Goals/Purpose The ligamentous system supporting the penis is widely called « penile suspensory ligament ». It supports and attaches the penis to the abdominal wall and the pubic bone. However, the complexity of the anatomy of the penile suspensory system was underestimated. The aim of this anatomical study was to describe the different ligamentous (features, insertion sites, path) and their functions. Methods/Technique The first step consisted in a description of the different components of the penile suspensory apparatus on two cadavers embalmed by the Thiel method. The second part was an evaluation of the function of each ligament on freshly dead cadavers. In the third stage, we applied those findings to an exploratory cohort of patients undergoing a penile lengthening surgery for buried penis. Results/Complications The penile suspensory apparatus consisted in four ligaments from anterior to posterior : the fundiform ligament, the suspensory ligament, dense vertical ligament and the arcuate ligament. The first two were loose, non-stabilizing and the two posterior ones were dense and stabilizing. In our cohort, 12 patients were included. The mean BMI was 38. The mean pre-operative flaccid length was 4.1 cm [2.2 cm – 7 cm] and at 6 months 8.2 cm [5.2 cm - 11.5 cm] which represents a mean augmentation of 100%. The mean pre-operative circumference at a flaccid state was 8.2 cm [2 cm – 11.5 cm] and at 6 months post-operative was 12.6 cm [8.3 cm - 16 cm] which represents a mean augmentation of 85.7%. The most common complication was a phimosis. Conclusion We have described the detailed anatomy of the penile suspensory apparatus. It is the first time that the dense vertical ligament was brought to light. To avoid instability in penile lengthening, the posterior part of the dense vertical ligament and the arcuate ligament must be spared.

With surgical education moving from a time-based to a competency-based model, developing high-fidelity simulation models has become a priority. The Thiel cadaveric model has previously been used for a number of medical and surgical simulations, including microvascular simulation. We aim to investigate the use of the Thiel model in peripheral nerve simulation and validate a novel evaluation instrument. Sixteen residents ranging from postgraduate years 1 to 6 participated in the study. Their nerve coaptations using Thiel cadaveric nerves were video recorded and evaluated by 5 fellowship-trained microsurgeons using the Micro-Neurorrhaphy Evaluation Scale (MNES). The intraclass correlation among the 5 evaluators was 0.75, revealing excellent interrater reliability. The Cronbach α was .77, underlining the internal consistency of the test items. Bivariate analysis revealed a significant association between the MNES scores and the participants' self-declared level of experience. This correlation was confirmed by mixed modeling. Our results validate the MNES and underscore the utility of the Thiel nerve tissue for peripheral nerve surgical simulation.Puisque l’enseignement de la chirurgie passe d’un modèle axé sur le temps à un modèle axé sur les compétences, il est devenu prioritaire de créer des modèles de simulation haute fidélité. La méthode cadavérique de Thiel a déjà été utilisée pour plusieurs simulateurs médicaux et chirurgicaux, y compris la simulation microvasculaire. Les chercheurs ont l’intention de l’explorer pour la simulation des nerfs périphériques et de valider un nouvel instrument d’évaluation. Seize résidents de première à sixième année ont participé à l’étude. Leur coaptation nerveuse à l’aide des nerfs cadavériques de Thiel a été enregistrée sur vidéo et évaluée par cinq microchirurgiens possédant un postdoctorat à l’aide de l’échelle d’évaluation de la microneurorraphie (ÉÉMN). La corrélation intraclasse entre les cinq évaluateurs était de 0,75, révélant une excellente fiabilité interévaluateur. Le coefficient alpha de Cronbach était de 0,77, soulignant la cohérence interne des éléments à l’essai. L’analyse bivariée a révélé une association significative entre les scores d’ÉÉMN et le taux d’expérience déclaré par les participants. Cette corrélation a été confirmée par modélisation mixte. Les résultats des chercheurs valident l’ÉÉMN et soulignent l’utilité des tissus nerveux de Thiel pour la simulation chirurgicale des nerfs périphériques.

