Background: This review article discussed the use of bridging therapy with low‐molecular‐weight heparin (LMWH) in patients who undergo noncardiac surgery (NCS) after percutaneous coronary intervention (PCI). Hypotheses: Patients who undergo PCI are at an increased risk of thrombotic events due to their underlying cardiovascular disease. However, many of these patients may require NCS at some point in their lives, which poses a significant challenge for clinicians as they balance the risk of thrombotic events against the risk of bleeding associated with antithrombotic therapy. Results: This review evaluates the current evidence on the use of bridging therapy with LMWH in patients undergoing NCS after PCI, focusing on outcomes related to the efficacy and safety of antithrombotic therapy. The article also discusses the limitations of the current evidence and highlights areas where further research is needed to optimize the management of antithrombotic therapy in this patient population. Conclusion: The goal of this review was to provide clinicians with a comprehensive summary of the available evidence to guide clinical decision‐making and improve patient outcomes. The idea of sustainable leadership is becoming more popular with organizations. Although sustainable leadership and its work‐associated outcomes have been examined in previous literature, only a small number of studies highlighted the role of sustainable leadership style in fostering employee innovation, mediated through their creative self‐efficacy (CSE) in the context of Pakistan. The purpose of this study is to explore the relationship between sustainable leadership and employee innovation in software houses. In addition, we intend to study the mediating role of CSE in the relationship between sustainable leadership and employee innovation. Data were collected via email from 250 staff working in Pakistan's software houses. They replied to questions about their leader's sustainable leadership style, employee CSE, and innovation. There was a response rate of 64.1%. PLS‐SEM has been used to analyze data. Study findings indicate that the effect of sustainable leadership on workplace creativity is essential. The results also revealed that CSE mediates the relationship between sustainable leadership and employee innovation. Although this study expands our knowledge about the mediating role of CSE between sustainable leadership and employee innovation, the prospects for future research are still present. The cross‐sectional design of the study might not have been able to extract the true essence of the cause‐and‐effect relationship between sustainable leadership and employee innovation. This study represents the first‐ever attempt to explore the feasibility of region‐specific application of the theory of generations in the context of Pakistan. Building upon the Schuman and Scott's (1989) historical method of identification of the predominant events from the coming‐of‐age era of various age groups through the study of the collective memories of various age groups, in this study, primarily, a qualitative methodology was deployed across three (3) focus group discussions involving 50 academicians and industrial experts to ascertain the presence or otherwise of distinct workplace generations of Pakistan. The discussions were centred on participant's recollection of the socio‐economic, political and technologically significant events that happened during their coming‐of‐age era and the way it impacted the formation of unique characteristics of each generation. These events thus served as the generational pointers which led to the indication towards the presence of three distinct Pakistani generational cohorts as Pakistani Baby Boomer Generation, Pakistani Generation X, and Pakistani Millennials/Generation Y who are currently comprising the workforce of Punjab, which is the second‐largest and most developed province of the country. In the second paper of this series, it is anticipated that a larger quantitative investigation may well open the pathway to the generalization of these findings in support of the context‐specific application of the theory of generations. The generational profiling emanating from this study has strong implications for the HR practitioners concerning the design of generationally tailored HR policies such that the generational differences are mitigated through various diversity and inclusion initiatives. ABSTRACT Skin substitutes are heterogeneous biomaterials designed to accelerate wound healing through provision of replacement extracellular matrix. Despite growing evidence for their use in chronic wounds, the role of skin substitutes in acute wound management and their influence on