Compact and low-cost SQUID electronics to operate double relaxation oscillation SQUIDs (DROSs) has been constructed for detecting magnetocardiogram (MCG) fields. SQUID electronics consists of low-noise preamplifier with a white noise of 0.6 nV/√Hz, flux-locked loop (FLL) circuit with auto-reset, and interface circuit to control FLL using 3 digital lines from a computer. The automatic control software adjusts SQUID parameters to the optimum operating condition within 15 seconds for 62 channels. Outputs of FLL circuits are passed through the hardware filter box to the 64-channel data acquisition board. Each SQUID electronics for FLL and digital control is built together on a printed circuit board of 45 mm × 95 mm. When SQUID electronics is connected to DROS gradiometer with typical flux-to-voltage transfers of 1 mV / Φ0, the noise contribution of FLL circuit is about 0.6 μΦ0/√Hz or 0.6 fT/√Hz at 100 Hz, low enough to measure MCG fields. Abstract Background Primary Sjögren’s Syndrome (pSS) is a rare autoimmune disease that is difficult to diagnose due to a variety of clinical presentations, resulting in misdiagnosis and late referral to specialists. To improve early-stage disease recognition, this study aimed to develop an algorithm to identify possible pSS patients in primary care. We built a machine learning algorithm which was based on combined healthcare data as a first step towards a clinical decision support system. Method Routine healthcare data, consisting of primary care electronic health records (EHRs) data and hospital claims data (HCD), were linked on patient level and consisted of 1411 pSS and 929,179 non-pSS patients. Logistic regression (LR) and random forest (RF) models were used to classify patients using age, gender, diseases and symptoms, prescriptions and GP visits. Results The LR and RF models had an AUC of 0.82 and 0.84, respectively. Many actual pSS patients were found (sensitivity LR = 72.3%, RF = 70.1%), specificity was 74.0% (LR) and 77.9% (RF) and the negative predictive value was 99.9% for both models. However, most patients classified as pSS patients did not have a diagnosis of pSS in secondary care (positive predictive value LR = 0.4%, RF = 0.5%). Conclusion This is the first study to use machine learning to classify patients with pSS in primary care using GP EHR data. Our algorithm has the potential to support the early recognition of pSS in primary care and should be validated and optimized in clinical practice. To further enhance the algorithm in detecting pSS in primary care, we suggest it is improved by working with experienced clinicians.

The inefficiency of cyanide/HCN (CN) binding with heme proteins (under physiological regimes) is demonstrated with an assessment of thermodynamics, kinetics, and inhibition constants. The acute onset of toxicity and CN’s mg/Kg LD50 (μM lethal concentration) suggests that the classical hemeFe binding-based inhibition rationale is untenable to account for the toxicity of CN. In vitro mechanistic probing of CN-mediated inhibition of hemeFe reductionist systems was explored as a murburn model for mitochondrial oxidative phosphorylation (mOxPhos). The effect of CN in haloperoxidase catalyzed chlorine moiety transfer to small organics was considered as an analogous probe for phosphate group transfer in mOxPhos. Similarly, inclusion of CN in peroxidase-catalase mediated one-electron oxidation of small organics was used to explore electron transfer outcomes in mOxPhos, leading to water formation. The free energy correlations from a Hammett study and IC50/Hill slopes analyses and comparison with ligands (CO/H2S/N3-)$\left( {\text{CO}}/{{{{\text{H}}_{2}}\text{S}}/{\text{N}_{3}^{\text{-}}}\;}\; \right)$ provide insights into the involvement of diffusible radicals and proton-equilibriums, explaining analogous outcomes in mOxPhos chemistry. Further, we demonstrate that superoxide (diffusible reactive oxygen species, DROS) enables in vitro ATP synthesis from ADP+phosphate, and show that this reaction is inhibited by CN. Therefore, practically instantaneous CN ion-radical interactions with DROS in matrix catalytically disrupt mOxPhos, explaining the acute lethal effect of CN.

Background: Mismanagement in disaster relief operations (DROs) has created a requirement for fast, fair, and safe humanitarian logistics (HL). The deployment of digital solutions in DROs is supported by humanitarian organizations (HOs) as well as recent research; therefore, the key purpose of this article is to investigate the impact of digitalization for fast, fair, and safe HL in DROs. Methods: Primary empirical data were acquired from 449 disaster relief workers (DRWs) via questionnaires and by employing the snowball sampling methodology and partial least squares structural equation modeling (PLS-SEM) in SmartPLS 3. Results: The result of this study indicates that fast, fair, and safe HL is crucial because it can save people’s lives and reduce suffering, and it achieves long-term sustainable development. The findings of the study could be beneficial to all catastrophe risk-management stakeholders as they look for ways to help victims. In addition, the ultimate beneficiaries of digitalized and effective HL will be all of society, notably disaster victims. Conclusions: By implementing proper technologies in DROs, the afflicted may receive needed resources on time, which can save many lives. The utility of the proposed variables is promoting awareness about HL in a fast, fair, and safe manner, and we discuss implications, limitations, and future research. The research adds to the literature by presenting the first quantitative evidence assessing the independent role of digitalization in generating fast, fair, and safe HL from one of the world’s most disaster-prone countries.

INTRODUCTION Dizziness is a common problem among older patients in primary care. (1) The annual prevalence of dizziness in adults ranges from 20% to 30% in population-based questionnaire studies. (2) [...], PURPOSE The prognosis of older patients with dizziness in primary care is unknown. Our objective was to determine the prognosis and survival of patients with different subtypes and causes of dizziness. METHODS In a primary care prospective cohort study, 417 older adults with dizziness (mean age 79 years) received a full diagnostic workup in 2006-2008. A panel of physicians classified the subtype and primary cause of dizziness. Main outcome measures were mortality and dizziness-related impairment assessed at 10-year follow-up. RESULTS At 10-year follow-up 169 patients (40.5%) had died. Presyncope was the most common dizziness subtype (69.1%), followed by vertigo (41.0%), disequilibrium (39.8%), and other dizziness (1.7%). The most common primary causes of dizziness were cardiovascular disease (56.8%) and peripheral vestibular disease (14.4%). Multivariable adjusted Cox models showed a lower mortality rate for patients with the subtype vertigo compared with other subtypes (hazard ratio [HR] = 0.62; 95% CI, 0.40-0.96), and for peripheral vestibular disease vs cardiovascular disease as primary cause of dizziness (HR = 0.46; 95% CI, 0.25-0.84). After 10 years, 47.7% of patients who filled out the follow-up measurement experienced substantial dizziness-related impairment. No significant difference in substantial impairment was seen between different subtypes and primary causes of dizziness. CONCLUSIONS The 10-year mortality rate was lower for the dizziness subtype vertigo compared with other subtypes. Patients with dizziness primarily caused by peripheral vestibular disease had a lower mortality rate than patients with cardiovascular disease. Substantial dizziness-related impairment in older patients with dizziness 10 years later is high, and indicates that current treatment strategies by family physicians may be suboptimal.
