Dural arteriovenous fistulas (DAVFs) are rare but significant intracranial vascular malformations that are usually idiopathic and can lead to severe complications like venous hypertension and intracranial hemorrhage. We present 2 cases of DAVF occurring after venous sinus stenting (VSS) for presumed idiopathic intracranial hypertension (IIH) in a 51-year-old and a 52-year-old female. In both patients, Cognard type 1 DAVF was detected by Catheter angiography and successfully obliterated with Onyx™ embolization. These cases suggest that VSS may either unmask pre-existing DAVFs by altering intracranial pressure dynamics or induce new fistula formation by angiogenic factors or stent-induced hemodynamic changes. Clinicians should maintain an index of suspicion for DAVF in patients with persistent symptoms post-VSS and catheter angiography might be needed to detect the DAVFs.

Background: Intracranial dural arteriovenous fistula (dAVF) is a rare arteriovenous malformation with potentially severe complications. This study investigates the efficacy and safety of transarterial embolization (TAE) in treating petrous dAVFs through a retrospective analysis and literature review., Case Description: A retrospective analysis of six patients with petrous dAVFs treated with TAE was conducted, accompanied by a systematic literature review to evaluate treatment outcomes. Data collection included patient characteristics, clinical presentation, Borden-Shucart and Cognard classifications, treatment specifics, and overall outcomes. TAE, particularly utilizing Onyx, demonstrated favorable outcomes in our six patients. Regarding literature review results, 102 articles were identified through PubMed Mesh tool search, but only five were included after careful evaluation. In addition, one article was manually added after searching for the remaining articles. Combining our six cases with literature findings, 79.8% ( n = 75) of patients undergoing TAE achieved a cure with the technique, though Onyx was reported in only 13.9% ( n = 13) of TAE cases. Complications were observed in 11.7% ( n = 11) of TAE cases., Conclusion: Our presented cases and literature review suggest that the TAE of dAVFs is feasible and curative for selected cases of petrous dAVFs. Dural arteriovenous fistulas (dAVFs) can occur as complications after surgical procedures, especially following the resection of meningiomas near the dural sinus. This case report presents a 74-year-old male who developed a recurrent sigmoid dAVF following meningioma resection. Initially treated with transvenous embolization and middle meningeal artery embolization, the dAVF recurred with worsening clinical symptoms. Conventional treatment options, including sinus sacrifice and transarterial embolization, were unsuitable due to the critical role of the patient's dominant right sigmoid sinus in cerebral venous drainage. Consequently, a reconstructive approach was employed using a pipeline embolization device (PED) construct. The PED successfully occluded the dAVF while preserving the function of the sigmoid sinus. A follow-up angiogram confirmed stable occlusion and normalization of intracranial venous drainage. In recent years, with the current access in techniques, studies have significantly advanced the knowledge on meningeal immunity, revealing that the central nervous system (CNS) border acts as an immune landscape. The latest concept of meningeal immune system is a tertiary structure, which is a comprehensive overview of the meningeal immune system from macro to micro. We comprehensively reviewed recent advances in meningeal immunity, particularly the new understanding of the dural sinus and meningeal lymphatics. Moreover, based on the clues from the meningeal immunity, new insights were proposed into the dural arteriovenous fistula (DAVF) pathology, aiming to provide novel ideas for DAVF understanding.

Among these, intracranial hemorrhage is a severe complication. A 72-year-old male presented with headache and pulsatile tinnitus. Cerebral angiography revealed a Borden II/Cognard IIa+b DAVF. He underwent stage 1 transarterial embolization of the occipital artery which reduced shunting by 30%. Several attempts were made to access the fistula during stage 2 transvenous embolization, but it was not possible to access the left transverse sinus fistula site since there was no communication across the torcula from the right transverse sinus and the left inferior sigmoid–jugular bulb was occluded. Therefore, a single burr hole was drilled and direct access to the DAVF was achieved with a micropuncture needle under neuronavigational guidance. The left transverse–sigmoid sinus junction was then embolized with coils. After the procedure, angiography revealed that the DAVF was cured with no residual shunting. This case demonstrates how minimally invasive surgery provides an alternative method to access a DVAF when conventional transarterial and/or transvenous embolization treatment options are not possible. Each DAVF case has unique anatomy and physiology, and creative multi-disciplinary strategies can often yield the best results.

