4. Giant chondroid syringoma on the upper lip: a case report

Chondroid syringoma is a benign, rare, asymptomatic, slow-growing mixed tumor. Tumors larger than 3cm are considered giant and may be malignant. We present a 65-year-old man with a chronic, indolent, subcutaneous tumor with upper lip deformity. An excisional biopsy was performed under local anesthesia. Based on histopathological analysis, the resected lesion was identified as a chondroid syringoma of the apocrine type. There was no recurrence during the one-year follow-up after surgery. The occurrence of a large chondroid syringoma in the upper lip is rare. more...

The main task of HTTP Adaptive Streaming is to adapt video quality dynamically under variable network conditions. This is a key feature for multimedia delivery especially when quality of service cannot be granted network-wide and, e.g., throughput may suffer short term fluctuations. Hence, robust bitrate adaptation schemes become crucial in order to improve video quality. The objective, in this context, is to control the filling level of the playback buffer and maximize the quality of the video, while avoiding unnecessary video quality variations. In this paper we study bitrate adaptation algorithms based on Backward-Shifted Coding (BSC), a scalable video coding scheme able to greatly improve video quality. We design bitrate adaptation algorithms that balance video rate smoothness and high network capacity utilization, leveraging both on throughput-based and buffer-based adaptation mechanisms. Extensive simulations using synthetic and real-world video traffic traces show that the proposed scheme performs remarkably well even under challenging network conditions. more...

7. Backward-Shifted Strategies Based on SVC for HTTP Adaptive Video Streaming

Although HTTP-based video streaming can easily penetrate firewalls and profit from Web caches, the underlying TCP may introduce large delays in case of a sudden capacity loss. To avoid an interruption of the video stream in such cases we propose the Backward-Shifted Coding (BSC). Based on Scalable Video Coding (SVC), BSC adds a time-shifted layer of redundancy to the video stream such that future frames are downloaded at any instant. This pre-fetched content maintains a fluent video stream even under highly variant network conditions and leads to high Quality of Experience (QoE). We characterize this QoE gain by analyzing initial buffering time, re-buffering time and content resolution using the Ballot theorem. The probability generating functions of the playback interruption and of the initial buffering latency are provided in closed form. We further compute the quasi-stationary distribution of the video quality, in order to compute the average quality, as well as temporal variability in video quality. This paper proposes a new prediction process to explain and predict popularity evolution of YouTube videos. We exploit our recent study on the classification of YouTube videos in order to predict the evolution of videos' view-count. This classification allows to identify important factors of the observed popularity dynamics. Our experimental results show that our prediction process is able to reduce the average prediction errors compared to a state-of-the-art baseline model. 10. Computing Quality of Experience of Video Streaming in Network with Long-Range-Dependent Traffic

We take an analytical approach to study the Quality of user Experience (QoE) for video streaming applications. Our propose is to characterize buffer starvations for streaming video with Long-Range-Dependent (LRD) input traffic. Specifically we develop a new analytical framework to investigate Quality of user Experience (QoE) for streaming by considering a Markov Modulated Fluid Model (MMFM) that accurately approximates the Long Range Dependence (LRD) nature of network traffic. We drive the close-form expressions for calculating the distribution of starvation as well as start-up delay using partial differential equations (PDEs) and solve them using the Laplace Transform. We illustrate the results with the cases of the two-state Markov Modulated Fluid Model that is commonly used in multimedia applications. We compare our analytical model with simulation results using ns-3 under various operating parameters. We further adopt the model to analyze the effect of bitrate switching on the starvation probability and start-up delay. Social scientists have observed that human behavior in society can often be modeled as corresponding to a threshold type policy. A new behavior would propagate by a procedure in which an individual adopts the new behavior if the fraction of his neighbors or friends having adopted the new behavior exceeds some threshold. In this paper we study the question of whether the emergence of threshold policies may be modeled as a result of some rational process which would describe the behavior of non-cooperative rational members of some social network. We focus on situations in which individuals take the decision whether to access or not some content, based on the number of views that the content has. Our analysis aims at understanding not only the behavior of individuals, but also the way in which information about the quality of a given content can be deduced from view counts when only part of the viewers that access the content are informed about its quality. In this paper we present a game formulation for the behavior of individuals using a meanfield model: the number of individuals is approximated by a continuum of atomless players and for which the Wardrop equilibrium is the solution concept. We derive conditions on the problem's parameters that result indeed in the emergence of threshold equilibria policies. But we also identify some parameters in which other structures are obtained for the equilibrium behavior of individuals. more...

