This article has restudied the Petersen family in Graph Theory. It discussed the process of establishing this family. This discussion leads to discovering some new properties of the Petersen family's members.

We present the imaging findings of thoracic systemic venous anomalies diagnosed by computed tomography and magnetic resonance imaging. Persistent left superior vena cava is the commonest anomaly of the thoracic systemic veins encountered either incidentally as an isolated finding or associated with congenital heart disease. Inferior vena cava (IVC) interruption with azygos continuation is the second most common anomaly, which may also be isolated or be associated with left isomerism syndrome. The article will also discuss other rarer systemic venous anomalies including retroaortic brachiocephalic vein and IVC drainage into the left atrium. Finally, the impact of pre-procedure reporting of thoracic systemic venous anomalies on the choice of intervention and patient outcome will be addressed.

This study evaluated whether the location of steroid deposition (intra-articular vs extra-articular) for thumb carpometacarpal (CMC) joint arthritis affects clinical outcomes.We prospectively enrolled 102 hands (82 patients) with thumb CMC joint arthritis. Patients received a CMC joint injection with Triamcinolone and radiopaque contrast. Wrist radiographs were used to visualize the injection location. Patients completed Disabilities of the Arm, Shoulder, and Hand Questionnaire (DASH) questionnaires and visual analog scale (VAS; scale, 1-100) pain scores before injection and then at 1 week and 1, 3, and 6 months after injection. Generalized linear regression models were constructed to identify variables associated with clinical outcomes.The rate of intra-articular injection was 80%. No differences were found between the 2 groups in preinjection DASH or VAS scores. After 1 week, both the intra-articular and extra-articular groups showed improvements of DASH (14.2 and 11.2, respectively) and VAS (15.5 and 15.0, respectively) scores. Although both groups were worse at 3 months, the intra-articular group had significantly lower DASH (26.7 vs 37.5, respectively) and VAS (26.5 vs 39.0, respectively) scores than the extra-articular group. There were no differences between the intra-articular and extra-articular groups for DASH (33.8 vs 42.5, respectively) or VAS scores at 6 months. The intra-articular group maintained significant improvements in outcomes for up to 6 months, while the extra-articular group only maintained them for up to 1 month. The Eaton-Littler classification was found to be a predictor of DASH and VAS scores at 3 and 6 months.Intra-articular injection in the thumb CMC joint provides significantly greater pain relief and functional improvement compared to extra-articular injection at 3 months. Inadvertent extra-articular injection is common and appears to provide short-term pain relief and functional improvement. Some patients receiving intra-articular injections continue experiencing relief for up to 6 months.Therapeutic II.

Background: Opioid usage in cardiac surgery is considered to be the corner stone in management. Inadequate pain control after cardiac surgery complicates patient recovery and increases the load on healthcare services. Multimodal analgesia can be used to achieve better analgesic effect and improves patient outcome. Material and methods: A total of 90 patients undergoing cardiac surgery with median sternotomy were randomly allocated equally into three groups intraoperatively where first group received continuous infusion of high dose opioids, second group received boluses of low dose opioids and third group received multimodal non opioid analgesics including dexmedetomidine, ketamine and magnesium sulphate. All patients received the same post-operative analgesic regimen consists of morphine patient controlled analgesia (PCA). Results: Patients in multimodal non opioid group had more stable hemodynamics intra and postoperatively. Also, patients in multimodal group had lower pain scores extubation, earlier extubation, shorter ICU stay, earlier mobilization and earlier return of bowel movements compared to patients of both groups received intraoperative higher opioid doses.

Objective Arteriovenous grafts (AVGs) are frequently needed in hemodialysis (HD) patients with unsuitable superficial veins. First cannulation of standard arteriovenous grafts (sAVGs) still require about 2 weeks after implantation. Early cannulation arteriovenous grafts (eAVGs) were suggested to overcome this shortcoming. The present randomized study proposed to compare the clinical outcomes of sAVGs and eAVGs in HD patients. Methods The present single-center randomized clinical study recruited 477 HD patients indicated for AVG creation. They included 236 in the sAVG group and 241 in the eAVG group. Eligible patients were simply randomized and allocated to the studied groups using 1:1 allocation ratio. Blinding was secured using the sealed envelope technique. Enrolled patients were followed up for 12 months. The primary outcome in the present study was primary, primary assisted, and secondary patency rates at 12 months. Other outcome parameters included time to first cannulation, graft complications, and mortality. Results Comparison between the studied groups regarding the primary outcomes revealed no statistically significant differences. Primary patency rate was 65.7% and 68.0% (P = .58) at 6 months and 53.8% and 56.4% (P = .57) at 12 months in the sAVG and eAVG groups, respectively. Primary assisted patency rate was 70.8% and 69.7% (P = .8) in patients with sAVG and eAVG, whereas the reported rates at 12 months were 59.3% and 61.0% (P = .71), respectively. Secondary patency rate was 78.4% and 73.9% (P = .25) at 6 months and 67.8% and 69.7% (P = .65) at 12 months in the sAVG and eAVG groups, respectively. As expected, patients in the eAVG group experienced significantly earlier cannulation when compared with patients in the sAVG group (median, 3.0 days; range, 1.0-9.0 days vs 19.0 days; range, 15.0-22.0 days; P Conclusions Patients in the eAVG group have comparable outcomes to those in the sAVG group at 12 months with the added advantage of earlier time to first cannulation.