Abstract Background Pipeline embolization device (PED) has proved its safety and efficacy in the treatment of intracranial large and giant side-wall aneurysms. With the accumulation of treatment experience, it is an inevitable trend to expand its off-label use on aneurysms. Whether flow diversion is safe and efficient in cases with special angioarchitecture has rarely been explored. Methods We performed a retrospective analysis of 210 consecutive patients treated by PED for intracerebral aneurysms in our center. Except for aneurysm, those patients also presented with special angioarchitecture: Fetal PCA, AVM, V-B junction and DAVF. Results Nine patients were qualified for the study. 1 was with fetal PCA, the aneurysm remained patent on 4-month follow-up. 2 with ipsilateral AVMs, one patient died due to brain hemorrhage 20 days after the operation, the other one was only partially embolised on 6 month follow up. 3 aneurysms located at V-B junction, angiographic follow up on 3 months demonstrated no complete occlusion of both the aneurysms, the other patients were still on follow up. All of the 3 cases with concomitant DAVF are completely occluded during short to midterm follow up. Conclusions PED for aneurysms incorporated the fetal PCA and V-B junction might meet a high propensity for incomplete occlusion during short term follow up. Aneurysm with ipsilateral AVM is not suitable for PED treatment due to the risk of hemorrhage and incomplete occlusion during midterm follow up. For aneurysm with concurrent DAVF, PED treatment is safe and efficient relatively in one session or by staged operation.

Introduction: We detail the management of a woman with a posterior fossa dAVF (dural arteriovenous fistula) that was unable to be treated by standard transarterial or transvenous embolization or microsurgical ligation. She underwent craniotomy for surgical exposure and direct access of her left middle meningeal artery followed by microcatheter embolization with favorable results. Methods: This is a case report, which describes a case of a difficult to access dAVF. Results: A 72 year‐old woman presented with vertigo, nausea, and vomiting one week following a fall. CT head disclosed cerebellar vermis intraparenchymal hematoma and CT head angiography was suspicious for underlying vascular malformation. Diagnostic cerebral angiogram demonstrated extensive tentorial and suboccipital dural arteriovenous fistula (dAVF) fed by branches of both middle meningeal and occipital arteries with direct cortical venous drainage and venous aneurysmal ectasia directly adjacent to the vermian hemorrhage (ruptured Cognard grade 4). Left vertebral artery angiogram demonstrated excessive tortuosity of vertebral enlarged posterior meningeal artery, which was unable to be catheterized sufficiently beyond its origin despite different microwires and microcatheters due to tortuosity. Transfemoral venous approach was also attempted, however, this was also unsuccessful and decision was made to proceed with microsurgical treatment. The following day a suboccipital craniotomy was performed, but was ultimately aborted due to nearly uncontrollable bleeding from bony exposure and dural access secondary to severe venous hypertension. The next day percutaneous endovascular treatment was attempted a second time. A small right middle meningeal artery (MMA) contribution to the fistula was embolized with liquid embolic but, again because of excessive tortuosity and insufficient microcatheter access, right MMA occlusion occurred without embolic agent reaching the fistula. Similar access related difficulties due to tortuosity were encountered in accessing the left middle meningeal and occipital arteries contributing to the fistula. Repeat transvenous access was also attempted from the occipital and right transverse sinuses, but microcatheter access to the fistula was unable to be established beyond the venous outflow from the aneurysm, and, given the risks of hemorrhage related to embolization of the venous outflow without occluding arterial inflow into the ruptured aneurysm, transvenous embolization was not performed. A few days later, after the patient was given time to recover from the prior procedures, the patient underwent left temporal craniotomy in a hybrid operating room/interventional radiology suite for direct cannulation of the left MMA. Localization of the craniotomy site over the left MMA access point was planned by transfemoral cerebral angiogram and a transcarotid/peripheral access kit was used to catheterize the left MMA directly following surgical exposure. An .017 microcatheter was advanced close to the fistula point using standard biplanar roadmap fluoroscopy, and Onyx embolization of the fistula was performed to complete occlusion, without complication. Conclusions: For cerebrovascular disorders that are inaccessible by traditional endovascular and surgical means, a hybrid approach should be considered.

