The 2011 Great East Japan Earthquake was a disaster that led to the greatest number of casualties related to any form of natural disaster seen in developed countries since World War II. Furthermore, this earthquake occurred in an area where tsunami countermeasures had been prioritized. This disaster, therefore, led to the question "Is it impossible to reduce the number of victims of huge catastrophes, even in cases in which advanced disaster prevention measures have been taken?" Part of the reason this particular earthquake caused the largest number of deaths was because the tsunami that followed—which exceeded the "design load" of the seawall—hit the urban area. In addition, the tsunami, which also exceeded the "estimated loads" of the established disaster prevention plan, caused many "evacuation failures." Another factor that contributed to the deaths was that the disaster prevention measures, up to that point, had relied primarily on the recognition that disasters could be prevented by the development of "hard," or tangible, disaster prevention facilities in addition to "soft," or intangible, measures, such as issuing warnings, without imposing space restrictions. Another characteristic of the Great East Japan Earthquake was that the largest reconstruction budget associated with any disaster in postwar Japan was compiled for it. Although the reconstruction project was over‐specified for the disaster‐afflicted area in terms of scale, cost, and duration of reconstruction, many unused land areas were also created in the new urban areas created during the reconstruction project. Furthermore, the reconstruction projects undertaken with the huge reconstruction budget were not based on the "choice to rebuild the lives" of the disaster‐afflicted areas and the victims, but were rather implemented while simultaneously "marginalizing" said victims and areas as a whole. The over‐specified reconstruction projects and the associated marginalization of disaster victims tend to exist in a mutually regulated relationship. Therefore, there are concerns about the future sustainability of the noted disaster‐afflicted areas, which are already suffering from a severe population decline. Based on the previously presented discussion, it is possible to highlight various issues associated with disaster measures implemented in developed countries. First, regardless of how advanced disaster measures are, a "surge in disaster damage" can occur, which can lead to a "black swan" event. Therefore, it is necessary to formulate disaster prevention measures based on the assumption that such crises will occur in the future. Second, it is necessary for developed countries to determine how best to formulate reconstruction policies to avoid marginalizing disaster victims as well as to prevent over‐specified reconstruction. In examining these two issues, the common problem that arises is how to conduct "risk assessment and enable its acceptance" most calmly immediately after a disaster and then formulate disaster prevention measures based on such assessment. Finally, the future of disaster sociology is detailed in this work. More specifically, in order for disaster sociology to escape its "marginal" status in sociology, it is necessary to consider "disaster" in the context of both a social structure and a social change—similar to how risk theory views the issue. The geographic fragmentation of the production process inevitably leads to the question of whether or not there is a relationship between the level of value‐added and the position of production stages along with a global production/value chain, known as the "smile curve" as the theoretical literature suggests. This study investigates the relationship between value‐added and production stages across more than 34 sectors over 40 countries for two different periods, 1995–2011 and 2000–2014, by taking into account a variety of different measures of production stage and country‐sector heterogeneity. In the first step, we utilize the decomposition methodology of Wang, Wei, Yu, and Zhu (2017) to track the production stages across country sector. In the second step, we test our hypotheses by employing the fixed effects (FE) estimation technique. The results show that the relationship between value‐added content in output and backward length exhibits a U‐shaped distributional pattern. The significant results for both developed and developing economies indicate the potential of economies to benefit from the functional upgrading along with production stages. Our general conclusion regarding production chains is also valid for stages mainly related to the global value chain (GVC), especially for developed countries. Furthermore, capital intensity and total factor productivity appear to be crucial factors for improvements in process upgrading. Given the robust and positive impacts of chain upgrading in all countries considering both the total production process and GVC part, industries should exploit the opportunities for higher value‐added in the global production system. This paper empirically estimates the trade effects of technical barriers to trade (TBT) based on all TBT notifications from 105 World Trade Organization (WTO) countries during 1995-2008. The paper adopts a modified two-stage gravity model to control for both sample selection bias and firm heterogeneity bias. It was found that a country's TBT notifications decrease other countries' probability of exporting, but increase their export volumes. The result can be explained by the TBT's differential effects on the fixed and variable cost of export, and consumer confidence. It was further found that (i) a developing country's TBT have significant effects on other developing countries' exports, but no significant effects on the developed countries' exports, (ii) a developed country's TBT have significant effects on the exports from both types of countries, and (iii) exports from developed countries are affected by a developed country's TBT more seriously than a developing country's TBT. ABSTRACT The past two decades have seen the extensive privatisation and marketisation of health care in an ever reaching number of developing countries. Within this milieu, medical tourism is being promoted as a rational economic development strategy for some developing nations, and a makeshift solution to the escalating waiting lists and exorbitant costs of health care in developed nations. This paper explores the need to problematize medical tourism in order to move beyond one dimensional neoliberal discourses that have, to date, dominated the arena. In this problematization, the paper discusses a range of understandings and uses of the term 'medical tourism' and situates it within the context of the neoliberal economic development of health care internationally. Drawing on theory from critical medical anthropology and health and human rights perspectives, the paper critically analyzes the assumed independence between the medical tourism industry and local populations facing critical health issues, where social, cultural and economic inequities are widening in terms of access, cost and quality of health care. Finally, medical tourism is examined in the local context of India, critiquing the increasingly indistinct roles played by government and private sectors, whilst linking these shifts to global market forces. This paper provides a comparison between the developed and developing European countries through investigating the profit-structure relationship in the banking industries. The reduced-form profit equations are estimated for each group of countries for the period 1995-2006. The results suggest that the inclusion of X-efficiency and scale efficiency directly in the reduced-form profit equation is crucial in explaining the bank profit-structure relationship in the European banking markets. When we control for direct measures of efficiency, the market share and concentration coefficient become insignificant in all regressions. The results support the efficiency versions of the efficient-structure hypothesis over the relative market power and structure-conduct-performance hypothesis. For the developing economies of Europe, the findings of the paper indicate that efficiency is a crucial factor for establishing a sound banking system and the banks in these countries should increase their scale of operations to attain an optimal profit level. The globalization of markets and business operations is a trend that will continue strongly in the coming decades. One inescapable aspect of globalization has been the trend toward global outsourcing, especially that of knowledge-based services. Due to firms' compulsion to reduce costs in the developed world, the issue is not if a particular firm will outsource or offshore work but when it will outsource it and how effectively it will leverage outsourcing to achieve superior competitive advantage. An important implication of the outsourcing of knowledge-based services is the management of intellectual property (IP). Managers and researchers alike are interested in understanding the effects of global outsourcing of knowledge-based services on the management of IP. The challenge of accessing, exploiting, and defending IP in global outsourcing relationships is first examined in this paper. IP can be managed by balancing the trust and control and verification in the outsourcing relationship. Given that defending IP is a major concern for outsourcing firms, the moderating roles of multitier suppliers, supplier country legal regimes, and global supplier communities of practice on defending IP is examined in detail through moderating effect propositions. Finally, the paper examines the effect of accessing, defending, and exploiting IP in global outsourcing relationships on the generation of incremental and radical innovation for the outsourcing firm. This research tries to extend current academic research on global outsourcing in three ways. First, it offers a framework to understand the management of the buyer-seller relationship in the global outsourcing of knowledge-based services and its relationship to the management of IP and innovation generation. Second, the framework takes a broader perspective of outsourcing and innovation generation, including globalization, tiered suppliers, supplier country legal regimes, and global supplier communities of practice on defending IP. Third the research examines the effect of accessing, exploitation, and defense of IP on generation of incremental and radical innovation for the outsourcing firm. The goal of this paper is both modest and ambitious. The modest goal is to show that intercountry adoption should be considered by ethicists and healthcare providers. The more ambitious goal is to introduce the many ethical issues that intercountry adoption raises. Intercountry adoption is an alternative to medical, assisted reproduction option such as in vitro fertilization (IVF), intracytoplasmic sperm injection, third party egg and sperm donation and surrogacy. Health care providers working with assisted reproduction are in a unique position to introduce their clients to intercountry adoption; however, providers should only do so if intercountry adoption is ethically equal or superior to the alternatives. This paper first presents a brief history of intercountry adoption. The second section compares intercountry adoption with medical alternatives. The third section examines the unique ethical challenges that are not shared by other medical alternatives. The final section concludes that it is simplistic for a healthcare provider to promote intercountry adoption unconditionally; however, in situation where intercountry adoption is practiced conscientiously it poses no greater ethical concern than several medical alternatives. South Korea is one of the most successful newly industrialized countries in recent history in terms of sustained growth and wealth distribution where poverty has been mostly eradicated. This paper joins the argument that economic growth and development in Korea would have been unlikely to take off and develop without economic nationalism as a binding ingredient - or force - that has given direction to modernization policies. This paper asserts that economic nationalism in Korea is not necessarily a negative force and that it remains effective despite globalization and the neoliberal influence, which apparently undervalues it. In order to demonstrate the validity of the argument, three industries are compared and analyzed: the automotive, information and communication technology, and aerospace industries. The paper reviews the trajectories of these industries in accordance with the present and past stages of economic development in Korea. The relevance of the paper to the current scholarship is that in order to understand the contemporary political and economic developments in South Korea and their implications for the regional political economy it is necessary to recognize the issue of economic nationalism as explained here. The comparative method used to analyze the case studies provides a clearer picture of the evolutionary process and, hopefully, it helps the reader to discern likely future trends in Korean economic policy. South Korea's official development assistance (ODA) has been increasing rapidly and will continue to do so. Korea is one of the few countries which have successfully transitioned from a recipient to a donor. It became a member of DAC (development assistance committee), OECD in November 2009. Korea's ODA policy, along with its growth in quantity, is at a crossroads for the enhancement of its quality. Discussions and debates are going on regarding the reforms in Korea's ODA activities, and this paper examines key issues raised. It first reviews the past and present of Korea's ODA, and identifies major characteristics including a low