Patologiile cronice invalidizante în timp , precum hipertensiunea arterială și diabetul zaharat tip 2 ramîn a fi în topul țintelor studiilor și abordărilor la nivel internațional. De aceea, oamenilor de știință , precum și personalului din cadrul bazelor practice nu le rămîne altceva decît să impună dezvoltarea si implementarea noilor preparate, care ar fi în stare să răspundă necesităților acestora. Un pas important ar fi sintetizarea moleculelor medicamentoase cu efecte pozitive în patologii concomitente, care de cele mai multe ori se întalnesc în perechi sau se complementează prin efectele adverse pe care le produc asupra organismului uman. Un astfel de preparat, care este la moment abordat prin prisma multitudinei efectelor benefice - este în plină companie de implementare în tratamentul a mai multor patologii cornice ca- diabet zaharat tip 2, insuficiența cardiacă cronică, insuficiența renală cronică si face parte din grupa inhibitorilor SGLT 2 este dapagliflozina.Efectul antihipertensiv al dapagliflozinei este relevant si impune o analiză bine documentată în această direcție

Utilizarea inhibitorilor sistemului renină-angiotensină-aldosteron la pacienții cu insuficiență cardiacă și fracție de ejecție redusă este asociată cu ameliorarea capacitatii funcționale, o creștere a calității si sperantei de viața percepute, o rata de deces cardiovascular scazuta și o reducere a numărului de spitalizări din exacerbarea insuficientei cardiace. Unele dintre aceste medicamente sunt, de asemenea, eficiente la pacienții cu boală renală cronică și albuminurie, precum și la pacienții cu hipertensiune arterială rezistentă. În ciuda numeroaselor beneficii, inhibitorii sistemului renină-angiotensină- aldosteron sau asociat cu creșterea incidenței hiperkalemiei, în special la pacienții cu boală cronică de rinichi concomitenta. Hiperkalemia este o tulburare electrolitică comună care este definită ca o creștere a concentrațiilor plasmatice de potasiu peste 5 mEq/L. Hiperkalemia s-a asociat cu cresterea ratei de respitalizari, aritmii maligne și o creștere a mortalității. Pe de altă parte, tratamentul optimizat cu inhibitorii sistemului renină-angiotensină-aldosteron necesită creșteri progresive ale dozei care pot determina, la rândul lor, o probabilitate mai mare de hiperkaliemie. Din toate aceste motive, aducem recomandarile ghidului de conduita și tratament pentru aceasta categorie de pacienti.

Datele prezentate fac parte componentă a reviului de literatură/rezultatele obținute în cadrul proiectului instituțional cu acronimul DIAFEREZIS. Hipertensiunea arterială (HTA) rezistentă la tratament reprezintă o povară semnificativă pentru sistemele de sănătate din întreaga lume datorită asocierii sale cu un risc cardiovascular extrem de ridicat și rămâne o dificultate terapeutică determinată de căi fiziopatologice complexe și insuficient abordate de terapia convențională. O serie de medicamente aflate în diferite stadii de investigare ar putea avea un impact semnificativ pe viitor asupra tratamentului HTA rezistente la tratament. Pe linga terapia farmacologică, studiile arată dovezi promițătoare în privința desimpatizării arterelor renale (DSAR) în gestionarea HTA rezistente. Utilizarea mai bună a terapiilor aprobate în prezent și integrarea terapiilor emergente sunt completări binevenite în arsenalul terapeutic antihipertensiv.

The main purpose of the article is to justify the most optimal strategy for improving product quality for agroindustrial enterprises in Ukraine. The object of study is the production of the agroindustrial complex of Ukraine. The scientific task involves forming a modern scientific approach to assessing the options for strategies to improve the quality of production of agroindustrial enterprises in Ukraine. The research methodology involves the application of the BOCR method, which consists of modelling with the establishment of expected positive outcomes, additional costs, new opportunities, and potential risks in the context of improving the financial performance of agroindustrial enterprises. Within the framework of the BOCR method, the expert analysis method was used to compare the objects of study through the auxiliary Delphi method. As a result of the conducted research, an information basis was created for the development and implementation of the most optimal strategy for improving product quality for agroindustrial enterprises in Ukraine. The innovation of the obtained results lies in the fact that, through a series of criteria, two potential strategies for improving product quality for agroindustrial enterprises in Ukraine were compared and the most optimal was chosen in the context of improving the financial condition. The prospects for further research should focus on applying the formed approach to comparing and selecting the optimal mechanisms for implementing these strategies.

