocentric) social network methods could aid in crafting research, policies, and practices that center equity and inform community-based solutions. The medieval inquisition of heresy strongly relied on depositions, where witnesses were expected to report on the crimes of others and oneself. The resulting patterns of incrimination could be influenced by various factors, including the characteristics of the underlying dissident social network; the investigators' choices and biases; the trial circumstances, some of which must have exerted considerable pressure upon deponents; and the deponents' decisions to protect some suspects more than others. This case study aimed at disentangling selected social factors of incrimination in the register of the inquisition in Bologna, 1291–1310. We used social network analysis and, more specifically, an Exponential Random Graph Model (ERGM) to assess the influence of four social predictors: gender, churchperson status, membership of the urban "middle class", and kinship ties between incriminators and the incriminated. To increase the validity of our results, we controlled for various trial circumstances and structural parameters of the incrimination network. Our model corroborated a tendency towards female-to-female incrimination, while we did not find any positive or negative tendency towards male-to-male incrimination. We identified no effect of churchperson status on incriminating, while we found that among Cathars, members of the middle class were more likely to be incriminated than people without this status. Our model also corroborated the tendency to incriminate one's kinship group. Overall, our study underlines the relevance, but also the non-trivial operation, of social and demographic predictors in medieval heresy trials. [ABSTRACT FROM AUTHOR]

The Holy Cross Mountains represent an isolated outcrop of Palaeozoic rocks located in the Trans-European Suture Zone, which is the boundary between the Precambrian East European Craton and Phanerozoic mobile belts of South-Western Europe. Despite extensive structural history studies, high-resolution seismic profiling has not been applied to this region until now. This research introduces near-surface seismic imaging of the Holy Cross Fault, separating two tectonic units of different stratigraphic and deformation history. In our study, we utilize a carefully designed weight drop source survey with 5 m shot and receiver spacing and 4.5 Hz geophones. The imaging technique, combining seismic reflection profiling and travel time tomography, reveals detailed fault geometries down to 400 m. Precise data processing, including static corrections and noise attenuation, significantly enhanced signal-to-noise ratio and seismic resolution. Furthermore, the paper discusses various fault imaging techniques with their shortcomings. The data reveal a complex network of intersecting fault strands, confirming general thrust fault geometry of the fault system, that align with the region's tectonic evolution. These findings enhance understanding of the Holy Cross Mountains' structural framework and provide valuable reference data for future studies of similar tectonic environments. [ABSTRACT FROM AUTHOR]

Background: Twenty percent mannitol is widely used to reduce brain bulk and facilitate the surgical approach in intracranial surgery. However, a dose-response relationship has not yet been established. In this study, we compared the effects of 0.7 and 1.4 g·kg(-1) mannitol on brain relaxation during elective supratentorial brain tumor surgery., Methods: In this prospective, randomized, double-blind study, we enrolled 80 patients undergoing supratentorial craniotomy for tumor resection. Patients were assigned to receive 0.7 g·kg(-1) (group L) or 1.4 g·kg(-1) (group H) of 20% mannitol at surgical incision. Brain relaxation was assessed immediately after opening of the dura on a scale ranging from 1 to 4 (1 = perfectly relaxed, 2 = satisfactorily relaxed, 3 = firm brain, 4 = bulging brain)., Results: There was no significant difference between the 2 groups regarding age, sex, body mass index, and brain tumor localization or size. In group L 52.5% of patients and in group H 77.5% of patients presented a midline shift (P = 0.03). The median scores of brain relaxation (interquantile range) were 2.0 (1.75-3) and 2.0 (1-3) (P = 0.16 for patients in group L and H, respectively). We then used a proportional odds model to adjust for this unbalanced distribution and to assess the group effect (low-dose versus high-dose mannitol) on brain relaxation scores. When adjusted for the presence of midline shift, the use of a higher dose of mannitol resulted in an odds ratio of 2.5 (P = 0.03). This indicates that, considering the effect of a midline shift, the odds of having a 1-level improvement in relaxation score in patients who received a higher dose of mannitol (group H) was 2.5 times as large as the odds for the low-dose group. The odds ratio of 0.29 (P = 0.007) for the midline shift indicates that its occurrence was associated with a higher probability of a lower relaxation score, on average., Conclusion: In this study, we show that 1.4 g·kg(-1) of 20% mannitol results in equivalent brain relaxation scores as 0.7 g·kg(-1) in patients undergoing craniotomy for supratentorial brain tumor. When corrected for the presence of midline shift, this study reveals that patients in the high-dose group had significantly more chances of obtaining a better relaxation score compared with the lower-dose group.

