Systemic sclerosis is a systemic connective tissue disease whose main pathophysiological mechanism is a progressive fibrosis of internal organs and skin leading to thickening and induration. Blood vessels may also be involved. However, systemic scleroderma is not the only disease causing cutaneous sclerosis. There is a group of diseases that mimic scleroderma in their clinical presentation - these are scleroderma-like syndromes. A distinction can be made between syndromes of inflammatory/autoimmune, genetic, metabolic, toxic, drug-induced, occupational, paraneoplastic and syndromes caused by deposition disorders. In the following paper, we have reviewed the literature on scleroderma-like syndromes. We have outlined the factors predisposing to the development of each disease, its pathogenesis, clinical presentation, diagnostic and treatment process and the differences between each syndrome and systemic scleroderma. Knowing energy performance level of an energy consumer is essential for substantiating the decisions for development, operation and maintenance. Energy performance level is highlighted through consecrated indicators which are obtained by means of energy balance. The present paper is a synthesis of electric energy balance that was done within a mixed fodder factory (MFF). In the first part a short presentation of the consumer (MFF) is presented and also analysis methodology. After that we refer to specificities regarding the energy balance model (EB) applied and to characterization elements of typical processes for MFF. An important part of the paper is dedicated to present the obtained results and the energy performance level. In the quest for sustainable and energy-efficient building solutions, the incorporation of phase change materials (PCMs) into building envelopes emerges as a groundbreaking strategy. PCMs, renowned for storing and releasing thermal energy during phase transitions, stand as a promising avenue to curtail energy consumption while enhancing thermal performance. This study rigorously explores the potential energy savings and thermal comfort benefits achievable through PCM integration into building envelopes. Multiple energy simulations are conducted on a residential building model in diverse locations, including Irbid, Amman, and Aqaba in Jordan, and the city of Oradea in Romania, utilizing the EnergyPlus simulation tool embedded in DesignBuilder software v7.0.2.006. The results reveal that BioPCM®, derived from renewable biomass, significantly elevates thermal performance owing to its heightened latent heat of fusion. Optimal outcomes materialize with a PCM melting point of 23 °C, a configuration closer to the interior surface, and a thickness of 37.1 mm. The study underscores the superior performance in moderate climates (Irbid and Amman) compared to hot-dry climates (Aqaba) and cold-wet climates (Oradea, Romania). Financially and environmentally, incorporating PCM in Amman demonstrates potential annual energy savings of 5476.14 kWh, translating to a cost reduction of 1150 USD/year, and a decrease in GHG emissions by 2382.31 kgCO2eq. The estimated payback period for PCM incorporation in external walls is four years, robustly emphasizing the feasibility and multifaceted benefits of this energy-efficient solution. Ultrasonography is an imaging technique based on sound waves used for the evaluation of soft tissues. Sound waves have been used for a long time in nonmedical fields, including military defense systems, radar systems, and detection of icebergs. Technological advances resulted in new techniques becoming available for medical imaging, including ultrasonography, magnetic resonance imaging, and computed tomography. Nowadays, the use of imaging has become a gold standard protocol in the diagnosis of many diseases, and recently developed diagnosis and therapy options provide more efficient treatment of rheumatic diseases. Thus, it has become possible to prevent structural damage and disability in patients with rheumatic disease. Musculoskeletal ultrasonography is becoming a preferred imaging technique for rheumatic diseases, as it has many advantages. Among its advantages are being inexpensive, being radiation-free, having a dynamic image capacity, helping to detect disease activity, and helping with early detection and diagnosis of structural damage. The article presents an analysis of the statistical relationship between the determinants of and barriers to the development of renewable energy sources (RESs) in the macroeconomic system and the development of renewable energy source consumption in individual European Union countries. The article considers four key categories of RES development barriers in the European Union: political, administrative, grid infrastructural, and socioeconomic. The work is based on publicly available historical data from European Union reports, Eurostat, and the Eclareon RES Policy Monitoring Database. The empirical analysis includes all 27 countries belonging to the European Union. The research aimed to determine the impact of all four types of factors, including socioeconomic, on the development of RESs in European Union countries. The analysis uncovered that describing the European Union as a consistent region regarding the speed of renewable energy advancement and the obstacles to such progress is not accurate. Notably, a significant link exists between a strong degree of societal development and the integration of renewable energy sources. In less prosperous EU nations, economic growth plays a pivotal role in renewable energy development. Barriers of an administrative nature exert a notable influence on renewable energy development, especially in less affluent EU countries, while grid-related obstacles are prevalent in Southern–Central Europe. In nations where the proportion of renewable energy sources in electricity consumption is substantial, an excess of