fibrogenesis remains unclear. Skin substitute characteristics including biocompatibility, porosity, and elasticity strongly influence cellular behavior during wound healing. Thus, we hypothesize that structural and biomechanical variation between biomaterials may induce differential scar formation after cutaneous injury. The following human prospective cohort study was designed to investigate this premise. Four 5-mm full thickness punch biopsies were harvested from 50 volunteers. In all cases, site 1 healed by secondary intention, site 2 was treated with collagen-GAG scaffold (CG), and decellularised dermis (DCD) was applied to site 3 while tissue extracted from site 4 was replaced (autograft). Healing tissue was assessed weekly with optical coherence tomography (OCT), before being excised on days 7, 14, 21, or 28 depending on study group allocation for later histological and immunohistochemical evaluation. Extracted RNA was used in microarray analysis and polymerase chain reaction of highlighted genes. Autograft treatment resulted in minimal fibrosis confirmed immunohistochemically and with OCT through significantly lower collagen I levels ( p = 0.047 and 0.03) and reduced mean grayscale values ( p = 0.038 and 0.015), respectively. DCD developed intermediate scar formation with partial rete ridge reformation and reduced fasiculonodular fibrosis. It was uniquely associated with late up-regulation of matrix metalloproteinases 1 and 3, oncostatin M, and interleukin-10 ( p = 0.007, 0.04, 0.019, 0.019). Regenerated dermis was significantly thicker in DCD and autografts 28 days post-injury compared with control and CG samples ( p = 0.003 and <0.0001). In conclusion, variable fibrotic outcomes were observed in skin substitute-treated wounds with reduced scarring in autograft and DCD samples compared with controls. Chronic wounds, including diabetic and venous ulcers, represent disruption of normal healing processes resulting in a pathological state of nonhealing cutaneous inflammation. They place an increasingly significant economic burden on healthcare providers as their prevalence is rising in keeping with an aging population. Current treatment modalities are slow acting and resource intensive. Bioengineered skin substitutes from autogenic, allogenic, or xenogenic sources have emerged as a new and alternative therapeutic option. A range of such products is licensed for clinical use, which differ in terms of structure and cellular content. Placed directly onto a prepared wound bed, skin substitutes may stimulate or accelerate healing by promoting revascularization, cellular migration, and repopulation of wound fields through provision of an appropriate scaffold material to facilitate these processes. Products containing fetal or autologous cells also benefit from early release of bioactive molecules including growth factors and cytokines. To date, limited numbers of randomized controlled trials studying skin substitutes have been published but evidence from case series and case-control studies is encouraging. This review discusses chronic wound biology, the influence that skin substitutes can exert on this environment, the products currently available, and examines the evidence for their use in chronic wound management. Background/Aim: Maxillofacial injuries are among the commonest forms of body injuries. There are three divisions, namely, facial bone fractures, soft tissue injuries, and dentoalveolar injuries. Etiologies include motor vehicle accidents, assaults, falls, and sporting injuries. The aim of this study was to determine the profiles including the causes of maxillofacial injuries seen in an urban government hospital in the mainland of Penang State, Malaysia. Materials and methods: This was a cross-sectional study that recruited cases reported within a period of 1 year. The source population was maxillofacial injury patients presenting to the Oral and Maxillofacial Surgery Department of an urban hospital in the Penang Mainland, North Malaysia between May 2007 and May 2008. Cases of patients involved in accidents that occurred outside the reference vicinity were excluded. A case report form was developed and completed by the attending clinicians. Data were analyzed using version 12.0. Results: A total of 194 cases were studied, with the mean patient age being 27.8 (SD 15.20) years. The majority of patients were Malay men between 20 and 29 years of age. The main cause of injury was motorcycle accident (53.6%). The commonest injury (in isolation/combination with other injuries) involved the soft tissues (87.2%), dentoalveolar region (33.4%), and facial bones (23.9%). Laceration was the commonest soft tissue injury, and crown fracture was the most frequent dentoalveolar injury. The facial bone that was most highly involved in the injury was the zygoma. Subjects involved in motorcycle accidents had a significantly higher incidence of sustaining facial bone fractures. Conclusions: Motorcycle accidents were the commonest cause of maxillofacial injuries in the Penang Mainland, Malaysia. Most patients were young men. Hence, it is prudent to reinforce appropriate road safety and awareness interventions particularly focusing young male motorcyclists so as to reduce the risk of accidents. [ABSTRACT FROM AUTHOR]