Collaborative environmental governance (CEG) is increasingly advocated to address the environmental risk issues in the integrated development of urban agglomerations. Constructing an effective CEG network from the perspective of interdependent multilevel network plays a vital role in promoting the environmental governance of urban agglomerations. To investigate the structure characteristics and formation mechanism of CEG network, this paper takes the Yangtze River Delta urban agglomeration as the research area, and employes the social network analysis and Exponential Random Graph Model (ERGM) methods to analyze the CEG network, which consists of the collaborative network of cities, relationship network of topics, and affiliation network connecting cities to topics. Research results show that the CEG level in the Yangtze River Delta urban agglomeration continues to improve, while the CEG network is still not in a tightly connected state. For the collaborative network of cities, it presents the small world characteristics and forms a cooperative trend of "central-subcentral-peripheral city."For the relationship network of topics, the evolution of environmental governance topics is characterized by "from aspect to point." For the affiliation network connecting cities to topics, as the diversity of environmental governance topics increases among cities, cities within the Yangtze River Delta urban agglomeration tend to share the similar topics. In addition, the interactive triangular structures, star structures, open triangular structures and closed triangular structures in the network can promote the formation of new cooperative relationships in CEG network. Although tourism mobility spillover continues to be a key indicator for tourism management, more innovative research must be conducted at the micro level and high sampling frequency. Against the backdrop of an increasing number of global cities, in this paper, we evaluate the daily tourism mobility spillover inside a worldwide city of China: Shanghai. Based on the Granger causal network model and an original mobile positioning dataset, we analyse the causal relationship between local tourism flows and the spillover effects of tourism mobility within Shanghai. By categorising tourists into 'local tourists from Shanghai' and 'tourists from out of Shanghai', we reveal a significant causal relationship between Shanghai districts and flows generated by 'tourists from out of Shanghai'. The analysis of the causal network structure also reveals key districts and points of interest that significantly contribute to congestion in tourism mobility and Shanghai's dynamics. Background: Recent studies in surgical techniques have significantly transformed therapeutic approaches, leading to substantial decreases in morbidity and mortality rates. Differential diagnosis plays a pivotal role in determining the most suitable surgical strategies for addressing aneurysms. Historically, clipping has been considered a longstanding tradition in the vascular field between 1937 and 1975, pioneered by Microneurosurgery pioneers Yasargil and Fox. Methods: This study followed the PRISMA (Preferred Reporting Items for Systematic Reviews), and the statistical analysis was performed using IBM SPSS Statistics for Windows, Version 26.0 (released 2020; IBM Corp., Armonk, New York, United States). And EXCEL: A comprehensive review of neurosurgical care for cerebral aneurysms in controlling subarachnoid haemorrhage through endovascular clipping and coiling was conducted by the followers' methods used on aneurysm patients. Databases like Science Direct and PubMed were utilized, and articles were searched from the earliest available dates up to January 2024. Tables 1-2-3 are shown. The search focused primarily on publications in English, adhering to PRISMA guidelines. The search process for scientific papers, including meta-analysis, centred on PubMed and Science Direct. Results: A total of 45,223.00 patients with aneurysms who underwent endovascular procedures or surgical methods involving clipping or colling are displayed in Table 1. Of these, 2769 patients had ruptured aneurysms associated with subarachnoid haemorrhage, while 985.00 patients did not rupture. In Table 2, 6090.14 patients with aneurysms are evaluated, and in Table 3, 36,251.00 patients with coils 31,502.00 and clips 28,803.00 were assessed. Conclusion: From the synthesis of various reviewed studies, effective management strategies involve early detection of bleeding using diagnostic tests like MRI or CT angiography, considering neurological