2. Microcatheter-assisted circumferential trabeculotomy in primary congenital glaucoma.

- Abstract
PURPOSE: To study circumferential trabeculotomy for congenital glaucoma using Glaucolight illuminated microcatheter. METHODS: This was a prospective, uncontrolled, consecutive interventional study of 25 eyes of 25 patients with primary congenital glaucoma (PCG) underwent circumferential trabeculotomy done with an illuminated microcatheter through a period of 18-24 months in ophthalmology department, faculty of medicine, Minia University, Egypt. Patients' data of 12 months' follow-up were recorded. The primary target was the mean intraocular pressure (IOP) study in which complete success was defined as an IOP ≤21 mm Hg and at least a 30% reduction without the use of antiglaucoma drugs and a qualified success when medications were used to reach this aim. The secondary target was studying the corneal diameter and cup disc ratio change. RESULTS: The mean IOP (mm Hg) was reduced from 27.28 ± 3.46 preoperatively to 17.50 ± 5.09 at the final postoperative visit (P < 0.001). The mean follow-up period length was (18 months). A qualified success rate was reached to 88%, complete success in 76% and failure in 12%. No serious surgical complications were observed. CONCLUSION: Circumferential trabeculotomy accomplished by an illuminated microcatheter achieved a significant IOP lowering with either qualified or complete success in most of eyes with no significant ocular complications and it can be the first option in pediatric PCG surgery. Purpose The purpose of this paper is to evaluate the efficacy, cosmetic results, and safety of transconjunctival tucking of the levator aponeurosis for correction of simple congenital ptosis. Patients and methods This was a prospective interventional noncomparative case-series study. This study was carried out at the Ophthalmology Department of Minia University Hospital. Thirty eyelids of 24 patients with simple congenital ptosis with fair to good levator muscle function (_5mm lid elevation) were subjected to transconjunctival tucking of levator aponeurosis. All patients were subjected to an assessment of history and a full ophthalmological examination. The degree of ptosis was evaluated using marginal reflex distance 1. Levator muscle function was evaluated while fixing the eyebrow. Preoperative and postoperative digital photographs were used for documentation. Results Anatomical success was achieved in 26 (86.7%) eyelids. Undercorrection was present in four (13.3%) eyelids. No case of overcorrection was encountered. Undercorrection was associated with more severe ptosis and less levator muscle function. Good cosmetic outcomes were obtained in the majority of cases. Apart from undercorrection, no significant postoperative complications occurred during the study. Conclusion Levator aponeurosis tucking using the posterior transconjunctival approach is safe and effective for correcting simple congenital blepharoptosis, with good cosmetic outcomes. This technique is especially useful for mild and moderate cases of congenital ptosis associated with fair to good levator muscle function. [ABSTRACT FROM AUTHOR]

4. Bicanalicular lacrimal intubation as a primary surgical treatment for nasolacrimal duct obstruction in children.

Purpose The purpose of this study was to evaluate the success of bicanalicular lacrimal intubation as a primary treatment in cases of epiphora caused by congenital nasolacrimal duct obstruction in children up to 6 years of age. Patients and methods A prospective surgical intervention case series study was carried out in the Ophthalmology Department of Minia University Hospital. Forty-six eyes of 43 children with epiphora (dated since birth or shortly after) were enroled in this study. A bicanalicular lacrimal silicon tube was introduced through the nasolacrimal duct to the nose. Results In this study, the primary lacrimal intubation was successful in 40 eyes out of 46 eyes (87%). For children aged 1-2 years, primary intubation was successful in 22 eyes out of 24 (91.6%), and for those aged 2-4 years it was successful in 12 eyes out of 14 (85.7%). On the other hand, for children aged between 4 and 6 years, the procedure was less successful: six eyes out of eight (75%). Improvement of symptoms occurred in two eyes out of four (50%) when the tubes were removed early, and this rate increased to 90.9% in 20 out of 22 eyes when the tubes were retained for up to 3-6 months and to 90% in 18 out of 20 eyes when the tubes were retained for up to 6-12 months. Conclusion Bicanalicular lacrimal intubation is a successful treatment for children with nasolacrimal duct obstruction, and the younger the age of the children the higher the success rate. Keratitis is a global health issue that claims the eye sight of millions of people every year. Dry eye, contact lens wearing and refractive surgeries are among the most common causes. The resistance rate among fluoroquinolone antibiotics is >30%. This study aims at formulating a newly synthesized ciprofloxacin derivative (2b) niosomes and Solulan C24-, sodium cholate- and deoxycholate-modified niosomes. The prepared niosomal dispersions were characterized macroscopically and microscopically (SEM) and by percentage entrapment efficiency, in vitro release and drug release kinetics. While the inclusion of Solulan C24 produced something discoidal-shaped with a larger diameter, both cholate and deoxycholate were unsuccessful in forming niosomes dispersions. Conventional niosomes and discomes (Solulan C24-modified niosomes) were selected for further investigation. A corneal ulcer model inoculated with colonies of Pseudomonas aeruginosa in rabbits was developed to evaluate the effectiveness of keratitis treatment of the 2b-loaded niosomes and 2b-loaded discomes compared with Ciprocin
® (ciprofloxacin) eye drops and control 2b suspension. The histological documentation and assessment of gene expression of the inflammatory markers (IL-6, IL1B, TNFα and NF-κB) indicated that both 2b niosomes and discomes were superior treatments and can be formulated at physiological pH 7.4 compatible with the ocular surface, compared to both 2b suspension and Ciprocin® eye drops.