This case describes surgical correction of unilateral coronoid hyperplasia in a 13-year-old boy due to trauma. Treatment included coronoidectomy by intraoral approach after the diagnosis was confirmed. Computed tomography scan and occipitomental view radiograph were utilized for investigation. The resected coronoid process was sent for histopathological study. The histopathological examination revealed decalcified sections showing osteochondroid hyperplasia at the expanded end. On clinical and radiographic review at 2 years postoperation, the patient was well, completely symptom free and had improved mouth opening. This case report demonstrates the management of sports injury related unilateral coronoid hyperplasia. Emphasis was also placed on postoperational rehabilitation to prevent any relapse. One hundred probands of vitiligo were studied for palmar dermatoglyphics, both qualitatively and quantitatively, and were compared with those of 100 phenotypically normal control subjects. Ulnar loop pattern was the most common digital pattern observed in both probands and controls. An increased incidence of whorls and arches in men and women probands, respectively, was found to be statistically significant when compared with those of controls. Simian crease and Sydney line also have been observed in the present study, which has not been previously reported. A statistically highly significant reduction has been observed in total finger ridge count (TFRC) and a-b ridge count in both sexes of vitiligo cases when compared with those of controls. Keloid disease ( KD) is a fibroproliferative disorder characterised partly by an altered extracellular matrix ( ECM) profile. In fetal scarring, hyaluronic acid ( HA) expression is increased, but is reduced in KD tissue compared with normal skin ( NS). The expression of Hyaluronan Synthase ( HAS) and hyaluronidase ( HYAL) in KD and NS tissue were investigated for the first time using a range of techniques. Hyaluronan synthase and HYAL mRNA expression were significantly increased in NS tissue compared with KD tissue ( P < 0.05). Immunohistological analysis of tissue indicated an accumulation of HAS and HYAL protein expression in KD compared with NS due to the thicker epidermis. No differences were observed in mRNA or protein expression in KD and NS fibroblasts. Reduced expression of HAS and HYAL may alter HA synthesis, degradation and accumulation in KD. Better understanding of the role of HA in KD may lead to novel therapeutic approaches to address the resulting ECM imbalance. [ABSTRACT FROM AUTHOR]

Objective: This study evaluates whether vitamin D receptor (VDR) gene variations are associated with premenopausal breast cancer in Pakistani cohorts. Methods: Genomic DNA was isolated from 228 breast cancer patients and 500 non‐cancer controls. Six polymorphic variants (rs11568820, rs4516035, rs2228570, rs1544410, rs7975232, rs731236) of the VDR gene were genotyped using PCR‐RFLP analysis. All statistical analysis was carried out on IBM‐SPSS 23 at p‐value <.05. Chi‐square test and odds ratios (ORs) along with 95% confidence interval (CIs) were applied to evaluate the relationship between VDR gene polymorphisms and breast cancer. Results: Results showed that the A/A genotype of EcoRV (OR = 2.125, 95% CI = 1.024 to 4.412) and the A/a genotype of Apa1 (OR = 6.094, 95%CI = 4.111 to 9.033) gene polymorphism had an increased risk of premenopausal breast cancer. No associations of the Bsm1 and Taq1 polymorphisms were observed in premenopausal women. Moreover, the Cdx2 GG (OR = 0.34, 95%CI = 0.192–0.602) genotype had a significant protective effect on breast cancer. However, strong LD was existed between Bsm1/Taq1 (D′ = 0.757, CI = 0.67–0.82) and Apa1/Taq1 (D′ = 0.695, CI = 0.6–0.77). Haplotype analysis showed no association between premenopausal breast cancer and VDR haplotypes. Conclusion: These analyses indicates that the VDR EcoRV A/A and Apa1 A/a genotypes may be risk factors for breast cancer development among premenopausal women. [ABSTRACT FROM AUTHOR]