With the anticipated growth of the internet of things (IoT) market, many low-power wide-area (LPWA) technologies have been introduced to connect a wide range of IoT devices with varying performance requirements. The narrowband internet of things (NB-IoT) is a 3rd generation partnership project (3GPP) standardized LPWA technology that meets most IoT service requirements. In this paper, the design and implementation of the physical uplink transmitting chain as well as the physical downlink receiving chain of the NB-IoT user equipment (UE) are presented. Both chains’ main blocks are designed to follow the 3GPP NB-IoT LTE standard Release 14 (Rel-14). The whole design is experimentally implemented on the Virtex 7 (VC 709) Connectivity kit, and all performance metrics are reported. Moreover, an NB-IoT base station is implemented and integrated with the two prototyped UEs to set-up an NB-IoT system, which is employed to send data from one UE to another UE through the NB-IoT base station using two FPGAs (one to implement the sending UE and the other one to implement the receiving UE) and three universal software radio peripherals (USRPs) B200 (one to implement the RF front-end of the transmitting UE FPGA, one to implement the base station RF front-end, and one to implement the RF front-end of the receiving UE FPGA). Experimental results show that the implemented NB-IoT system is working successfully, as the two NB-IoT UEs communicate together successfully through the NB-IoT base station and exchange the data properly.

Background: Acral vitiligo is usually more resistant to medical management due to minimal density of hair follicles and greater chance of repeated friction or trauma. Objective: To determine efficacy and safety of adding topical platelet rich plasma (PRP) to automated micro needling and narrow band ultra violet B (NB- UVB) in treatment of resistant acral vitiligo and to assess the best treated sites with this combination therapy. Patients and methods: Forty-two patients with stable acral vitiligo were recruited. lesions at the hand were classified according to their sites into (Dorsum, periungual, knuckles and inversed knuckles)while lesions at the feet were classified into (Dorsum, lateral side and medial side of the feet). One side was treated with automated micro needling and topical PRP (group A) and the other side with automated micro needling only (group B) twice monthly for 3 months. Both sides received NB-UVB phototherapy twice weekly. Results: One month after the last session, in both groups A and B, a statistically significant better percent of pigmentation was noticed at the dorsum of the hands 38.95 ± 39.81 and 42.11 ± 33.51 respectively followed by inverted knuckles 28.42 ± 37.01 and 25.53 ± 37.19 respectively , while no significant difference was noted in comparing both groups A and B either at the hands or the feet lesions, regarding the percent of regimentation, the percent of change in VASI, change in number of the lesions and color match. Conclusion: Adding PRP to NB-UVB and micro needling has no significant advantage in treating patients with resistant acral vitiligo. The response to treatment is better in area with hair follicles as the dorsum of the hand and inverted knuckles, while areas over bony prominences (knuckles) and devoid of hair follicles (periungual) are resistant to this combination therapy.

Background Dermatomycosis affects millions of people worldwide; the availability of data on the prevalence of superficial fungal infections and some associated epidemiological factors in our region is little. Objective To compare conventional and molecular methods to characterize some fungal species causing human mycotic infection. Patients and methods The present cross-sectional study of fungal skin diseases was done on 100 patients. Skin scrapings and nail cuttings were subjected to direct microscopic examination using potassium hydroxide and lactophenol cotton blue, followed by culturing on Sabouraud’ dextrose agar. HiCrome agar was used for characterization of Candida species. Molecular diagnosis of some fungal isolates was performed by sequencing the ribosomal DNA internal transcribed spacer region. Results Tinea capitis was the commonest clinical presentation in 48% of cases, followed by tinea corporis 20% and tinea faciei 13%. Fungi were demonstrated in 88% of cases by direct microscopy and/or culture. Direct microscopy was positive in 51% of cases. A higher percentage of positivity 77% was obtained from samples cultured on Sabouraud’ dextrose agar. Dermatophytes were isolated from patients who complained of tinea capitis and tinea corporis (75 and 70%, respectively). Comparing molecular with conventional results, it was found that molecular identification was confirmatory to conventional results, except in two isolates: Chrysosporium lobatum was previously diagnosed as Chrysosporium tropicum and Trichophyton interdigitale was previously diagnosed as Trichophyton mentagrophytes by conventional methods. Conclusion Trichophyton violaceum and Microsporum canis were the main etiologic agents of tinea capitis (45.8 and 27%, respectively). Epidermophyton floccosum was only isolated from four cases of tinea corporis and one of the tinea cruris. Candida species were the main causal agents of onychomycosis. Acremonium spinosum and C. lobatum appeared in this study as new etiologic agents of nail infections. Molecular identification is confirmatory to conventional results, but it is expensive and faces some difficulties, in our country, its use is encouraged to identify dermatophytes in atypical isolates.

