Spor müsabakaları organizasyonları insanların zaman geçirdikleri önemli faaliyetler arasında yer almaktadır. Spor müsabakaları arasında futbol, dünyadaki en yüksek popülerlik oranına sahiptir. Birçok futbol müsabakası hem bölgesel bazda hem de uluslararası bazda düzenlenmektedir. Futbol maçlarının sonuçları futbol takımı taraftarları için çok önemlidir ve günlük yaşamlarında destekledikleri takımın sonuçlarına göre mutlu ya da üzgün olabilirler. Yapay zekâ ve makine öğrenmesindeki gelişmeler sonucunda birçok alanda yapılan tahminler futbol sonuçlarının tahminini de içermektedir. Ekonomik getirileri de olan futbol bahis sistemi düşünüldüğünde, futbol maç sonuçlarının tahmin edilmesi önem kazanmıştır. Bu çalışma, İtalya-Serie A, İngiltere-Premier Lig ve Alman-Bundesliga liglerindeki sonuçları tahmin etmek için bilindiği kadarı ile literatürde ilk kez sunulan taktik tabanlı bir tahmin yaklaşımı sunmaktadır. Bu yaklaşımda oyuncuların sahadaki pozisyonları ve piyasa değerleri web sayfalarından otomatik olarak alınmakta ve veri madenciliği yöntemlerine açık hale getirilmektedir. Veri madenciliği aşamasında Torbalama, Yinelemeli Sınıflandırıcı İyileştirici ve Derin Öğrenme gibi farklı yöntemler karşılaştırılmakta ve üç yöntemin farklı liglerde benzer sonuçlar verdiği görülmektedir. Veri madenciliği yöntemlerinde yaklaşık %60 doğruluk elde edilmektedir. Her üç lig için makine öğrenmesi yöntemlerinde kullanılan özellikler incelendiğinde takım sıralamasının maç sonuçlarını etkileyen en önemli özellik olduğu görülmüştür. Serie A'da Stoper ve Merkez Orta Saha özellikleri öne çıkarken, Premier Lig'de Sağ Kanat ve Kaleci gibi farklı özelliklerin önemli olduğu görülmüştür. Bundesliga'da Stoper ve Santrafor özelliklerinin maç sonuçlarına daha çok etki ettiği görülmüştür. Organizations of sports competitions are among the important activities that people spend their time on. Among the sports competitions, football has the highest popularity rate in the world. Many football competitions are organized both on a regional basis and on an international basis. The results of football matches are very important for the fans of the football team, and they can be happy or sad according to the results of the team they support in their daily lives. As a result of developments in artificial intelligence and machine learning, predictions made in many fields include the prediction of football results. Considering the football betting system, which also has economic returns, the prediction of football match results has become important. This study presents a tactical-based forecasting approach to be presented for the first time in the literature to our knowledge, to predict results in Italy-Serie A, England-Premier League and Germany-Bundesliga leagues. In this approach, players' on-field placements and market values are automatically obtained from web pages and made available for data mining methods. The data mining phase compares different methods such as Bagging, Iterative Classifier Optimizer and Deep Learning, and it is seen that the three methods give similar results in different leagues. An accuracy of about 60% is achieved in data mining methods. When the features used in machine learning methods are examined for all three leagues, it is the most important feature in predicting the team ranking. While Center-Back and Central Midfielder features stand out in Serie A, different features such as Right Winger and Goalkeeper are seen to be important in Premier League. In the Bundesliga, the Centre-Back and Centre-Forward features have had a greater impact on the matches.