capacity in the renewable energy market significantly affects its growth. Systemic connective tissue disorders constitute a heterogenous group of autoimmune diseases with the potential to affect a range of organs. Rheumatoid arthritis (RA) is a chronic, progressive, autoimmune inflammatory disease affecting the joints. Systemic lupus erythematosus (SLE) may manifest with multiple system involvement as a result of inflammatory response to autoantibodies. Spondyloarthropathies (SpAs) such as ankylosing spondylitis (AS) or psoriatic arthritis (PsA) are diseases characterised by the inflammation of spinal joints, paraspinal tissues, peripheral joints and enthesitis as well as inflammatory changes in many other systems and organs. Physiologically, sclerostin helps to maintain balance in bone tissue metabolism through the Wnt/β-catenin pathway, which represents a major intracellular signalling pathway. This review article aims to present the current knowledge on the role of sclerostin in the Wnt/β-catenin pathway and its correlation with clinical data from RA, SLE, AS and PsA patients. This study deepens and further concretizes an analysis conducted in a prior study highlighting Romania's goals and objectives in comparison with the sustainable development principles established through the adoption of the European Green Deal (EGD) at the European Union (EU) level. The second section of this paper presents this study's methodology, aiming to highlight the quantification of the compatibility of the targets of Romania's pertinent strategies with the EGD after evoking the principles of sustainable development (SD) and—in this context—the importance of achieving the objectives set by the EGD. Non-atherosclerotic aortic arch pathologies (NA-AAPs) and anatomical variants are characterized as rare cardiovascular diseases with a low incidence rate, below 1 case per 2000 population, but enormous heterogeneity in terms of anatomical variants, i.e., Takayasu disease (TAK) and fibromuscular dysplasia (FMD). In specific clinical scenarios, NA-AAPs constitute life-threatening disorders. Methods: In this study, 82 (1.07%) consecutive patients with NA-AAPs (including 38 TAKs, 26 FMDs, and 18 other AAPs) out of 7645 patients who underwent endovascular treatment (EVT) for the aortic arch and its side-branch diseases at a single institution between 2002 and 2022 were retrospectively reviewed. The recorded demographic, biochemical, diagnostic, operative, and postoperative factors were reviewed, and the functional outcomes were determined during follow-up. A systematic review of the literature was also performed. Results: The study group comprised 65 (79.3%) female and 17 (21.7%) male subjects with a mean age of 46.1 ± 14.9 years. Overall, 62 (75.6%) patients were diagnosed with either cerebral ischemia symptoms or aortic arch dissection on admission. The EVT was feasible in 59 (72%) patients, whereas 23 (28%) patients were referred for medical treatment. In EVT patients, severe periprocedural complications occurred in two (3.39%) patients, including one periprocedural death and one cerebral hyperperfusion syndrome. During a median follow-up period of 64 months, cardiovascular events occurred in 24 (29.6%) patients (5 deaths, 13 ISs, and 6 myocardial infarctions). Repeated EVT for the index lesion was performed in 21/59 (35.6%) patients, including 19/33 (57.6%) in TAK and 2/13 (15.4%) in FMD. In the AAP group, one patient required additional stent-graft implantation for progressing dissection to the iliac arteries at 12 months. A baseline white blood count (odds ratio [HR]: 1.25, 95% confidence interval [CI]: 1.11–1.39; p < 0.001) was the only independent prognostic factor for recurrent stenosis, while a baseline hemoglobin level (HR: 0.73, 95%CI: 0.59–0.89; p = 0.002) and coronary involvement (HR: 4.11, 95%CI: 1.74–9.71; p = 0.001) were independently associated with a risk of major cardiac and cerebral events according to the multivariate Cox proportional hazards regression analysis. Conclusions: This study showed that AAPs should not be neglected in clinical settings, as it can be a life-threatening condition requiring a multidisciplinary approach. This work aims to study the performances of the hydro aggregates in the Drăgan-Iad Development, focusing on determining the performances in order to make them more efficient. The objective of the tests was to evaluate the real possibilities of operation of hydro aggregates equipped with Francis turbines, in primary and secondary regulation regimes. For this purpose, the hydro aggregates from CHE Remeți, as well as the hydro aggregates from CHE Munteni, were subjected to some testing, which implies taking some data from the system, as well as electrical and mechanical measurements from the field, in order to use them for system services.[In the first part of the work, the essential characteristics of the Dragan-Iad Development [ADI] and the related hydro-units [AHE] are presented. (1) Background: We aim to investigate age-related changes in cartilage structure and composition in the metacarpophalangeal (MCP) joints using magnetic resonance (MR) biomarkers. (2) Methods: The cartilage tissue of 90 MCP joints from 30 volunteers without any signs of destruction or inflammation was examined using T1, T2, and T1ρ compositional MR imaging techniques on a 3 Tesla clinical scanner and correlated with age. (3) Results: The T1ρ and T2 relaxation times showed a significant correlation with age (T1ρ: Kendall-τ-b = 0.3, p < 0.001; T2: Kendall-τ-b = 0.2, p = 0.01). No significant correlation was observed for T1 as a function of age (T1: Kendall-τ-b = 0.12, p = 0.13). (4) Conclusions: Our data show an increase in T1ρ and T2 relaxation times with age. We hypothesize that this increase is due to age-related changes in cartilage structure and composition. In future examinations of cartilage using compositional MRI, especially T1ρ and T2 techniques, e.g., in patients with osteoarthritis or rheumatoid arthritis, the age of the patients should be taken into account. Simple Summary: Patients diagnosed with primary Sjögren's syndrome are characterized by an increased accumulation of mucosa-associated lymphoid tissue in the salivary and lacrimal glands due to chronic inflammation. Consequently, these patients present up to 40-fold higher risk of developing lymphoma, especially in the parotid gland, compared to the healthy population. Radiomics has recently proved its value in assessing tissue heterogeneity and proposing textural features that might become surrogates for biopsy. This retrospective study aimed to assess the potential value of radiomics in discovering textural analysis biomarkers associated with lymphoma development in the parotid glands of patients with primary Sjögren's syndrome based on MR images, which might provide new directions in assessing the disease. Non-Hodgkin Lymphoma (NHL) represents a severe complication and the main cause of morbidity in patients with primary Sjögren's syndrome (pSS). This study aimed to assess the role of textural analysis (TA) in revealing lymphoma-associated imaging parameters in the parotid gland (PG) parenchyma of patients with pSS. This retrospective study included a total of 36 patients (54.93 ± 13.34 years old; 91.6% females) diagnosed with pSS according to the American College of Rheumatology and the European League Against Rheumatism criteria (24 subjects with pSS and no lymphomatous proliferation; 12 subjects with pSS and NHL development in the PG, confirmed by the histopathological analysis). All subjects underwent MR scanning between January 2018 and October 2022. The coronal STIR PROPELLER sequence was employed to segment PG and perform TA using the MaZda5 software. A total of 65 PGs underwent segmentation and texture feature extraction (48 PGs were included in the pSS control group, and 17 PGs were included in the pSS NHL group). Following parameter reduction techniques, univariate analysis, multivariate regression, and receiver operating characteristics (ROC) analysis, the following TA parameters proved to be independently associated with NHL development in pSS: CH4S6_Sum_Variance and CV4S6_Inverse_Difference_Moment, with an area under ROC of 0.800 and 0.875, respectively. The radiomic model (resulting by combining the two previously independent TA features), presented 94.12% sensitivity and 85.42% specificity in differentiating between the two studied groups, reaching the highest area under ROC of 0.931 for the chosen cutoff value of 1.556. This study suggests the potential role of radiomics in revealing new imaging biomarkers that might serve as useful predictors for lymphoma development in patients with pSS. It is often said that early Romanian biblical translations from Church Slavonic follow the source texts slavishly. This is believed to be especially true about the 16th century Romanian Psalters, a group of seven texts (both printed and hand-copied) descending from a single translation. Indeed, these texts stay close to their Church Slavonic originals in topic, lexical content, and orthographical rules. However, we aim to describe how the 16th century translators and redactors dealt with Church Slavonic structures that could not be easily adapted into Romanian by means of formal equivalence. The Slavonic present participle, which appears plenty in the Slavonic Psalter, was chosen as litmus test. While theoretically having a formal correspondent in Old Romanian (the gerund), the Slavonic present participle has a range of uses and meanings that the Old Romanian gerund lacks. Thus, Romanian scribes must depart from the comfort of formal equivalence that calques and loans provide and choose the translation that convey meaning. The dynamic equivalence is obtained by selecting different solutions: gerunds, adjectives, objects and, most often, clauses, especially relative ones. Rendering participles with clauses (i.e. adjectives with verbs) forces the translator to make decisions going beyond the Slavonic participle itself. The analysis shows a tension between betraying the Slavonic text as little as possible and rendering it to the best of the redactor's ability. [ABSTRACT FROM AUTHOR]

The energy efficiency of a system and the performance level of its equipment and installations are the two key elements based on which the investment decision in its modernization is made. They are also very important for setting up optimal operation strategies. The energy audit is a well-known and worldwide recognized tool for calculating energy performance indicators and developing improvement measures. This paper is a synthesis of the energy audit results performed for a district heating network that uses geothermal energy as its primary source of energy. The location of the heating system is inside a university campus. The first part explains the necessity of a comprehensive study on district heating networks and introduces the defining elements that characterize the analyzed equipment and installations. The complex energy balance methodology that has been developed and applied to this district heating system is presented in the second part of the paper. Next, the methodology for collecting the input data for the energy and mass balance is explained. In the final part, the numerical values of the performance indicators and the technical measures that must be applied to improve energy efficiency are shown, and conclusions are drawn. Among other systemic autoimmune diseases, primary Sjögren syndrome (pSS) bears the highest risk for lymphoma development. In pSS, chronic antigenic stimulation gradually drives the evolution from polyclonal B-cell expansion to oligoclonal/monoclonal B-cell predominance to malignant B-cell transformation. Thus, most pSS-related lymphomas are B-cell non-Hodgkin lymphomas (NHLs), with