BACKGROUND: The aim of the study was to evaluate familial Mediterranean fever (FMF) mutation analysis in pediatric patients with inflammatory bowel disease (IBD). The relation between MEFV mutations and chronic inflammatory diseases has been reported previously. METHODS: Children with IBD (334 ulcerative colitis (UC), 224 Crohn’s disease (CD), 39 indeterminate colitis (IC)) were tested for FMF mutations in this multicenter study. The distribution of mutations according to disease type, histopathological findings, and disease activity indexes was determined. RESULTS: A total of 597 children (mean age: 10.8 ± 4.6 years, M/F: 1.05) with IBD were included in the study. In this study, 41.9% of the patients had FMF mutations. E148Q was the most common mutation in UC and CD, and M694V in IC (30.5%, 34.5%, 47.1%, respectively). There was a significant difference in terms of endoscopic and histopathological findings according to mutation types (homozygous/heterozygous) in patients with UC (P < .05). There was a statistically significant difference between colonoscopy findings in patients with or without mutations (P = .031, P = .045, respectively). The patients with UC who had mutations had lower Pediatric Ulcerative Colitis Activity Index (PUCAI) scores than the patients without mutations (P = .007). Conclusion: Although FMF mutations are unrelated to CD patients, but observed in UC patients with low PUCAI scores, it was established that mutations do not have a high impact on inflammatory response and clinical outcome of the disease.

Tam Metin / Full Text Bu çalışmanın amacı parametrik bir model olan Analitik Hiyerarşik Süreç yardımıyla Doğrusal Kombinasyon Tekniği kullanılarakçalışma alanına ait arazilerin arazi uygunluk sınıflamasının belirlenmesi ve haritalanmasıdır. Çalışma alanı, Samsun İli Çarşambailçesine bağlı Sefalı Köyü ve yakın çevresini kapsamakta olup, yaklaşık 972 ha’dır. Çalışma alanına ait haritalama birimleri vemodeller için gerekli olan toprak parametrelerinin belirlenmesinde detaylı toprak haritasından yararlanılmıştır. Ayrıca, CBSprogramı kullanılarak çalışma alanının arazi uygunluk haritaları oluşturulmuştur. Tarımsal yönden arazi uygunluk haritasına göre,araştırma alanının büyük bir kısmı olan 6106.3 da’ ı (%62.85) uygun ve çok uygun sınıfları oluştururken, %27.1’i (2633.4 da) azuygun sınıfa girmektedir. Toplam alanın yalnız yaklaşık %10’unu oluşturan Tp1.Dd31o, Yk1.Ed21i ve Yk1.Dd21i haritalamabirimleri ise tarımsal kullanıma uygun değildirler. Bitkisel üretimi sınırlandıran ana faktörler olarak eğim, ağır bünye ve sığ topraközellikleri olduğu belirlenmiştir. The main aim of this study is to determine land suitability class and mapping using Analitic Hierarchical Process associated with Linear Combination Technique that is a parametric model. This study was carried out in Sefalı village and its near vicinity located at Çarşamba district of Samsun province covers about 972.2 ha. Land mapping units and some soil parameters required for models were taken from detailed soil map. In addition, land suitability map was produced using GIS programme for the study area. The results of land suitability for agricultural use showed that while 610.63 ha (62.85%) of the study area soils were classified as best and good, about %27.1 (263.34 ha) were classified as moderately good lands. Only about 10.0% of the study area located on Tp1.Dd31o, Yk1.Ed21i and Yk1.Dd21i land mapping units were not suitable for agricultural uses. In addition, it was determined also some plant growing limitation factors such as, heavy texture, steep slope and shallow soil depth.

