- Abstract
Background: Extracorporeal membrane oxygenation (ECMO) has been widely used in patients with COVID-19, but uncertainty remains about the determinants of in-hospital mortality and data on post-discharge outcomes are scarce. The aims of this study were to investigate the variables associated with in-hospital outcomes in patients who received ECMO during the first wave of COVID-19 and to describe the status of patients 6 months after ECMO initiation. Methods: EuroECMO-COVID is a prospective, multicentre, observational study developed by the European Extracorporeal Life Support Organization. This study was based on data from patients aged 16 years or older who received ECMO support for refractory COVID-19 during the first wave of the pandemic—from March 1 to Sept 13, 2020—at 133 centres in 21 countries. In-hospital mortality and mortality 6 months after ECMO initiation were the primary outcomes. Mixed-Cox proportional hazards models were used to investigate associations between patient and management-related variables (eg, patient demographics, comorbidities, pre-ECMO status, and ECMO characteristics and complications) and in-hospital deaths. Survival status at 6 months was established through patient contact or institutional charts review. This study is registered with ClinicalTrials.gov, NCT04366921, and is ongoing. Findings: Between March 1 and Sept 13, 2020, 1215 patients (942 [78%] men and 267 [22%] women; median age 53 years [IQR 46–60]) were included in the study. Median ECMO duration was 15 days (IQR 8–27). 602 (50%) of 1215 patients died in hospital, and 852 (74%) patients had at least one complication. Multiorgan failure was the leading cause of death (192 [36%] of 528 patients who died with available data). In mixed-Cox analyses, age of 60 years or older, use of inotropes and vasopressors before ECMO initiation, chronic renal failure, and time from intubation to ECMO initiation of 4 days or more were associated with higher in-hospital mortality. 613 patients

- Abstract
BACKGROUND: Protein tyrosine kinases are important regulators of cellular homeostasis with tightly controlled catalytic activity. Mutations in kinase-encoding genes can relieve the autoinhibitory constraints on kinase activity, can promote malignant transformation, and appear to be a major determinant of response to kinase inhibitor therapy. Missense mutations in the EGFR kinase domain, for example, have recently been identified in patients who showed clinical responses to EGFR kinase inhibitor therapy. METHODS AND FINDINGS: Encouraged by the promising clinical activity of epidermal growth factor receptor (EGFR) kinase inhibitors in treating glioblastoma in humans, we have sequenced the complete EGFR coding sequence in glioma tumor samples and cell lines. We identified novel missense mutations in the extracellular domain of EGFR in 13.6% (18/132) of glioblastomas and 12.5% (1/8) of glioblastoma cell lines. These EGFR mutations were associated with increased EGFR gene dosage and conferred anchorage-independent growth and tumorigenicity to NIH-3T3 cells. Cells transformed by expression of these EGFR mutants were sensitive to small-molecule EGFR kinase inhibitors. CONCLUSIONS: Our results suggest extracellular missense mutations as a novel mechanism for oncogenic EGFR activation and may help identify patients who can benefit from EGFR kinase inhibitors for treatment of glioblastoma.

In this work, the polymeric precursor method (Pechini method) was used to synthesize titanium dioxide. This process allowed a bigger control on the purity of the oxide and the crystalline phase present in the material. In this case, the principal phase was anatase. The resine obtained in this process was characterized using NMR and IR spectroscopy to determine their structure. With this information we proposed a resine structure model. To finish the process, the resin was thermally treated to obtain TiO2. This oxide was characterized using different techniques: ray-X diffraction (RDX) and electron microscopy (TEM and SEM). The results indicated that the TiO2 anatase phase can be obtained at 450ºC with a particle size < 100nm.En este trabajo se ha utilizado el método de precursor polimérico (método de Pechini) para sintetizar dióxido de titanio, proceso que ha permitido tener un mayor control sobre la pureza del óxido y el tipo de fase que ha presentado el material obtenido, fase anatasa en el presente caso. Durante el proceso de síntesis se ha obtenido una resina que se ha caracterizado utilizando la espectroscopia de Resonancia Magnética Nuclear (RMN) y la espectroscopia infrarroja con transformada de Fourier (FTIR) para determinar su estructura y con base en ella se ha propuesto un modelo para la conformación de la misma. Posteriormente esta resina se ha sometido a un tratamiento térmico para obtener los polvos cerámicos que se han caracterizado empleando diferentes técnicas, principalmente, difracción de rayos X (DRX) y microscopia electrónica (MET y MEB). Los resultados indican que se puede obtener TiO2 fase anatasa a los 450ºC con un tamaño de partícula < 100nm.