mucosa-associated lymphoid tissue (MALT) lymphomas predominating, followed by diffuse large B-cell lymphomas (DLBCLs) and nodal marginal zone lymphomas (NMZLs). Since lymphomagenesis is one of the most serious complications of pSS, affecting patients' survival, a plethora of possible predisposing factors has been studied over the years, ranging from classical clinical, serological, hematological, and histological, to the more recently proposed genetic and molecular, allowing clinicians to timely detect and to closely follow-up the subgroup of pSS patients with increased risk for lymphoma development. Overall predisposing factors for pSS-related lymphomagenesis reflect the status of B-cell hyperactivity. Different clinical features have been described for each of the distinct pSS-related B-cell NHL subtypes. While generally pSS patients developing B-cell NHLs display a fairly good prognosis, outcomes in terms of treatment response and survival rates seem to differ depending on the lymphoma subtype, with MALT lymphomas being characterized by a rather indolent course and DLBCLs gravely affecting patients' survival. This study aimed to assess the effectiveness of MRI-based texture features of the lacrimal glands (LG) in augmenting the imaging differentiation between primary Sjögren's Syndrome (pSS) affected LG and healthy LG, as well as to emphasize the possible importance of radiomics in pSS early-imaging diagnosis. The MRI examinations of 23 patients diagnosed with pSS and 23 healthy controls were retrospectively included. Texture features of both LG were extracted from a coronal post-contrast T1-weighted sequence, using a dedicated software. The ability of texture features to discriminate between healthy and pSS lacrimal glands was performed through univariate, multivariate, and receiver operating characteristics analysis. Two quantitative textural analysis features, RunLengthNonUniformityNormalized (RLNonUN) and Maximum2DDiameterColumn (Max2DDC), were independent predictors of pSS-affected glands (p < 0.001). Their combined ability was able to identify pSS LG with 91.67% sensitivity and 83.33% specificity. Biological soft tissues are characterized by viscoelastic properties. The propagation of shear waves within tissues is influenced by both elasticity, which is linked to the shear wave speed, and viscosity, which is linked to the shear wave dispersion. This study aimed to functionally assess the parotid glands (PG) and submandibular glands (SMG) in a group of 40 healthy subjects using the novel Viscosity PLUS (Vi.PLUS) and 2D Shear-Wave Elastography PLUS (2D-SWE.PLUS) techniques. The viscosity and stiffness of PG and SMG were measured before and after gustatory stimulation with a sialagogue agent (commercially available lemon juice) using the new SuperSonic MACH 30 ultrasound system equipped with a curvilinear C6-1X transducer. PG presented a mean basal viscosity and elasticity of 2.10 ± 0.19 Pa.s and 11.32 ± 1.91 kPa, respectively, which significantly increased poststimulation to 2.39 ± 0.17 Pa.s (p < 0.001) and 12.58 ± 1.92 kPa (p < 0.001), respectively. SMG did not present statistically increased values of viscosity and elasticity following stimulation (2.31 ± 015 Pa.s vs. 2.37 ± 0.18 Pa.s, p = 0.086, and 10.40 ± 1.64 kPa vs. 10.90 ± 1.98 kPa, p = 0.074, respectively). Vi.PLUS measurements presented a good positive correlation with 2D-SWE.PLUS values for PG and SMG, before and after stimulation. Gender and BMI were not confounding factors for these two parameters. Vi.PLUS represents an innovative non-invasive imaging technique that, together with 2D-SWE.PLUS proves to be useful in functionally assessing the major salivary glands in healthy subjects. Background: Shoulder impingement syndrome is the painful entrapment of the soft tissues between the acromion and the humeral head. The severity of shoulder impingement could be graded according to the limitation of shoulder joint moment. The reliability of sonographic findings in the grading of shoulder impingement severity grading is required to be evaluated by the consistency of findings between the observers. Purpose: To assess the interobserver agreement in the sonographic severity grading of shoulder impingement syndrome with the help of a ratio between acromion-to-greater tuberosity distance in the abduction and neutral arm position. Material and methods: Patients were examined by two independent observers in the coronal approach with neutral arm position. Acromion-to-greater tuberosity distance was measured in abduction and neutral shoulder position. The ratios of the distances in the abduction and neutral position were calculated to grade the severity of shoulder impingement syndrome. Results: A total of 78 shoulders were included in this study. A strong agreement was found for the grading of shoulder impingement severity grading between the two independent observers with Kappa value of 0.94. And correlation between the results of the two observers for the severity grading of shoulder impingement syndrome was significant at 0.01 level. Conclusion: Severity grading of the shoulder impingement syndrome was performed based on the ratio of acromion-to-greater tuberosity distance in abduction and neutral arm position. Spondylolisthesis refers to the slippage of one vertebral body over the adjacent one. It is a chronic condition that requires early detection to prevent unpleasant surgery. The paper presents an optimized deep learning model for detecting spondylolisthesis in X-ray radiographs. The dataset contains a total of 299 X-ray radiographs from which 156 images are showing the spine with spondylolisthesis and 143 images are of the normal spine. Image augmentation technique is used to increase the data samples. In this study, VGG16 and InceptionV3 models were used for the image classification task. The developed model is optimized by utilizing the TFLite model optimization