Abstract

BACKGROUND: Bioprosthetic valve fracture (BVF) can be used to improve transcatheter heart valve (THV) haemodynamics following a valve-in-valve (ViV) intervention. However, whether BVF should be performed before or after THV deployment and the implications on durability are unknown. Aims: We sought to assess the impact of BVF timing on long-term THV durability. METHODS: The impact of BVF timing was assessed using small ACURATE neo (ACn) or 23 mm SAPIEN 3 (S3) THV deployed in 21 mm Mitroflow valves compared to no-BVF controls. Valves underwent accelerated wear testing up to 200 million (M) cycles (equivalent to 5 years). At 200M cycles, THV were evaluated by hydrodynamic testing, second-harmonic generation (SHG) microscopy, scanning electron microscopy (SEM) and histology. RESULTS: At 200M cycles, the regurgitant fraction (RF) and effective orifice area (EOA) for the ACn were 8.03±0.30%/1.74±0.01 cm2 (no BVF), 12.48±0.70%/1.97±0.02 cm2 (BVF before ViV) and 9.29±0.38%/2.21±0.0 cm2 (BVF after ViV), respectively. For the S3 these values were 2.63±0.51%/1.26±0.01 cm2, 2.03±0.42%/1.65±0.01 cm2, and 1.62±0.38%/2.22±0.01 cm2, respectively. Further, SHG and SEM revealed a higher degree of superficial leaflet damage when BVF was performed after ViV for the ACn and S3. However, the histological analysis revealed significantly less damage, as determined by matrix density analysis, through the entire leaflet thickness when BVF was performed after ViV with the S3 and a similar but non-significant trend with the ACn. Conclusions: BVF performed after ViV appears to offer superior long-term EOA without increased RF. Ultrastructure leaflet analysis reveals that the timing of BVF can differentially impact leaflets, with more superficial damage but greater preservation of overall leaflet structure when BVF is performed after ViV.

16. Discourse analysis of biblical Hebrew texts : a stylistics based on systemic linguistics for textual analysis

Abstract

The thesis is written against a fairly recent background of dispute and acrimony centring on the analysis and interpretation of secular texts using either the long-established, literary-critical approach, or the newer, stylistics approach based on modern linguistics. Linguistic stylistics seeks to relate the observations of literary criticism rigorously to the linguistic infra-structure. The thesis sets out to test a form of stylistics based on systemic linguistics, a linguistics originating with J.R. Firth and developed especially by M.A.K. Halliday. It is ideally suited to the interpretation of all kinds of texts, because it is based on a semanticized grammar, i.e. it relates grammar to meaning and to social context and use. The thesis explores how the language of a text constructs meaning, and stylistics is offered as a methodology to evaluate the detailed data of linguistic analysis and to articulate the relationship between the given of a text and the intuitions of the reader. It does not eschew polysemy, which is of the very nature of texts, especially literary ones, and a major constituent in the pleasure of reading, but explores the limits set by the language to interpretation. I have applied it to a broad range of excerpts from Biblical Hebrew writings covering narrative, conversation, persuasion and poetry. Stylistics is not meant to replace or subordinate other approaches, but is a preliminary and complementary method, demanding of the interpreter a serious regard for language.

Türkiye'nin başkenti Ankara, ülkenin büyümesine paralel olarak özellikle 1970'li yıllarda hızlı bir şehirleşme sürecinden geçmiştir. Artan göç ve çoğalan konut ihtiyacı kentin kontrol edilemez biçimde büyümesini tetiklerken, birçok konuda da şehrin kendisini geliştirmesini hızlandırmıştır. Bu yıllarda hızla büyümesi ve gelişmesiyle göze çarpan Ankara, İngiliz akademisyen Geoffrey Payne'in 1974-1975 yılları arasında yürüttüğü kapsamlı bir şehircilik araştırmasına konu olmuştur. Bu çalışma 2019 yılında boylamsal bir yaklaşımla yeniden güncellenmeye başlanmıştır. Bu nedenle, Payne'in çalışmasına Çorum'dan Ankara'ya göç edip Dikmen'de mütevazı bir gecekonduda hayatını sürdürürken konu olan Eryılmaz ailesinin üyeleri ile 2019 yılında tekrar buluşma sağlanmış ve görüşme gerçekleştirilmiştir. Bu buluşmada, Eryılmaz ailesinin farklı kuşaklarıyla biraraya gelinmiş ve ailenin Ankara ile bütünleşerek gelişen ve değişen 50 yıllık yaşam hikayesi, geçmişe ışık tutacak şekilde yarı yapılandırılmış sorularla derinlemesine bir görüşme tekniğiyle irdelenmiştir. Böylelikle kentte yaşayanların kent ile etkileşimleri, yaşanan sosyo-ekonomik değişimler, konut kavramındaki gelişmeler ve kentin yaşayanları üzerindeki yansımaları gerçek bir hikaye üzerinden sunulmuştur. 1974’te başlayan ve 2019 yılından itibaren güncellenerek devam eden bu araştırma Prof. Dr. Ruşen Keleş’in desteği ile sürdürülmüştür., Ankara, the capital city of Turkey, went through a rapid urbanization process, especially in the 1970s, in parallel with the growth of the country. While immigration and increasing housing needs triggered the uncontrollable growth of the city, it accelerated its development in many areas. Standing out with its rapid growth and development in these years, Ankara has been the subject of a comprehensive research conducted by Geoffrey Payne between 1974-1976. The current study provides a longitudinal approach to the city’s development and started in 2019 with members of the Eryılmaz family, who moved from Çorum to Ankara and lived in a modest gecekondu in Dikmen and were representative of the many families who were the subject of Payne's work. During a reunion in 2019, the family's story was examined via a semi-structured in-depth interview in a way that sheds light on the past as it applied to numerous households. Thus, the interactions of the people with the city, the socio-economic changes, the developments in housing and its reflections on the inhabitants are presented through a real story. This research, which started in 1974 and has been evolving since 2019 is carried out with the support of Professor Ruşen Keleş.