Our purpose in this paper is to characterize buffer starvations for streaming services. The buffer is modeled as an M/M/1 queue, plus the consideration of bursty arrivals. When the buffer is empty, the service restarts after a certain amount of packets are \emph{prefetched}. With this goal, we propose two approaches to obtain the \emph{exact distribution} of the number of buffer starvations, one of which is based on \emph{Ballot theorem}, and the other uses recursive equations. The Ballot theorem approach gives an explicit result. We extend this approach to the scenario with a constant playback rate using T\`{a}kacs Ballot theorem. The recursive approach, though not offering an explicit result, can obtain the distribution of starvations with non-independent and identically distributed (i.i.d.) arrival process in which an ON/OFF bursty arrival process is considered in this work. We further compute the starvation probability as a function of the amount of prefetched packets for a large number of files via a fluid analysis. Nucleoside analogues have been in clinical use since 1960s and they are still used as the first therapeutic option for several cancers and viral infections, due to their high therapeutic efficacy. However, their wide clinical acceptance has been limited due to their high toxicity and severe side effects to patients. Herein, we report on a nanocarrier system that delivers nucleosides analogues in a target-specific manner, making nucleoside-based therapeutics safer and with the possibility to be used in other human conditions. This system, named, Therapeutic OligonUCleotides Activated by Nucleases" (TOUCAN) combines: i) the recognition power of oligonucleotides as substrates, ii) the use of nucleases as enzymatic biomarkers and iii) the clinical efficacy of nucleoside analogues, in a single approach. As a proof-of-concept, we report on a TOUCAN that is activated by a specific nuclease produced by bacteria and releases a therapeutic nucleoside, floxuridine. We demonstrate, for the first time, that, by incorporating a therapeutic nucleoside analogue into oligonucleotide probes, we can specifically inhibit bacterial growth in cultures. In this study, Staphylococcus aureus was selected as the targeted bacteria and the TOUCAN strategy successfully inhibited its growth with minimal inhibitory concentration (MIC) values ranging from 0.62 to 40 mg/L across all tested strains. Moreover, our results indicate that the intravenous administration of TOUCANs at a dose of 20 mg/kg over a 24-h period is a highly effective method for treating bacterial infections in a mouse model of pyomyositis. Importantly, no signs of toxicity were observed in our in vitro and in vivo studies. This work can significantly impact the current management of bacterial infections, laying the grounds for the development of a different class of antibiotics. Stress drives neuroendocrine signals with detrimental effects to the intestinal homeostasis. The aim of this study was to evaluate the effect of stress on intestinal hypoxia response elements, including G protein-coupled receptor 41 (GPR41), GPR43, and hypoxia inducible factor (HIF)-1α. Groups of five BALB/c mice were subjected to acute (2 h per day) and chronic (2 h per day for 4 days) stress induced by restraint, and the results were compared to those of an unstressed control group. Whole mucosal samples from the colon were collected to evaluate the expression of GPR41, GPR43 and HIF-1α using Western blot chemiluminescent analysis. Compared to the control group, in the chronic stress group the expression of GPR43 (P = 0.0092) and HIF-1α (P < 0.0001) were significantly lower and the expression of GPR41 was similar (P = 0.9184); acute stress significantly increased HIF-1α expression (P = 0.0030) and increased GPR41 expression (P = 0.0937), without affecting GPR43 (P = 0.9184). These findings offer insights into the modulation of hypoxia response elements under stress conditions and their pharmacological implications for developing drugs that mitigate the effects of stress on intestinal homeostasis. Titanocene(III) chemistry has emerged in the last decades as an indispensable tool in C−C bond-forming reactions. In this context, pinacol and related reactions allow the stereoselective synthesis of vicinal diols. In this work, we present new applications of these reactions using as starting materials aromatic ketones. Simple and smooth reaction conditions have been developed and have been applied for inter- and intramolecular processes. - Abstract
Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk. more...

