Introduction: This discussion paper investigates workforce shortage of Australian sonographers through identifying educational responses to the shortage. Method: An ethnographic content analysis of insights into the sonographer workforce provided in the Australian Government Department of Employment occupational reports (2007‐2016) and current education models across providers of prequalification sonographer education and data correlation with Australian Sonographer Accreditation Registry, education provider correspondence, and website information were conducted. Results: Industry‐identified shortage factors related to prequalification education including sonographer quantity and suitability, education model and location, student admission, and skill and attribute training. Educational changes related to these factors were identified. Queensland demonstrated the greatest increase in sonographer (166%) and student (1000%) numbers (2007‐2016). Population‐weighted binomial ratios identified South Australia with the highest number of sonographers and students per head of population (2016). In 2016, sonographers graduated from the UG+1 Postgraduate Model (74.6%), Direct Entry Postgraduate Model (18%), and UG+1 Postgraduate Model with mandatory simulated skill training (7.4%). Discussion Sonographer and student supply increased, indicating growth in clinical training capacity (2007‐2016). Increased sonographer demand meant workforce shortage was relatively static. Educational response involved change to education models with characteristics related to shortage factors. The significant risk associated with fault reactivation often necessitates slip tendency analyses for effective risk assessment. However, such analyses are challenging, particularly in large areas with limited or absent reliable stress measurements and where the cost of extensive geomechanical analyses or simulations is prohibitive. In this paper, we propose a novel approach using a physics‐informed neural network that integrates stress orientation and satellite displacement observations in a top‐down multi‐scale framework to estimate two‐dimensional slip tendency analyses even in regions lacking comprehensive stress data. Our study demonstrates that velocities derived from a continental scale analysis, combined with reliable stress orientation averages, can effectively guide models at smaller scales to generate qualitative slip tendency maps. By offering customizable data selection and stress resolution options, this method presents a robust solution to address data scarcity issues, as exemplified through a case study of the South Australian Eyre Peninsula. The article reports on the discovery of the trunk contained a significant portion of the papers and photographs of Ursula Hope McConnel from 1888-1957, an Australian who was trained by A.R. Radcliffe-Brown at Sydney and Edward Sapir at Yale in Adelaide, South Australia. An approximately 450 field photographs are included in the trunk. It also contains hundreds of pages of typewritten and handwritten drafts of her publications, a number of unpublished drafts and public lectures.

Introduction: This paper explores the fate of South Australia's highly regarded Police Drug Diversion Initiative over the past 10 years. Methods: The paper uses historical and legal analysis, including publicly available data as to rates of drug use, arrests and diversions, media reports, legislation and Hansard debates. Results: In the mid‐2010s, amidst growing anxiety about levels of amphetamine‐type stimulants use in Adelaide, a coroner's report became the poster child for what was seen by the Liberal opposition as the excessive leniency and 'waste of resources' of the Police Drug Diversion Initiative. Despite being universally praised for its efficacy, the Liberal government, when elected, passed amendments to the legislation which 'capped' diversions at two. This ended the period in which diversions from criminal to social justice systems was mandatory for all persons charged with simple drug possession. But since then, the diversion rate in South Australia has plummeted in ways that cannot be explained merely because of these amendments. Discussion and Conclusions: The most highly regarded scheme for diversion of minor drug offenders to treatment is no longer effective. Indeed, the best scheme in the country may now be the worst. The essay draws attention to a catastrophe whose extent is not yet fully appreciated and suggests ways in which we can begin to explain why such a small change has had such dramatic and unforeseen consequences. Background: Medication errors are a leading cause of harm to patients in hospital that can be decreased by electronic medication management systems. Aims: This study determined the effects of electronic prescribing using the Enterprise Patient Administration System (EPAS) on prescribing errors and adverse drug reaction (ADR) documentation in South Australian public hospitals. Methods: A prospective structured medication chart audit was conducted before (in 2012) and after the implementation (in Dec 2014) of EPAS, comparing the paper‐based National Inpatient Medication Chart (NIMC) with prescribing. All inpatients were eligible for inclusion in the study. Prescribing order errors were determined by the percentage of medication orders that had unclear, illegal or unsafe prescriptions. The quality of ADR documentation was assessed by the percentage of patients with correct documentation. Results: The total number of prescribing errors before and after the implementation of EPAS was 67.7 and 2.8 per 100 orders, respectively. The frequency of prescribing order errors due to unclear, illegal and unsafe prescriptions decreased to almost zero with EPAS. Prescriptions classified as unsafe due to incorrect dose, route of administration or dosing frequency decreased from 5.8% to 0.03% of medication orders before versus after EPAS implementation (p < 0.0001). Completed ADR documentation was similar between the paper‐based NIMC and EPAS, but the percentage of patients with ADR alerts who required them was 100% in EPAS compared with 27% on the NIMC (p < 0.0001). Conclusions: E‐prescribing using EPAS significantly decreased prescribing errors and increased ADR alerts. Analyses of the prevalence of homelessness suggest homelessness is increasing in Australia and other countries. Yet, difficulties exist in obtaining an accurate picture of homelessness due to a dearth of robust data and inconsistent definitions. This study aimed to build a comprehensive descriptive profile of homelessness and associated health needs in Adelaide. Five data sources were analysed and compared to produce descriptive sociodemographic and health statistics. Across data sources, people experiencing homelessness had a high prevalence of poor health outcomes and service utilisation. Consistent with the international literature, high rates of physical and mental health conditions were reported, including depression, anxiety and dental problems. While there was variability in demographic data, Aboriginal and Torres Strait Islander peoples were consistently over‐represented. Analysing data from multiple sources provided a richer understanding of who is experiencing homelessness and their health needs, highlights it is not always necessary to collect new data to overcome dataset limitations and illustrates how data comparison can improve the use of existing data. The paper concludes with reflections on the challenges and potential of the methodology. Overall, the study shows analysing data from multiple sources can provide rich information to service providers and government departments to inform more nuanced and effective services. Issue addressed: Wellbeing SA, an independent South Australian government agency, developed a Wellbeing Index for South Australia across a 12‐month period in 2021–2022 to enable the measurement of wellbeing outcomes for the state. Methods: The development of the Index was