Enerji kaynakları kullanım alanları ilkel toplumdan günümüze kadar sürekli evrilerek gelişimini sürdürmüştür. Enerji kaynakları kullanım alanları teknoloji ve bilimin gelişmesine paralel olarak çeşitliliğini arttırmıştır. Buharlı motorların icadı, kömürle çalışan enerji santrallerinin kullanılmaya başlanması, içten yanmalı motorun icadı ile insanlık medeniyet tarihinin gelişiminde ilerleme göstererek enerjinin kullanım alanları ve enerji kaynaklarının çeşitliliğinin farklılaşma göstermesine neden olmuştur. Enerji kavramı ve endüstri gelişimi birbirine bağımlı iki değişken olup toplumların endüstri alanındaki gelişimini sağlamış ve enerji kullanım miktarını da arttırmıştır. Endüstri gelişimi yarışında öne çıkan toplumlar enerji ihtiyacını karşılamak için kendi bünyesinde olmayan enerji kaynaklarını diğer toplumlardan ithal etme yolunu seçmiştir. Enerji kaynaklarının bölgeler arası aktarımı, tarihsel süreçte bilim ve teknolojinin gelişimine paralel olarak evrim göstermiştir. Örneğin ilk zamanlarda kömür enerji kaynağı olarak fiziki araçlarla taşınmıştır. Fosil enerji kaynağı olarak doğal gaz ve petrolün kullanılmaya başlanması ile doğal gaz ve petrol aktarımı boru hatları vasıtası ile kesintisiz olarak yapılmaya başlamıştır. Şüphesiz bir ülkenin gelişimi için endüstriyel gelişmişlik gerekmekte olup bunun için de enerjiye ihtiyacı vardır. Dolayısı ile endüstri yarışında pozisyonlarını korumak için toplumlar birbirlerine karşı var olan enerji hatları üzerinden ya da yeni planlanan enerji aktarım hatları üzerinden hegemonya savaşları yürütmektedir. Bu makalede makro boyutta var olan dünya enerji aktarım hatları incelenecektir. Mikro boyutta ise Türkiye üzerinden geçen enerji aktarım hatları incelenecektir. İleride gerçekleştirilmesi planlanan enerji aktarım hatları üzerine inceleme yapılarak konu ile ilgili ileriye yönelik projeksiyon sunulacaktır., The areas of use of energy resources have continued to evolve from the primitive society to the present day. Energy resources usage areas increased in parallel with the development of technology and science. With the invention of steam engines, the use of coal-fired power plants, the invention of the internal combustion engine, humanity has progressed in the history of civilization, causing the diversity of energy use areas and energy resources to differ. The concept of energy and industrial development are two interdependent variables, which have ensured the development of societies in the field of industry and increased the amount of energy use. Societies that stand out in the race for industrial development have chosen to import energy resources that are not within their own body from other societies in order to meet their energy needs. Interregional transfer of energy resources has evolved in parallel with the development of science and technology in the historical process. For example, coal was transported by physical vehicles as an energy source in the first days. With the use of natural gas and oil as a fossil energy source, natural gas and oil transfer started to be carried out continuously through pipelines. Undoubtedly, industrial development is required for the development of a country and energy is needed for this. Therefore, societies are waging wars of hegemony over existing energy lines or through newly planned energy transmission lines in order to maintain their positions in the industrial race. In this article, world energy transmission lines that exist in macro scale will be examined. At the micro size of the power transmission lines will be examined through Turkey. The future projection of the subject will be presented by examining the energy transmission lines planned to be realized in the future.

The mortality of orthopedic surgery of hip ingeriatric patients is high due to thelimited physiologic capacity of the patients’ and comorbidities associated withembolism. Besides neuroaxial blocks, peripheral nerve blocks can be safely preferredin these high-risk patients. As peripheral neural blocks are related to limitedsympathetic blockade and hypotension, hypotension may be avoided as a result ofvasodilatation and bradycardia may alsobe avoided as a result of sympathetic blockade of spinal and epiduralanesthesia. Also, postoperative analgesia achieved by peripheral neural blockshelps early mobilization. This case is related to the combination of femoraland sciatic blocks performed in two ASA IV geriatric patients undergoingorthopedic surgery  , Öz: Geriatrik hastalarda kalça cerrahisininmortalitesi, bu hastaların kısıtlı fizyolojik adaptasyon kapasiteleri, emboliriski, eşlik eden komorbiditeleri nedeniyle yüksektir. Bu operasyonlar içinyüksek anestezik risk taşıyan geriatrik hastalarda nöroaksiyel bloklarındışında periferik sinir blokları da güvenle kullanılabilir. Böylece rejyonel anestezi tekniği olarakuygulanan spinal ve epidural anestezi sonrası blok seviyesinin altında oluşanvazodilatasyona bağlı hipotansiyon ve sempatik blokaj sonucu gelişenbradikardiden de nispeten kaçınılmış olunur. Çünkü periferik sinir bloklarındasınırlı sempatik blokaj ve hipotansiyon gözlenir. Ayrıca periferik sinirbloklarının sağladığı postoperatif analjezi de erken mobilizasyona desteksağlamaktadır. Bu yazıda ASA IV, ileri yaş iki olguda ortopedik cerrahi içinuygulanan femoral-siyatik sinir blok kombinasyonu sunulmaktadır.