Amaç: Yaş aralığı üç-altı yıl olan Türk çocuklarında gastroözofageal reflü hastalığı tanısı için pratikte kullanılabilir bir anket formu oluşturmak ve bu anket formu ile üç-altı yaş arası çocuklarda gastroözofageal reflü hastalığı sıklığını araştırmaktır. Gereç ve Yöntemler: Çalışmaya üç grup alınmıştır. Grup 1; gastroözofageal reflü hastalığı tanısı olan 45 hasta, grup 2 sağlıklı 100 çocuk, grup 3 ise Menderes ilçesinde yaşayan, yaş aralığı üç-altı yıl olan 283 çocuktan oluşmaktadır. Daha önce oluşturulmuş anket formları Türkçeye çevrilerek ve yeni sorular eklenerek modifiye edildi. Anket formu oluşturulduktan sonra grup 1 ve grup 2’deki çocuklara uygulandı. İki grup arasındaki semptomların odds oranı dikkate alınarak her bir soru için puanlar oluşturuldu. Elde edilen puanlarla alıcı işlem karakteristiği eğrisi oluşturuldu ve duyarlılık ve özgüllük dikkate alınarak gastroözofageal reflü hastalığı tanısı için eşik değer saptandı. Oluşturulan anket formu, daha sonra Menderes ilçesindeki, yaş aralığı üç-altı yıl olan çocuğa (grup 3) uygulanarak gastroözofageal reflü hastalığı sıklığı araştırıldı. Bulgular: Grup 1 ve grup 2’ye uygulanan modifiye edilmiş anket formunda sırasıyla alınan reflü puanı ortalama±SD 16±8,24 ve 2±3,52 olarak hesaplandı. Üç-altı yaş arası reflü skoru ?6 için duyarlılık ve özgüllük değerleri sırasıyla %92 ve %88,9 olup, gastroözofageal reflü hastalığı tanısı için skor?6 kabul edildi. Menderes bölgesinde, üç-altı yaş arası çocuklarda (grup 3) gastroözofageal reflü hastalığı sıklığı %18,7 bulundu. Gastroözofageal reflü hastalığı olan grupta anne sütü alma süresi daha kısa ve doktor tanılı gastroözofageal reflü hastalığı olan ebeveyn sayısı daha fazla idi. Sonuç: Üç-altı yaş çocuklar için hazırladığımız bu anket formunun gastroözofageal reflü hastalığı tanı ve izleminde güvenilebilir şekilde kullanılabileceği düşünülmektedir., Objective: We aimed to develop a questionnaire form for the diagnosis of gastroesophageal reflux disease in Turkish children aged 3-6 years old, and to analyze the frequency of gastroesophageal reflux disease in children aged 3-6 years old. Material and Methods: the study included the 3 groups of patients. Group 1 included 45 patients with gastroesophageal reflux disease, group 2 included 100 healthy children and group 3 included 283 children aged 3-6 years old in Menderes district. Previous questionnaire forms translated to Turkish and then modified with novel questions. the novel questionnaire form performed to children in group 1 and group 2. Each item was given a score according to odds ratio of symptoms between two groups. Reflux score was calculated according to sensitivity and specificity. Then, frequency of gastroesophageal reflux disease was analyzed in children aged 3-6 years old in Menderes district (group 3). Results: Reflux scores in children in group 1 and 2 (mean±SD) were 16±8.24 and 2±3.52, respectively. Sensitivity and specificity of reflux score ?6 was 92% and 88.9%, respectively and reflux score ?6 was diagnostic for gastroesophageal reflux disease. the frequency of gastroesophageal reflux disease in children aged 3-6 years old in Menderes district (group 3) was 18.7%. the duration of breastfeeding was short, and the frequency of doctor diagnosed gastroesophageal reflux disease was high in children with gastroesophageal reflux disease. Conclusion: Our questionnaire form may be used reliably in diagnosis and follow-up of the gastroesophageal reflux disease in children aged 3-6 years old.

Amaç: Çocuklarda özofagus patolojilerinin sıklığı, bu hastalıkların yaş, cinsiyet ve semptomlarla ilişkisini içeren veriler sınırlıdır. Çalışmanın amacı, çocuklardaki özofagus patolojilerinin sıklığının, etiyolojisinin, endoskopik ve histopatolojik özelliklerinin incelenmesidir.Gereç ve Yöntemler: Birimimizde Mayıs 2009 - Haziran 2015 tarihleri arasında özofagogastroduodenoskopi yapılmış 1-17 yaş aralığında hastalar retrospektif olarak incelendi (n=3081). Hastaların dosya bilgilerinden yaşı, cinsiyeti, şikayetleri ve endoskopik bulguları kaydedildi. İstatiksel değerlendirmede iki grup karşılaştırmalarında normal dağılım gösteren değişkenler için Bağımsız İki Örnek t testi, normal dağılım göstermeyen değişkenler için Mann-Whitney U testi, kategorik değişkenlerin karşılaştırılmasında Ki-kare testi kullanıldı.Bulgular: Toplam 335 hastanın (%10.8) özofagusunda patoloji mevcuttu (205 hastada endoskopik, 12 hastada histopatolojik, 118 hastada hem endoskopik hem de histopatolojik). The purpose of this study was to investigate the prevalence, etiology and endoscopic and histopathological characteristics of esophageal pathologies