a methodical process based on two key emphases—evidence‐based action and working in partnership. The steps included a scoping of existing wellbeing measurement frameworks and then working with a broad range of partner agencies to define the purpose, scope and structure for the Index and subsequently the content (indicators, their measures and the most robust data available). Results: A Wellbeing Index for South Australia was launched in June 2022 on the Wellbeing SA website. It is structured against domains of physical, mental, social/community, Aboriginal cultural wellbeing and the underpinning determinants of wellbeing. Conclusions: The Index communicates the importance and the breadth of wellbeing outcomes addressed by the various health promotion and policy actors in South Australia and provides a sound basis for monitoring the wellbeing of the community over time. So what?: Measuring wellbeing is identified by proponents of a Wellbeing Economy as one useful mechanism, alongside legislation, policy and funding, for shaping a broader definition of a successful society. The health promotion community, and their partners, have a role to play in promoting the importance of a broad definition of wellbeing, and in identifying robust and meaningful indicators. Despite the ongoing failure of 'one‐size‐fits‐all' policies in the realm of local government policymaking worldwide, such as municipal mergers, public policymakers across Australian local government, including South Australia (SA), remain committed to uniform policy prescriptions that do not take into account the characteristics of different categories of local authorities. Furthermore, although a substantial body of empirical work exists on municipal expenditure, the vast bulk of research effort has been directed at (a) determining whether council size by population systematically affects average costs and (b) ascertaining whether scale economies exist in specific categories of municipal service provision, especially in waste collection and water services. However, this empirical literature has largely ignored the question of whether the characteristics of specific categories of municipality systematically affect outlays, especially non‐discretionary environmental factors. This paper seeks to address this gap in the literature by examining empirically how council type influences the determinants of municipal expenditure in SA. We find that there is a significant difference in per household expenditures between the different categories of SA local authorities in terms of the various types of municipal outlays. Moreover, environmental factors have a differential impact on the various kinds of per household expenditures of each category of local councils. This paper investigates factors underlying the variation in per household expenditure across different kinds of local councils in South Australian local government. This paper determines the optimal capacity of solar photovoltaic (PV) and battery energy storage (BES) for a grid‐connected house based on an energy‐sharing mechanism. The grid‐connected house, also mentioned as house 1 where it is relevant, shares electricity with house 2 under a mutually agreed fixed energy price. The objective is to minimize the cost of electricity (COE) for house 1 while decreasing the electricity cost of house 2. Practical factors such as real data for solar insolation, electricity consumption, grid constraint, ambient temperature, electricity rate, and battery degradation are considered based on actual data. The developed methodology is examined by taking the actual load data of two houses in South Australia. Different scenarios of contract years between the houses are investigated to make it more practical in real life. Sensitivity analyses are conducted for the sharing of energy between the houses and by changing parameters like export power limitation, load of houses, and costs of PV and BES. Likewise, operational analysis is done for two days of summer and winter. It is found that when energy sharing is applied, the optimal design of the PV‐BES system will achieve lower COE for both houses. [ABSTRACT FROM AUTHOR]

Theories of vulnerability are most often seen in the anthropology of disaster studies, where socio‐economic and political inequalities produce environmental vulnerabilities, and the people situated in these locations are positioned as vulnerable and dependent 'Others'. Rather than reproduce vulnerability as a concept denoting weakness, this paper seeks to examine the generative capacities of vulnerability practised in parallel in ethnographic and community spaces. As a form of witnessing and participating in and out of differing social worlds, anthropology engages in different vulnerabilities with and between multiple actors. This paper examines how a community program working with families identified as 'disadvantaged' in South Australia strategically uses vulnerability as a productive resource and a practice of care. In theorising vulnerability through parallel practices in both ethnographic approaches and this community program, we argue that vulnerability can be leveraged away from negative welfare discourses towards alternative politics of radical care and social change. Introduction: In the past 30 years, 60% of South Australia's rural maternity units have closed. Evidence demonstrates midwifery models of care offer regional Australia sustainable birthing services. Five birthing sites within the York and Northern Region of South Australia, designed in collaboration with key stakeholders, offered a new all‐risk midwifery continuity of care model (MMoC). All pregnant women in the region were allocated to a known midwife once pregnancy was confirmed. In July 2019, the pilot program was implemented and an evaluation undertaken. Objective: The study aimed to evaluate the effectiveness, acceptability, and sustainability of the new midwifery model of care from the perspective of health care providers. Design: The evaluation utilised a mixed methods design using focus groups and surveys to explore experiences of health care providers impacted by the implementation of the MMoC. This paper reports on midwives, doctors and nurses experiences at different time points, to gain insight into the model of care from the care providers impacted by the change to services. Findings: The first round of focus groups included 14 midwives, 6 hospital nurses/midwives and 5 doctors with the overarching theme that the 'MMoC was working well.' The second round of focus groups were undertaken across the five sites with 10 midwives, 9 hospital nurses/midwives and 5 doctors. The overarching theme captured all participants commitment to the MMoC, with agreement that 'there is no other option ‐ it has to work'. Discussion: All participants reported positive outcomes and a strong commitment to navigate the changes required to implement the new model of care. Collaboration and communication was expressed as key elements for success. Specific challenges and complexities were evident including a need to clarify expectations and the workload for midwives, and for nurses who were accustomed to having midwives 24 hours a day in hospitals. Conclusion: This innovative model responds to challenges in providing rural maternity care and offers a sustainable model for maternity services and workforce. Soil corrosivity is a term used to describe the corroding susceptibility (risk) of metal infrastructure in different soil environments. Soil corrosivity mapping is a crucial step in identifying potentially problematic, high-maintenance fence lines and can help improve fence longevity by identifying soil environments where the use of more expensive, corrosion-resistant materials would be more cost-effective in the long term. Soil corrosion damage sustained on exclusion fences can be a serious management issue for conservation programs and initiatives, as it weakens the fence netting and provides opportunities for invasive