The rate of uric acid and HDL cholesterol – so called as uric acid to HDL ratio (UHR) has been shown to be elevated in inflammatory conditions diseases. We aimed to compare UHR and other laboratory parameters of the patients with HT to those values in healthy controls.

4. Kırma mercan (Pagellus erythrinus L., 1758)’da hipofiz-gonad histolojisi ve üreme ile ilgili hormonal durumun belirlenmesi

- Abstract
Bu çalışmada, Antalya Körfezi'nden yakalanan Sparidae familyasına ait Kırma Mercan balıklarının (Pagellus erythrinus L. 1758) hipofiz-gonad histolojisi ve üremedeki hormon değişimleri incelenmiştir. Kırma Mercan balıklarının protojinik (protogynous) hermafrodit olmaları nedeniyle gonad gelişiminde önce dişilik, sonra geçiş ve daha sonra da erkeklik karakterlerinin geliştiği toplam 15 gelişim evresi ışık mikroskopik incelemelerle tespit edilmiştir. Bu evreler olgunlaşmamış dişi, F1, F2, F3, F4, F5, F6 dişilik evreleri, T1, T2, T3 geçiş evreleri ve M1, M2, M3, M4, M5 erkeklik evreleri olarak belirlenmiştir. Transmission elektron mikroskobu P. erythrinus balıklarının gonad ve hipofiz yapılarının ince yapısını incelemek için kullanılmıştır. Oositlerin elektron mikroskopik incelemelerinde normal elektron densitedeki bu hücreler de nukleus, nukleolus, uzantılı sitoplazmik endoplazmik retikulum, mitokondri, golgi kompleksi ve çok sayıda salgı vezikülleri içerdiği görülmektedir. Kırma mercan balığında spermatozoa, küresel bir baş ve akrozom, koni şeklindeki bir boyun ve uzun bir kuyruğa sahiptir. Boyunda tek bir mitokondrisi bulunur. Bu çalışmada kırma mercan balıklarının hipofiz bezindeki hücre tiplerinin histokimyasal yöntemlerle belirlenmesi amaçlanmıştır. Organın infundibulum bölgesi, anterior ve posterior lobları oldukça belirgindir. Anterior lopta bölgesel ve hücresel farklılaşmalar farklı boyanma özellikleri şeklinde yansımıştır. Elde edilen örneklerden hipofiz bezinin düzgün dairesel yuvarlak biçimde olduğu görülmüştür.Özellikle rostral pars distalisinde yer alan asidofil hücreler oldukça yoğun yerleşimli olup, koyu boyanmış bölgeyi oluşturmaktadır. Proksimal pars distalisteki bazofilik hücre sayısı da fazladır. Pars intermedia sınırları çok belirgin olmayıp kolloid varlığı ile ayırt edilmektedir. Küçük asidofillerin ve büyük bazofillerin farklı şekilde boyanmaları, granüllerindeki hormon içeriğinin farklılığını yansıtmaktadır. Hipofizin elektron mikroskobik incelemelerinde bu hücrenin ince yapı özellikleri protein salgılayan endokrin hücreleriyle benzerlik göstermektedir. Hormon sentezi için gerekli iyi gelişmiş bir golgi kompleks ve membrana yakın yerleşimli çok sayıda yuvarlak yada oval ortası daha yoğun salgı vezikülleri bulunmaktadır. Oval ökromatik çekirdeğe sahiptir.Kan serumunda 17ß-estradiol (E2), testosteron (T) ve 11-ketotestosteron (11KT) seviyelerindeki değişiklikler Pagellus erythrinus'un yıllık üreme evresi sırasında ovaryum ve testis histolojisine göre vitellojenin konsantrasyonu (VTG), gonadosomatik indeks (GSI) değişimlerin ilişkileri incelendi. Mercan balığının histolojik gelişim evrelerine göre E2 seviyesinin en yüksek değere olgunluk evresi olan F5'te ulaştığı görülmektedir. Vitellojenin düzeylerinin GSI değerlerine paralel bir dalgalanma gösterdiği tespit edilmiştir. F1 evresinden F5 evresine kadar artış olduğu görülmektedir. Erkeklerde GSI değerleri gelişim evrelerine göre incelendiğinde M1 evresinden M3 evresine kadar doğrusal bir artma görülmektedir. En yüksek değere M3 (spermiasyon evresi)'de ulaştığı tespit edilmiştir. 11 KT hormonu spermiogenezis (M2) ve spermiasyon (M3) evrelerinde yüksek iken yumurtlama sonrası (M4) ve dinlenme (M5) evrelerinde düşüş göstermektedir. Erkek balıklarda T hormon değerlerinin spermiogenez (M2) ve spermiasyon (M3) evreleri esnasında önemli ölçüde artış gösterdiği tespit edilmiştir. Yumurtlama sonrası (M4) evresinde de bir azalma görülmektedir.Hipofizde üreme aktivitelerini başlatan salgı hücrelerinin, ışık ve elektron mikroskopik incelemeleri yanısıra gonad gelişiminin histolojik ve elektron mikroskopik incelemelerine paralel olarak, % GSI değerleri, E2, VTG, T, 11 KT hormon ölçümlerinden elde edilen sonuçlara göre sahillerimizde kırma mercan balıklarının Nisan sonundan-Eylül ayı başına kadar üremelerini gerçekleştirdikleri tespit edilmiştir.