in children.Material and Methods: Patients aged 1-17 and undergoing esophagogastroduodenoscopy in our unit between May 2009 and June 2015 were investigated retrospectively (n=3081). Patients’ ages, sex, symptoms and endoscopic findings were recorded from their medical files. Amaç: Bu çalışma ile amacımız spontan mezenterik fibromatozis vakalarında kliniğimizin cerrahi yaklaşımını, tümörlerin özellikleri, klinik prezentasyonlarını ve uzun dönem takip sonuçlarını literatür eşliğinde ortaya koymaktır. Gereç ve Yöntem: Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Genel Cerrahi Anabilim Dalında Ocak 2005 ve Aralık 2014 tarihleri arasında sporadik mezenterik fibromatozis nedeni ile opere edilen 8 hastanın verileri geriye dönük olarak incelenmiştir. Bulgular: Hastaların ortalama yaşı 46.719.9 idi. Hastalardan 4 (%50)'ü ileus tanısı ile acil şartlarda ameliyat edilmiş, müphem karın ağrısı şikayeti olan 2 (%25) mezenterik kitle saptanmış, 1 (%12.5) hasta üroloji tarafından yapılan benign sebepli bir cerrahi esnasında ince barsak mezosunda kitle saptanması üzerine opere edilmiş ve 1 (%12.5) hastada distal pankreas kaynaklı olabilecek kitle nedeni ile yapılan ameliyat sonrası piyesin patolojisi mezenterik fibromatozis olarak raporlanmıştır. Ortalama 33.534.5 aylık takip sonrasında sadece 1 (%12.5) hastada nüks ve reoperasyon ihtiyacı olmuştur. Sonuç: Patolojik olarak benign bir tümör özelliği gösterse de klinik olarak agresif seyreden ve nüks oranı yüksek olan bu tümörün tedavisinde temel prensip geniş cerrahi rezeksiyondur., Objective: Our aim with this study is to present the surgical approach of our clinic, the characteristics of the tumor, clinical presentation and long-term follow-up results with the literature in the cases of spontaneous mesenteric fibromatosis. Material and Methods: The data collected from 8 cases of patients, who had underwent surgery because of sporadic mesenteric fibromatosis at Necmettin Erbakan University Medical School's Department of General Surgery between January 2005 and December 2014, were retrospectively evaluated. Results: The mean age of the patients was 46.7±19.9. Four (50%) of the patients were taken into emergency surgery having been diagnosed with ileus, 2 (25%) patients were ascertained to have mesenteric mass upon complaints of obscure abdominal pain, 1 (12.5%) patient underwent surgery on the detection of a mass in the meso of the small intestines during a surgical procedure performed by urologists because of a benign cause and in 1 (12.5%) patient's case, the pathology results of the piece, which was removed after a surgical procedure performed because of a mass that might have been originated from the distal pancreas, was reported to be mesenteric fibromatosis. Only 1 (12.5%) patient had recurrence and necessitated re-operation following an average of 33.5±34.5 months of follow-up. Conclusion: The fundamental principle in the treatment of this clinically aggressive tumor with a high recurrence rate, although it has the pathological characteristics of a benign tumor, is wide surgical resection.

Superior mesenteric artery syndrome is a rare and life-threatening clinical condition caused by the compression of the third portion of the duodenum between the aorta and the superior mesenteric artery\'s proximal part. This compression may lead to chronic intermittent, acute total or partial obstruction. Sudden weight-loss and the related decrease in the fat tissue are considered to be the etio­logical reason of acute stenosis. Weight-loss accompa­nied by nausea, vomiting, anorexia, epigastric pain, and bloating are the leading complaints. Barium radiographs, computerized tomography, conventional angiography, tomographic and magnetic resonance angiography are used in the diagnosis. There are medical and surgical ap­proaches to treatment. We hereby present the case of a patient with superior mesenteric artery syndrome with delayed diagnosis., Superior mezenterik arter sendromu duodenum üçüncü kısmının aorta ve süperior mezenterik arter proksimal kıs­mı arasında basıya uğraması sonucu ortaya çıkan, nadir görülen ve yaşamı tehdit eden bir klinik tablodur. Bu bası kronik aralıklı, akut total ya da parsiyel obstrüksiyona yol açabilir. Ani gelişen kilo kaybı ve buna bağlı yağ doku­da azalma akut daralmanın etyolojik sebebi olarak kabul edilmektedir. Kilo kaybı ile birlikte giden bulantı, kusma, anoreksi, epigastrik ağrı ve şişkinlik başlıca yakınmalar­dır. Tanıda baryumlu grafiler, bilgisayarlı tomografi, kon­vansiyonel anjiografi, tomografik ve manyetik rezonans anjiografi kullanılmaktadır. Tedavide medikal ve cerrahi yaklaşımlar