functioning levels assessed through scales such as the Fisher scale or the Hunt and Hess scale for subarachnoid haemorrhage or potential stroke. Background: Inflammatory myofibroblastic tumors (IMTs) are rare soft-tissue neoplasms. Accordingly, there is no standardized therapy for unresectable or advanced IMT. Chemotherapy, radiotherapy, and targeted molecular therapy play an important role in unresectable or advanced IMT. Case presentation: We present a 54-year-old man with a cough and chest distress case report. The thoracic surgeon performed the right upper pulmonary occupying lesion wedge resection and enlarged lymph node excision biopsy. Pathologic diagnosis revealed that the morphology of "right upper lung mass" was considered as Inflammatory Myofibroblastic Tumor (IMT). Radiotherapy was indicated at a high dose: 5400cGy in 27 fractions of 2Gy over 5 weeks were delivered combined with cisplatin. The patient was given a CT/MRI and hematological index every 3 months and experienced no more adverse events. The patient survives with no tumor recurrence as of the last follow-up. Progression-free survival (PFS) exceeded 5 years. Conclusions: We have reviewed the literature and summarized and discussed the radiotherapy treatment options and challenges for IMT. We first reported high-dose radiotherapy combined with chemotherapy treatment for unresectable IMT. Diyarbakir province mean rock formation is basaltic due to its location. Basaltic parent rocks release much silt or clay when imposed to chemical or physical weathering. The soils are residual in origin. In other words, they formed in situ where the basaltic parent rocks exist. They were not transported by means of wind or water. In this research, firstly, the properties of Diyarbakir basaltic clays were investigated from literature. After that, one soil sample from Kayapinar vicinity has been taken to laboratory to find index properties. The values from two different locations which are obtained from literature and values which are obtained from laboratory were compared. The results indicated that the three samples from different locations showed roughly the same index properties. As per the results, the clay is high plastic (CH), and it has swelling potential. In this thesis, design parameters of posttensioned slabs on ground under different climatic scenarios were found. The widely used method for slabs on ground under swelling soils is the Post‐Tensioning Institute (PTI) method. In order to use this method, Thornthwaite moisture index is required. This index is used in geotechnical engineering to identify moisture flux boundary conditions for unsaturated soils and depth of active zone for expansive soils. This index has been found using water balance sheet tables for Diyarbakir. So as to find the future moisture index, water balance sheet tables have been formed finding the climate projection data for Turkey in literature. In this way, five moisture index values of Diyarbakir have been found for the current situation and with regard to representative concentration pathway (RCP4.5 and RCP8.5) scenarios for 2050. The parameters for the maximum differential swell (em) and the angular moisture change index (ym) for design procedure according to the PTI method were found for each moisture index. Results showed that the ym and em values increased when the Thornthwaite moisture index is reduced. Also results showed that the slab thickness is increased when moisture index is reduced. Knowledge Graphs are repositories of information that gather data from a multitude of domains and sources in the form of semantic triples, serving as a source of structured data for various crucial applications in the modern web landscape, from Wikipedia infoboxes to search engines. Such graphs mainly serve as secondary sources of information and depend on well-documented and verifiable provenance to ensure their trustworthiness and usability. However, their ability to systematically assess and assure the quality of this provenance, most crucially whether it properly supports the graph's information, relies mainly on manual processes that do not scale with size. ProVe aims at remedying this, consisting of a pipelined approach that automatically verifies whether a Knowledge Graph triple is supported by text extracted from its documented provenance. ProVe is intended to assist information curators and consists of four main steps involving rule-based methods and machine learning models: text extraction, triple verbalisation, sentence selection, and claim verification. ProVe is evaluated