ODA/GNI ratio, a high percentage of concessional loans compared to grants, a high portion of tied aid, regional bias and a relatively large number of recipients. The paper argues that those characteristics arise from a lack of consensus on some fundamental issues like the objective of ODA, positioning of Korea's ODA as an emerging donor and the nature of aid to North Korea. We also argue that a shift of ODA policy is required to promote reform, based on a thorough reflection on the role of ODA in the alleviation of poverty and promoting sustainable development in developing countries, rather than serving as an economic tool. This paper empirically investigates the effectiveness and feasibility of two FDI policies, fiscal incentives and deregulation, aimed at improving the attractiveness of a country in the short run. Using disaggregated data on sales by US MNEs’ foreign affiliates in 43 developed and developing countries over the 1982–94 period, results show that the provision of fiscal incentives or the deregulation of the labour market would exert a positive impact on total FDI. Given the drawbacks frequently associated with the use of incentive packages, economy-wide policies which ease firing procedures and reduce severance payments would certainly be the best policy option. This paper also highlights the different aggregation and omitted variable biases that have affected results of previous studies and provides some support to recent theoretical models of FDI by showing that third-country effects and spatial interdependence influence respectively the location of export-platform FDI and vertical FDI. (1286) Hiau Looi Kee, Hong Ma and Muthukumara Mani Under the Kyoto Protocol, industrialised countries (called Annex I countries) have to reduce their combined emissions to 5 per cent below 1990 levels in the first commitment period of 2008–12. Efforts to reduce emissions to meet Kyoto targets and beyond have raised issues of competitiveness in countries that are implementing these policies, as well as fear of leakage of carbon-intensive industries to non-implementing countries. This has also led to proposals for tariff or border tax adjustments to offset any adverse impact of capping CO2 emissions. In this paper we examine the implications of climate change policies such as carbon tax and energy efficiency standards on competitiveness across industries, as well as issues related to leakage, if any, of carbon-intensive industries to developing countries. Though competitiveness issues have been much debated in the context of carbon taxation policies, the study finds no evidence that industries’ competitiveness is affected by carbon taxes. In fact, the analysis suggests that exports of most energy-intensive industries increase when a carbon tax is imposed by the exporting countries, or by both importing and exporting countries. This finding gives credence to the initial assumption that recycling the taxes back to the energy-intensive industries by means of subsidies and exemptions may be overcompensating for the disadvantage to those industries. The time profile of inflation in China resembles the one experienced in major industrial countries. Given the uncertainty surrounding the sources of economic shocks, the present paper compares results from three sets of alternative identification conditions. Our principal finding is that inflation in China has been primarily driven by monetary factors. Although aggregate supply factors might have pushed inflation to cross the threshold leading to deflation, monetary policy is primarily responsible for Chinese inflationary outcomes. This paper examines the implications of offshoring by French manufacturing firms on their productivity and profitability. Offshoring is considered as endogenous and its impact is estimated on ex post measures of firm performance. The dataset used allows the distinction between two forms of offshoring: vertical foreign direct investment (FDI) and international outsourcing. It also gives us the possibility of distinguishing between offshoring to developed and developing countries. The results show, first, that offshoring has a stronger effect on profitability and, second, that the implications on performance are significant only in the case of international outsourcing to developing countries. This paper examines the impact of bilateral real exchange rate volatility on real exports of five emerging East Asian countries among themselves as well as to 13 industrialised countries. We recognise the specificity of the exports between the emerging East Asian and industrialised countries and employ a generalised gravity model. In the empirical analysis we use a panel comprising 25 years of quarterly data and perform unit-root and cointegration tests to verify the long-run relationship among the variables. The results provide strong evidence that exchange rate volatility has a negative impact on the exports of emerging East Asian countries. In addition, the results suggest that the pattern of bilateral exports is influenced by third-country variables. An increase in the price competitiveness of other emerging East Asian countries has a negative impact on a country’s exports to a destination market, but the magnitude of the impact is relatively small. These results are robust across different estimation techniques and do not depend on the variable chosen to proxy exchange rate uncertainty. The results of the GMM-IV estimation also confirm the negative impact of exchange rate volatility on exports and suggest that this negative relationship is not driven by simultaneous causality bias. This paper is concerned with the robustness of equilibrium exchange rate estimations based on the BEER approach for a set of both industrial and emerging countries. The robustness is studied in four directions, successively. First, we investigate the impact of using alternative proxies for relative productivity. Second, we analyze the impact of estimating the equilibrium equation on one single panel covering G20 countries, or separately for G7 and non-G7 countries. Third, we measure the influence of the choice of the numeraire on the derivation of bilateral equilibrium rates. Finally, we study the temporal robustness of the estimations by dropping one or 2 years from the estimation period. Our main conclusion is that BEER estimations are quite robust to these successive tests, although at one point of time misalignments can differ by several percentage points depending on the methodology. The choice of the productivity proxy is the most sensible one, followed by the country sample. In contrast, the choice of the numeraire and the time sample have a relatively limited impact on estimated misalignments. [ABSTRACT FROM AUTHOR]