Objective: Turner syndrome (TS) patients are at high risk of cardiometabolic disorders. Cardiometabolic risk factors are more commonly related to visceral rather than total body adiposity. Adipocytokines have been explored as a potential link between obesity and obesity-related cardiometabolic dysfunction. This study explored the validity of epicardial fat-thickness (EFT) and perihepatic fat-thickness (PHFT) measurement as cardiometabolic-risk predictors in TS-girls in relation to standard obesity-indices and metabolic syndrome (MetS) components. Methods: Forty-six TS girls and twenty-five controls (10-16 years) were subdivided into two age-groups (10 to less than 13 and 13-16). Participants were assessed for body mass index (BMI) Z-scores, waist circumference (WC), total-fat mass (FM) and trunk-FM by bioimpedance-technique, EFT and PHFT by cardiovascular magnetic resonance, lipid-profile, homeostasis model assessment of insulin resistance (HOMA-IR), and serum chemerin. MetS was defined according to International Diabetes Federation criteria. Results: Overweight/obesity and MetS were detected in 45.7% and 37% of TS-girls respectively. BMI Z-score, WC, total-FM, trunk-FM, EFT and PHFT values were significantly higher in TS-age groups compared to age-matched control groups, being more pronounced in the older group when TS-girls had been exposed to estrogen. Dyslipidemia, higher HOMA-IR, chemerin, EFT and PHFT values were observed in lean-Turner compared to BMI-Z-matched controls. EFT and PHFT were significantly correlated with chemerin and several components of MetS. EFT at a cut-off-value of 6.20 mm (area under the curve=0.814) can predict MetS in TS-girls. Conclusion: TS-girls displayed an adverse cardiometabolic profile during late childhood and adolescence. EFT and PHFT are emerging cardiometabolic risk predictors in TS-patients. Excess EFT rather than total body adiposity may contribute to altered metabolic profile among lean-Turner patients.

Background Behçet’s disease (BD) is a multisystemic vasculitis that may affect the heart. However, the incidence and nature of cardiac involvement in BD have not been clearly documented yet. The aim of this study was to delineate the cardiac magnetic resonance imaging (MRI) appearances of cardiac involvement in BD patients. Methods This cross-sectional observational study was carried out 30 BD patients without known cardiac disease. Patients were subjected to history taking, physical examination, echocardiography and cardiac MRI. Results At least one abnormality on cardiac MRI was observed in 20/30 patients (66.67%). Myocardial oedema was observed in 3 patients (10%) and late gadolinium enhancement in 1 patient (3.3%). Pericardial effusion was found in 3 patients (10.0%), global hypokinesia in 6 patients (20.0%) and intra-cardiac thrombosis in only 1 patient (3.3%). Pulmonary artery was dilated in 4 patients (13.3%). Left ventricular (LV) and right ventricular (RV) end diastolic volume were altered in 4 patients (13.3%) and 7 patients (23.3%) respectively. LV and RV end systolic volume were abnormal in 7 patients (23.3%) and 5 patients (16.7%) respectively. There was aortic valve regurge in 2 patients (6.7%), tricuspid valve regurge in 9 patients (30%), and mitral valve regurge in 9 patients (30%). Dilated left main coronary artery was found in 2 patients (6.7%) and arrhythmogenic right ventricular dysplasia in only one patient 1 patient (3.3%). On logistic regression analysis, BD activity index score was a significant predictor of cardiac abnormalities. Conclusion BD may cause cardiac abnormalities without clinical manifestations and cardiac MRI may represent a tool for early detection of these subtle abnormalities. Higher BD activity index scores are strongly linked to cardiac problems.

The need for more efficient drug delivery strategies with ultraprecision and control over the release of drugs has led to the growth of more sophisticated drug-releasing systems as a promising alternative to conventional clinical therapies. This new seed of strategies has explored an encouraging property to overcome the inherent problems of traditional therapies. One of the major challenges for any drug delivery system is the introduction of a complete view of the delivery system. In this article, we intend to elucidate the theoretical proof of concept of the electrosynthesis ATN@DNA core-shell like structure as a model system. Therefore, we present a fractal kinetic model (non-exponential model) taking into consideration the concept of time-dependent diffusion coefficient, which was developed using a numerical method with the help of COMSOL Multiphysics. In addition to that, we present here a general fractional kinetic model in sense of the tempered fractional operator, which leads to better characterized memory properties of the release process. Also, the fractional model is compared with the fractal kinetic model and both offer a good description of drug release processes that present anomalous kinetics. The solutions of the fractal and fractional kinetic models are also fitted successfully with our real-release results.