Summary: Sensitivity to face-to-face stimuli configurations, which likely indicates interaction, seems to appear early in infants’ development, and recently a preference for face-to-face (vs. other spatial configurations) has been shown to occur in macaque monkeys. It is unknown, however, whether such a preference is acquired through experience or as an evolutionary-given biological predisposition. Here, we exploited a precocial social animal, the domestic chick, as a model system to address this question. Visually naive chicks were tested for their spontaneous preferences for face-to-face vs. back-to-back hen dyads of point-light displays depicting biological motion. We found that female chicks have a spontaneous preference for the facing interactive configuration. Males showed no preference, as expected due to the well-known low social motivation of males in this highly polygynous species. These findings support the idea of an innate and sex-dependent predisposition toward social and interacting stimuli in a vertebrate brain such as that of chicks.

Glioblastoma (GB) is the most aggressive and common primary malignant tumor of the brain and central nervous system. Without treatment, the average patient survival time is about six months, which can be extended to fifteen months with multimodal therapies. The chemoresistance observed in GB is, in part, attributed to the presence of a subpopulation of glioblastoma-like stem cells (GSCs) that are characterized by heightened tumorigenic capacity and chemoresistance. GSCs are situated in hypoxic tumor niches, where they sustain and promote the stem-like phenotype and have also been correlated with high chemoresistance. GSCs have the particularity of generating high levels of extracellular adenosine (ADO), which causes the activation of the A3 adenosine receptor (A3AR) with a consequent increase in the expression and activity of genes related to chemoresistance. Therefore, targeting its components is a promising alternative for treating GB. This analysis determined genes that were up- and downregulated due to A3AR blockades under both normoxic and hypoxic conditions. In addition, possible candidates associated with chemoresistance that were positively regulated by hypoxia and negatively regulated by A3AR blockades in the same condition were analyzed. We detected three potential candidate genes that were regulated by the A3AR antagonist MRS1220 under hypoxic conditions: LIMD1, TRIB2, and TGFB1. Finally, the selected markers were correlated with hypoxia-inducible genes and with the expression of adenosine-producing ectonucleotidases. In conclusion, we detected that hypoxic conditions generate extensive differential gene expression in GSCs, increasing the expression of genes associated with chemoresistance. Furthermore, we observed that MRS1220 could regulate the expression of LIMD1, TRIB2, and TGFB1, which are involved in chemoresistance and correlate with a poor prognosis, hypoxia, and purinergic signaling.

Datele prezentate fac parte componentă a reviului de literatură/rezultatele obținute în cadrul proiectului „Program de Stat 2020-2023” cu cifrul 20.80009.8007.04.Hipertensiunea arterială rezistentă a devenit un subiect controversat în ultima decadă. Conform Ghidului European a Societății de Cardiologie pentru managementul HTA, HTA rezistentă se defineşte ca lipsa controlului valorilor tensionale în pofida controlului factorilor de risc şi a complianţei bune la un tratament cu cel puţin trei antihipertensive în dozele maxime tolerate, dintre care unul este un diuretic, la pacienţii la care s-au exclus posibile cauze de hipertensiune arterială secundară. În poziţia oficială a AHA (Asociaţia Americană a Inimii) pe acest subiect, HTA rezistentă este considerată ca HTA necontrolată cu trei medicamente antihipertensive sau HTA controlată dar care necesită cel puţin patru medicamente.Cu toate că sistemul de sănătate deține un arsenal complex de preparate antihipertensive, nu se reușește corijarea valorilor la o buna parte dintre pacienți. O soluție ar fi dezvoltarea și studiul eficacității terapiei de activare baroreflexă