Frostbite causes tissue damage through five major mechanisms, out of which two are amenable to treatment. The first-line treatment is rapid rewarming therapy using water at 40°C to 42°C, which addresses the formation of ice crystals in the intra and extra cellular compartments. The second mechanism is progressive tissue ischemia after rewarming and is only accessible to a second-line therapy represented by thrombolysis. This study aimed to determine the efficacy of thrombolysis. This is a single-center retrospective cohort study, where it was aimed to evaluate two groups of patients. A total of 18 patients were included in this study. Mean times between injury to thrombolytic therapy and admission to thrombolytic therapy was 26.04 hours (SD 13.6) and 9.65 hours (SD 9.89), respectively. All patients suffered injuries ranging from second-degree deep to third degree. The rate of patients having complete, partial, and no angiographic responses were 55.6%, 11.1%, and 33.3%, respectively. The main outcome of interest showed that 11 (61.1%) patients in total had amputations at different levels. Results showed that in the intervention group, five (55.6%) of the patients had amputations compared with six (66.7%) from the control group (P = .6) at comparable levels of amputation. The literature supports that the use of intra-arterial tissue plasminogen activator might be beneficial for severe cases of frostbites; however, it lacks of studies of major significance and results are often controversial. Our study has not shown statistically significant results on amputation levels and cannot support the hypothesis of efficacy of thrombolytic therapy.

Background Although increasingly reported in the literature, most plastic surgeons cannot define the etiology of double capsules. Often an incidental finding at implant exchange, double capsules are frequently associated with macrotextured devices. Several mechanisms have been proposed, including at the forefront that shearing causes a delamination of the periprosthetic capsule into a double capsule. Objectives This study was designed to confirm the hypothesis that mechanical forces are involved in formation of double capsules by histological analysis. Methods A prospective analysis of consecutive implants with double capsules removed over 2 years was performed. Data collected at the time of surgery included Baker classification, reason for explant, implant manufacturer and style, and any presence of a seroma associated with the capsule. Specimens were sent for analysis by histology utilizing hematoxylin and eosin and alpha-smooth muscle actin staining techniques. Results Eight double capsules were collected for specimen analysis. All capsules demonstrated evidence of granulation tissue, alpha-smooth muscle actin positive myofibroblasts, and folds with embedded texture. Fibrosis surrounded weak areas with presence of layering and splitting, creating a potential space that is prone to separation. Tears and folds from granulomatous reaction are also present within the outer layer of the double capsule, which can only be explained by a mechanical shearing force as a pathogenic mechanism. Conclusions Understanding the pathogenesis of double capsules may allow plastic surgeons to refine their indications for macrotextured implants while providing guidance to patients on avoidance of activities that produce shear-forces. The findings support the hypothesis that shearing forces delaminate the capsule into 2 separate distinct capsules. Level of Evidence: 5

The Thiel embalmment method has recently been used in a number of medical simulation fields. The authors investigate the use of Thiel vessels as a high fidelity model for microvascular simulation and propose a new checklist-based evaluation instrument for microsurgical training.Thirteen residents and 2 attending microsurgeons performed video recorded microvascular anastomoses on Thiel embalmed arteries that were evaluated using a new evaluation instrument (Microvascular Evaluation Scale) by 4 fellowship trained microsurgeons. The internal validity was assessed using the Cronbach coefficient. The external validity was verified using regression models.The reliability assessment revealed an excellent intra-class correlation of 0.89. When comparing scores obtained by participants from different levels of training, attending surgeons and senior residents (Post Graduate Year [PGY] 4-5) scored significantly better than junior residents (PGY 1-3). The difference between senior residents and attending surgeons was not significant. When considering microsurgical experience, the differences were significant between the advanced group and the minimal and moderate experience groups. The differences between minimal and moderate experience groups were not significant. Based on the data obtained, a score of 8 would translate into a level of microsurgical competence appropriate for clinical microsurgery.Thiel cadaveric vessels are a high fidelity model for microsurgical simulation. Lorsqu’on compare les scores obtenus par des participants de divers niveaux de formation, les chirurgiens et les résidents seniors (de quatrième ou cinquième année) ont obtenu des résultats significativement supérieurs à ceux des résidents juniors (de première à troisième année). La différence entre les résidents seniors et les chirurgiens traitants n’était pas significative. Les différences d’expérience microchirurgicale étaient significatives entre le groupe avancé et les groupes ayant une expérience minime ou modérée. D’après les données obtenues, un score de 8 désignerait un niveau de compétence microchirurgicale acceptable pour la microchirurgie clinique.Les vaisseaux cadavériques Thiel sont un modèle haute-fidélité de simulation microchirurgicale. D’excellentes mesures de validité interne et externe ont été obtenues au moyen de l’échelle de simulation microchirurgicale en dix étapes.