mevcuttur. Hidatik kist bazı bölgelerde yaygın görülen bir paraziter hastalıktır. Vücutta birçok organda yerleşim gösterebilir. Ancak pankreas nadir tutulan bir organdır. Ameliyat öncesi tanısı konulamayan 48 yaşındaki bir hasta ameliyat edildikten sonra pankreas unsinat proseste yerleşmiş hidatik kist saptandı. Postoperatif albendazol verildi. Kontrol ultrasonografide herhangi bir problemle karşılaşılmadı. Nadir görülen bu olgu pankreasın kistik maligniteleri ve psödokistleriyle karıştırılabilir. (Turkiye Parazitol Derg 2011; 35: 214-6), Hydatid cyst is a parasitic disease that is commonly seen in certain areas. It may localize in many organs in the body. Pancreas is a rarely involved organ. A 48 years old patient who could not be diagnosed before surgery was then diagnosed with hydatid cyst localized in the uncinate process of the pancreas post-operatively. Albendazole treatment was begun after the operation. No problem was observed in the follow-up ultrasonography. This rare case may be mistaken for cystic malignancies and pseudocysts of pancreas. Amaç: Non-hodgkin lenfoma (NHL)’lar lenfositlerden kaynaklanan tümörlerin farklı bir grubudur. Kolonun Nonhodgkin lenfomaları nadirdir. Gastrointestinal sistem ekstranodal non-hodgkin lenfomaların yaygın yerleşim bölgesidir. Akut batın oluşturmuş kolonun NHL’sini sunmayı amaçladık. Olgu sunumu: Seksen iki yaşında erkek hasta yaklaşık 1 aydır sağ alt kadranda karın ağrısı şikayeti nedeniyle kliniğimize başvurdu. Hastanın yapılan fizik muayenesinde akut batın bulguları olması üzerine acil operasyona alındı. Sağ hemikolektomi uygulandı. Sonuç: Nadir görülen intestinal NHL’nin prognozu histolojik tipe, evreye ve total eksizyona bağlıdır. Bu tümörlerde ilk tedavi protokolü cerrahi olmakta ve sonrasında kemoterapi veya radyoterapi kombinasyonu eklenmektedir, Objective: Non-Hodgkin’s lymphomas (NHL) are a diverse group of cancer which arises from lymphocytes. NHL of colon is rare. The gastrointestinal tract is the predominant site of extranodal NHL. We aimed to present a case of NHL of colon with acute abdominal appearance. Case report: Eighty-two years-old male patient admitted to our service for right lower quadrant of stomach ache lasted approximately for 1 month. The patient had operated for acute abdomen signs in physical examination urgently. It was applied right hemicholectomy procedure. Conclusion: Prognosis depends on type of histology, stage and total excision. In these tumors surgery is the first choice of therapy protocol and then combination of chemotherapy and radiotherapy is added.

Karaciğer kist hidatiği dünya çapında görülen bir hastalıktır. Kist ciddi boyutlara ulaştığı zaman dispepsi, karın ağrısı gibi spesifik olmayan bası semptomlarına yol açabilir. Sık görülen komplikasyonlar rüptür ve sekonder enfeksiyondur. Bununla birlikte hidatik kistin gastrointestinal sisteme fistülizasyonu oldukça nadir görülen bir komplikasyondur. Altı yıl önce hidatik kist nedeni ile kistektomi yapılan 56 yaşında erkek hasta dispeptik yakınmaları olması üzerine kliniğimize başvurdu. Batın ultrasonografisinde nüks kiste rastlanan hastaya ileri tetkik olarak BT, MRCP görüntüleme ve üst gastrointestinal sistem endoskopisi yapıldı. Nüks hidatik kist ve kistoduodenal fistül tespit edildi. Cerrahi müdahale planlandı ve perikistektomi ile beraber fistül traktı eksize edildi. Ameliyat sonrası dönemde herhangi bir komplikasyon gelişmeyen hastaya antihelmitik tedavi uygulandı. Primer ve nüks hidatik kist olgularında kistoduodenal fistül gibi çok nadir komplikasyonlar görülebileceği akılda tutulmalıdır., A Rare Recurrent Hydatid Cyst Complication: Cystoduodenal Fistula Hepatic hydatid cyst is a disease seen worldwide. The cyst may cause non-specific pressure symptoms like dyspepsia and abdominal pain when it grows into significant sizes. Rupture and secondary infections are the frequently seen complications. Nonetheless, the fistulization of the hydatid cyst into the gastrointestinal system is a very rare complication. A 56-year-old male patient had had cystectomy because of hydatid cyst 6 years ago. He presented to our clinic with dyspeptic complaints. The patients abdominal ultrasonography showed recurrent mass and CT and MRCP imaging and upper gastrointestinal system endoscopy procedures were performed for the patient as advanced diagnostic techniques. The patient was seen to have recurrent hydatid cyst and cystoduodenal fistula. The case was followed-up for two years and did not have any other recurrences. It should be noted that very rare complications like cystoduodenal fistula may be seen in primary and recurrent hydatid cyst cases.