on a Wikidata dataset, achieving promising results overall and excellent performance on the binary classification task of detecting support from provenance, with 87.5 % accuracy and 82.9 % F1-macro on text-rich sources. Although endovascular management has been increasingly adopted for basilar apex aneurysms (BAAs), microsurgery still represents an amenable treatment option. In this case series, six female patients (median age: 46 years) with six saccular high-riding BAAs (50% ruptured) were included. The median neck size was 5.3 mm (range: 2.9–7.9), and the median length from base to dome was 7.25 mm (range: 5.2–11.4). Preoperative complications included hydrocephalus (22%) and rebleeding (22%). All patients underwent clipping with a pterional craniotomy with extension into the temporal bone base. Intraoperative aneurysm rupture occurred in one patient (17%). Postoperative complications occurred in two patients (34%), of which one died because of extensive cerebral vasospasm and hospital-acquired pneumonia. At the 6-month follow-up, all remaining patients had modified Rankin scale scores ≤ 2. Microsurgery remains a viable option for BAAs in limited-resource settings. Background: The recurrent artery of Heubner (RAH) is typically the largest medial lenticulostriate branch of the anterior cerebral artery (ACA). Neurosurgical procedures such as aneurysm treatment on the anterior part of the circle of Willis can result in damage of the RAH leading to neurological deficits. The aim of this study was to identify the gaps and provide comprehensive data on the prevalence and anatomical characteristics of the RAH with neurosurgical considerations. Methods: The major electronic databases were thoroughly searched to identify the eligible studies. The information concerning study type, geographical origin, prevalence of the RAH, course and origin of the RAH, symmetry of origin and number of RAHs in each hemisphere, and morphometric data were extracted. The PRISMA guidelines were rigorously followed throughout the study. The AQUA tool was used to evaluate the reliability of included studies. Results: A total of 34 studies (n = 3645 hemispheres) were included in the meta-analysis. The analysis revealed that the RAH was present in 97.5% (95%CI: 95.5–98.6) of the hemispheres, originating most frequently from the A2 segment (42.2%, 95%CI: 35.0–49.7) or the ACoA-ACA junction (41.6%, 95%CI: 34.0–49.6), and coursing anteriorly (47.6%, 95%CI: 38.7–56.6) or superiorly (43.9%, 95%CI: 34.4–53.8) in relation to ACA. Almost a quarter of patients had more than one RAH, which was on average 22.82 mm (SD: 1.35, 95%CI: 20.16–25.47; I2 = 99.1%, p < 0.01) long and reached 0.76 mm (SD: 0.05, 95%CI: 0.66–0.85; I2 = 99.4%, p < 0.01) in diameter. Conclusions: As the RAH is present in the majority of the population, it is important to be aware of the wide variations in its anatomy. This will help to prevent postoperative neurological deficits by avoiding undesirable complications during surgeries that are performed in close proximity to the anterior segment of the circle of Willis. The objective of the study is to evaluate if the rupture of an aneurysm located on the middle cerebral artery (MCA) results in disorders of music recognition. To this aim, 20 patients having undergone brain surgery for the clipping of a unilateral left (LBS), right (RBS) or bilateral (BBS) aneurysm(s) of the MCA and 20 neurologically intact control subjects (NC) were evaluated with a series of tests assessing most of the abilities involved in music recognition. In general, the study shows that a ruptured aneurysm on the MCA that is repaired by brain surgery is very likely to produce deficits in the auditory processing of music. The incidence of such a deficit was not only very high but also selective. The results show that the LBS group was more impaired than the NC group in all three tasks involving musical long-term memory. The study also uncovered two new cases of apperceptive agnosia for music. These two patients (N.R. and R.C.) were diagnosed as such because both exhibit a clear deficit in each of the three music memory tasks and both are impaired in all discrimination tests involving musical perception. Interestingly, the lesions overlap in the right superior temporal lobe and in the right insula, making the two new cases very similar to an earlier case report. Altogether, the results are also consistent with the view that apperceptive agnosia results from damage to right hemispheric structures while associative agnosia results from damage to the left hemisphere.