Abstract In the past, the cavernous dural arteriovenous fistula was categorized as spontaneous cavernous carotid fistula [1] due to the lack of knowledge and limitation of imaging equipment. In the recent time, with the accumulation of knowledge of DAVF’s etiology, mechanism, physiology, clinical symptoms and imaging data, the diagnostic methods and treatment have achieved novel understandings and progresses. In fact, it’s a specific type of dural arteriovenous fistula---- cavernous dural arteriovenous fistula. The purpose of this paper is to tell the difference between cavernous dural arteriovenous fistula and traumatic carotid cavernous fistula, and to redefine cavernous dural arteriovenous fistula from the aspects of etiology, mechanism, pathology, clinical symptoms, DSA characteristic and therapy. DAVF is an independent disease. The Cavernous dural arteriovenous fistula can not be classified as spontaneous CCF any longer, but a specific type of DAVF--- cavernous DAVF.

There are multiple treatment alternatives for cavernous dAVFs, with transvenous routes being most common. Among these routes, occluded inferior petrosal sinus is well-described, and, apart from being imaginative and elegant, it is also safe and effective. Herein we describe the application of this method to reach the fistulous pouch of a cavernous dAVF via an occluded superior petrosal sinus.

Abstract Background The Onyx™ Liquid Embolic System is a non-adhesive liquid embolic agent, which has been proved by the US FDA for embolization of lesions in the peripheral and neurovasculature since 2005. We reported a case of ischemic optic neuropathy after using Onyx-18 to embolize the anterior ethmoid arteries that feeding dural arteriovenous fistulas (DAVF). Case presentation A 57-year-old Asian male presented with anterior cranial fossa DAVF underwent embolotherapy by delivering Onyx-18 through a microcatheter into the anterior ethmoid arteries under angiography guidance. The interventional procedure was successful and no clear evidence was found pointing to untargeted occlusive embolus. But after the surgery the patient experienced delayed painless vision loss in the right eye (RE). The fundoscopy showed unilateral papilledema with pale optic disc in RE, accompanied by significant edema and thickening in the retinal nerve fiber layer (RNFL) of macula. The fundus fluorescence angiography showed that most of the optic disc in RE had postponed or absent fluorescence filling. Visual evoked potential (VEP) confirmed that the amplitude of the P100 component was decreased in RE without significant prolongation of the latency. The patient was diagnosed with anterior ischemic optic neuropathy, but immediate pulse steroid therapy failed to rescue his vision. Conclusion Preoperative evaluation of the patient’s hemodynamic status and fundus examination are essential for assessing the risk of ischemic ocular complications, and the non-adhesive liquid embolic agent Onyx-18 should be used cautiously during endovascular embolization of intracranial artery.

A dural arteriovenous fistula (DAVF) is a dural-based shunt between meningeal arteries and meningeal veins, sinuses and/or cortical veins; they have been classified and named according to the location and the flow pattern. Petrous apex DAVFs are located where the petrosal vein penetrates the dura mater into the superior petrosal sinus; there are only few cases reported in the literature, they can show an aggressive behaviour (subarachnoid haemorrhage, severe brainstem oedema) with a high mortality rate. The described case is, to the best of our knowledge, the first case of a DAVF presenting with symptoms mimicking idiopathic normal pressure hydrocephalus. After worsening of gait impairment, memory loss and urinary incontinence an urgent CT of the brain showed hydrocephalus and a hyperdense mass in the pineal region mimicking a pineal tumour; an emergent digital subtraction angiogram showed a left petrous apex Borden type III DAVF. Dural arteriovenous fistula (DAVF) is a rare condition affecting approximately 1.5% of 1,000,000 individuals annually. It frequently occurs in the transsigmoid and cavernous sinuses. An isolated sigmoid sinus is extremely rare and is treated by performing transfemoral transvenous embolization along the opposite transverse sinus. A 69-year-old woman presented with asymptomatic Borden type III/Cognard type III DAVF involving an isolated sigmoid sinus. She underwent a staged operation in which a navigation system was used to expose the sigmoid sinus in the operating room before transferring the patient to the angio suite for transvenous embolization. Various modalities have been used to treat DAVF, including surgical disconnection, transarterial embolization, transvenous embolization, and stereotactic radiosurgery. However, treating DAVF cases where the affected sinus is isolated can be challenging because an easily accessible surgical route may not be available. A woman in her mid-50s who had undergone a subtotal resection of a peritorcular meningioma 3 years earlier presented with symptoms suggestive of increased intracranial pressure. A delayed diagnosis of a torcular dural arteriovenous fistula (dAVF) diagnosis was made on MRI. Digital subtraction angiography confirmed a torcular dAVF (Borden type II). Staged 3 rounds of transarterial (TA) embolisation were performed using n-butyl cyanoacrylate, ethylene vinyl alcohol copolymer and coils of the complex torcular dAVF, which significantly reduced the flow and corresponded with improved symptoms of pulsatile tinnitus, vision and headache. We reviewed previously reported cases of dAVFs that developed after craniotomy for meningioma resection. Despite its rarity, a torcular dAVF can develop after craniotomy and progress from low to high grade due to its location. Diagnosis requires a thorough understanding of subtle imaging findings, which may be present in follow-up studies obtained for tumour surveillance. [ABSTRACT FROM AUTHOR]