Amaç: Gastrointestinal stromal tümörlerin lokal etkileriyle immünsupresyonda rol oynadığı düşünülmektedir. İnflamasyonla ilişkili bazı kanserlerde tedavi öncesi nötrofil lenfosit oranındaki artış kötü prognozun göstergesi olarak kabul edilmektedir. Çalışmamızın amacı gastrointestinal stromal tümör hastalarında kan nötrofil lenfosit oranı ile hastalığın prognozunu değerlendirmektir. Gereç ve Yöntemler: Kliniğimizde primer gastrointestinal stromal tümör nedeniyle cerrahi uygulanan 78 hastanın verileri retrospektif olarak değerlendirildi. Hastaların operasyon öncesi periferik kandan bakılan nötrofil lenfosit oranları belirlendi. Nötrofil lenfosit oranı ile tümör riski ve prognoz arasındaki ilişki karşılaştırıldı. Verilerin değerlendirilmesinde Pearson korelasyon analizi ve ANOVA Welch testi kullanıldı. Bulgular: Preoperatif nötrofil lenfosit oranındaki artış patolojik olarak yüksek riskli gastrointestinal stromal tümör gruplarında anlamlıydı (p0,05). Preoperatif nötrofil lenfosit oranı yüksek olan hastaların sağkalımları düşüktü (r- 0,32, p0,009). Ayrıca nötrofil lenfosit oranı artışı ile tümör mitotik aktivitesi artışı benzerdi (r-0,364, p0,025). Sonuç: Gastrointestinal stromal tümörlerde preoperatif nötrofil lenfosit oranı yüksek riskli tümörlerin belirlenmesinde ve kötü prognozun göstergesi olarak kullanılabilecek bir parametredir., Objective: Gastrointestinal stromal tumors are believed to play a role in immunosuppression with their local effect. In some cancers with associated inflammation, an increased pretreatment neutrophil lymphocyte ratio is considered as an indicator of poor prognosis. The aim of our study was to assess the relationship of increased blood neutrophil lymphocyte ratio with disease prognosis in patients with gastrointestinal stromal tumors. Material and Methods: The data of 78 patients who underwent surgery with the diagnosis of primary gastrointestinal stromal tumor in our clinic were evaluated retrospectively. The preoperative neutrophil lymphocyte ratio in the peripheral blood was determined. The neutrophil lymphocyte ratio and its relationship with tumor risk and prognosis were compared. The data were evaluated by Pearson's correlation analysis and the Welch ANOVA test. Results: The preoperative neutrophil lymphocyte ratio was significantly increased in the high-risk groups (p>0.05). An increased preoperative neutrophil lymphocyte ratio was associated with shorter survival (r-0.32, p0.009). In addition, an increase in the neutrophil lymphocyte ratio was associated with an increase in the mitotic activity of the tumor (r-0.364, p0.025). Conclusion: The preoperative neutrophil lymphocyte ratio in gastrointestinal stromal tumors can be used as an indicator of high-risk tumors and poor prognosis.

Amaç: Restoratif proktokolektomi günümüzde ülseratif kolit ve FAP hastalığında kullanılan sfinkter koruyucu seçkin bir tedavidir. Kliniğimizde yapılan ileal poşlu girişimleri ve sonuçlarını literatür eşliğinde irdelemeyi amaçladık.Gereç ve Yöntemler: Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi'nde 2006-2013 yılları arasında yapılan 35 restoratif proktokolektomi ve J poş ileoanal anastomoz ameliyatlarının sonuçları retrospektif olarak hastaların dosyalarından incelendi. Hastalar yaş, cinsiyet, hastanede kalış süresi, tanı, takip süresi ve poş komplikasyonlarına göre değerlendirildi. Tüm hastalara telefonla ulaşıldı ve kontrol için poliklinik randevusu verildi.Bulgular: Hastaların 19'u erkek (%54) ve 16'sı kadın (%46) idi. Ortalama yaş 45 idi. Ortalama hastanede kalış süresi 11 gündü. Hastaların 22'si (%63) FAP, 12'si (%34) senkron rektum kanseri ile birlikte kolon tümörü veya polibi ve 1'i (%3) ülseratif kolit nedeniyle ameliyat edildi. Tüm hastalara J poş ve koruyucu ileostomi uygulandı. İleostomi kapatıldıktan sonra iki olguda J poş fistülü tespit edildi. Hastalar 6 ay ile 7 yıl arasında takip edildi. Olgulara telefonla ulaşılıp aktif şikâyetleri, defekasyon sayısı üriner ve cinsel disfonksiyonları soruldu. Ortalama günde 5 dışkılama olduğu, 4 (%11) olguda 1 defa olan gece dışkılaması tespit edildi. Yapılan endoskopik kontrol incelemesinde poşit tespit edilmedi.Sonuç: Restoratif proktokolektomi ve J poş ileoanal anastomoz ameliyatı ileri yaş dâhil düşük morbidite ve mortalite oranları ile uygulanabilir bir cerrahi prosedürdür., Objective: Retrospective proctocolectomy is a distinguished, sphincter saving treatment used for the treatment of ulcerative colitis and FAP disease. We aimed to evaluate ileal pouch interventions performed at our clinic and their results in the light of literature.Material and Methods: Medical records of 35 restorative proctocolectomy and J pouch ileo-anal anastomosis surgeries performed at Necmettin Erbakan University, Meram School of Medicine between the years 2006-2013 were retrospectively examined. The patients were assessed according to their age, gender, length of hospital stay, diagnosis, follow-up duration and pouch-related complications. All patients were contacted by phone and they were scheduled for controls at the outpatient clinic.Results: Nineteen patients were male (54%) and 16 were female (46%). Their mean age was 45 years . The mean length of hospital stay was 11 days. Twenty two (63%) patients were operated on due to FAP, 12 (34%) due to synchronous rectum cancer and colon tumor or polyp, and one (3%) due to ulcerative colitis. All patients received J pouch and protective ileostomy. After the closure of ileostomy, two cases were identified to have J pouch fistulas. The patients were followed up for 6 months to 7 years. They were contacted by phone and they were questioned about their active complaints, number of defecations, urinary and sexual dysfunctions. It was identified that they had 5 defecations per day on average and that 4 (11%) cases had one nocturnal defecation. No pouchitis were identified in the follow-up endoscopic examinations.Conclusion: Restorative proctocolectomy and ileo-anal anastomosis technique is a surgical procedure that can be performed with low rates of morbidity and mortality, including the elderly.