Ongoing modern computational advancements continue to make it easier to collect increasingly large and complex datasets, which can often only be realistically analyzed using models defined by intractable likelihood functions. This Stats invited feature article introduces and provides an extensive simulation study of a new approximate Bayesian computation (ABC) framework for estimating the posterior distribution and the maximum likelihood estimate (MLE) of the parameters of models defined by intractable likelihoods, that unifies and extends previous ABC methods proposed separately. This framework, copulaABCdrf, aims to accurately estimate and describe the possibly skewed and high-dimensional posterior distribution by a novel multivariate copula-based meta-t distribution based on univariate marginal posterior distributions that can be accurately estimated by distribution random forests (drf), while performing automatic summary statistics (covariates) selection, based on robustly estimated copula dependence parameters. The copulaABCdrf framework also provides a novel multivariate mode estimator to perform MLE and posterior mode estimation and an optional step to perform model selection from a given set of models using posterior probabilities estimated by drf. The posterior distribution estimation accuracy of the ABC framework is illustrated and compared with previous standard ABC methods through several simulation studies involving low- and high-dimensional models with computable posterior distributions, which are either unimodal, skewed, or multimodal; and exponential random graph and mechanistic network models, each defined by an intractable likelihood from which it is costly to simulate large network datasets. This paper also proposes and studies a new solution to the simulation cost problem in ABC involving the posterior estimation of parameters from datasets simulated from the given model that are smaller compared to the potentially large size of the dataset being analyzed. This proposal is motivated by the fact that, for many models defined by intractable likelihoods, such as the network models when they are applied to analyze massive networks, the repeated simulation of large datasets (networks) for posterior-based parameter estimation can be too computationally costly and vastly slow down or prohibit the use of standard ABC methods. The copulaABCdrf framework and standard ABC methods are further illustrated through analyses of large real-life networks of sizes ranging between 28,000 and 65.6 million nodes (between 3 million and 1.8 billion edges), including a large multilayer network with weighted directed edges. The results of the simulation studies show that, in settings where the true posterior distribution is not highly multimodal, copulaABCdrf usually produced similar point estimates from the posterior distribution for low-dimensional parametric models as previous ABC methods, but the copula-based method can produce more accurate estimates from the posterior distribution for high-dimensional models, and, in both dimensionality cases, usually produced more accurate estimates of univariate marginal posterior distributions of parameters. Also, posterior estimation accuracy was usually improved when pre-selecting the important summary statistics using drf compared to ABC employing no pre-selection of the subset of important summaries. For all ABC methods studied, accurate estimation of a highly multimodal posterior distribution was challenging. In recent years, food authentication has acquired significant importance due to the increase in the incidence of fraud and counterfeiting. Alcoholic beverages are among the food products most susceptible to these kinds of illicit practices due to their high commercial value. In the EU alone, there are 47 categories of spirit drinks and approximately 250 geographical indications (GIs). The production and labeling of GIs are strictly regulated, and developing analytical procedures that can ensure compliance with the legislation is essential to guarantee the typicality of these products. The aim of this review is to summarize the most relevant analytical techniques used for the authentication and quality control of two well-renowned GIs: "Grappa" and wine brandy. It considers the last decade of advancements for both conventional targeted chromatographic techniques and less common methods mainly based on spectrometry coupled with chemometrics for quick and non-destructive discrimination of samples. Objective: To explore the spatial relationship between A1 segment proximal anterior cerebral artery aneurysms and their main trunks, classify them anatomically and develop targeted treatment strategies. Methods: This single-center retrospective analysis involved 39 patients diagnosed with aneurysms originating from the proximal of A1 segment of the anterior cerebral artery (2014-2023). Classify the patient's aneurysm into 5 types based on the location of the neck involving the carrier artery and the spatial relationship and projection direction of the aneurysm body with the carrier artery, and outcomes from treatment methods were compared. Results: Among 39 aneurysms, 18 cases underwent endovascular intervention treatment, including 6 cases of stent assisted embolization, 1 case of flowdiverter embolization, 5 cases of balloon assisted embolization, and 6 cases of simple coiling. At discharged, the mRS score of all endovascularly treated patients was 0, and the GOS score was 5 at 6 months after