- Abstract
This present integrative project called "MANAGEMENT AUDIT TO THE MUNICIPAL DECENTRALIZED AUTONOMOUS GOVERNMENT OF THE CANTON GUACHAPALA, PERIOD 2020" has as main objective to evaluate the degree of compliance of the processes based on the criteria of efficiency, effectiveness and economy. For the development of the project, a methodology based on a descriptive, explanatory and exploratory research was applied, with a mixed approach both qualitative and quantitative which are presented in four chapters: In Chapter I, the background and generalities of the Municipal Decentralized Autonomous Government of Canton Guachapala are detailed, such as: background, description of the object of study, legal nature, legal basis and strategic planning. Chapter II, includes the conceptual framework that allows us to schematize the subject and expand the knowledge on performance auditing. Chapter III, presents the development of the performance audit phases, starting with the preliminary planning where a general knowledge of the entity was obtained, then in the specific planning the work programs were designed by component, which were developed in the execution and finally in the phase of communication of results a report containing comments, conclusions and recommendations was issued. The fibroblast activation protein (FAP) is heavily expressed in fibroblasts associated with the tumor microenvironment, while the prostate-specific membrane antigen (PSMA) is expressed in the neovasculature of malignant angiogenic processes. Previously, we reported that [177Lu]lutetium sesquioxide-iFAP/iPSMA nanoparticles ([177Lu]Lu−iFAP/iPSMA) inhibit HCT116 tumor progression in mice. Understanding the toxicity of [177Lu]Lu−iFAP/iPSMA in healthy tissues, as well as at the tissue and cellular level in pathological settings, is essential to demonstrate the nanosystem safety for treating patients. It is equally important to demonstrate that [177Lu]Lu−iFAP/iPSMA can be prepared under good manufacturing practices (GMP) with reproducible pharmaceutical-grade quality characteristics. This research aimed to prepare [177Lu]Lu−iFAP/iPSMA under GMP-compliant radiopharmaceutical processes and evaluate its toxicity in cell cultures and murine biological systems under pathological environments. [177Lu]Lu2O3 nanoparticles were formulated as radiocolloidal solutions with FAP and PSMA inhibitor ligands (iFAP and iPSMA), sodium citrate, and gelatin, followed by heating at 121 °C (103-kPa pressure) for 15 min. Three consecutive batches were manufactured. The final product was analyzed according to conventional pharmacopeial methods. The Lu content in the formulations was determined by X-ray fluorescence. [177Lu]Lu−iFAP/iPSMA performance in cancer cells was evaluated in vitro by immunofluorescence. Histopathological toxicity in healthy and tumor tissues was assessed in HCT116 tumor-bearing mice. Immunohistochemical assays were performed to corroborate FAP and PSMA tumor expression. Acute genotoxicity was evaluated using the micronuclei assay. The results showed that the batches manufactured under GMP conditions were reproducible. Radiocolloidal solutions were sterile and free of bacterial endotoxins, with radionuclidic and radiochemical purity greater than 99%. The lutetium content was 0.10 ± 0.02 mg/mL (0.9 GBq/mg). Significant inhibition of cell proliferation in vitro and in tumors was observed due to the accumulation of nanoparticles in the fibroblasts (FAP+) and neovasculature (PSMA+) of the tumor microenvironment. No histopathological damage was detected in healthy tissues. The data obtained in this research provide new evidence on the selective toxicity to malignant tumors and the absence of histological changes in healthy tissues after intravenous injection of [177Lu]Lu−iFAP/iPSMA in mammalian hosts. Actinium-225 and other alpha-particle-emitting radionuclides have shown high potential for cancer treatment. Reconstituted high-density lipoproteins (rHDL) specifically recognize the scavenger receptor B type I (SR-BI) overexpressed in several types of cancer cells. Furthermore, after rHDL-SR-BI recognition, the rHDL content is injected into the cell cytoplasm. This research aimed to prepare a targeted 225Ac-delivering nanosystem by encapsulating the radionuclide into rHDL nanoparticles. The synthesis of rHDL was performed in two steps using the microfluidic synthesis method for the subsequent encapsulation of 225Ac, previously complexed to a lipophilic molecule (225Ac-DOTA-benzene-p-SCN, CLog P = 3.42). The nanosystem (13 nm particle size) showed a radiochemical purity higher than 99% and stability in human serum. In vitro studies in HEP-G2 and PC-3 cancer cells (SR-BI positive) demonstrated that 225Ac was successfully internalized into the cytoplasm of cells, delivering high radiation doses to cell nuclei (107 Gy to PC-3 and 161 Gy to HEP-G2 nuclei at 24 h), resulting in a significant decrease in cell viability down to 3.22 ± 0.72% for the PC-3 and to 1.79 ± 0.23% for HEP-G2 at 192 h after 225Ac-rHDL treatment. After intratumoral 225Ac-rHDL administration in mice bearing HEP-G2 tumors, the biokinetic profile showed significant retention of radioactivity in the tumor masses (90.16 ± 2.52% of the injected activity), which generated ablative radiation doses (649 Gy/MBq). The results demonstrated adequate properties of rHDL as a stable carrier for selective deposition of 225Ac within cancer cells overexpressing SR-BI. 177 Lu2 O3 nanoparticles (<100 nm) functionalized with target-specific