Beta-blocantele sunt considerate drept “piatra de temelie” în tratamentul multor boli cardiovasculare. Deși beta-blocantele au proprietăți comune “de clasă” ele diferă mult în funcție de caracteristicile farmacocinetice (liposolubilitate, metabolizare) și farmacodinamice (intensitatea blocarii receptorilor beta β, β1 – selectivitatea, activitatea simpatomimetica intrinseca, prezenta efectului alfa blocant etc.). Beta-blocantele au fost descoperite grație efectului antianginal în anii 1960 și în present sunt pe larg utilizate în insuficiența cardiacă, aritmii cardiace și cardiopatia ischemica. In acest articol, vor fi relatate efectele benefice ale beta-blocantelor în ateroscleroza, la fel ca și recomandarile date de ghidurile clinice in utilizarea acestor remedii. Surprinzator, este ca au trecut mai mult de 4 decenii de la descrierea primelor mecanisme ale beta-blocantelor, și, până present se mai descopera noi mecanisme celulare de acțiune, anterior neelucidate și continue sa deschidă noi perspective de utilizare a acestei clase de remedii.

Research on face perception has revealed highly specialized visual mechanisms such as configural processing, and provided markers of interindividual differences –including disease risks and alterations– in visuo-perceptual abilities that traffic in social cognition. Is face perception unique in degree or kind of mechanisms, and in its relevance for social cognition? Combining functional MRI and behavioral methods, we address the processing of an uncharted class of socially relevant stimuli: minimal social scenes involving configurations of two bodies spatially close and face-to-face as if interacting (hereafter, facing dyads). We report category-specific activity for facing (vs. non-facing) dyads in visual cortex. That activity shows face-like signatures of configural processing –i.e., stronger response to facing (vs. non-facing) dyads, and greater susceptibility to stimulus inversion for facing (vs. non-facing) dyads–, and is predicted by performance-based measures of configural processing in visual perception of body dyads. Moreover, we observe that the individual performance in body-dyad perception is reliable, stable-over-time and correlated with the individual social sensitivity, coarsely captured by the Autism-Spectrum Quotient. Further analyses clarify the relationship between single-body and body-dyad perception. We propose that facing dyads are processed through highly specialized mechanisms –and brain areas–, analogously to other biologically and socially relevant stimuli such as faces. Like face perception, facing-dyad perception can reveal basic (visual) processes that lay the foundations for understanding others, their relationships and interactions.

Controlul valorilor tensionale la pacienții cu HTA rezistentă rămâne a fi o problemă globală cu care se confruntă cardiologia contemporană. În acest context, desimpatizarea arterelor renale s-a poziționat recent ca o metodă minim invazivă eficace și inofensivă. Articolul dat prezintă datele unui studiu clinic original desfășurat în Departamentul Hipertensiuni arteriale, dedicat evaluării comparative a efectului antihipertensiv al DSAR versus tratament farmacologic. Datele obținute au demonstrat eficacitatea în reducerea valorilor tensionale atât a tratamentului farmacologic, cât și cel intervențional, DSAR însă având un impact net superior

Circa 50% din pacienţii cu semne şi simptome de insuficiență cardiacă cronică au funcţie sistolică normală sau aproape normală. Sunt cunoscute riscurile sporite de evenimente cardiovasculare şi mortalitate crescută la pacienţii cu insuficiență cardiacă şi fracția de ejecție preservată. Interesul sporit pentru insuficiența cardiacă cu fracția de ejecție preservată a condus la o explozie de cercetări clinice şi iniţierea mai multor trialuri în ultimii 20 de ani. În acest articol au fost elucidate date epidemiologice, fiziopatologice, trialuri existente şi studii clinice de perspectivă.