technique. The experimental result shows that the VGG16 model has achieved a 98% accuracy rate, which is higher than InceptionV3's 96% accuracy rate. The size of the implemented model is reduced up to four times so it can be used on small devices. The compressed VGG16 and InceptionV3 models have achieved 100% and 96% accuracy rate, respectively. Our finding shows that the implemented models were outperformed in the diagnosis of lumbar spondylolisthesis as compared to the model suggested by Varcin et al. (which had a maximum of 93% accuracy rate). Also, the developed quantized model has achieved higher accuracy rate than Zebin and Rezvy's (VGG16 + TFLite) model with 90% accuracy. The paper presents possibilities to determine the factors that influence the availability of hydro aggregates. The case study presented in the paper is focused on the hydro-energy units from the Remeți and Munteni hydroelectric power plants belonging to Bihor Power System. The analyzed performance indicators refer to the specific activities of electricity production, respectively the management and exploitation of hydro aggregates at the maximum allowed capacity, as well as the specific activities of the system service, respectively the coordination of operation to meet the demand of National Power System. This study discusses the eRomLex Project, aiming at elaborating a comparative digital edition of the six bilingual Slavonic--Romanian lexicons from the 17th century, based on Pamvo Berynda's Slavonic--Ruthenian Lexicon from the same age. Following a brief presentation of the general context of these lexicons' appearance and the current research on this topic, we will discuss the project's aims, pointing to the benefits of a comparative digital edition. In the second section, we will comparatively examine a few strategies used by the Romanian compilers to deal with the Slavonic--Ruthenian source, both at the level of lexical inventory and definitions. The analysed examples are currently available on the project's digital platform. Our comparative analysis shows that the information included in the Slavonic-- Romanian lexicons is closed quantitively to the source, except for Lex.Mard. Romanian scribes did not use all the material provided by the source, yet they added new (lists of ) entries. Although most definitions are shorter than the ones in the source, there are many instances where additional information of various types was provided. This study is based on the consideration that the patients with rheumatoid arthritis and ankylosing spondylitis undergoing biological therapy have a higher risk of developing tuberculosis. The QuantiFERON-TB Gold test result was the output of the models and a series of features related to the patients and their treatments were chosen as inputs. A distribution of patients by gender and biological therapy, followed at the time of inclusion in the study, and at the end of the study, is made for both rheumatoid arthritis and ankylosing spondylitis. A series of classification algorithms (random forest, nearest neighbor, k-nearest neighbors, C4.5 decision trees, non-nested generalized exemplars, and support vector machines) and attribute selection algorithms (ReliefF, InfoGain, and correlation-based feature selection) were successfully applied. Brick manufacturing processes are energyconsuming processes which justify the concerns for identifying their level of energy performance (EP) and establishing measures to increase energy efficiency. The level of energy performance stands out through established indicators, mainly, specific energy consumption. This paper represents a synthesis of an energy audit (EA) conducted on "poroton" type brick manufacturing process, from a factory equipped with two tunnel type ovens. In the first part are highlighted energy-related characteristics of the process. Next, is presented analysis method, including the specificities of energy balance (EB) model. A good portion of paper is dedicated to present the results obtained therefore the level of energy performance. Bone erosions are the hallmark of structural damage in rheumatoid arthritis (RA). Among imaging techniques, ultrasonography (US) has emerged as an accurate, reliable, repeatable, low-cost and non-invasive imaging modality to detect erosive changes in RA. However, small interruptions of the cortical bone detectable by last generation US equipment do not necessarily represent bone erosions. According to the available data, in addition to cortical bone interruption itself, only a few morphological US findings have been proposed to define RA bone erosions. However, other additional features may be considered to facilitate the interpretation of US cortical bone interruptions in RA. These could be summarised using the following four domains: size, site, shape and scenery. This hypothesis article provides a critical literature review of US features characteristic of RA bone erosions and pictorial evidence supporting the potential role of a morphological analysis in the US identification of bone erosions in RA patients. Knowing the level of energy efficiency is essential for substantiating investment decisions, operation and maintenance of energy consuming equipment and machines. Urban public transport is an important consumer of energy and, at the same time, an important source of pollution of the urban environment with greenhouse gases and micro particles. The present paper is aimed to identify the level of the energy performance of a public transport car fleet, having buses equipped with Diesel engines. The analysis methodology and the mathematical model used are applicable to other similar structures. The obtained results are valid for the analyzed car fleet, highlighting the values of the energy performance indicators, on the whole, by bus categories and for each bus from the park component. Based on the results obtained within