Wind turbine multidisciplinary design optimization is currently the focus of several investigations because it has showed potential in reducing the cost of energy. This design approach requires fast methods to evaluate wind turbine loads with a sufficiently high level of fidelity. This paper presents a method to estimate wind turbine fatigue damage suited for optimization design applications. The method utilizes a high-order linear wind turbine model. The model comprehends a detailed description of the wind turbine and the controller. The fatigue is computed with a spectral method applied to power spectral densities of wind turbine sensor responses to turbulent wind. In this paper, the model is validated both in time domain and frequency domain with a nonlinear aeroservoelastic model. The approach is compared quantitatively against fatigue damage obtained from the power spectra of time series evaluated with nonlinear aeroservoelastic simulations and qualitatively against rainflow counting. Results are presented for three cases: load evaluation at normal operation in the full wind speed range, load change evaluation due to two different controller tunings at normal operation at three different wind speeds above rated and load dependency on the number of turbulence seeds used for their evaluation. For the full-range normal operation, the maximum difference between the two frequency domain-based estimates of the tower base lateral fatigue moments is 36%, whereas the differences for the other sensors are less than 15%. For the load variation evaluation, the maximum difference of the tower base longitudinal bending moment variation is 22%. Such large difference occurs only when the change in controller tuning has a low effect on the loads. Furthermore, results show that loads evaluated with the presented method are less dependent on the turbulent wind realization; therefore, less turbulence seeds are required compared with time-domain simulations to remove the dependency on the wind realization used to estimate loads. Wind turbine resonant vibrations are investigated based on aeroelastic simulations both in frequency and time domain. The investigation focuses on three different aspects: the need of a precise modeling when a wind turbine is operating close to resonant conditions; the importance of estimating wind turbine loads also at low turbulence intensity wind conditions to identify the presence of resonances; and the wind turbine response because of external excitations. In the first analysis, three different wind turbine models are analysed with respect to the frequency and damping of the aeroelastic modes. Fatigue loads on the same models are then investigated with two different turbulence intensities to analyse the wind turbine response. In the second analysis, a wind turbine model is excited with an external force. This analysis helps in identifying the modes that might be excited, and therefore, the frequencies at which minimal excitation should be present during operations. The study shows that significant edgewise blade vibrations can occur on modern wind turbines even if the aeroelastic damping of the edgewise modes is positive. When operating close to resonant conditions, small differences in the modeling can have a large influence on the vibration level. The edgewise vibrations are less visible in high turbulent conditions. Using simulations with low-level turbulence intensity will ease this identification and could avoid a redesign. Furthermore, depending on the external excitation, different aeroelastic modes can be excited. Objective We used data from a national study of pregnant women with HIV to evaluate the prevalence of congenital abnormalities in newborns from women with HIV infection. Design Observational study. Setting University and hospital clinics. Population Pregnant women with HIV exposed to antiretroviral treatment at any time during pregnancy. Methods The total prevalence of birth defects was assessed on live births, stillbirths, and elective terminations for fetal anomaly. The associations between potentially predictive variables and the occurrence of birth defects were expressed as odds ratios ( ORs) with 95% confidence intervals (95% CIs) for exposed versus unexposed cases, calculated in univariate and multivariate logistic regression analyses. Main outcome measures Birth defects, defined according to the Antiretroviral Pregnancy Registry criteria. Results A total of 1257 pregnancies with exposure at any time to antiretroviral therapy were evaluated. Forty-two cases with major defects were observed. The total prevalence was 3.2% (95% CI 1.9-4.5) for exposure to any antiretroviral drug during the first trimester (23 cases with defects) and 3.4% (95% CI 1.9-4.9) for no antiretroviral exposure during the first trimester (19 cases). No