În articol se abordează unele aspecte fiziopatologice, clinice și de diagnostic al hiperaldosteronismului primar, care este o cauză a hipertensiunii arteriale secundare și reprezintă una din formele de HTA endocrine cu o prevalență crescută. Luând în considerație heterogenitatea fenotipică a hiperaldosteronismului primar sunt necesare investigații complexe pentru precizarea formei etiopatogenice și alegerea tratamentului corect. De o importanță vădită sunt testele bazale și dozările hormonale în screening-ul pacienților hipertensivi, deoarece hiperaldosteronismul corect tratat duce la vindecarea HTA sau la scăderea necesarului de medicamente hipotensoare

Denervarea arterelor renale reprezintă o metodă inovațională și de perspectivă în tratamentul hipertensiunii arteriale rezistente la tratament, care denotă aplicări tot mai vaste. În ultimii ani, această procedură induce interes nu numai ca o metodă de reducere suplimentară a tensiunii arteriale la pacienții care nu răspund la terapia standard cu antihipertensive, dar, de asemenea, și datorită efectelor sale pleiotrope asupra altor sindroame și patologii patogeneza cărora este asociată cu hiperactivitatea sistemului nervos simpatic. Articolul oferă o sinteză per ansamblu a studiilor clinice de denervare renală cu diabet zaharat, hipertrofi e ventriculară stângă, insuficiență cardiacă congestivă, fibrilație atrială și boala renală cronică. În pofida rezultatelor contradictorii ale studiilor de denervare renale la pacienții cu hipertensiune arterială, se atestă impactul pozitiv al procedurii asupra metabolismului glucidic, dinamica hipertrofiei ventriculare stângi și a fracției de ejecție. Există tot mai multe date despre siguranța denervării renale în boala cronică renală. În prezent, sunt în derulare diverse studii în cohorta de pacienți care implică denervarea renală.

This work is dedicated to a theme very important - content of availability in the civil and procedure execution. The principle of availability means that the parties can determine not only the existence process by triggering court proceedings and freedom to end the process before working a decision on the merits of the claim before the judge, but also the contents of the by framing on procedural and other participants in the process, phases and steps that the lawsuit could scroll through. As the holder of subjective material has the exclusive prerogative to exercise his right, and the procedural plan, the exclusive prerogative of the exercise of subjective procedural law is manifested, it representing the content of the principle of availability. Taking a general characterization of this notion are analyzed in this paper means procedural principle content is concerned and on which parties can exercise availability.

The embodied cognition hypothesis suggests that motor and premotor areas are automatically and necessarily involved in understanding action language, as word conceptual representations are embodied. This transcranial magnetic stimulation (TMS) study explores the role of the left primary motor cortex in action-verb processing. TMS-induced motor-evoked potentials from right-hand muscles were recorded as a measure of M1 activity, while participants were asked either to judge explicitly whether a verb was action-related (semantic task) or to decide on the number of syllables in a verb (syllabic task). TMS was applied in three different experiments at 170, 350 and 500 ms post-stimulus during both tasks to identify when the enhancement of M1 activity occurred during word processing. The delays between stimulus onset and magnetic stimulation were consistent with electrophysiological studies, suggesting that word recognition can be differentiated into early (within 200 ms) and late (within 400 ms) lexical-semantic stages, and post-conceptual stages. Reaction times and accuracy were recorded to measure the extent to which the participants' linguistic performance was affected by the interference of TMS with M1 activity. No enhancement of M1 activity specific for action verbs was found at 170 and 350 ms post-stimulus, when lexical-semantic processes are presumed to occur (Experiments 1-2). When TMS was applied at 500 ms post-stimulus (Experiment 3), processing action verbs, compared with non-action verbs, increased the M1-activity in the semantic task and decreased it in the syllabic task. This effect was specific for hand-action verbs and was not observed for action-verbs related to other body parts. Neither accuracy nor RTs were affected by TMS. These findings suggest that the lexical-semantic processing of action verbs does not automatically activate the M1. This area seems to be rather involved in post-conceptual processing that follows the retrieval of motor representations, its activity being modulated (facilitated or inhibited), in a top-down manner, by the specific demand of the task.