The central component of most economic models that analyse the transition from the Malthusian regime to self-sustaining developed economies is education. Improved health is normally envisaged as simply a by-product of economic growth. Whereas growth does, indeed, tend to improve health status, the reverse is also true, namely that health improvements are a dynamic force capable of driving economic expansion. This paper underlines the importance of health improvements in escaping from Malthusian stagnation. Further, and in contrast to existing literature, which emphasizes the effects of changes in mortality rates, this paper focuses on the relationship between health status and the efficiency of human capital technology. Through this channel, health improvements stimulate investments in child quality in terms of both nourishing and schooling and drive the economy towards the Modern Growth regime. This paper examines twelve banking crises that occurred in developed countries between the 1970s and 1990s and highlights the macroeconomic adjustment paths experienced during these episodes. The banking crises were generally preceded by financial deregulation, which sparked rapid real credit growth and asset price inflation. During the booms, the lending standards of banks deteriorated and prudential supervision was inadequate. The most serious crises coincided with deep and prolonged slowdowns in economic activity, starting with falling credit growth and asset prices, followed by a sharp fall in investment and a slowing in consumption, partly offset by a rise in net exports. Governments responded with a range of policies to restore the health of the banking sector, along with monetary and fiscal stimulus. The paper concludes by comparing these past crises with the recent banking crises in the United States and parts of Europe, where the deterioration of lending standards appears to have been an important factor once again. This paper uses non-parametric kernel methods to construct observation-specific elasticities of substitution for a balanced panel of 73 developed and developing countries to examine the capital–skill complementarity hypothesis. The exercise shows some support for capital–skill complementarity, but the strength of the evidence depends upon the definition of skilled labour and the elasticity of substitution measure being used. The added flexibility of the non-parametric procedure is also capable of uncover ing that the elasticities of substitution vary across countries, groups of countries and time periods. This paper discusses inclusion of persons with disabilities in social cash transfer programmes in developing countries. Drawing on material from literature reviews and interviews, it considers potential challenges in the design of these programmes, particularly barriers to access and the complexities of assessment, and the impact of transfers for persons with disabilities. The paper identifies some key principles for including persons with disabilities in social transfer schemes, specifically: strong legal foundations; participation of persons with disabilities in programme design, implementation and evaluation; and embedding transfers within a wider framework of action to tackle discrimination and empower persons with disabilities. This paper reviews the empirical literature on the effects of offshoring and foreign activities of multinational enterprises on developed countries' labour markets. Results suggest that material offshoring worsens wage inequality between skilled and unskilled workers; it also seems to make employment more volatile, by raising the elasticity of labour demand and the risk of job losses. Service offshoring exerts at most small negative effects on total employment, and changes the composition of the workforce in favour of high-skilled white-collar employees. Multinationals tend to substitute domestic and foreign labour in response to changes in relative wages across countries; substitutability is weak, however, and mainly driven by horizontal, market-seeking foreign direct investments. This paper examines the effect of foreign direct investment (FDI) on growth by focusing on the complementarities between FDI inflows and financial markets. In our earlier work, we found that FDI is beneficial for growth only if the host country has well-developed financial institutions. In this paper, we investigate whether this effect operates through factor accumulation and/or improvements in total factor productivity (TFP). Factor accumulation – physical and human capital – does not seem to be the main channel through which countries benefit from FDI. Instead, we find that countries with well-developed financial markets gain significantly from FDI via TFP improvements. These results are consistent with the recent findings in the growth literature that shows the important role of TFP over factors in explaining cross-country income differences. Objectives: To identify and synthesise the available evidence regarding the oral health needs of and the oral health promotion provided for older, cultural and linguistically diverse (CALD) migrants in developed countries. Method: Databases were searched using search terms to identify all relevant English‐language research studies published between 2000 and January 2018. Results: Thirty‐nine articles met the review criteria. Thirty‐three papers reported on the oral health needs of CALD older migrants and 6 on oral health programs provided for older migrants. Conclusions: Cultural and linguistically diverse older migrants have high oral health needs and poor oral status and face a variety of barriers in accessing oral health care in their host countries. Recommendations to improve their access to oral health programs include building transcultural dental training into the educational requirements of dental students and developing oral health promotion programs that include culturally and linguistically customised information. Although there are numerous studies that have looked at the spillover effects in equity markets, little attention has been paid to explore the integration of bond markets of developed and emerging economies. Our paper is an attempt to fill this void by quantifying the spillovers from developed countries on the bond markets of 25 emerging economies. We apply volatility and return spillover models to quantify the extent of the spillovers from developed markets (i.e. the United States, UK and Japan) into emerging bond markets. We find that the extent of the return spillovers and volatility spillovers has not been symmetric across emerging markets. We explain these differences using bilateral factors such as trade volume, portfolio investment, cultural and geographical factors. The world is experiencing the fourth industrial revolution, and developing countries are experiencing it differently than developed countries. Developed countries have an advantage over developing countries in that they adopted industrialisation early, and this created a large gap between the two. Developed countries are not necessarily sustainable. Sustainable development is equally important