13. Obtención de nano-estructuras bi-dimensionales de SnO2 utilizando el método pechini: estudio de la conformación de la resina

Pechini method has been widely used in the synthesis of oxides, but the physicochemical process that happens during this one have not been discussed in detail, for example the resin conformation that is obtained in one of the stages of the process. Knowing the resin evolution and since the most important parameter of this one are modified, it could determinate the most appropriate conditions to conform thin films though the Pechini method. In this work the resin evolution of the tin system considering parameters like pH, viscosity, time and temperature of heating were studied. Infrared spectroscopy (FTIR) and viscosity measures in differents steps of the process were used. The IR spectroscopy results shows that as it advances the aging time of the resin, to a temperature between 80 and 100ºC and a pH of 6,5, Sn-O bonds are strengthen. Films were conformed with the resin obtained during the pechini process, it being defined a value of 110cP as the most appropriate to favor the homogeneity and adherence of this ones. The grain size film was of ∼110nm what indicates that the realized process allows to obtain bidimensional nano-structures.A pesar que el método de precursor polimérico, Pechini es ampliamente utilizado en la síntesis de óxidos, los procesos fisicoquímicos que ocurren durante el mismo no se han discutido en detalle; tal es el caso de la conformación de la resina que se obtiene en una de las etapas del proceso. Conociendo la evolución de esta resina, y como se modifican los parámetros más importantes de la misma, se podrían determinar las condiciones más adecuadas para conformar películas delgadas a través de este método. En este trabajo se estudio la evolución de la resina del sistema de estaño considerando parámetros como pH, viscosidad, tiempo y temperatura de calentamiento de la misma. Para este estudio se utilizó espectroscopía infrarroja (FTIR) y medidas de viscosidad de la resina en diferentes instantes del proceso. Los resultados de espectroscopía IR indican que a medida que avanza el tiempo de envejecimiento de la resina, a una temperatura entre 80 y 100ºC y un pH de ∼6,5, se fortalecen los enlaces Sn-O. Con esta resina se conformaron recubrimientos definiéndose un valor de 110cP como el más adecuado para favorecer la homogeneidad y adherencia de los mismos. El tamaño de grano en la película fue de ∼110nm lo que indica que el proceso realizado permite obtener nano-estructuras bidimensionales.

Dc-dc converters for high voltage direct current (HVDC) applications are interesting devices for the development of multi-terminal and dc grid schemes. They can adapt the differences between the dc systems (voltage level, technology and/or line topology) and provide additional protection, controllability and redundancy services. This paper presents a case study of a non-isolated dc-dc converter interconnecting the NordLink and Cobra cable projects. This interconnection requires a dc-dc converter to adapt the dc voltage and the line topology (the first project being a ±525 kV rigid bipole while the second is a ±320 kV symmetric monopole). The case study has been modelled in Matlab/Simulink and several scenarios are simulated. A first set of 8 simulations change the power flow directions verifying the adequate behavior of the dc-dc converter and HVDC links. Based on the previous simulations, additional 16 cases are simulated to verify the system behavior during faults. Two faults are proposed: a pole-to-ground on the positive pole of the rigid bipole line and another on the positive pole of the monopole. It is shown that the dc-dc converter can interconnect two existing lines without changing their initial control strategy. For the normal operation simulations, a fast power variation through the dc-dc converter is tested proving that the line protections are not triggered. The power perturbations created by the dc-dc converter are small enough to keep the system under normal operation conditions. During the fault simulations, the dc-dc converter is able to isolate the healthy side from the fault disturbance (firewall capability). The healthy dc side observes a power disturbance (loss of power flow through the dc-dc converter) without triggering the protections, allowing the continuous power transmission and regaining steady state conditions. The dc-dc converter employs full bridge submodules to be able to block the fault currents, without the presence of dc circuit breakers.