Background: Intracranial dural arteriovenous fistulas (DAVFs) have been documented to occasionally spontaneously regress. However, the mechanism responsible for this occurrence remains speculative. Methods: We present a case of a Borden II - Cognard IIa+b DAVF involving the superior sagittal sinus (SSS) with bilateral external carotid artery supply that regressed spontaneously. A systematic literature review was conducted to explore the current theories explaining the spontaneous regression of DAVFs according to Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. Results: A total of 26 studies and 54 cases were included in our results. Of the included cases, 57.14% of cases were Borden I, 16.33% were Borden II, and 26.53% were Borden III. Ruptured status or intracranial hemorrhage was documented in 24.1% of all cases, the majority of which (69.2%) were in cases with aggressive lesions (Borden II or greater). The most commonly involved location was the transverse sinus (38.89% of cases, n = 21), and the SSS was only involved in 12.96% of all cases. 50% of included cases proposed a mechanism responsible for spontaneous regression. The most frequently proposed mechanisms were thrombosis of the involved sinus/chronic inflammatory changes or direct endothelial injury, endoluminal stasis, and thrombogenic effects of contrast medium during angiography. We present the case of a 54-year-old woman with an aggressive ruptured DAVF that likely developed following a pediatric traumatic brain injury that was left untreated before she presented to our institution after significant delay. Her DAVF regressed on repeat angiography before neurovascular intervention without a clear identifying mechanism as proposed by the current literature. Conclusion: Our results suggest that spontaneous regression is not necessarily associated with lower risk DAVFs. The present case offers a unique long-term insight into the natural history of an aggressive ruptured DAVF of the SSS that regressed without intervention. Background: Typical treatment of dural arteriovenous fistula (DAVF) emphasizes clinical procedures such as surgery, radiotherapy, and endovascular treatment. However, external factors of adverse psychological emotions in patients with DAVF before treatment have not yet been evaluated in a multicenter study. Objectives: This study aimed to fill the research gap regarding the effects of factors by evaluating the anxiety and depression status among patients with DAVF before and after endovascular treatment and identify factors influencing the development of anxiety and depression before their endovascular treatment. Methods: This retrospective study included 168 DAVF patients treated at two independent neurosurgical medical centers from December 2012 to December 2022. The Hamilton Anxiety Scale (HAMA) and Hamilton Depression Scale (HAMD) were assessed before and after endovascular treatment. Demographic and clinical data were also collected. Statistical tests were performed to calculate the proportion of DAVF patients with anxiety and depression. A multivariate analysis was conducted to identify independent risk factors for anxiety and depression in patients with DAVF before endovascular treatment. Results: A significant decrease in the percentage of patients with anxiety and depression was observed in patients after endovascular treatment compared with before treatment. Patients with DAVF who were at a higher risk for anxiety and depression were females had multiple visits, comorbid conditions, and a lower economic status and were presented with symptoms. Conclusion: After endovascular treatment, the anxiety or depression symptoms of DAVF patients significantly improved. Our findings provide additional evidence to support the role of intravascular treatment in improved DAVF patients' psychological outcomes. [ABSTRACT FROM AUTHOR]