animal migration and occupation (e.g. feral cats and foxes) into areas of high conservation value. The increasing accessibility of geospatial analysis software and the availability of open-source soil data provide land managers with the opportunity to implement digital soil databases and pedotransfer functions to produce fence corrosion risk maps using commonly measured soil attributes. This paper uses open-source government agency soil data (shapefiles) to map fence corrosion risk in the southern part of the Yorke Peninsula in South Australia, with the intention to assist with the installation of a new barrier (exclusion) fence as part of the Marna Banggara rewilding project. The risk classifications (low, moderate and high risk) made by this map were compared with rates of zinc corrosion (µm/year zinc loss) observed at field sites and correctly predicted the amount of fence damage sustained at five of the eight sites. Background: The development of vaccines against SARS‐CoV2 has been a key public health response to the COVID‐19 pandemic. However, since their introduction, there have been reports of anaphylactic reactions to vaccines in individuals with history of allergic reactions to other vaccines, excipients or to COVID vaccines. Aim: A dedicated adult COVID vaccine allergy clinic with a standardised allergy testing protocol was set up to investigate safety and suitability of available COVID vaccines in Australia. Methods: Patients referred to a state‐wide COVID‐19 vaccine allergy clinic between March and August 2021 with a history of allergy underwent skin‐prick testing and intradermal testing to both available vaccine formulations (BNT162b2 and ChAdOx1‐S), excipients (polyethylene glycol and polysorbate 80), excipient‐containing medications and controls. Basophil activation testing was conducted in few subjects with convincing history of immediate type reactions. Results: Fifty‐three patients underwent testing for possible excipient allergy (n = 19), previous non–COVID vaccine reaction (n = 13) or previous reaction to dose 1 of COVID‐19 vaccine (n = 21). Patients were predominantly female (n = 43, 81%), aged 18–83 (median 54) years. Forty‐four patients tested negative and 42 of these received at least their first dose of a COVID‐19 vaccine. Nine patients tested positive to excipients or excipient‐containing medication only (n = 3), or vaccines (n = 6). Five patients were positive to just BNT162b2, 3/5 have been vaccinated with ChAdOx1‐S. One who was skin test positive to both vaccines, but negative BAT to ChAdOx1‐S was successfully vaccinated with ChAdOx1‐S. Conclusion: Even in a high‐risk population, most patients can be vaccinated with available COVID‐19 vaccines. This paper employs a critical race theory (CRT) perspective to probe the experiences of skilled African migrants parenting Black children in Australia, a predominantly White country. Two key themes emerged from this study: (1) the need to explicitly teach children about race and racism and to foster positive racial identities and (2) the complexities of navigating tensions between 'African' and 'Western' cultural values. Participants demonstrated high levels of awareness of intercultural parenting approaches and a desire to blend the best aspects of African and Australian cultural values in their own parenting practice. A significant paradox was also apparent in the tension between parental desires to inculcate pride in African ancestry and culture, while simultaneously encouraging children to 'curate' their blackness to minimize experiences of racialization. Social workers in Australia often play a critical role in the lives of migrant families as they support them to negotiate transitions in parenting contexts. Although this paper only offers a perspective on the parenting experiences of skilled African migrants and how they creatively manage the tensions and change emerging from this process, we suggest that this understanding helps to expand knowledge on the complexity of parenting in multicultural, transcultural and intercultural contexts. The life expectancy of persons with intellectual disability is increasing, and this is often occurring concurrently with the aging of their long‐term parental carers. Research in both Australia and around the world indicates that proactive post‐parental care planning is not widely implemented, and transitions primarily happen suddenly following a personal crisis for the primary caregiver. Little focus in Australia has been placed on identifying the barriers that inhibit post‐parental care planning in rural areas, specifically in the context of the newly implemented National Disability Insurance Scheme. This paper examines the factors that limit disability services and their staff in supporting successful post‐parental care planning for individuals with intellectual disability and their aging carers in rural South Australia. Small focus groups were conducted with three groups of rural disability support workers using online technologies to assist with the participants' geographic disparity. A semi‐structured interview guide was developed prior to commencement and was used to initiate discussions on key points. A thematic analysis methodological approach was used for data analysis. There were three themes identified through the analysis; Recruitment; Provision of Care; and Retention. The key findings relating to these themes are presented and supported with exemplar quotes. This research proposes three recommendations for policy or practice change: developing a national advertising campaign for new disability staff that positively emphasizes the high‐level skillset need for proactive planning; supporting rural disability providers to collaborate to establish shared teams of staff with expertise in post‐parental care planning; and, using incentive payments to retain staff with these invaluable skillsets in the disability sector. A vast empirical literature has investigated economies of scale in municipal operations, especially in water and wastewater provision as well as domestic waste management. By contrast, comparatively few studies have been conducted on the extent of scale economies in local government administration. Given the stress placed on scale economies in Australian state and territory government policies aimed at the structural reform of local government through municipal mergers, including in South Australian (SA) local government, the absence of empirical research into administrative scale economies is unfortunate. To address this gap in the empirical literature, in this paper, we consider administrative scale economies in the SA local government system using four‐year panel data from 2015–2016 to 2018–2019. We find limited evidence for a relationship between administrative intensity and municipal by population size and for a difference between the administrative intensity of urban and rural councils. This paper investigates the viability of Energy Justice as a framework to assist the governance of multinational risk. Positioned between local and universal scales, it advocates for the approach of multinational energy justice as a means of considering justice manifestations either between neighboring countries, or between geographically isolated countries that share common energy concerns or systems. Specifically, it focuses on how to govern the risk of nuclear waste in a multinational fashion, and questions the extent to which this approach could offer a useful account to help understand the justice issues multinational repositories give rise to. We present a content analysis of 10‐years' worth of policy reports on nuclear waste repositories (2006–16), including 25 full‐length reports documents relating to the Government of South Australia's now abandoned repository proposal. Following a discussion around the merit of energy justice approaches in relation of transboundary issues of justice, intergenerational justice and the role conflicting justice demands