peptides. This research aimed to evaluate the dosimetry and therapeutic response of Lu2 O3 -iPSMA and Lu2 O3 -iFAP nanoparticles activated by neutron irradiation to demonstrate their potential for theranostic applications in nuclear medicine. The biokinetic behavior, radiation absorbed dose, and metabolic activity ([18 F]FDG/micro-PET, SUV) in preclinical tumor tissues (athymic mice), following treatment with177 Lu2 O3 -iPSMA,177 Lu2 O3 -iFAP or177 Lu2 O3 nanoparticles, were assessed. One patient with multiple colorectal liver metastases (PSMA-positive) received177 Lu2 O3 -iPSMA under a "compassionate use" protocol. It is acknowledged that antiviral immune response contributes to dengue immunopathogenesis. To identify immunological markers that distinguish dengue fever (DF) and dengue hemorrhagic fever (DHF), 113 patients with confirmed dengue infection were analyzed at 6 or 7 days after fever onset. Peripheral blood mononuclear cells (PBMC) were isolated, lymphocyte subsets and activation biomarkers were identified by flow cytometry, and differentiation of T helper (Th) lymphocytes was achieved by the relative expression analysis of T-bet (Th1), GATA-3 (Th2), ROR-γ (Th17), and FOXP-3 (T regulatory) transcription factors quantified by real-time PCR. CD8+, CD40L+, and CD45+ cells show higher numbers in DF compared to DHF patients, whereas CD4+, CD19+, and CD25+ cells show higher numbers in DHF than DF patients. High expression of GATA-3 accompanied by low expression of T-bet indicates predominance of Th2 response. In addition, higher expression of FOXP-3 and reduced functional cytotoxic T cells (CD8+perforin+) were observed in DHF patients. In further experiments, PBMC were stimulated ex vivo with dengue virus E, NS3, NS4, and NS5 peptides, and proliferating T cell subsets were determined. Lower proliferative responses to NS3 and NS4 peptides and reduced CD8+ cytotoxic T cells were observed in DHF patients. Our results suggest that immune response to dengue is dysregulated with predominance of CD4+ T cells, low activation of Th1 cells, and downregulation of the antiviral cytotoxic activity during severe dengue, likely induced by regulatory T cells. El presente plan de negocio está orientado a la creación de la pequeña empresa “Malta Cacique” dedicada a la producción y comercialización de malta elaborada a base de Cebada, con el carácter de innovar en una bebida natural, libre de químicos, con el propósito de brindar beneficios a la salud de los consumidores ya que posee propiedades de gran valor nutritivo, previene enfermedades y renueva nuestras energías. La empresa “Malta Cacique” será establecida geográficamente en el casco urbano de la ciudad de Jinotepe; la ubicación de la planta es de gran importancia para el proceso, pues debe permitir el fácil acceso de los insumos y la fácil distribución del producto a los puntos de venta. Para el desarrollo del presente documento se estructurará en cuatro planes fundamentales: mercadotecnia, gestión operativa, gestión organizativa y gestión financiera, en donde se establecieron las bases fundamentales de la empresa. En el plan de mercadotecnia se elaboró una investigación de mercado para lograr identificar de manera concreta los principales clientes del producto y como resultado de ello se ha obtenido un 92% de como porcentaje de aceptabilidad, por lo tanto, es un indicador positivo para su realización; también se hace la descripción de los aspectos fundamentales del producto recalcando aspectos importantes como: la marca, etiqueta, logotipo, envase, beneficios nutricionales, así como también identificar la competencia, precios, presentación, plaza, promociones por medio de las 4p. Para determinar la gestión operativa se describe la materia prima, la maquinaria que se utilizara en el proceso productivo, mano de obra, con el objetivo de hace que el proceso se realice de manera eficiente, garantizando así la optimización de los recursos, para ello se diseñó un flujo grama de procesos productivo. De igual manera se especifica el tipo de maquinaria a utilizar en la empresa, mediante una ficha técnica que garantiza una adecuada descripción de esta. Mediante la gestión organizativa se logró identificar la constitución de la pequeña empresa aprovechando los beneficios que otorga la ley 645 (ley de fomento y desarrollo ix de la Micro, Pequeña y mediana empresa), siendo esta constituida como personal natural ante las instancias correspondientes; así mismo se estableció la distribución de funciones, establecida a través de un organigrama funcional, así como también los requisitos de cada colaborador de la empresa para la puesta en marcha de la empresa. Artículo de investigación La etapa de posacuerdo que atraviesa Colombia luego de la firma del acuerdo de paz, lleva a que sea necesario plantearse una serie de programas y proyectos de reorganización en materias concretas, una de estas que se circunscribe concretamente a la compensación de los costos ambientales que dejo la guerra en Colombia. Este artículo de investigación, busca identificar los principales retos que tienen las entidades públicas del sector ambiental, y de esta manera formular una serie de recomendaciones que le permitan enfrentar los desafíos ambientales que surgirán en el marco del posacuerdo, esto teniendo en cuenta la importancia de la implementación de programas de desarrollo sostenible en el sector agrario y el uso adecuado de la tierra, situaciones. Que tienen un efecto positivo en el objetivo de lograr una paz estable y duradera.