Background Based on increased understanding of the idiopathic intracranial hypertension (IIH) pathophysiology, venous sinus stenting (VSS) has emerged as an effective treatment for patients with transverse sinus stenosis (TSS). The presence of a reliable TSS screening tool is warranted. The combined conduit score (CCS) is the most widely used score for evaluation of the cerebral sinovenous stenosis in contrast-enhanced magnetic resonance venography (CE-MRV). Purpose of the Study To evaluate the interobserver agreement between neuro-interventionalists and radiologists with respect to the CCS in evaluation of transverse sinus stenosis in patients with idiopathic intracranial hypertension using CE-MRV. Methods A retrospective study was conducted on 26 consecutive patients diagnosed with IIH and underwent CE-MRV. The 2 neuro-interventionalists and 2 radiologists separately evaluated the cerebral venous sinuses using the CCS. Results The mean CCS was significantly different between the neuro-interventionalists and radiologists (p Conclusion Despite the excellent agreement between the neuro-interventionists and the good agreement between the radiologists, there was no agreement between the neuro-interventionists and the radiologists. Our finding suggests that there is a gap between the 2 specialties but does not favor any of them. Factors related to the observers, the venous sinus system, the MRV or the CCS score may have resulted in this discrepancy. Automatic or semi-automatic feature extractions to produce quantifiable biomarkers for IIH are warranted. The clinical decisions should not depend only on strongly observer-dependent scores with training and/or experience-dependent influences.

Microbiologically influenced corrosion (MIC) is a major problem in the petroleum industry. Culture-based techniques such as most probable number (MPN) and isolation only reveal a very small data of the microorganisms present. By application of metagenomic analysis, the microbial community present in the sample has been identified, which helps in understanding what microorganisms are doing and even what they are capable of doing, by identifying their genomic and thus metabolic potential. For this reason, formation water samples were collected from an oil field production tank located in the North Bahrya Petroleum Company (NORPETCO) Egypt. The results revealed that the formation water sample has a high degree of microbial variation, where the phylum Proteobacteria was most dominant with 60% of total reads in the phylum level, and it was also overwhelmingly dominated by the family Desulfovibrionaceae and it was found to be about (53%) of total reads.

The practical application of genistein (GEN) as a drug system is hindered by its low aqueous solubility and poor oral bioavailability. Encapsulation of poor water-soluble drug was considered as one of the widely used approaches to overcome such of these obstacles. The primary goal of this research was to in situ encapsulate the hydrophobic GEN during the electrosynthesis of DNA nanospheres as a delivery system (Es GEN@DNA) with appropriate drug release properties. The prepared Es GEN@DNA nanospheres were characterized using UV-visible spectroscopy, X-ray diffraction analysis (XRD), transmission electron microscope (TEM), zeta potential and stability test. The results revealed that GEN was successfully encapsulate in situ during the DNA electrospherization (Es GEN@DNA) as core shell like structure (Core: GEN and Shell: DNA) with a wonderful stability against time. Furthermore, the drug encapsulation % was studied. In addition, the drug release efficiency of Es GEN@DNA was recorded and theoretically visualized to understand the mechanism and kinetics of GEN drug release. %Encapsulation of GEN within DNA nanospheres was found to be 89.62%. Es GEN@DNA release profile explored that the well entrapped GEN within the DNA nanospheres could be a promising for sustained drug release. Besides, we overcome the dilemma of using a fractal or fractional kinetics model by introducing a general fractional kinetic equation that involves a time-dependent rate coefficient, which introduced that the solution of the fractional kinetic model is capable of fitting the release data profiles of free GEN and Es GEN@DNA.

Rheumatoid arthritis (RA) is a chronic autoimmune disease with multiple morbidity burdens. Early diagnosis of RA is the main key in management and prevention of disease complications. Much research nowadays is looking for a serological marker with high accuracy in diagnosis of early RA cases. Our aim in this study was to evaluate the role of anti-mutated citrullinated vimentin (anti-MCV) antibodies in the early diagnosis of RA. In addition to compare its diagnostic sensitivity and specificity with anti-cyclic citrullinated peptide antibodies (anti-CCP) and RF antibodies in early versus established RA patients. This prospective cross-sectional study included 80 participants: 40 RA patients (20 early RA patients and 20 established RA patients), 20 patients with other rheumatic diseases (as a disease control group), and 20 apparently healthy participants as normal controls. All participants underwent history taking, clinical examination (general, articular assessment and calculation of disease activity score (DAS28-ESR)) for RA patients, radiological and laboratory investigations (RF, anti-CCP2 and anti-MCV antibodies measurements by ELISA technique). The results showed that the mean values of anti-CCP2 and anti-MCV were significantly increased in RA cases compared to the control groups (p=0.00 and p=0.01, respectively). Anti-MCV had sensitivity and specificity of 63% and 83%, respectively for diagnosing of early RA at area under curve of 0.80 compared to sensitivity and specificity 37% and 100%, respectively for anti-CCP2. Also, both (anti-CCP2 and anti-MCV) had positive significant correlations with ESR (p0.001 and p=0.02, respectively), CRP (p=0.01 and p=0.02, respectively) and DAS 28 (p0.001 for both). In conclusion, our data indicated that anti-MCV antibodies may represent a valuable marker for diagnosis of early RA cases.