across time and space, we present three areas for further consideration: (i) We call for greater attention to issues of spatial conflict; (ii) further reflect on temporal justice conflict and its integration into energy justice frameworks; and (iii) reflect upon multinational responsibility for energy justice. Community treatment orders (CTOs) require individuals with a mental illness to accept treatment from mental health services. CTO legislation in South Australia states that treatment and care should be recovery‐focused, although justification for use is predominantly risk‐based, and care often coercive. Although CTOs are contested, individuals, families, and clinicians frequently engage in care planning within this context. This paper examines how the concepts of risk and risk management impact care planning from the perspectives of individuals on CTOs, their families, and clinicians. Ethnographic methods of observation and interview provided a detailed account of the perspectives of each group over an 18‐month period from two community mental health teams in South Australia. Findings show that care planning occurred within a culture of practice dominated by risk. Risk, however, was understood differently by each participant group, with the dominant narrative informed by biogenetic understandings of mental illness. This dominance impacted on the positioning of participant groups in care planning, focus of care contacts, and care options available. To improve care experiences and outcomes for individuals on CTOs, narrow conceptualizations of risk and recovery need to broaden to include an understanding of personal and social adversities individuals face. A broader understanding should reposition participants in the care planning context and rebalance care discussions, from a focus on clinical recovery to recovering citizenship. Objective: Inter‐professional education is a growing area of importance that enables training of health care professionals and students to develop skills in collaborative clinical practice, a critical aspect of disability care. However, research is limited on appropriate on‐site inter‐professional training for the rural and remote disability workforce. This paper aims to explore the features of an effective inter‐professional training approach for rural disability workforce. Setting: Riverland, South Australia. Participants: Clinical educators, allied health professionals, health and service providers and students. Design: A qualitative‐explorative research design, involving focus group discussion and a thematic analysis method were employed in this study. Participants of the focus group discussion completed a capacity building training program centred on inter‐professional education, cultural‐safety and the National Disability Insurance Scheme. National Disability Services Social Impact Measurement Tool was used to evaluate and explore the features of effective inter‐professional training program for existing and emerging disability workforce in rural regions. Results: Four themes emerged from data analysis: inter‐professional education focus; structured inter‐professional training; building collaborative learning environment; and culturally appropriate care practice. Inter‐professional supervision was identified as a key enabler for capacity building in an area with limited health workforce. Inter‐agency collaboration and professional network were identified as important elements to support disability health workforce retention and the transition from novice to practitioner. Prior knowledge about the needs of persons with disability and empathetic relationships influenced the quality of practice. Conclusion: In situ training programs, which provide real‐life rural practice context and harness inter‐agency collaboration, improve effectiveness of rural disability workforce readiness for practice. Background Flexible training options are sought by an increasing number of Australasian surgical trainees. Reasons include increased participation of women in the surgical workforce, postgraduate training and changing attitudes to family responsibilities. Despite endorsement of flexible training by the Royal Australasian College of Surgeons and Board in General Surgery, part-time ( PT) training in General Surgery in Australia and New Zealand is not well established. A permanent 'stand-alone' PT training position was established at the Royal Adelaide Hospital in 2007 under the Surgical Education and Training Program. This position offered 12 months of General Surgical training on a 0.5 full-time (FT) equivalent basis with pro rata emergency and on-call commitments and was accredited for 6 months of General Surgical training. This paper reviews the PT training experience in South Australia. Methods De-identified logbook data were obtained from the South Australian Regional Subcommittee of the Board in General Surgery with consent of each of the trainees. Totals of operative cases were compared against matched FT trainees working on the same unit. Results Overall, PT trainees achieved comparable operative caseloads compared with their FT colleagues. All trainees included in this review have subsequently passed the Royal Australasian College of Surgeons Fellowship Examination in General Surgery and returned to FT workforce positions. Conclusion This paper presents two validated models of PT training. Training, resource and regulatory requirements and individual and institutional barriers to flexible training are substantial. Successful PT models offer positive and beneficial training alternatives for General Surgical trainees and contribute to workforce flexibility. [ABSTRACT FROM AUTHOR]

This paper reports an action research project exploring the experiences of nurses on an acute mental health unit for older adults staffed with a skillmix of mental health and general nurses, which recently transitioned from a psychiatric to a general hospital. The new service provides comprehensive health care, including the management of physical co-morbidity and a recovery orientation. Recovery acknowledges the role and rights of consumers and carers in planning and management of care, choice and individual strengths (Shepherd). The new ward received additional resources to establish the model of care, including a broader skillmix. The paper explores the dynamics of development of a new model of care and of bringing together staff with different professional orientations, cultures and priorities. Focus groups and interviews were conducted with 18 staff. Analysis resulted in three themes relating to the impact of competing goals and foci of care upon professional boundaries; competing organisational cultures and the impact of service change upon work practices. Since its inclusion amongst Olympic sports in the 1990s, women's soccer has grown impressively worldwide. Despite its rapid global expansion and growth in the number of playing participants, the sport has been neglected by geographers. In Australia, which is currently the fifth women's soccer country in the world as per registered players, the popularity of the sport has grown significantly in recent years. Perhaps even more strikingly, however, the approach to the sport has changed, to focus on the achievement of results. The shift in the purpose of women's soccer, from a solely social and recreational activity to an achievement sport, is a result of the increasing links between the local women's soccer systems and the global world of sport. The paper examines an exemplar of South Australia, and in particular the Adelaide metropolitan region. Here, in the last 30 years, women's soccer has evolved from a geography of foundation, defined by informal organisation and localised scope, to a geography of achievement, characterised by an institutionalised focus on the production of players, the introduction of higher-profile ‘sportscapes’, a broader pattern of clubs distribution, and a new set of connections with global women's