Abstract

Purpose To estimate the exchange rates of labile 1H in citrate and spermine, metabolites present in prostatic secretions, to predict the size of the citrate and spermine CEST effects in vivo. Methods CEST z‐spectra were acquired at high‐field [11.7 Tesla (T)] from citrate and spermine solutions at physiological pH (6.5) using saturation power 6 μT. CEST was performed at different temperatures to determine exchange regimes (slow, intermediate or fast). For low pH solutions of spermine, exchange rates were estimated from resonance line width, fitting z‐spectra using the Bloch equations incorporating exchange, and using quantifying exchange using saturation time experiments (QUEST). These rates were extrapolated to physiological pH. Results Citrate showed little CEST effect at pH 6.5 and temperature (T) = 310 K (maximum 0.001% mM‐1), indicating fast exchange, whereas spermine showed greater CEST effects (maximum 0.2% mM‐1) indicating intermediate‐to‐fast exchange. Extrapolating data acquired from low pH spermine solutions predicts exchange rates at pH 6.5 and T of 310 K of at least 2 × 104s‐1. Conclusion Citrate and spermine show minimal CEST effects at 11.7T even using high saturation power. Abstract

Measurement of tissue lactate using 1H MRS is often confounded by overlap with intense lipid signals at 1.3 ppm. Single‐voxel localization using PRESS is also compromised by the large chemical shift displacement between voxels for the 4.1 ppm (–CH) resonance and the 1.3 ppm –CH3 resonance, leading to subvoxels with signals of opposite phase and hence partial signal cancellation. To reduce the chemical shift displacement to negligible proportions, a modified semi‐LASER sequence was written (“FOCI‐LASER”, abbreviated as fLASER) using FOCI pulses to permit high RF bandwidth even with the limited RF amplitude characteristic of clinical MRI scanners. A further modification, MQF‐fLASER, includes a selective multiple‐quantum filter to detect lactate and reject lipid signals. The sequences were implemented on a Philips 3 T Achieva TX system. In a solution of brain metabolites fLASER lactate signals were 2.7 times those of PRESS. MQF‐fLASER lactate was 47% of fLASER (the theoretical maximum is 50%) but still larger than PRESS lactate. In oil, the main 1.3 ppm lipid peak was suppressed to less than 1%. Enhanced suppression was possible using increased gradient durations. The minimum detectable lactate concentration was approximately 0.5 mM. Coherence selection gradients needed to be at the magic angle to avoid large water signals derived from intermolecular multiple‐quantum coherences. In pilot patient measurements, lactate peaks were often observed in brain tumours, but not in cervix tumours; lipids were effectively suppressed. In summary, compared with PRESS, the fLASER sequence yields greatly superior sensitivity for direct detection of lactate (and equivalent sensitivity for other metabolites), while the single‐voxel single‐shot MQF‐fLASER sequence surpasses PRESS for lactate detection while eliminating substantial signals from lipids. This sequence will increase the potential for in vivo lactate measurement as a biomarker in targeted anti‐cancer treatments as well as in measurements of tissue hypoxia. Abstract

Physical diminishes with ageing, but little is known of how the microvascular supply to skeletal muscle fibres is affected. To test the hypothesis that ageing alters blood flow control, we investigated network architecture and vasomotor responses of arterioles in the gluteus maximus muscle of young (2-3 months), adult (12-14 months) and old (18-20 months) C57BL6 male mice (n = 83) (Young, Adult and Old, respectively). Microvascular casts revealed that the total number, length and surface area of arteriolar segments (diameter, 10-50 microm) were not significantly different across age-groups. However, for arterioles with diameter of 30 microm, tortuosity and branch angles increased with age (P0.05). In anaesthetized mice, second-order (2A) distributing arterioles had similar resting (17 +/- 1 microm) and maximal (37 +/- 1 microm) diameters and similar responsiveness to cumulative (10(-10)-10(-4) M) superfusion of acetylcholine or phenylephrine. With superfusate oxygen level raised from 0 to 21%, 2A arteriolar constriction in Young (11 +/- 1 microm) was greater (P0.05) than Adult and Old (5 +/- 1 microm). Observed 1 mm upstream from microiontophoresis of ACh (1 microA, 1 s), conducted vasodilatation was 10 +/- 1 microm in Young, 17 +/- 1 microm in Adult and 6 +/- 1 microm in Old (P0.05). With muscle contractions (2, 4 and 8 Hz; 30 s) arteriolar diameter increased similarly across age-groups (6 +/- 1, 11 +/- 1 and 18 +/- 1 microm, respectively). Muscle mass and active tension were similar across age-groups yet postcontraction vasodilatation recovered more rapidly in Old versus Adult and Young (P0.05). With arteriolar network architecture maintained during ageing, the impairment in conducted vasodilatation and attenuation of postcontraction vasodilatation may compromise exercise tolerance.