Globally, root rot disease of tomato plants caused by Sclerotium rolfsii is a severe disease leading to the death of infected plants. The effect of some commercial antiseptics and disinfectant agents, such as chloroxylenol (10%), phenic (10%) and formulated phenol (7%) on the control of root rot pathogen and its impact on growth and chemical constituents of tomato seedlings cv. Castle Rock were investigated in vitro and in vivo. The antifungal activity was measured in vitro following the poisoned food technique at different concentrations of 1000, 2000, 3000 and 4000 µL/L. Disinfectant agents and atrio (80%) were tested in vivo by soaking 20-day-old tomato seedlings in four concentrations of 125, 250, 500 and 1000 µL/100 mL water for 5 min and thereafter planting in soil infested by S. rolfsii. Fresh and dry weight, shoot and root length, and chemical constituents of tomato seedlings infected by S. rolfsii were investigated at 35 days after planting (DAP). Experimental results indicated that chloroxylenol (10%) was the most effective on fungus in vitro, recorded an effective concentration (EC50 = 1347.74 µL/L) followed by phenic (10%) (EC50 = 1370.52 µL/L) and formulated phenol (7%) (EC50 = 1553.59 µL/L). In vivo, atrio (80%) and disinfectant agents at different concentrations significantly (p ≤ 0.05) reduced disease incidence, increased shoot and root lengths and increased dry and fresh weight. Additionally, it significantly increased chlorophyll a, chlorophyll b, total carotenoids, total carbohydrates, total proteins, and total phenols. The highest reduction of root rot incidence and increase tomato growth parameters, as well as chemical compositions, were recorded on tomato seedlings treated with atrio (80%) as well as formulated phenol (7%) at different concentrations, followed by chloroxylenol (10%) at 125 and 250 µL/100 mL, whereas phenic (10%) was found to be the least effective treatment. Therefore, the application of formulated phenol (7%) could be commercially used to control tomato root rot diseases and increase the quality and quantity of tomato plants since it is promising against the pathogen, safe, and less expensive than fungicides.

Background Cerebral small vessel disease (SVD) is associated with acute events such as lacunar and hemorrhagic strokes, or chronic events such as cognitive deficit in the form of subcortical dementia, mood deficit in the form of late onset depression, sphincteric affection, and gait apraxia. Under conditions of moderate blood flow deficit, the inability of sclerotic vessels to dilate due to impairment of the cerebral autoregulation, renders the periventricular white matter seriously ischemic. Therefore, it is important to detect the implications of cerebral large artery disease on the severity of SVD, and the ability of transcranial duplex (TCD) to evaluate it in people at risk. Methods Fifty lacunar stroke patients were recruited, and evaluated using MRI brain to assess SVD score, carotid duplex and TCD to assess extracranial and intracranial stenoses, respectively. Results Both intracranial and extracranial stenoses showed significant relation to the severity of cerebral SVD. Moreover, there were significant relation between intracranial stenosis and presence of lacuna and EPVS. Conclusion Cerebral large artery disease contributes to the pathogenesis and severity of cerebral SVD. Therefore, TCD may be a useful tool for the prediction of occurrence of cerebral SVD in high-risk individuals, especially hypertensives.

Background Using of nanoparticles in various aspects of life including agriculture, medicine and industry is very crucial. One of the important source for Fe nutrition in plants is Iron oxide nanoparticles (Fe3O4NPs) due to its efficiency in releasing under different pH range. Thus, in the Model Farm of National Research Centre Egypt at El Tour South Sinai, a field experiment was carried out, to study the effect of different concentration of Fe3O4NPs (0, 20, 40, 60 ppm) on the physiological parameters and the nutritive value of Moringa under saline condition. Results The obtained results indicate that foliar spraying of Fe3O4NPs significantly promote growth (plant height, branches leaves number per plant, leaf area, stem diameter and biomass). Foliar treatment also increased photosynthetic pigments (chlo.a, chlo b, chlo a/b and carotenoids) and indole acetic acid (IAA) contents comparing with control. Hydrogen peroxide and lipid peroxidation contents of Moringa oleifera leaves were decreased significantly as compared with control plant. The maximum activities of antioxidant enzymes Peroxidase (POX), poly phenol oxidase (PPO), super oxide dismutase (SOD) and nitrate reductase (NR) were observed in plants treated with 40 ppm. Different concentrations of Fe3O4NPs increased significantly crude protein, crude fiber and ash percentages as well as, some nutrient contents of moringa leaves (N, P, K and K/Na) compared with untreated control plants, meanwhile decreased Na contents. Conclusion Treatment of Moringa oleifera plant with Fe3O4NPs at different concentrations greatly decrease the harmful effect of salinity on growth by its promotive role on different studied biochemical and physiological aspects.