Amaç: Nöroektodermal orijinli nadir bir tümör olan Feokromasitomalı olgulardaki klinik deneyimlerimizi gözden geçirmektir. Yöntem: 2005 ve 2012 yılları arasında kliniğimizde feokromasitoma tanısı konmuş 14 hastanın klinik, laboratuar, radyolojik ve cerrahi verileri geriye dönük olarak değerlendirilmiştir. Bulgular: Kliniğimizde feokromasitoma tanısı ile tedavi gören 14 hastanın 8'i erkek, 6'sı kadın olup yaşları 18 ve 60 arasında değişmekte idi. Hastaların 10 (%71)'unda hipertansiyon mevcuttu. En sık semptom karın ağrısı (%57) idi. Yerleşim yerleri açısından sekizi (%57) solda, beşi (%35) sağ ve birinde (%7) tümör bilateral adrenal yerleşimliydi. Tanıda kullanılan görüntüleme yöntemi ultrasonografisi ve bilgisayarlı tomografiydi. Hastalarımızın hiç birisinde ailesel feokromasitoma sendromu ile ilişkili değildi. Bütün hipertansif hastalara preoperatif fenoksibenzamin ve propranolol verildi. Cerrahi işlem dokuz olguda laparoskopik, beşinde ise laparotomi ile gerçekleştirildi. Hipertansif hastaların % 80'inde cerrahi sonrasında remisyon gözlendi. İki hastada malign feokromasitoma tespit edildi. Tüm hastaların takibi 24 saatlik idrarda VMA düzeyi ve batın BT ile düzenli olarak yapıldı. Takiplerde olguların birinde karaciğer metastazı tespit edildi. Sonuç: Bu çalışma ile feokromasitomanın nonspesifik ve değişken bir kliniğinin olduğu gösterilmek istendi. Sıklıkla tümör rastlantısal olarak bulunmaktadır. Feokromasitoma nadir bir tümör olmasına rağmen, doğru bir değerlendirme, iyi bir preoperatif hazırlık ve tam cerrahi eksizyon ile tedavi edilebilir., Objective: The aim of our study is to review our clinical experiences with pheochromocytoma case s which are rare tumors with neuroectodermal origins. Method: The clinical, lab, radiological, and surgical data of a total of 14 patients diagnosed with pheochromocytoma between 2005 and 2012 at our clinic were evaluated retrospectively. Results: Eight out of 14 patients who received treatment at our clinic having been diagnosed with pheochromocytoma were male, while 6 were female and their ages ranged between 18 and 60. 10 (71%) of the patients had hypertension. The most frequent symptom was abdominal pai n (57%). Eight tumors were located on the left (57%) and five were located on the right (35%), while one tumor (7%) had bilateral adrenal location. The imaging techniques used in diagnosis were ultrasonography and computerized tomography. None of our patie nts had a history of familial pheochromocytoma. All the hypertensive patients were administered pre-op phenoxybenzamine and propranolol. The surgical procedures were performed by laparoscopy in nine cases, and by laparotomy in five. Post-op remission was o bserved in 80% of the hypertensive patients. Malign pheochromocytoma was detected in two patients. The follow-up of all the patients was conducted through testing the VMA level in 24-hour urine and through abdominal CT regularly. The follow-ups revealed that one case had liver metastasis. Conclusion: This study aims at demonstrating that pheochromocytoma has a non-specific and unstable clinic. The tumor is frequently found randomly. Although pheochromocytoma is a rare tumor, it can be treated through a right evaluation, good pre-op preparation, and complete surgical excision.