soccer. The current geography of achievement links local and global women's soccer scenes. On the other hand, it funnels access to the achievement level of South Australian women's soccer to a limited central area of Adelaide's metropolitan region. The paper also draws attention to the part that social capital, and especially ‘bridging’ social capital, played in enabling the evolution of Adelaide women's soccer. An exploration of the ethical implications of mining uranium in South Australia by weighing up the impact on future generations in social, economic and environmental terms. In South Australia, we have ‘a knot of problems’ in the sense used by Bateson that can be described in a case study that illustrates the complexities for planners and policy makers. The paper suggests that one of the greatest challenges for policy makers is to match the right kind of knowledge to an issue. This requires ‘phronesis’ or a process of matching based on dialogue (see Aristotle in Nicomachean Ethics translated by Irwin). A score card needs to be developed to support ethical, sustainable governance. It is based on the assumption that we can create our own futures. Freshwater availability to meet the growing needs of humankind has raised serious concerns in the recent past. Two immediate responses to counter this challenge are efficient allocation of the scarce resources, and development and use of alternative sources of water. While ‘water markets’ are seen as a means to achieve efficient allocation of the scare resources, treated wastewater and low-quality water are now considered as potential sources of water to supplement the freshwater supplies. However, the latter option, that is use of reclaimed water as an alternative, imposes concerns regarding its suitability to sustain development. This is because of various issues related to wastewater usage and application. At the same time, it is also true that a successful and well-planned reuse scheme can help achieve sustainability as evidenced in some isolated cases around the world. In line with this, the current paper adopts a case-study approach to demonstrate how a successful reuse scheme in Adelaide, South Australia, has contributed to the sustainable development of the region. The paper looks into the socio-economic and environmental dimensions of sustainability and arrives at a conclusion that properly planned and managed reuse schemes backed with effective regulatory and policy measures can lead to sustainable development. Background and Aims: This paper is the second in a series of two that presents and discusses an exploratory evaluation study of the ‘Motor Magic’ program conducted in Adelaide, South Australia. Methods: A realist evaluation approach was used, and two focus groups (one with parents and one with kindergarten staff) were held to identify program outcomes and to develop and refine key hypotheses about how these outcomes occurred. Results and Conclusions: Results for kindergarten staff involved in the program are presented and discussed in this paper, including improved early identification and early intervention for children with, or at risk of, fine motor developmental difficulties; increased confidence in the effectiveness of their practice with these children; improved practice of kindergarten staff with both targeted and all children; improved empathy with, and support for, parents; and increased interagency links and ongoing advocacy for further resources. These results suggest that the program provides an effective model for building the capacity of kindergarten staff to support children with, or at risk of, developmental difficulties. Accounting can both shape and be shaped by organisational reality (Nahapiet, 1988). This paper focuses on the shaping of an ABC (activity based costing) system in a public sector authority during a period of rapid change from 1993 to 1998. The reasons why management invested substantially in such a system are outlined, and are compared and contrasted with the views of the employees. Over time the ABC system became locked in as the transfer pricing system to meet the needs of a new style of business unit accountability where quasi–profit centres were set up within a public sector authority. A new CEO eliminated the profit–centre concept and returned to budgetary control as the primary form of financial accountability. At this stage ABC became side–lined but was still useful for strategic decisions relating to out–sourcing and improving lines of business, more in line with its initial objectives. Finally, the paper explores why ABC ceased to exist. It is a rare case of a longitudinal study of an accounting technology in a public sector organisation and provides insights into the moulding of accounting by organisational change. Winds are one of the major meteorological contributors to deaths, damage and insured losses in Australia. A 'freak storm' hit the state of South Australia on 28 September 2016, causing state‐wide blackouts and leaving 1.7 million people without power. In the first part of this two‐part study, we analyse this event and find that it was indeed extreme, deepening more explosively than all but two Adelaide‐affecting extratropical cyclones over the past 37 years and exhibiting the lowest central pressure. This generated hurricane force winds, with the central South Australia site of Neptune Island recording a gust of over 120kmh−1. We show that this storm potentially contained a sting jet. Such jets are well known as a cause of major damage across Europe, and this is the first study which investigates whether a sting jet can be produced over Australia. The main deepening of the system occurred over the Great Australian Bight, so if a sting jet did form and make it to the surface, it was not the cause of the state‐wide damage. However, the cyclone did contain numerous extreme gust‐producing mesoscale features, as explored in part II of this paper (Earl and Simmons, 2018). [ABSTRACT FROM AUTHOR]

Objectives: To report on the safety and efficacy of transvenous cardiac pacing wire insertion outside a tertiary hospital by a medical retrieval service. Methods: SAAS MedSTAR Emergency Medical Retrieval Service transports symptomatic bradycardic patients in rural South Australia to Adelaide on transvenous pacing for ongoing management. This is a retrospective case review of all transvenous cardiac pacing wires inserted by SAAS MedSTAR between January 2015 and October 2017. Results: This study demonstrated successful insertion of cardiac transvenous pacing wires and cardiac capture in 10 of 11 cases (91%) by pre‐hospital and retrieval doctors. There were no immediate or long‐term complications from insertion. All of the patients were successfully transferred by helicopter or fixed wing to their receiving facility, with nine of the 11 patients (82%) surviving to hospital discharge. Conclusion: This paper demonstrates that transvenous cardiac pacing can be safely and successfully implemented for symptomatic patients by pre‐hospital and retrieval physicians in the aeromedical retrieval setting. Purpose: Patients' perceptions about the functional impact of amblyopia and strabismus in daily life have not been explored extensively. Therefore, this study aimed to explore the lived experiences of adults with these conditions and understand the functional limitations they face in their day‐to‐day life. Methods: A qualitative study design was adopted. Participants over 18 years of age, with a primary diagnosis of amblyopia (with or without strabismus) were recruited from the community and various eye care practices in South Australia and Victoria, Australia. Participants took part in either focus group discussions or individual interviews and described the functional limitations they experienced in their daily life due to their eye condition. These sessions were audio recorded, transcribed verbatim, coded inductively, and analysed iteratively to form emergent themes. Results: Thirty‐seven