- Abstract
Environmental monitoring in inland waters is an important issue for adequate planning and management of this kind of territories. This paper shows the implementation of the concepts and strategies for a boat to follow a pre-established route through waypoints oriented to these water bodies. An extended Kalman filter is used to estimate orientation and position using the readings of an inertial unit fused with a magnetometer and GPS. The strategy shows to be valid in calm conditions, which is sufficient for the mission. © 2018 IEEE.

Radio spectrometers of the CALLISTO type to observe solar flares have been distributed to nine locations around the globe. The instruments observe automatically, their data is collected every day via internet and stored in a central data base. A public web-interface exists through which data can be browsed and retrieved. The nine instruments form a network called e-CALLISTO. It is still growing in the number of stations, as redundancy is desirable for full 24h coverage of the solar radio emission in the meter and low decimeter band. The e-CALLISTO system has already proven to be a valuable new tool for monitoring solar activity and for space weather research

Unmanned Surface Vehicles (USV) have been used in a wide range of applications, in particular in environmental monitoring. In this work we propose the design and control of an USV which is intended to be used for monitoring environmental variables of the main inlet waters located in Cartagena de Indias, Colombia, such as Chambacu, Cabrero and San Lazaro. The paper shows the results obtained in the naval design, focused on the hull design, and the control strategy for unmanned navigability which is tested in simulation. © 2016 IEEE.

Photodegradation effect is widely used for water purification this contributes to preservation and protection of environment. Titanium oxide, (TiO2), is a compound that shows up this phenomenon. TiO2 is a semiconductor which may degradate pollutants through of a oxidation process. It permit the treatment of the residual water. It this work we has conformed coatings of TiO2 y TiO2-SiO2, utilized Sol-Gel method and investigated the degradation of the blue Methylene. For this, we introduced these coatings in the blue methylene solution which was illuminated with radiation of λ=365nm to activate its photocatilist properties. The structures of the coatings were characterized using Atomic Force Microscopy (AFM) and X-ray Photoelectron Spectroscopy (XPS).El efecto de fotodegradación es ampliamente utilizado para la purificación del agua, acción que contribuye a la conservación y protección del medio ambiente; el óxido de titanio (TiO2) es uno de los semiconductores que pueden degradar contaminantes mediante procesos de oxidación, lo que lo hace apto para el tratamiento de aguas residuales. En este trabajo se conformaron recubrimientos de TiO2 y TiO2-SiO2, por el método Sol-Gel, y se estudio la degradación que experimentaba una solución de azul de metileno al introducirle estos recubrimientos e iluminarlos con una radiación de λ=365nm para activar su propiedad fotocatalítica. Los recubrimientos fueron caracterizados microestructuralmente utilizando Microscopía de Fuerza Atómica (MFA) y Espectroscopía de Fotoelectrones de rayos X (XPS). Los resultados obtenidos de los ensayos de fotodegradación indican que los recubrimientos con una cantidad pequeña de silicio presentan un mayor efecto de fotodegradación indicando que el silicio puede generar puntos de anclaje que facilitan las reacciones de fotocatálisis. Por otro lado, la formación de centros activos, constituidos principalmente por carbono, también contribuyeron al desarrollo de estas reacciones.