Every individual in our technologically evolved world needs proper data security. The procedure of exchanging medical information is increasingly concerned with data privacy. Many techniques have been offered for preserving data security. These techniques use approaches such as k‐anonymity, l‐diversity, and others. However, such solutions are vulnerable to attribute disclosure, homogeneity, and background knowledge risks due to their syntactic nature. In this work, we describe a safe and secure architecture and semantic approach for data sharing that is based on blockchain, local differential privacy (LDP), and federated learning (FL). The proposed framework generates an atmosphere devoid of trust in which data owners are no longer required to have trust in the controllers. The FL models enable the whole network to decentralize its data‐driven learning. Interplanetary file system (IPFS) is used to provide data security in a distributed environment because each file in IPFS has a digital fingerprint that is computed using a cryptographic hash function on the file's whole contents. Due to the rigorous privacy guarantee, data owners no longer need to be worried about the security of their data. The proposed model's assessment parameters include latency, throughput, privacy, and accuracy. The data privacy of the proposed model is protected via LDP and FL, and its latency and throughput communication transactions on permissioned blockchain are calculated and compared with those of the benchmark model. Background: Early childhood caries poses a significant health issue in children under 6 years old. It is determined that Streptococcus mutans is a primary etiological agent, likely to be transferred through maternal contact. Objectives: To determine the association of maternal S. mutans counts with S. mutans counts in their children between 6 and 30 months of age, and to determine the maternal and child DMFT (decayed, missing, and filled teeth) indices. Material and Methods: A community‐based cross‐sectional study was conducted in Karachi, Pakistan. A sample of 193 dyads of mother–children (6–30 months of age) was selected via purposive sampling. Saliva samples of the dyads were collected to assess S. mutans count. Caries assessment was performed for both using the DMFT index. A pretested questionnaire was used. The association of bottle‐feeding, oral hygiene measures, and other factors with S. mutans counts in children were also explored. Zero‐inflated negative binomial regression model at a 5% level of significance was applied using STATA version 12.0. Results: Out of 193 children, 109 (56.47%) were males and 84 (43.52%) were females. The mean age of mothers and children was 29.4 ± 6.2 years and 19.54 ± 6.8 months, respectively. Maternal S. mutans counts were not statistically associated with child's S. mutans counts (Mean child's S. mutans count ratio: 1; 95% confidence interval [CI]: 1, 1.01; p =.882). Compared with children who were breastfed, S. mutans counts were higher in children who were bottle‐fed (mean S. mutans count ratio= 4.85 [95% CI: 1.53, 15.41], p =.007). Age of mother and present caries status of mothers was significantly associated with the child's S. mutans count. Conclusion: No association between maternal S. mutans and child S. mutans was observed. However, maternal age, children who were breastfed, children who did not use pacifiers, and children with mothers who did not have caries, exhibited low S. mutans counts in their saliva. Graphical processing unit (GPU) has proven a beneficial tool in handling computationally intensive algorithms, by introducing massive parallelism in the calculations. In this study, an effective and low‐cost fingerprint identification (FI) solution is proposed that can exploit the parallel computational power of GPU proficiently. It is achieved by mapping a generalised minutia neighbour‐based novel encoding and matching algorithm on low‐cost GPU technology. The proposed solution achieves high accuracy in comparison with two open source matchers and it is shown to be scalable by comparing matching performance on different GPUs. The proposed GPU implementation employs multithreading and loop unrolling, which minimises the use of nested loops and avoids sequential matching of encoded minutia features. After a thorough and careful designing of data structures, memory transfers and computations, a GPU‐based fingerprint matching system is developed. It achieves on average 50,196 fingerprint matches per second on a single GPU. As compared to the sequential central processing unit implementation, the proposed system achieves a speed up of around 92 times, while maintaining the accuracy. The proposed system with matcher integrated on GPU can be considered as a good, low‐cost, robust and efficient solution for large‐scale applications of automated FI systems. [ABSTRACT FROM AUTHOR]