OBJECTIVE The objective of this study was to compare Federation of Gynecology and Obstetrics (FIGO) 2009 and 2018 staging systems in patients with uterine cervical cancer. METHODS Medical records of 571 patients who were treated with adjuvant radiotherapy or radiochemotherapy between 2001 and 2018 were retrospectively reviewed. Differences in overall survival (OS) and progression-free survival (PFS) rates according to FIGO 2009 and FIGO 2018 staging systems were compared using the log-rank test. Cox regression model was used to identify independent prognostic factors for survival. RESULTS The median follow-up was 59 months. Five-year OS and PFS rates were 81.1% and 77.7%, respectively. Stage migration was recorded in 401 patients (70.2%) and the most remarkable stage migration was detected in stage I patients (60%). A total of 157 (27.5%) patients upstaged to stage IIIC disease. According to FIGO 2009, 5-year OS rates were 87.3%, 80.5% (p=0.076), and PFS rates were 82.8%, 77.5% (p=0.036) for stage IB1 and IB2, respectively. According to FIGO 2018, the 5-year OS rates for stage IB1, IB2, and IB3 were 89.8%, 87.1%, and 81.4% (p=0.310), and PFS rates were 90.2%, 80.5%, and 80.1% (p=0.189), respectively. Patients with ≥2 pelvic lymph node (LN) metastases had worse 5-year OS and PFS rates than patients with one metastasis (p=0.015 and p=0.006). Number of para-aortic LN metastasis and metastatic LN ratio (MLNR) were also correlated with 5-year OS and PFS. CONCLUSION Current FIGO staging system better discriminates patients with cervical cancer. However, integration of metastatic LN number and/or MLNR to the upcoming FIGO staging system may improve the prognostic value of the staging.

7. One simple question detects motion sickness susceptibility in migraine patients.

- Abstract
Objective: To find out if motion sickness susceptibility (MSS) of vestibular migraine (VM) patients and migraine only (MO) patients can be reliably detected with a single simple question: "Can you read while travelling in a car without getting motion sick?"., Method: Ninety-two definite VM and 58 MO patients and 74 healthy control (HC) subjects were asked about their MSS and about being able to read while riding in a car without becoming motion sick. A Motion Sickness Susceptibility Questionnaire (MSSQ-Short) including childhood (MSA), adulthood (MSB) and total (MST) parts was also administered to all participants. ROC curves of MSSQ-Short were prepared for "not being able to read in a car" as the gold standard., Results: Mean MSA scores were significantly higher in both VM and MO patients than in HCs (p < 0.001), but their scores were not significantly different (p = 0.171). Mean MSB and MST scores were significantly higher in VM than in MO patients (p < 0.001) and both VM and MO patients had significantly higher scores than HCs (p < 0.001). MSA scores were significantly higher than MSB scores in MO patients (p < 0.001). All sections of the questionnaire were associated with high area-under-curve values for MSS detected by the question about being able to read in a car without becoming motion sick., Conclusion: We propose that all migraine patients could have the same level of MSS in childhood but MO patients are able to compensate over years, but VM patients are not. 8. The Role of AMACR, CD10, TMPRSS2-ERG, and p27 Protein Expression Among Different Gleason Grades of Prostatic Adenocarcinoma on Needle Biopsy.