Background: Pressure ulcers affect a large scale of elderly people. Despite advances in technology, the rate continues to increase. This increases morbidity, health care costs and mortality. Aim: searching for risk factors of pressure ulcers in elderly patients admitted to acute geriatric care units. Methods: A case control study was conducted recruiting100 elderly male and female patients above the age of six ty. History and physical examination were done, as well as nutritional and functional assessment. Nursing care was assessed using a checklist and Braden scale was done as a risk assessment scale for pressure ulcers. Blood samples were collected for laboratory measures. Results: pressure ulcers result from contribution of many factors as cardiovascular and cerebrovascular diseases, urinary incontinence, poor nutrition, immobility. Patients free from pressure ulcers had better nursing care with more frequent repositioning and use of air mattress (in more than 80%, compared use of air mattress in about 40% only of patients acquiring pressure ulcers). Braden scale was highly sensitive for the occurrence of pressure ulcers (cutoff point ≤ 17) with 77.4% positive predictive value and 97.3% negative predictive value. Conclusion: Pressure ulcers are multifactorial in origin. Nursing care is of utmost importance for their prevention. Braden scale should be applied to elderly pa tients upon admission to hospitals

Anemia can be defined as a state of inadequate O2 delivery to tissues due to reduction of the O2 carrying capacity of blood practically, anemia is diagnosed when there is reduction in the hemoglobin concentration or haematocrit level below normal values. Iron deficiency anemia is a major public health problem among women of reproductive age in most of developing countries approximately 50% of pregnant women and 35% of non pregnant women are anemic. These nutritional disorders, has profound negative effects on pregnancy outcome, health of mothers and babies. The objective of this study: Is to evaluate the maternal anemia during pregnancy as a risk factor for fetal growth restriction. The study was done this study conducted on 120 pregnant women. Pregnant women with inclusion criteria: Singleton pregnancy, Gestational age more than 28 week and Hb level less than 10mg/dl. We exclude multiple pregnancy, Gestational age less than 28 week, Hb level more than 10 mg/dl and other medical disorder with pregnancy. The results of this study reveal the following: There was significant difference between group 1, 2, 3, 4 as regard to Hb level, (PCV), (MCV), (MCH), (MCHC), (Ht) and (CBC). The restriction of fatal growth was significant lower in sever anemic patient rather than mild or moderate anemic patient In conclusions: The result of this study shows that there was restriction of fatal growth was significantly high in sever anemic group than mild or moderate group also, incidence of low birth weight and risk of fatal morbidity or mortality was significantly high in sever anemic group than mild or moderate group.

Future fertility after ectopic pregnancy is dependent on several factors, including age, history of infertility, history of previous EP, tubal rupture, and contralateral tubal lesion. Thus, it seems reasonable to assess tubal patency following a treatment of an ectopic pregnancy in those women who are willing to have future pregnancy. Aimed to: Compare between tubal patency after methotrexate & laparoscopic salpingostomy. The study included 72 patients equally divided in number into 2 main groups. First group: (36cases) Tubal ectopic pregnancy treated by MXT therapy single or multiple doses. Single dose regimen (MTX 1.0 mg/kg or 50 mg/m2 i.m or multiple dose regimen (MTX 1.0 mg/kg i.m days (0,2,4,6). Second group: - (36 cases) Tubal ectopic pregnancy treated by laparoscopic salpingostomy. After 3 months, we used laparoscopy with administration of methylene blue (MB) as a marker to detect the tubal patency. As regard to tubal patency, of 31 cases (86.1%) from 36 cases that treated by MTX were patent and 5 cases (13.9 %) were blocked. On the other side 21 cases (58.3 %) from 36 cases that treated by laparoscopic salpingostomy were patent and 15 cases (41.7 %) were blocked). Conclusions:Methotrexate is better than laparoscopic salpingostomy in treating undisturbed tubal pregnancy.

Background: Stroke is one of the leading causes of death and long-term disability worldwide. Aim of Study: This study aimed to conduct the effect of selected exercise programe on quality of life in stroke patients. Material and Methods: Ten male patients with stroke enrolled in this study. A selected physical therapy program was provided for the selected group (GS) including: Passive Range of motion exercises for tightening muscles, Facilitation for weak muscles through: PNF technique, quick stretching. The Outcome Measures: The Arabic version of a short form dimension test (SF-36) used to assess the QOL Items of the SF-36 were divided into eight different domains. The control group was evaluated before and after intervention with a QOL questionnaire (SF36). Results: There was a revealed significant increase in physical functioning, energy/fatigue, emotional well-being, social functioning, pain and general health post treatment compared with that pretreatment. Also, there was no significant difference in role limitations due to physical health and role limitations due to emotional problem between pre and post treatment in the selected group. Conclusion: Selected physical program, significantly improves quality of life in patients with stroke.

Alkaline soils have fertility issues due to poor physical qualities, which have a negative impact on crop growth and output. Solidago is used in flower arrangements, bouquet filler, and traditional medicine. The possible biological fertilizers’ eco-friendly and cost-effective nature favours farmers because of the vital role in soil productivity and environmental sustainability. A field experiment was performed during two successive seasons to explore the effect of applying yeast extract (YE) at (0, 0.5, 1.0, and 1.5 g/L) and/or gibberellic acid (GA3) at (control, 100, 200, and 300 ppm) on the morpho-physiological parameters, macronutrients, and biochemical constituents of Solidago virgaurea. The results emphasize that YE (1.5 g/L) and/or GA3 (300 ppm) treatments show the highest significant increase in plant growth (i.e., plant height, no. of branches, fresh and dry weight of shoots); photosynthetic efficiency (i.e., chlorophyll (a), chlorophyll (b) and total carotenoids); macronutrient content (i.e., N, P, and K); and biochemical constituents (i.e., total soluble sugars, total phenolic, total flavonoids, and total glycosides). The study results recommend using YE and GA3 in combination at concentrations of 1.5 g/L and 300 ppm, respectively, to improve Solidago production sustainability under alkaline soil conditions.