the energy audit, comparisons are made for the level of energy performance with similar consumers, and feasible measures to improve energy efficiency are identified. We report a case of 30-year-old female who presented initially with hair loss, photosensitive malar rash, morning stiffness and synovitis. She was diagnosed with Rhupus syndrome based on clinical and laboratory findings. Few months after starting hydroxychloroquine, esomeprazole and azathioprine, and failing methotrexate (because of erosive pill-induced esophagitis), she presented with generalized maculopapular dusky reddish rash in her body, back and extremities. Her anti-double stranded-DNA, anti-nuclear antibody, anti-Ro/SSA and anti-La/SSB were positive. Anti-cyclic citrullinated peptide antibody was moderately positive. She had low complements: C3 and C4. Herpes simplex IgM and mycoplasma tested negative. Skin biopsy from right arm showed evidence of erythema multiform. She met the criteria for the diagnosis of Rowell syndrome. We managed her with hydroxychloroquine, prednisolone, mycophenolate mofetil and topical agents and discontinued esomeprazole. We also review the management of Rowell syndrome in the literature. Objective: To preliminarily explore the diagnostic potential of ultrasound (US) in detecting calcium pyrophosphate (CPP) crystal deposits at the hip joint in a cohort of patients with CPP deposition disease (CPPD) who were previously evaluated by conventional radiography (CR) and to assess the sensitivity and specificity as well as the agreement between US and CR in the evaluation of hip CPP crystal deposits.Methods: Fifty consecutive patients with definite CPPD and 40 age/sex/body mass index-matched disease control subjects who had undergone hip CR within the previous 6 months were enrolled. Bilateral hip US examination was carried out to assess the presence of CCP crystal deposits at the acetabular labrum fibrocartilage and at the femoral head's hyaline cartilage. Two independent radiologists evaluated the presence of hip CPP crystal deposits on CR in both groups.Results: US findings indicative of CPP crystal deposits were found in at least 1 hip in 45 of 50 patients with CPPD (90.0%) and in 73 of 100 hips (73.0%). CPP crystal deposits were more frequently found at the acetabular labrum fibrocartilage than at the femoral head's hyaline cartilage (72% and 17% of the hips in patients with CPPD, respectively). US and CR sensitivity was 90% and 86%, whereas US and CR specificity was 85% and 90%, respectively. Total agreement between the US and CR findings was 77.8%.Conclusion: Our results provide new evidence supporting US as a first-line, sensitive, safe, and reliable imaging technique in detecting CPP crystal deposits at the hip level. Background: Fibrinogen to albumin ratio (FAR) is a newly investigated indicator for inflammation. The study aimed to explore the potential ability of FAR in assessing the severity of inflammation in spondyloarthritis., Methods: The clinical data of 196 spondyloarthritis (SpA) patients, 66 osteoarthritis (OA) patients, and 81 healthy controls (HC) were collected in this retrospective study. The SpA group included 69 psoriatic arthritis patients, 47 reactive arthritis patients and 80 ankylosing spondylitis patients. Chi-square test and Mann-Whitney U test, Spearman's correlation test, regression analysis, and ROC analyses were used for the analysis of FAR., Results: FAR level in group SpA was higher than in OA or HC. In the SpA group, the reactive arthritis group was characterized by the highest FAR level. After matching the erythrocyte sedimentation rate, a significant difference occurred between groups SpA and OA, but not in SpA subgroups. The FAR level was significantly related to erythrocyte sedimentation rate and C-reactive protein. After regression and receiver operating characteristics analysis, FAR was considered the most potential pointer to evaluate inflammation in SpA with the area under curve of 0.95. The recommended cut-off value of FAR was 9.44 for serious inflammation and 8.34 for mild conditions., Conclusion: FAR is closely related to inflammatory biomarkers and can be a potential indicator in the assessment of inflammation in spondyloarthritis., (© 2022. Objectives We aimed to study the ability of board-certified rheumatologists, blinded to all prior diagnostic test results, to establish the presence/absence of an inflammatory rheumatic disease (IRD) or RA among polyarthralgia or arthritis patients, solely relying on clinical assessment. Methods We performed a prospective, examiner-blinded, cross-sectional study documenting the diagnostic work in four sequential steps (medical history, physical examination, musculoskeletal ultrasonography and laboratory tests) of board-certified rheumatologists in a convenience cohort of 100 patients referred for inpatient diagnostic workup to a tertiary care rheumatology centre. Results The ability to correctly identify patients with or without an IRD (diagnostic accuracy) increased from 27% after the clinical assessment to 53% after the ultrasonography and to 70% after taking laboratory test results into account. The corresponding values for correctly identifying patients with or without RA were 19, 42 and 60%, respectively. Therefore the diagnostic accuracy of solely clinical assessment for determining the diagnosis of IRD or RA compared with the diagnosis established by a consecutive thorough in-patient workup was only 27 and 19% in our cohort, respectively. Pretreatment with corticosteroids (in the prior 7 days) vs none did not alter these results substantially (20 vs 29% for IRD, 15% vs 20% for RA). Conclusion Experienced rheumatologists, if deprived of information on prior external imaging and laboratory workup by blinding, were not able to correctly classify the majority of patients presenting with polyarthralgia or arthritis symptoms for inpatient workup, relying