- Abstract
Objective: We examined the immunohistochemical expression of α-methyl acyl coenzyme A racemase (AMACR), CD10, TMPRSS2-ERG, and p27 in prostate adenocarcinoma tumors with different Gleason growth patterns and nonneoplastic prostate tissues to elucidate their roles in prostate carcinogenesis and histological aggressiveness., Material and Methods: In total, 80 archival core biopsy tissues diagnosed as prostate carcinoma, benign prostate hyperplasia, and atrophy cases were included. Immunoreactivity was evaluated by calculating the percentage of positively stained cells and the staining intensity. The mean values and test of significance were obtained using the Kruskal-Wallis test., Results: We obtained mostly intense immunoreactivity for AMACR, CD10, and ERG in adenocarcinomas. Although no significant differences were noted regarding AMACR and ERG expression, samples with Gleason growth patterns 3 and 5 tended to be strongly positive for ERG. Pattern 3 tumors exhibited the weakest positivity for CD10. The p27 expression was strong and diffuse in nonneoplastic prostate tissues. The loss of p27 expression was more frequent for pattern 5 tumors., Conclusion: ERG and AMACR were powerful markers to detect cancer. Especially, ERG is evident in early tumors may reflect its interaction with functional androgen receptors in cancer initiation. Pattern 5 tumors associated with stroma may have been exposed to more stromal substrates and upregulate their CD10 content as a protein degrader. We suggest that CD10 expression is associated with an increasing tumor grade. - Abstract
Objective: To investigate the effect of continuous lung ventilation with low tidal volume on oxidation parameters, such as thiol/disulphide homeostasis and albumin-adjusted ischemia-modified albumin (AAIMA), during cardiopulmonary bypass (CBP) in coronary artery bypass grafting (CABG)., Methods: Seventy-four patients who underwent elective CABG with CPB were included in the study. Blood samples were taken in the preoperative period, 10 minutes after CPB, and six and 24 hours postoperatively. Patients were assigned to the continuous ventilation group (Group 1, n=37) and the non-ventilated group (Group 2, n=37). The clinical characteristics, thiol/disulphide homeostasis, ischemia-modified albumin (IMA), and AAIMA levels of the patients were compared., Results: A significant difference was found between the groups regarding native thiol, total thiol, and IMA levels at the postoperative 24th hour (P=0.030, P=0.031, and P=0.004, respectively). There was no difference between the groups in terms of AAIMA. AAIMA levels returned to preoperative levels in Groups 1 and 2, at the 6th and 24th postoperative hours, respectively. Length of hospital stay was significantly shorter in Group 1 (P<0.001) than in Group 2., Conclusion: Continuous ventilation during CPB caused an increase in native and total thiol levels, an earlier return of AAIMA levels, and shorter hospital stay. Continuous ventilation may reduce the negative effects of CPB on myocardium (Table 2, Figure 1, and Reference 31).

10. Insomnia and Dysautonomia with Contactin-Associated Protein 2 and Leucine-Rich Glioma Inactivated Protein 1 Antibodies: A "Forme Fruste" of Morvan Syndrome?

- Abstract
Morvan syndrome (MoS) is typically characterized by neuromyotonia, sleep dysfunction, dysautonomia, and cognitive dysfunction. However, MoS patients with mild peripheral nerve hyperexcitability (PNH) or encephalopathy features have been described. A 46-year-old woman presented with a 2-month history of constipation, hyperhidrosis, and insomnia. Neurologic examination revealed muscle twitching and needle electromyography showed myokymic discharges in all limbs. No clinical or electrophysiological features of neuromyotonia were present. Although the patient denied any cognitive symptoms, neuropsychological assessment revealed executive dysfunction, while other cognitive domains were preserved. Cranial and spinal MRIs were unrevealing and tumor investigation proved negative. Polysomnography examination revealed total insomnia, which was partially reversed upon immune-modulatory therapy. Investigation of a broad panel of antibodies revealed serum leucine-rich glioma inactivated protein 1 and contactin-associated protein 2 antibodies. The features of this case indicate that the presentation of PNH syndromes may show significant variability and that MoS patients may not necessarily exhibit full-scale PNH and encephalopathy symptoms., Competing Interests: The authors declare that they have no conflicts of interest. 12. Management of the large upper eyelid defects with cutler-beard flap.