adult participants took part in the study: 23 (62%) had strabismic amblyopia; 5 (14%) anisometropic amblyopia;, 7 (19%) combined‐mechanism amblyopia; and 2 (5%) deprivational amblyopia. Their median age was 54 years (range: 21–82 years) and 19 (51%) were female. Participants reported several challenges in performing everyday tasks such as driving (e.g. judging distances, changing lanes), reading (e.g. fine print, reading for prolonged time) and sports (e.g. catching a ball). They also articulated trouble in navigating safely (e.g. using stairs, bumping into objects), performing work‐tasks (e.g. taking longer than peers to complete tasks) and other routine tasks (e.g. chopping vegetables with care). Conclusions: Several functional limitations were encountered by adults living with amblyopia and strabismus. Participants recognised these limitations in their normal day‐to‐day life and related the challenges they faced to symptoms associated with their eye condition. Aim: The prevalence of diabetes in older adults in residential aged care (RAC) is twice that of community dwelling older adults. Older adults with diabetes have been highlighted as being at high risk of frailty and malnutrition, particularly when managed on a therapeutic diet. However, assumptions may be based on clinical presentation of our oldest old which may be at conflict with the clinical presentation of younger older adults. The aim of this retrospective audit was to identify the characteristics of aged care residents with diabetes, their comorbidities and malnutrition risk. Methods: Residents' with diabetes paper‐based records were audited for demographic data, diet code, medical and medication history, malnutrition risk screening scores and anthropometric data. Results: A total of 295 residents with diabetes records were audited across 13 sites in South Australia. Younger older adults (65–74 years) were generally in a healthy weight range (23–30 kg/m2) or obese (>30 kg/m2) and had gained weight since admission to RAC; whereas the oldest old were more likely to have lost weight and had a lower body mass index range 22.6–28.9 kg/m2. An unexpected finding was the variability between RAC sites in dietary management of diabetes and potential inappropriate dietary management of malnutrition, as indicated by their food service diet codes. Conclusions: Findings suggest that younger older adults in RAC are more likely to be overweight which is maintained over a typical length of stay. Issue addressed: This paper examines recent Australian health reform policies and considers how the primary health care (PHC) workforce experiences subsequent change and perceives its impact on health promotion practice. Methods: Health policy documents were analysed to determine their intended impact on health promotion. Interviews were conducted with 39 respondents from four State-funded PHC services to gain their perceptions of the impact of policy change on health promotion. Results: There have been a plethora of policy and strategy documents over the last decade relevant to PHC, and these suggest an intention to strengthen health promotion. However, respondents report that changes to the role and focus of PHC services have led to fewer opportunities for health promotion. Services are struggling to engage in health promotion activity, while funding and policy directions are prioritised to targeted, individual behaviour change. Conclusion: The experience of PHC workforce respondents in South Australia suggests that, despite policy intentions, health promotion practice is much reduced. Our research suggests that rigorous evaluation of health sector reforms should be undertaken to assess both intended and unintended outcomes in terms of service quality and delivery. The paper outlines water conservation behaviours and assesses the level of congruity between the stated water use of householders against their actual metered consumption. A profile of high water users in three parts of South Australia is offered: two metropolitan areas differing in socio-economic characteristics and a regional town. The research used a postal questionnaire, a follow-up telephone interview and corresponding household water meter readings. Location, household size and annual household income have significant predictive qualities for high per capita water use. The number of times gardens were watered in a week, watering the garden more often than was permitted under the restrictions, and the manner in which conservation behaviours were carried out helped predict high per capita water use. Participants had an accurate idea of the magnitude of their water use and how it compared with that of other households. High water users knew that they were high consumers of water. Aspects of mental health nursing and its subspecialties are not easily defined. Child and adolescent mental health ( CAMH) nursing is a subspecialty of mental health nursing, and some of its characteristics are tacit. This paper presents a deeper understanding of the meaning that CAMH nurses make of their role and work in the inpatient setting. The research was undertaken through a PhD candidature. The epistemological framework for the research was social constructionism. Interpretive enquiry was the methodology, as it allowed for the interpretation of multiple realities, which resulted in a rich description of the role and work of CAMH nurses. Methods of data collection were document analysis, focus group interviews, and individual interviews. Participants included nurses and multidisciplinary staff. Iterative and aggregative analyses were utilized for the documents. The focus group and individual interview data were analysed utilizing a thematic analysis process. This paper presents the findings of the combined analysis and the resultant holistic conceptual framework for the work of the CAMH nurse in the inpatient unit. The findings have contributed new knowledge to mental health nursing, specifically CAMH nursing, making the parameters of practice more explicit. Becoming an object of touristic interest is only one of a series of ways that Aboriginality is being transformed in contemporary Australia, as the space opens up for individuals and groups to reposition themselves as Aborigines within the nation, with a distinctive culture in various forms. The nation's appetite for Aboriginal 'culture', within desirable limits (Povinelli 2002) and energised by a sentimental politics (Cowlishaw 2010), continues to grow. There is, however, a destructive flip side to the politics of difference being played out within Aboriginal societies. This is evidenced by the many battles for access to or control of 'cultural' resources for their commercial benefits or collective survival value. In many places communities or groups are faced with the terrible choice of distinction or extinction (Comaroff & Comaroff 2009). That is, they must find, and make alienable, something distinctive about themselves or face collective extinction. How one Aboriginal community is responding to these threats and challenges is the subject of this paper. Aim: To determine the validity of electronic hospital pharmacy dispensing records as an indicator of late gestation exposure to antidepressants using medical records as the gold standard. Method:Medical records (gold standard) were reviewed for a consecutive sample of 400 women who delivered at a large maternity hospital during 2005. Data on antidepressants used in pregnancy from the medical records were compared with the electronic pharmacy dispensing records to ascertain sensitivity and specificity. Results: According to the medical records, 12 (3%) women were identified as taking antidepressants during late gestation. The electronic pharmacy dispensing records correctly identified 9 women as exposed to antidepressants (sensitivity score 75%; 95%CI 43-95). No women were classified as exposed according to pharmacy dispensing records and classified as not exposed according to the medical records (specificity score 100%; 95%CI 99-100). 