in both developed and developing countries, but in different ways. Developed and developing countries will try to achieve sustainability development goals in different ways. Developed countries will most likely use the fourth industrial revolution to integrate technology into achieving their goals, while some developing countries might first need to catch up on industrial revolutions that they have skipped. Industrialisation, specifically that of the current revolution, will occur differently in developing countries. This paper describes this and discusses some of the hurdles that might hinder developing countries from adopting Industry 4.0, and develops an initial framework for readiness assessment. Software Outsourcing Partnership (SOP) is considered as a type of risk and reward sharing relationship between a client organisation, in the developed countries, and its overseas vendor organisation. Regardless of numerous benefits, the development of SOP still remnants in its infancy stage due to several interactive barriers. Some studies have been conducted to examine the barriers to SOP formation. However, no attempt has been reported so far to explore the multifaceted interrelationships amongst them. To bridge the gap, this study implements Interpretive Structural Model (ISM) approach to reconnoitre the interrelationships amongst the barriers in the context of SOP formation. The objective of this research paper is to develop a framework for modelling structural association amongst the barriers. To achieve the objective, we used a hybrid methodology based on systematic literature review (SLR), empirical survey, and ISM. Firstly, via SLR study, we identified 27 barriers to SOP formation. Secondly, to empirically explore the interrelationships amongst the identified barriers, a questionnaire survey was performed with 50 experts from a total of 20 different countries. Further, interrelationships amongst the barriers were identified using ISM via panel review, and their classifications were carried out via Cross‐Impact Matrix Multiplication Applied to the Classification Approach. Aim: This paper describes the incidence and outcomes of childhood renal malignancies in Australia using national population-based data from the Australian Childhood Cancer Registry.Methods: De-identified data for children (0-14 years) diagnosed with renal malignancies from 1983 to 2015 inclusive were extracted. Cause-specific (CSS) and event-free survival up to 20 years from diagnosis were estimated using the cohort method. Adjusted excess mortality hazard ratios were calculated using a multivariable flexible parametric survival model. Details relating to second primary malignancies (SPMs) were also examined.Results: There were 1046 children diagnosed with renal malignancies in Australia between 1983 and 2015 (91% nephroblastoma), generating an annual age-standardised incidence rate of 8 per million children, which remained constant over the study period. CSS was 89% (95% confidence interval = 87-91%) and 88% (86-90%) at 5 and 20 years, respectively, and 5-year event-free survival was 82% (80-84%). Five-year CSS did not change over the study period and was highest for nephroblastoma (91%). Of the 94% of patients achieving remission, 15% relapsed and subsequent 5-year CSS was 49% (40%-58%). Eleven children were diagnosed with SPM (standardised incidence ratio = 2.9, 95% confidence interval = 1.6-5.3, P < 0.001), and five of them (45%) died within 5 years of the second diagnosis.Conclusions: Children treated for renal malignancies in Australia have excellent long-term survival, which is unchanged since 1983. SPMs are uncommon following treatment for childhood renal cancer but carry a poor prognosis. Relapse carries a similarly poor prognosis to SPM but is more common. Background: Patient and public engagement in paediatric health‐care decision making is under‐researched, and there is a lack of systematically reviewed literature in this area. Objective: To examine the extent, range and nature of published research investigating the engagement of children/youth, families and the public in paediatric service improvement, to summarize key aspects of the research identified and to identify gaps to help inform future research needs. Methods: Literature was sought in MEDLINE, EMBASE, PsycINFO and CINAHL. Eligible articles presented research focused on patient, family and public engagement strategies in the paediatric health‐care setting. Two reviewers extracted and charted data and analysed findings using a descriptive numerical summary analysis and a thematic analysis. Results: From 4331 articles, 21 were eligible. Most were from the United States. The majority of studies were undertaken in hospital settings and used quantitative methods. Various patient and public engagement strategies/interventions were examined, including shared decision‐making tools, questionnaires, youth councils/family advisory groups, patient portals and online networks. Most of the studies examined child/youth/parent satisfaction, with fewer investigating treatment outcomes or service improvement. The majority of studies investigated an engagement strategy at the 'individual treatment' level of engagement. Regarding the continuum of engagement, most of the studies were at either the 'consultation' or 'involvement' stage. Conclusion: Future research needs to focus on the investigation of engagement strategies delivered in primary care, and the use of more qualitative and mixed methods approaches is recommended. In developed countries, public health engineering has been closely associated with extensions of life expectancy and declines in infant mortality well beyond the contributions of medicine. However, different approaches have had to be taken in poorer countries. This paper uses cases from South and Southern Africa to illustrate the challenges and the responses and to consider whether these reflect a formal 'development engineering' approach. While external agencies have actively promoted a variety of approaches, engineered interventions were most successful where they took account of social, economic and environmental contexts, reflected local preferences and were developed by, or in close coordination with, the operational institutions concerned. © 2020 John Wiley & Sons, Ltd. The standard growth model predicts that allowing labour mobility across regions would increase the speed of convergence in per capita income levels and that migration has a negative causal impact on regional growth rates. Although the empirical literature has uncovered some evidence for the former implication, the latter has not been verified empirically. This paper provides empirical evidence for the negative causal impact of migration on provincial growth rates in a developing country with a high level of internal migration characterized by unskilled labour exiting rural areas for urban centres. We utilize an instrumental variables