BACKGROUND: Few data are available regarding the use of antithrombotic strategies in coronary artery disease patients with atrial fibrillation (AF) in everyday practice. We sought to describe the prevalence of AF and its antithrombotic management in a contemporary population of patients with stable coronary artery disease.METHODS AND FINDINGS: CLARIFY is an international, prospective, longitudinal registry of outpatients with stable coronary artery disease, defined as prior (≥12 months) myocardial infarction, revascularization procedure, coronary stenosis >50%, or chest pain associated with evidence of myocardial ischemia. Overall, 33,428 patients were screened, of whom 32,954 had data available for analysis at baseline; of these 2,229 (6.7%) had a history of AF. Median (interquartile range) CHA2DS2-VASc score was 4 (3, 5). Oral anticoagulation alone was used in 25.7%, antiplatelet therapy alone in 52.8% (single 41.8%, dual 11.0%), and both in 21.5%. OAC use was independently associated with permanent AF (pCONCLUSIONS: In this contemporary cohort of patients with stable coronary artery disease and AF, most of whom are theoretical candidates for anticoagulation, oral anticoagulants were used in only 47.2%. Half of the patients received antiplatelet therapy alone and one-fifth received both antiplatelets and oral anticoagulants. The results of a surveillance study conducted by the International Nosocomial Infection Control Consortium (INICC) from January 2004 through December 2009 in 422 intensive care units (ICUs) of 36 countries in Latin America, Asia, Africa, and Europe are reported. During the 6-year study period, using Centers for Disease Control and Prevention (CDC) National Healthcare Safety Network (NHSN; formerly the National Nosocomial Infection Surveillance system [NNIS]) definitions for device-associated health care-associated infections, we gathered prospective data from 313,008 patients hospitalized in the consortium's ICUs for an aggregate of 2,194,897 ICU bed-days. Despite the fact that the use of devices in the developing countries' ICUs was remarkably similar to that reported in US ICUs in the CDC's NHSN, rates of device-associated nosocomial infection were significantly higher in the ICUs of the INICC hospitals; the pooled rate of central line-associated bloodstream infection in the INICC ICUs of 6.8 per 1,000 central line-days was more than 3-fold higher than the 2.0 per 1,000 central line-days reported in comparable US ICUs. The overall rate of ventilator-associated pneumonia also was far higher (15.8 vs 3.3 per 1,000 ventilator-days), as was the rate of catheter-associated urinary tract infection (6.3 vs. 3.3 per 1,000 catheter-days). Notably, the frequencies of resistance of Pseudomonas aeruginosa isolates to imipenem (47.2% vs 23.0%), Klebsiella pneumoniae isolates to ceftazidime (76.3% vs 27.1%), Escherichia coli isolates to ceftazidime (66.7% vs 8.1%), Staphylococcus aureus isolates to methicillin (84.4% vs 56.8%), were also higher in the consortium's ICUs, and the crude unadjusted excess mortalities of device-related infections ranged from 7.3% (for catheter-associated urinary tract infection) to 15.2% (for ventilator-associated pneumonia). Ventilator-associated pneumonias (VAPs) are a worldwide problem that significantly increases patient morbidity, mortality, and length of stay (LoS), and their effects should be estimated to account for the timing of infection. The purpose of the study was to estimate extra LoS and mortality in an intensive-care unit (ICU) due to a VAP in a cohort of 69 248 admissions followed for 283 069 days in ICUs from 10 countries. Data were arranged according to the multi-state format. Extra LoS and increased risk of death were estimated independently in each country, and their results were combined using a random-effects meta-analysis. VAP prolonged LoS by an average of 2.03 days (95% CI 1.52-2.54 days), and increased the risk of death by 14% (95% CI 2-27). The increased risk of death due to VAP was explained by confounding with patient morbidity.

- Abstract
It is here presented the mechatronic design of an system with automatic storing and picking of small products. The design concept presents the system as a four degrees-of-freedom robot capable of storing objects at different heights. The novelty of the design lies in the space optimization using stacked trays for storing the desired objetcs. Other key components of the system are a device for entry and exit of products, and a robotic arm for manipulating objects. A typical application of such a system is the storage of products inside safe-deposit boxes is presented. © 2014 IEEE.

This article present the design and construction of a PID controller for direct current motors motion control in low-cost robotic applications. The reasons which led us to the make the proposal and design of the system were, on the one hand, the need for robotic developments by students in Colombians academic environments with the resources available locally, and on the other hand, the need for a teaching of the robotics based on laboratory practices of the most commonly used control algorisms in this area, such as: the inverse kinematics based PID control and the dynamic control by calculated torque. © 2014 IEEE.