Structural and functional deficiencies that affect survival during acclimatization are present in micropropagated plants. The current study was carried out to determine the effect of three in vitro culture conditions (autotrophic, mixotrophic and moderate mixotrophic) on the morph-physiological and biochemical behavior of plantlets of sugar cane propagated in Temporary Immersion Bioreactors (BIT). Morphological and physiological indicators and key enzymes of carbon metabolism were evaluated. Plantlets that were subjected to autotrophic conditions did not survive due to the stress caused by the absence of sucrose in the culture medium. For this reason it could not be evaluated. The best results were obtained for the condition of moderate mixotrofical. In this case, the morphological variables, height of the plants, fresh weight and dry weight, responded positively. It was minimum the answer of the net photosynthesis in vitro and increased it notably from the second week of acclimatization. Plants with a mixotrophic growth showed very high values of transpiration and estomatic conductance at the beginning of the elongation and during the first week of acclimatization. This group of plants took a longer time than the rest in achieving the control of the loss of water and in reaching a functional stability. Plants that showed a bigger activity of pyruvate kinase were those coming from mixotrophic conditions. An inverse behavior was observed in the phosphoenolpyruvate carboxylase. The activities of syntase sucruse and the neutral invertase were superior for the mixotrofic conditions. Key words: autotrofic, enzymes, mixotrofic, photosynthesis

Se realizó un metanálisis con el objetivo de revisar todas las investigaciones realizadas en las diferentes ediciones de la Maestría de Medicina Natural y Tradicional de nuestra Universidad Médica “Carlos J Finlay” constituyendo nuestro universo 84 ensayos clínicos los cuales fueron objeto de revisión. El procedimiento metanalítico que utilizamos es el Diagnostic Tests para aglomerar los resultados de los estudios primarios, los datos recogidos de las variables se analizaron teniendo en cuenta las normas establecidas para la conducción de un ensayo clínico aleatorizado. En los primeros ensayos revisados no es tratado correctamente el planteamiento del problema destacándose negativamente la definición del problema objeto de estudio. Existe un adecuado uso de la Bibliografía. En el material y método encontramos problema en: tratamiento y descripción de eventos adversos, enmascaramiento del ensayo, y dosis/respuesta. En la totalidad se da salida a los objetivos en el análisis e interpretación de los resultados y en las conclusiones. Al calcular OR la mayoría alcanzó en la evaluación de los modelos y procedimientos estadísticos criterios de inclusiónA meta analysis was carried out with the objective of revising all the investigations carried out in the different editions of the Master's in degree Natural and Traditional Medicine in Medical University “Carlos J. Finlay” constituting our universe 84 clinical research which were revised. The procedure applied was the Diagnostic test put together the results of the primary studies, the data of the variables were analysed keeping in mind the established norms for the conduction of a randomised clinical research. In the first revised research the position of the problem is not treated correctly standing out the definition of the problem object of study negatively. An appropriate use of the Bibliography exists. In the material and method we find problem in: treatment and description of adverse events, although the tests were not clear enough the objectives are completely fulfilled in the analysis and interpretation of the results and in the conclusions. Se presenta el desarrollo de la psiquiatría comunitaria en Cuba, los factores que han contribuido a ello, y se propone cómo articular un nuevo paradigma basado en el modelo de atención comunitaria de salud mental, un modelo de organización de los servicios de salud mental más avanzado. Dicho modelo se materializa en los centros comunitarios de salud mental, con una base teórico y práctica que integra todos los aspectos que debe contemplar la atención integral al proceso salud-enfermedad, como forma de abordar este complejo sistema.

Se opina acerca de las características de las acciones del psiquiatra militar en casos de desastres y guerras. Teniendo en cuenta las características y preparación que demandan estas situaciones excepcionales, el psiquiatra militar debe estar dotado de una preparación, que le permita atender en breve tiempo, gran cantidad de pacientes. Esta base, no está en los currículos de preparación de tiempo de paz, lo cual establece una necesidad, de forma que se empleen los escenarios de desastres, naturales o no, a fin de obtener el entrenamiento necesario para alcanzar las habilidades.