Amaç: Bu çalışmada kliniğimizde meme kanseri nedeni ile meme koruyucu cerrahi uygulanmış olan hastaların uzun dönem sonuçları sunulmuştur. Yöntemler: Necmettin Erbakan Üniversitesi Meram Tıp Fakültesi Genel Cerrahi anabilim dalında Ocak 2005 ve Aralık 2013 tarihleri arasında meme kanseri nedeni ile meme koruyucu cerrahi uygulanan 99 hastanın verileri geriye dönük olarak incelenmiştir. Çalışmamıza ait analizler SPSS 21,0 paket programı kullanılarak yapıldı. Bulgular: Toplam hasta sayısı 99 ve yaş ortalaması 51.11 9,62 (31-74) idi. En sık görülen patoloji infiltratif duktal karsinom idi (62 (%62,6) hastada). Vakaların %88,7 (87 olgu)'sine operasyon sonrası adjuvan radyoterapi uygulandı. Ortalama 38,86 23,43 (5-92) aylık takipler sonrasında sadece 1 hasta yaşamını yitirmiştir. 1 hasta operasyondan 70 ay sonra gelişe lokal nüks ve 3 hasta cerrahi sınırda tümör tespit edilmesi nedeni ile yeniden opere edildi. Sonuç: Meme kanserinin cerrahi tedavisindeki temel yaklaşım kabul edilebilir onkolojik sonuçlar sağladıktan sonra hastanın yaşam kalitesini en az etkileyecek yöntemi seçmektir. Bu noktada meme koruyucu cerrahinin doğru seçilmiş hastalarda radikal cerrahi yöntemler yerine tercih edilmesi uygun bir yaklaşımdır., Objective: This study presents the long-term results of patients who had breast conserving surgery at our clinic because of breast cancer. Methods: The data of 99 patients, who had breast conserving surgery because of breast cancer between January 2005 and December 2013 at Necmettin Erbakan University, Meram Medical Schools Department of General Surgery, were retrospectively evaluated. The analyses of the study were conducted by the SPSS 21.0 program. Results: The total number of patients was 99 and the mean age was 51.11 ± 9.62 (31-74). The most frequently seen pathology was infiltrative ductal carcinoma and it was detected in 62 (62.6%) patients. 88.7% (87 cases) of the cases received adjuvant radiotherapy in the post-op period. Following the mean follow-up period of 38.86 ± 23.43 (5-92) months, only one patient passed away. Further, 1 patient underwent surgery again after having local recurrence 70 months later and 3 patients also underwent surgery again upon detecting that they had tumors on the surgical border. Conclusion: After obtaining acceptable oncological results, the fundamental approach in the surgical treatment of breast cancers is to select the method that will have the least negative effect on the patients quality of life. At this point, selecting breast conserving surgery over radical surgical methods is an appropriate approach for suitable patients chosen carefully.

Akut gastrik dilatasyon ilk olarak 1833 yılında Duplay tarafından tarif edilmiştir. Biz akut mide dilatasyonuna bağlı mide nekrozu ve perforasyonu saptanan 55 yaşındaki bir erkek hastayı literatür eşliğinde sunmak istedik. Mide nekrozu ve perforasyonu oldukça nadir görülür, çünkü mide zengin bir kan dolaşımına sahiptir. Klinik olarak olguların %90'ından fazlasında kusma şikayeti mevcuttur. Tanıyı ve etyolojiyi ortaya koymada bilgisayarlı tomografi önemlidir. Peritonit bulguları olmayan ve endoskopide nekroz gelişmemiş hastalarda medikal tedavi yararlı olabilir. Cerrahi tedavide geç kalınan olgularda mortalite oranı yüksektir, Acute gastric dilatation was first defined by Duplay in 1833. We herein present the case of a 55-year-old male patient diagnosed with gastric necrosis and perforation caused by acute gastric dilatation. Since the stomach has a rich blood circulation, necrosis and perforation are rarely seen. Clinically, more than 90% of cases have complaints of vomiting. The most useful method in revealing the diagnosis and aetiology is computerized tomography. Medical treatment is appropriate for cases where no necrosis and peritonitis findings are detected through endoscopy. Delays in surgical treatment increase the risk of mortality.