only on a brief symptom-focused medical history and physical examination. Among the basic problems that are involved in the decision to implement an energy project with Renewable Energy Sources(RES) are the identification of resources and the assessment of their regional potential, the possibilities of connecting to the existing electricity network in the area, the efficiency of conversion of natural RES in electricity, sources of financing and the possibilities of expanding the regional energy system based on RES. Then there are problems of endowing with modern RES-based power plants, the quality of the electricity supplied in the grid, the reliability of the connection schemes and the continuity of the power supply while reducing the energy losses on the grids. All these problems can pose risks to prevent the construction of a RES power plant or the good functioning of RES connected to the regional power system. This paper is dedicated to the analysis of the possibilities of assessing the level of electromagnetic pollution in the public spaces and the lucrative areas on the application of some statistical and probabilistic methods and by fuzzy modeling. The proposed models are applied to several components of the electrical grids located in Oradea or Bihor County. The first part of the paper ishows the statistic-probabilistic methods. There are presented models for calculating the level of electromagnetic pollution based on the probability of exceeding the dangerous threshold values of the characteristic magnitudes of the electromagnetic field. The second part is allocated to assessing the level of electromagnetic pollution by using fuzzy logic. There are elaborated models for fitting the values of electromagnetic field values into the risk levels given by the exposure of the organism in the electromagnetic field. By using the fuzzy toolbox from the Matlab software, the rules of inference for the specific conditions of electromagnetic field exposure and exposure, applied in power plants in Bihor County, are applied. Functional failures and structural deterioration defects are among the leading causes of growing failure probabilities of the road systems and networks. Thus, asset maintenance intervention is an essential task for the unified management of road assets and systems. The objective of this research is to consider reliability based probabilistic approach established on functional failure mode, effects and criticality analysis parametric reliability analysis, risk-based assessment and strategic asset deterioration decisions. The combined progressive assessment frameworks and algorithms based procedures utilize service inspection, safety inspection and survey inputs. The illustrated reliability maintenance based methodologies offer sustainable asset management for highway transport infrastructure and systems with emphasis on resolutions to their functional failures, defect related risk and appropriate deterioration treatment. Transformers are sophisticated equipment in the power system compared to other elements, and as such, they require a continuous monitoring and diagnosis for safe and reliable operation. Their importance in the functionality of the power system is multiple. Monitoring techniques are used to follow their operational work and consistency. There should be listed monitoring and testing methods, collection data, parameters, failures and outage statistics, electrical and thermal parameter status, and diagnosis for determining their operating performance. Based on case analysis according to loads of the power system, a load of transformers and network topology are discussed regarding the results. In this case, are taken into account the network topology of the Kosovo Transmission System, failure statistics, failure effects and reliability indices. The model and simulation cases are performed with the NEPLAN software. These models also illustrate the cost of a failure modes in the power transformers in the different scenarios. The effects of the omnipresence of the electromagnetic field are certain and recognized. Assessing as accurately as possible these effects, which characterize random phenomena require the use of statistical-probabilistic calculation. This paper aims at assessing the probability of exceeding the admissible values of the characteristic sizes of the electromagnetic field - magnetic induction and electric field strength. The first part justifies the need for concern and specifies how to approach it. The mathematical model of approach and treatment is presented in the second part of the paper and the results obtained with reference to 14 power stations are synthesized in the third part. Background: Physical examination tests of the shoulder (PETS) are clinical examination maneuvers designed to aid the assessment of shoulder complaints. Despite more than 180 PETS described in the literature, evidence of their validity and usefulness in diagnosing the shoulder is questioned.Methods: This meta-analysis aims to use diagnostic odds ratio (DOR) to evaluate how much PETS shift overall probability and to rank the test performance of single PETS in order to aid the clinician's choice of which tests to use. This study adheres to the principles outlined in the Cochrane guidelines and the PRISMA statement. A fixed effect model was used to assess the overall diagnostic validity of PETS by pooling DOR for different PETS with similar biomechanical rationale when possible. Single PETS were assessed and ranked by DOR. Clinical performance was assessed by sensitivity, specificity, accuracy and likelihood ratio.Results: Six thousand nine-hundred abstracts and 202 full-text articles were assessed for eligibility; 20 articles were eligible and data from 11 articles could be included in the meta-analysis. All PETS for SLAP (superior labral anterior posterior) lesions pooled gave a DOR of 1.38 [1.13, 