This paper studies the problems associated with the construction of price indexes for commercial properties that could be used in the System of National Accounts (SNA). Property price indexes are required for the stocks of commercial properties in the Balance Sheets of the country. Related service price indexes for the land and structure input components of a commercial property are required in the Production Accounts of the country if the Multifactor Productivity of the Commercial Property Industry is calculated as part of the SNA. The paper reviews existing methods for constructing an overall Commercial Property Price Index (CPPI) and concludes that most methods are biased (due to their neglect of depreciation) and more importantly, not able to provide separate land and structure subindexes. Objectives The objective of this study was to determine the incidence of olfactory dysfunction in workers following head injury in the work place, to define its relationship to the site, severity of injury and direction of force. The demographics of head injured workers were also assessed to determine whether those with olfactory loss were more likely to have sustained a cochleovestibular injury. Design Retrospective case analysis. Setting Tertiary referral university hospital in Toronto, Ontario. Participants A total of 3438 consecutive patients referred from the Workplace Safety and Insurance Board ( WSIB) in the province of Ontario who sustained a work-related head injury were assessed between 1987 and 2014. Main outcome measures Olfactory and cochleovestibular dysfunction assessed by history, clinical examination and subjective and objective tests. Results Olfactory dysfunction ( OD) was identified in 413 of 3438 patients (12.0%) of which 321 were diagnosed with anosmia and 92 with hyposmia. In our series, injuries from a fall were the commonest cause for OD and a frontal or mid-face impact was more likely to result in OD than other regions ( P = 0.0002). A loss of consciousness ( LOC) of any duration correlated with OD. In those with olfactory dysfunction, an associated skull fracture occurred in 37.1% of patients and a CSF leak in 4.1%, which was significantly higher compared with those without OD(<0.0001). Patients with OD had a higher incidence of cochlear and vestibular loss (19.9% and 20.6%, respectively) compared with those without OD (14.3% and 17.1%, respectively). Conclusions Post-traumatic olfactory dysfunction is more likely to occur in patients who experienced a moderate to severe head injury, LOC and more likely to result from a frontal or mid-face blow to the skull. In this study polymer-matrix composite has been processed using autoclave and Quickstep techniques. A phenomenological approach was adopted for the optimization of cure cycle for Quickstep of an aerospace grade epoxy composite (Cycom 977-2A carbon/epoxy prepregs). Though there are reports concerning the processing and properties of materials using Quickstep technique, little work has been reported on the design of optimized cure cycle for Quickstep processing based on rheology, ultrasonic testing, and mechanical characterization. A step by step account is provided in this study, describing the introduction, balancing and combination of isothermal dwell periods and temperature ramps in order to control the resin viscosity. All the panels manufactured were qualitatively assessed using ultrasonic C-scan and afterwards fiber and void content, panel thickness, flexural strength, and interlaminar shear strength (ILSS) were assessed and compared with those obtained from traditional autoclave- and oven-cured laminates. An aerospace grade prepreg 977-2A carbon/epoxy composite was processed using traditional oven/autoclave and relatively new technique, Quickstep (QS). The manufacturer's recommended autoclave cure cycle was employed for oven/autoclave processing while due to greater flexibility to control the cure cycle temperature in order to manipulate the resin viscosity, several cure cycles were employed for Quickstep processing. Optimization methodology and effect of these cure schedules and their comparison with traditional oven/autoclave curing is presented in previously published article. However, in order to reduce the total cycle time of QSAD (QS-60) cure schedule, an attempt was made to facilitate the excess resin to squeeze out during QS-35 and QS-45 cure. This attempt was based on the incorporation of double vacuum bagging (DVB) technique into Quickstep processing. Statistical analysis was performed to investigate any effect of DVB in Quickstep processing and it was observed that incorporation of DVB in Quickstep processing is not found to be significantly