Objective: Central venous catheterization (CVC) is an invasive procedure used in intensive care units with various indications. Malposition of the catheter has been reported in the literature ranging from 3.6-14% in practice. In this study, we aimed to determine the preference of catheter placement and malposition rates in CVC applications performed by specialist physicians in our intensive care units without using ultrasonography. Method: After the approval of the ethics committee, medical files of 1710 patients who were applied to the Mersin City Training and Research Hospital in 2017-2019 were reviewed. Permanent port catheter, dialysis catheter and catheters placed in pediatric patients under 18 years of age were excluded from the study. All catheterization procedures were performed by a specialist physician using 7 F catheters with Seldinger technique without the use of ultrasound. Results: Of the 1463 patients included in the study, the catheters were inserted intointernal jugular vein in 842, subclavian vein in 467,and femoral vein in 154 patients. When femoral catheterization was excluded from assessments, malposition was found in 1309 patients with a rate of 1.68%. There were 14 malpositioned catheters in the right internal jugular vein, 6 in the right subclavian vein and 2 in the left subclavian vein. Conclusion: In conclusion, it has been reported that ultrasound-guided catheter placement may prevent malposition and reduced complication rate when performed by experienced clinicians. However,when the ultrasound is not available, we believe that performing the procedure by experienced clinicians and confirming the accuracy of the catheter position with lung x-ray after the procedure may reduce malposition rates. - Abstract
Transfusion-related acute lung injury (TRALI) is the second leading cause of transfusion-associated mortality in high-risk patients undergoing major surgery. TRALI presenting with acute hypoxia and non-cardiac edema is a vital clinical entity which must be considered in the differential diagnosis in transfusion-associated patients. Fresh frozen plasma, erythrocyte suspension and thrombocyte suspension transfusion were performed during perioperative and postoperative periods to the patient undergoing supracoronary graft interposition due to ascending aortic aneurysm. Respiratory support treatment with positive pressure mechanical ventilation was initiated for acute hypoxia and bilateral pulmonary edema developed immediately after the procedure. The patient thought to have delayed TRALI was extubated on postoperative 5. day. Noninvasive mechanical respiratory support treatment was maintained. On postoperative 10. day the patient did not need any oxygen and ventilatory support. TRALI should be considered in patients with acute onset of hypoxia, ARDS especially in cases undergoing major surgery and transfusion of large amounts of ES, FFP, platelet suspension. Noninvasive mechanical ventilation should be considered as a treatment option. [ABSTRACT FROM AUTHOR]

13. Can Pre-Operative HbA1c Values in Coronary Surgery be a Predictor of Mortality?

- Abstract
Objective: One of the most frequently studied parameters in terms of outcome estimation in cardiac surgery is HbA1c. Several studies in literature suggest that high HbA1c value increases mortality and morbidity, but there is no relation between them. The primary aim of the present study is to investigate whether HbA1c value in diabetic patients undergoing coronary bypass graft surgery is an independent predictor for post-operative mortality and morbidity, and our secondary goal was to determine independent risk factors that cause mortality and morbidity in the same patient population., Methods: 380 diabetic patients diagnosed with diabetes who underwent coronary surgery with cardiopulmonary bypass in Mersin State hospital between July 2014 to December 2016 after the approval of the Mersin University Faculty of medicine ethics committee were included in this retrospective, observational, and cross-sectional study. Patient demographic and perioperative information were obtained from the electronic information operating system and from anesthesia-intensive care follow-up forms. The HbA1c threshold was accepted as 7%, which was reported to be more appropriate for evaluating high-risk groups., Results: Three hundred and fifty-four patients with complete access to the data were included in the study. The mean age of the patients was 60.8±9.4 years. 37% of the patients (131 patients) were female. The number of patients with HbA1c≥7 was 194 (54,8%) in the entire patient population. In this study, high HbA1c (≥7) values in diabetic patients undergoing isolated coronary bypass graft surgery were not found to be independent predictors of post-operative mortality and morbidity. Mortality was seen in 28 patients (7.9%). Ejection fraction (EF) was found to be an independent predictor factor for pre-operative factors in logistic regression models constructed according to mortality predictors (OR:0.94; 95% CI: 0.90-0.99; p=0.016). Complications were seen in 50 patients (14.1%). In the models formed from the point of view of the complication predicators, only EF was found to be independent predictor (OR:0.95; 95% CI: 0.92-0.98; p=0.004). It was found that HbA1c was not predictive in all models for mortality and complication (p>0.05)., Conclusion: There are reports in the literature that mortality increases 4-fold when HbA1c value is higher than 8.6% in coronary surgery. However, there is a view that HbA1c alone cannot predict mortality in coronary surgery if diabetes associated factors are excluded. In this study, high HbA1c (≥7) values in diabetic patients undergoing isolated coronary bypass graft surgery were not found to be independent predictors of post-operative mortality and morbidity. Pre-operative low ejection fraction was found as an independent risk factor for post-operative mortality and morbidity in the general patient population., Competing Interests: Conflict of Interest: No conflict of interest was declared by the authors.