Currently, the human being faces a contagious virus called SARS-CoV-2 with a high transmission ability, infection, and a relatively high mortality rate, especially for the elderly. All research interest is directed to finding a drug or vaccine to save millions of people’s lives; some have succeeded and applied now in some countries. Medicinal plants represent a green treasure that renders plentiful primary and secondary metabolites characterized by potent biological activities. Interestingly, the secondary metabolites, alkaloids, flavonoids, tannins, and terpenoids, own a broad antiviral history. The plant antiviral capabilities may be due to inhibition of viral attachment to/penetration of the host cell; inhibition of the virus replication; suppressing RNA replication; downregulation of viral proteins translation; interfering with virus non-structural/structural protein; downregulation of virus-induced pro-inflammatory/inflammatory signaling pathways in the host cell, and antioxidant activity thus abating the virus-induced oxidative stress effect. Presently, as we are trapped with a surly virus, there are attempts to exploit the power of nature. Therefore, this review’s main objective is to refer to some medicinal plants or their derivatives that limit the virus’s virulence or alleviate the symptoms associated with this virus. Objectives: In this work we presented a highly sensitive, selective and validated method for determination of tedizolid phosphate (TEDP) antibiotic, based on its native fluorescence in aqueous solution. Methods: The maximum fluorescence intensity was measured at 408 nm after excitation at 298 nm after optimization of all experimental conditions. Results: The measured fluorescence was directly proportional to the concentration of the drug over the range of 2-30 ng/mL with a limit of detection of 0.13 and limit of quantification of 0.44 ng/mL The method succeeded to determine TEDP in its pharmaceutical dosage form and in human plasma with mean % recovery of 100.49 ±1.32 and 99.40 ± 2.18 respectively. Conclusion: The developed data was found to be with a good agreement with a valid method. The method was validated according to ICH guidelines for determination of the drug in its pure form and dosage form and according to FDA Guidance for Industry, Bioanalytical Method Validation for determination of TEDP in human plasma.

Purpose Scaphotrapezium-trapezoid (STT) joint arthritis is one of the most common forms of wrist arthritis. Conservative management often involves corticosteroid injection. Despite this, there is a scarcity of literature on palpation-guided injection techniques for the STT joint. We aimed to determine a standardized palpation-guided injection method that is easily reproducible and poses minimal risk to local anatomic structures. Methods Six fresh-frozen cadaveric upper extremity specimens were tested. Access to the STT joint was attempted with dorsal, volar, and radial approaches. Fluoroscopy was used to confirm accurate placement within the joint. Needle placement was documented in relation to the surrounding soft tissue and bony landmarks were measured with a ruler, and the angle of the needle entry was recorded using a goniometer. The cadavers were carefully dissected to identify the surrounding neurovascular structures at risk of injury. Results To access the STT joint with the dorsal approach, the needle was angled at 90o and inserted one-third of the distance from the prominence of the base of the second metacarpal to Lister tubercle. No neurovascular structures were found in the immediate vicinity of the needle. For the volar approach, the needle was angled at 65o and inserted at the distal wrist crease, 1 cm ulnar to the radial border of the wrist, in line with the second metacarpal. The volar branch of the radial artery was at risk with this approach. For the radial approach, the needle was angled at 60o and inserted immediately dorsal to the extensor pollicis brevis tendon, midway between the radial styloid and the prominence of the thumb metacarpal base. The dorsal branch of the radial artery was at risk with this approach. Conclusions In a clinical setting where fluoroscopy or ultrasound is not readily available, the dorsal approach may allow for safe and accurate placement of the injectate into the STT joint. Recent strides in wire + arc additive manufacturing (WAAM) of aluminum components present unique chances for more enhancements in their properties. More studies were done by controlling manufacturing parameters, the wire properties, arc mode, and applying post-process treatment to regulate the microstructure and mechanical characteristics of the fabricated aluminum parts. In this paper, a wide range of previous late studies has been chosen to research the different attempts leading to an enhancement in the mechanical properties of aluminum components fabricated with WAAM. A discussion of the challenges of applying WAAM for aluminum alloys is also reviewed. The main purpose of this paper is to evolve a detailed understanding of the mechanical characteristics of the deposited aluminum components at various conditions.