associations were found between major birth defects and first-trimester exposure to any antiretroviral treatment ( OR 0.94, 95% CI 0.51-1.75), main drug classes (nucleoside reverse transcriptase inhibitors, OR 0.95, 95% CI 0.51-1.76; non-nucleoside reverse transcriptase inhibitors, OR 1.20, 95% CI 0.56-2.55; protease inhibitors, OR 0.92, 95% CI 0.43-1.95), and individual drugs, including efavirenz (prevalence for efavirenz, 2.5%). ABSTRACT This work is concerned with the design of wind turbine blades with bend-twist-to-feather coupling that self-react to wind fluctuations by reducing the angle of attack, thereby inducing a load mitigation effect. This behavior is obtained here by exploiting the orthotropic properties of composite materials by rotating the fibers away from the pitch axis. The first part of this study investigates the possible configurations for achieving bend-twist coupling. At first, fully coupled blades are designed by rotating the fibers for the whole blade span, and a best compromise solution is found to limit weight increase by rotations both in the spar caps and in the skin. Next, partially coupled blades are designed where fibers are rotated only on the outboard part of the blade, this way achieving good load mitigation capabilities together with weight savings. All blades are designed with a multilevel constrained optimization procedure, on the basis of combined cross-sectional, multibody aero-servo-elastic and three-dimensional finite element models. Finally, the best configuration of the passive coupled blade is combined with an active individual pitch controller. The synergistic use of passive and active load mitigation technologies is shown to allow for significant load reductions while limiting the increase in actuator duty cycle, thanks to the opposite effects on this performance metric of the passive and active control solutions. Objectives The aim of the study was to investigate the prevalence of and risk factors for hepatitis C or B virus (HCV or HBV) coinfection among HIV-infected pregnant women, and to investigate their immunological and virological characteristics and antiretroviral therapy use. Methods Information on HBV surface antigen (HBsAg) positivity and HCV antibody (anti-HCV) was collected retrospectively from the antenatal records of HIV-infected women enrolled in the European Collaborative Study and linked to prospectively collected data. Results Of 1050 women, 4.9% [95% confidence interval (CI) 3.6–6.3] were HBsAg positive and 12.3% (95% CI 10.4–14.4) had anti-HCV antibody. Women with an injecting drug use(r) (IDU) history had the highest HCV-seropositivity prevalence (28%; 95% CI 22.8–35.7). Risk factors for HCV seropositivity included IDU history [adjusted odds ratio (AOR) 2.92; 95% CI 1.86–4.58], age (for ≥35 years vs. <25 years, AOR 3.45; 95% CI 1.66–7.20) and HBsAg carriage (AOR 5.80; 95% CI 2.78–12.1). HBsAg positivity was associated with African origin (AOR 2.74; 95% CI 1.20–6.26) and HCV seropositivity (AOR 6.44; 95% CI 3.08–13.5). Highly active antiretroviral therapy (HAART) use was less likely in HIV/HCV-seropositive than in HIV-monoinfected women (AOR 0.34; 95% CI 0.20–0.58). HCV seropositivity was associated with a higher adjusted HIV RNA level (+0.28log10 HIV-1 RNA copies/mL vs. HIV-monoinfected women; P=0.03). HIV/HCV-seropositive women were twice as likely to have detectable HIV in the third trimester/delivery as HIV-monoinfected women (AOR 1.95; P=0.049). Conclusions Although HCV serostatus impacted on HAART use, the association between HCV seropositivity and uncontrolled HIV viraemia in late pregnancy was independent of HAART. [ABSTRACT FROM AUTHOR]

We used data from the main surveillance study of HIV and pregnancy in Italy to evaluate possible differences in pregnancy care and outcomes according to nationality. Among 960 women followed in 2001–06, 33.5% were of foreign nationality, mostly from African countries. Foreign women had lower rates of preconception counselling and planning of pregnancy. They had more frequently HIV diagnosed during pregnancy, with a later start of antiretroviral treatment and lower treatment rates at all trimesters but not when the entire pregnancy, including delivery, was considered. No differences were observed between the two groups in ultrasonography assessments, hospitalisations, AIDS events, intrauterine or neonatal deaths, and mode and complications of delivery. Foreign women had a slightly lower occurrence of preterm delivery and infants with low birthweight. The results indicate good standards of care and low rates of adverse outcomes in pregnant women with HIV in Italy, irrespective of nationality. Specific interventions, however, are needed to increase the rates of counselling and HIV testing before pregnancy in foreign women. [ABSTRACT FROM AUTHOR]