In order to improve the microstructural and electrical properties in SnO2 varistor ceramics, the influence of differents dopants used, like TiO2 and Al2O3, is under research. The effect of TiO2 and Al2O3 on the properties of Sn-Co-Nb varistor systems obtained by the Pechini method has been investigated in this work. Characterization of synthesized raw material was performed by X-Ray Diffraction (XRD) and Scanning Electronic Microscopy (SEM). The microstructural and electrical characterization of sintered samples show that the TiO2 favors the grain growth and the Al2O3 contributes to the decrease it, effect that is manifested in the Sn-Co-Nb varistor systems. Breakdown field increase up to 6300V/cm with increasing Al2O3 content and non-linear coefficients with alpha=22 were obtained. - Abstract
ACuteTox is a project within the 6th European Framework Programme which had as one of its goals to develop, optimise and prevalidate a non-animal testing strategy for predicting human acute oral toxicity. In its last 6 months, a challenging exercise was conducted to assess the predictive capacity of the developed testing strategies and final identification of the most promising ones. Thirty-two chemicals were tested blind in the battery of in vitro and in silico methods selected during the first phase of the project. This paper describes the classification approaches studied: single step procedures and two step tiered testing strategies. In summary, four in vitro testing strategies were proposed as best performing in terms of predictive capacity with respect to the European acute oral toxicity classification. In addition, a heuristic testing strategy is suggested that combines the prediction results gained from the neutral red uptake assay performed in 3T3 cells, with information on neurotoxicity alerts identified by the primary rat brain aggregates test method. Octanol-water partition coefficients and in silico prediction of intestinal absorption and blood-brain barrier passage are also considered. This approach allows to reduce the number of chemicals wrongly predicted as not classified (LD50 > 2000 mg/kg b.w.).

This paper will provide an overview of the research work of the European Enhanced Vehicle-safety Committee (EEVC) in the field of crash compatibility between passenger cars. Since July 1997 the EC Commission is partly funding the research work of EEVC. The running period of this project will be two years. The progress of five working packages of this research project is presented: literature review, accident analysis, structural survey of cars, crash testing, and mathematical modelling. According to the planned time schedule the progress of research work is different for the 5 working packages.

The effect of synthesis conditions and niobium incorporation levels on the photocatalytic properties of Nb/MCM-41 molecular sieves was assessed. Niobium pentoxide supported on MCM-41 mesoporous silica was obtained using two methods: sol-gel and incipient impregnation, in each case also varying the percentage of niobium incorporation. The synthesized Nb-MCM-41 ceramic powders were characterized using the spectroscopic techniques of infrared spectroscopy (IR), Raman spectroscopy, X-ray diffraction (XRD), and transmission electron microscopy (TEM). The photodegradation capacity of the powders was studied using the organic molecule, methylene blue. The effect of both the method of synthesis and the percentage of niobium present in the sample on the photodegradation action of the solids was determined. The mesoporous Nb-MCM-41 that produced the greatest photodegradation response was obtained using the sol-gel method and 20% niobium incorporation. [ABSTRACT FROM AUTHOR]

Receptor tyrosine kinase genes were sequenced in non-small cell lung cancer (NSCLC) and matched normal tissue. Somatic mutations of the epidermal growth factor receptor gene ECFR were found in 15 of 58 unselected tumors from Japan and 1 of 61 from the United States. Treatment with the EGFR kinase inhibitor gef itinib (Iressa) causes tumor regression in some patients with NSCLC, more frequently in Japan. ECFR mutations were found in additional lung cancer samples from D.S. patients who responded to gefitinib therapy and in a lung adenocarcinoma cell line that was hypersensitive to growth inhibition by gefitinib, but not in gefitinib-insensitive tumors or cell lines. These results suggest that ECFR mutations may predict sensitivity to gefitinib. [ABSTRACT FROM AUTHOR]