Obiective. Studiile existente relevă că pancreatita acută biliară reprezintă 35-60% din totalitatea cazurilor de pancreatită acută. Concepțiile actuale de diagnostic și tratament expuse în literatură sunt diverse și, în unele cazuri, chiar contradictorii. Scopul studiului a fost de a analiza experienţa în diagnosticul și tratamentul pancreatitei acute biliare asociată cu icter colestatic. Material și metode. Materialul reprezintă un studiu clinic prospectiv, ce a cuprins pacienți tratați în IMSP Spitalul Clinic Municipal „Sfântul Arhanghel Mihail”, în perioada anilor 2006-2017. Rezultatele expuse includ evaluarea diagnostică și curativă a unui lot de 63 cazuri de pancreatită acută biliară asociată cu colemie colestatică, selectate din numărul total de 191 pacienți cu sindrom de icter mecanic, prezent în complicațiile benigne ale litiazei biliare. Rezultate și discuții. Analiza a prezentat prevalența sexului feminin – 76.1%. Comparativ cu alte complicații, pancreatita acută biliară apare mai precoce în litiaza biliară, iar în 6.4% – fără un anamnestic litiazic cunoscut. Diagnosticul clinic a prezentat un tablou de pancreatită acută combinată cu colemie colestatică și colangită variabilă. Investigațiile instrumentale au relatat: coledocul cu diametrul >0.8cm la toți pacienții, dimensiunile cefalopancreasului >3.5cm, coledocolitiaza (33 cazuri), absența calculilor în coledoc (30), papilodită stenozantă (16), papilită fără stenoză (15) și diverticul parapapilar (2). Diagnosticul de laborator a evidențiat elevarea enzimelor pancreatice în asociere cu colestază și citoliză hepatică variabilă. S-a practicat tratamentul secvențional cu soluționarea primară a simptomatologiei pancreatice și sindromului icteric, cu colecistectomizare ulterioară, care a fost efectuată în termeni adaptați fiecărui caz. Papilosfincterotomia endoscopică imediată a fost practicată în 4 cazuri de absență totală a fluxului biliar în duoden. În cazul prezenței fluxului biliar a fost instituit un tratament medicamentos complex pentru 4-5 zile, care în 32.1% s-a estimat cu o diminuare evidentă a simptomatologiei pancreatice și restabilirea fluxului biliar. La acești pacienți, colecistectomia laparoscopică a fost efectuată în termeni de 6-7 zile de la spitalizare. Alți 44 bolnavi au necesitat decompresie biliară cu sau fără litextracție (n=42), dintre care 16 papilosfincterotomii endoscopice „oarbe” și 2 duodenotomii suprapapilare în diverticulul parapapilar. În coledocolitiaza reziduală (4), papilosfincterotomia endoscopică a reprezentat rezolvarea definitivă. Din 39 pacienți ce au necesitat colecistectomizare, în 5 cazuri de lipsă a calculilor în coledoc s-a intervenit peste 24-48 ore după Papilosfincterotomia endoscopică 19 bolnavi suportând colecistectomie peste 3-4 zile după decompresie. În 16 cazuri de menținere a valorilor alaninaminotransferazei majorate, colecistectomia s-a practicat la diminuarea acestora, peste 18-26 zile de la decompresie, în perioada spitalizării repetate. Concluzii. Spitalizarea precoce, diagnosticul etiologic și un tratament argumentat în pancreatita acută biliară are o influență majoră în evitatea formelor severe de pancreatită și a complicațiilor acesteia. Diagnosticul în faza de debut a pancreatitei acute biliare este de preferință în următoarea consecutivitate: ultrasonografie complexă, fibroesofagogastroduodenoscopie, Rezonanță Magnetică Nucleară în regim Colangiografic. Tomografia Computerizată la această etapă prezintă informații net inferioare Rezonanței Magnetice Nucleare în regim Colangiografic. Colangiopancreatografia endoscopică are indicații limitate în etapa diagnostică a pancreatitei acute biliare, fiind de mare utilitate în etapa curativ-intervențională. Tratamentul conservativ constituie pilonul de bază în managementul inițial în cazul pancreatitei acute biliare cu flux biliar păstrat. Fluxul biliar absent este o indicație absolută pentru decompresie biliară, premergătoare tratamentului conservativ. O oportunitate de restabilire a fluxului biliar, în cazul calculilor mici și stenozelor oddiene, este papilosfincterotomia endoscopică „oarbă”, în cadrul căreia sunt mai puține șanse pentru o pancreatită acută biliară recurentă. Colecistectomia are indicaţii certe, neîntârziate în evitarea recurenței pancreatitei acute biliare, însă pentru selectarea momentului sunt necesare condiții speciale: restabilirea totală a fluxului biliar intern și regresarea completă a semnelor de inflamație pancreatică și hepatică.