7 (1.8%) women were identified in the medical records as taking antidepressants but had either stopped prior to pregnancy or on finding out that they were pregnant. These patients were not identified in the pharmacy dispensing records. Conclusion: Electronic hospital pharmacy dispensing records may provide an efficient alternative to paper records for identifying late gestation exposure to antidepressants, but will underestimate the exposure by 25%. Although sport is considered an important component of Australian society and a precious vehicle of social interaction, sports geography remains in many ways a neglected field of investigation. Nevertheless, geographical studies of sports can add valuable insights to more acknowledged geographical discourses. They can also contribute to regional sporting success. This paper analyses the current spatial organisation of women's soccer in Adelaide and outlines the unequal spatial expression of its recent professionally-oriented approach, the achievement phase. A significant proportion of Adelaide's female population experiences limited opportunity to participate fully in the sport. The sport therefore fails to maximise its human resources and its spatial organisation constitutes a limit to the competitiveness of South Australian women's soccer as a system. The paper uses the concept of social capital to explore the unequal engagement of four sub-regions in women's soccer. Many of the areas experiencing relative exclusion from women's soccer are the same ones that suffer the most from disengagement from the global economy. In those areas, socio-economic disadvantage is matched by limited opportunities for self-fulfilment through sport, and the effectiveness of social networks is weaker. This work aims to provide information for South Australian women's soccer institutions to foster enhanced equity in terms of access to the sport in metropolitan Adelaide. It also provides a base from which to investigate the reasons behind sub-regional differences in the ability to produce quality players, knowledge that, if applied to these less productive areas, may contribute to the general enhancement of overall sporting outcomes. There is a paucity of literature on the topic of sobering-up centres (non-custodial safe overnight accommodation for the publicly intoxicated). This paper presents findings of a retrospective longitudinal case study of a sobering-up centre in regional South Australia over the ten years 1991 to 2000. There were 6,486 admissions during this period, 97.1% of which were of Aboriginal people. We collated and analysed primary data including demographic details of admissions and re-admissions, and qualitative and quantitative measures of intoxication. The findings from this case study, considered together with contextual understandings from a wider social study in this region by three of the authors, provide supporting evidence of the important role of sobering-up centres in averting the known harms of a custodial response to public drunkenness, as well as avoiding the potential harm of alcohol-related injury among vulnerable Aboriginal people. Regional planning for and management of the environment and natural resources are the new foci of the Australian Government's funding initiative, the Natural Heritage Trust Phase Two (NHT2). NHT2 attempts to integrate and coordinate the efforts of the many local environmental community groups that work on-ground. By operating at the regional scale, it is expected that more strategic and targeted outcomes will be achieved. This paper examines how the shift to regional-scale delivery has affected key stakeholders in South Australia, focussing on the experiences of a sub-regional environmental community group. Interviews with key regional stakeholders, participant observation and document analysis highlighted difficulties in the transition to regional delivery, for both regional and local groups. Limited funding, restrictive timelines, vague guidelines and prescriptive information, slowed the development of the regional plan and frustrated regional groups. A lack of communication and limited community participation in developing the regional plan created uncertainty and confusion in the sub-regional community group, resulting in mistrust of and alienation from the regional groups. Concern was also expressed over the loss of the local-scale focus, particularly as community leaders moved to higher decision-making positions within the region. It is too early too assess the effectiveness of the regional groups. Since late 2000, anecdotal reports from drug users and health professionals have suggested that there was a reduction in the supply of heroin in Adelaide in the first half of 2001, referred to as a heroin 'drought'. The aim of this paper was to critically review evidence for this, using data obtained from 100 injecting drug users surveyed for the 2001 Illicit Drug Reporting System (IDRS). This project is carried out annually in all Australian jurisdictions, and collects up-to-date information on the markets for heroin, methamphetamine, cocaine and cannabis. This paper also investigates the possible implications of this 'drought' on patterns of drug use and drug-related harms. The 2001 IDRS found consistent reports by users of an increase in the price of heroin, together with decreases in purity and availability. These factors resulted in a decrease in the frequency of self-reported heroin use among those surveyed in 2001, and a concomitant increase in the use of other drugs, in particular methamphetamine and morphine. The heroin 'drought' appears to have had a substantial impact on several indices of drug-related harm. There was a marked decrease in the number of opioid-related fatalities, and hospital data also showed reductions in heroin-related presentations. Treatment service data showed an increase in the number of admissions related to amphetamines. There is a need for health promotion and education on the adverse effects of methamphetamine use, and the development of improved treatment protocols for methamphetamine abuse and dependence. ABSTRACT Seasonal rainfall forecasts are an important tool for risk management across many sectors. However, significant challenges arise in the development of skilful and practically useful seasonal forecasts for regions where the temporal and spatial variability of rainfall is large and/or knowledge about what causes this variability is in its infancy. This is evident in the state of South Australia ( SA), where seasonal rainfall currently has low predictive skill. The key climate processes have yet to be fully identified in SA and therefore may not be adequately represented in forecast models. The aim of this paper is to identify and quantify relationships between large-scale ocean-atmospheric processes and seasonal rainfall variability across SA. We identify two distinct climate zones: (1) the arid northern region, where rainfall is mostly influenced by climate processes stemming from the tropical Indian and/or Pacific Oceans and (2) southern SA, which is dominated by Southern Ocean processes. The average percent of variability of SA rainfall accounted for by any single large-scale climate process (i.e. linear regression using a single predictor) is 8% in summer, 19% in autumn, 33% in winter and 24% in spring. However, when two or more processes are considered in combination (through multiple linear regression), this rises to 13, 26, 46, and 33%, respectively, highlighting the importance of capturing the interaction among multiple climate processes. Importantly, the findings from this study provide a set of metrics against which existing statistical and dynamical forecasting schemes can be tested and highlight processes that should be focused on in order to improve (or develop new) forecasting schemes. The study also recommends the need for further investigations into non-linear relationships