discharge. At discharge, the mRS score of microsurgical clipping treated patients was 0 for 15 cases, 3 for 1 case, 4 for 1 case and 5 for 2 cases. Six months after discharge, the GOS score was 5 for 16 cases, 4 for 2 cases, 3 for 2 cases, and 1 for 1 case. GOS outcomes at 6 months were better for endovascularly treated patients (p = 0.047). Conclusion: Results showed better outcomes for the endovascular treatment group compared to microsurgical clipping at 6 months after surgery. Food traceability and authentication systems play an important role in ensuring food quality and safety. Current techniques mainly rely on direct measurement by instrumental analysis, which is usually designed for one or a group of specific foods, not available for various food categories. To develop a general strategy for food identification and discrimination, a novel method based on fluorescence sensor arrays is proposed, composed of supramolecular assemblies regulated by non‐covalent interactions as an information conversion system. The stimuli‐responsiveness and tunability of supramolecular assemblies provided an excellent platform for interacting with various molecules in different foods. In this work, five sensor arrays constructed by supramolecular assemblies composed of pyrene derivatives and perylene derivatives are designed and prepared. Assembly behavior and sensing mechanisms are investigated systematically by spectroscopy techniques. The traceability and authentication effects on several kinds of food from different origins or grades are evaluated and verified by linear discriminant analysis (LDA). It is confirmed that the cross‐reactive signals from different sensor units encompassing all molecular interactions can generate a unique fingerprint pattern for each food and can be used for traceability and authentication toward universal food categories with 100% accuracy. Flow diversion for intracranial aneurysms emerged as an efficacious and durable treatment option over the last two decades. In a paradigm shift from intrasaccular aneurysm embolization to parent vessel remodeling as the mechanism of action, the proliferation of flow-diverting devices has enabled the treatment of many aneurysms previously considered untreatable. In this review, we review the history and development of flow diverters, highlight the pivotal clinical trials leading to their regulatory approval, review current devices including endoluminal and intrasaccular flow diverters, and discuss current and expanding indications for their use. Areas of clinical equipoise, including ruptured aneurysms and wide-neck bifurcation aneurysms, are summarized with a focus on flow diverters for these pathologies. Finally, we discuss future directions in flow diversion technology including bioresorbable flow diverters, transcriptomics and radiogenomics, and machine learning and artificial intelligence. Purpose: Intracranial aneurysms present significant health risks, as their rupture leads to subarachnoid haemorrhage, which in turn has high morbidity and mortality rates. There are several elements affecting the complexity of an intracranial aneurysm. However, criteria for defining a complex intracranial aneurysm (CIA) in open surgery and endovascular treatment could differ, and actually there is no consensus on the definition of a "complex" aneurysm. This DELPHI study aims to assess consensus on variables defining a CIA. Methods: An international panel of 50 members, representing various specialties, was recruited to define CIAs through a three-round Delphi process. The panelists participated in surveys with Likert scale responses and open-ended questions. Consensus criteria were established to determine CIA variables, and statistical analysis evaluated consensus and stability. Results: In open surgery, CIAs were defined by fusiform or blister-like shape, dissecting aetiology, giant size (≥ 25 mm), broad neck encasing parent arteries, extensive neck surface, wall calcification, intraluminal thrombus, collateral branch from the sac, location (AICA, SCA, basilar), vasospasm context, and planned bypass (EC-IC or IC-IC). For endovascular treatment, CIAs included giant size, very wide neck (dome/neck ratio ≤ 1:1), and collateral branch from the sac. Conclusions: The definition of aneurysm complexity varies by treatment modality. Since elements related to complexity differ between open surgery and endovascular treatment, these consensus criteria of CIAs could even guide in selecting the best treatment approach. Background: Normal saline (NS) infusion in large volumes may result in hyperchloremic metabolic acidosis and renal compromise. Balanced crystalloid (BC) with physiochemical composition akin to that of plasma may avoid these problems associated with NS. The present study aimed to evaluate effects of NS versus BC on acid–base balance and renal functions in patients undergoing intracranial tumor resection surgeries. Methods: Fifty adult patients scheduled to undergo elective neurosurgery for intracranial tumor resection were randomized to receive either NS or BC as intraoperative or maintenance fluid. Metabolic and renal parameters were estimated prior to induction (baseline), at 1 h and 2 h after induction, at the end of surgery and 4 h after extubation. Serum neutrophil gelatinase-associated lipocalin (NGAL) was measured postoperatively. Brain relaxation score was assessed by the operating surgeon. Results: Baseline values of variables were similar between the groups. At rest of the observed time-points, pH was significantly lower, while blood urea, serum creatinine, sodium, chloride, NGAL and plasma osmolality were significantly higher in the NS group as compared to the BC group. Brain relaxation score, serum bicarbonate and base excess were comparable between the two groups. Conclusion: Use of balanced crystalloid (plasmalyte) resulted in better metabolic and renal profile as compared to normal saline in neurosurgical patients. [ABSTRACT FROM AUTHOR]