Introduction: Dural arteriovenous fistula (DAVF) is an uncommon malformation involving an abnormal connection between dural arteries, or the pachymeningeal branches of cerebral arteries, and dural veins. Its exact pathogenesis remains elusive. Known potential triggers for DAVF include cerebral venous sinus thrombosis (CVST), trauma, ear infections, and cranial surgeries. Due to its rarity and diverse clinical presentations, diagnosing DAVF can be a challenge. Case description: We present a case of DAVF associated with CVST, manifesting as rapidly advancing parkinsonism accompanied by dementia over a month. Brain magnetic resonance imaging (MRI) revealed bilateral symmetric T2 hyperintensities in the basal ganglia and brain stem. Cerebral angiography further confirmed a fistula between the torcular herophili and the transversesigmoid sinuses. Despite strong recommendations for transvenous embolization of the fistula, the patient declined the procedure. The anticoagulant therapy and symptomatic treatments administered did not yield any improvement in the patient's condition. Additionally, we reviewed 27 DAVF-derived parkinsonism and dementia cases. Conclusion: DAVF must be considered in the differential diagnosis of cases of rapidly progressive parkinsonism with concurrent dementia. Intracranial dural arteriovenous fistula (DAVF) is a relatively complex intracranial condition, and its clinical presentation and treatment strategies often vary significantly due to various factors. Although the cure rate of intracranial DAVF is currently high, there is still a lack of understanding of its etiology and pathogenesis. There is ongoing controversy regarding the treatment strategies for DAVF associated with the pial arteries, and there is a lack of understanding of its pathogenesis. The author conducted a brief literature review on DAVF with pial arterial supply and presented some treatment experiences from their own medical center. Background: The formation of a dural arteriovenous fistula (DAVF) at the craniocervical junction is rare. Such a fistula can be fed by the branches of the vertebral and external carotid arteries. The lateral spinal artery can branch from the vertebral artery. The feeders of a DAVF at the craniocervical junction are often diagnosed on preoperative angiography, and there is little reported evidence on the intraoperative diagnosis of the lateral spinal artery. Case description: An 84-year-old man presented with motor weakness and sensory disturbance of the lower extremities. Edematous changes in the medulla oblongata and cervical spinal cord were observed on magnetic resonance imaging. Cerebral angiography revealed a DAVF fed by a branch of the vertebral artery, with a shunting point located in the dura of the right condyle; the main drain was the anterior spinal vein. The DAVF drain was surgically obliterated to prevent hemorrhagic events and improve neurological symptoms. Intraoperatively, an artery branching from the feeder of the DAVF was identified and preserved. The patient had a good postoperative course, and the neurological symptoms were ameliorated. Follow-up cerebral angiography revealed proximal branching of the lateral spinal artery from the feeding artery of the DAVF. Conclusion: A lateral spinal artery was identified intraoperatively while a DAVF at the craniocervical junction was obliterated. This suggests that preoperative imaging should be carefully reviewed, and endovascular procedures should consider such possibilities to avoid adverse ischemic outcomes. [ABSTRACT FROM AUTHOR]

Objective: For curative Onyx embolization of dural arteriovenous fistulas (dAVF) with multiple feeders, it is essential to select the optimal target artery as well as to control the blood flow at the fistula point. We report a case of tentorial dAVF (TdAVF) treated by Onyx embolization under flow control using balloon catheters., Case Presentation: A 66-year-old male was admitted to our hospital for treatment of TdAVF detected incidentally by MRI, which revealed a dilated and tortuous vein around the cerebellum. Cerebral angiography demonstrated a TdAVF, fed mainly by bilateral middle meningeal arteries (MMA) and bilateral occipital arteries (OA), with the fistula point at the torcular and venous drainage to the two superior vermian veins (SVVs). Onyx 18 was injected from the low-flow feeder of the MMA under flow control by occluding the high-flow feeder of the OA using balloon catheters, obliterating the arteriovenous shunt., Conclusion: In treatment of TdAVF involving low- and high-flow feeders, Onyx embolization via the low-flow feeder with temporary balloon occlusion of other high-flow feeders is a useful method. Background: Dural arteriovenous fistulas (DAVF) represent almost 10–15% of intracranial malformations that cause intracranial hemorrhage and focal neurological deficits. Seldom tentorial DAVF cases present with ocular manifestations initially, which occur frequently in carotid–cavernous fistula (CCF) and cavernous sinus DAVF (CS DAVF). Case presentation: We report an unusual falcotentorial DAVF case draining via the superior and inferior ophthalmic veins that caused left-side increased intraocular pressure. The patient's chief complaint was swelling on the left side, pain and conjunctival congestion. He received endovascular embolization via a transarterial approach, and postoperative angiography demonstrated that the falcotentorial DAVF was occluded completely. Conclusion: Except for CCF and CS DAVF, some specific subtypes of DAVF should be considered if the patient initially presents with ocular symptoms. Differential diagnosis and definitive treatment are mandatory to avoid a delayed diagnosis and irreversible symptoms. [ABSTRACT FROM AUTHOR]