Background Controversy exists regarding the appropriate level of sympathectomy for primary palmar hyperhidrosis (PH) as different levels are associated with variable postoperative patients’ satisfaction and potential complications. The aim of this study was to investigate the safety, efficacy, and outcome of T2-T3 thoracoscopic sympathectomy (TS) versus T4 TS in this age group. Results This prospective study included 32 patients (17 males and 15 females) with primary PH treated at the department of pediatric surgery, in our institution during the period from February 2019 to February 2020. Inclusion criteria included moderate and/or severe degrees of PH not responding to conservative measures. The patients were divided randomly into two groups: group I treated by T2-T3 TS and group II underwent only T4 TS. All patients were evaluated regarding operative details and postoperative outcome. Follow-up ranged from 6 to 24 months. Group I included 18 patients (14 operated on both sides and 4 operated on one side), and group II included 14 patients (11 operated on both sides and 3 operated on one side). The ages ranged between 5 and 18 years (mean 14.25 ± 3.14 years). The difference in mean age among both groups (14.5 versus 13.9 years) was not statistically significant. The mean operative time was significantly longer in group I (22.4 versus 17.2 min, p value 0.046). The hospital stay (1 day) was similar for both groups. Postoperative compensatory hyperhidrosis (CH) was more frequent in group I (n=7, 50% versus n=5, 45.5%), but the difference was not statistically significant. Postoperative over dryness occurred in 5 patients in group I (28.6%) and temporary Horner’s syndrome in one patient (7.14%). No over dryness or Horner’s syndrome occurred in any patient in group II. The QOL score has improved in both groups; the degree of improvements was better in group II. Conclusion Both T2-T3 TS and T4 TS are effective in treating primary palmar hyperhidrosis in children and adolescents. T4 TS is preferred than T2-3 TS due to less frequent postoperative complications and better patients’ satisfaction.

Al-Mg aluminum alloy thin-wall components were deposited using wire + arc additive manufacturing (WAAM). Gas tungsten arc welding (GTAW) was adopted using ER5356 wire as the filler metal to fabricate components. Three travel speeds (TS) of 200, 320, and 500 mm/min were adopted to achieve changed heat input. Its effects on microstructure characteristics and tensile behavior were studied. The microstructure of alloys deposited with a TS of 500 mm/min contained fine and equiaxed grains compared with other deposited alloys. The TS 500 mm/min had a good assisting influence of controlling the forming quality and little pores, and cracks were observed. Alloys deposited at the TS 500 mm/min displayed higher mechanical properties with an ultimate tensile strength (UTS) of 245.5 MPa and elongation of 35.28%. Besides, changing the TS had a small effect on the microhardness with a value of 2.3Hv.

Introduction: Intralesional antigen immunotherapy represents a promising therapeutic approach for the treatment of different types of warts, particularly if multiple and/or recalcitrant.Aim: This work aimed to investigate the efficacy and safety of combined cryotherapy with intralesional purified protein derivative (PPD) immunotherapy in the treatment of multiple common warts.Methods: Fifty patients were randomly divided into two groups (25 patients each): Group A: receiving intralesional PPD immunotherapy, while group B: receiving cryotherapy plus intralesional PPD. Treatments were performed every 2 weeks for a maximum of four sessions. Photographs were taken at baseline and at each visit and clinical response was evaluated by the reduction in number and size of warts. Adverse effects were recorded.Results: There was a significant reduction in size and number of warts in both groups (PConclusion: Both intralesional PPD alone and combined cryotherapy with PPD are safe and effective in clearing of common warts. Cryotherapy may be a successful adjunct to intralesional PPD immunotherapy that helps in reducing the number of treatment sessions.

Climate change is an anthropogenic phenomenon that is alarming scientists and non-scientists alike. The emission of greenhouse gases is causing the temperature of the earth to rise and this increase is accompanied by a multitude of climate change-induced environmental exposures with potential health impacts. Tracking human exposure has been a major research interest of scientists worldwide. This has led to the development of exposome studies that examine internal and external individual exposures over their lifetime and correlate them to health. The monitoring of health has also benefited from significant technological advances in the field of “omics” technologies that analyze physiological changes on the nucleic acid, protein, and metabolism levels, among others. In this review, we discuss various climate change-induced environmental exposures and their potential health implications. We also highlight the potential integration of the technological advancements in the fields of exposome tracking, climate monitoring, and omics technologies shedding light on important questions that need to be answered.

Due to the lack of treatment options for the genetic disease primary hyperoxaluria (PH), including three subtypes PH1, PH2, and PH3, caused by accumulation of oxalate forming kidney stones, there is an urgent need for the development of a drug therapy aside from siRNA drug lumasiran for patients with PH1. After the recent success of drug therapies based on small interfering RNA (siRNA), nedosiran is currently being developed for the treatment of three types of PH as a siRNA-based modality. Through specific inhibition of lactate dehydrogenase enzyme, the key enzyme in biosynthesis of oxalate in liver, phase 1, 2, and 3 clinical trials of nedosiran have achieved the desired primary end point of reduction of urinary oxalate levels in patients with PH1. More PH2 and PH3 patients need to be tested for efficacy. It has also produced a favorable secondary end point on safety and toxicity in PH patients. In addition to common injection site reactions that resolved spontaneously, no severe nedosiran treatment-associated adverse events were reported. Based on the positive results in the clinical studies, nedosiran is a candidate siRNA drug to treat PH patients.