- Abstract
Background. To assess Cutler-Beard procedure results in patients after wide excision of malignant eyelid tumours. Materials and Methods. The records of two women and two men (four patients) referred to our clinic with eyelid mass complaints and malign eyelid tumour diagnosis according to the histopathological examination were examined retrospectively. Results. The patients were 60-73 years old and their average age was 66 ± 11.10. The follow-up period of the cases was 16 (6-25) months. Total excisional biopsy was applied to all patients and then Cutler-Beard full thickness lid reconstruction was done because of the wide localization of the tumour. The patients' diagnoses were consistent with basal cell carcinoma, sebaceous gland carcinoma, eyelid lymphoma, and squamous cell carcinoma. The patients' eyelids were separated from each other 1 month postoperatively with a second operation. Superior eyelid entropium and blepharochalasis were seen in one patient during followup. Conclusions. Cutler-Beard flap is a successful procedure for superior eyelid tumours accompanied by wide tissue loss. The long-time closure of the eyelids and the need for secondary surgery are the major disadvantages of this procedure. Our experience with this procedure will reveal better results with large case series.

13. Accelerated hypertension after venlafaxine usage.

- Abstract
Venlafaxine is the first antidepressant that acts via inhibiting serotonin and noradrenaline reuptake. Hypertension is observed in doses exceeding 300 mg/day and is the most feared complication. We report a patient with accelerated hypertension after venlafaxine use observed at a dose of 150 mg/day. A 23-year-old patient with symptoms of insomnia, depression, anhedonia, fatigue admitted our clinic. Venlafaxine at a dose of 75 mg/day was initiated after he was diagnosed with major depressive disorder. After 5 months, venlafaxine dose was uptitrated to 150 mg/day due to inadequate response to drug. After using venlafaxine for ten months at the dose of 150 mg/day, he admitted our clinic with headache and epistaxis. He was hospitalized after his blood pressure was measured as 210/170 mmHg. No secondary causes for hypertension were found, and venlafaxine treatment was considered possible etiologic factor. After stopping venlafaxine treatment, his blood pressure was reverted back to normal limits. While mild elevation of blood pressure could be observed after venlafaxine treatment, this case shows that accelerated hypertension with a diastolic blood pressure rise above 120 mmHg could be observed at relatively low doses of venlafaxine. Close monitoring of blood pressure is necessary after initiation of treatment, as accelerated hypertension could cause endorgan damage with potentially catastrophic results.

14. Prevalence of mitral valve prolapse in residents living at moderately high altitude.

- Abstract
Objective: Prolapse of mitral valve leaflets is a frequent disorder and the most common cause of severe mitral regurgitation in western countries. However, little is known about the effects of altitude on mitral valve prolapse. We studied the prevalence and echocardiographic characteristics of mitral valve prolapse at moderately high altitude and sea level., Methods: A total of 936 consecutive subjects who were admitted to 2 study institutions at Kars, Turkey (1750 m) and Istanbul, Turkey (7 m) were enrolled in this study to determine prevalence of mitral valve prolapse. Demographic and 2-dimensional echocardiographic characteristics of participants were recorded., Results: Prevalence of mitral valve prolapse was found to be significantly higher in people living at moderate altitude compared with those living at sea level (6.2% vs 2.0%; P = .007). Overall echocardiographic features regarding valve thickness (4.1 ± 0.80 mm vs 3.6 ± 0.66 mm; P = 0.169), maximal valve prolapse (4.6 ± 2.08 mm vs 3.9 ± 0.91 mm; P = .093), and frequency of mitral regurgitation (89% vs 73%; P = .65) were similar between groups, although anterior valve prolapse was seen more frequently at moderate altitude (50% vs 11%; P = .056) and posterior leaflet prolapse was significantly more frequent at sea level (66% vs 10%; P = .002)., Conclusions: Mitral valve prolapse is more frequently observed at moderately high altitudes. - Abstract
Therapeutic effect of interferon-β (IFN-β) treatment has been associated with modulation of the balance between Th1, Th17, Th2 and regulatory T (Treg) cells, whereas the impact of disease modifying drugs on Th9-immunity in multiple sclerosis (MS) has not been studied. To investigate the short-term effects of IFN-β treatment on cytokines in MS, we determined serum levels of IL-17, IL-23, IL-10, IL-4, IFN-γ, IL-9 and TGF-β in relapsing remitting MS patients before and 2 months after IFN-β treatment by ELISA. MS patients showed increased IL-17, IL-23 and IL-4 levels and decreased IL-9 levels as compared to healthy controls. IFN-β treatment only reduced IL-17 and IL-23 levels, whereas the levels of other cytokines remained unchanged. IFN-β treatment appears to exert its earliest therapeutic effect on Th17-immunity.