estimation method with an instrument unique to the country examined, controlling for provincial fixed effects. The aim of this paper is to apply recently developed panel cointegration techniques proposed by Pedroni ( Oxford Bulletin of Economics and Statistics 61 (1999): Supplement, 653–670; Econometric Theory 20 (2004): 597–625) and generalized by Banerjee and Carrion-i-Silvestre (Working Paper 591, European Central Bank, February 2006) to examine the robustness of the PPP concept for a sample of 80 developed and developing countries. We find that strong PPP is verified for OECD countries and weak PPP for Middle East and North African countries. However, in African, Asian, Latin American and Central and Eastern European countries, PPP does not seem relevant to characterize the long-run behavior of the real exchange rate. Further investigations indicate that the nature of the exchange rate regime does not condition the validity of PPP, which is more easily accepted in countries with high rather than low inflation. Offering the world's Least Developed Countries (LDCs) unhindered access to European markets for all exports other than arms, the European Union's Everything but Arms programme is the most favourable of its preferential trade arrangements. This paper aims to investigate the existence of productivity heterogeneity among foreign‐owned firms (FOFs) based in the European Union (EU). Using firm‐level data for a sample of FOFs investing in the EU over the period 2006–14, we find that foreign affiliates from advanced countries (AFOFs) show a positive productivity gap compared to foreign affiliates from emerging countries (EFOFs). However, when we consider the type and the motivation of foreign direct investment, our results reveal that, while AFOFs always seem to be more productive than EFOFs in manufacturing sectors, EFOFs appear to enjoy a productivity premium compared to AFOFs in the services, when their activity occurs in the same industry as their parent and they operate in less knowledge‐intensive market sectors. Objective: To measure the number of submissions to Acta Pyschiatrica Scandinavica from low- and middle-income countries (LIC/MIC) compared with submissions from high income countries (HIC), to compare the way through the peer review process for the three groups respectively, as well as how they do as published articles eventually. Method: By help of the Acta Psychiatrica Scandinavica editorial office database all submissions of reviews of the literature, clinical overviews, original articles, brief communications, and case reports from 2002 through 2005 were analysed as to external/in-house review, acceptance/rejection, number of downloads from Blackwell-Synergy, and number of citations [Institute of Scientific Information (ISI)] in a comparison between HIC and LIC/MIC. Results: About 14.6% of the total submissions in 2002/2003 came from LIC and MIC countries, 15.5% for the 2004/2005 period. In both periods, a larger portion of LIC/MIC manuscripts were reviewed exclusively in-house compared with HIC ones and among those papers reviewed by external experts a smaller proportion of the submissions from HIC countries were rejected than of those from LIC and MIC countries. From the first to the second period there is a significant increase of proportion of submitted LIC/MIC papers accepted for publication compared with HIC papers. Full text download and citation statistics did not differ significantly between HIC and LIC/MIC. Conclusion: Low- and middle-income countries manuscripts do gradually better in the competition with HIC papers in Acta Psychiatrica Scandinavica. Adolescent pregnancy is a major public health challenge for many industrialized countries and is associated with significant medical, nutritional, social and economic risk for mothers and their infants. Despite this, relatively little is known about the nutritional status of this population. The aim of this paper was to conduct a systematic review of the current evidence relating to the biochemical markers of nutritional status of pregnant adolescents living in industrialized countries. Six papers were identified that fulfilled the inclusion criteria, the majority of which were conducted in the United States. The studies were of variable quality and most failed to control for potential confounders which may have strongly influenced the findings. Due to limited research, conclusions cannot be drawn about the zinc and calcium status of pregnant adolescents, and data on folate and vitamin B12 status appeared conflicting. There was some consensus among studies, however, to suggest that indicators of anaemia and iron status were compromised in pregnant adolescents, particularly during the third trimester of pregnancy. Chronological age did not appear to influence nutritional status, although there was some evidence to suggest that increasing gynaecologic age may positively influence plasma ferritin levels. Current research is limited by sampling and measurement bias, and research is urgently required to address these limitations. Further consideration should also be made of the influence of the role of socio-economic support on pregnant adolescents’ nutritional status. The achievement of improved nutrition in pregnancy among adolescents requires multidisciplinary collaborations of adolescent healthcare providers, academics, professional organizations, policymakers, industry and service users. Global trade expansion after 1870 had potentially powerful effects on income distribution, especially in land-abundant less industrialised economies, by increasing land prices relative to wages. The papers in this issue add evidence on wage–rentals for a range of countries, specifically Australia, Canada, Ghana, India, and Sweden. These new data offer partial support for Jeffrey Williamson’s view that the distributional effects of booming global trade to 1914 were powerful and ubiquitous, but they highlight that more attention might be given to geographical boundaries and to other distribution forces including technology and wage bargaining conditions. This paper is an attempt to analyse the consequence of trade liberalization in agriculture in the developed countries on the incidence of child labour in a developing economy in terms of a three-sector general equilibrium model with informal sectors. Adult labour and child labour are substitutes for each other in the two informal sectors of the economy and are used together apart from capital in producing two exportable commodities. The interesting result that appears from the analysis is that agricultural trade liberalization in the developed countries may be effective in bringing down the incidence of child labour in the system. The paper substantiates the desirability of trade liberalization in agriculture in the developed nations from the perspective of the developing economies for reasons other than welfare improvement. Purpose: This paper