Purpose. The purpose of this quality improvement (QI) project was to implement an evidence-based follow-up guideline tool for allogeneic stem cell recipients who are over three months—or approximately 100 days—post-transplant at Banner MD Anderson Cancer Center (BMDACC). The goal was to encourage providers at BMDACC to utilize the guidelines to provide long-term follow-up care for allogeneic stem cell transplant patients after three months post-transplant.Background. Allogeneic (non-self-donor) hematopoietic stem cell transplant (HSCT) is a potentially curative procedure for several hematological malignancies. Allogeneic HSCT is associated with late complications—occurring after 100 days post-transplant—that impact survivors’ life expectancy and quality of life. Over 65,000 allogeneic HSCTs occur worldwide every year. Nearly 75% of survivors will experience at least one late complication. These complications physically, mentally, and emotionally affect survivors. Providing long-term follow-up care for these patients improves health and safety by preventing complications that put them at risk for early mortality. Methods. This QI project was implemented by providing education at a synchronous online meeting regarding allogeneic HSCT and late effects and introducing the guidelines. Two surveys were delivered via email to participants to complete before and after the education. A third survey was delivered via email three weeks after implementing the guidelines to evaluate provider use and impression. Results. Of the 12 individuals invited to participate, five participants responded to the pre-survey, one responded to the post-survey, and five responded to the follow-up survey. The one participant who completed the pre- and post-survey answered more questions correctly on the post-survey than the pre-survey. Valerie Nelson, Jacqueline Ziehr, Mark Agulnik, Melissa JohnsonRobert H Lurie Comprehensive Cancer Center of Northwestern University, Chicago, IL, USAAbstract: The discovery of epidermal growth-factor receptor (EGFR)-activating mutations and the introduction of oral EGFR tyrosine kinase inhibitors (EGFR-TKIs) have expanded the treatment options for patients with non-small cell lung cancer. The first two reversible EGFR-TKIs, erlotinib and gefitinib, are approved for use in the first-line setting in patients with known EGFR-activating mutations and in the second- and third-line settings for all NSCLC patients. These first-generation EGFR-TKIs improve progression-free survival when compared to chemotherapy in patients with EGFR-activating mutations in the first-line setting. However, nearly all patients develop resistance to EGFR-directed agents. There is a need for further therapy options for patients with disease progression after treatment with reversible EGFR-TKIs. Afatinib is an irreversible ErbB family blocker that inhibits EGFR, HER2, and HER4. In vitro and in vivo, afatinib have shown increased inhibition of the common EGFR-activating mutations as well as the T790M resistance mutation when compared to erlotinib and gefitinib. Clinically, afatinib has been evaluated in the LUX-Lung series of trials, with improvement in progression-free survival reported in patients with EGFR-activating mutations in both first- and second-/third-line settings when compared to chemotherapy. The significance of private standards and associated local level initiatives in agri-food value chains are increasingly recognised. However whilst issues related to compliance and impact at the smallholder or worker level have frequently been analysed, the governance implications in terms of how private standards affect national level institutions, public, private and non-governmental, have had less attention. This article applies an extended value chain framework for critical analysis of Private Standards Initiatives (PSIs) in agrifood chains, drawing on primary research on PSIs operating in Kenyan horticulture (Horticulture Ethical Business Initiative and KenyaGAP). The paper explores the legislative, executive and judicial aspects of governance in these southern PSIs highlighting how different stakeholders shape debates and act with agency. It is argued that governance is exercised ‘beyond the vertical’ in that one can identify wider horizontal processes of governance, including how the scope of key debates is constructed (especially in legislative governance) but analysis of executive governance emphasises the dominant role of the lead buyers.