BackgroundPreterm birth has been associated with increased risk of cardiovascular morbidity in adult life. We evaluated whether preterm birth is associated with deviating cardiac structure and function before school start. Methods and ResultsIn total, 176 children aged 6 years and born extremely preterm (EXPT; gestational age of 22–26 weeks) and 134 children born at term (control [CTRL]) were studied. We used echocardiography to assess left heart dimensions, geometry, and functions. Recording and off‐line analyses of echocardiographic images were performed by operators blinded to group belonging. Body size, blood pressure, and heart rate were also measured. Rates of family history of cardiovascular disease and sex distribution were similar in the EXPT and CTRL groups. Heart rate and systolic blood pressure did not differ, whereas diastolic blood pressure was slightly higher in EXPT than CTRL participants. After adjusting for body surface area, left ventricular length, width, and aortic valve annulus diameter were 3% to 5% smaller in EXPT than CTRL participants. Left ventricular longitudinal shortening and systolic tissue velocity were 7% to 11% lower, and transversal shortening fraction was 6% higher in EXPT than CTRL participants. The EXPT group also exhibited lower atrial emptying velocities than the CTRL group. Sex, fetal growth restriction, or a patent ductus arteriosus in the neonatal period did not contribute to cardiac dimensions or performance. ConclusionsSix‐year‐old children born extremely preterm exhibit a unique cardiac phenotype characterized by smaller left ventricles with altered systolic and diastolic functions than same‐aged children born at term.

Embodied theories hold that understanding what another person is doing requires the observer to map that action directly onto his or her own motor representation and simulate it internally. The human motor system may, thus, be endowed with a "mirror matching" device through which the same motor representation is activated, when the subject is either the performer or the observer of another's action ("self-other shared representation"). It is suggested that understanding action verbs relies upon the same mechanism; this implies that motor responses to these words are automatic and independent of the subject of the verb. In the current study, participants were requested to read silently and decide on the syntactic subject of action and nonaction verbs, presented in first (1P) or third (3P) person, while TMS was applied to the left hand primary motor cortex (M1). TMS-induced motor-evoked potentials were recorded from hand muscles as a measure of cortico-spinal excitability. Motor-evoked potentials increased for 1P, but not for 3P, action verbs or 1P and 3P nonaction verbs. We provide novel demonstration that the motor simulation is triggered "only" when the conceptual representation of a word integrates the action with the self as the agent of that action. This questions the core principle of "mirror matching" and opens to alternative interpretations of the relationship between conceptual and sensorimotor processes.

Coarctation of the aorta (CoA), a congenital narrowing of the proximal descending thoracic aorta, is a relatively common form of congenital heart disease. Untreated significant CoA has a major impact on morbidity and mortality. In the past 3 decades, transcatheter intervention (TCI) for CoA has evolved as an alternative to surgery. The authors report on all TCIs for CoA performed from 2000 to 2016 in 4 countries covering 25 million inhabitants, with a mean follow-up duration of 6.9 years. During the study period, 683 interventions were performed on 542 patients. The procedural success rate was 88%, with 9% considered partly successful. Complications at the intervention site occurred in 3.5% of interventions and at the access site in 3.5%. There was no in-hospital mortality. During follow-up, TCI for CoA reduced the presence of hypertension significantly from 73% to 34%, but despite this, many patients remained hypertensive and in need of continuous antihypertensive treatment. Moreover, 8% to 9% of patients needed aortic and/or aortic valve surgery during follow-up. TCI for CoA can be performed with a low risk for complications. Lifetime follow-up after TCI for CoA seems warranted. [Display omitted] [ABSTRACT FROM AUTHOR]