effective, however, it is believed that small resin rich patches of QS-45 cured panels can be removed with controlled application of external pressure using DVB. The authors report a case of paraganglioma of the skull base presenting as nasal polyps. A 29-year-old patient presented with epistaxis and was found to have nasal polyps. The patient underwent a nasal polypectomy. After pathology showed an unusual appearance, the mass was subsequently excised endoscopically using radiofrequency coblation, and it was found to be originating from the skull base. The diagnosis was made using a combination of clinical findings, radiology, and histopathology examination. The purpose of this study was to evaluate the safety and efficacy of the use of radiofrequency coblation for endoscopic resection of intranasal and sinus tumors. A review was conducted of 15 adult patients with intranasal and or sinus tumors endoscopically treated with radio frequency coblation between November 2008 and November 2010 at St. John's Hospital at Livingston, a tertiary referral center that covers otolaryngology services for the southeast of Scotland. Fifteen patients with intranasal and sinus tumors were treated with transnasal endoscopic resection using radiofrequency coblation. The tumors included inverted papilloma (seven), paraganglioma (one), glomangiopericytoma (one), capillary hemangioma (one), hemangiopericytoma (one), juvenile angiofibroma (one), juvenile ossifying fibroma (one), oncocytic adenoma (one), and transitional cell carcinoma (one). We found that radiofrequency coblation is a useful and safe tool associated with minimal blood loss (<200 mL to 600 mL) in the resection of these tumors, and the average operating time was 1.67 hours. Radio frequency is a rapidly evolving technique and in the future will have an increasing role to play in the endoscopic resection of intranasal and sinus tumors. [ABSTRACT FROM AUTHOR]

Objective: Centrosome amplification as detected by gamma tubulin (GT) immunostaining is associated with genetic instability and tumor aggressiveness. We assessed GT for its ability to predict recurrence of squamous cell carcinoma of the larynx (SCCL). Study Design: Case series with chart review. Materials and Methods: Five micron sections of 35 archival SCCL samples were subjected to antigen retrieval and immunostaining with antibody to GT. The keratin antibody CK5 served as a positive control for antigen retrieval, and tonsillar tissue was used as a negative control. Results: Of the 35 tumors analyzed, 22 were associated with recurrence(R) and 13 were not (NR). Fourteen of the 22 R tumors, but 0 of 13 of the NR tumours had a GT staining score of 2+ or 3+ (P < 0.0002). GT was also related to recurrence in node-negative tumors (P < 0.006) but was unrelated to T stage (P = 0.726). Conclusions: GT staining appears to be a better predictor of tumor recurrence than T stage and also predicts recurrence in N0 tumors. [Copyright &y& Elsevier]

: While there is very little material in the Muslim world dealing directly with the topic of “theology and science”, one can still discuss the topic fruitfully. Beginning with the vibrant discussion in the area of “religion and science” one can infer what types of relationship are possible between “ kalam and science”. Most of the inferred positions do not see any positive relationship between kalam and science; and one of them would see such a relationship to be actually detrimental to kalam. In the work of Muhammad Iqbal, we have an example of a Muslim thinker who envisions the possibility of a mutually enriching exchange between kalam and modern science. Drawing upon the resources in the Qur'an and work done by Gerald Holton in the history of science, this article demonstrates that Iqbal's vision is scripturally and historically grounded. With the “discovery” of scanner data by statistical agencies and researchers comes a wealth of new information upon which price index calculations can be based. Old problems, such as the appearance and disappearance of goods over time, are likely to be an important feature of such data. However, given that scanner data includes the prices and quantities of the population of transactions we have more information than is traditionally available to deal with the new and disappearing goods problem. We adopt a recently developed approach using the Constant Elasticity of Substitution cost function to provide a detailed empirical analysis of the effects of new and disappearing goods for an Australian scanner data set of supermarket products. Our results indicate that the failure to account for new and disappearing goods in the cost-of-living index leads to a significant upward bias. [ABSTRACT FROM AUTHOR]