1.69]. The Supraspinatus test for any full thickness rotator cuff tear obtained the highest DOR of 9.24 (sensitivity was 0.74, specificity 0.77). Compression-Rotation test obtained the highest DOR (6.36) among single PETS for SLAP lesions (sensitivity 0.43, specificity 0.89) and Hawkins test obtained the highest DOR (2.86) for impingement syndrome (sensitivity 0.58, specificity 0.67). No single PETS showed superior clinical test performance.Conclusions: The clinical performance of single PETS is limited. However, when the different PETS for SLAP lesions were pooled, we found a statistical significant change in post-test probability indicating an overall statistical validity. We suggest that clinicians choose their PETS among those with the highest pooled DOR and to assess validity to their own specific clinical settings, review the inclusion criteria of the included primary studies. We further propose that future studies on the validity of PETS use randomized research designs rather than the accuracy design relying less on well-established gold standard reference tests and efficient treatment options. Objective. Alkaptonuria (AKU) is a rare autosomal recessive disease resulting from a single enzyme deficiency in tyrosine metabolism. As a result, homogentisic acid cannot be metabolized, causing systemic increases. Over time, homogentisic acid polymerizes and deposits in collagenous tissues, leading to ochronosis. Typically, this occurs in joint cartilages, leading to an early onset, rapidly progressing osteoarthropathy. The aim of this study was to examine tissue turnover in cartilage affected by ochronosis and its role in disease initiation and progression. Methods. With informed patient consent, hip and knee cartilages were obtained at surgery for arthropathy due to AKU (n = 6; 2 knees/4 hips) and OA (n = 12; 5 knees/7 hips); healthy non-arthritic (non-OA n = 6; 1 knee/5 hips) cartilages were obtained as waste from trauma surgery. We measured cartilage concentrations (normalized to dry weight) of racemized aspartate, GAG, COMP and deamidated COMP (D-COMP). Unpaired AKU, OA and non-OA samples were compared by non-parametric Mann-Whitney U test. Results. Despite more extractable total protein being obtained from AKU cartilage than from OA or non-OA cartilage, there was significantly less extractable GAG, COMP and D-COMP in AKU samples compared with OA and non-OA comparators. Racemized Asx (aspartate and asparagine) was significantly enriched in AKU cartilage compared with in OA cartilage. Conclusions. These novel data represent the first examination of cartilage matrix components in a sample of patients with AKU, representing almost 10% of the known UK alkaptonuric population. Background: Rheumatoid Arthritis (RA) is a chronic inflammatory disease presenting with inflammation, tenderness and destruction of the synovial joints, resulting in severe disability and early death due to complication of disease. Previous diagnostic criteria are not useful for identifying patients who need early treatment. Thus, new diagnostic criteria for faster diagnosis of disease are introduced in 2010. The aim of this study was to compared 1987 ACR (American College of Rheumatology) criteria and 2010 ACR/EULAR (European League Against Rheumatism) classification criteria for diagnosis of rheumatoid arthritis. Methods: In this Cohort prospective study, patients with early arthritis were evaluated according to the old and new diagnostic criteria and followed-up every two monthly for one year (2012-2013) in Hazrat-e Rasool University Hospital, Tehran. Inclusion criteria of this study were age more than 18 year and indefinite diagnosis of arthritis. For all of patients physical examination by expert rheumatologist was done and lab data include erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), anti-cyclic citrullinated peptide (Anti-CCP) and rheumatoid factor was requested. The sensitivity, specificity, positive and negative predictive values were then determined for each diagnostic criteria. Results: In this study 104 patients including 28 males (26.9%) and 76 females (73.1%) with the mean age of 44.2±13.7 years were included. At the end of one year follow-up, 82 were diagnosed to have RA while other 22 patients were not categorized as RA. Sensitivity for ESR, CRP, Anti-CCP and rheumatoid factor in 2010 ACR/EULAR criteria was 52%, 19%, 48%, 28% and specificity for them was 45%, 71%, 27%, 79% respectively. Number of small and large joint arthritis were more in patients with Rheumatoid Arthritis (RA) rather than other arthritis (P=0.0001). Sensitivity and specificity for small joints involvement was 87% and 54% and for large joints involvement was 81% and 59%. The sensitivity, specificity, positive and negative predictive values for 2010 ACR/EULAR criteria were 65%, 40%, 81%, and 23%, respectively. The sensitivity, specificity, positive and negative predictive values for 1987 ACR criteria were 51%, 62%, 83%, and 25% respectively. This paper addresses an emergency scheduling for manufacturing resources (ESMR) problem and a method based on event-driven rolling optimization according to the production task priority is developed for solving it. A mathematical model aiming to maximum resource scheduling satisfaction, minimize tardiness penalties and minimize crash cost is proposed firstly. Then, the priority of the tasks in rolling window is analysed based on production system vulnerability. An adaptive multi-objective dynamic resource scheduling algorithm is proposed as a solution of the model above. Analytic results show that it is not only able to effectively reduce the complexity of ESMR, but also to reveal influence of task importance on ESMR. With the analysis, it can provide way of measuring the overall emergency impacts on the systems, and allow the decision-makers to respond to unexpected events that what the best way of resources scheduling is.