Peut-on dégager une typologie des ingénieurs en Europe occidentale selon leur parcours de formation, leur mode de socialisation et leur intégration dans les entreprises ? Cette interrogation entre en résonance avec la nouvelle question d’histoire contemporaine pour les concours d’enseignement : « Le travail en Europe occidentale des années 1830 aux années 1930. Mains-d’œuvre artisanales et industrielles, pratiques et questions sociales ». Dans cet article, nous relions dans un premier temps ingénieurs et processus d’industrialisation. Puis nous corrélons la structuration des systèmes de formation aux configurations étatiques de l’ouest européen. Nous étudions ensuite les modalités de socialisation des ingénieurs et abordons les enjeux de la féminisation des mondes d’ingénieur. Enfin, nous nous intéressons aux ingénieurs en action et dans les entreprises. L’ingénieur est un personnage à facettes multiples. Son portrait et celui de son groupe, change au gré des transformations du paysage dans lequel il interagit. Nous nous efforçons d’en dégager de grandes caractéristiques. Engineers work. Who are they? How were they trained? What are they doing? For which companies and under what conditions? Can we identify a typology of engineers in Western Europe of the xixth-xxth century? It is not easy to answer these interrogations which resonate with the new subject of contemporary history for teaching competitions in France: “Work in Western Europe from the 1830s to the 1930s. Hands craft and industrial work, practices and social issues”. This opening paper has been informed by this specific focus. First, we link the need for new engineers with the industrialization processes. Then we correlate the structuring of training systems with the specific state configurations. Next we study the ways of socialization of engineers and tackle the challenges of the feminization of the engineering profession. Finally, we study a few cases of engineers in action in the companies they work for. An engineer is a multifaceted character. His portrait and that of his group, changes according to the transformations of the landscape in which he interacts. We strive to bring out the major features of European engineers.

Cet article présente le cas de Pierre Bézier (1910-1999), un ingénieur issu des Arts et Métiers puis de l’École supérieure d’électricité qui a fait toute sa carrière (1933-1975) dans l’entreprise automobile Renault. Bézier a été tour à tour l’artisan d’une forme originale d’automatisation (1947), un pionnier en Europe de la commande numérique (1958), puis l’un des fondateurs, à l’échelle mondiale, de l’informatique industrielle (1966). Les « courbes et surfaces de Bézier » sont au fondement de la Conception et fabrication assistées par ordinateur (CFAO) qui ont réorganisé le design, la production de série et notre monde virtuel. Pourquoi cet innovateur important est-il resté un « inconnu célèbre » ? En confrontant les archives de l’entreprise aux documents conservés par l’un de ses ingénieurs l’article mesure le destin d’un électromécanicien de génie à l’aune de l’évolution d’une usine. Il montre qu’un parcours individuel peut requestionner le passé d’une entreprise. Comment une biographie sert-elle l’histoire globale ? This paper presents the case-study of Pierre Bézier (1910-1999) a former student of the École des arts et métiers then of the École supérieure d’électricité who spent his entire career (1933-1975) as an engineer in the Renault car company. Bézier was alternately the architect of an original form of automation (1947), a pioneer in Europe of digital control (1958), and last, one of the world founders of industrial computing (1966). The “Bézier curves and surfaces” are the roots of Computer-Aided Design and Manufacturing (CADM) that have reorganized industrial drawing, mass production and our virtual world. Why has this key engineer remained an “unknown celebrity”? By comparing the company’s archives with the engineer’s personal documents, the paper measures the fate of a brilliant electromechanical innovator by the yardstick of his factory’s transformations. It shows how the story of a person can renew our understanding of the past of a firm. How can a biography serve global history?

Les historiens ont longtemps privilégié le facteur technique dans l’approche des révolutions industrielles. Dans cette logique monocausale, le progrès technique était assimilé à une succession d’inventions apparues dans des secteurs pionniers, moteurs de la croissance, entraînant le reste de l’économie, dite traditionnelle, dans son sillage. L’un des paradoxes de cette approche consistait à valoriser l’innovation tout en évitant d’interroger les pratiques inventives. La dynamique interne du progrès technique et les traits de génie des inventeurs tenaient lieu de modèles explicatifs. La remise en cause de ces approches suscite bien des interrogations de méthode. Comment repérer les formes de l’invention ordinaire, en cerner les acteurs ? Comment assigner une origine à des nouveautés dont l’antériorité se perd dans la mémoire commune ? Comment appréhender des savoirs pratiques instables et non codifiés que ne livrent pas les corpus constitués de sources ? Comment concilier les définitions construites de l’invention et de l’inventeur, les catégories déjà forgées par les institutions et le corps social, et les mentions informelles ou indirectes de l’invention ? Ces questions débordent l’écrit. Cet ouvrage, issu d’un colloque international tenu à Paris en 2003, élargit le concept de sources : au-delà des « sources-textes », il considère les dessins, les enregistrements sonores, les instruments et outils, les installations, les échantillons, les modèles, les prototypes, etc. Il propose une réflexion originale sur le statut des archives de l’invention, sur leur mode de production et sur les méthodologies mises en œuvre dans leur exploitation.