Adrenal bezin kistik lezyonları nadir görülür ve bunların ancak %6'sını hidatik kistler oluşturur. Adrenal bezi tutmuş dev hidatik kist olgusunu literatür eşliğinde sunmayı amaçladık. 30 yaşında kadın hasta, 3 aydır karın şişliği ve ağrısı mevcuttu. Fizik muayenede batın sağ yarısın da belirgin bir asimetri ve düzgün yüzeyli kitle lezyonu palpe edilmekte idi. Yapılan tetkiklerde sağ sürrenal hidatik kist tanısı kondu. Laparoskopik parsiyel kistektomi işlemi uygulandı ve hasta ameliyat sonrası 3. günde 20 mg/kg albendazol tedavisi ile sorunsuz şekilde taburcu edildi. Adrenal kaynaklı kistlerin ayırıcı tanısında, özellikle endemik olduğu bölgelerde hidatik kist de mutlaka akılda tutulmalıdır., Cystic lesions of the adrenal gland are rarely seen and only 6% of these account for hydatid cysts. We aim at presenting a case of giant hydatid cyst involving the adrenal gland alongside with literature. 30-year-old female patient was presented with abdominal distensi- on and pain present for 3 months. The patient's physical examination revealed a distinctive asymmetry and a palpable mass lesion with a smooth surface on the right side of the abdomen. The patient was diagnosed with right surrenal hydatid cyst through the imaging techniques. Laparoscopic partial cystectomy was performed and the patient was discharged without any problems on post-op 3rd day with 20 mg/kg albendazole treatment. Hydatid cyst should be kept in mind of the differential diagnosis of adrenal cyst, especially in areas where it is endemic

Hidatik kist en sık karaciğer ve akciğerde yerleşerek hastalık oluşturur. Retroperitoneal yerleşimli hidatik kist çok nadir olarak görülür. Kırk beş yaşında kadın hasta sırt ağrısı, kilo kaybı ve yorgunluk şikayetleri ile hastanemize müracaat etti. Bilgisayarlı tomografide (BT) sol böbrek posteriorunda, paravertebral alanda retroperitoneal yerleşimli komşuluğundaki kostayı destrükte eden yaklaşık 8x7x6 cm boyutlarında septalı kistik lezyon izlendi. Laparotomide retroperitoneal yerleşimli kistin duvarı kısmen eksize edilerek kalan kavite drene edildi, dren ameliyat sonrası 5. gün çıkartıldı. Histopatolojik tanısı hidatik kist olarak rapor edildi. Postoperatif albendazol verilen hastanın kontrollerinde herhangi bir problemle karşılaşılmadı. Hidatik kist ülkemizde endemik bir hastalık olup atipik yerleşimleri de olduğu bilinmelidir., Hydatid cysts cause diseases most frequently by localizing in the liver and the lungs. Hydatid cysts with retroperitoneal localization are very rare. A 45-year-old female patient presented to our hospital with complaints of back pain, weight loss, and fatigue. The computerized tomography (CT) revealed that the patient had a septated cystic lesion of about 8x7x6 cm localized in the posterior of the left kidney, in the paravertebral site causing destruction of the neighboring costa. During laparotomy, the wall of the cyst with retroperitoneal localization was partially excised and the remaining cavity was drained. The drain was removed on post-op day 5. The histopathological diagnosis was reported to be a hydatid cyst. No problems were seen during the follow-ups of the patient who was administered post-op albendazole. Hydatid cysts are an endemic disease in our country and it should be kept in mind that they also have atypical localizations.

TÜRK CEZA HUKUKUNDA DAVA ZAMANAŞIMI

Amaç: Gastrointestinal sistemde Malign Melanom beklenmedik bir durumdur fakat nadir değildir. Primer Malign melanom anorektol bölgede nadir görülür. Uzun dönem yaşam beklentisi nadirdir. Çoğu hasta tedavi edilemeyen sistemik hastalık nedeniyle kaybedilmektedir. Bu çalışmada Malign Melanomla ilgili klinik deneyimlerimizi aktararak, tanı ve tedavisi ile ilgili güncel literatür bilgilerini paylaşmayı amaçladık. Materyal ve metot: 2006-2011 tarihleri arasında cerrahi girişimde bulunulan ve patolojik tanısı Malign Melanom olan 10 olgu retrospektif olarak incelendi. Hastalar yaş, cinsiyet, klin görünüm, metastaz varlığı, immünhistokimyasal özelliği, takip süreleri ve uygulanan cerrahi girişimin açısından değerlendirildi. Bulgular: Hastaların 4'ü kadın, 6'si erkek olup yaş ortalaması 67,7 (50-83)' idi. Beş hastada cerrahi tedavi ile kür sağlandı. Dört hasta inoperable olarak tespit edildi. Tüm vakalarda S-100 pozitifti. Hastalar ortalama 15 (5-24) ay takip edildi. Sonuçlar: Primer anorektal malign melanom, anorektal tümörlerin ayırıcı tanısında nadir bir nedendir. Tercih edilen tedavi seçeneği cerrahidir. Palyatif amaçlı kemoterapi ve immünoterapi kullanılır. Tedavi yöntemlerindeki gelişmelere rağmen, bu tümörün prognozu halen kötüdür., Background: Malignant melanoma in the gastrointestinal tract is uncommon but not rare. Primary malignant melanoma is a rare entity in the anorectum. Longterm survival is rare, as most patients die of disseminated systemic disease regardless of treatment. In this study we are aiming to share the most up to date knowledge about diagnosis and treatment of these tumors by transferring our clinical experience about malignant melanoma. Methods: The 10 patients who was operated and pathologic examination were reported as malignant melanoma between 20062011 years have been viewed retrospectively. Patients' age, sex, clinical, presence of metastasis, survival and immunohistopathologic features and treatment methods were evaluated. Results: Four of patients were women and 6 of them were men whose age averages were 67,7 (50-83). With a surgical intervention 5 of them had been cured. 4 cases accepted as an inoperable. S-100 was positive in our all cases. Patients had been observed for 15 (5-24) months. Conclusions: Primary anorectal malignant melanoma is a rare cause in the differential diagnosis of anorectal tumors. Surgery remains the therapy of choice. Chemotherapy and immunotherapy are principally used in a palliative setting. Despite advances in therapy, the prognosis of this tumor entity remains unfavorable.