focuses attention on declining rates of instrumental (vacuum or forceps) delivery. The decline often means that women must travel further to deliver in hospitals with capacity for cesarean delivery.Background: The paper illustrates recent trends in the use of vacuum extraction and forceps in low- and high-income countries. It describes some of the obstacles to the use of instrumental delivery and why the techniques, especially vacuum extraction, should be reintroduced. Over the past two decades, many countries have observed a decline in instrumental delivery rates while cesarean rates have increased. Objections to instrumental delivery are largely due to the potential harm it causes newborns. Some medical schools no longer train their professionals to perform instrumental delivery. Elsewhere, only specialists are permitted to perform the procedures.Methods and Results: As this is a policy paper rather than a research report, the methods and results sections are not applicable.Conclusions: Vacuum extraction can be taught to midlevel practitioners (midwives, nurse practitioners and general physicians), thereby increasing access to emergency obstetric care especially at the periphery. This allows women to give birth closer to home in midlevel facilities when hospitals are not easily accessible or are overcrowded. Where instrumental and cesarean delivery are both available, instrumental delivery could potentially reduce the risks associated with cesarean delivery and reduce the costs of obstetric care. Over the past decade neoliberalism has come to represent the dominant policy discourse and ideology in the majority of western economies. Following the attention given to this phenomenon at a global (and national) scale, there has been a recent rise in interest in its regional implementation and expression and in how these are mediated through the embedded institutional associations of context dependent localities. Using Brenner and Theodore's (2002a) notion of ‘actually existing neoliberalisms’, this paper explores the hybrid nature of the residential property market. Using one of Greater Sydney's fastest growing regions as an example, this paper shows how residential developments are hybrid constructions, framed by institutional and policy resonances constituted by both state and market actants. Key factors here are the processes of partnership (a process of joining the market and the state), master planning (a process of recognising and pursuing the goals of both the market and the state) and negotiation and provision of state infrastructure (a process of direct state construction of the market). Liberalisation of international trade in services through the Movement of Natural Persons (Mode 4) remains one of the least negotiated issues of trade policy among the 144 members of the World Trade Organisation. Economists believe that there is a basic convergence of economic interest between the developed and the developing world for liberalising Mode 4. Yet the multilateral trading system has not facilitated greater worker mobility between the labour-surplus and labour-scarce countries. Is there any economic logic as to why cross-border movements of workers have not followed the pattern predicted by international trade theory? Or are there strong socio-political barriers that have come in the way of liberalising Mode 4? These are some of the questions the paper attempts to answer. The paper shows that the economic arguments against the free movement of natural persons are based on the narrow perspective of the welfare of domestic workers while ignoring the benefit it brings to the economy as a whole. Further, non-economic arguments miss the point that the movement of workers under Mode 4 of GATS is temporary in nature, and so unlikely to have any lasting social and cultural spillovers. The paper gives specific illustrations from the recent past where temporary import of workers from labour-surplus countries has enabled both developed and developing countries sustain their economic growth. It concludes by arguing that the environment for renegotiating WTO commitments under this important sector of international trade in services is better than ever before, even though the current world economic slowdown may delay actual negotiations for a while. This paper evaluates aid by considering how effective aid has been in exerting leverage on policy choices. It is rather easy to demonstrate that if a country is unwilling to implement policy reforms, attaching conditions to aid will not ensure sustained reform. In this sense conditionality does not work. This ignores the fact that donors, through aid and conditions, can influence recipient policies. The argument of this paper is that if the analysis focuses on channels of influence, one can better identify ways to enhance aid effectiveness. Reform is a slow and difficult process and donors would be more effective ‘development partners’ if they see their role as being to support rather than force this process. In simple terms, donors should provide the information and technical assistance to help governments to make policy choices, rather than dictating choices by imposing conditions. A major problem hindering rigorous analysis in social policy is the diverse and imprecise meanings attached to the term "welfare state". In widespread usage the term has become emptied of all explanatory meaning and is used as a synonym for modem industrial states, all of which provide welfare for some of their inhabitants. The converse of the term (what modem industrial state is not a welfare state?) is rarely if ever addressed. In an attempt to encourage debate on the development of productive theory, the paper therefore discusses issues surrounding a discriminating definition of "welfare states" and points to former assumptions that they "ensure a minimum real income for all" to distinguish them from unwelfare states which benefit some but not all. It suggests a research agenda to provide empirical data which would enable welfare states to be identified and classified in terms of the effectiveness of their policies and provisions. This paper studies tariff‐tax reforms in a two‐region global New Keynesian model composed of a developing and an advanced region. In our baseline calibration, a revenue‐neutral reform that lowers tariffs in developing countries can reduce domestic welfare. The reason is that the increase in developing countries welfare due to higher output is dominated by the welfare losses stemming from the deterioration of the terms of trade. On the other hand, the reform increases output and welfare in the advanced countries and in the world as a whole. The effects that we highlight have not been studied in previous contributions to the literature, which looks at tariff‐tax reforms using a small open economy framework. Nominal rigidities have important implications for adjustment dynamics in our model. In the case of a ‘point‐for‐point’ reform, for example, price stickiness implies that the international dynamics of output is reversed compared to a revenue‐neutral reform. [ABSTRACT FROM AUTHOR]