Simple Summary: Tumors contain cancerous cells with different mutations and different types of non-cancerous cells. It is important to understand the molecular properties of these cells in order to develop effective treatments for cancers. We describe a new strategy for studying tumor heterogeneity and apply it to human astrocytomas, a type of brain tumor. By slicing astrocytomas into a large number of serial sections and analyzing patterns of mutation frequencies, we reconstruct the evolutionary history of cancer cell mutations in each tumor. By comparing these patterns to gene activity measured in the same sections, we identify molecular features that optimally distinguish different types of cancerous and non-cancerous cells and validate these with different techniques, including analysis of individual nuclei. By integrating analyses of multiple molecular species at multiple scales, our strategy provides a powerful approach for precisely deconstructing intratumoral heterogeneity. Tumors may contain billions of cells, including distinct malignant clones and nonmalignant cell types. Clarifying the evolutionary histories, prevalence, and defining molecular features of these cells is essential for improving clinical outcomes, since intratumoral heterogeneity provides fuel for acquired resistance to targeted therapies. Here we present a statistically motivated strategy for deconstructing intratumoral heterogeneity through multiomic and multiscale analysis of serial tumor sections (MOMA). By combining deep sampling of IDH-mutant astrocytomas with integrative analysis of single-nucleotide variants, copy-number variants, and gene expression, we reconstruct and validate the phylogenies, spatial distributions, and transcriptional profiles of distinct malignant clones. By genotyping nuclei analyzed by single-nucleus RNA-seq for truncal mutations, we further show that commonly used algorithms for identifying cancer cells from single-cell transcriptomes may be inaccurate. We also demonstrate that correlating gene expression with tumor purity in bulk samples can reveal optimal markers of malignant cells and use this approach to identify a core set of genes that are consistently expressed by astrocytoma truncal clones, including AKR1C3, whose expression is associated with poor outcomes in several types of cancer. In summary, MOMA provides a robust and flexible strategy for precisely deconstructing intratumoral heterogeneity and clarifying the core molecular properties of distinct cellular populations in solid tumors. Background: Intraventricular hemorrhage (IVH) refers to bleeding within the brain's ventricular system, and hydrocephalus is a life-threatening complication of IVH characterized by increased cerebrospinal fluid accumulation in the ventricles resulting in elevated intracranial pressure. IVH poses significant challenges for healthcare providers due to the complexity of the underlying pathophysiology and lack of standardized treatment guidelines. Herein, we performed a systematic review of the treatment strategies for hydrocephalus secondary to IVH. Methods: This systematic review was prospectively registered with PROSPERO (CRD42023450786). The search was conducted in PubMed, Cochrane Library, and Web of Science on July 15, 2023. We included original studies containing valid information on therapy management and outcome of hydrocephalus secondary to primary, spontaneous, and subarachnoid or intracranial hemorrhage following IVH in adults that were published between 2000 and 2023. Glasgow Outcome Scale (GOS) or modified Ranking Scale (mRS) scores during follow-up were extracted as primary outcomes. The risk of bias was assessed using the Newcastle–Ottawa Scale for Cohort Studies or Cochrane Risk of Bias 2.0 Tool. Results: Two hundred and seven patients from nine published papers, including two randomized controlled trials, were included in the analysis. The GOS was used in five studies, while the mRS was used in four. Seven interventions were applied, including craniotomy for removal of hematoma, endoscopic removal of hematoma with/without endoscopic third ventriculostomy (ETV), traditional external ventricular drainage (EVD), and various combinations of EVD, lumbar drainage (LD), and intraventricular fibrinolysis (IVF). Endoscopic removal of hematoma was performed in five of nine studies. Traditional EVD had no obvious benefit compared with new management strategies. Three different combinations of EVD, LD, and IVF demonstrated satisfactory outcomes, although more studies are required to confirm their reliability. Removal of hematoma through craniotomy generated reliable result. Generally, endoscopic removal of hematoma with ETV, removal of hematoma through craniotomy, EVD with IVF, and EVD with early continuous LD were useful. Conclusion: EVD is still crucial for the management of IVH and hydrocephalus. Despite a more reliable result from the removal of hematoma through craniotomy, a trend toward endoscopic approach was observed due to a less invasive profile. [ABSTRACT FROM AUTHOR]