between rainfall and large-scale ocean-atmospheric processes and the development of more objective methods for determining which climate process, or combination of processes, are most important for a certain season or location. Background Intravenous iron is commonly utilised in pregnancy when treatment with oral is not tolerated or where rapid replenishment of iron stores is required. Aims To examine the relationship between doses of intravenous iron administered during pregnancy according to different maternal bodyweight measures and subsequent treatment response. Methods Retrospective cohort study of pregnant women with confirmed iron deficiency anaemia who received intravenous iron polymaltose at a tertiary teaching hospital in Australia from 1 January 2014 to 31 January 2016. Diagnosis of anaemia and/or iron deficiency, infusion dosage characteristics and haematological parameters were collected from paper-based case notes and electronic records. The dose of intravenous iron administered was examined relative to maternal total bodyweight ( TBW), ideal bodyweight ( IBW) (equation = 45.5 kg + 0.9 kg/cm for each cm over 152 cm) and adjusted bodyweight (equation = IBW + [0.4 × ( TBW - IBW)]). Results A total of 122 pregnancies was identified where women had confirmed iron deficiency anaemia and received a single infusion of intravenous iron polymaltose. Dose-response relationships were evident between change in haemoglobin from treatment until delivery and intravenous iron dose according to adjusted bodyweight (adjusted beta coefficient 0.70 (95% CI 0.24-1.15)) and pre-pregnancy total bodyweight (adjusted beta coefficient 0.83 (95% CI 0.36-1.29)), but not ideal bodyweight (adjusted beta coefficient 0.37 (95% CI −0.04-0.78)). Calculating iron deficit using adjusted bodyweight most closely matched that based on a physiological estimate of iron deficit according to weight-based total blood volume. Conclusion Optimal treatment outcomes in pregnant women requiring intravenous iron may be reached by dosing according to adjusted pre-pregnancy bodyweight rather than ideal bodyweight. Objective The aim of the present paper is to study the indications for fluid bolus therapy (FBT) and its associated physiological changes in ED patients. Methods Prospective observational study of FBT in a tertiary ED, we recorded indications, number, types and volumes, resuscitation goals and perceived success rates of FBT. Moreover, we studied key physiological variables before, 10 min, 1 h and 2 h after FBT. Results We studied 500 FBT episodes (750 [500-1250] mL). Median age was 59 (36-76) years and 57% were male. Shock was deemed present in 135 (27%) patients, septic shock in 80 (16%), and cardiogenic shock in 30 (6%). Overall, 0.9% saline (84%) was the most common fluid and hypotension the most common indication (70%). 'Avoidance of hospital/ICU admission' was the goal perceived to have the greatest success rate (85%). However, although mean arterial pressure (MAP) increased ( P < 0.01) and heart rate (HR) decreased ( P = 0.04) at 10 min ( P = 0.01), both returned to baseline at 1 and 2 h. In contrast, respiratory rate (RR) increased at 1 ( P < 0.01) and 2 h ( P = 0.03) and temperature decreased at 1 and 2 h (both P < 0.001). In patients with shock, 1 h after FBT, there was a median 3 mmHg increase in MAP ( P = 0.01) but no change in HR ( P = 0.44), while RR increased ( P < 0.01) and temperature decreased ( P = 0.01). Conclusions In ED, FBT is used mostly in patients without shock. However, after an immediate haemodynamic effect, FBT is associated with absent or limited physiological changes at 1 or 2 h. This study was designed as a needs assessment for interventions to respond to hazardous and harmful amphetamine use, including options for treatment. Research techniques from Rapid Assessment Methodology (RAM) were utilized to collect data. This included a survey of 100 amphetamine users, from which the data for the current paper has been drawn. This paper is focused on the characteristics of those who had sought information and help in the sample, and on those who were dependent on amphetamine and who felt they needed treatment, in comparison to those who were also dependent but did not feel the need for treatment. Findings in regard to treatment-seekers were largely consistent with previous Australian and overseas research, and thus underscore the importance of considering psychological morbidity, both prior and subsequent to amphetamine use, in treatment settings for amphetamine users. Having previously sought general information about amphetamines, safer drug use and blood-borne diseases was best predicted by higher sexual risk-taking and poorer physical functioning. Having previously sought help for an amphetamine-related concern or problem was best predicted by higher severity of dependence on amphetamines and poorer social functioning. This paper examines a specific controversy over the control and content of sex education in South Australia in 1985 that was triggered by concerns about homosexuality. Drawing on Foucault's idea of the "deployment of sexuality", the paper examines the way in which the issue of homosexuality became the focal point for a concerted struggle over sex education in state schools. It is argued that the idea of homosexuality, or more accurately male homosexual sexuality, served as a boundary marker that both defined and revealed the content and scope of school-based sex education. It is shown that this controversy was an instance of the "deployment of sexuality" through which public opinion was concentrated and mobilised to ensure that prevailing heterosexual norms remained the defining parameters for sex education. Trilobites are one of the most diverse and abundant fossil groups from the early Palaeozoic, and as such are useful for answering important questions about early animal evolution, including developmental processes. Ontogenetic information for a large number of trilobite species has been published, but cases where multiple articulated specimens are known across the full range of developmental stages are rare. The early Cambrian (Series 2, Stage 4) Emu Bay Shale biota from Kangaroo Island (South Australia) is numerically dominated by trilobites, particularly articulated specimens of the ellipsocephaloid Estaingia bilobata, which are present in densities of >600 individuals per square metre on certain bedding planes. Here we describe the essentially complete post‐embryonic ontogenetic series of E. bilobata from the Emu Bay Shale, and investigate patterns of growth relating to articulation and segmentation in this early Cambrian arthropod. Estaingia bilobata exhibits the hypoprotomeric mode of growth, with the epimorphic phase (the cessation of trunk segment generation) reached prior to the onset of the holaspid period. The meraspid pygidium had an extended equilibrium period in which anterior segment release into the thorax was matched by subterminal segment generation. Previously undocumented morphological features of E. bilobata, including the hypostome and bispinose pleural tips in holaspides, are also described. The growth characteristics and morphological features of E. bilobata documented herein strengthen close phylogenetic relationships between the Estaingiidae, Ellipsocephalidae and Xystriduridae. This paper traces the background to the 1979 and 1984 equal opportunity policy statements in the South Australian Department of TAFE. It documents the process of policy development and planning for implementation and, through an examination of the content of each statement, evaluates the intentions of the authors. It concludes that there is little evidence of commitment at political and bureaucratic levels to deal with the issues the policies sought to address. By use of documents produced within the department at the time, the paper traces resistance to external pressures for change and failure to establish a firm and realistic program for policy implementation.