This paper explores the meaning, and some of the practical implications, of health sector reform in less developed countries. It reviews the problems that reforms have to address, and the policy objectives they are designed to achieve. It argues that the process of reform is not concerned only with defining priorities and refining policies, but also with reforming and restructuring the institutions through which health policies are implemented. Even though some organizational principles are likely to be common to all reform programmes, a prescriptive approach to institutional reform is inappropriate. The choice of reform options can be influenced by technical advice and analysis, but the decision to proceed, and the subsequent success of implementation, will be dependent on political support. The latter part of the paper presents examples of institutional reform from several less developed countries and includes a consideration of the role and influence of donor agencies. International financial relationships should be interpreted in the context of a comprehensive conceptual framework; this paper advocates the use of concepts developed to measure and analyze balance of payments flows. Broad-based, empirical estimates of the international wealth of most countries of the western world are presented on the basis of cumulating balance of payments flows over a lengthy period. Among the more interesting aspects of the results are: the importance of intra-industrial country capital flows in a global context; the propensity of debtors to regard a larger share of their aggregate external debt as long term than do their creditors; the overwhelming importance of banks located in the industrial countries in global external asset and liability positions, and the preponderance of short-term positions taken by those banks; and the tendency for balance of payments records to report more direct investment assets than liabilities. The paper also contains some observations, based on the cumulations of balance of payments capital flows, concerning the nature and size of certain deficiencies in alternative sources--particularly the World Bank's Debtor Reporting System, and the Bank for International Settlements' banking data--of information on outstanding external debt positions. This paper considers the ethics of placebo-controlled trials in developing countries, where a treatment already exists but is not available due to the low local standard of care. Such trials would not be permitted in more developed nations where a higher standard of care is available. I argue that there are moral intuitions against such trials, but a further intuition that if the trials were aimed at producing treatment options for the developing world, that would be more permissible than if the trials were designed with the benefit of rich world people in mind. An approach based upon GA Cohen's work on interpersonal justifiability is suggested to allow us to explain these intuitions. Cohen's framework shows that these trials are ethically problematic because the inequality in healthcare provision between developing and developed nations that allows them to take place is at least partly the pharmaceutical corporations' fault. Following Cohen's argument, this means the trials are non-comprehensively justified. This allows for a more complete explanation of our intuitions than to consider such trials as cases of exploitation, because intuitions on the ethicacy of research can vary even when the exploitation relation remains the same. It is then established that there is good empirical evidence to believe that pharmaceutical corporations do fail the interpersonal justifiability test. The policy implications of this judgement are then considered, and it is suggested that the framework might be equally applicable to examining the permissibility of research conducted on vulnerable people within more developed nations. With ever‐growing populations, cities are increasingly interested in ensuring a well‐functioning food system. However, knowledge of variation between individual city food systems is limited. This is particularly true in countries such as India, experiencing significant issues related to food security and sustainability. This paper advances the understanding of urban food systems, by analyzing the unique food systems of nine cities within India, through the integration of multiple city‐specific data sources including demand of residents, visitors and industries, and commodity‐specific supply chains to assess nutrition, environmental impact, and supply risk. This work finds a large degree of intercity food system variability across multiple food system characteristics. Specifically, levels of undernutrition vary, with the percentage of city populations who are underconsuming protein ranging from 0% to 70%, and for calories 0% to 90%. Environmental impacts (consumptive water loss, land use, and greenhouse gas emissions) of urban food demand also show variation, largely influenced by differing composition of residential diet. Greenhouse gas emissions are also largely influenced by location of production and spatially informed energy intensity of irrigation. Supply chain distance ("food‐miles") also vary by city, with the range of 196 (Pondicherry) to 1,137 (Chennai) km/Mg—shorter than more industrialized nations such as the United States. Evaluating supply chain risk in terms of water scarcity in food‐producing regions that serve city demand finds production locations, on average, to be less water‐scarce than the watersheds local to the urban environments. This suggests water‐intensive agriculture may at times be best located at a distance from urban centers and competing demands. This paper provides a systematic review of studies on the effects of human capital interventions on entrepreneurial performance in industrialized countries. We identify 21 experimental and quasi‐experimental studies published before September 2018. These studies examine the effects of business training, formal education, and entrepreneurship education. Their performance outcomes include firm profits, firm size, and entrepreneurial earnings. The main finding across these studies is that these interventions do not have statistically significant effects. Formal education is the only exception, showing positive effects on firm profits and entrepreneurial earnings, yet these effects are small in magnitude. Evidence is inconclusive regarding effect duration. These findings stand in stark contrast to correlational studies, which tend to find large positive correlations between human capital interventions and entrepreneurial performance. We therefore conclude that correlational studies tend to overestimate the benefits of human capital interventions.
